ABSTRACT The expression ‘you need to know to conserve’ is a well-known cliche among biologists. Documenting the richness of a group of organisms is the first step towards understanding biodiversity and preparing efficient conservation plans. In this context, many efforts have been made to quantify the number of species on Earth and estimate the number of species still unknown to science. A few countries have complete and integrated databases estimating the approximate number of species recorded for their territory, particularly in the Global South. In Brazil, a country of continental dimensions, revealing the richness of the second most diverse clade of invertebrates (=Mollusca) has been a goal of taxonomists. Recently, in an unprecedented, collective, and integrated effort among Brazilian malacologists, it was possible to estimate how many valid species of molluscs are there in Brazil. In this effort, more than 30 mollusc experts joined together to update the Taxonomic Catalogue of the Brazilian Fauna (TCBF), a governmental website that allows a quick and real-time updating of all Metazoan. So far, more than 5,000 updates have been made in TCBF, indicating the presence of 3,552 valid species of molluscs in Brazil, distributed among the main clades as follows: Caudofoveata (10 spp.), Solenogastres (6 spp.), Polyplacophora (35 spp.), Scaphopoda (43 spp.), Cephalopoda (92 spp.), Bivalvia (629 spp.) and Gastropoda (2,737 spp.). The present study, in addition to demonstrating for the first time the richness of Brazilian molluscs, also presents the state of the art of this important phylum of invertebrates highlighting its most representative and neglected groups.

Abstract Individuals with malignancies and COVID-19 have a lower survival compared with the general population. However, the information about the impact of COVID-19 on the whole hematological population is scarce. We aimed to describe the 30th day overall survival (OS) after COVID-19 infection in pa tients with a hematological disease in Argentina. A completely anonymous survey from the Argentine Society of Hematology was delivered to all the hematologists in Argentina; it started in April 2020. A cut-off to analyze the data was performed in December 2020 and, finally, 419 patients were reported and suitable for the analysis (average age: 58 years, 90% with malignant diseases). After the COVID-19 diagnosis, the 30-day OS for the whole population was 80.2%. From the entire group (419), 101 (24.1%) individuals required intensive care unit admission, where the 30-day OS was 46.6%. Among allogeneic stem cell transplant recipients, the 30-day OS was 70.3%. Factors associated with a low OS were two or more comorbidities, an active hematological disease and history of chemotherapy. In individuals with the three factors, the 30-day OS was 49.6% while the 30-day OS in those without those factors was 100%. Patients with hematological diseases have a higher mortality than the general population. This group represents a challenge and requires careful decision-making of the treatment in order not to compromise the chances of cure.
Resumen El presente estudio tuvo por objetivo primario conocer la mortalidad de pacientes con enfermedad hematológica que presentaron infección por COVID-19 en Argentina. Para ello se difundió una encuesta desde la Sociedad Argentina de Hematología (SAH) entre los hematológos para informar sobre los pacientes con enfermedades hematológicas y diagnóstico de infección por SARS- CoV-2, entre el 19/4/2020, y el 7/12/2020. Se incluyeron individuos de todas las edades con diagnóstico de enfermedad hematológica benigna o maligna e infección por SARS-CoV-2 confirmada por técnica de RT-PCR. Se analizaron 419 pacientes (mediana 58 años; 90% enfermedades malignas). La supervivencia al día 30 fue de 80.2%. La supervivencia fue menor en aquellos que requirieron internación (74.2%), cuidados intensivos (46.6%) y asistencia respiratoria mecánica (36.8%). Entre los trasplantados alogénicos la supervivencia fue 70.3%. Los factores vinculados a la supervivencia global fueron las comorbilidades, el estado de la enfermedad al momento de la infección y el antecedente de quimioterapia. Se pudo establecer un score en el que aquellos que tuvieron un puntaje de 4 alcanzaron una supervivencia del 49.6% al día 30, mientras que la de los pacientes con score 0 fue del 100% a 30 días. En comparación con la población general, los pacientes con enfermedades hematológicas presentan una mayor mortalidad vinculada al COVID-19, motivo por el cual es primordial definir pautas destinadas a disminuir la exposición de los mismos sin comprometer las posibilidades de beneficiarse del tratamiento de la enfermedad de base.

OBJECTIVE: Coronavirus disease 2019 (COVID-19) is associated with high mortality among hospitalized patients and incurs high costs. Severe acute respiratory syndrome coronavirus 2 infection can trigger both inflammatory and thrombotic processes, and these complications can lead to a poorer prognosis. This study aimed to evaluate the association and temporal trends of D-dimer and C-reactive protein (CRP) levels with the incidence of venous thromboembolism (VTE), hospital mortality, and costs among inpatients with COVID-19. METHODS: Data were extracted from electronic patient records and laboratory databases. Crude and adjusted associations for age, sex, number of comorbidities, Sequential Organ Failure Assessment score at admission, and D-dimer or CRP logistic regression models were used to evaluate associations. RESULTS: Between March and June 2020, COVID-19 was documented in 3,254 inpatients. The D-dimer level ≥4,000 ng/mL fibrinogen equivalent unit (FEU) mortality odds ratio (OR) was 4.48 (adjusted OR: 1.97). The CRP level ≥220 mg/dL OR for death was 7.73 (adjusted OR: 3.93). The D-dimer level ≥4,000 ng/mL FEU VTE OR was 3.96 (adjusted OR: 3.26). The CRP level ≥220 mg/dL OR for VTE was 2.71 (adjusted OR: 1.92). All these analyses were statistically significant (p<0.001). Stratified hospital costs demonstrated a dose-response pattern. Adjusted D-dimer and CRP levels were associated with higher mortality and doubled hospital costs. In the first week, elevated D-dimer levels predicted VTE occurrence and systemic inflammatory harm, while CRP was a hospital mortality predictor. CONCLUSION: D-dimer and CRP levels were associated with higher hospital mortality and a higher incidence of VTE. D-dimer was more strongly associated with VTE, although its discriminative ability was poor, while CRP was a stronger predictor of hospital mortality. Their use outside the usual indications should not be modified and should be discouraged.

Yeasts isolated from tropical fruit ice creams: diversity, antifungal susceptibility and adherence to buccal epithelial cells

Resumo Os sorvetes à base de frutas são produtos amplamente consumidos em países tropicais e, devido à sua composição, podem ser uma boa fonte de crescimento microbiano, incluindo patógenos oportunistas. Os objetivos deste estudo foram caracterizar as populações de leveduras presentes nos sorvetes brasileiros à base de frutas e investigar sua susceptibilidade aos antifúngicos anfotericina B, fluconazol e itraconazol, e a capacidade dos isolados, que puderam crescer a 37 °C, de aderir em células epiteliais bucais. Duzentos e sessenta e sete isolados de levedura obtidos das amostras de sorvete foram identificados como pertencentes a 29 espécies, com contagens variando de 1,5 a 5,2 log UFC/mL. As espécies predominantes foram Candida intermedia, Torulaspora delbrueckii, C. parapsilosis, Clavispora lusitaniae, Saccharomyces cerevisiae e Pichia kudriavzevii. Pelo menos 16 espécies de leveduras isoladas neste estudo foram relatadas como agentes patogênicos oportunistas. Quarenta e um isolados de leveduras apresentaram resistência ou susceptibilidade dose-dependente a pelo menos um dos antifúngicos testados. Um isolado de C. parapsilosis foi resistente a todos os antifúngicos testados e mostrou capacidade de aderir a células epiteliais bucais. A porcentagem de adesão foi alta principalmente para isolados de P. kudriavzevii, Meyerozyma guilliermondii, C. parapsilosis, S. cerevisiae e Debaromyces hansenii. Os dados sugerem que a presença dessas leveduras oportunistas como contaminantes nos sorvetes pode representar um risco para o consumidor final, especialmente para indivíduos imunocomprometidos que podem consumir esses produtos.
Abstract Fruit-based ice creams are products widely consumed in tropical countries and, because of their composition, can be a good source for microbial growth, including opportunistic pathogens. The aims of this study were to characterize the yeast populations present in Brazilian fruit-based ice creams, and to investigate the antifungal susceptibility to amphotericin B, fluconazole and itraconazole, and the ability of the isolates that were able to grow at 37 ºC to adhere to buccal epithelial cells (BEC). Two hundred and sixty-seven yeast isolates obtained from the ice cream samples were identified as belonging to 29 species, with counts that ranged from 1.5 to 5.2 log CFU/mL. The predominant species were Candida intermedia, Torulaspora delbrueckii, C. parapsilosis, Clavispora lusitaniae, Saccharomyces cerevisiae and Pichia kudriavzevii. At least 16 yeast species isolated in this study have been reported as opportunistic pathogens. Forty-one yeast isolates showed resistance or dose-dependent susceptibility to at least one of the antifungal drugs tested. One isolate of C. parapsilosis was resistant to all the antifungals tested and showed ability to adhere to BEC. The percentage of adhesion to BEC was high mainly for isolates of P. kudriavzevii, Meyerozyma guilliermondii, C. parapsilosis, S. cerevisiae and Debaromyces hansenii. The data suggest that the presence of these opportunistic yeasts as contaminants in ice creams may represent a potential risk to the final consumer, especially to immunocompromised individuals who may consume these products.

Resumo:OBJETIVO:descrever a auto-avaliação de otorrinolaringologistas e fonoaudiólogos do Distrito Federal acerca do conhecimento em relação ao implante coclear.MÉTODOS:estudo transversal e analítico com coleta de dados obtidos por meio da aplicação de questionários on-line. Participaram do estudo 73 fonoaudiólogos e 33 otorrinolaringologistas que responderam questões acerca do conhecimento do Implante Coclear na cidade de Brasília-Distrito Federal.RESULTADOS:31% (n=22) dos fonoaudiólogos acreditam ter conhecimento suficiente no que se refere ao procedimento cirúrgico, enquanto que no grupo dos otorrinolaringologistas esse número chegou a 59,4% (n=19), apresentando significância estatística entre os grupos (p<0,01). Ambos os grupos mostraram conhecimento insatisfatório em relação ao tipos de implante, mapeamentos, ativação, acompanhamento, custos, manutenção e o IC inserido no SUS.CONCLUSÃO:os profissionais otorrinolaringologistas e fonoaudiólogas pesquisados, atuantes no Distrito Federal, não apresentaram o conhecimento satisfatório acerca do implante coclear na análise geral dos domínios pesquisados.
Abstract:PURPOSE:to describe the self-assessment of Otolaryngologists, Audiologists, and Speech-Language Pathologists from the Federal District on their knowledge concerning cochlear implants.METHODS:cross-sectional and analytical study with data collection obtained by applying online questionnaires.73 Audiologists and Speech-Language Pathologists, and 33 Otolaryngologists participated in the study. They answered questions regarding their knowledge of Cochlear Implants in Brasília - Federal District, Brazil.RESULTS:31% (n=22) of the Audiologists and Speech-Language Pathologists believe they have sufficient knowledge regarding the surgical procedure, whereas in the group of otolaryngologists, the figure was 59.4% (n = 19). Statistical significance between groups is observed (p <0.01). Both groups showed poor knowledge regarding implant types, mappings, activation, monitoring, and maintenance costs and cochlear implantation performed by the SUS (Brazilian Unified Health System).CONCLUSION:Otolaryngologists, Audiologists, and Speech-Language Pathologists surveyed, who work in the Federal District, did not show satisfactory knowledge of the cochlear implant in the overall analysis of the areas surveyed.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

INTRODUÇÃO: O ultrassom intracoronário (USIC) é um método capaz de fornecer medida sensível e reprodutível das dimensões da artéria coronária, da placa aterosclerótica e do lúmen arterial. Avanços em sua tecnologia permitem agora a caracterização da composição e da morfologia das placas ateroscleróticas. Embora estudos prévios tenham reportado dados utilizando USIC com análise de radiofrequência, o uso de uma nova modalidade (iMap®, Boston Scientific, Santa Clara, Estados Unidos) de caracterização da placa aterosclerótica é muito pouco conhecida. Nosso objetivo será analisar as características morfológicas, teciduais e fenotípicas das placas ateroscleróticas consideradas angiograficamente "culpadas" e "não-culpadas" em pacientes submetidos a angiografia coronária decorrente de infarto agudo do miocárdio (IAM). MÉTODOS: Estudo prospectivo, transversal, em único centro (Hospital São Paulo - Escola Paulista de Medicina - Universidade Federal de São Paulo/UNIFESP, São Paulo, SP, Brasil). Serão selecionados 50 pacientes para análise ultrassonográfica, de acordo com os seguintes critérios de inclusão: idade < 75 anos, IAM sem supradesnivelamento do segmento ST ou IAM com supradesnivelamento do segmento ST recente, com ou sem uso de fibrinolítico prévio. CONCLUSÕES: O presente estudo objetivará a caracterização morfológica, tecidual e fenotípica da placa aterosclerótica utilizando uma nova modalidade de imagem ainda pouco estudada em pacientes com IAM.
BACKGROUND: Intravascular ultrasound (IVUS) provides a sensitive and reproducible measurement of coronary artery dimensions, atherosclerotic plaque, and arterial lumen. Advances in IVUS technology now allow the characterization of the composition and morphology of atherosclerotic plaques. Although previous studies have reported data using IVUS radiofrequency analysis, the use of a new modality (iMap®, Boston Scientific, Santa Clara, USA) of atherosclerotic plaque characterization is very little known. Our objective is to analyze the morphological, phenotypic and tissue characteristics of "culprit" and "non-culprit" atherosclerotic plaques determined by angiography in patients undergoing coronary angiography due to acute myocardial infarction (AMI). METHODS: Prospective, cross-sectional, single center study (Hospital São Paulo - Escola Paulista de Medicina - Universidade Federal de São Paulo/UNIFESP, São Paulo, SP, Brazil). Fifty patients will be selected for ultrasound analysis according to the following inclusion criteria: age < 75 years, non-ST-segment elevation AMI or recent ST-segment elevation AMI, with or without previous fribrinolytic agents. CONCLUSIONS: The present study is aimed at obtaining morphological, phenotypic and tissue characteristics of the atherosclerotic plate using an imaging modality not extensively evaluated in patients with AMI.

Sistema radicular de seis gramíneas irrigadas em diferentes adubações nitrogenadas e manejos

Objetivou-se avaliar a densidade de raiz e a profundidade efetiva do sistema radicular de seis gramíneas irrigadas sob efeito de diferentes adubações nitrogenadas e manejos. O experimento foi analisado em esquema de parcelas sub-subdivididas, tendo nas parcelas os manejos (pastejo e corte), nas subparpelas as gramíneas (Estrela, Marandu, Mombaça, Pioneiro, Tanzânia e Xaraés) e nas sub-subparcelas as doses nitrogenadas (100, 300, 500 e 700 kg ha-1 ano-1). A densidade de raiz foi obtida pela densidade média de todas as amostras, e a profundidade efetiva foi definida na profundidade onde se concentravam 80% de peso das raízes. Observou-se efeito das gramíneas, adubação nitrogenada e manejos nas características avaliadas. O capim-Marandu, seguido do Xaraés, apresentou maior densidade de raiz. No geral, os capins Marandu e Pioneiro proporcionaram maior e menor profundidade efetiva do sistema radicular, respectivamente. O manejo corte proporcionou maior densidade de raiz, porém não influenciou a profundidade efetiva. A adubação nitrogenada dependeu da gramínea e do manejo para conferir efeito nos parâmetros avaliados.
This study aimed to evaluate the root density and root depth of six irrigated grasses under different nitrogen fertilization and management. The experiment was analyzed in a split-split plot design. Two management (grazed and cut) constituted the plots, six grasses (Estrela, Marandu, Mombaça, Pioneiro, Tanzânia and Xaraes) the split-plots, and four nitrogen fertilization (100, 300, 500 and 700 kg ha-1 year-1) the split-split-plots. The root density was obtained by the medium density of all the samples and root depth was based on the first 80% root weight. Effects of the grasses, nitrogen fertilization and management were observed in the appraised characteristics. Marandu, followed by Xaraes, presented larger root density. In general, Marandu and Pioneiro provided larger and smaller root depth, respectively. The cut provided larger root density, even though it did not influence root depth. Nitrogen fertilization depended on the grass and management to have an effect in the parameters.

Sucesso no tratamento do zumbido com terapia em grupo

Embora os tratamentos em grupos para o zumbido sejam uma área bem documentada na literatura internacional, até onde sabemos nunca foram realizados no Brasil. Este estudo apresenta uma experiência bem-sucedida de um tratamento em grupo para zumbido baseado na terapia de retreinamento do zumbido aliada com as técnicas cognitivo-comportamentais. O objetivo deste estudo é avaliar pacientes com zumbido antes e depois do tratamento em grupo estruturado baseado da terapia de retreinamento do zumbido e na terapia cognitivo comportamental. MATERIAL E MÉTODO: Estudo coorte prospectivo: 27 sujeitos aplicaram-se para a pesquisa. Responderam ao inventário do Handicap do Zumbido (THI) e à escala da Ansiedade e Depressão Hospitalar (HAD),antes e depois do tratamento. Foram realizadas 6 sessões estruturadas segundo os princípios da terapia de retreinamento do zumbido associada com técnicas cognitivo-comportamentais. RESULTADOS: 27 pacientes iniciaram e 19 terminaram o tratamento (8 excluídos). 47,4% homens, média das idades 47.6. O resultado do THI antes e depois do tratamento foi respectivamente: funcionais, 29 e 14; emocional, 24 e 10; e catastrófico, 12 e 5; a escala HAD: ansiedade, 12 e 9 e depressão, 10 e 6. CONCLUSÃO: O tratamento descrito é efetivo na melhora do zumbido.
Although group therapy for tinnitus is a well documented field in the international literature, as far as we know it has never been held in Brazil. This study represents a successful experience of a group therapy for tinnitus based on the tinnitus retraining therapy associated with behavioral cognitive techniques. The goal of the present study is to assess patients with tinnitus before and after the group therapy based on the tinnitus retraining therapy and the behavioral cognitive therapy. MATERIALS AND METHODS: Prospective cohort study: 27 subjects signed in for the study, and answered the Tinnitus Handicap Inventory (THI) and the Hospital Depression and Anxiety Scale (HAD), before and after the treatment. We held six structured sessions according to the principles of tinnitus retraining therapy associated with behavioral cognitive techniques. RESULTS: 27 patients started and 19 finished the treatment (8 were taken off). 47.4% men, mean age of 47.6. THI results before and after treatment were respectively: functional: 29 and 14, emotional 24 and 10 and catastrophic 12 and 5 and the HAD scale: anxiety 2 and 9 and depression 10 and 6. CONCLUSION: The treatment described is effective in improving tinnitus symptoms.

Lâminas de irrigação e estações anuais na cobertura do solo e altura de gramíneas cultivadas sob corte

Objetivou-se avaliar a cobertura do solo e altura de planta em seis gramíneas manejadas por corte sob efeito de diferentes lâminas de irrigação e estações do ano. O experimento foi conduzido em esquema de parcelas subsubdivididas, tendo seis gramíneas (Xaraés, Mombaça, Tanzânia, Pioneiro, Marandu e Estrela) nas parcelas, seis lâminas de irrigação (0, 18, 45, 77, 100 e 120% da referência) nas subparcelas e as estações (outono/inverno e primavera/verão) nas subsubparcelas. Para diferenciar a aplicação das lâminas de irrigação, utilizou-se o sistema por aspersão em linha. Observou-se efeito das gramíneas, estações anuais e lâminas de irrigação nas duas características estudadas. O capim-Pioneiro apresentou a maior altura e o capim-Estrela a maior cobertura ao solo. A maior temperatura ocorrida nas estações primavera/verão proporcionou aumento na altura e não afetou a cobertura do solo. A cobertura do solo respondeu positivamente ao aumento da lâmina de irrigação. Nas estações outono/inverno, o aumento da lâmina de irrigação proporcionou aumento na altura; nas estações primavera/verão, não afetou ou reduziu a altura das gramíneas.
The study aimed to evaluate the soil cover and plant height of six grasses under different seasons and irrigation depths. The experiment was conducted in a completely randomized design, with two replications, in a split-split plot design. Six grasses (Xaraes, Mombaça, Tanzania, Pioneiro, Marandu and Estrela) constituted the plots, six irrigation depths (0, 18, 45, 77, 100 and 120% of the reference) the split-plots, and two seasons (autumn/winter and spring/summer) the split-split-plots. In order to vary the application of irrigation depths, a line source sprinkler system was used. The effect of the cultivars, annual seasons and irrigation depth in the two studied characteristics were analyzed. The Pioneiro grass presented the greatest height, while Estrela provided the largest soil cover. The higher temperature in the spring/summer season provided an increase in the height and did not affect soil cover. The soil cover answered positively to the increase in irrigation depth. During the autumn/winter season, the increase in irrigation depth provided an increase in height, and during the spring/summer season, and did not affect or reduce the height of the grasses.

Resumo Os modelos de estadiamento neuropatológico da doença de Alzheimer (DA) têm sido desenvolvidos nos últimos 20 anos. Entretanto, têm sido quase exclusivamente testados em caucasianos de ascendência norte-européia ou em asiáticos. Objetivos: verificar quais dos critérios neuropatológicos discrimina melhor entre a DA e o envelhecimento normal em uma casuística clinicopatológica brasileira bem caracterizada. Métodos: uma amostra aleatória de 89 casos do Banco Brasileiro de Encéfalos do Estudo de Envelhecimento Cerebral foi submetida à avaliação neuropatológica completa com imunohistoquímica. As condições clínicas e funcionais foram avaliadas mediante entrevista com informante confiável. Os escores na Clinical Dementia Rating Scale (CDR) foram comparados com os escores dos estágios de Braak e Braak, do CERAD (Consortium to Establish a Registry for Alzheimer's Disease) e do consórcio NIA-Reagan (National Institute of Aging-Reagan Institute). Casos com diagnósticos neuropatológicos diferentes de DA foram excluídos (n=27). Resultados: Os 62 casos foram classificados em: CDR0=39, CDR0,5=9, CDR³1=14. Não havia diferenças quanto a idade, gênero e escolaridade entre os grupos. Os escores no CERAD correlacionaram-se melhor com os do CDR (r=0,5303; p<0,001). Os escores nos estágios de Braak e Braak foram significativamente mais elevados nos casos com CDR mais altos. A correlação do CDR com os escores dos critérios NIA-Reagan foi parcialmente rompida porque grande proporção de casos não se enquadrava em nenhuma das categorias diagnósticas destes critérios. Conclusões: Nesta casuística, os critérios do CERAD correlacionam-se melhor com os do CDR. Como observado por outros estudos, alguns casos de indivíduos cognitivamente normais, preencheram critérios neuropatológicos para o diagnóstico de DA.
Abstract The current neuropathological staging models of Alzheimer's disease (AD) have been developed within the last 20 years. Nevertheless, they were mostly tested on Caucasians of Northern European ancestry or on Asians. Objective: To verify which of the accepted neuropathologic criteria best discriminates AD from normal aging in a well characterized Brazilian clinicopathological series. Methods: A random sample consisting of 89 subjects belonging to the Brazilian Brain Bank of the Aging Brain Study were clinically and neuropathologically fully assessed using immunohistochemistry. Clinical and functional statuses were assessed by interviewing a reliable informant. The Clinical dementia rating scale (CDR) was compared to Braak and Braak stage, the consortium to establish a registry for Alzheimer's disease (CERAD) score and NIA-Reagan (National Institute of Aging - Reagan Institute) score. Subjects with a neuropathologic diagnosis other then AD were excluded (n=27). Results: The CDR score distribution for the 62 selected subjects was as follows: CDR0=39, CDR0.5=9, CDR³1=14. There were no differences regarding age, gender and education among the groups. CDR score correlated best with the CERAD score (r=0.5303; p<0.001) . Braak and Braak stage was significantly higher in subjects with higher CDR. Correlation of the NIA-Reagan criteria was partially disrupted because a large proportion of subjects did not fit any of its categories. Conclusions: In this series, CERAD criteria better correlated with the CDR groups. Consistent with earlier studies, some cognitively normal subjects have AD neuropathological diagnosis.

CASUÍSTICA E METODOS: Entre janeiro de 1983 e maio de 1999, 12.405 pacientes com diagnóstico de infarto agudo do miocárdio (IAM) foram atendidos no InCor. Destes, 127 (1,02%) apresentaram rotura de parede livre do ventrículo esquerdo como complicação do IAM. A rotura miocárdica aguda ocorreu em 98 (77,1%) pacientes e a subaguda em 29 (22,9%). RESULTADOS: Foram operados 5 pacientes na forma aguda, com 80% de mortalidade e 19 na forma subaguda, com 15,8% de mortalidade. A sobrevida pós-operatória abrangendo os dois grupos foi de 70,8%. CONCLUSÃO: Concluímos que a rotura de parede livre do ventrículo esquerdo é uma grave complicação do IAM, necessitando de atuação imediata. Nas formas agudas, observa-se rápida deterioração hemodinâmica do paciente, muitas vezes sem tempo hábil para tentativa de correção cirúrgica. As formas subagudas podem ser detectadas e monitorizadas através de exames ecocardiográficos seriados após o IAM. Nestes casos a intervenção cirúrgica precoce, muitas vezes sem a necessidade de instalação de circulação extracorpórea, tem contribuído para a sobrevida da maioria dos pacientes.
MATERIAL AND METHODS: Between January 1983 and May 1999, 12.405 patients were treated by the surgical team of the Heart Institute (InCor) with the diagnosis of acute myocardial infarction (AMI). From these patients, 127 (1.02%) had left ventricular free wall rupture as an ischemic complication of the myocardial infarction. The cardiac rupture was acute in 98 patients (77.1%) and sub-acute in 29 (22.9%). RESULTS: Twenty-four patients were operated on, 5 on acute rupture with 80% of hospital mortality and 19 on sub-acute rupture with 15.8% of hospital mortality. The post-operative overall survival including both groups was 70.8%. CONCLUSION: The conclusion was drawn that left ventricular free wall rupture is a severe complication of acute myocardial infarction that needs an immediate action. In acute ruptures, most patients develop hemodynamic deterioration without enough time to try to proceed any surgical correction. The sub-acute cases can be detected and monitored through periodic ecocardiographic exams after the AMI. In these cases the early surgical intervention, many times without using extra-corporeal circulation, has been increasing the chances of survival of the majority of these patients.
