Decreasing delays in the diagnosis and treatment of rheumatoid arthritis in Brazil: a nationwide multicenter observational study

Abstract Background Management delays imply worse outcomes in rheumatoid arthritis (RA) and, therefore, should be minimized. We evaluated changes in diagnostic and treatment delays regarding RA in the last decades in Brazil. Methods Adults fulfilling the ACR/EULAR (2010) criteria for RA were assessed. Delays in diagnosis and treatment, and the frequencies of early management initiation within thresholds (windows of opportunity) of 3, 6, and 12 months from symptoms onset were evaluated. The Mann–Kendall trend test, chi-squared tests with Cramer’s V effect sizes and analysis of variance were conducted. Results We included 1116 patients: 89.4% female, 56.8% white, mean (SD) age 57.1 (11.5) years. A downward trend was found in diagnostic (tau = - 0.677, p < 0.001) and treatment (tau = - 0.695, p < 0.001) delays from 1990 to 2015. The frequency of early management increased throughout the period, with ascending effect sizes across the 3-, 6-, and 12-month windows (V = 0.120, 0.200 and 0.261, respectively). Despite all improvements, even in recent years (2011–2015) the diagnostic and treatment delays still remained unacceptably high [median (IQR): 8 (4–12) and 11 (5–17) months, respectively], with only 17.2% of the patients treated within the shortest, 3-month window. Conclusion The delays in diagnosis and treatment of RA decreased during the last decades in Brazil. Improvements (effect sizes) were greater at eliminating extreme delays (≥ 12 months) than in attaining really short management windows (≤ 3 months). Very early treatment was still an unrealistic goal for most patients with RA.

Resumo Objetivos: Descrever perfil das mulheres com filhos menores de quatro meses residentes nas capitais brasileiras e no Distrito Federal segundo situação de trabalho e analisar a influência da licença-maternidade sobre o aleitamento materno exclusivo entre as mulheres trabalhadoras. Métodos: Estudo transversal com dados extraídos da II Pesquisa Nacional de Prevalência do Aleitamento Materno feita em 2008. Inicialmente foi feita análise descritiva do perfil das 12.794 mulheres participantes do estudo segundo situação de trabalho e de licença-maternidade, bem como a frequência de licença-maternidade nas regiões brasileiras e capitais. Em seguida, para identificar a influência da licença-maternidade na interrupção do AME, fez-se modelo múltiplo, no qual foram incluídas 3.766 mulheres que declararam trabalhar e estar em licença-maternidade no momento da entrevista. O desfecho adotado no estudo foi a interrupção do AME, classificado de acordo com a definição da OMS. Resultados: Em relação à situação de trabalho, 63,4% das mães entrevistadas no Brasil não trabalhavam fora do lar e dentre as que trabalhavam fora 69,8% usufruíam da licença-maternidade. Verificou-se maior concentração de mulheres que trabalhavam fora entre aquelas com mais de 35 anos, mais de 12 anos de escolaridade, primíparas, das regiões Sudeste e Sul. Não estar em licença-maternidade aumentou em 23% a chance de interrupção do AME. Conclusão: Constatou-se que a licença-maternidade contribuiu para aumentar a prevalência do AME nas capitais brasileiras, reforça a importância da ampliação da licença-maternidade para seis meses.
Abstract Objectives: To describe the profile of women with children aged under 4 months living in the Brazilian state capitals and in the Federal District according to their working status and to analyze the influence of maternity leave on exclusive breastfeeding (EBF) among working women. Methods: This was a cross-sectional study with data extracted from the II National Maternal Breastfeeding Prevalence Survey carried out in 2008. Initially, a descriptive analysis of the profile of 12,794 women was performed, according to their working status and maternity leave and the frequency of maternity leave in the Brazilian regions and capitals. The study used a multiple model to identify the influence of maternity leave on EBF interruption, including 3766 women who declared they were working and were on maternity leave at the time of the interview. The outcome assessed in the study was the interruption of the EBF, classified by the WHO. Results: Regarding the working status of the mothers, 63.4% did not work outside of their homes and among those who worked, 69.8% were on maternity leave. The largest prevalence among workers was of women older than 35 years of age, with more than 12 years of schooling, primiparous and from the Southeast and South regions. The lack of maternity leave increased by 23% the chance of EBF interruption. Conclusion: Maternity leave contributed to increase the prevalence of EBF in the Brazilian states capitals, supporting the importance of increasing the maternity leave period from four to six months.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

In vitro effect of the association of citronella, Santa Maria herb (Chenopodium ambrosioides) and quassia tincture on cattle tick Rhipicephalus (Boophilus) microplus

O carrapato Rhipicephalus (Boophilus) microplus é o ectoparasita que mais causa prejuízos à pecuária brasileira. O controle desse parasita é realizado por meio da administração de produtos químicos, sendo que estes são utilizados de forma inadequada, gerando alto custo e estimulando a seleção de parasitas resistentes. Tendo em vista essa situação, o objetivo deste experimento foi avaliar a eficácia in vitro de diferentes formulações contendo tintura de citronela (Cymbopogon nardus), erva de Santa Maria (Chenopodium ambrosioides) e quássia (Quassia amara) sobre o carrapato bovino R. microplus. Para isso, fêmeas ingurgitadas do parasita foram submetidas ao teste laboratorial de biocarrapaticidograma. As soluções testadas continham as três plantas em diferentes concentrações e utilizadas em associação. Aproximadamente 84% dos tratamentos apresentou eficiência igual ou superior a 95%. Os tratamentos que continham citronela em concentração igual ou superior a 5% apresentaram melhores resultados e a concentração de 10% apresentou máxima eficiência, independente das concentrações da quássia e da erva de Santa Maria. A alta eficácia das soluções foi atribuída principalmente à redução na postura dos ovos e na taxa de eclosão da postura, e em menor escala à morte das teleóginas. Isso sugere que as soluções contendo os três fitoterápicos provocam alteração no sistema e na capacidade reprodutiva das fêmeas de R. microplus e também apresenta ação carrapaticida em testes in vitro.
The tick Rhipicephalus (Boophilus) microplus is an ectoparasite which causes high financial losses to the production of Brazilian cattle. The control of this parasite is accomplished by the administration of chemical products, but they are not adequately used, leading to high costs and stimulating the selection of resistant parasites. Thus, the objective of this experiment was to assess the in vitro efficiency of different formulations containing an association of citronella (Cymbopogon nardus), Santa Maria herb (Chenopodium ambrosioides) and quassia (Quassia amara) tinctures on the bovine tick R. microplus. For this, engorged females of the parasite were submitted to the laboratory immersion test. The solutions containing all three plants were tested in different concentrations. Approximately 84% of the treatments presented efficiency equal or superior to 95%. The treatments with citronella in concentration equal or superior to 5% showed better results and the concentration of 10% showed maximum efficiency, independently of the concentrations of the other two plants. The high efficiency of the solutions was attributed mainly to the reduction of egg laying and hatching rate, and, in a minor scale, to death of the engorged females. It suggests that the phytotherapic solutions are able to induce alterations on the system and reproductive capacity of the females of R. microplus in in vitro tests, besides presenting a tickicide action.

O nó sinoatrial, por se encontrar topograficamente instalado como componente inicial do sistema de condução, é responsável pela geração dos impulsos nervosos determinantes da contração cardíaca. Estudos relacionados à morfologia do nó, visando conhecer a origem, trajeto e distribuição dos vasos neste tecido são conhecidos, contudo, no que diz respeito a estes aspectos e aos dados quantitativos da irrigação nodal, no que se refere ao comportamento vascular arterial e a densidade vascular arterial desta região, a literatura é escassa. Com este objetivo foram utilizados 30 corações de suínos SRD, sendo 27 injetados com resina vinílica corada, para análise da origem e trajeto da ANSA (artéria do nó sinoatrial) e 3 corações injetados com solução aquosa de carvão coloidal (tinta nanquim) para proceder à análise estereológica. As artérias atriais originaram-se tanto da artéria coronária direita quanto da esquerda, com predominância da primeira (66,66% e 33,33%, respectivamente). Quando originada da coronária direita, a irrigação ocorreu pelo ramo AADAM (artéria atrial direita cranial medial) em 14 casos e pelos ramos AADAI (artéria atrial direita cranial medial) em 2 casos e AADAL (artéria atrial direita cranial lateral) em 2 casos. Em 9 casos (33,33%) originou-se pela artéria coronária esquerda: quatro pelo ramo AASPL (artéria atrial esquerda caudal lateral), dois pelo ramo AASAI (artéria atrial esquerda cranial intermédia) e três pelo ramo AASAM (artéria atrial esquerda cranial medial). Anastomoses interarteriais, com participação dos vasos responsáveis pela irrigação do território do nó sinoatrial foram observadas na maioria dos casos (25 corações). O Volume do órgão ou Volume Referência (V(ref)) foi de 35,32x10(4)µm³. Para as variáveis estereológicas analisadas, a estimação da densidade de comprimento do vaso (Lv) foi de 766; o comprimento do vaso (L) - mm - foi de 27,06x10(5)µm; a densidade de superfície de área (Sv) foi de 182 e a superfície de área (S) - mm² - foi de 64,3x10(6)µm². A estimação da densidade numérica vascular (Nv(vasc)), quantidade de vasos por unidade de volume (cm³), foi de 2,19 10-5 e o número total de vasos no órgão (N(vasc)), estimado pelo método dissector físico em combinação com a estimativa do número de Euler (Xv), foi de 773,6832 x10-2. A elevada densidade vascular e do número total de vasos na região do nó sinoatrial de suínos sugere a existência de uma complexa e densa rede vascular perinodal, ratificando a importância deste marca-passo pelo seu suprimento sangüíneo.
The sinoatrial node, for being topographically installed as the initial component of the conduction system, is responsible for the production of the nervous impulses, which determines the cardiac contraction. There have been made studies related to the node's morphology in order to know the origin, distribution and quantification of the vases in this tissue, however, in spite of the results and quantitative data of the nodal irrigation - arterial vascular conduct and arterial vascular density, at the nodal level - the literature is scarce. With this objective 27 SRD swine hearts, injected with colored resin for macroscopic analysis of the origin and distribution of ANSA (sinoatrial node artery), 3 others injected with watery solution of colloidal coal (dyed nanquim) were used, to mark the route of the vases on the node level for stereological analysis. The atrial arteries originated as well from the right coronary artery as from the left one, with predominance of the first (66.66% and 33.33%, respectively). When originated from the right coronary artery, there existed the following branches: AADAM (right cranial medial atrial artery) in 14 cases, AADAI (right cranial intermedial atrial artery) in 2 cases, and AADAL (right cranial lateral atrial artery) in 2 cases. In 9 cases (33.33%) the following branches originated from the left coronary artery: 4 through branch AASPL (left caudal lateral atrial artery), 2 through branch AASAI (left cranial intermedial atrial artery) and 3 through branch AASAM (left cranial medial atrial artery). Interarterial anastomoses with vessels responsible for irrigation of the sinoatrial node area were observed in most of the cases (25 hearts). The Volume or Reference Volume (V(ref)) of the hearts were 35.32x10(4)µm³. For the stereological variables analyzed, the estimate density of the vessel length (Lv) was 766; the vessel length (L) - mm - was 27.06x10(5)µm; the area's surface density (Sv) was 182 and the area's surface (S) - mm² - was 64.3x10(6)µm². The estimate of the vascular numeric density (Nv(vasc)) - total of vessels per unit of volume (cm³) was 2.19 10-5, and the total number of vessels in the organ (N(vasc)), estimated by the physical dissector method in combination with the estimate of the number of Euler (Xv), was 773.6832 x10-2. The raised vascular density and total number of vases in the swine's sinoatrial node area suggests the existence of a vascular perinodal network complex and dense, ratifying the importance of this mark-pass for its blood supply.

Characterization of rotavirus P genotypes circulating among paediatric inpatients in Northern Brazil

De novembro de 1992 a agosto de 1993, 28 amostras fecais positivas para rotavírus, obtidas de pacientes pediátricos hospitalizados em Belém, Brasil, com idades inferiores a 4 anos, foram testadas por RT-PCR visando à determinação dos genótipos P. Com exceção de 7 crianças não diarréicas, todos os pacientes apresentavam diarréia à admissão ou a desenvolviam enquanto internados no hospital. Cepas de rotavírus com especificidades antigênicas P correspondentes aos genótipos P1B[4] e P1A[8] (ambos pertencentes ao tipo G2) predominaram, representando 78,6% das amostras. Enquanto apenas uma cepa classificada como P2A[6] - com dupla especifidade antigênica (1 e 4) para o tipo G - foi detectada, não se identificou o genótipo P em 4 (14,3%) das cepas sob exame. A maioria (81%) dos espécimes fecais foi obtida de crianças com até 18 meses de vida. As cepas de rotavírus com genótipo único P1B[4] foram identificadas em crianças diarréicas (quer as infecções de origem nosocomial, 28,6%, quer as oriundas da comunidade, 28,6%), assim como entre as assintomáticas (42,8%). O genótipo P1A[8], por outro lado, foi detectado somente entre pacientes com diarréia, com frequências de 57,1% e 42,9% entre os casos classificados como nosocomiais e de origem comunitária, respectivamente. Infecção mista envolvendo os genótipos P1A[8] e P1B[4] se caracterizou em apenas uma situação de infecção sintomática (gastroenterite) contraida na comunidade.
Between November 1992 and August 1993, twenty-eight rotavirus-positive stool samples obtained from paediatric inpatients in Belém, Brazil, aged less than four years, were tested by RT-PCR to determine the P genotype specificities. With the exception of 7 non-diarrhoeic children, all patients were either diarrhoeic at admission or developed diarrhoea while in hospital. Rotavirus strains with the gene 4 alleles corresponding to P1B[4] and P1A[8] types (both of which bearing G2 specificity) predominated, accounting for 78.6% of the strains. While only one P2A[6] type strain - with (mixed) G1 and 4 type specificities - was detected, the gene 4 allele could not be identified in 4 (14.3%) of the strains. Most (81%) of the specimens were obtained from children during their first 18 months of life. Rotavirus strains bearing single P1B[4] type-specificity were identified in both diarrhoeic (either nosocomial, 28.6% or community-acquired diarrhoea, 28.6%) and non-diarrhoeic (42.8%) children. P1A[8] gene 4 allele, on the other hand, was detected only among diarrhoeic children, at rates of 57.1% and 42.9% for nosocomial- and- community acquired diarrhoea, respectively. Mixed P1A[8],1B[4] type infection was identified in only one case of community-acquired diarrhoea.
