ELISA of amyloid A in paired bronchoalveolar lavage fluid and serum samples of healthy horses

RESUMO: Distúrbios pulmonares são comuns nos cavalos e a eficiência do tratamento depende de um diagnóstico adequado e precoce. A amilóide A é um biomarcador sensível na deteccção de patologias inflamatórias e infecciososa em cavalos. O objetivo deste estudo foi estabelecer a concentração de amilóide A no líquido broncoalveolar (LBA) em cavalos saudáveis. Os cavalos foram considerados saudaveis baseado nos achados de normalidade do exame físico, hemograma, parâmetros bioquímicos e citologia do LBA. Sangue e LBA foram coletados de 30 fêmeas equinas adultas. Os níveis de Amilóide A no soro e no LBA foram mensurados por meio do teste de ELISA. A amilóide A foi detectada no soro (média ± DP = 3,71±2,51) e no LBA (média ± DP = 0,000745±0,000785) de todos os animais. Conclui-se que a amilóide A também pode ser mensurada no LBA, auxiliando no diagnóstico precoce de processos inflamatórios e infecciosos pulmonares.
ABSTRACT: Pulmonary disorders are common in horses, and treatment efficiency depends on an adequate diagnosis. Amyloid A is the most sensitive indicator of pathology in horses. The objective of this study was to establish the concentration of amyloid A of bronchoalveolar lavage fluid (BALF) in healthy horses. Health condition of horses was considered normal based on physical examination, complete blood count, biochemical parameters, and BALF cytology. Blood and BALF were collected from thirty adult female horses. Amyloid A concentrations in serum and BALF were measured using commercial ELISA tests. Amyloid A was detected in serum (mean ± SD = 3.71±2.51) and BALF (mean ± SD = 0.000745±0.000785) of all horses. In conclusion, SAA can also be measured in bronchoalveolar fluid, affording early detection of respiratory infections or inflammatory conditions.

OBJECTIVES: We designed a cohort study to describe characteristics and outcomes of patients with coronavirus disease (COVID-19) admitted to the intensive care unit (ICU) in the largest public hospital in Sao Paulo, Brazil, as Latin America becomes the epicenter of the pandemic. METHODS: This is the protocol for a study being conducted at an academic hospital in Brazil with 300 adult ICU beds dedicated to COVID-19 patients. We will include adult patients admitted to the ICU with suspected or confirmed COVID-19 during the study period. The main outcome is ICU survival at 28 days. Data will be collected prospectively and retrospectively by trained investigators from the hospital’s electronic medical records, using an electronic data capture tool. We will collect data on demographics, comorbidities, severity of disease, and laboratorial test results at admission. Information on the need for advanced life support and ventilator parameters will be collected during ICU stay. Patients will be followed up for 28 days in the ICU and 60 days in the hospital. We will plot Kaplan-Meier curves to estimate ICU and hospital survival and perform survival analysis using the Cox proportional hazards model to identify the main risk factors for mortality. RESULTS: We expect to include a large sample of patients with COVID-19 admitted to the ICU and to be able to provide data on admission characteristics, use of advanced life support, ICU survival at 28 days, and hospital survival at 60 days. CONCLUSIONS: This study will provide epidemiological data about critically ill patients with COVID-19 in Brazil, which could inform health policy and resource allocation in low- and middle-income countries.

Abstract This paper presents an evaluation of multicarrier channel sounding techniques using different random and pseudo-random sequences to modulate the OFDM sounding signal. The Random (Rand), Pseudo-Noise (PN) and Zadoff-Chu (ZC) were tested, both in laboratory simulations and in field measurements. For the laboratory simulations Matlab routines were used to generate OFDM signals modulated with each of the three sounding signals, that were then convoluted with a synthesized transfer function of a test channel with six multipath components and added Gaussian noise and Doppler fading. The resulting signals are then correlated with a copy of the original signal to provide the multipath power delay profiles. The root mean square deviation (RMSD) and the relationship between peak power and mean power (PAPR) were used as metrics for the comparison between the test transfer function and the simulation detected multipath delay profiles, showing slight advantages of the ZC sequence. The three sequences were then used in field measurements to characterize an urban channel at 700 MHz. In the field measurements, the ZC sequence showed the lowest detection threshold, allowing for the detection of a larger number of multipath components.

Unilateral acute posterior multifocal placoid pigment epitheliopathy in a patient with a strongly positive purified protein derivative test

RESUMO Um homem de 27 anos apresentou uma queixa de diminuição da acuidade visual no olho direito. A acuidade visual melhor corrigida foi 20/60 no olho direito e 20/20 no olho esquerdo. A fundoscopia do olho direito revelou vitreíte leve, lesões amareladas multifocais e descolamento seroso da retina em região macular. A avaliação do olho esquerdo foi normal. Epiteliopatia pigmentar placóide multifocal posterior aguda foi diagnosticada no olho direito. Os exames complementares revelaram forte reação ao teste do derivado proteico purificado Mycobacterium tuberculosis, iniciando terapia antibiótica contra tuberculose. A angiografia fluoresceínica basal do olho direito revelou hipofluorescência precoce e tardia das lesões. A tomografia de coerência óptica do olho direito demonstrou fluído sub- e intrarretiniano associado a edema macular cistóide. Durante o seguimento, os fluídos e cistos retinianos desapareceram, seguido da desorganização das zonas de interdigitação e elipsóide em região foveal. Este é o segundo caso descrito de epiteliopatia pigmentar placóide multifocal posterior aguda unilateral em um paciente com um forte resultado positivo para o teste tuberculínico.
ABSTRACT A 27-year-old man presented with a complaint of decreased visual acuity in the right eye. Best-corrected visual acuity was 20/60 in the right eye and 20/20 in the left eye. Right eye fundoscopy revealed mild vitritis, multifocal yellowish lesions, and macular serous retinal detachment. Left eye evaluation was normal. Acute posterior multifocal placoid pigment epitheliopathy was diagnosed in the right eye. Complementary exams revealed a strong reaction to the Mycobacterium tuberculosis purified protein derivative test, thus treatment for tuberculosis was initiated. Baseline fluorescein angiography of the right eye revealed early hypofluorescence and late staining of the lesions. Optical coherence tomography of the right eye demonstrated the accumulation of subretinal and intraretinal fluid associated with cystoid macular edema. During follow-up, the retinal fluid and cysts disappeared, which was followed by disorganization of foveal interdigitation and ellipsoid zones. This is the second described case of unilateral acute posterior multifocal placoid pigment epitheliopathy in a patient with a strong positive result to the M. tuberculosis purified protein derivative test.

RESUMO: Sida carpinifolia é uma planta responsável por intoxicar várias espécies animais. Este artigo descreve hipomielinogênese em fetos e neonatos de bovinos que consumiram S. carpinifolia. Os neonatos manifestaram ataxia e tremores musculares. Dois neonatos e quatro fetos bovinos foram necropsiados e não havia alterações macroscópicas significativas. Os achados histopatológicos incluíram vacuolização de células acinares do pâncreas, células foliculares da tireoide, hepatócitos, células renais tubulares e neurônios nos fetos. Nos neonatos havia vacuolização na substância branca do lobo frontal telencefálico, além de esferoides axonais no encéfalo dos fetos e dos recém-nascidos. A avaliação lectino-histoquímica demonstrou marcação para as lectinas Con-A, WGA e s-WGA. A coloração de Luxol Fast Blue revelou diminuição acentuada da mielina no telencéfalo de todos os fetos e diminuição moderada nos neonatos. Os achados histológicos e lectina-histoquímicos indicam que o consumo de S. carpinifolia por fêmeas bovinas gestantes pode causar hipomielinogênese em fetos e neonatos.
ABSTRACT: Sida carpinifolia is a plant responsible for poisoning several species of animals. This paper describes Hypomyelinogenesis in fetuses and neonates of cattle that consumed S. carpinifolia. Neonates manifested ataxia and muscle tremors. Two bovine newborns and four fetuses were necropsied and showed no significant gross changes. Histopathologic findings included vacuolation of pancreatic acinar cells, thyroid follicular cells, hepatocytes, cells of renal tubules and neurons of the fetus and the white matter of the telencephalic frontal lobe of the neonates and also revealed axonal spheroids in the brain of the fetuses and neonates. The lectin-histochemical evaluation shoved staining for the lectins Con-A, WGA and s-WGA. The Luxol Fast Blue staining revealed a marked decrease of myelin in the brain of all the fetuses and a moderate decrease in the neonates. Histologic and lectin-histochemic findings indicate that the consumption of S. carpinifolia by pregnant bovine females can cause hypomyelinogenesis in fetuses and neonates.

RESUMO Abordamos aqui a nucleação de um modelo de inovação aberta na Vale (uma das maiores mineradoras do mundo) apresentando o contexto histórico em que se deu esse processo. Discutimos algumas das questões que possivelmente haviam até então dificultado a implantação de uma estrutura de pesquisa e desenvolvimento focando o longo prazo pela indústria no Brasil. Destacamos algumas das dificuldades encontradas ao longo do processo, bem como algumas das razões que levaram ao sucesso da iniciativa.
ABSTRACT We discuss how Vale (one of the world’s largest diversified mining companies) implemented an organizational structure that enabled an open innovation model. We describe the historical perspective in which this process took place. In addition, we present some of the potential causes underlying the slow advance of Brazilian industry in undertaking long-term research and development agendas. Lastly, we list some of the obstacles encountered in this process, as well as some of the potential reasons that might have contributed to the success of the initiative.

RESUMO O objetivo deste trabalho foi determinar a composição química e os valores de energia metabolizável aparente (EMA) e metabolizável aparente corrigida pelo balanço de nitrogênio, EMAn, de alguns alimentos para frangos de corte. Foram utilizados 432 pintos de corte macho, de 14 a 24 dias de idade, distribuídos em delineamento inteiramente casualizado com 12 tratamentos (11 alimentos e uma dieta referência), seis repetições e seis aves por unidade experimental. A metodologia utilizada foi da coleta total de excretas. Foram objetos de estudo: Duas farinhas de vísceras (FV), duas farinhas de penas e sangue (FPS), duas farinhas de carnes e ossos (FCO), soja integral tostada (SIT), farelo de soja (FS), quirera de milho (QM), farelo de trigo (FT) e milheto (M). Os valores de proteína bruta e extrato etéreo (%), EMA e EMAn (kcal/kg) foram, respectivamente: 58,84; 12,64; 3705 e 3340 para a FV 1; 57,64; 11,66; 3565 e 3241 para a FV 2; 80,58; 6,79; 3070 e 2740 para a FPS 1; 82,27; 6,38; 3060 e 2703 para a FPS 2; 48,06; 9,60; 1574 e 1283 para a FCO 1; 41,21; 9,08; 1353 e 1088 para a FCO 2; 36,45; 20,78; 3192 e 2989 para a SIT; 46,22; 1,72; 2648 e 2336 para o FS; 8,51; 3,61; 3354 e 3265 para a QM; 17,11; 3,14; 1948 e 1788 para o FT e 12,01; 4,09; 3072 e 2948 para o M. As diferenças observadas na composição química e energética evidenciam a necessidade de pesquisas para atualizar os valores nutricionais dos alimentos.
ABSTRACT The aim of this work was to determine the chemical composition and values for apparent metabolisable energy (AME), and apparent metabolizable energy corrected for nitrogen (AMEn), in broiler feeds. In the experiment, 432 male broiler chicks, 14 to 24 days old were used, distributed in a completely randomised design with 12 treatments (11 feeds and a reference diet), six replications and six birds per experimental unit. The method employed was the total collection of excreta. The feeds under study were: Two of viscera meal (VM), two feather and blood meal (FBM), two meat and bone meal (MBM), roasted whole soybean (RWS), soybean meal (SM), ground corn (GC), wheat bran (WB) and millet (M). The values for crude protein and ether extract (%), AME and AMEn (kcal/kg) were respectively: 58.84, 12.64, 3705 and 3340 for VM1; 57.64, 11.66, 3565 and 3241 for VM 2; 80.58, 6.79, 3070 2740 for FBM 1; 82.27, 6.38, 3060 2703 for FBM 2; 48.06, 9.60, 1574 and 1283 for MBM 1; 41.21, 9.08, 1353 and 1088 for MBM 2; 36.45, 20.78, 3192 and 2989 for RWS; 46.22, 1.72, 2648 and 2336 for SM; 8.51, 3.61, 3354 and 3265 for GC; 17.11, 3.14, 1948 and 1788 and for WB; and 12.01, 4.09, 3072 and 2948 for M. The differences found in chemical and energy composition highlight the need for research to revise the nutritional values of feed.

Desiccation time of the spring sorghum as a predecessor crop for summer soybean and autumn bean in a no-tillage system

RESUMO O intervalo de dessecação entre plantas de cobertura e semeadura da soja vem sendo questionado por diversos autores. O trabalho teve como objetivo avaliar intervalos de dessecação do sorgo de primavera como cultura antecessora da soja de verão e, posteriormente, a cultura do feijão no outono, com ênfase para a formação de palhada para a manutenção do Sistema Plantio Direto e para melhorar a produtividade de grãos da soja e feijão, bem como analisar a interferência da palhada de sorgo nessas culturas. O ensaio foi desenvolvido na Fazenda de Ensino e Pesquisa da FE/UNESP – Ilha Solteira, sendo o delineamento experimental o de blocos casualizados, com 5 tratamentos e 6 repetições. Os tratamentos foram constituídos por 5 intervalos distintos entre dessecação do sorgo e semeadura da soja (sendo D7- Dessecação do sorgo 7 dias antes da semeadura da soja; e assim, sucessivamente, para D14, D21, D28 e D35). Para a análise dos resultados, foi utilizado o teste de Tukey para um nível de significância de 10% e o programa estatístico Sisvar. O sorgo “Cober Crop” exerceu influência sobre a produtividade da soja, e seu efeito não foi duradouro para o cultivo do feijão em sucessão à soja.
ABSTRACT Several authors have been questioned the desiccation interval between the coverage plants and sowing plants for the soybean crop. Therefore, this study aimed to evaluate the desiccation time of the spring sorghum as a predecessor crop for summer soybean and then for autumn bean, focusing on the straw formation for maintenance of the no-tillage system and to evaluate the yield of soybeans and beans grain, as well as analyzing the interference of sorghum straw in these cultures. The experiment was developed in the Teaching and Research Farm of FE/UNESP located in Ilha Solteira/SP where it was used an experimental design of randomized blocks with five treatments and six repetitions. The treatments consisted of five different intervals between sorghum desiccation and soybean sowing (being with D7 - Drying sorghum seven days before the soybean sowing; and successively for D14, D21, D28 and D35). In order to analyze the results, it was used the Tukey test for a 10% level of significance and the statistical program called Sisvar. The "Cober Crop" sorghum exerted influence on soybean yield but this effect was not lasting for the bean crop in succession to the soybeans.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Corn production for silage intercropped with forage in the farming-cattle breeding integration

Na cultura de milho para silagem, devido à exportação do material vegetal, a consorciação com forrageiras proporciona maior cobertura do solo e formação de palhada para a continuidade do Sistema Plantio Direto (SPD). O objetivo do trabalho foi avaliar a produção de milho para silagem, em SPD, consorciado com quatro forrageiras (Urochloa brizantha cv. Marandu, U. ruziziensis cv. Ruziziensis, Panicum maximum cv. Tanzânia e P. maximum cv. Áries), em três modalidades de semeadura (na linha, junto com o adubo do milho; a lanço simultâneo ao milho e a lanço no estádio V4 do milho) e milho sem consorciação. O experimento foi conduzido no outono/inverno de 2010, no município de Selvíria - MS, com delineamento em blocos casualizados, em esquema fatorial (4x3+1) e quatro repetições. Na cultura do milho, foram avaliados: altura de plantas, diâmetro basal do colmo, estande inicial e final, e produção de silagem, e, nas forrageiras, a produção de matéria seca. As características morfológicas e a produtividade de milho não foram influenciadas pelos consórcios quando comparados ao milho sem consorciação. Destaca-se a produção de matéria seca da forragem na modalidade de semeadura na linha do milho junto ao adubo, que além de proporcionar maior produtividade, aproveita a operação por ocasião da semeadura.
Corn cropping for silage, due to the plant material exportation, intercropping with forage provides greater ground cover and straw formation for the Direct Planting System (DPS) continuity. The objective of this study was to evaluate corn production for silage in DPS intercropped with four forages (Urochloa brizantha cv. Marandu, U. ruziziensis cv. Ruziziensis, Panicum maximum cv. Tanzânia and P. maximum cv. Áries). We applied three sowing methods (in row together with corn fertilizer; by no-till sowing simultaneously to corn sowing and at V4 corn stage) and corn without intercropping. The experiment was conducted in autumn/ winter of 2010, in Selvíria - MS, in a randomized block design in factorial arrangement (4 x 3 + 1) and 4 replications. For corn, we evaluated plant height, basal stem diameter, initial and final stand and silage production and for forage dry matter production. Morphological characteristics and corn yield were not affected by intercropping when compared to sole corn crop. Forage dry matter production sown in corn row with fertilizer is a highlight, which in addition to providing greater productivity, harnesses the operation during sowing.

A avaliação das tecnologias empregadas ao sistema de produção agrícola, tais como rotação de culturas e preparo de solo, ambos associados à integração lavoura-pecuária, é importante para tomadas de decisões futuras. Portanto, objetivou-se, no presente trabalho, avaliar a incorporação de calcário por três sistemas de preparo do solo e dois manejos culturais, em sistema de integração lavoura-pecuária, com ênfase para produtividade de grãos de milho. O experimento foi conduzido no período compreendido de janeiro de 2003 a abril de 2005, no município de Selvíria-MS, em Latossolo Vermelho distroférrico, textura argilosa. O delineamento experimental adotado foi o de blocos ao acaso, com parcelas subdivididas, constituídas por três tratamentos principais, visando o condicionamento físico do solo e a incorporação de calcário: PD - Plantio Direto; CM - Cultivo Mínimo, e PC - Preparo Convencional; e por dois tratamentos secundários, relacionados ao manejo: rotação e sucessão de culturas, com quatro repetições. As características agronômicas do milho analisadas foram: altura de planta, diâmetro de colmo, altura de inserção da primeira espiga, massa de 100 grãos e produtividade de grãos. Os resultados mostraram que não houve diferenças estatísticas na produtividade do milho, tanto em função dos tratamentos de manejo do solo para incorporação de calcário, como nos manejos culturais, rotação e sucessão, após dois anos de cultivo.
The evaluation of technologies employed at the agricultural production system such as crop rotation and soil preparation, both associated with crop-livestock integration, is crucial. Therefore, the aim of the present study was to evaluate the incorporation of lime for three no-tillage systems and cultural managements in system of crop-livestock integration, with emphasis on corn grain yield. The experiment was conducted from January 2003 to April 2005 at Selvíria city, MS, in Dystroferric Red Latosol, clay texture. The experimental design was randomized blocks with split plots consisted of three main treatments, aimed the soil physics conditioning and the incorporation of lime: PD - No-no-tillage; CM - minimum no-tillage, and PC - conventional no-tillage; and of two secondary treatments related to the management: rotation and crop succession, with four replications. Data on agronomic traits of maize were analyzed: plant height, stem diameter, height of the first spike insertion, 100 grains weight and grain yield. The results showed that the maize produced under the system of crop-livestock integration is quite feasible, showing that grain yields are comparable to averages in the region and the different soil physical conditioning and incorporation of lime did not influence the corn yield as well as the cultural managements, rotation and succession, did not affect the maize crop behavior after two years of cultivation.

O trabalho foi conduzido em área de expansão de cana-de-açúcar da Usina Vale do Paraná, no município de Suzanápolis - SP, na região do noroeste paulista. Foi utilizada a variedade de cana RB92-5345, espaçamento de 1,5 m entre linhas, em ARGISSOLO VERMELHO. O trabalho objetivou avaliar a produtividade em cana-planta e 1ª cana-soca e alguns atributos químicos de solo, em função dos métodos de preparo do solo e aplicação ou não de gesso. O delineamento experimental utilizado foi o de blocos ao acaso, com seis tratamentos, fatorial 3x2, e seis repetições. Os tratamentos principais foram preparos de solo com três equipamentos: arado de aivecas, escarificador e grade pesada, e dois tratamentos secundários com aplicação de 1 t ha-1 de gesso e sem gesso. Após cada colheita da cana, o solo foi caracterizado quanto aos indicadores de fertilidade nas camadas de 0,0-0,15; 0,15-0,30 e 0,30-0,45 m. As diferenças dos atributos químicos do solo, devido aos métodos de preparo ocorridas na cana-planta, não perduraram até a colheita da 1ª cana-soca e também não influenciaram na produtividade da cultura. A gessagem proporcionou maiores valores de ATR e produtividade de TCH, para cana-planta e 1ª cana-soca, respectivamente, confirmando a hipótese inicial.
The study was conducted in an area of expansion of sugarcane at Vale do Paraná factory in Suzanápolis city - São Paulo (SP), in Brazil, in the northwestern region of the State of São Paulo. It was used the sugarcane variety RB92-5345, 1.5m of spacing between rows, in an Ultisol. The study aimed to evaluate the productivity of sugarcane and first ratoon and some soil chemical attributes in function of soil tillage and application or not of gypsum. The experimental design was randomized blocks with six treatments, in a factorial 3x2 and six replicates, the main treatments were soil tillage with three equipments, moldboard plow, chisel plow, and heavy harrow, and two secondary treatments with application of 1 t ha-1of gypsum and no gypsum. After each harvest of cane, the soil was characterized as to its fertility indicators in layers of 0.0-0.15; 0.15-0.30 and 0.30-0.45m. Differences in values of soil chemical attributes due to the methods of preparation occurred in the sugarcane did not last until the harvest of the 1st ratoon cane, and also did not influence the crop productivity. The gypsum application resulted in higher values of total recoverable sugar (TRS) and the productivity of tons of stems per hectare (TSH) to sugarcane and 1st ratoon cane, respectively, confirming the initial hypothesis.

RACIONAL: Aproximadamente 50% dos pacientes com tumor colorretal apresentam metástase hepática sendo a hepatectomia o procedimento terapêutico de escolha. Discutem-se diversos fatores prognósticos; entre eles, a margem cirúrgica é fator sempre recorrente, pois não existe consenso da distância mínima necessária entre o nódulo metastático e a linha de secção hepática. OBJETIVOS: Avaliar as margens cirúrgicas nas ressecções de metástases hepáticas de câncer colorretal e sua correlação com recidiva local e sobrevida. MÉTODOS: Estudo retrospectivo, baseado na revisão dos prontuários de 91 pacientes submetidos à ressecção de metástases hepáticas de neoplasia colorretal. Foi realizada revisão histopatológica de todos os casos com aferição da menor margem cirúrgica e observar o resultado tardio em relação à recidiva e sobrevida. RESULTADOS: Não houve diferença estatística nas taxas de recidiva e no tempo de sobrevivência global entre os pacientes com margens livres ou acometidas (R0vsR1), assim como não houve diferença entre as margens subcentimétricas e as maiores de 1 cm. A sobrevida livre de doença dos pacientes com margens microscopicamente acometidas foi significativamente menor do que dos com margens livres. A análise uni e multivariada não identificou a margem cirúrgica (R1, exígua ou menor que 1 cm) como fator de risco para recidiva. CONCLUSÕES: As ressecções de metástases hepáticas com margens livres de doença, independentemente das dimensões da margem, não influenciou na recidiva tumoral (intra ou extra-hepática) ou na sobrevida dos pacientes.
BACKGROUND: Approximately 50% of the patients with a colorectal tumor develop liver metastasis, for which hepatectomy is the standard care. Several prognostic factors have been discussed, among which is the surgical margin. This is a recurring issue, since no consensus exists as to the minimum required distance between the metastatic nodule and the liver transection line. AIM: To evaluate the surgical margins in liver resections for colorectal metastases and their correlation with local recurrence and survival. METHODS: A retrospective study based on the review of the medical records of 91 patients who underwent resection of liver metastases of colorectal cancer. A histopathological review was performed of all the cases; the smallest surgical margin was verified, and the late outcome of recurrence and survival was evaluated. RESULTS: No statistical difference was found in recurrence rates and overall survival between the patients with negative or positive margins (R0 versus R1); likewise, there was no statistical difference between subcentimeter margins and those greater than 1 cm. The disease-free survival of the patients with microscopically positive margins was significantly worse than that of the patients with negative margins. The uni- and multivariate analyses did not establish the surgical margin (R1, narrow or less than 1 cm) as a risk factor for recurrence. CONCLUSION: The resections of liver metastases with negative margins, independently of the margin width, had no impact on tumor recurrence (intra- or extrahepatic) or patient survival
