Is Frailty Syndrome a Predictor of Morbimortality in Postoperative Cardiac Surgery? – A Retrospective Cohort Study

Abstract Background Frailty is a biological syndrome suggested as a better predictor of morbimortality than chronological age. Objective To assess associations between frailty and morbimortality outcomes in postoperative cardiac surgery. Methods A retrospective cohort study was conducted with cardiac surgery patients. Frailty and maximal inspiratory pressure (MIP) were assessed before surgery. Postoperative outcomes were: extracorporeal circulation time; use of vasopressor; mean arterial pressure (MAP); red blood cell (RBC) transfusion; cardiac arrhythmia and/or heart arrest; presence of intra-aortic balloon pump; antibiotic use; extubation time; length of stay in the intensive care unit (ICU); length of postoperative stay; mortality. One-way ANOVA was used to compare postoperative variables between frailty categories; Spearman was used to evaluate the correlations between frailty and postoperative variables. Age, sex, and MIP were introduced into multiple regression models to find the independent association between postoperative variables and frailty. A significance level of p < 0.05 was adopted. Results The medical records of 200 patients were analyzed (65.7±7.2 years; 68.5% men; 63.5% non-frail, 22.5% pre-frail, 14% frail). Frailty was not a predictor of postoperative outcomes. Age was an independent predictor for alterations in MAP (PR: 1.028, 95% CI: 1.003-1.053, p=0.025), need for RBC transfusion (PR: 1.034, 95% CI: 1.007-1.062, p=0.014), longer extubation time (PR: 1.052, 95% CI: 1.023-1.083, p<0.001), length of stay in the ICU (ß: 0.031, 95% CI: 0.010-0.053, p=0.005), length of postoperative stay (ß: 0.017, 95% CI: 0.003-0.031, p=0.015). Conclusions Frailty was not a predictor of morbimortality following cardiac surgery in middle-aged and older adults; however, age did predict morbidities in this setting.

Resumo Dioctophyme renale é nematoda parasita de rins, cavidade peritoneal e, mais raramente, de outros órgãos de canídeos e mustelídeos. Esta parasitose tem ocorrência elevada no Município de Três Barras - SC, tornando essa área interessante para o estudo de aspectos epidemiológicos da infecção por D. renale em cães. De 197 cães examinados, 14,2% apresentaram ovos do parasita na urina e 16,4% apresentaram anticorpos IgG anti-D. renale no soro pelo método de ELISA indireto e, entre estes, 15 (37,5%) não apresentavam ovos do parasita sendo eliminados pela urina. O parasitismo foi mais frequente nas fêmeas, e não houve constatação de interferência da idade no parasitismo. Fatores como potencial hídrico e presença de hospedeiros paratênicos na região estudada foram relatados pelos tutores dos cães e podem contribuir para a ocorrência do parasitismo.
Abstract Dioctophyme renale is a nematode that can be found parasitizing the kidney, peritoneal cavity and, rarely, other organs of canids and mustelids. This disease has high occurrence in the municipality of Três Barras, state of Santa Catarina, thus making this an interesting area to study the epidemiological aspects of infection by D. renale in dogs. Among 197 dogs, 14.2% showed the parasite eggs in urine and 16.4% showed IgG antibodies anti-D. renale in serum samples according to the indirect ELISA method; among seropositive dogs, 15 (37.5%) animals did not show any parasite eggs in their urine. Parasitism was more frequent in females, and there was no finding of interference from age on parasitism. Factors such as water potential and presence of paratenic hosts in the studied region were reported by the owners of dogs and may have contributed to the occurrence of parasitism.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Acetic acid-induced writhing, hot-plate, carrageenan-induced pleurisy, formalin-induced pain, croton oil-induced ear edema, vascular permeability tests and phospholipase A2 activity assay were used to study the analgesic and/or anti-inflammatory activity of the hydromethanolic fraction of ethanolic extract from Spiranthera odoratissima A. St.-Hil., Rutaceae, leaves (HMF) and its subfraction (sub-Fr10-28). HMF and sub-Fr10-28 reduced the leukocyte migration on the carrageenan-induced pleurisy test; sub-Fr10-28 reduced the pain reaction time in the second phase of formalin-induced pain, as well as the ear edema and vascular permeability. Both HMF and sub-Fr10-28 inhibited the phospholipase A2 activity. These results suggest that the analgesic effect of this plant could be, in part, due to an anti-inflammatory action produced by the inhibition of phospholipase A2 activity.

Morfologia e função cardíacas em pacientes renais crônicos, com ou sem diurese residual, em tratamento hemodialítico

Em pacientes com doença renal crônica (DRC) em hemodiálise (HD), a hipertrofia ventricular esquerda (HVE) está relacionada ao aumento do índice de resistência vascular periférica (IRVP) total e à sobrecarga de volume. A presença da diurese residual (DR) nesses pacientes possibilita maior controle volêmico. Avaliamos as modificações morfofuncionais do ventrículo esquerdo (VE) em pacientes com DRC em HD com e sem diurese residual. Trinta e um pacientes não diabéticos foram divididos em dois grupos: com diurese residual (DR+) (n = 17) e sem diurese residual (DR-) (n = 14). Em ambos os grupos, DR+ vs. DR-, ocorreram diferenças no índice cardíaco (3,9 ± 0,20 vs. 3,0 ± 0,21 L/min/m²; p = 0,0056), no índice sistólico (54 ± 2,9 vs. 45 ± 3,3 mL/b/m²; p = 0,04), no volume diastólico final (141 ± 6,7 vs. 112 ± 7,6 mL; p = 0,008), no diâmetro diastólico final (52 ± 0,79 vs. 48 ± 1,12 mm; p = 0,0072) e no IRVP total (1.121 ± 56 vs. 1.529 ± 111 dina.seg.cm-5; p = 0,001). O grupo DR+ apresentou menor espessamento relativo de parede (ERP) do que o DR- (0,38 ± 0,01 vs. 0,45 ± 0,01; p = 0,0008). A fração de ejeção (66,00 ± 1,24 vs. 66,0 ± 1,46%; p = 0,873) e o índice de massa ventricular esquerda (132 ± 6,0 vs. 130 ± 8,3 g/m; p = 0,798) foram similares em ambos os grupos. O volume de diurese residual correlacionou-se com o espessamento da parede ventricular (r = 0,42; p = 0,0186) e com o índice de resistência vascular periférica (r = -0,48; p = 0,0059). Em conclusão, a presença ou não da diurese residual, em pacientes com doença renal crônica e em hemodiálise, pode ser responsável por modificações na função cardíaca sistólica.
In patients with chronic renal failure on hemodialysis, left ventricular hypertrophy is related to the increase in total peripheral vascular resistance and volume overload. The presence of residual diuresis enables greater control of the volemia of these. We evaluated the morpho-functional changes of the left ventricle in patients with chronic kidney disease on hemodyalisis treatment with and without residual diuresis. A total of 31 non diabetic patients were studied and they were divided into two groups: with residual diuresis (RD+) (n = 17) and without residual diuresis (RD-) (n = 14). In both groups, RD+ vs. RD-, using data from a Doppler echocardiogram differences were found, respectively, in the cardiac index (3.9 ± 0.2 vs. 3.0 ± 0.2 L/min/m²; p = 0.0056), systolic index (54 ± 2.9 vs. 45 ± 3.3 mL/b/m²; p = 0.04), end diastolic volume (141 ± 6.7 vs. 112 ± 7.6 mL; p = 0.008), end diastolic diameter (52 ± 0.7 vs. 48 ± 1.1 mm; p = 0.0072) and total peripheral resistance index (1121 ± 56 vs. 1529 ± 111 dyne.sec.cm-5; p = 0.001). RD+ had lower relative wall thickness than RD- (0.38 ± 0.01 vs. 0.45 ± 0.01; p = 0.0008). The ejection fraction and the left ventricular mass index were similar in both groups. The urinary 24-hour volume correlated with the relative wall thickness (r = -0.42; p = 0.0186) and with peripheral resistance index (r = -0.48; p = 0.0059). In conclusion, there were distinct ventricular geometric patterns and different functional performances between RD+ and RD- groups. The presence of residual diuresis can be responsible by these modifications in systolic function.

Este estudo teve a finalidade de fornecer dados morfológicos de ovos de D. renale e do desenvolvimento de larvas de primeiro estádio em ovos mantidos em diferentes temperaturas. Os ovos foram obtidos por centrífugação da urina de cães parasitados e colocados em placas de Petri em estufa BOD, durante 90 dias. O experimento consistiu de três tratamentos (GI - 15 ºC, GII - 20 ºC e GIII - 26 ºC) com cinco repetições cada. Os ovos apresentaram tamanho médio de 67,23 x 42,78 µm, e o tempo médio de incubação foi inversamente proporcional à temperatura de incubação e as larvas apresentaram motilidade por aproximadamente uma semana após sua formação.
This study aims to provide information on morphological data of D. renale eggs, as well as on first-stage larvae development into eggs kept at different temperatures. Eggs were obtained by centrifugation of infected dog urine, placed in Petri dishes, and stored in BOD chamber for a 90-day period. Each treatment (GI - 15 ºC, GII - 20 ºC, and GIII - 26 ºC) was repeated five times. Eggs showed average measures of 67.23 x 42.78 µm, and the mean incubation time was inversely proportional to the incubation temperature. Larvae motility was observed one week after being observed in eggs.
