Longitudinal whole-brain analysis of multi-subject diffusion data in diffuse axonal injury

RESUMO Antecedentes: A lesão axonial difusa ocorre em traumas com alta energia de aceleração e desaceleração, determinando desorganização da microestrutura cerebral, levando a algum déficit. A escala de Glasgow Estendida (GOS-E) é indicada na avaliação clínica das vítimas de trauma cranioencefalico. Imagens por tensor de difusão (DTI) estudam a microestrutura cerebral a partir da difusão das moléculas de água. Objetivo: Investigar a microestrutura cerebral no primeiro ano após trauma, avaliar clinicamente os pacientes e testar para correlações entre estes resultados. Métodos: 20 vítimas de TCE moderado e grave foram avaliados 2 meses e 1 ano depois do trauma. O processamento foi feito usando o software FMRIB (FSL) e a análise estatística foi feita com tract-based spatial statistics software para extrair os parâmetros de DTI. Calculamos a diferença da média entre as duas observações de cada sujeito e fizemos um teste-t para uma amostra e threshold-free cluster enhancement. Realizamos correções para múltiplas comparações e determinamos o valor de p < 0.05 como significativo. Um ano após o trauma, a avaliação clínica foi feita usando a GOS-E. Testamos para associações entre os resultados clínicos e os valores médios de FA dos clusters. Resultados: Os clusters significativos foram identificados usando o atlas JHU WM. Observamos aumento de FA predominantemente no hemisfério cerebral direito e cerebelar à esquerda e também extensas áreas de aumento nos demais parâmetros de DTI. A recuperação dos pacientes foi satisfatória. Conclusões: Não encontramos associações entre os resultados, no entanto alguma associação pequena a moderada não pode ser excluída.
ABSTRACT Background: Diffuse axonal injury occurs with high acceleration and deceleration forces in traumatic brain injury (TBI). This lesion leads to disarrangement of the neuronal network, which can result in some degree of deficiency. The Extended Glasgow Outcome Scale (GOS-E) is the primary outcome instrument for the evaluation of TBI victims. Diffusion tensor imaging (DTI) assesses white matter (WM) microstructure based on the displacement distribution of water molecules. Objective: To investigate WM microstructure within the first year after TBI using DTI, the patient’s clinical outcomes, and associations. Methods: We scanned 20 moderate and severe TBI victims at 2 months and 1 year after the event. Imaging processing was done with the FMRIB software library; we used the tract-based spatial statistics software yielding fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (AD), and radial diffusivity (RD) for statistical analyses. We computed the average difference between the two measures across subjects and performed a one-sample t-test and threshold-free cluster enhancement, using a corrected p-value < 0.05. Clinical outcomes were evaluated with the GOS-E. We tested for associations between outcome measures and significant mean FA clusters. Results: Significant clusters of altered FA were identified anatomically using the JHU WM atlas. We found increasing spotted areas of FA with time in the right brain hemisphere and left cerebellum. Extensive regions of increased MD, RD, and AD were observed. Patients presented an excellent overall recovery. Conclusions: There were no associations between FA and outcome scores, but we cannot exclude the existence of a small to moderate association.

Development and validation of predictive model for long-term hospitalization, readmission, and in-hospital death of patients over 60 years old

ABSTRACT Objective To develop and validate a high-risk predictive model that identifies, at least, one common adverse event in older population: early readmission (up to 30 days after discharge), long hospital stays (10 days or more) or in-hospital deaths. Methods This was a retrospective cohort study including patients aged 60 years or older (n=340) admitted at a 630-beds tertiary hospital, located in the city of São Paulo, Brazil. A predictive model of high-risk indication was developed by analyzing logistical regression models. This model prognostic capacity was assessed by measuring accuracy, sensitivity, specificity, and positive and negative predictive values. Areas under the receiver operating characteristic curve with 95% confidence intervals were also obtained to assess the discriminatory power of the model. Internal validation of the prognostic model was performed in a separate sample (n=168). Results Statistically significant predictors were identified, such as current Barthel Index, number of medications in use, presence of diabetes mellitus, difficulty chewing or swallowing, extensive surgery, and dementia. The study observed discrimination model acceptance in the construction sample 0.77 (95% confidence interval: 0.71-0.83) and good calibration. The characteristics of the validation samples were similar, and the receiver operating characteristic curve area was 0.687 (95% confidence interval: 0.598-0.776). We could assess an older patient’s adverse health events during hospitalization after admission. Conclusion A predictive model with acceptable discrimination was obtained, with satisfactory results for early readmission (30 days), long hospital stays (10 days), or in-hospital death.

Association between respiratory capacity, quality of life and cognitive function in elderly individuals

RESUMO Objetivo Investigar a associação entre capacidade respiratória, qualidade de vida e função cognitiva em idosos. Métodos A amostra incluiu 386 idosos (232 mulheres). A capacidade respiratória foi avaliada pela pressão expiratória máxima obtida no pico de fluxo expiratório. Com base nos resultados máximos de fluxo expiratório, os indivíduos foram classificados de acordo com o valor previsto para sexo, idade e estatura em capacidade respiratória normal (curva de fluxo expiratório de pico inferior a 80% e superior a 60%) ou capacidade respiratória reduzida (curva de fluxo expiratório de pico menor que 60%). O questionário World Health Organization Quality of Life e o Miniexame do Estado Mental foram utilizados para avaliar a qualidade de vida e a função cognitiva, respectivamente. Resultados Em comparação com mulheres idosas com capacidade respiratória normal, as pessoas com capacidade respiratória reduzida apresentaram escores mais baixos para o Miniexame do Estado Mental (p=0,048) e qualidade de vida (p=0,040). Nos homens, não foram observadas diferenças (p>0,05). Conclusão Capacidade respiratória reduzida foi associada à pior qualidade de vida e à função cognitiva em mulheres idosas. Essas associações não foram observadas em idosos do sexo masculino.
ABSTRACT Objective To investigate associations between respiratory capacity, quality of life and cognitive function in elderly individuals. Methods The sample included 386 elderly individuals (232 women). Respiratory capacity assessment was based on maximal expiratory pressure measured at peak expiratory flow. Subjects were classified according to peak expiratory flow values adjusted for sex, age and height of individuals with normal (peak expiratory flow curve <80% and >60%) or reduced (peak expiratory flow curve < 60%) respiratory capacity. The World Health Organization Quality of Life Questionnaire and the Mini-Mental State Examination were used to assess quality of life and cognitive function, respectively. Results Elderly women with reduced respiratory capacity scored lower on the Mini-Mental State Examination (p=0.048) and quality of life questionnaire (p=0.040) compared to those with normal respiratory capacity. These differences were not observed in men (p>0.05). Conclusion Reduced respiratory capacity was associated with poorer quality of life and cognitive function in elderly women. These associations were not observed in elderly men.

RESUMO Objetivo: Descrever as manifestações clínicas de pacientes com infecção pulmonar por micobactérias do complexo Mycobacterium abscessus (CMA) e compará-las com as daqueles infectados com outras micobactérias não tuberculosas (MNT). Métodos: Estudo de coorte retrospectivo envolvendo 43 pacientes divididos em dois grupos: grupo CMA, com pacientes com infecção pulmonar por CMA (n = 17); e grupo MNT, com pacientes com infecção pulmonar por MNT que não CMA (n = 26). Os pacientes foram previamente tratados com o esquema rifampicina, isoniazida, pirazinamida e etambutol antes de o diagnóstico de MNT ser confirmado com culturas positivas em duas amostras de escarro diferentes. As sequências nucleotídicas dos genes hsp65, RNAr 16S e/ou rpoB foram analisadas para a identificação das micobactérias. Foram coletadas características demográficas, clínicas e radiológicas, assim como respostas terapêuticas e desfechos. Resultados: A única manifestação clínica significativamente mais frequente no grupo CMA que no grupo MNT foi hiporexia (p = 0,0306). A chance de haver a necessidade de utilização de um segundo esquema terapêutico foi quase 12 vezes maior no grupo CMA que no grupo MNT. O sucesso terapêutico foi significativamente maior no grupo MNT que no grupo CMA (83,2% vs. 17,6%; p < 0,0001). A chance de recidiva no grupo CMA foi aproximadamente 37 vezes maior que no grupo MNT. Conclusões: Na amostra estudada, a resposta terapêutica da doença pulmonar causada por CMA evoluiu de forma menos favorável do que naquela causada pelas demais MNT.
ABSTRACT Objective: To describe the clinical manifestations of patients with pulmonary infection caused by mycobacteria of the Mycobacterium abscessus complex (MABSC), and to compare these manifestations with those of patients infected with other nontuberculous mycobacteria (NTM). Methods: This was a retrospective cohort study involving 43 patients divided into two groups: the MABSC group, consisting of patients with pulmonary infection caused by MABSC (n = 17); and the NTM group, consisting of patients with pulmonary infection caused by NTM other than MABSC (n = 26). Patients were previously treated with a regimen of rifampin, isoniazid, pyrazinamide, and ethambutol before the diagnosis of NTM was confirmed by two culture-positive sputum samples. The nucleotide sequences of the hsp65, 16S rRNA, and/or rpoB genes were analyzed to identify the mycobacteria. Data were collected on demographic, clinical, and radiological characteristics, as well as on treatment responses and outcomes. Results: Loss of appetite was the only clinical manifestation that was significantly more common in the MABSC group than in the NTM group (p = 0.0306). The chance of having to use a second treatment regimen was almost 12 times higher in the MABSC group than in the NTM group. Treatment success was significantly higher in the NTM group than in the MABSC group (83.2% vs. 17.6%; p < 0.0001). The chance of recurrence was approximately 37 times higher in the MABSC group than in the NTM group. Conclusions: In the study sample, treatment response of pulmonary disease caused by MABSC was less favorable than that of pulmonary disease caused by other NTM.

Health-related quality of life in Brazilian community-dwelling and institutionalized elderly: Comparison between genders

RESUMO Objetivo comparar os indicadores de qualidade de saúde de vida (QV) entre idosos homens e mulheres institucionalizados e aqueles que vivem na comunidade sozinhos ou com a família. Método estudo transversal com amostra composta por 496 idosos homens e mulheres, entrevistados por pesquisadores em um hospital privado que atende idosos institucionalizados e da comunidade. Os indicadores de QV (World Health Organization Quality of Life), atividades da vida diária (Katz questionnaire), atividades instrumentais da vida diária (Lawton questionnaire), exame do estado mental (mini-mental), teste de força de preensão manual (Handgrip) e capacidade funcional (Timed up to go) foram obtidos. Resultados homens institucionalizados apresentaram maiores escores de QV no domínio físico e psicológico em relação aos idosos que vivem na comunidade sozinhos (p<0,05). Nas mulheres, os escores em todos os domínios de QV (físico, psicológico, relações sociais e ambientais) foram semelhantes entre as idosas institucionalizadas e as residentes na comunidade, que vivem sozinhas ou com a família. Conclusão homens idosos institucionalizados relataram melhores escores de QV nos domínios físico e psicológico em comparação com pares que vivem na comunidade, enquanto, em mulheres, resultados similares foram observados nas idosas institucionalizadas e da comunidade.
SUMMARY Objective To compare the health-related quality of life (HRQL) indicators between institutionalized and community-dwelling elderly men and women. Method This was a cross-sectional study with a sample of 496 elderly men and women, surveyed by researchers at a private hospital that attends institutionalized and community-dwelling elderly. HRQL (World Health Organization Quality of Life), daily living activities (Katz questionnaire), and instrumental daily living activities (Lawton questionnaire), mini-mental state examination, handgrip strength test, and function capacity (timed up and go test) were obtained. Results Institutionalized men presented higher scores in physical and psychological domains of HRQL compared to elderly men living alone (p<0.05). Among women, the scores in all domains (physical, psychological, relationship, and environment) were similar between institutionalized and community-dwelling individuals. Conclusion Institutionalized elderly men reported better scores in physical and psychological domains of HRQL compared to their community-dwelling pairs, while both institutionalized and community-dwelling elderly women presented similar HRQL.

Peak expiratory flow mediates the relationship between handgrip strength and timed up and go performance in elderly women, but not men

OBJECTIVE: The aim of the present study was to verify if there is sex difference in the associations among handgrip strength, peak expiratory flow (PEF) and timed up and go (TUG) test results. METHODS: The sample included 288 consecutive elderly men (n=93) and women (n=195). Functional capacity was measured using the TUG test, and muscle strength was measured based on handgrip. Moreover, as a measure of current health status, PEF was evaluated. Linear regression procedures were performed to analyze the relationships between handgrip and both PEF and TUG test results, with adjustment for confounders, and to identify the possible mediating role of PEF in the association between handgrip strength and TUG test results. RESULTS: In men, handgrip strength was associated with both PEF and TUG performance (p<0.01). After adjustment for PEF, the relationship between handgrip strength and TUG performance remained significant. In women, handgrip strength was also associated with both PEF and TUG performance (p<0.01). However, after adjustment for PEF, the relationship between handgrip strength and TUG performance was no longer significant. CONCLUSION: Mobility in the elderly is sex dependent. In particular, PEF mediates the relationship between handgrip strength and TUG performance in women, but not in men.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Produção e qualidade de melão influenciados por matéria orgânica, nitrogênio e micronutrientes

Avaliou-se a influência de nitrogênio, boro, molibdênio e zinco aplicados por fertirrigação e adição de esterco de gado ao solo sobre a produtividade e qualidade dos frutos de melão (Cucumis melo), em dois solos do Submédio São Francisco. Foram realizados seis experimentos nos anos de 1998, 1999 e 2000, sendo três num Latossolo Vermelho-amarelo, textura arenosa, em Petrolina (PE) e três num Vertissolo, em Juazeiro (BA). O delineamento experimental foi em blocos ao acaso, com quatro repetições e nove tratamentos, assim constituídos: 1- N1, 2- N2, 3- N3, 4- N1 + EG, 5- N2 + EG, 6- N2 + (B + Mo + Zn), 7- N2 + (B + Mo), 8- N2 + (B + Zn) e 9- N2 + (Mo +Zn), em que N1, N2 e N3 representam 0 ou 30, 80 e 160 kg ha-1 de N, respectivamente; EG, 20 m³ ha-1 de esterco de gado; B, 1,1 kg ha-1 de B; Mo, 0,4 kg ha-1 de Mo e Zn, 4 kg/ha de Zn. O N e os micronutrientes foram aplicados via água de irrigação. A produtividade do melão apresentou resposta a nitrogênio no Latossolo em 1998 e 2000 e no Vertissolo em 1999. Foi com 80 kg/ha de N que se obtiveram produtividade significativamente superior (32,0 a 40,2 t ha-1) em maior ocorrência dos experimentos. O EG provocou aumentos significativos apenas na produtividadede frutos, em 1998 e no peso médio dos frutos (PMF) em 2000, no Vertissolo. O teor de sólidos solúveis dos frutos aumentou com a aplicação de N no Latossolo, em 2000. Os micronutrientes não exerceram efeito significativo consistente. Em dois anos, o Vertissolo proporcionou PMF superior ao do Latossolo.
The effects of nitrogen, boron, molybdenum and zinc applied as fertigation, and cattle manure applied to the soil, were evaluated on the yield and quality of melon (Cucumis melo) in two types of soils of the Submedio São Francisco River Valley, Brazil. Six experiments were carried out in 1998, 1999 and 2000, three being in a red yellow Latosoil in Petrolina, and three in a Vertisoil, in Juazeiro, in a randomized complete block design with four replications and nine treatments, as follows: 1. N1; 2. N2; 3. N3; 4. N1 + C.M.; 5. N2 + C.M.; 6. N2 + B + Mo + Zn; 7. N2 + B + Mo; 8. N2 + B + Zn, and 9. N2 + Mo + Zn, where N1, N2 and N3 represent, respectively, 0 or 30, 80 and 160 kg/ha of N; C.M. was 20 m³ ha-1 of manure; B = 1.1 kg ha-1; Mo = 0.4 kg ha-1, and Zn = 4 kg ha-1. Nitrogen and the micronutrients were applied through irrigation water. Melon yield showed response to nitrogen in the Latosoil in 1998 and in 2000, and in the Vertisoil in 1999. Higher significant yields were obtained with 80 kg ha-1 of N, varying among experiments from 32.0 to 40.2 t ha-1. Cattle manure caused significant increase only in yield in 1998 and in mean fruit weight in 2000 in the Vertisoil. Soluble solid contents in fruits increased with nitrogen application in the Latosoil in 2000. The micronutrients did not have significant effect on yield. In two years, mean fruit weight was higher in Vertisoil than in Latosoil.

Níveis de nitrogênio por fertirrigação e densidade de plantio na cultura do melão em um Vertissolo

O trabalho constou de um experimento com melão (Cucumis melo L.), conduzido em um Vertissolo, em Juazeiro, BA, em 1995, com o objetivo de avaliar o efeito de níveis de N por fertirrigação e de densidades de plantio na produtividade e qualidade de fruto. Os níveis de N foram 0, 80, 130 e 180 kg/ha, combinados com os espaçamentos 2,00 e 1,80 m entre linhas e 0,20 m entre plantas, com uma ou duas plantas por cova. A fonte de N foi a uréia, aplicada diariamente até 42 dias após a germinação, por meio da irrigação por gotejamento. Todos os tratamentos receberam uma adubação uniforme de 120 kg/ha de P2O5 e 120 kg/ha de K2O. Os espaçamentos entre linhas não causaram diferenças significativas em nenhuma variável estudada. O nível de 80 kg/ha de N combinado com uma planta por cova proporcionou uma produtividade de 34,07 t/ha, com 55,7% de frutos próprios para o mercado interno, não-significativamente (P <FONT FACE=Symbol>£</FONT> 0,05) inferior à produtividade obtida com os níveis mais elevados de N em qualquer combinação. Com este mesmo nível, obtiveram-se frutos com 10,22º Brix significativamente (P<FONT FACE=Symbol>£</FONT> 0,05) superior ao do tratamento sem N e não-significativamente inferior ao dos outros níveis. Para se obter uma maior parte de frutos próprios para o mercado externo, foi necessário elevar a densidade para duas plantas por cova e o nível de N para 130 ou 180 kg/ha. O peso médio dos frutos aumentou de 1,008 para 1,705 kg, à medida que foram aumentados os níveis de N ou se diminuiu a densidade de plantio de duas para uma planta por cova.
This study consisted of one experiment with melon (Cucumis melo L.), carried out in a Vertisol in Juazeiro, BA, Brazil, in 1995, with the objective of evaluating the effects of nitrogen levels through fertirrigation and plant density on fruit yield and quality. The N levels were 0, 80, 130 and 180 kg/ha, combined with row spacings of 2.0 and 1.8 m and 0.20 m between plants within the row, with one or two plants/hole. The source of N was urea applied daily up to 42 days after germination, through drip irrigation. All treatments had a uniform fertilization of 120 kg/ha of P2O5 and 120 kg/ha of K2O. No significant difference was caused by spacing between rows in the studied variables. Eighty kg/ha of N combined with one plant per hole gave a yield of 34.07 ton/ha, being 55,7% of fruit allocated to inside market, not significantly lower than those obtained with the highest N levels in any combination. This same N level gave fruits with 10.22º Brix, significantly higher than the treatment without N and not significantly lower than the other levels. In order to get high number of good fruit for out side market it was necessary to elevate the density to two plants per hole and N level to 130 or 180 kg/ha. Mean weight of fruits increased from 1.008 to 1.705 kg with the increase in the levels of N and the decrease in plant density of two plants per hole to one plant per hole.
