ABSTRACT Purpose To characterize hearing thresholds at frequencies of 500, 1000, 2000 and 4000 Hz in children undergoing reimplantation with a follow-up of at least 10 years. Methods Retrospective review of medical records of children who underwent reimplantation surgery for at least 10 years. The auditory thresholds obtained in free-field pure tone audiometry with the cochlear implant were evaluated at frequencies of 500, 1000, 2000 and 4000 Hz at four different times: 1 (before failure), 2 (activation), 3 (five years after reimplantation) and 4 (ten years after reimplantation, regardless of the time of use of the 2nd CI) in patients with a follow-up of at least 10 years. Results Evaluating patients who underwent reimplantation, it was observed that the thresholds of 500, 1000, 2000, 4000 Hz were similar in the long term to those obtained in patients who were implanted only once, thus not presenting damage in the detection of sounds. Conclusion Reimplantation had no long-term effect on the hearing thresholds obtained in children who underwent this surgery due to internal component failure.

RESUMO Objetivo Caracterizar os limiares auditivos nas frequências de 500, 1000, 2000 e 4000 Hz em crianças submetidas ao reimplante com follow-up de no mínimo 10 anos. Método Revisão retrospectiva de prontuários de crianças submetidas a cirurgia de reimplante há pelo menos 10 anos. Foram avaliados os limiares auditivos obtidos na audiometria tonal liminar em campo livre com o implante coclear nas frequências de 500, 1000, 2000 e 4000 Hz em quatro momentos distintos: 1 (antes da falha), 2 (ativação), 3 (cinco anos após o reimplante) e 4 (dez anos após o reimplante, independentemente do tempo de uso do uso do 2o IC) em pacientes com follow-up de no mínimo 10 anos. Resultados Avaliando-se pacientes submetidos ao reimplante, observou-se que os limiares de 500, 1000, 2000, 4000 Hz apresentaram-se a longo prazo semelhante àqueles obtidos nos pacientes implantados somente uma única vez, não apresentando prejuízo assim na detecção dos sons. Conclusão O reimplante não teve efeito de longo prazo sobre os limiares auditivos obtidos em crianças que se submeteram a esta cirurgia por falha do componente interno.

Abstract Speech accuracy is an indicator for evaluating the language of children with cochlear implant (CI). Verifying the teaching that increases this accuracy and how efficiency measures can be adopted are relevant questions. The current study evaluated the acquisition of speech accuracy, using pictures of scenes as controls. Sentences were employed after equivalence-based instruction. Eight children with CI and who were readers participated in the study. The sentences had the following structure: [subject]-[verb]-[object]. The teaching included selecting pictures and constructing printed sentences, conditionally to dictated sentences. Naming and reading was monitored by multiple probes. Two judges transcribed the participants’ vocalizations and the accuracy was measured by the correspondence with target-phonemic units of sentences. All the participants increased the speech accuracy in response to pictures of scenes after the teaching. Acquisition standards were observed and the accuracy was firstly obtained in the terminal portion, followed by the initial portion and, lastly in the medial portion. These results suggest how and under conditions establish the speech accuracy of the sentences in children with CI.

Résumé La précision de la parole est un indicateur permettant d’évaluer le langage des enfants porteurs d’implants cochléaires (HF). Vérifiez que l’enseignement qui augmente cette précision et comment les mesures d’efficacité peuvent être adoptées sont des questions pertinentes. Le présent travail a évalué l’acquisition d’une précision de la parole contrôlée par des figures de la scène, à l’aide de phrases, après un enseignement basé sur l’équivalence. Huit enfants avec IC et lecteurs ont participé. Les phrases avaient [sujet] - [verbe] - [objet]. L’enseignement consistait à sélectionner des figures et à construire des phrases imprimées, conditionnellement aux phrases dictées. Le rendez-vous et la lecture ont été surveillés par plusieurs sondes. Deux juges ont transcrit les vocalisations des participants et la précision a été mesurée par correspondance avec les phonèmes visés par les phrases. Tous ont augmenté la précision de la parole avant les figures de scènes après l’enseignement et des schémas ont été observés dans lesquels la précision était obtenue d’abord dans la partie terminale, suivie de la première et enfin de la médiale. Ces résultats suggèrent comment et quelles conditions établissent le discours précis des phrases chez les enfants atteints d’insuffisance cardiaque.

Resumen La precisión del habla es un indicador para evaluar el lenguaje de niños con implante coclear (IC). Verificar la enseñanza que aumenta esa precisión, y cómo las medidas de eficiencia pueden ser adoptadas, son cuestiones relevantes. El presente trabajo evaluó la adquisición de la precisión del habla controlada por figuras de escenas, usando oraciones, después de la enseñanza basada en equivalencia. Participaron ocho niños con IC, lectores. Las oraciones tenían [sujeto]-[verbo]-[objeto]. La enseñanza consistió en seleccionar figuras y construir oraciones impresas, condicionalmente a las oraciones dictadas. El nombramiento y la lectura fueron supervisadas por múltiples sondas. Dos jueces transcribieron las vocalizaciones de los participantes y la precisión fue medida por la correspondencia con fonemas objetivo de las oraciones. Todos aumentaron la precisión del habla ante las figuras de escenas después de la enseñanza. Patrones de la adquisición fueron observaron y la precisión se obtuvo primero en la porción terminal, seguida de la inicial y por último en la medial. Estos resultados sugieren cómo y qué condiciones establecen la precisión del habla de las oraciones en los niños con IC.

Resumo A precisão da fala é um indicador para avaliar linguagem de crianças com implante coclear (IC). Verificar o ensino que aumenta essa precisão, e como medidas de eficiência podem ser adotadas são questões relevantes. O presente trabalho avaliou a aquisição da precisão da fala controlada por figuras de cenas, usando sentenças, após o ensino baseado em equivalência. Participaram oito crianças com IC e leitoras. As sentenças tinham [sujeito]-[verbo]-[objeto]. O ensino consistiu em selecionar figuras e construir sentenças impressas, condicionalmente às sentenças ditadas. A nomeação e a leitura foram monitoradas por múltiplas sondas. Dois juízes transcreveram as vocalizações dos participantes e a precisão foi medida pela correspondência com fonemas-alvo das sentenças. Todos aumentaram a precisão da fala diante das figuras após o ensino. Foram observados padrões de aquisição e a precisão era obtida primeiramente na porção terminal, seguida da inicial e por último na medial. Esses resultados sugerem como e quais condições estabelecem a fala precisa de sentenças em crianças com IC.

Resumo Considerando os efeitos do ensino de leitura sobre a precisão da fala de crianças com deficiência auditiva e implante coclear apresentados na literatura, o objetivo deste estudo foi verificar os efeitos do Módulo 2 do ALEPP® sobre a precisão da fala em leitura de palavras com dificuldades ortográficas a uma criança usuária de implante coclear. A participante tinha nove anos, com deficiência auditiva profunda, bilateral e sensorioneural. O programa expôs a contingências de ensino de seleção de palavras e sílabas impressas condicionalmente a palavras e sílabas ditadas com seis conjuntos de estímulos (ç, lh, ch, vLc, vNc e vRc); cada conjunto era composto por 16 palavras, totalizando 96 palavras de ensino; outras 64 palavras funcionaram como controle. Testes das 160 palavras intercalaram o ensino de acordo com o delineamento de múltiplas sondas. A participante aprendeu a ler as 96 palavras-alvo de ensino, com estabelecimento de controle pelas unidades mínimas alvo, manutenção de desempenho e generalização na leitura de palavras controle. Erros na precisão da fala diminuíram somente após o ensino. Os resultados replicam estudos anteriores com palavras simples e futuras investigações podem isolar os tipos de treino do ALEPP® e explorar seus efeitos sobre a precisão da fala e sobre o controle pelas unidades mínimas.

Abstract Considering the effects of reading on the accuracy of the speech of deaf and hard and hearing children and cochlear implants presented in the scientific literature, the main aim of this study was to evaluate the effects of Module 2 of the ALEPP software regarding teaching words with orthographic difficulties to a cochlear implanted child. The participant was a girl, had nine years of age, with profound bilateral and sensorineural hearing loss. The program aims to teach contingencies of the selection of printed words and syllables conditioned to dictated words and syllables of six different stimuli sets; each set contained 16 words, with a total of 96 teaching words and 64 control words. Probes of 160 words were interspersed with the teaching steps according to the multiple probe design. The participant learned to read the 96 taught words, controlled by the words’ minimal units. She presented maintenance of performance and reading generalization of the control words. The number of speech accuracy errors only decreased after the teaching intervention. The results replicated those of previous studies with orthographically easier words and future studies may explore the different types of training provided by the ALEPP and its effects on the accuracy of the speech of cochlear implanted children.

Resumen Considerando los efectos de la enseñanza de lectura sobre la precisión del habla en niños con deficiencia auditiva e implante coclear presentados en la literatura, el objetivo de este estudio fue verificar los efectos del Módulo 2 de ALEPP® sobre la enseñanza de lectura de palabras con dificultades ortográficas a una niña usuaria de implante coclear. La participante tenía nueve años, deficiencia auditiva profunda, bilateral y neurossensorial. El programa exposu a contingencias de enseñanza de selecciòn de palabras y silabas impresas condicionalmente a las palabras y silabas dictadas con seis conjuntos de estímulos; cada conjunto estaba compuesto por 16 palabras, totalizando 96 palabras de enseñanza; otras 64 palabras funcionaron como control. Las pruebas de las 160 palabras intercalaron la enseñanza de acuerdo con el delineamiento de múltiples sondas. La participante aprendió a leer las 96 palabras objetivo de enseñanza, con establecimiento de control por las unidades mínimas, mantenimiento de desempeño y generalización en la lectura de palabras control. Los errores en la precisión del habla disminuyeron sólo después de la inserción de la enseñanza. Los resultados replican estudios anteriores con palabras sin dificultades ortográficas y futuras investigaciones pueden explorar los efectos de los tipos de entrenamientos ofrecidos por el ALEPP® y sus efectos sobre la precisión del habla y el control por las unidades mínimas.

RESUMO Objetivo Verificar a influência da idade de implantação no desenvolvimento das habilidades de reconhecimento auditivo em conjunto fechado e compreensão auditiva em crianças usuárias de implante coclear unilateral, comparando grupos distintos e determinar os marcadores clínicos de desenvolvimento destas habilidades. Método Participaram do estudo 180 crianças operadas e ativadas até os 36 meses de idade e que utilizaram o IC durante, no mínimo, 60 meses. Foram analisadas as habilidades de reconhecimento auditivo em conjunto fechado e compreensão auditiva por meio das Provas 5 e 6 do GASP. Para investigar a influência da idade na implantação, as crianças foram divididas em três grupos: implantadas antes dos 18 meses (G1), entre 19 e 24 meses (G2) e entre 25 e 36 meses de idade (G3). Resultados Não houve diferença estatisticamente significante quando comparados os três grupos. As crianças desenvolveram progressivamente as habilidades auditivas, apresentando a habilidade de reconhecimento auditivo em conjunto fechado por volta dos 41±4 meses de uso do IC e a de compreensão auditiva por volta dos 53±4 meses. Conclusão Não houve correlação entre o desempenho auditivo e a idade de implantação para as crianças implantadas antes dos 36 meses de idade. Para as habilidades de reconhecimento e compreensão auditivas, o marcador clínico foi aos 41±4 e aos 53±4 meses de idade auditiva, respectivamente. Para tanto, espera-se que, por volta dos 60 meses de uso do IC, as crianças implantadas durante o período sensível possam compreender a fala sem o auxílio da leitura orofacial, alcançando as habilidades auditivas mais complexas.

ABSTRACT Purpose To verify the influence of the age of implantation in the development of closed-set auditory recognition and auditory comprehension abilities in children using unilateral cochlear implants (CI), comparing distinct groups and determining clinical markers. Methods Participants were 180 children operated and activated until 36 months of age and who used a CI for at least 60 months. Abilities of auditory recognition in closed-set and auditory comprehension were analyzed through the GASP Tests 5 and 6. The influence of age of implantation was investigated with three groups of children: implanted before 18 months (G1), between 19 and 24 months (G2) and between 25 and 36 months of age (G3). Results There was no statistically significant difference when comparing the three groups. Children progressively developed auditory abilities, presenting auditory recognition ability together at approximately 41±4 months of CI use and auditory comprehension at 53±4 months. Conclusion There was no correlation between hearing performance and age of implantation for children implanted before 36 months of age. For the abilities of auditory recognition and comprehension, the clinical marker was 41±4 and 53±4 months of auditory age, respectively. Therefore, it is expected that, around 60 months of CI use, children implanted during the sensitive period can understand speech without the aid of orofacial reading, reaching the most complex hearing abilities.

RESUMO Objetivo Verificar o efeito do ensino por múltiplos exemplares na aquisição e integração dos comportamentos de ouvinte e falante, com substantivos e combinações substantivo-adjetivo, em crianças com Desordem do Espectro da Neuropatia Auditiva (DENA) e implante coclear (IC). Método Participaram duas crianças com DENA que usavam IC. Foram adotados estímulos ditados e figuras que correspondiam a palavras (substantivo) e unidades sintáticas substantivo+adjetivo. O estudo foi organizado em passos de ensino que foram intercalados por avaliações dos comportamentos de ouvinte e falante, com todos os estímulos. O ensino por múltiplos exemplares apresentou tarefas de imitação oral (ecoico), reconhecimento auditivo (ouvinte) e nomeação de figuras (tato) de maneira rotativa; os substantivos foram ensinados primeiro e, em seguida, as combinações substantivo-adjetivo. Resultados No pré-teste, os participantes mostraram variabilidade e discrepância nas porcentagens de acertos de ouvinte e de falante. Todos alcançaram primeiro 100% de acertos nas tarefas de ouvinte e os desempenhos de falante ficaram próximos aos de ouvinte após o ensino. Todos estenderam a aprendizagem dos substantivos para as unidades sintáticas substantivo-adjetivo. Conclusão Crianças com DENA e IC podem aprender e integrar comportamentos de ouvinte e de falante por meio do ensino por múltiplos exemplares, de palavras a unidades sintáticas.

ABSTRACT Purpose To verify the effect of the multiple exemplar instruction at the acquisition and integration of listening and speaking behaviors, with substantives and substantive-adjective combinations, in children with Auditory Neuropathy Spectrum Disorder (ANSD) and cochlear implant (CI). Methods Participants were two children with ANSD that were users of CI. We adopted dictated stimulus and pictures that corresponded to words (substantive) and substantive-adjective syntactic units. The study was arranged in teaching steps that were intercalated with listening and speaking behaviors probes, with all stimuli. The multiple exemplar instruction presented oral imitation (echoic), auditory recognition (listening) and pictures naming (touch) tasks, on a rotating way; the substantives were taught first and, after that, the substantive-adjective combinations. Results In the pre-test, the participants showed variability and discrepancy in the correct responses percentages of listening and speaking. All achieved firstly 100% correct responses in the listening task and the speaking performances were close to listening after the teaching. All extended substantive learning to substantive-adjective syntactic units. Conclusion Children with ANSD and CI can learn and integrate listening and speaking behaviors by multiple exemplar instruction, from words to syntactic units.

RESUMO: Os objetivos desta pesquisa foram (a) verificar a quantidade de produções científicas que descrevessem procedimentos de ensino de repertório expressivo, de acordo com as categorias verbais de Skinner (1957), a crianças com implante coclear; (b) caracterizar seus métodos, considerando participantes, estímulos, materiais e delineamentos utilizados; (c) identificar variável independente manipulada e variável dependente mensurada. A metodologia seguiu o modelo PRISMA para revisões de literatura e meta-análises. A busca foi feita na Web of Science com os descritores e marcadores boleados: "Cochlear Implant", [and] "Children" [and] "Language", [and] "Teaching". Foram encontrados 39 artigos e, após aplicação dos critérios de exclusão, obteve-se 12 artigos para análise. Dentre estes, oito descreveram metodologias de ensino de repertório expressivo. Os dados apontaram uma concentração de publicações internacionais entre os anos de 2012 e 2015. O operante intraverbal tem sido o maior alvo das intervenções encontradas, seguido por tato e textual. Mais de 50% dos estudos fizeram uso de delineamentos experimentais de sujeito único, corroborando com as indicações sobre a importância desse tipo de delineamento para a pesquisa em saúde e educação. Publicações nacionais não foram encontradas, embora haja uma forte linha de pesquisa no país. Para contornar tal situação, sugere-se a inserção intencional das pesquisas que se tem ciência ao conjunto de pesquisas encontradas por meio de revisões sistemáticas. Sintetizar as publicações que apresentam metodologias de ensino a essa população pode favorecer os profissionais de Educação Especial na elaboração de suas intervenções e currículos.

ABSTRACT: The aims of this research were (a) to verify the quantity of scientific productions that described procedures of teaching of expressive repertoire, according to Skinner's verbal categories (1957), to children with cochlear implants; (b) to characterize their methods, considering participants, stimuli, materials and experimental design used; (c) to identify manipulated independent variable and dependent measured variable. The methodology followed the PRISMA model for literature reviews and meta-analyzes; the search was done in the Web of Science with the descriptors and bullet points: "Cochlear Implant", [and] "Children" [and] "Language", [and] "Teaching". We found 39 papers and, after applying the exclusion criteria, we obtained 12 papers for analysis. Among them, eight papers described methodologies of expressive repertoire teaching. The data pointed out that a concentration of international publications between the years 2012 and 2015. The intraverbal operant has been the major target of the interventions found, followed by tact and textual ones. More than 50% of the studies made use of single-subject experimental designs, corroborating with the indications about the importance of this type of design for research in health and education. National publications were not found, although there is a strong line of research in the country. In order to avoid this situation, it is necessary to insert intentionally the research that is known to the set of researches found through systematic reviews. Synthesizing publications that present teaching methodologies to this population may favor Special Education professionals in the elaboration of their interventions and curricula.

Abstract Children who use cochlear implants (CI) and who are readers usually produce more accurate speech in response to text than to pictures. Equivalence-based instruction (EBI) can be a route to establish functional interdependence between these verbal operants. The present study investigated whether children with CI who read would improve speech accuracy when tacting pictures of scenes after EBI that included dictated sentences, pictures of scenes, and printed sentences. This study evaluated whether teaching verbal relations to diagonal sentences from a matrix with subject-verb-object combinations promoted recombinative generalization to untrained sentences. Participants were three children with CI with a more accurate speech when reading print than when tacting pictures of scenes. They were taught to select pictures of scenes in response to dictated sentences (AB) by matching-to-sample (MTS) and to construct printed sentences in response to dictated sentences (AE) by constructed-response-matching-to-sample (CRMTS). Speech production in response to print (CD) and in response to pictures of scenes (BD) were probed for both trained and untrained sentences, using a multiple baseline design across participants. All participants learned the trained relations, showed emergence of derived relations, and improved speech accuracy when tacting pictures of scenes. They were able to recombine sentence components and tact novel pictures using untrained sentences from the matrix. These results indicate that speech accuracy and generative sentence production can be improved in children with CI from interventions that incorporate EBI and matrix training. RESUMO Objetivo Avaliar a qualidade de vida (QV) de adultos usuários de implante coclear (IC), comparando-a com a QV de adultos com audição normal, além de estudar a influência, na QV dos adultos usuários de IC, destas variáveis: nível socioeconômico, escolaridade, idade na avaliação, tempo de privação sensorial auditiva, tempo de uso do dispositivo e desempenho nos testes de percepção auditiva da fala. Desenho A QV foi avaliada segundo o questionário genérico de avaliação World Health Organization Quality of Life (WHOQOL-bref). Amostra do estudo Setenta adultos usuários de IC formaram o grupo experimental (GE) e 50 adultos com audição normal fizeram parte do grupo controle (GC). Resultados O GE apresentou escores muito próximos à pontuação máxima que representa QV satisfatória para todos os domínios do questionário WHOQOL-bref e houve resultados semelhantes entre os GE e GC. Na avaliação, as variáveis idade, tempo de privação sensorial auditiva, tempo de uso do IC e desempenho em percepção auditiva da fala não influenciaram os resultados de QV de adultos usuários de IC. Conclusão Avaliar a QV deve ser uma preocupação das equipes interdisciplinares em IC para uma intervenção com um cuidado humanizado.

ABSTRACT Objective To evaluate the quality of life (QOL) of adult cochlear implant users (CI) and compare it with the QOL of adults with normal hearing; and study the influence of the variables socioeconomic status, education, age at assessment, auditory sensory deprivation time, device usage time and performance in auditory speech perception tests in the QOL of adult cochlear implant users. Design The QOL was assessed using the World Health Organization Quality of Life (WHOQOL-BREF) generic assessment questionnaire. Study sample Seventy adult CI users formed the experimental group (EG) and 50 adults with normal hearing formed the control group (CG). Results The EG scores were close to the maximum score in satisfactory quality of life for all domains of the WHOQOL-BREF and there were similar results between the EG and CG. The variables age at assessment, duration of auditory sensory deprivation, duration of CI use and performance in auditory speech perception did not influence the results of the QOL of adult cochlear implant users. Conclusion Evaluating the QOL should be a concern of interdisciplinary teams in CI for an intervention with humanized care.

Resumo: A reabilitação auditiva por meio do implante coclear tem constituído um campo de atuação e de pesquisa interdisciplinar, cujos interesses têm incidido na investigação das variáveis e processos relacionados às habilidades de linguagem dessa população. O presente estudo teve como objetivo apresentar uma revisão sistemática das pesquisas que investigaram as relações entre o uso do implante coclear e o desenvolvimento da linguagem. Para isso, foi realizada uma busca nas bases científicas Web of Science(r), Scielo(r) e LILACS(r), de 2003 a 2013, de estudos sob escopo da Audiologia (e de áreas correlatas), Educação, Reabilitação e Ciências do Comportamento; aplicando os unitermos "cochlear implant", "auditory recognition", "recognition", "speech", "speech production" e "language". Após um tratamento inicial dos resultados, foram selecionados para análise 86 artigos que foram classificados de acordo com os seguintes critérios: ano de publicação, periódico, área de pesquisa, tipo de estudo, habilidade investigada e a temática. Os resultados indicaram crescente avanço nesses estudos e uma ênfase em avaliar e/ou identificar possíveis fatores que interferem nos processos de linguagem de implantados cocleares, sendo essas pesquisas conduzidas majoritariamente pela Audiologia e áreas correlatas. Os estudos comparativos e explicativos foram os mais frequentes e têm destacado as habilidades auditivas, especificamente os fatores que afetam a percepção auditiva. Enquanto perspectivas futuras, poderão ser vislumbrados maior envolvimento científico de áreas interdisciplinares à Audiologia, ampliação de pesquisas sobre habilidades expressivas (como a produção oral) e fomento à estudos que investiguem intervenções (práticas baseadas em evidências) em linguagem para esse público.

Abstract: The auditory rehabilitation by means of cochlear implant has constituted an field of interdisciplinary activity and research, whose interests have focused on the investigation of variables and processes related to the language skills of this population. This study aimed to present a systematic review of studies that investigated the relations between the use of cochlear implant and language development. For this, was conducted a search in scientific bases Web of Science(r), Scielo(r) and LILACS(r), of 2003-2013, the studies in scope of Audiology (and related areas), Education, Rehabilitation and Behavioral Sciences; applying the terms "cochlear implant", "auditory recognition" , "recognition", "speech", "speech production" and "language". After an initial processing of the results, were selected for analysis 86 articles that were classified according to the following criteria: year of publication, journal, research area, study type, skill and investigated topic. The results indicate growing progress in these studies and an emphasis on evaluating and/or identify possible factors that interfere language processes' cochlear implant users, and that these research conducted majority by the Audiology and related areas. The comparative and explanatory studies were the most frequent and have emphasized the auditory skills, specifically the factors that affect the auditory perception. While future perspectives, can be glimpsed largest scientific involvement of interdisciplinary areas of Audiology, expansion of research on expressive skills (such as oral production) and promotion of studies investigating interventions (evidence-based practices) in language to the public.

OBJETIVO: Analisar o desempenho motor grosso, motor fino-adaptativo, linguagem, pessoal-social e comportamentos comunicativos de crianças com paralisia cerebral (PC) usuárias de implante coclear (IC) e crianças com PC sem deficiência auditiva (DA), e compará-las com crianças com desenvolvimento típico de linguagem. MÉTODOS: Estudo prospectivo transversal de 12 crianças, idade média de 63 meses, distribuídas em dois grupos experimentais: G1 - quatro crianças com PC e IC; G2 - quatro crianças com PC sem DA; e G3 - quatro crianças do grupo controle. Seis crianças foram classificadas no nível II e duas no nível IV no Gross Motor Function Classification System (GMFCS). Foram utilizados o Teste de Screening do Desenvolvimento Denver-II (TSDD-II) e a Observação do Comportamento Comunicativo (OCC). RESULTADOS: G3 apresentou desempenho superior ao de G1 e de G2 em todas as avaliações. G2 foi superior a G1 em linguagem, comunicação, pessoal-social e motor fino-adaptativo, exceto na área motora grossa. Os aspectos de linguagem e comportamentos comunicativos foram inferiores nos dois grupos experimentais, especialmente no G1. As habilidades avaliadas na área pessoal-social não apresentaram diferenças entre os grupos. CONCLUSÃO: A limitação motora de G1 e G2 e a DA de G1 influenciaram o desenvolvimento nas áreas avaliadas, porém esses fatores não restringiram o desenvolvimento pessoal-social. A habilidade pessoal-social também esteve rebaixada para os grupos com PC, porém a diferença dessa função com G3 não foi significante. O IC proporcionou um canal de recepção da linguagem oral e interação social, o que o determina como uma ferramenta para a melhoria da qualidade de vida nessas crianças.

PURPOSE: To analyze gross motor, fine motor-adaptive, language, social function performance, and communicative behaviors among cochlear-implanted children with spastic cerebral palsy (CP) and children with CP without hearing loss (HL) and to compare them with children with normal development. METHODS: Prospective cross-sectional study involving 12 children with mean age of 63 months, distributed into two experimental groups: G1 - 4 children with CP and cochlear implant (CI) users and G2 - 4 children with CP without HL. A third group (G3) was the control group with four typically developing children. In the experimental groups, six children were classified in level II and two in level IV, using the Gross Motor Function Classification System. We used the Denver Developmental Screening Test II and the Communicative Behavior Observation (CBO). RESULTS: G3 showed better performance than G1 and G2 in all evaluations. G2 showed better results than G1 in language, communication, personal-social, and fine motor-adaptive areas, except in the gross motor area. Aspects of language and communicative behaviors were lower in both experimental groups, especially in G1. Skills related to personal-social area showed no differences among the groups. CONCLUSION: Motor impairment of G1 and G2 and HL in G1 affected the development in the assessed areas, but these factors did not restrict personal-social development. Children with CP did not achieve high development in social function; however, the difference with relation to G3 was not statically significant. The CI provided a channel for oral language reception and social interaction, which has a key role in determining the quality of life.

OBJETIVO: Avaliar a percepção de fala ao telefone em indivíduos que receberam o implante coclear multicanal no período de 1993 a 2003. MÉTODOS: Vinte e sete usuários de IC foram divididos em grupos pré e pós-lingual, sendo que a percepção de fala foi avaliada em dois momentos: primeiramente por uma lista de sentenças aplicada a viva-voz com o mesmo mapeamento utilizado na avaliação ao telefone e, em um segundo, utilizando-se o telefone fixo, telefone fixo com adaptador para IC e telefone celular. RESULTADOS: No grupo de deficiência auditiva pré-lingual, 75% dos indivíduos foram capazes de manter o diálogo com o interlocutor e 19% o fizeram com dificuldade. Já no pós-lingual, 89% foram capazes de manter o diálogo com o interlocutor e 11% o fizeram com dificuldade. Tanto os indivíduos dos grupos pré quanto os do pós-lingual utilizam o telefone como meio de comunicação, e a maioria apresenta desempenho satisfatório sem a necessidade de auxílio ou acessórios do IC. CONCLUSÃO: Um dos benefícios do IC é introduzir na vida do deficiente auditivo o uso do telefone independente de sua tecnologia e acessórios, bem como o telefone com adaptador e o celular. Neste estudo, a maioria dos indivíduos usuários de IC apresentou desempenho satisfatório de compreensão de sentenças e perguntas ao telefone, com melhor desempenho, apesar de discreto, do grupo pós-lingual. Desta forma, verifica-se a importância da indicação criteriosa do IC e do programa de reabilitação auditiva, que minimizam os prejuízos da surdez na comunicação, principalmente via telefone, independente do momento no qual a deficiência auditiva foi adquirida. Assim, melhoram-se as condições para os usuários de IC manterem suas atividades diárias acompanhando o dinamismo da vida contemporânea.

OBJECTIVE: To evaluate telephone speech perception in individuals who received cochlear implant in the period 1993-2003. METHODS: Twenty seven CI users were divided into pre and post-lingual groups, being the speech perception assessed in two stages: first by a list of sentences imposed on speakerphone with the same mapping used to evaluate the phone and, in a second stage, using the landline, landline phone adapter with CI and cell phone. RESULTS: In the group of pre-lingual hearing loss, 75% of subjects were able to maintain a dialogue with the interlocutor and 19% did so with difficulty. In the post-lingual group, 89% were able to maintain the dialogue with the interlocutor and 11% did so with difficulty. Both groups of subjects pre and post-lingual use the phone as a media, and most have satisfactory performance without the need for aid or CI accessories. CONCLUSION: One of the benefits of the CI is to introduce the life of the hearing impaired phone use regardless of their technology and accessories, as well as with the phone adapter and cell phone. In this study, most individuals CI users showed satisfactory performance in the comprehension of sentence and questions by the telephone, with better performance, although discrete, for the group post-lingual. Thus, there is the importance of careful indication of the IC and auditory rehabilitation program that minimize the impact of deafness in the communication, especially via telephone, regardless of the time when the hearing was acquired. So, this fact improves the conditions for CI users to maintain their daily activities, following the dynamism of contemporary life.

O processamento das pistas acústicas dos sons da fala depende da adequada percepção da freqüência e da duração dos estímulos enquanto seqüência de eventos. OBJETIVO: Avaliar as habilidades de ordenação temporal em usuários de IC multicanal. MÉTODO: 14 indivíduos com audição normal, formadores do grupo controle equiparados em idade e gênero com 14 usuários de IC, que formaram o grupo experimental, foram avaliados e comparados quanto ao Teste de Padrões de Freqüência (TPF) e Teste de Padrões de Duração (TPD). RESULTADOS: Os usuários de IC apresentaram bom desempenho nas tarefas de ordenação temporal, com resultados médios de 48,7% no TPF e 59,6% no TPD. Para o grupo controle, o desempenho médio no TPF foi de 63,4% e no TPD de 64,6%. Não foi obtida diferença significativa estatisticamente entre os resultados dos grupos controle e experimental. CONCLUSÃO: O IC proporcionou desempenho favorável nas tarefas que exigem habilidades de ordenação temporal para os indivíduos avaliados neste estudo.

Processing acoustic clues from the sounds of speech depends on the proper perception of the frequency and duration of stimuli as a sequence of events. AIM: To assess the capacity for temporal organization in users of multichannel CI. METHOD: 14 normal hearing individuals formed the control group, matching in age and gender other 14 users of multichannel CI, who made up the study group, and they were assessed and compared as to the Frequency Patterns Test (FPT) and Duration Patterns Test (DPT). RESULTS: CI users had good performance in temporal organization tasks, with mean results of 48.7% in the FPT and 59.6% in the DPT. For the control group, mean performance at the FPT was of 63.4% and in the DPT of 64.6%. We did not see statistically significant difference between the results from the control and study groups. CONCLUSION: the CI provided favorable performance in the tasks that required temporal organization skill for individuals evaluated in this study.

TEMA: implante coclear (IC) em crianças. Objetivo: traçar um perfil de linguagem oral receptiva e expressiva de crianças usuárias de implante coclear há cinco anos a cinco anos e onze meses; verificar a influência do tempo de privação sensorial na linguagem oral receptiva e expressiva dessas crianças. MÉTODO: 19 crianças deficientes auditivas usuárias de IC com deficiência auditiva pré-lingual, com tempo de uso do IC variando de 5a a 5a11m e média do tempo de privação sensorial de 3a (desvio padrão um ano) foram avaliadas por meio da Reynell Developmental Language Scales (RDLS) (Reynell e Gruber, 1990) que é composta pela Escala de Compreensão (C), Escala de Expressão (E) e suas Sub-Escalas Estrutura (Ee), Vocabulário (Ev) e Conteúdo (Ec). RESULTADOS: a mediana e os valores do quartil 75 e quartil 25 encontrados foram: 44, 57 e 54 para C; 48, 60 e 55 para E; 20, 21 e 20 para Ee; 15, 19 e 17 para Ev; 15, 22 e 18 para Ec; 96, 116 e 108 para a pontuação total. Houve correlação estatística entre o tempo de privação sensorial e a pontuação de C (p = - 0,62; R = 0,0044) e Ec (p = - 0,48; R = 0,0348) tornando o tempo de privação influente na pontuação total (p = - 0,53; R = 0,0174). CONCLUSÃO: o perfil de linguagem das crianças usuárias de implante coclear há cinco anos é desviante e semelhante ao perfil das crianças ouvintes de cinco anos para a Expressão e ao das crianças ouvintes de quatro anos para a Compreensão; a influência do tempo de privação sensorial foi estatisticamente significante na pontuação da C - linguagem receptiva - e na pontuação de uma seção (Ec) da E - linguagem expressiva, sendo significante na pontuação total da RDLS.

BACKGROND: cochlear implant (CI) in children. AIM: 1) to delineate a profile of receptive and expressive verbal language of children who have been using cochlear implant for five years and five years and eleven months; 2) to verify the influence of time of auditory sensorial privation in the receptive and expressive verbal language of these children. METHOD: 19 children users of CI with auditory deficiency acquired before language development, who have been using CI for 5y - 5y11m and who have an average time of sensorial privation of 3y (standard deviation of 1 year). These children were assessed using the Reynell Developmental Scales (RDLS) (Reynell e Gruber, 1990) which is composed of: Comprehension Scale (C), Expression Scale (E) and its Structure Sub-Scales (Es), Vocabulary (Ev) and Content (Ec). RESULTS: the median values and the values found for quartile 75 and quartile 25 were:.44, 57 and 54 for C; 48, 60 and 55 for E; 20, 21 and 20 for Es; 15, 19 and 17 for Ev; 15, 22 and 18 for Ec; 96, 116 and 108 for the total score. A statistical correlation between the time of sensorial privation and the score obtained for C (p= - 0.62; R= 0.0044) and Ec (p= - 0.48; R = 0.0348) was observed. Therefore the time of sensorial privation had an influence on the overall score (p= - 0.53; R = 0.0174). CONCLUSION: the language profile of children who use CI for five years is devious and similar to that of five year old hearing children regarding Expression and to that of four year old hearing regarding Comprehension; time of sensorial privation was statistically significant for the score obtained in C - receptive language - and for the score obtained in the E section (Ec) - expressive language, as well as in the overall score of RDLS.

OBJETIVOS: Avaliar os efeitos das diferentes relações sinal/ruído no reconhecimento da fala com o implante coclear (IC); comparar o reconhecimento da fala no ruído com diferentes tipos de implantes cocleares (ICs) multicanais e avaliar o grau de dificuldade dos usuários de IC em situações com ruído competitivo no dia-a-dia. FORAMA DE ESTUDO: coorte transversal. MATERIAL E MÉTODO: Quarenta adultos com deficiência auditiva pós-lingual, com os IC Nucleus 22, Nucleus 24, Combi 40, Combi 40+ e Clarion. Foi avaliado o reconhecimento das sentenças CPA no silêncio e nas relações S/R de +15 dB, +10 dB, +5 dB e aplicado o questionário Social Hearing Handicap Index (SHHI) para a auto-avaliação do desempenho com o IC. RESULTADOS E CONCLUSÕES: Os usuários de todos os tipos de IC apresentaram redução significativa dos índices de reconhecimento de sentenças CPA em função da diminuição da relação S/R, a curva das medianas de reconhecimento das sentenças CPA alcançou 50% na relação S/R de +10 dB. Não houve diferença estatisticamente significante nos índices de reconhecimento das sentenças CPA e escores de dificuldade do SHHI, obtidos com os diferentes tipos de ICs. As dificuldades dos usuários de IC foram raras nas situações de silêncio e ocasionais nas situações com ruído competitivo no questionário SHHI.

AIM: to evaluate the effects of different signal-to-noise ratios on speech recognition obtained by the use of cochlear implant (CI); to compare the speech recognition in noise with different types of multichannel cochlear implants (CIs) and to evaluate the degree of difficulty for speech understanding in noise in daily life situations. STUDY DESIGN: cohort transversal. MATERIAL AND METHOD: Forty adults with post-lingual hearing loss implanted with Nucleus 22, Nucleus 24, Combi 40, Combi 40+ and Clarion. We evaluated the recognition for CPA sentences in quiet and in S/N +15, +10 and +5 dB. We also applied the Social Hearing Handicap Index (SHHI) questionnaire for self-assessment in daily life. RESULTS AND CONCLUSION: All the implanted adults presented a significant reduction in the scores for sentences recognition as the S/N decreased. The medians' curve for sentence recognition reached 50% in the signal-to-noise ratio of +10 dB. There was no statistically significant difference in sentences' recognition scores and difficulty scores obtained with the SHHI, for all types of implants. The difficulties of implanted adults were rare in quiet and occasional in noisy situations according to SHHI questionnaire.
