ABSTRACT The SISBIOTA-BRASIL was a three-year multimillion-dollar research program of the Brazilian government to document plants and animals in endangered/understudied areas and biomes in Brazil. Distributional patterns and the historical events that generated them are extensively unknown regarding Brazilian fauna and flora. This deficiency hinders the development of conservation policies and the understanding of evolutionary processes. Conservation decisions depend on precise knowledge of the taxonomy and geographic distribution of species. Given such a premise, we proposed to research the diversity of Diptera of the Brazilian western arc of Amazon, Cerrado, and Pantanal in the states of Mato Grosso, Mato Grosso do Sul, and Rondônia. Three important biomes of the South American continent characterize these Brazilian states: Amazon forest, Cerrado (Brazilian Savannah), and Pantanal. Besides their ecological relevance, these biomes historically lack intensive entomological surveys. Therefore, they are much underrepresented in the Brazilian natural history collections and in the scientific literature, which is further aggravated by the fact that these areas are being exponentially and rapidly converted to commercial lands. Our project involved over 90 collaborators from 24 different Brazilian institutions and one from Colombia among researchers, postdocs, graduate and undergraduate students, and technicians. We processed and analyzed nearly 300,000 specimens from ~60 families of Diptera collected with a large variety of methods in the sampled areas. Here, we provide a detailed overview of the genera and species diversity of 41 families treated. Our results point to a total of 2,130 species and 514 genera compiled and identified for the three states altogether, with an increase of 41% and 29% in the numbers of species and genera known for the three states combined, respectively. Overall, the 10 most species-rich families were Tachinidae, Cecidomyiidae, Tabanidae, Psychodidae, Sarcophagidae, Stratiomyidae, Bombyliidae, Syrphidae, Tephritidae, and Asilidae. The 10 most diverse in the number of genera were Tachinidae, Stratiomyidae, Asilidae, Mycetophilidae, Syrphidae, Tabanidae, Muscidae, Dolichopodidae, Sarcophagidae, and Chloropidae. So far, 111 scientific papers were published regarding taxonomic, phylogenetic, and biogeographical aspects of the studied families, with the description of 101 new species and three new genera. We expect that additional publications will result from this investigation because several specimens are now curated and being researched by specialists.

This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.
nbsp;
Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

RESULTS OF THE SURGICAL TREATMENT OF ARTICULAR SCAPULAR FRACTURES

RESUMO Introdução: As fraturas de escápula são lesões raras e muitas vezes associadas a traumas de alta energia, sendo que as fraturas articulares correspondem a apenas 15% de todas fraturas escapulares. O tratamento cirúrgico é indicado para fraturas com grandes desvios e com instabilidade articular. Objetivo: Avaliar os resultados clínicos e funcionais após o tratamento cirúrgico das fraturas de escápula. Métodos: Oito pacientes com fraturas da escápula foram tratados cirurgicamente entre o período de 2013 e 2019. Para indicação do tratamento cirúrgico, levou-se em consideração os desvios mediolaterais, ângulo glenopolar, desvios angulares e desvio articular maior que 4 mm. Resultados radiográficos de consolidação, arco de movimento, escore funcional e escala visual analógica de dor foram obtidos. Resultados: No seguimento médio de 29 meses (13-40 meses), todos os pacientes apresentaram consolidação da fratura. A média do escore UCLA foi de 29 pontos (sendo 75% de resultados bons e 25% de resultados moderados). Com relação ao arco de movimento, a elevação média foi de 146° (variando de 110° a 160°), rotação lateral de 62° (36-80°) e rotação medial no nível de T7 (T6-T10). A média final do EVA foi de 2,3. Todos os pacientes retornaram ao nível pré-lesão de trabalho. Conclusão: Nesta série de casos, o tratamento cirúrgico das fraturas da escápula com envolvimento articular proporcionou resultados satisfatórios com baixa taxas de complicações, mostrando ser uma opção em casos selecionados de fraturas desviadas. ABSTRACT Introduction: Scapular fractures are rare injuries and are often associated with high-energy trauma, with joint fractures accounting for only 15% of all scapular fractures. Surgical treatment is indicated for fractures with large deviations and with joint instability. Objective: This study evaluates the clinical and functional results after surgical treatment of scapular fractures. Methods: Eight patients with scapular fractures were surgically treated between 2013 and 2019. For indication for surgical treatment, mediolateral deviations, glenopolar angle, angular deviations and joint deviations greater than 4 mm were taken into account. Radiographic results of consolidation, range of motion, functional score and visual analogue pain scale were obtained. Results: In the mean follow-up of twenty-nine months (13-40 months), all patients presented fracture consolidation. The mean UCLA score was 29 points (with 75% good results and 25% moderate results). Regarding the range of motion, the mean elevation was 146° (ranging from 110° to 60°), lateral rotation of 62° (36-80°) and medial rotation at the level of T7 (T6-T10). The final VAS mean was 2.3. All patients returned to the pre-injury level at work. Conclusion: In this series of cases, surgical treatment of scapular articular fractures provided satisfactory results with low rates of complications, showing to be an option in selected cases of deviated fractures. Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

OBJECTIVES: To compare demographic/clinical/laboratory/treatments and outcomes among children and adolescents with laboratory-confirmed coronavirus disease 2019 (COVID-19). METHODS: This was a cross-sectional study that included patients diagnosed with pediatric COVID-19 (aged <18 years) between April 11, 2020 and April 22, 2021. During this period, 102/5,951 (1.7%) of all admissions occurred in neonates, children, and adolescents. Furthermore, 3,962 severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) detection samples were processed in patients aged <18 years, and laboratory-confirmed COVID-19 occurred in 155 (4%) inpatients and outpatients. Six/155 pediatric patients were excluded from the study. Therefore, the final group included 149 children and adolescents (n=97 inpatients and 52 outpatients) with positive SARS-CoV-2 results. RESULTS: The frequencies of sore throat, anosmia, dysgeusia, headache, myalgia, nausea, lymphopenia, pre-existing chronic conditions, immunosuppressive conditions, and autoimmune diseases were significantly reduced in children and adolescents (p<0.05). Likewise, the frequencies of enoxaparin use (p=0.037), current immunosuppressant use (p=0.008), vasoactive agents (p=0.045), arterial hypotension (p<0.001), and shock (p=0.024) were significantly lower in children than in adolescents. Logistic regression analysis showed that adolescents with laboratory-confirmed COVID-19 had increased odds ratios (ORs) for sore throat (OR 13.054; 95% confidence interval [CI] 2.750-61.977; p=0.001), nausea (OR 8.875; 95% CI 1.660-47.446; p=0.011), and lymphopenia (OR 3.575; 95% CI 1.355-9.430; p=0.010), but also had less hospitalizations (OR 0.355; 95% CI 0.138-0.916; p=0.032). The additional logistic regression analysis on patients with preexisting chronic conditions (n=108) showed that death as an outcome was significantly associated with pediatric severe acute respiratory syndrome (SARS) (OR 22.300; 95% CI 2.341-212.421; p=0.007) and multisystem inflammatory syndrome in children (MIS-C) (OR 11.261; 95% CI 1.189-106. 581; p=0.035). CONCLUSIONS: Half of the laboratory-confirmed COVID-19 cases occurred in adolescents. Individuals belonging to this age group had an acute systemic involvement of SARS-CoV-2 infection. Pediatric SARS and MIS-C were the most important factors associated with the mortality rate in pediatric chronic conditions with COVID-19.

Abstract Companies producing lead-acid batteries are required by law to recycle their battery scrap. The recycling can be performed by a pyrometallurgical process resulting in slags containing mainly lead and iron. A study was carried out with the aim of recovering the lead contained in this slag. This work has economic and environmental implications. Slag characterization was performed by X-ray diffraction, X-ray fluorescence and scanning electron microscopy (SEM). A laboratory-scale mechanical cell was used for flotation tests. The results showed that Pb-Fe selectivity is possible using ethyl xanthate as collector. The coke floats together decreasing concentrate grade. However, this is no problem because coke participates in the metallurgical process for the production of lead. A concentrate containing 22.9% Pb was obtained from a slag with 2.2% Pb. The process removes more than 99.6% of the iron contained. However, the recovery was only 19.0 %. Analyses from scanning electron microscopy detected the presence of lead inside the iron particles, limiting the possibility of lead recovery.

Indivíduos com claudicação intermitente (CI) comumente apresentam comorbidades associadas e hábitos não saudáveis, como o tabagismo e a inatividade física. Todavia, o quanto esses fatores contribuem para a intolerância à caminhada desses indivíduos permanece incerto. Assim, o objetivo deste estudo foi analisar a associação das comorbidades e hábitos não saudáveis com a capacidade de caminhada de indivíduos com CI. A amostra foi composta por 66 indivíduos com CI de ambos os gêneros. Todos os indivíduos realizaram teste ergométrico máximo em esteira, sendo registrada o tempo de claudicação (TC) e o tempo total de caminhada (TTC). Os dados referentes às comorbidades e hábitos não saudáveis foram obtidos por meio de entrevista. Para análise estatística foi empregada análise de regressão simples univariada, com p < 0,05. A comorbidade mais prevalente foi a hipertensão arterial (86,4%), ao passo que a inatividade física foi o hábito não saudável mais prevalente (47%). Além disso, 90,9% dos pacientes apresentaram duas ou mais comorbidades e/ou hábitos não saudáveis. A inatividade física foi associada com menor TC (β = -154 s; p = 0,02; IC95% = -283; -25 s) e TTC (β = -189 s; p = 0,02; IC95% = -384; -29 s). Além disso, o maior número de comorbidades e hábitos não saudáveis também foram associados com menor TC (β = -55 s; p = 0,02; IC95% = -102; -8 s). Como conclusão, os resultados deste estudo indicam que a inatividade física e o número de comorbidades e/ou hábitos não saudáveis estão associados com a intolerância à caminhada de indivíduos com CI.
Comorbid conditions and unhealthy habits, such as smoking and physical inactivity, are commonly observed in patients with intermittent claudication (IC). However, the impact of these factors on walking impairment in IC patients remains uncertain. Thus, the purpose of this study was to analyze the association between comorbid conditions and non-healthy habits and walking capacity in subjects with IC. Sixty-six men and women with IC were recruited. Subjects underwent a treadmill test in which claudication time (CT) and total walking time (TWT) were obtained. An interview was performed and comorbid conditions and unhealthy habits were obtained. Univariate linear regression was employed for statistical analysis (p < 0.05). The most prevalent comorbid condition was hypertension (86.4%). Among unhealthy habits physical inactivity was the most prevalent (47.0%). Moreover, 90.9% of the subjects had 2 or more comorbid conditions and/or unhealthy habits. Physical inactivity was associated with decreased CT (β = -154 s; p = 0.02; IC95% = -283;-25 s) and TWT (β = -189 s; p = 0.02; IC95% = -384; -29 s). Furthermore, a higher number of factors were associated with lower CT (β = -55 s; p = 0.02; IC95% = -102; -8 s). In conclusion, the results showed that physical inactivity and number of comorbid condition and/or unhealthy habits are associated to walking impairment in subjects with IC

Somatotropina bovina recombinante (rBST) nos aspectos hematológicos e metabólitos do sangue de novilhas (½ Nelore x ½ Red Angus) em confinamento

O objetivo deste trabalho foi avaliar o efeito da somatotropina bovina recombinante (rBST - análogo do BST, obtido comercialmente pela técnica do DNA recombinante), sobre os aspectos hematológicos (hematócrito, eritrócitos, hemoglobina, leucócitos, neutrófilos, eosinófilos, linfócitos e monócitos) e metabólitos (glicose, insulina, IGF-I, triglicérides, colesterol total e uréia) do sangue de novilhas confinadas. Foram utilizadas 24 novilhas mestiças (½ Nelore x ½ Red Angus), com aproximadamente 18 meses de idade e peso médio 255 kg. Os animais foram alimentados com uma dieta contendo silagem de milho como volumoso e polpa de citrus peletizada e farelo de soja, como concentrado, durante 84 dias. Essa dieta foi utilizada para os três tratamentos, que se diferenciaram pela aplicação de 250 mg de rBST, por via subcutânea, na fossa ísqueo-retal, onde: 1) controle (aplicação de dois mL de solução salina); 2) dose única e 3) uma dose a cada 14 dias. Os animais foram distribuídos em delineamento inteiramente casualizado, com três tratamentos e oito repetições. Coleta de sangue foi realizada no início do experimento e nos dias 28, 56 e 84 do experimento, para as determinações dos aspectos hematológicos e metabólitos. Não houve influência da aplicação de rBST sobre os aspectos hematológicos do sangue no início e final do experimento. Todavia, independentemente do tratamento, o dia de coleta teve um efeito linear positivo sobre os níveis de hematócrito, linear negativo sobre os níveis de glicose e triglicérides e quadrático positivo sobre os níveis de IGF-I e uréia. Ainda, não foi observado efeito do dia de coleta sobre os níveis de insulina e colesterol total.
This work was carried out to evaluate the recombinant bovine somatotropin effect (rBST - BST analogous, commercially obtained by recombinant DNA technique), on hematological aspects (hematocrit, erythrocyte, hemoglobin, leukocyte, neutrophil, eosinophil, lymphocyte and monocyte) and metabolites (glucose, insulin, IGF-I, triglycerides, total cholesterol and urea) in heifers blood. Twenty-four crossbred heifers were used (½ Nellore x ½ Red Angus), they were approximately 18 months old and 225 kg of average body weight. All animals were fed with corn silage, citrus pulp and soybean meal during 84 days. 250 mg of rBST were administered subcutaneously, by intradermic way in the ischiorectal fossa. Treatments were: 1) control (administration of two mL of saline solution); 2) Single dose and 3) one dose each 14 days. The animals were used in a completely randomized design, with three treatments and eight replicates. The blood collect was realized in the beginning of the experiment and in the days 28, 56 and 84 of the experiment to determine the hematological aspects and metabolites. There was no influence of the rBST treatment on hematological aspects in the beginning and at the end of the experiment. However, the collect day had a positive linear effect on the hematocrit levels, negative linear on the glucose and triglycerides levels and positive quadratic on the IGF-I and urea levels. Also, the collect day effect was not observed on the insulin and total cholesterol levels.

OBJETIVO: Este trabalho relata a experiência no manuseio da crise hipertensiva pulmonar (CHP) refratária, com o uso do óxido nítrico inalatório (NOi) no pós-operatório imediato do transplante cardíaco (TC) ortotópico. MÉTODOS: De outubro/1997 a fevereiro/2002 foram realizados 31 TC em adultos, sendo incluídos pacientes com RVP < 6uW, ou < 2,5uW na prova farmacológica. Após o período de circulação extracorpórea (CEC) (M=101±21 minutos), todos fizeram uso de dobutamina, dopamina e milrinona; entretanto, frente à ausência de resposta adequada e CHP, administrou-se NOi em doses crescentes de 20 a 40ppm. Foram utilizados registros das pressões através de cateter no átrio esquerdo e no tronco da artéria pulmonar, gasometria arterial seriada e ecocardiograma transtorácico (ETT). O tempo médio de morte encefálica (ME) do doador foi de 16<FONT FACE=Symbol>±5,1</FONT>horas<FONT FACE=Symbol>.</FONT>RESULTADOS: Cinco pacientes (1 mulher), com idade média de 42 anos, fizeram uso de NOi por apresentarem critérios de CHP, todos com sinais de baixo débito cardíaco. O tempo médio de CEC foi de 150,8±34,3 minutos, a média sistólica arterial pulmonar foi de 87mmHg (75-115) e a PO2 média de 60mmHg (FiO2 100%), sendo evidenciada dilatação moderada a severa do ventrículo direito no ETT. Foi administrado NOi durante uma média 35h (6-96), iniciando-se desmame após estabilização hemodinâmica. O tempo médio de ME do doador foi de 27,6±5,5horas, a internação hospitalar média foi de 63 dias (17-145), uma morte ocorreu por sepse no 17º PO e os demais pacientes estão em CF I (NYHA). CONCLUSÕES: O tempo de CEC superior a 120 minutos e tempo de ME do doador superior a 20 horas sugerem fatores de risco para CHP no período pós-operatório imediato do TC. O NOi é uma ferramenta útil no manuseio desta complicação, refratária ao tratamento convencional no POI de pacientes submetidos ao TC.
OBJECTIVE: This study reviews our experience with the use of nitric oxide inhalation (NOi) in the treatment of the refractory pulmonary hypertensive crisis (PHC) during the immediate post-operative period (IPOP) of heart transplantation (HT). METHOD: Between Oct/97 and Feb/02, 31 HT in adults were performed. Among them were patients with Pulmonary Vascular Resistance (PVR) greater than 6 uW, or patients with PVR greater than 2,5 uW during the pharmacological test. All patients were weaned from the CPB (M=101±21 minutes) in use of dobutamine, dopamine and milrinone. In the absence of an adequate response to these drugs and with the evidence of PHC, NOi was given, beginning with 20ppm and increasing, when necessary, up to 40ppm. The recording of the left atrium and pulmonary artery pressures, serial arterial blood gases samples and trans-thoracic echocardiograms (TTE) were performed. The donor's brain death (BD) time mean was 16±5,1 hours. RESULTS: In five patients (1 female) (mean age = 42 years) NOi was employed, due to low cardiac output signs and evidence of PHC and right ventricular dysfunction. The mean CPB time was 150,8±34,3 min. The pre-NOi mean PAP was 87 mmHg (range 75-115), and the mean arterial PO2 = 60mmHg (FiO2=100%). The TTE showed moderate-severe dilation of the right ventricle. NOi was given during a mean of 35h (range 6 to 96), being weaned only after hemodynamic stability was achieved. There was only one early death, at the 17th post op. day due to sepsis. All the remaining patients are in NYHA class I. CONCLUSIONS: The prolonged CPB time (> 120 min) and the time of brain death of the donor are factors that increase the risk of PHC. The NOi is a useful tool in dealing with this complication when there is an inadequate response to the conventional treatment.
