Resumo: Introdução: O trabalho discute a prática da psicoterapia na modalidade online, por meio do uso de dispositivos tecnológicos. Conhecida como psicoterapia online, a possibilidade de psicólogos e psicólogas realizarem atendimentos por meio de celulares e computadores mediados pela Internet é uma alternativa de trabalho existente há muitos anos, acompanhando os avanços acelerados da tecnologia no mundo inteiro. Com o surgimento da pandemia da COVID-19, e levando em consideração as orientações recomendadas pela OMS, a psicoterapia online emergiu como uma alternativa de trabalho para psicólogos e psicoterapeutas, possibilitando aos profissionais adaptarem os seus atendimentos para dar continuidade ao objetivo de mobilizar sentidos subjetivos. Objetivos: Baseando-se na Teoria da Subjetividade, proposta por González Rey, a pesquisa teve por objetivo de investigar o uso de dispositivos tecnológicos para realizar sessões de psicoterapia online e as implicações desta modalidade para futuras práticas de psicólogos e psicoterapeutas. Métodos: Com base nos fundamentos da Epistemologia Qualitativa, utilizando o método construtivo-interpretativo, foram realizados encontros virtuais por meio do Google Meet. Os encontros foram gravados e transcritos para a coleta de informação e construção de hipóteses e para desenvolver reflexões sobre as dificuldades em mudar os hábitos e formas de trabalho, a necessidade de superar os desafios impostos por essas mudanças e o fortalecimento do compromisso do profissional com sua profissão e seus pacientes. Resultados: A pesquisa mostrou que a psicoterapia online é uma maneira eficiente de ajudar a melhorar a saúde mental dos pacientes, mas encontra ainda desafios, tais como: adaptação das formas de trabalho, superação de preconceitos e adesão ao ambiente virtual. Considerações Finais: A psicoterapia online, com suas vantagens e desvantagens levantadas neste trabalho, ainda é um processo que está sendo subjetivado, ganhando maior relevância na atualidade devido às restrições da pandemia. A modalidade precisa ser repensada e refletida para outras situações atípicas.

Abstract: Introduction: This research paper discusses the practice of psychotherapy using technological devices. Known as online psychotherapy, the possibility for psychologists to provide aid through cell phones and computers mediated by the Internet is an alternative method that has existed for many years, thanks to the accelerated advances of technology worldwide. With the emergence of COVID-19 pandemic and considering the recommended guidelines by the WHO, online psychotherapy emerged as a viable alternative for psychologists and psychotherapists, making it possible for professionals to adapt their work to continue the objective of mobilizing subjective senses. Goals: Based on González Rey’s Theory of Subjectivity, this research aimed to inquire into the use of technological devices to carry out online psychotherapy sessions and its implications for future practices of psychologists and psychotherapists. Methods: Based on the foundations of Qualitative Epistemology and using the constructive-interpretative method, virtual meetings were held through Google Meet. The meetings were recorded and transcribed to gather information and build hypotheses and raise several considerations on the difficulties in changing working habits, on the need to overcome the challenges imposed by these changes, and the commitment to the profession and the patient. Results: The research has shown that online psychotherapy is an efficient way to help improve patients' mental health, but it still faces challenges, such as: adapting ways of working, overcoming prejudices, and adhering to the virtual environment. Conclusions: Online psychotherapy, with advantages and disadvantages presented in this paper, is still a process that is being subjectivized. Despite its relevance under the pandemic, the modality requires further elaboration and methodological inquiries for its use in other atypical situations.

SUMMARY OBJECTIVE: It is not uncommon for medical residents to deal with critically ill patients who frequently show several ethical and human dilemmas, highlighting the need for a consultation with ethical specialists. The objective of this article is to present a description of a Brazilian Ethics Consultation group designed to attend psychiatry residents. METHODS: This article reports a case of a critically ill patient with Borderline Personality Disorder with multiple intervention failures and several ethical conflicts who was seen by a resident and supported by an ethics consultation group. RESULTS: When medical residents and medical staff face severe and unusual ethical dilemmas, they might feel unprepared and have ones’ mental health impaired. Thus, this article reports a successful ethics consultation and discusses its development in other academic institutions. CONCLUSION: Medical educators and staff from academic hospitals should pay attention to the needs of the medical residents. The development and support of ethics consultation groups must be provided to fulfill the need of those residents who face serious ethical and human dilemmas.

OBJECTIVE: In 2020, the COVID-19 pandemic brought a work and stress overload to healthcare workers, increasing their vulnerability to mental health impairments. In response, the authors created the COMVC-19 program. The program offered preventive actions and mental health treatment for the 22,000 workers of The Hospital das Clinicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP). This paper aims to describe its implementation and share what we have learned from this experience. METHODS: Workers were able to easily access the program through a 24/7 hotline. Additionally, a mobile phone app that screened for signs and symptoms of emotional distress and offered psychoeducation and/or referral to treatment was made available. Data from both these sources as well as any subsequent psychiatric evaluations were collected. RESULTS: The first 20 weeks of our project revealed that most participants were female, and part of the nursing staff working directly with COVID-19 patients. The most frequently reported symptoms were: anxiety, depression and sleep disturbances. The most common diagnoses were Adjustment, Anxiety, and Mood disorders. CONCLUSIONS: Implementing a mental health program in a multimodal intervention was feasible in a major quaternary public hospital. Our data also suggests that preventive actions should primarily be aimed at anxiety and depression symptoms, with a particular focus on the nursing staff.

Abstract The present study sought to evaluate the impact of quarantine resulting from the coronavirus disease 2019 (COVID-19) pandemic on dental appointments and patients’ positions and concerns regarding their ongoing dental treatment. Patients from private dental clinics answered an online questionnaire anonymously regarding their treatment, availability and willingness to attend dental appointments, and concerns about contamination. Descriptive statistics of the responses were performed with percentages and responses were compared between sexes, regions, and other aspects using the chi-squared test. Five hundred ninety-five patients (412 females and 183 males; mean age: 38.21 years) answered the questionnaire. Most patients reported they were receiving dental treatment (orthodontics) and would attend to a dental appointment; meanwhile, those patients not receiving treatment would not attend or would visit only in the case of an emergency. Males reported to be calmer than females, who were more anxious and afraid; as such, males reported more willing to go a dental appointment while, in general, females were not worried about how quarantine could affect dental treatment. Patients actively undergoing treatment and orthodontic patients were more concerned about a delay in treatment. There was a significant association between feelings about the COVID-19 pandemic and the level of willingness to attend a dental appointment. The quarantine recommended due to the COVID-19 pandemic was shown to have an impact on dental appointments and the anxiety levels of patients, since there was a significant association between patients’ feelings and their willingness to attend a dental appointment. Overall, patients undergoing dental treatment and orthodontics were more willing to attend an appointment and were more concerned about an increase in treatment duration.

Objective: To compare the presence of criteria listed in the DSM-5 and ICD-10 diagnostic manuals in a Brazilian sample of transgender persons seeking health services specifically for physical transition. Methods: This multicenter cross-sectional study included a sample of 103 subjects who sought services for gender identity disorder in two main reference centers in Brazil. The method involved a structured interview encompassing the diagnostic criteria in the two manuals. Results: The results revealed that despite theoretical disagreement about the criteria, the manuals overlap regarding diagnosis confirmation; the DSM-5 was more inclusive (97.1%) than the ICD-10 (93.2%) in this population. Conclusions: Although there is no consensus on diagnostic criteria on transgenderism in the diversity of social and cultural contexts, more comprehensive diagnostic criteria are evolving due to society’s increasing inclusivity.

A investigação de trabalhadores dos serviços de saúde como reservatório e disseminadores de bactérias patogênicas tem sido referida como estratégia de prevenção e controle das infecções relacionadas à assistência à saúde. Este estudo buscou avaliar a presença de Enterobacteriaceae na cavidade bucal de trabalhadores de hospital oncológico do Centro-Oeste brasileiro, bem como caracterizar o perfil fenotípico dos isolados. Foi coletada amostra de saliva de 294 trabalhadores pertencentes às equipes de saúde e de apoio. Procedimentos microbiológicos foram realizados segundo técnicas referendadas. Dentre os participantes, 55 (18,7%) estavam colonizados por Enterobacteriaceae na cavidade bucal. Foram isoladas 64 bactérias, incluindo espécies potencialmente patogênicas. A espécie mais prevalente foi Enterobacter gergoviae (17,2%). As maiores taxas de resistências foram observadas para os β-lactâmicos e 48,4% dos isolados foram considerados multirresistentes. Para as enterobactérias pesquisadas, a produção de ESBL e KPC foi negativa. Porém, dentre os 43 isolados do grupo CESP, 51,2% foram considerados produtores de β-lactamase AmpC por indução e 48,8% mutantes hiperprodutores. Considera-se a prevalência de portadores de Enterobacteriaceae significativa e o perfil fenotípico dos isolados preocupante, especialmente pela multirresistência e produção de β-lactamases AmpC.

The evaluation of workers as potential reservoirs and disseminators of pathogenic bacteria has been described as a strategy for the prevention and control of healthcare-associated infections (HAIs). The aim of this study was to evaluate the presence of Enterobacteriaceae in the oral cavity of workers at an oncology hospital in the Midwest region of Brazil, as well as to characterize the phenotypic profile of the isolates. Saliva samples of 294 workers from the hospital’s healthcare and support teams were collected. Microbiological procedures were performed according to standard techniques. Among the participants, 55 (18.7%) were colonized by Enterobacteriaceae in the oral cavity. A total of 64 bacteria were isolated, including potentially pathogenic species. The most prevalent species was Enterobacter gergoviae (17.2%). The highest rates of resistance were observed for β-lactams, and 48.4% of the isolates were considered multiresistant. Regarding the enterobacteria isolated, the production of ESBL and KPC was negative. Nevertheless, among the 43 isolates of the CESP group, 51.2% were considered AmpC β-lactamase producers by induction, and 48.8% were hyper-producing mutants. The significant prevalence of carriers of Enterobacteriaceae and the phenotypic profile of the isolates represents a concern, especially due to the multiresistance and production of AmpC β-lactamases.

Background: Warthin's tumor, a benign tumor of the salivary glands, was first described by Dr. Hildrebrand in 1896. It was later extensively described by Dr. Warthin in 1929. It represents the second most frequent neoplasia found in the salivary glands. It constitutes 10% of all salivary glands tumors. Methods: A salivary gland retrospective study was conducted at the Pathological Anatomy Department of the «Calixto García» Hospital in Havana, Cuba. The study was conducted in the period comprised between January 2001 and December 2008. Two oral pathology experts re-diagnosed the cases following diagnostic criteria proposed in 2005 by the World Health Organization (WHO). Results: Out of 159 studied cases, 113 (71.1%) were benign tumors (48 male patients, 65 female, average age 50.8 [± 15], age range 20-80 years). Most frequently found tumor was pleomorphic adenoma, which counted 77 cases (68.1%) followed by Warthin's tumor with 33 cases (20.7%) of all included salivary glands tumors and 29.2% of benign tumors. Conclusions: The present study revealed the fact that Warthin's tumor proportion is among the highest reported in recent years in scientific literature.

Antecedentes: El tumor de Warthin -tumor benigno de glándulas salivales- fue reportado por primera vez en 1895 por Hildebrand y posteriormente en 1929 Warthin lo describió ampliamente. Es la segunda neoplasia más frecuente de las glándulas salivales, representando el 10% de todos los tumores de glándulas salivales. Métodos: Se realizó un estudio retrospectivo de tumores de glándulas salivales del Departamento de Anatomía Patológica del Hospital «Calixto García» de La Habana, Cuba, durante el periodo de enero del 2001 a diciembre del 2008. Los casos fueron rediagnosticados por dos expertos en patología bucal, basados en los criterios diagnósticos propuestos por la Organización Mundial de la Salud en el 2005. Resultados: De 159 casos, 113 (71.1%) fueron tumores benignos (48 hombres, 65 mujeres; promedio de edad 50.8 [± 15], rango de edad 20-80 años). El tumor más frecuente fue el adenoma pleomorfo con 77 casos (68.1%), seguido del tumor de Warthin con 33 casos, que constituyen el 20.7% del total de tumores de glándulas salivales incluidos y el 29.2% del total de los tumores benignos. Conclusiones: En nuestro estudio la proporción del tumor de Warthin está dentro de las más altas, acorde a lo reportado en la literatura de los últimos años.

A escolha de alimentos na composição da dieta é determinante do estado de saúde dos indivíduos. Atualmente, há gradualmente menos condições de escassez e mais situações caracterizadas por excesso de alimentos. Modificações na renda e preços relativos geram efeitos mensuráveis no padrão alimentar da população. O poder explicativo de modelos econômicos sobre demanda de alimentos é significativo, sendo que interações entre consumo, renda e preços são usualmente expressas sob forma de elasticidade. Entretanto, devem-se ressaltar deficiências na construção de alguns estudos, especialmente quanto à aplicação em políticas públicas. O presente artigo conceitual discute o potencial de contribuição de estudos de demanda de alimentos, buscando sugerir aperfeiçoamentos ao desenho estrutural de tais estudos para inclusão de conceitos atuais em nutrição no redirecionamento da transição nutricional diretamente da desnutrição à alimentação saudável, evitando a tendência atual à obesidade epidêmica.

Food choice in diet composition is a determinant of individual health status. Currently, there are gradually fewer conditions involving food scarcity and more involving excess food. Changes in income and relative prices generate measurable effects on the population's food intake patterns. Economic models have significant explanatory power for food demand, and the interactions between consumption, income, and prices are usually expressed as elasticity. However, the construction of some studies shows important shortcomings, especially for public policy application. This conceptual article discusses the potential contribution of food demand studies, suggesting improvements in the structural design of such studies with the inclusion of current nutritional concepts for redirecting the nutritional transition from under-nutrition to healthy eating, avoiding the present trend towards epidemic obesity.

O objetivo deste trabalho é salientar uma alternativa eficaz no tratamento da ruptura dos ligamentos cruzados cranial e caudal de cães, sem associação de imobilização externa. Foram atendidos seis animais portadores de ruptura de ambos os ligamentos cruzados. Optou-se pela correção cirúrgica intracapsular, utilizando-se dois implantes sintéticos de polipropileno, para a estabilização da articulação. Os pacientes retornaram ao apoio completo do membro em 11,8±3,5 dias e não mantiveram instabilidade articular, após 0, 30 e 90 dias de avaliação clínica pós-operatória. Aos 90 dias após a cirurgia, não se percebeu claudicação nos pacientes avaliados. Concluiu-se que a técnica utilizada promoveu total estabilidade articular na ausência dos ligamentos cruzados cranial e caudal.

The objective of this study is to point out an effective alternative in the treatment of the cranial and caudal cruciate ligaments rupture in dogs, with no association of external immobilization. Six dogs with rupture of both cruciate ligaments were included in the present study. Stifle joint was surgically stabilized by an intracapsular technique, using two polypropylene synthetic implants. The animals returned to full member support in 11.8±3.5 days and did not sustain any joint instability degree after 0, 30 and 90 days of clinical evaluation after surgery. At 90 days after surgery, it became apparent lameness in any of the six patients. It was concluded that the technique employed caused total joint stability in the absence of caudal and cranial cruciate ligaments.

A cultura do Eucalipto tem importância econômica muito elevada no Brasil e vem sofrendo inúmeros ataques de patógenos, além de essa cultura estar sujeita aos inúmeros estresses ambientais. A resistência e sobrevivência desta cultura frente às circunstâncias ambientais desfavoráveis, e ataques de pragas e doenças, revelaram que o Eucalipto apresenta um sistema de defesa evoluído e altamente eficiente. Aqui nós apresentamos os resultados obtidos no projeto do genoma de ESTs do Eucalyptus (FORESTs). Agrupando as seqüências de cDNA expressas (ESTs) obtidas pelo projeto, montou-se grupos (contigs) de seqüências similares provenientes de cada biblioteca de cDNA induzidas e não induzidas por agentes estressantes, identificou-se os grupos de seqüências de cDNA similares a outras moléculas já conhecidas, como moléculas de plantas-sinalizadoras, fitoalexinas e biossíntese de vias de lignina, proteínas-RP e supostos genes correspondendo às enzimas envolvidas na detoxificação de moléculas de oxigênio reativo. Apresentamos também, considerações gerais sobre os mecanismos de defesa do Eucalipto contra o estresse biótico e abiótico. Estas informações são de extraordinário valor para a produção de Eucalipto e ajudaram no futuro a direcionar programas de melhoramento do Eucalipto, visando assim obter plantas com melhor resistência contra os patógenos e estresses ambientais.

Eucalypt plantation has high economical importance in Brazil; however, it has been attacked by various pathogens under different environmental stress conditions. Disease resistance and survival under unfavorable environmental conditions have revealed that the eucalypt has developed highly efficient defense systems. Here we show the results of the Eucalyptus ESTs Genome Project (FORESTs). Using the expressed sequence tags (ESTs) obtained by the Project, contigs of similar sequences from each cDNA library induced and not induced by stress agents were formed, and cDNA sequences similar to other already known molecules, such as plant-signaling molecules, phytoalexins, lignin biosynthesis pathways, PR-proteins and putative genes corresponding to enzymes involved in the detoxification of reactive oxygen species, were identified. We also present general considerations about the mechanisms of Eucalyptus defense against biotic and abiotic stresses. These data are of extreme importance for future eucalypt breeding programs aimed at developing plants with enhanced resistance against pathogens and environmental stresses.

Pseudomonas aeruginosa é uma bactéria frequentemente isolada no ambiente hospitalar. Este estudo teve como objetivo avaliar o perfil de suscetibilidade de Pseudomonas aeruginosa previamente isoladas de pacientes internados em um hospital de Goiânia (Goiás-Brasil); realizar a triagem fenotípica para a produção de metalo-beta-lactamase e detectar os genes das mesmas pela técnica de "Polimerase Chain Reaction". Foram avaliadas 75 Pseudomonas aeruginosa isoladas no período de janeiro de 2005 a janeiro de 2007. A identificação bioquímica foi realizada pelo sistema API 20E® e o antibiograma pelo método de Kirby-Bauer. Entre os 62 isolados que foram resistentes ao imipenem e à ceftazidima, 35 (56,4%) apresentaram produção de metalo-beta-lactamase e em 26 (74,3%) destes, foi detectado o gene blaSPM-1. A frequência de Pseudomonas aeruginosa produtoras de metalo-beta-lactamase sugere um maior controle da disseminação de resistência no ambiente hospitalar.

Pseudomonas aeruginosa is a bacterium frequently isolated from hospital environments. This study had the aims of evaluating the susceptibility profile of Pseudomonas aeruginosa previously isolated from patients in a hospital in Goiânia (Goiás, Brazil), performing phenotypic screening for metallo-beta-lactamase production and detecting its genes using the polymerase chain reaction technique. Seventy-five 75 Pseudomonas aeruginosa isolates were evaluated between January 2005 and January 2007. Biochemical identification was performed using the API 20E® system and an antibiogram was produced using the Kirby-Bauer method. Among the 62 isolates that were resistant to imipenem and ceftazidime, 35 (56.4%) produced metallo-beta-lactamase, while 26 (74.3%) showed the blaSPM-1 gene. The frequency of Pseudomonas aeruginosa that produces metallo-beta-lactamase suggests that greater control over the dissemination of resistance in hospital environments is needed.

O presente estudo teve como objetivo desenvolver um procedimento de baixo custo, preciso, rápido e prático para estimar o grau de acidose metabólica, para tratar bovinos com quadros de acidose láctica ruminal. A acidose ruminal foi induzida experimentalmente em 40 novilhos mestiços de 1,5 anos de idade, implantados com cânula ruminal. Essa indução causou o surgimento de sinais clínicos muito típicos da enfermidade aguda, com o aparecimento de pronunciada acidose ruminal e acidose metabólica de grau moderado, caracterizado por baixo pH sangüíneo e diminutos teores de bicarbonato e excesso de base (BE) no sangue. Verificou-se uma alta correlação positiva (r = 0,80) entre o pH urinário e o BE e entre o pH urinário e o pH sangüíneo (r = 0,75). A concentração de BE estimado pelo pH urinário foi similar à obtida pela análise do hemogasômetro (P = 0,99). Além disso, os resultados apresentados pela fórmula de predição foram muito significativos. Dessa forma, conclui-se que a mensuração do pH urinário é uma boa alternativa para estimar a quantidade necessária de tampão para tratar o quadro de acidose metabólica em bovinos com acidose láctica ruminal aguda.

The purpose of the present study was to establish a practical, fast, precise and low-cost procedure to estimate the degree of metabolic acidosis in cattle with acute rumen lactic acidosis for further treatment. The rumen acidosis was induced experimentally in 40 crossbreed rumen-cannulated 1.5-year-old steers. The induction caused the development of the most characteristic clinical signs of acute rumen lactic acidosis, severe rumen acidosis and a moderate metabolic acidosis, which was evidenced by low blood pH, and blood bicarbonate concentration and base excess (BE). A highly positive correlation (r=0.80) between urinary pH and BE concentration, and between urinary pH and blood pH (r=0.75) was observed. The BE concentration estimated by urinary pH was similar to that determined by venous blood gas analysis (P>0.99). Furthermore, the results presented by the predictive formula were very significant. In conclusion, urinary pH is a good tool to predict the quantity of buffers needed to treat metabolic acidosis in cattle with acute rumen lactic acidosis.

OBJETIVO: Analisar a evolução (1971-1999) da disponibilidade domiciliar de alimentos e nutrientes no município de São Paulo. MÉTODOS: O estudo utiliza dados de duas Pesquisas de Orçamentos Familiares, conduzidas pela Fundação Instituto de Pesquisas Econômicas, durante os períodos de 1971-1972 (2.380 domicílios) e 1998-1999 (2.351 domicílios) na região urbana do município de São Paulo. A disponibilidade diária per capita de alimentos foi estimada pelo cálculo da fração da quantidade total de alimentos adquirida em um mês, atribuível a cada indivíduo participante na pesquisa, com posterior divisão pelo número de dias do mês. O padrão alimentar da população foi caracterizado de acordo com a participação relativa de 18 grupos de alimentos e nutrientes na aquisição do total de alimentos nos respectivos anos. RESULTADOS: Os resultados mostram aumento no consumo de carnes, lácteos, alimentos processados e refeições prontas. Por outro lado, observou-se redução no consumo de cereais, leguminosas, frutas e hortaliças. O consumo de açúcar, óleos e gorduras apresentou leve redução, entretanto, considerando-se a elevação no consumo de alimentos processados observada, não há como caracterizá-la como melhoria na qualidade da dieta. Em paralelo, observou-se aumento no consumo de proteínas, gorduras e cálcio; enquanto o consumo de carboidratos, fibras, folato e vitamina C apresentou redução. CONCLUSÃO: As mudanças observadas no padrão alimentar da população de São Paulo são consistentes com os crescentes índices de prevalência de obesidade e doenças crônicas não-transmissíveis observados no município.

OBJECTIVE: Food budget surveys data were analyzed in order to describe the evolution (1971-1999) of food and nutrient availability in São Paulo, Brazil. METHODS: The study uses data from two Family Budget Surveys conducted by Fundação Instituto de Pesquisas Econômicas, carried out during the periods of 1971-1972 (2,380 households) and 1998-1999 (2.351 households) in the urban area of São Paulo city. Per capita daily food availability was estimated by calculating the fraction of all the food bought for the month attributed to each participant of this study divided by the number of days in the month. The food patterns of this population were characterized according to the caloric and nutrient distribution of the 18 food groups and nutrients obtained in the respective surveys. RESULTS: The results show increased consumption of meats, dairy, processed foods and chilled and frozen meals. On the other hand, the consumption of grains, fruits and vegetables decreased. The consumption of sugar, oils and fats reduced slightly but since the consumption of processed foods increased, it is not possible to say that diet quality improved. At the same time the consumption of proteins, fats and calcium increased while the consumption of carbohydrates, fiber, folate and vitamin C decreased. CONCLUSION: The changes in the eating patterns of the population of São Paulo are consistent with the growing prevalence of obesity and non-communicable chronic diseases observed in the city.
