OBJETIVO: Avaliar se o tratamento com L-arginina influencia a cicatrização de retalhos cutâneos em ratos expostos à nicotina. MÉTODOS: Foram utilizados 40 ratos Wistar pesando 142,4±10,1g separados em quatro grupos: GC- tratamento com solução tampão fosfatos pH 7,4, confecção de retalho cutâneo, observação por 10 dias; GN- exposição à nicotina por quatro semanas, confecção de retalho cutâneo, observação por dez dias; GA- tratamento com solução tampão fosfatos pH 7,4 por quatro semanas, confecção de retalho cutâneo, tratamento com arginina por dez dias; GAN- exposição à nicotina por quatro semanas, confecção de retalho cutâneo, tratamento com arginina por dez dias. Foram avaliadas as áreas de necrose, re-epitelização, reação inflamatória e formação de tecido de granulação, pela coloração HE, a área de deposição total e a diferenciação de colágenos I e III por histometria com a coloração de picrosirius, e, através da marcação imunoistoquímica com anticorpo monoclonal anti-CD34, a densidade vascular cicatricial. RESULTADOS: As porcentagens de áreas de necrose de GN e GNA foram maiores (p<0,001) do que GC e GA. Nos escores histológicos, a deposição de colágeno e a porcentagem de colágeno tipo I, no GC e GA foram similares (p>0,05) e maiores (p<0,001) do que em GA e em GNA e, nas densidades vasculares, GN e GAN foram menores (p<0,001) do que em GC e em GA. CONCLUSÃO: A exposição à nicotina inibiu os efeitos da arginina, e nos ratos não expostos, induziu melhora na angiogênese e na deposição de colágeno total nos retalhos cutâneos.

OBJECTIVE: To evaluate whether treatment with L-arginine influences the healing of skin flaps in rats exposed to nicotine. METHODS: 40 male Wistar rats weighing 142.4 ± 10.1 g were separated into four groups: GC: treatment with 7.4 pH phosphate buffer, submitted to skin flap and observation for ten days; GN: exposure to nicotine for four weeks, submitted to skin flap and observation for ten days; GA: treatment with 7.4 pH phosphate buffer for four weeks, submitted to skin flap and arginine treatment for ten days; GAN: exposure to nicotine for four weeks, submitted to skin flap and treatment with arginine for ten days. We evaluated: areas of necrosis, re-epithelialization, inflammatory reaction and formation of granulation tissue by HE stain; the total area of deposition and differentiation of collagens I and III by histometry with picrosirius staining; and the scar vascular density by immunohistochemical staining with monoclonal anti-CD34 antibodies. RESULTS: The percentages of necrotic areas in GN and GNA were higher (p <0.001) than in GC and GA. In histological scores, collagen deposition, and the percentage of type I collagen, GA and GC were similar to each other (p> 0.05), but higher (p <0.001) than GA and GNA; as for vascular densities, they were lower in GN and GAN (p <0.001) than in GC and GA. CONCLUSION: Exposure to nicotine inhibited the effects of arginine and in unexposed mice there was induction of angiogenesis and improvement in the total collagen deposition in the skin flaps.

A hepatectomia em duas etapas utiliza a capacidade de regeneração do fígado, após uma primeira hepatectomia não curativa, para permitir uma segunda ressecção. Neste artigo relatamos os aspectos técnicos do manejo de uma doente de 37 anos de idade, com metástases colorretais sincrônicas, onde uma única hepatectomia não era suficiente para remover todas as lesões, mesmo em combinação com quimioterapia, embolização portal ou radiofrequência. Porém as metástases poderiam ser removidas por duas ressecções sequenciais.

Two-stage hepatectomy uses compensatory liver regeneration after a first noncurative hepatectomy to enable a second curative resection. Herein we report the tecnical aspects of the management of a thirty-seven years-old woman with colorectal metastasis to the liver eligible because single resection could not achieve complete treatment, even in combination with chemotherapy, portal embolization, or radiofrequency, but tumors could be totally removed by two sequential resections.

O transplante hepático inter vivos adulto expandiu as possibilidades terapêuticas para pacientes com insuficiência hepática terminal. A necessidade de um volume hepático adequado que será retirado do doador e necessário ao receptor limita sua utilização em alguns casos. Apresentamos um caso em que se utilizou dois lobos esquerdos de dois doadores vivos no intuito de prover parênquima hepático suficiente ao receptor.

Living donor liver transplantation expanded the therapeutic possibilities for liver failure patients. The necessary correct match between the liver mass donated and received sometimes limits its use. A case that was used two left liver grafts from adult living donors is reported.

RACIONAL: O transplante hepático é a melhor modalidade terapêutica para pacientes em estágio final de doença hepática. Minimização de morte, enquanto se espera o procedimento, envolve priorização de acordo com o estado clínico e a alocação adequada de fígados de doadores. OBJETIVO: Análise da mortalidade na lista de espera de fígado no estado do Paraná, PR, Brasil. MÉTODOS: Foram analisados os dados sobre todos os pacientes (n = 65) que foram registrados na lista de espera de fígado durante um período de 32 meses. RESULTADOS: A morte em lista de espera foi de 41,5% (n = 27). Nenhuma diferença estatística foi observada em relação aos MELD / MELD-Na entre o grupo que faleceu (19,88 / 21,6) e não morreu (17,28 / 19,47). MELD-Na previu maior mortalidade, especialmente no subgrupo de pacientes com gravidade intermediária da doença (classe B) previsto pelo escore de CTP. CONCLUSÃO: É crítica a escassez de doadores de órgãos nessa região e a taxa de mortalidade em lista de espera excede em muito o risco inerente de um transplante de fígado, especialmente entre pacientes com MELD mais baixos. É desejável a utilização de um protocolo agressivo de doadores com critérios expandidos, split liver e transplante de doador vivo.

BACKGROUND: Orthotopic liver transplantation is the best therapeutic modality for patients with end stage of liver disease. Minimization of death, while waiting for the procedure, involves accurate priorization according to clinical status and appropriate allocation of donor livers. AIM: The mortality analysis in the liver waiting list in Paraná state, PR, Brazil. METHODS: Were analyzed the data on all patients (n=65) who were registered on the liver waiting list during a 32 months period in the state of Paraná, southern Brazil. RESULTS: The death rated in waiting list was 41,5% (n=27). No statistic difference was observed regarding the MELD/MELD-Na scores between the group who died (19,88/21,6) and not died (17,28/19,47). MELD-Na predicted a higher mortality, especially in the subgroup of patients with intermediate severity of disease (class B) predicted by the CTP score. CONCLUSION: It´s critical the shortage of donor organs in our region, waiting list mortality rate far exceeds the inherent risk of a liver transplant, especially among patients with lower MELD scores. It´s desirable to use an aggressive protocol of expanded criteria donors, split liver and living donor transplant.

OBJETIVO: Correlacionar a dosagem sérica pré-operatória e pós-operatória de interleucina-6 (IL-6) e interleucina-10 (IL-10) entre pacientes submetidos à colecistectomia laparotômica versus videolaparoscópica. MÉTODOS: De um total de 20 pacientes, 18 foram incluídos no estudo, sendo nove submetidos à colecistectomia laparoscópica e os outros nove utilizando a técnica laparotômica. As concentrações séricas de IL-6 e IL-10 foram dosadas em ambos os grupos. As amostras de sangue foram obtidas nos tempos de 24 horas no pré-operatório, quatro, 12 e 24 horas após o procedimento. Os grupos foram comparados em relação à idade, sexo, índice de massa corpórea (IMC), tempo de anestesia e de operação. RESULTADOS: Não houve diferenças significativamente estatísticas entre os grupos relacionadas à idade, sexo, IMC, tempo de anestesia e de operação. A comparação entre a colecistectomia laparotômica e laparoscópica demonstrou diferenças estatísticas nos níveis de IL-6 no tempo 12 horas após operação (218,64pg/ml laparotômica versus 67,71pg/ml laparoscópica, p=0,0003) e IL-10 no tempo de 24 horas após o procedimento (24,46pg/ml aberta versus 10,17pg/ml laparoscópica, p <0,001). CONCLUSÃO: Houve aumento das dosagens de interleucinas-6 e 10 após o trauma cirúrgico. Ocorreu aumento significativo dos níveis das interleucinas analisadas no grupo laparotômico em comparação com o grupo laparoscópico.

OBJECTIVE: To correlate serum preoperative and postoperative interleukin-6 and interleukin-10 levels in patients undergoing laparotomy versus laparoscopic cholecystectomy. METHODS: From a total of 20 patients, 18 were included in the study, nine underwent cholecystectomy by laparoscopy and the other nine by laparotomy. Serum concentrations of IL-6 and IL-10 were measured in both groups. Blood samples were obtained in the times of 24 hours preoperatively and four, 12 and 24 hours after the procedure. The groups were compared regarding age, gender, body mass index (BMI), duration of anesthesia and operation. RESULTS: There was no significant statistical differences between groups related to age, gender, BMI, duration of anesthesia and operation. The comparison between the two procedurs demonstrated statistical differences for IL-6 in time 12 hours after operation (218.64 pg/ml laparotomic versus 67.71 pg/ml laparoscopic, p = 0.0003) and for IL-10 in time 24 hours after the procedure (24.46 pg/ml open versus 10.17 pg/qml laparoscopic, p <0.001). CONCLUSION: There was an Increase in plasma levels of interleukin-6 and 10 after surgical trauma with a significant increase in levels of interleukins in the laparotomic group in comparison with the laparoscopic group.

OBJETIVO: verificar se o tacrolimus administrado em ratos, em vigência de pancreatite induzida pela L-Arginina, interfere nos níveis séricos da amilase e glicose e no padrão histológico do parênquima pancreático. MÉTODOS: quarenta ratos Wistar foram distribuídos em quatro grupos com 10 ratos cada. Grupo controle (C), grupo tacrolimus (T), grupo pancreatite (P) e grupo pancreatite-tacrolimus (PT). Foram avaliados os níveis séricos de amilase, glicose e tacrolimus e feitas avaliações histológicas do pâncreas, A indução de pancreatite foi feita pela inoculação de L-Arginina na dose de 500mg/100g de peso corporal por via intraperitoneal e o tratamento com tacrolimus na dose de 1ìg/kg por via subcutânea durante quatro dias. RESULTADOS: a amilasemia estava mais elevada (p=0,0000) nos grupos PT, T e P do que no grupo controle. A média do grupo PT foi maior (p=0,0009) que a do grupo T, mas não diferiu (p=0,6802) da média do grupo P. Entre os grupos P e T não houve diferença (p=0,2568). Não houve diferença nas médias de glicemia entre os grupos (p=0,4920) e os níveis séricos de tacrolimus foram similares nos grupos PT e T (p=0,7112). Não ocorreram alterações histológicas nos grupos T e C e não ocorreu hemorragia no pâncreas dos ratos dos grupos P e PT. No grupo P, em 30% não se observou edema, em 20% observou-se a forma leve e em 50%, a moderada; quanto à infiltração inflamatória, em 80% moderada e em 20% não ocorreu, e a atrofia do parênquima foi de 60% moderada e 40% acentuada. No grupo PT, houve ocorrência de edema, infiltração inflamatória e atrofia do pâncreas em todos os ratos. CONCLUSÃO: o tratamento pelo tacrolimus induziu aumento nos níveis séricos de amilase em ratos normais, não alterou a glicemia nem o padrão histológico do parênquima pancreático. Na vigência de pancreatite induzida pela L-Arginina o tacrolimus induziu edema, infiltração inflamatória e atrofia com maior gravidade no parênquima pancreático.

OBJECTIVE: To determine whether tacrolimus administered to rats, in the presence of pancreatitis induced by L-Arginine, interferes with the serum levels of amylase and glucose and the histological pattern of the pancreatic parenchyma. METHODS: Forty Wistar rats were divided into four groups with 10 rats each: control group (C), tacrolimus group (T), pancreatitis group (P) and pancreatitis-tacrolimus group (PT). We evaluated serum levels of amylase, glucose, and tacrolimus and made histological assessments of the pancreas. Induction of pancreatitis was made by inoculation of L-Arginine at a dose of 500mg/100g body weight intraperitoneally, and tacrolimus treatment at a dose of 1ìg/kg subcutaneously for four days. RESULTS: Serum amylase was higher (p = 0.0000) in groups PT, P and T than in the control group. The PT group mean was higher (p = 0.0009) than in the T group, but did not differ (p = 0.6802) from the average of the P group. There was no difference between groups P and T (p = 0.2568). Neither in mean blood glucose between the groups (p = 0.4920); serum levels of tacrolimus were similar in PT and T groups (p = 0.7112). There were no histological changes in groups T and C and no hemorrhage in the pancreas of rats in groups P and PT. In group P, there was no edema in 30%, mild edema in 20% and in 50%, moderate; as for inflammatory infiltration, it was moderate in 80% and absent in 20%, and atrophy of the parenchyma was moderate in 60% and severe in 40%. In the PT group, there was edema, inflammatory infiltration or atrophy in the pancreas in all rats. CONCLUSION: Treatment with Tacrolimus induced an increase in serum amylase in normal mice, but did not affect blood glucose or the histological pattern of the pancreatic parenchyma. In the presence of pancreatitis induced by L-Arginine tacrolimus induced edema, inflammatory infiltration and more severe atrophy in the pancreatic parenchyma.

OBJETIVO: Relatar nossa experiência com 100 transplantes de pâncreas realizados em um período de sete anos. MÉTODOS: Entre janeiro de 2001 e janeiro de 2008, 100 pacientes foram submetidos a transplante de pâncreas em nosso serviço, sendo 88 transplantes de pâncreas e rim simultâneo (TPRS) e 12 transplantes de pâncreas isolado (TPI). Todos foram transplantes primários. O manejo da porção exócrina do enxerto pancreático envolveu drenagem entérica em oito casos (todos TPRS) e a bexiga em 92 casos. O sistema venoso sistêmico do receptor foi utilizado para a drenagem venosa do enxerto em todos os casos. Nossos últimos 30 pacientes submetidos à TPRS não receberam terapia de indução independentemente do painel imunológico.Os pacientes TPRS receberam basiliximab e TPI receberam timoglobulina nos casos induzidos. Imunossupressão de manutenção foi realizada com tacrolimus, micofenolato mofetil e corticóides. O volume de perfusão do enxerto pancreático foi limitado a 800ml da solução de Celsior ou UW. RESULTADOS: Demonstram que os enxertos ainda funcionantes são atualmente 64 dos 100 realizados. Perda do enxerto foi causada por: rejeição (oito pacientes), trombose venosa (nove pacientes), trombose arterial (um paciente) Complicações cirúrgicas encontradas: fístula anastomótica (tres pacientes), infecção peri-enxerto (10 pacientes), pancreatite do enxerto (cinco pacientes). A Rejeição foi observada com menos freqüência nos TPRS (5/92) que nos TPI (3/12). A morte ocorreu em 24 pacientes. CONCLUSÃO: Nossa impressão é que o transplante de pâncreas é altamente efetivo como terapia para o diabetes mellitus apesar da morbidade do procedimento.

OBJECTIVE: Report our experience with 100 pancreas transplants performed in a period of seven years. METHODS. Between January 2001 and January 2008, 100 patients underwent pancreatic transplantation at our center, 88 simultaneous pancreas kidney transplantation (SPK) and 12 pancreas transplantation alone (PTA). All of these were primary transplants. Pancreas graft management of the exocrine drainage technique involved enteric drainage in 8 (all SPK) and bladder in 92 cases. The recipient systemic venous system was used for the pancreas graft venous effluent in all cases. Our last thirty patients submitted to SPK did not receive any induction therapy regardless of the PRA. SPK received basiliximab and PTA patients received thymoglubulin. Maintance imunossupression was with TAC, MMF and corticosteroids. Graft perfusion volume was limited to 800ml of Celsior or UW solution. RESULTS. Overall results show that the number of functioning pancreatic grafts is 64 after 100 performed. Graft losses were: rejection (8 cases), venous thrombosis (9 cases) arterial thrombosis(1 case), or surgical complications such as anastomotic leak (3 cases), perigraft infection (10 cases), pancreatitis of the graft(5 cases). Rejection was observed less frequently in SPK recipients 5 cases (5/92 recipients) than PTA recipients (3/12). Death was observed in 24 cases. CONCLUSION. Our impression is that pancreas transplantation is highly effective therapy for diabetes mellitus and there is still a role in the diabetes treatment for allograft transplantation in a near future.

RACIONAL: A videocirurgia pode apresentar complicações inerentes ao método e dentre elas destacam-se as relacionadas ao aumento da pressão na cavidade intra-abdominal. OBJETIVO: Analisar os efeitos do pneumoperitônio em modelo experimental de endotoxemia causada por lipopolissacarídeo. MÉTODO: Foram utilizados 32 ratos Wistar dos quais foi coletado sangue 24 horas prévias para obtenção dos valores de referência nas provas de função renal, hepática e do estado endotóxico (contagens total e diferencial de leucócitos, contagem de plaquetas e dosagens de lipopolissacarídeo, TNFα, IL6). A seguir os ratos foram separados em quatro grupos com oito: grupos controle inoculados com lipopolissacarídeo (10 mg/kg via intraperitoneal) e mantidos por quatro e 11 horas (C-LPS4 e C-LPS11). Grupo C-PP, submetido ao pneumoperitônio por CO2 por uma hora e mantido sob observação por seis horas. Grupo experimento (E) inoculados com 10 mg/kg de lipopolissacarídeo por via intraperitoneal, após quatro horas submetidos ao pneumoperitônio por CO2 por uma hora e mantidos sob observação por seis horas. Ao término destes períodos foram coletadas amostras de sangue para as mesmas avaliações iniciais. Utilizou-se o método de T-Student para as comparações dos resultados, com nível de significância de 0,05. RESULTADOS: Em comparação aos valores iniciais e aos grupos controle, o grupo E apresentou maiores valores nas dosagens de creatinina e uréia, bilirrubina direta, ALT e TAP, aumento nas dosagens de IL6, TNFα e diminuição de plaquetas, aumento de leucócitos e bastonetes e detecção de endotoxina circulante. CONCLUSÃO: O pneumoperitônio por dióxido de carbono induz aumentos na liberação de TNFα, IL6, piora das funções hepática e renal em modelo experimental de endotoxemia induzida por lipopolissacarídeo.

BACKGROUND: Videosurgery can bring complications with it´s manipulation and among them the hyperpression into the abdominal cavity is important factor. AIM: To analyze the pneumoperitoneum effects in an endotoxemia experimental model caused by lipopolyssacharide. METHOD: Thirty-two Wistar rats were utilized, from which blood was collected 24 hours prior to the experiment for the initial evaluations of the renal function, the hepatic function and the endotoxic state (total counts and differential of leukocytes, platelet count and dosages of lipopolyssacharide, TNFα, IL6). The rats were then separated into four groups of eight: control groups inoculated with lipopolyssacharide (10 mg/kg via intraperitoneum) and kept for four and 11 hours (C-LPS4 and C-LPS11). Group C-PP, control of pneumoperitoneum by carbon dioxide at 10 mmHg for one hour and kept under observation for six hours. Experiment group (E) in which the rats were inoculated with 10 mg/kg of lipopolyssacharide via intraperitoneum and after four hours were submitted to pneumoperitoneum by carbon dioxide at 10 mmHg for one hour and kept under observation for six hours. At the end of these periods, blood samples were collected for the same initial evaluations. The T-student method was utilized for statistical comparisons of results, with a significance level of 0.05. RESULTS: Compared to the initial values and the control groups, Group E had higher values in the renal function tests (creatinine=0.85 ± 0.24 and urea=106.38 ± 37.61 mg/dL). In the hepatic function tests, there were higher values in the dosages of direct bilirubin (0.34 ± 0.09mg/dL), ALT (390,38 ± 351.88) and TAP (17.01 ± 2.18). The endotoxic state was confirmed by the dosages of IL6 (36.0 ± 11.23 pg/mL), TNFα (518.36 ± 203.39 pg/mL) and by the hematological alterations: reduction of platelets, increased leukocytes and band neutrophils and the detection of circulating endotoxin (0.21 ± 0.08 UE/mL). CONCLUSION: Pneumoperitoneum by carbon dioxide induces increase of TNFα, IL6, worsens the hepatic and renal function in the experimental endotoxemia model induced by lipopolyssacharide.

Juxtahepatic veins, retrohepatic vena cava or major hepatic veins injuries carry a very high mortality rate regardless the technique used for its repair. Isolated survivor reports have for the most part been managed by vascular isolation techniques, usually with atriocaval shunt. We report one case with a unique management technique in a patient who sustained gunshot wound to the retrohepatic vena cava. Hemorrhage control had been obtained by intrahepatic balloon tamponade after others maneuvers have failed. The balloon was left in situ for two days as damaged control laparotomy principles and planned reoperation has been chosen.

OBJETIVO: determinar as características comuns referentes ao mecanismo de trauma e às lesões anatômicas e fisiológicas dos pacientes vítimas de trauma esplênico. MÉTODO: revisão dos prontuários de dezembro de 1999 a janeiro de 2003, sendo selecionados 524 vítimas de doença tóraco-abdominal e posteriormente internados. Desses prontuários, foram excluídos aqueles que não apresentavam mecanismo de trauma contuso e os menores de 16 anos. Resultando em uma amostra com N = 200. As variáveis de interesse no estudo foram: etiologia do trauma, órgão lesado, grau e segmento da lesão, pressão arterial sistólica e freqüência cardíaca na admissão hospitalar e fraturas de costela. RESULTADOS: a lesão esplênica isolada teve a maior incidência 39%, já a lesão associada hepato-esplênica representou 14%. Dos 116 pacientes com diagnóstico de lesão esplênica, a classificada anatomicamente em Grau IV foi a mais comum (44 pacientes). A maioria das lesões teve como mecanismo de trauma veículo auto-motor (VAM) 56 pacientes, entre os quais a lesão Grau III teve maior prevalência 33,93%. Observou-se que 20% dos pacientes com lesão Grau III, IV ou V não havia descrito dor abdominal em seus prontuários, já nas lesões Grau I ou II 30,13% não se descreveu dor. Freqüência cardíaca média (FC média) e a Pressão arterial sistólica (PA sistólica) mostraram-se equivalentes entre as diferentes lesões. A maior incidência de lesão esplênica está relacionada com ausência de fratura dos arcos costais. Quando diagnosticada fratura há predomínio do grupo com arcos costais esquerdos, nos quais as lesões Grau III, IV ou V predominam com 62,50%. CONCLUSÕES: os acidentes por veículo automotor foram a causa mais comum de trauma esplênico, sendo que a lesão Grau III foi a mais comum dentre as vítimas de VAM. Em 80% dos pacientes com lesão Grau III, IV ou V havia sido descrito dor abdominal, revelando-se um parâmetro significativo. FC média e PA sistólica não variaram, tampouco quando comparado ao grupo sem lesão em órgão sólido. A probabilidade de lesão esplênica cresceu com fraturas de arcos costais esquerdos isolados. No presente estudo, há predomínio do grupo com lesões Grau III, IV ou V.

BACKGROUND: determining the common characteristics referring to the trauma mechanism and to the anatomic and physiological lesion of patients victim of splenic trauma. METHODS: The records of December/1999 to January/2003 were revised, being selected 524 of those by the Software TNVT Plus (version 2.0.0.213 - year 1996), admitted in the Emergency Service victims of thoracic-abdominal disease and subsequently interned. Of these records, were excluded those which did not present mechanism of contused trauma, the ones that deceased in the emergency room and the minors of 16 years of age. A sample with N=200 patients were studied. The variables of interest in the study were: etiology of the trauma, organ with the lesion, degree and segment of the lesion, systolic arterial pressure, cardiac frequency in the hospital admission and rib fracture. RESULTS: the splenic injury isolated had the greater incidence 39%, the hepatic-splenic associated lesion represented 14%. Of the 116 patients with diagnosis of splenic lesion, the classified anatomically in Degree IV was the most common (44 patients). The majority of the lesions had an auto-engine vehicle as the trauma mechanism - 56 patients, among which the lesion Degree III had greater prevalence 33,93%, standard comparable with the lesion Degree IV 32,14%. It was observed that 20% of the patients with lesion Degree III, IV or V had not described abdominal pain in their records, and in the lesions Degree I or II 30,13% did not describe pain. Medium Cardiac Frequency and Systolic Arterial Pressure were shown equivalent among different lesions. The greater incidence of splenic lesion was related to the absence of rib fracture. When diagnosed fracture, there was predominance of the group with isolated left ribs fractured, in which lesions Degree III, IV or V predominate with 62,50%. CONCLUSION: the accidents by automobile vehicle are the most common cause of splenic trauma and the mechanism of trauma which causes the most serious lesions, with the lesion Degree III being the most common among the victims of automobile vehicle accidents. 80% of the patients with lesion Degree III, IV or V described abdominal pain in their records revealing a significant parameter. Medium Cardiac Frequency and Systolic Arterial Pressure did not vary, neither when compared to the group with no lesion in solid organ. The probability of splenic lesion grows with fractures of isolated left ribs. In the present study, the group with lesions Degree III, IV or V predominates.

OBJETIVO: Esta pesquisa tem por objetivo quantificar a perda de energia calórica que ocorre ao realizarmos uma laparotomia com exposição de alças intestinais à atmosfera em ratos; determinar a perda termodinâmica representada por watts/m² em função da superfície peritoneal exposta do animal e discutir meios de tratamento para a perda de calor. MÉTODO: Foram utilizados 30 ratos machos, Wistar, distribuídos em cinco grupos de seis animais cada, com idade de 8 a 9 semanas, com o peso entre 200 e 220g. No grupo A ou grupo controle, os animais foram submetidos a anestesia inalatória. No grupo B, os ratos foram submetidos a uma laparotomia mediana com exposição das alças intestinais. Nos grupos C, D e E, os ratos foram submetidos a uma laparotomia mediana com exposição das alças intestinais e tratados respectivamente, por gaze úmida, gaze seca e empacotamento com filme de poliéster. RESULTADOS: Observou-se uma significativa perda de energia calórica quando se realiza uma laparotomia com exposição das alças intestinais em ambiente não controlado, da ordem de 620,72 kJ/m² A análise da perda termodinâmica, em função da superfície corpórea exposta do rato, apresentou um valor de 382,97 W/m². CONCLUSÕES: Na avaliação dos diferentes tipos de tratamento utilizados para prevenir a perda de calor corpóreo, ficou evidente que a utilização do filme de poliéster (PVC) foi o método mais efetivo na conservação do calor, em relação à gaze seca e, ou úmida respectivamente.

BACKGROUND: This study has the intent to quantify the caloric energy loss that occurs when a laparotomy is done and the bowel is exposed to room temperature in rats; to find the thermodynamic loss measured in watt/m² over the rat peritoneal area exposed and discuss ways to manage that heat loss. METHODS: This research project used 30 male rats, Wistar, separated in 5 groups of 6 animals, from 8 to 9 weeks old and weighting between 200 to 220g. Group A animals, the control group, received inhalation general anesthesia only. Group B rats were submitted to a median laparotomy with bowel loops exposure to open room temperature. Groups C, D and E rats were subjected to median laparotomy with bowel loops exposure to open room temperature and treated with wet sponge, dry sponge and polyester film packing, respectively. RESULTS: The result showed there is a significant 620,72 kJ/m² heat energy loss with laparotomy procedures and bowel loops exposure to room temperature in a non-monitored environment. The thermodynamic loss analysis showed, due to rat exposed body surface, there was a 382,97 W/m² heat loss. CONCLUSION: Different types of treatment assessments used in this research showed the most effective method for maintaining heat was the polyester film use (PVC), when compared to t wet sponge and/or dry sponge methods, respectively.

Injuries to the main hepatic veins carry a very high mortality rate, regardless of the technique used for repair. Isolated reports of survivors have, for the most part, been managed by hepatic vascular exclusion (EVE) techniques, usually with an atriocaval shunt. Herein we report a case of a severe intrahepatic major hepatic vein injury successfully managed with EVE.

OBJETIVOS: Analisar as características demográficas, a mortalidade e morbidade associada ao procedimento. MÉTODOS: Estudo retrospectivo dos casos de reconstrução intestinal colônica um hospital universitário. Todos os pacientes tiveram o cólon preparado por solução de manitol. RESULTADOS: Do total de 42 pacientes, 80,9% (n=34) eram do sexo masculino com idade média de 42 anos. Causas que levaram a confecção da ostomia: 50% traumáticas, 29% abdome agudo clínico. A colostomia terminal foi o tipo de ostomia preferencialmente realizada em 65% dos casos acompanhado pela colostomia em alça com 35% dos casos. A técnica empregada para a anastomose foi predominantemente a manual, realizada em 69,05% dos casos (n=29). O tempo médio de internação hospitalar foi de 8,74 dias. O índice de morbidade global foi de 26,2% (n=11), destacando-se a reoperação em 9,52% (n=4) e a fístula em 7,14% (n=3) dos casos. Não ocorreu infecção de ferida operatória nessa série. A mortalidade foi de 2,38% (n=1). CONCLUSÕES: Os resultados obtidos em um hospital universitário são semelhantes aos relatados na literatura mundial. Cuidados pré e pós operatórios adequados se somam a experiência do cirurgião nas cirurgias de reconstrução de trânsito. A escolha da técnica cirúrgica deve ser padronizada através de trabalho randomizado, permitindo adoção de protocolo.

OBJECTIVES: To study demographyc caractheristics, morbidity and mortality associated to the procedure. METHODS: Retrospective study of colostomy closure in 42 patients. Male sex was predominant (80,9%) with median age of 42 years. Causes of colostomy were traumatic in 50% and clinic acute abdomen in 29% of the cases. Terminal colostomy was the more frequent procedure (65%) followed by loop colostomy in 35% of the cases. The predominant anastomotic technique was manual (69,05%). The morbidity rate was 26,2%, including reoperation (9,52%) and colonic fistulae in 7,14% cases. No postoperative wound infection was observed. Overall mortality rate was 2,38%. CONCLUSION: The results observed in a university hospital are as good as reported in world literature. Adequate pre and postoperative care works together with surgeon experience on colostomy closure surgeries. The surgical technique must be adopted using a randomized study, allowing establishment of a protocol.

Cholangiocarcinoma of the hepatic hilum is a seldom curable lesion. Those patients are often managed with prostheses. Cholangiojejunostomy on the third segment of the liver seems to offer effective biliary drainage with better quality of life compared to other forms of palliation. The aim of this paper is to report a successful case of a patient with irressectable biliary tumor submitted to a surgical biliary drainage.

OBJETIVO: Determinar as características comuns referentes ao mecanismo de trauma, lesões anatômicas e fisiológicas, dor abdominal e à fratura de costelas das vítimas de trauma hepático contuso, no momento da admissão hospitalar. MÉTODO: Revisão dos prontuários de dezembro de 1999 a janeiro de 2003, e selecionados 524 através do Software TNVT Plus (versão 2.0.0.213 - ano 1996), admitidos no Serviço de Emergência, vítimas de trauma tóraco-abdominal contuso e posteriormente internados, que apresentavam hepatectomia parcial, hepatorrafia, laparotomia exploradora, redução de fratura de costelas ou tratamento conservador de fratura de costelas. Foram excluídos aqueles que apresentavam trauma penetrante, que foram a óbito na sala de emergência e os menores de 16 anos, restando 200 prontuários. RESULTADOS: Considerando todos os traumas hepáticos, 37,63% foram causados por acidentes com veículo automotor, sendo que os traumas por esse mecanismo provocaram 54,29% das lesões mais graves (grau III, IV e V). Estas lesões causaram dor abdominal em 75,86% dos casos, e as mais leves (grau I e II), em 69,70%. A freqüência cardíaca e a pressão arterial sistólica média dos pacientes com lesões grau I ou II, foram, respectivamente, 100,37 batimentos por minuto e 114,45 mmHg, e naqueles com lesões grau III, IV, ou V, foram 105,69 batimentos por minuto e 107,88 mmHg, respectivamente. A fratura de costela à direita associou-se à lesão hepática grau I ou II em 29,41% dos casos. CONCLUSÃO: Acidentes por veículo automotor foram a causa mais comum de trauma hepático e o mecanismo de trauma que provocou as lesões mais graves. Estas lesões causaram dor abdominal com a mesma freqüência que as mais leves. A freqüência cardíaca e a pressão arterial sistólica média não possuíram relação com o grau da lesão hepática e a fratura de costelas à direita influenciou apenas na presença da lesão hepática.

BACKGROUND: To determine during hospital admissions, the trauma mechanism, anatomical and physiological injuries, abdominal pain and rib fractures of hepatic trauma victims. METHODS: Records from December/1999 to January/2003 were reviewed and 524 were selected by the Software TNVT Plus (version 2.0.0.213 - year 1996), from patients admitted to the Emergency Service, victims of blunt thoracic-abdominal trauma and submitted partial hepatectomy, hepatorrhaphy, laparotomy, reduction or conservative treatment of rib fractures. Those records which didn't present blunt trauma, were excluded, as well as the ones who deceased in the emergency room and those under 16 years old. Therefore, 200 records were analysed. RESULTS: Regarding all liver traumas, 37.63% were caused by motor vehicle accidents,responsible for 54.29% of the most serious injuries (degrees III, IV and V). These injuries caused abdominal pain in 75.86% of the cases, and minor injuries (degrees I and II) caused abdominal pain, in 69.70%. The heart rate and the mean systolic arterial pressure with degree I or II injuries were, respectively, 100.37 beats per minute and 114.45 mmHg; and in those with degree III, IV, or V injuries were 105.69 beats per minute and 107.88 mmHg, respectively. Right rib fracture was associated to degrees I or II injuries in 29.41% of the cases. CONCLUSION: Motor vehicle accidents are the most common cause of hepatic trauma and is the main causes of the most serious injuries. These injuries caused abdominal pain with the same frequency that minor injuries. The heart rate and mean systolic arterial pressure showed no relation to degree of hepatic injury.
