Abstract Hereditary transthyretin amyloidosis with peripheral neuropathy (ATTRv-PN) is an autosomal dominant inherited sensorimotor and autonomic polyneuropathy with over 130 pathogenic variants identified in the TTR gene. Hereditary transthyretin amyloidosis with peripheral neuropathy is a disabling, progressive and life-threatening genetic condition that leads to death in ~ 10 years if untreated. The prospects for ATTRv-PN have changed in the last decades, as it has become a treatable neuropathy. In addition to liver transplantation, initiated in 1990, there are now at least 3 drugs approved in many countries, including Brazil, and many more are being developed. The first Brazilian consensus on ATTRv-PN was held in the city of Fortaleza, Brazil, in June 2017. Given the new advances in the area over the last 5 years, the Peripheral Neuropathy Scientific Department of the Brazilian Academy of Neurology organized a second edition of the consensus. Each panelist was responsible for reviewing the literature and updating a section of the previous paper. Thereafter, the 18 panelists got together virtually after careful review of the draft, discussed each section of the text, and reached a consensus for the final version of the manuscript.
Resumo Polineuropatia amiloidótica familiar associada a transtirretina (ATTRv-PN) é uma polineuropatia sensitivo-motora e autonômica hereditária autossômica dominante com mais de 130 variantes patogênicas já identificadas no gene TTR. A ATTRv-PN é uma condição genética debilitante, progressiva e que ameaça a vida, levando à morte em ~ 10 anos se não for tratada. Nas últimas décadas, a ATTRv-PN se tornou uma neuropatia tratável. Além do transplante de fígado, iniciado em 1990, temos agora 3 medicamentos modificadores de doença aprovados em muitos países, incluindo o Brasil, e muitas outras medicações estão em desenvolvimento. O primeiro consenso brasileiro em ATTRv-PN foi realizado em Fortaleza em junho de 2017. Devido aos novos avanços nesta área nos últimos 5 anos, o Departamento Científico de Neuropatias Periféricas da Academia Brasileira de Neurologia organizou uma segunda edição do consenso. Cada panelista ficou responsável por rever a literatura e atualizar uma parte do manuscrito. Finalmente, os 18 panelistas se reuniram virtualmente após revisão da primeira versão, discutiram cada parte do artigo e chegaram a um consenso sobre a versão final do manuscrito.

Abstract The antinociceptive and anti-inflammatory activities of crude ethanolic extract of Celtis iguanaea leaves and their active fractions are reported. The oral treatment with crude ethanolic extract (CEE; 100, 300 or 1000 mg/Kg) inhibited the number of writhings in a dose-dependent manner. The intermediate dose also inhibited formalin-induced nociception in both phases. The oral treatment with dichloromethane fraction (DF; 9 mg/Kg) produced antinociceptive effect in both phases of formalin test; however, the treatment with ethyl acetate fraction (EAF; 16 mg/Kg) reduced pain only in the second phase of this test. The oral treatments with CEE (300 mg/Kg) or DF (9 mg/Kg) reduced the nociception induced by capsaicin and pre-treatment with naloxone did not change these effects. The oral administration of CEE (300 mg/Kg), DF (9 mg/Kg) or ethyl EAF (16 mg/Kg) reduced ear edema, leukocytes migration and myeloperoxidase activity. Furthermore, the oral treatment with CEE (300 mg/Kg) or EAF (16 mg/Kg) reduced the level of Tumor Necrosis Factor - Alpha (TNF-α) in the pleurisy test. In conclusion, the DF showed antinociceptive activity that involves the vanilloid system as well as anti-inflammatory effect and the EAF showed anti-inflammatory activity involving the reduction of TNF-α cytokine.

Abstract INTRODUCTION: Cardiovascular diseases (CDVs) have become increasingly important for progressively older people living with HIV (PLHIV). Identification of gaps requiring improvement in the care cascade for hypertension, a primary risk factor for CVDs, is of utmost importance. This study analyzed the prevalence of hypertensive status and described the care cascade for hypertension screening, diagnosis, treatment, treatment adherence, and management in PLHIV. METHODS: This cross-sectional study included 298 PLHIV (age >40 years) who visited a referral center in the western Brazilian Amazon. Data were collected through a structured questionnaire interview and medical examinations. Thus, information regarding sociodemographic and clinical aspects, blood pressure, weight, height, body mass index, and laboratory profile was obtained. Descriptive and analytical statistics were performed, and results were considered significant ifp <0.05. RESULTS: In total, 132 (44.3%) participants reported that their blood pressure was never measured. The prevalence of hypertension was found to be 35.9% (107/298). Of these 107 participants, only 36 (33.6%) had prior knowledge of their hypertensive status, and 19 of 36 (52.7%) participants had visited a physician or cardiologist to seek treatment. Adherence to the BP-lowering treatment was noted in 11 (10.2%) participants. CONCLUSIONS: An increased prevalence of hypertension was found, and most of the hypertensive participants were unaware of their hypertensive status. In addition, blood pressure control was poor in the study population. This indicated that public health professionals did not sufficiently consider the full spectrum of healthcare and disease management for PLHIV.

RESUMO Polineuropatia amiloidótica familiar é uma polineuropatia sensitivo-motora e autonômica de herança autossômica dominante, que caso não seja tratada leva a morte em aproximadamente 10 anos. O transplante de fígado e o tafamidis são os únicos tratamentos disponíveis no Brasil. Essa revisão consiste em um consenso do Departamento Científico de Neuropatias Periféricas da Academia Brasileira de Neurologia. O primeiro e último autores produziram um texto resumindo os principais aspectos sobre o tema e enviaram para os outros 10 especialistas por email. A literatura relevante sobre o assunto foi revisada por cada participante e utilizada para revisão individual do texto. Foi esperado que cada participante revisasse o texto e enviasse suas sugestões por e-mail. Finalmente, os 12 panelistas se encontraram na cidade de Fortaleza para discutir os pontos controversos e chegar a um consenso sobre texto final.
ABSTRACT Transthyretin familial amyloid polyneuropathy is an autosomal dominant inherited sensorimotor and autonomic polyneuropathy, which if untreated, leads to death in approximately 10 years. In Brazil, liver transplant and tafamidis are the only disease-modifying treatments available. This review consists of a consensus for the diagnosis, management and treatment for transthyretin familial amyloid polyneuropathy from the Peripheral Neuropathy Scientific Department of the Brazilian Academy of Neurology. The first and last authors produced a draft summarizing the main views on the subject and emailed the text to 10 other specialists. Relevant literature on this subject was reviewed by each participant and used for the individual review of the whole text. Each participant was expected to review the text and send a feedback review by e-mail. Thereafter, the 12 panelists got together at the city of Fortaleza, discussed the controversial points, and reached a consensus for the final text.

RESUMO O crescent arsenal terapêutico na esclerose múltipla (EM) tem permitido tratamentos mais efetivos e personalizados, mas a escolha e o manejo das terapias modificadoras da doença (TMDs) tem se tornado cada vez mais complexos. Neste contexto, especialistas do Comitê Brasileiro de Tratamento e Pesquisa em Esclerose Múltipla e do Departamento Científico de Neuroimunologia da Academia Brasileira de Neurologia reuniram-se para estabelecer este Consenso Brasileiro para o Tratamento da EM, baseados no entendimento de que neurologistas devem ter a possibilidade de prescrever TMDs para EM de acordo com o que é melhor para cada paciente, com base em evidências e práticas atualizadas. Por meio deste documento, propomos recomendações práticas para o tratamento da EM, com foco principal na escolha e no manejo das TMDs, e revisamos os argumentos que embasam as estratégias de tratamento na EM.
ABSTRACT The expanding therapeutic arsenal in multiple sclerosis (MS) has allowed for more effective and personalized treatment, but the choice and management of disease-modifying therapies (DMTs) is becoming increasingly complex. In this context, experts from the Brazilian Committee on Treatment and Research in Multiple Sclerosis and the Neuroimmunology Scientific Department of the Brazilian Academy of Neurology have convened to establish this Brazilian Consensus for the Treatment of MS, based on their understanding that neurologists should be able to prescribe MS DMTs according to what is better for each patient, based on up-to-date evidence and practice. We herein propose practical recommendations for the treatment of MS, with the main focus on the choice and management of DMTs, as well as present a review of the scientific rationale supporting therapeutic strategies in MS.

Prevalence of cardiovascular risk factors, the association with socioeconomic variables in adolescents from low-income region

Objectives: To estimate the prevalence of obesity, overweight, abdominal obesity and high blood pressure in a sample of adolescents from a low-income city in Brazil and to estimate the relationship with the socioeconomic status of the family, the education level of the family provider and the type of school. Methods: This cross-sectional study randomly sampled 1,014 adolescents (54.8% girls), between 14-19 years of age, attending high school from Imperatriz (MA). The outcomes of this study were: obesity and overweight, abdominal obesity and high blood pressure (systolic and/or diastolic). The independent variables were: socioeconomic status (SES) of the family, education level of the family provider (ELFP) and type of school. The confounding variables were: gender, age and physical activity level. Prevalence was estimated, and the association between the endpoints and the independent variables was analyzed using a prevalence ratio (PR), with a 95% confidence interval, estimated by Poisson regression. Results: The overall prevalence of obesity was 3.8%, overweight, 13.1%, abdominal obesity, 22.7% and high blood pressure, 21.3%. The adjusted analysis indicated that girls with high SES showed an increased likelihood to be overweight (PR=1.71 [95% IC: 1.13-2.87]), while private school boys had an increased likelihood of obesity (PR=1.79 [95% CI: 1.04-3.08]) and abdominal obesity (PR =1.64 [95% CI: 1.06-2.54]). Conclusion: The prevalence of CVDR is high in adolescents from this low-income region. Boys from private schools are more likely to have obesity and abdominal obesity, and girls with high SES are more likely to be overweight.
Objetivos: Estimar la prevalencia de obesidad y sobrepeso, obesidad abdominal y hipertensión arterial en una muestra de adolescentes pertenecientes a una ciudad de baja renta en Brasil y su relación con el nivel socioeconómico, nivel educativo de lo responsable de la familia y tipo de escuela. Métodos: Estudio transversal con una muestra de 1014 adolescentes (54,8% chicas), con edades entre 14-19 años, estudiantes de las escuelas de la ciudad de Imperatriz (Brasil), seleccionadas por un muestreo aleatorio. Las variables dependientes evaluadas son: obesidad general y sobrepeso, obesidad abdominal, y tensión arterial alta (sistólica y/o diastólica). Las variables independientes son: nivel socioeconómico de la familia (NSO), el nivel de educación de lo responsable de la familia (NERF) y tipo de escuela. Las variables de confusión son: sexo, edad y nivel de actividad física. La prevalencia fue estimada, y la asociación entre las variables dependientes y las variables independientes se analizaron mediante razón de prevalencia (RP), con intervalo de confianza (IC) del 95%, estimado por la regresión de Poisson. Resultados: La prevalencia de la obesidad general fue de 3,8%, sobrepeso 13,1%, obesidad abdominal 22,7% y la tesión arterial alta 21,3%. Las análisis ajustadas indicaron que las chicas con NSO alto tienen mayor probabilidad de tener sobrepeso (RP=1,71 [IC95%: 1,13 a 2,87]), y chicos de las escuelas privadas tienen más probabilidad de tener obesidad (RP=1,79 [IC95%: 1.04- 3,08]) y obesidad abdominal (RP=1,64 [IC95%: 1,06 a 2,54]). Conclusión: La prevalencia de los FRC es alta en adolescentes de una región de baja renta. Los chicos de las escuelas privadas son más propensos a tener obesidad y obesidad abdominal, y las chicas con NSO alto son más propensas a tener sobrepeso.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

A Hydrocotyle umbellata L., é uma espécie pertence à família Araliaceae, popularmente conhecida como acariçoba. Suas indicações na medicina popular incluem tratamento de úlceras cutâneas, eczemas e reumatismo. O objetivo deste trabalho é avaliar as atividades antinociceptiva e/ou anti-inflamatória do extrato etanólico das partes subterrâneas da acariçoba (EEA). O EEA reduziu a resposta nociceptiva do animais no teste das contorções abdominais induzidas por ácido acético e em ambas as fases do teste da formalina. O EEA também mostrou uma atividade analgésica supra-espinhal ao aumentar o limiar nociceptivo no método da placa quente. O EEA reduziu tanto o número de leucócitos migrados quanto o extravasamento plasmático para a cavidade pleural na pleurisia induzida por carragenina. Além de reduzir o edema de pata induzido por carragenina até a segunda hora e também o edema de pata induzido por dextrana. Em conclusão os nossos resultados mostram que o EEA de H. umbellata L. possui atividades analgésica e anti-inflamatória, e que um bloqueio da atividade ou redução da liberação de diferentes mediadores tais como histamina e serotonina, podem estar envolvidos nestes efeitos farmacológicos.
The Hydrocotyle umbellata L. is a specimen of the Araliaceae family popularly known as acariçoba. Its indications in folk medicine include treatment of skin ulcers, and rheumatism. The aim of this study was to evaluate the antinociceptive and anti-inflammatory activities of the ethanolic extract from acariçoba's underground parts (EEA). EEA reduced the nociceptive response of the animals as evaluated in the acetic acid-induced writhing test and in both phases of formalin test. EEA also presented a supraspinal analgesic activity by increasing the pain latency in the hot plate test. Moreover, EEA reduced the leukocytes migration and plasma extravasation to pleural cavity in the carrageenan-induced pleurisy, besides reducing the edema induced by carrageenan until the second hour and also the edema induced by dextran. In conclusion our results showed that EEA of H. umbellata L. presents analgesic and anti-inflammatory activities, and that a blockade of activity or reduction in the release of different mediators, such as histamine and serotonin, could be involved in these pharmacologic effects.

O pacari (Lafoensia pacari A. St.-Hil., Lythraceae) é uma espécie vegetal nativa do cerrado, o extrato da casca de caule é utilizado popularmente para dores e inflamação, tendo mostrado atividade sedativa. Este trabalho objetivou avaliar os efeitos do extrato etanólico das folhas do pacari (EEFP) como analgésico e antiinflamatório. As atividades analgésica e antiinflamatória foram verificadas em camundongos. O tratamento prévio com EEFP 1,0 g/kg mostrou atividade antinociceptiva tanto no método das contorções abdominais induzidas por ácido acético como também no modelo de dor induzida por formalina, tanto na fase neurogênica quanto na fase inflamatória. Os pré-tratamentos com o EEFP reduziram o edema de orelha, induzido por óleo de cróton, de forma dose-dependente. Os testes de atividade no sistema nervoso central mostraram que o extrato não provoca incoordenação motora nem hipnose ou sedação. Os resultados mostram que o EEFP mantém as atividades analgésica e antiinflamatória do extrato das cascas do caule do pacari, sendo que a coleta das folhas favorece a preservação desta espécie nativa do cerrado.
Lafoensia pacari A. St.-Hil., Lythraceae, popularly known as pacari, is a Cerrado's native specimen; the stem bark extract is used in folk for pain and inflammation, also showing sedating activity. This study aimed to evaluate the analgesic and anti inflammatory activities of ethanol extract from pacari leaves (EEPL). These activities were verified in mice. The previous treatment with EEPL 1.0 g/kg showed antinociceptive activity both in the acetic acid-induced writing test and in the formalin-induced model of pain, and in neurogenic and inflammatory phases as well. In the croton oil-induced ear edema, the pre-treatments with EEPL reduced the edema in a dose-dependent manner. Also in the carrageenan-induced peritonitis, the two major doses tested (2.0 and 1.5 g/kg p.o.) were able to reduce the leukocyte migration in a dose-dependent manner. The Central Nervous System tests showed that the extract does not elicit uncoordinated motricity, hypnosis or sedating effects. The results showed that EEPL maintains the analgesic and anti-inflammatory effects of the stem bark of pacari, being the collect of leaves more favorable to the preservation of this Cerrado's native specimen.

Avaliação da uniformidade de aplicação de água em um sistema de gotejamento para pequenas propriedades

O uso de sistemas de irrigação por gotejamento à baixa pressão tem como objetivo oferecer uma técnica de irrigação viável para pequenas propriedades. Foram dimensionados oito sistemas de irrigação, um com os seguintes acessórios: 210 metros de tubo gotejador com diâmetro interno (DI) de 10,4 mm, sete conectores de 12 mm, e seis metros de tubo de polietileno, um adaptador para tubo de polietileno, uma curva de 90º e três metros de tubo de PVC com DI igual a 20,6 mm. Quatro sistemas foram montados com um filtro de tela de 130 μm e os demais com um filtro de disco de 200 μm. Em todos os sistemas foram utilizados um reservatório de 1 m³ localizado a 3 m acima do nível do solo. Irrigou-se 1296 m² de uma cultura de tomate, em canteiros com dimensões de 1,20 m por 60 m. Para avaliação técnica do projeto foi obtida a uniformidade de aplicação de água através do coeficiente de uniformidade de emissão (UE) e coeficiente de uniformidade de distribuição (UD), durante um período de 90 dias. Os resultados indicaram que o UE e o UD foram excelentes e que os valores de grau de entupimento aumentaram com o uso da irrigação. Assim, de acordo com os resultados, comprova-se a viabilidade técnica da utilização de sistemas de irrigação por gotejamento funcionando a baixa pressão para pequenas áreas.
A low pressure drip irrigation gravity feed system may be a viable technique to small farms. So, according with it, eight irrigation systems with 10.4 mm internal diameter driptubing 210 m long, seven 12 mm adapters, 20.6 mm polyethylene pipe 6 m long, polyethylene adapter, 20.6 mm 90º elbow, 20.6 mm PVC pipe 3 m long and 1 m³ water reservoir at 3 m height, were designed and assembled to irrigate 1.296 m² with tomato cultivated in 0.8 x 60 m beds. One 130 mm screen filter was used to four systems and one 200 mm disc filter for the other ones. Emission uniformity coefficients (UE) and distribution uniformity coefficients (UD) were obtained throughout 90 days. UE and UD were excellent and the degree of clogging at the emitters increased with time. Therefore, according to the results, the low pressure drip irrigation systems tested are technically viable to small areas.
