Resumo O objetivo principal desta pesquisa foi realizar uma análise espacial das precipitações no Estado do Rio Grande do Norte (RN), através de técnicas geoestatísticas, comparando a média histórica com anos padrão chuvoso e seco. Para análise geoestatística, foram utilizados três indicadores denominados Índice Global de Moran, Moran Map e Diagrama de Espalhamento de Moran. Os resultados mostraram que as precipitações seguem um padrão mais evidente na mesorregião Leste potiguar. Tanto no ano considerado chuvoso, quanto no ano considerado seco, a maior parte dessa mesorregião apresentou um padrão Alto-Alto (HH), indicando que os municípios formam um agrupamento que representa autocorrelação espacial associada a valores mais elevados de precipitação no estado. Por outro lado, as regiões do Agreste e Central potiguar foram as mais secas.

Abstract The main goal of this research was to perform a spatial analysis of precipitation in the state of Rio Grande do Norte (RN) through geostatistical techniques, comparing the historical average data with rainy and dry standard years. Three indicators called Moran’s Global Index, Moran Map, and Moran’s Scattering Diagram were used for geostatistical analysis. The results showed that precipitation has a more evident pattern in the Eastern Potiguar mesoregion since both in the year considered rainy and, in a year considered dry, most of this mesoregion presented a (HH) pattern, that is, the municipalities form a cluster that represents spatial autocorrelation associated with higher values of precipitation in the state. The regions of Agreste and Central Potiguar were the driest.

Resumen El objetivo principal de esta investigación fue realizar un análisis espacial de la precipitación en el Estado de Rio Grande do Norte (RN), a través de técnicas geoestadísticas, comparando el promedio histórico con años patrones lluviosos y secos. Para el análisis geoestadístico se utilizaron tres indicadores denominados Índice Global de Moran, Mapa de Moran y Diagrama de Dispersión de Moran. Los resultados mostraron que la precipitación sigue un patrón más evidente en la mesoregión oriental de Potiguar, ya que, tanto en el año considerado lluvioso, como en un año considerado seco, la mayor parte de esta mesoregión mostró un patrón Alto, Alto (HH), es decir, los municipios forman un conglomerado que representa la autocorrelación espacial asociada a los valores más altos de precipitación en el estado. Las regiones de Agreste y Potiguar Central fueron las más secas.

Abstract The increase in sexually transmitted infections (STIs) in Europe and the USA, especially among men who have sex with men (MSM) and transgender women (TW), has raised concerns and prompted the exploration of doxycycline prophylaxis as a potential intervention. Doxycycline prophylaxis can be administered either as a daily 100 mg dose (DoxyPrEP) or a single 200 mg dose post-sexual activity (DoxyPEP). Recent clinical trials, primarily focusing on higher-risk groups, have shown reductions of approximately 70% in syphilis and chlamydia infections and conflicting results regarding gonorrhea infection (up to 50%). Despite these advancements, the effectiveness of doxycycline prophylaxis among women has not been established and this strategy raises concerns about community acceptability, adverse events, safety, antimicrobial resistance, microbiome disruption, and cost-effectiveness. Ongoing clinical trials and agent-based models aim to address these uncertainties to predict the impact on a population level and on specific groups. This review aims to assess the existing data of doxycycline STI prophylaxis, identify knowledge gaps, and synthesize existing literature and guidelines about the current recommendations.

Resumo O aumento de infecções sexualmente transmissíveis (IST) na Europa e nos EUA, especialmente entre homens que fazem sexo com homens (HSH) e mulheres transexuais (TW), levantou preocupações e levou à exploração da profilaxia com doxiciclina como uma intervenção potencial. A profilaxia com doxiciclina pode ser administrada em dose diária de 100 mg (DoxyPrEP) ou em dose única de 200 mg pós-atividade sexual (DoxyPEP). Ensaios clínicos recentes, centrados principalmente em grupos de maior risco, mostraram reduções de aproximadamente 70% nas infecções por sífilis e clamídia e resultados conflituantes em relação à infecção por gonorreia (até 50%). Apesar destes avanços, a eficácia da profilaxia com doxiciclina entre as mulheres não foi demonstrada e esta estratégia levanta preocupações sobre a aceitabilidade da comunidade, eventos adversos, segurança, resistência antimicrobiana, perturbação do microbioma e relação custo-eficácia. Os ensaios clínicos em curso e os modelos baseados em agentes visam abordar estas incertezas para prever o impacto a nível populacional e em grupos específicos. Esta revisão tem como objetivo avaliar os dados existentes sobre a profilaxia de IST com doxiciclina, identificar lacunas de conhecimento e sintetizar a literatura e as diretrizes existentes sobre as recomendações atuais.

ABSTRACT The SPIRIT 2013 statement aims to improve the completeness of clinical trial protocol reporting by providing evidence-based recommendations for the minimum set of items to be addressed. This guidance has been instrumental in promoting transparent evaluation of new interventions. More recently, there has been a growing recognition that interventions involving artificial intelligence (AI) need to undergo rigorous, prospective evaluation to demonstrate their impact on health outcomes. The SPIRIT-AI (Standard Protocol Items: Recommendations for Interventional Trials–Artificial Intelligence) extension is a new reporting guideline for clinical trial protocols evaluating interventions with an AI component. It was developed in parallel with its companion statement for trial reports: CONSORT-AI (Consolidated Standards of Reporting Trials–Artificial Intelligence). Both guidelines were developed through a staged consensus process involving literature review and expert consultation to generate 26 candidate items, which were consulted upon by an international multi-stakeholder group in a two-stage Delphi survey (103 stakeholders), agreed upon in a consensus meeting (31 stakeholders) and refined through a checklist pilot (34 participants). The SPIRIT-AI extension includes 15 new items that were considered sufficiently important for clinical trial protocols of AI interventions. These new items should be routinely reported in addition to the core SPIRIT 2013 items. SPIRIT-AI recommends that investigators provide clear descriptions of the AI intervention, including instructions and skills required for use, the setting in which the AI intervention will be integrated, considerations for the handling of input and output data, the human–AI interaction and analysis of error cases. SPIRIT-AI will help promote transparency and completeness for clinical trial protocols for AI interventions. Its use will assist editors and peer reviewers, as well as the general readership, to understand, interpret and critically appraise the design and risk of bias for a planned clinical trial.

RESUMO A declaração SPIRIT 2013 tem como objetivo melhorar a integralidade dos relatórios dos protocolos de ensaios clínicos, fornecendo recomendações baseadas em evidências para o conjunto mínimo de itens que devem ser abordados. Essas orientações têm sido fundamentais para promover uma avaliação transparente de novas intervenções. Recentemente, tem-se reconhecido cada vez mais que intervenções que incluem inteligência artificial (IA) precisam ser submetidas a uma avaliação rigorosa e prospectiva para demonstrar seus impactos sobre os resultados de saúde. A extensão SPIRIT-AI (Standard Protocol Items: Recommendations for Interventional Trials - Artificial Intelligence) é uma nova diretriz de relatório para protocolos de ensaios clínicos que avaliam intervenções com um componente de IA. Essa diretriz foi desenvolvida em paralelo à sua declaração complementar para relatórios de ensaios clínicos, CONSORT-AI (Consolidated Standards of Reporting Trials - Artificial Intelligence). Ambas as diretrizes foram desenvolvidas por meio de um processo de consenso em etapas que incluiu revisão da literatura e consultas a especialistas para gerar 26 itens candidatos. Resumo O sarcoma de Ewing é o tumor ósseo maligno da pelve mais comum em crianças e adultos jovens. Mesmo com tratamento agressivo, sua taxa de sobrevivência está entre as piores. A apresentação clássica pode não ser a regra. Ele pode simular clinicamente, imaginologicamente e histopatologicamente outras entidades não malignas. Portanto, sua suspeita não deve ser negligenciada. Relatamos dois casos de sarcoma pélvico: o primeiro imitando granuloma eosinofílico e o segundo imitando osteomielite. Neste último, também relatamos um achado atípico de sua história natural: uma resposta inicial ao antibiótico e ao tratamento anti-inflamatório. Abstract Ewing sarcoma is the most common malignant bone tumor of the pelvis in children and young adults. Even with aggressive treatment, its survival rate is amongst the poorest. Classical presentation may not be the rule. It may simulate clinically, imagiologically and histopathologically other nonmalignant entities. Therefore, its suspicion should not be overlooked. We report two cases of pelvic Ewing sarcoma: the first mimicking eosinophilic granuloma, and the second mimicking osteomyelitis. In the latter, we also report an atypical finding of its natural history: an initial response to antibiotic and anti-inflammatory treatment. ABSTRACT Cowpeas are of high socioeconomic importance in the North and Northeast regions of Brazil, whereby the search for alternatives that offer increased productivity with financial returns has become an important challenge. Thus, the objective of this study was to estimate and evaluate the economic indicators of cowpea production in 2015 and 2016 when applying drip irrigation to different depths. Irrigation treatments were based on crop evapotranspiration (ETc). The experiment was laid in a randomized complete blocks design with four treatments (0, 25, 50, and 100% ETc) and six replications. For economic analysis, the results were extrapolated to an area of one hectare. Productivity in 2015 was lower than that in 2016; however, in all treatments, it was always higher when plants were grown under adequate water supply. In both years, the total operating costs were higher under irrigation than under rainfed conditions. The 100% ETc treatment resulted in a greater number of cowpea bags (60 kg) produced and a higher gross revenue than the rainfed treatment by 17 bags ha-1, equivalent to 1,020 kg ha-1 (gross revenue US$ 711.48) and 16 bags ha-1, equivalent to 960 kg ha-1 (gross revenue US$ 867.12), in 2015 and 2016, respectively. Water deficit limited cowpea yield in both years but made cultivation economically unfeasible only in 2015.

RESUMO O feijão-caupi apresenta grande importância socioeconômica nas regiões Norte e Nordeste do Brasil, sendo que a busca por alternativas que ofereçam aumento de produtividade com retorno financeiro tem se tornado um importante desafio. Assim, o objetivo deste trabalho foi estimar e avaliar os indicadores econômicos da produção de feijão-caupi nos anos de 2015 e 2016 quando da aplicação de irrigação por gotejamento em diferentes lâminas. Os tratamentos de irrigação foram baseados na evapotranspiração da cultura (ETc). O experimento foi instalado em blocos ao acaso com quatro tratamentos (0, 25, 50 e 100% da ETc) e seis repetições. Para análise econômica, os resultados foram extrapolados para uma área de um hectare. A produtividade em 2015 foi inferior à de 2016; porém, em todos os tratamentos, a produtividade foi sempre maior quando as plantas foram cultivadas sob irrigação adequada. Em ambos os anos, os custos operacionais totais foram maiores sob irrigação do que sob condições de sequeiro. O tratamento 100% ETc resultou em maior número de sacas de feijão-caupi (60 kg) produzidas e receita bruta superior ao tratamento de sequeiro em 17 sacas ha-1, equivalente a 1.020 kg ha-1 (receita bruta US$ 711,48) e 16 sacas ha-1, equivalente a 960 kg ha-1 (receita bruta US$ 867,12), em 2015 e 2016, respectivamente. A deficiência hídrica limitou a produtividade do feijão-caupi nos dois anos, mas inviabilizou economicamente o cultivo apenas em 2015.

Abstract Introduction: Residency in general practice and family medicine (GP/FM) is essential for acquiring knowledge and skills. As patients play a fundamental role in residents’ training, it is important to understand their perception of GP/FM residents. Objectives: To evaluate patients’ perception of the role of GP/FM residents and their trust in the medical care provided by them at USF Santiago de Leiria, a family unit in Portugal. Methods: Cross-sectional study with a convenience sample of 450 patients of USF Santiago through a questionnaire consisting of a sociodemographic survey and questions designed to assess patients' perception of the role of GP/FM residents. Results: We obtained 405 valid questionnaires, where 87.7% of patients reported having already had an appointment with a resident and 57.0% reported that they had been previously explained what medical residency was. Of the patients who had already been consulted by a resident, the majority believed that residency was important for the training of family physicians (95.0%). When asked whether they accepted the consultation to be carried out by a resident, 94.2% responded affirmatively, however when asked whether they felt free to refuse to be seen by a resident, only 70.1% replied positively. Of the total sample, 93.3% reported feeling comfortable communicating their problems to residents and 96.0% felt confident in the care they received from them. Conclusions: Although there is still some lack of knowledge about what residency is, most patients believe that residency in general practice is important for the training of future family physicians and accept their consultations to be carried out by residents.

Resumo Introdução: A formação especializada em medicina geral e familiar (MGF) é essencial para a aquisição de conhecimentos e competências. Tendo os utentes um papel fundamental na formação dos médicos internos é importante compreender a perceção dos utentes sobre os médicos internos de MGF. Objetivos: Avaliação da perceção dos utentes da USF Santiago de Leiria sobre o papel dos médicos internos de MGF e a confiança nos cuidados médicos por estes prestados. Método: Estudo transversal com uma amostra de conveniência de 450 utentes da USF Santiago através de um questionário constituído por um inquérito sociodemográfico e questões desenhadas para avaliar a perceção dos utentes sobre o papel dos médicos internos de MGF. Resultados: Foram obtidos 405 questionários válidos: 87,7% dos utentes referiam já terem tido uma consulta com um médico interno e 57,0% referiam que já lhes tinha sido explicado previamente o que era o internato médico. Dos doentes que já tinham sido consultados por um médico interno, a maioria acreditava que o internato era importante para a formação dos médicos de família (95,0%). Quando questionados sobre se aceitavam que a consulta fosse realizada por um médico interno, 94,2% responderam afirmativamente; quando inquiridos sobre se sentiam que tinham liberdade para recusar serem vistos por um médico interno apenas 70,1% responderam positivamente. Do total da amostra 93,3% referiam sentir-se confortáveis para comunicar os seus problemas aos médicos internos e 96,0% sentiam confiança nos cuidados que destes recebiam. Conclusões: Apesar de haver ainda algum desconhecimento sobre o que é um médico interno, a maioria dos utentes acredita que o internato em MGF é importante para a formação dos futuros médicos de família e aceitam que as suas consultas sejam realizadas por médicos internos.

Abstract The Centers for Disease Control and Prevention propose the term long COVID or post-COVID conditions for symptoms that develop during or after COVID-19, continue for at least two months, and are not explained by an alternative diagnosis. In this cross-sectional study, we aimed at estimating the proportion of COVID-19 patients that presented post-COVID conditions, describing their baseline characteristics and main persisting symptoms. We included 497 adult patients who had recovered from mild to moderate COVID-19, using a structured questionnaire, 12 to 16 weeks after disease onset. The cumulative incidence for post-COVID conditions was 47.5% (n=236). The most frequent symptoms were fatigue (n=151, 30.4%) and dyspnea (n=131, 26.4%). After adjusting for covariables male gender was found to be associated with lower odds of post-COVID conditions (OR=0.36, p<0.001). Body Mass Index <20 (OR=0.28, p=0.014) and [30-35[ (OR=0.42, p=0.006) seemed to have less persistent symptoms when compared to [20-25[. No other baseline demographic or clinical features were found to be significantly associated with post-COVID conditions. Our study suggests that post-COVID conditions persist in a large subset of non-severe diseases. Physicians should continue to monitor these patients to identify and treat post-acute sequelae of SARS-CoV-2 infection, particularly in what concerns primary care.

Resumo O Centro para Controlo e Prevenção de Doenças propõe o termo COVID longo ou condições pós-COVID para sintomas desenvolvidos durante ou após a COVID-19 e que persistem pelo menos dois meses, não sendo explicados por outros diagnósticos. Este estudo transversal tem como objetivo determinar a proporção de doentes com condições pós-COVID, descrevendo as suas características demográficas e clínicas e sintomas persistentes mais comuns. Foram incluídos 497 adultos com doença ligeira a moderada por COVID-19, avaliados por questionário estruturado após 12 a 16 semanas da infeção por SARS-CoV-2. A incidência cumulativa de condições pós-COVID foi de 47,5% (n=236). Os sintomas persistentes mais frequentes foram a astenia (n=151, 30,4%) e a dispneia (n=131, 26,4%). Após análise multivariada verificou-se que o sexo masculino está associado a menor incidência de condições pós-COVID (OR=0,36, p<0,001). Constatou-se ainda que índices de massa corporal <20 (OR=0,28, p=0,014) e [30-35[ (OR=0,42, p=0,006) apresentaram menor persistência de sintomas quando comparados com [20-25[. Nenhuma outra característica demográfica ou clínica apresentou associação estatisticamente significativa para condições pós-COVID. O estudo sugere que as condições pós-COVID são frequentes na doença não-severa. O acompanhamento destes doentes deverá manter-se com o intuito de identificar e tratar sequelas pós-infeção aguda por SARS-CoV-2, particularmente no que respeita aos cuidados de saúde primários.

Abstract HELLP Syndrome is one of the most severe forms of hypertensive disorders of pregnancy and is most commonly diagnosed in the third trimester. We present a rare case of a progressive partial hydatidiform mole pregnancy at 17 weeks gestation complicated by HELLP Syndrome. We aim to raise awareness of the key findings at the first-trimester prenatal screening to suspect molar pregnancies and how appropriate prenatal counselling for couples may affect their choices and outcomes. Diagnosing a molar pregnancy as early as possible is of utmost importance to prevent this serious pregnancy-related complication with a high risk of maternal morbimortality.

Resumo O Síndrome de HELLP é uma das doenças hipertensivas da gravidez mais graves e é mais frequente no terceiro trimestre. Apresentamos o caso duma gravidez molar parcial evolutiva às 17 semanas de gestação, complicada de Síndrome de HELLP. Pretendemos alertar para alterações que podem estar associadas a uma gravidez molar no rastreio pré-natal de primeiro trimestre e como o aconselhamento do casal pode afetar suas escolhas e o desfecho. O diagnóstico precoce de gravidez molar é fundamental para prevenir complicações da gravidez associadas a elevada morbimortalidade.

ABSTRACT Increasing orchard productivity is among the main objectives of pitaya producers. However, the commercial productivity may reduce due to reduction of fruit size as a function of the high amount per plant. This study evaluated the influence of the number of fruit per cladode in commercial productivity and in quality of fruits of two pitaya species at different harvest times. The experiment was carried out in a six-year-old orchard formed by the species Selenicereus undatus and Hylocereus polyrhizus. At each flowering time, the number of fruits set per cladode was counted and plants with two, four, five and six fruits per cladode were selected, and surplus fruits removed. The evaluations followed a split plot design over time, with the main plot as the number of fruit per cladode and the subplot as the harvest seasons: December, February and March, with four replicates and one plant per plot. Plants grown with two fruits per cladode presented the highest percentage of fruits with commercial size and commercial productivity. Both species showed fruits with commercial size in the first harvest of the cycle (December). S. undatus fruits classified as Class I and H. polyrhizus fruits classified as Extra and Class I showed the best quality.

Abstract Objective: To assess the predictive value of selected growth phenotypes for neonatal morbidity and mortality in preterm infants < 30 weeks and to compare them with INTERGROWTH-21st (IG21). Methods: Retrospective analysis of data from the Brazilian Neonatal Research Network (BNRN) database for very low birth weight (VLBW) at 20 public tertiary-care university hospitals. Outcome: the composite neonatal morbidity and mortality (CNMM) consisted of in-hospital death, oxygen use at 36 weeks, intraventricular hemorrhage grade 3 or 4, and Bell stage 2 or 3 necrotizing enterocolitis. Selected growth phenotypes: small-for-gestational-age (SGA) defined as being < 3rd (SGA3) or 10th (SGA10) percentiles of BW, and large-for-gestational-age (LGA) as being > 97th percentile of BW. Stunting as being < 3rd percentile of the length and wasting as being < 3rd percentile of BMI. Single and multiple log-binomial regression models were fitted to estimate the relative risks of CNMM, comparing them to IG21. Results: 4,072 infants were included. The adjusted relative risks of CNMM associated with selected growth phenotypes were (BNRN/IG21): 1.45 (0.92–2.31)/1.60 (1.27–2.02) for SGA; 0.90 (0.55–1.47)/1.05 (0.55–1.99) for LGA; 1.65 (1.08–2.51)/1.58 (1.28–1.96) for stunting; and 1.48 (1.02–2.17) for wasting. Agreement between the two references was variable. The growth phenotypes had good specificity (>95%) and positive predictive value (70-90%), with poor sensitivity and low negative predictive value. Conclusion: The BNRN phenotypes at birth differed markedly from the IG21 standard and showed poor accuracy in predicting adverse neonatal outcomes.

RESUMO: O objetivo deste estudo foi avaliar se o teste do anel do leite (TAL), utilizado para o diagnóstico da brucelose em nível de rebanho leiteiro, pode ser realizado em amostras de leite com conservador (Bronopol®) coletado para contagem de células somáticas (CCS) que é utilizado para monitoramento de mastite. Foram analisadas 38 amostras de leite de tanques de expansão (ALTE) de 19 rebanhos leiteiros produtores de queijo artesanal e 36 ALTE de rebanho leiteiro livre de brucelose, utilizadas como controle negativo (CN) e controle positivo (CP), quando foi adicionado soro de animais vacinados. As amostras de leite foram coletadas em dois frascos diferentes (com e sem Bronopol®). Todos as 38 ALTE de produtores artesanais de queijo (amostras com e sem Bronopol®) foram não reagentes ao TAL, como todos os CN. Todos os CP foram reagentes ao TAL, sem interferência do Bronopol®. Os resultados mostraram que a amostra de leite utilizada para CCS apresentaram a proporção de concordância dos resultados em todas as amostras de leite (100%), ou seja, não foram observados resultados falso-positivos e falso-negativos. Os resultados indicam que o Bronopol® não interferiu na cor no momento da leitura do TAL, sugerindo que as mesmas amostras de leite poderiam ser usadas para monitorar mastite e brucelose.

ABSTRACT: This study evaluated if the milk ring test (MRT) used to the brucellosis diagnostic at dairy herd level can be done in milk samples with conservative (Bronopol®) collected to the somatic cell count (SCC) that is used to mastitis monitoring. It were analyzed 38 bulk tank milk samples (BTMS) from 19 dairy herds artisanal cheese producers and 36 BTMS from a brucellosis free dairy herd, used as negative control (NC) and positive control (PC), when was added serum from vaccinated animals. The milk samples were collected in two different bottles (with or without Bronopol®). All the 38 BTMS from artisanal cheese producers (samples with and without Bronopol®) were non-reagent to MRT, as all NC. All PC were reagent to MRT, without interference of Bronopol®. Results showed that the milk sample used to SCC presented the proportion of agreement of results in all milk samples (100%), that is, false positive and false negative results were not observed. Results indicated that Bronopol® did not interfere with the color in the time of MRT reading suggesting that the same milk samples could be used to monitoring mastitis and brucellosis.

Abstract Consumers are more demanding in relation to meat quality, developing new demands, such as increased shelf life and good sensory quality. The tenderness of the meat is one of the most important elements in the choice of the product, the modification in the intrinsic structure of the meat, for example, increase of the proteolysis and fragmentation of the myofibrils contribute to the improvement of the tenderness. However, it is necessary to develop technological strategies so that this ultrasound technology can be added to improve meat tenderness. Thus, the objective of this review was to know the main aspects of the use of ultrasound and acerola residue in meat tenderness. Overall, existing research demonstrates excellent prospects for this new redesign approach.

Resumo: Este artigo avalia as repercussões da pandemia da COVID-19 no cotidiano de trabalho da equipe da Estratégia Saúde da Família (ESF) em diversos municípios do Nordeste brasileiro, na perspectiva dos agentes comunitários de saúde (ACS). Um questionário estruturado foi utilizado para coletar informações sobre a COVID-19, atividades dos ACS e equipes da ESF em 2021. Participaram 1.935 ACS de quatro capitais (Fortaleza - Ceará, João Pessoa - Paraíba, Recife - Pernambuco e Teresina - Piauí) e quatro cidades do interior (Crato, Juazeiro do Norte, Barbalha e Sobral, Ceará). A idade média dos ACS era de 46,25±8,54 anos, sendo a maioria mulheres (82,42%). Muitos (39,92%) estavam infectados com COVID-19, dos quais 70,78% acreditavam ter sido infectados no ambiente de trabalho. Ao todo, 77,82% definiam seu papel como linha de frente no combate à COVID-19, 16,07% relataram receber treinamento para a COVID-19 e 13,74% tinham acesso a equipamentos de proteção suficientes contra a COVID-19. A maioria (90,27%) acredita que suas rotinas de trabalho foram modificadas pela pandemia, fortalecendo o espírito de equipe (41,46%) ou enfraquecendo-o (44,41%). Houve uma redução na promoção da saúde nas escolas (75,66%) e na frequência de visitas domiciliares (60,55%), de grupos específicos na comunidade (93,96%) e outros serviços comunitários locais (66,01%). Nos municípios avaliados, observou-se uma heterogeneidade significativa em relação à resposta à pandemia de COVID-19, possivelmente associada à falta de coordenação do Governo Federal. A pandemia levou a uma reconfiguração dos sistemas locais de saúde, fluxos de trabalho e protocolos de atenção primária para as equipes da ESF. A importância do Sistema Único de Saúde (SUS) e seu potencial de reorganização durante as crises devem ser reconhecidos, preservando-se os avanços alcançados até o momento.

Resumen: Este artículo evalúa las repercusiones de la pandemia de COVID-19 en el trabajo cotidiano del equipo de la Estrategia Salud de la Familia (ESF) en diversos municipios del Nordeste brasileño, desde la perspectiva de los agentes comunitarios de salud (ACS). Se utilizó un cuestionario estructurado para recopilar información sobre COVID-19, actividades de los ACS y equipos de la ESF en el 2021. Participaron 1.935 ACS de cuatro capitales (Fortaleza - Ceará, João Pessoa - Paraíba, Recife - Pernambuco y Teresina - Piauí) y cuatro ciudades del interior (Crato, Juazeiro do Norte, Barbalha y Sobral - Ceará). La edad media de los ACS era de 46,25±8,54 años, y la mayoría eran mujeres (82,42%). Muchos (39,92%) estaban infectados con COVID-19, de los cuales el 70,78% creía haberse contagiado en el entorno laboral. En total, el 77,82% definió su papel como línea de frente en el combate a la COVID-19, el 16,07% informó haber recibido capacitación para la COVID-19 y el 13,74% tuvo acceso a equipos de protección suficiente contra la COVID-19. La mayoría (90,27%) cree que sus rutinas de trabajo se vieron modificadas por la pandemia, fortaleciendo el espíritu de equipo (41,46%) o debilitándolo (44,41%). Hubo una reducción en la promoción de la salud en las escuelas (75,66%) y en la frecuencia de las visitas domiciliarias (60,55%), de grupos específicos en la comunidad (93,96%) y otros servicios comunitarios locales (66,01%). En los municipios analizados, se observó una heterogeneidad significativa con relación a la respuesta a la pandemia de COVID-19, posiblemente asociada a la falta de coordinación del Gobierno Federal. La pandemia condujo a una reconfiguración de los sistemas locales de salud, los flujos de trabajo y los protocolos de atención primaria para los equipos de la ESF. Se debe reconocer la importancia del Sistema Único de Salud y (SUS) su capacidad de reorganización durante las crisis, preservando los avances logrados hasta el momento.

Abstract: This article evaluates the COVID-19 pandemic impacts on the Family Health Strategy (FHS) team’s work routines across a range of northeast Brazilian cities as perceived by community health workers (CHW). Data on COVID-19, CHW activities, and FHS teams were collected in 2021 by a structured questionnaire. A total of 1,935 CHWs from four state capitals (Fortaleza - Ceará State, João Pessoa - Paraíba State, Recife - Pernambuco State, Teresina - Piauí State) and four hinterland cities (Crato, Juazeiro do Norte, Barbalha, Sobral - Ceará State) participated in the study. Most CHWs were women (82.42%), with mean age 46.25±8.54 years. Many (39.92%) were infected with COVID-19, of which 70.78% believed they were infected in the workplace. A total of 77.82% defined their role as frontline in the fight against COVID-19, 16.07% reported receiving training for COVID-19, and 13.74% had access to sufficient protective equipment. Most (90.27%) believed their work routines were modified by the pandemic, either strengthening (41.46%) or weakening (44.41%) the team spirit. Home visits (60.55%), health promotion actions in schools (75.66%) and in specific community groups (93.96%), and other on-site community services (66.01%) showed a reduction in frequency. The sampled cities revealed a significant heterogeneity regarding responses to the COVID-19 pandemic, possibly associated with a lack of coordination by the Federal Government. Regardless of context, the pandemic led to a reconfiguration of local health systems, workflows, and primary care protocols for FHS teams. The importance of the Brazilian Unified National Health System (SUS) and its potential for reorganization during crisis should be acknowledged while preserving the headway made thus far.
