Genetic Syndromes Associated with Congenital Cardiac Defects and Ophthalmologic Changes - Systematization for Diagnosis in the Clinical Practice

Resumo Fundamento: O número de síndromes genéticas descritas que apresentam alguma forma de cardiopatia e manifestações oculares associadas é grande. Contudo, estas síndromes ainda não foram reunidas e sintetizadas para melhor consulta e comparação. Objetivo: O objetivo deste trabalho é sistematizar a literatura, avaliando evidências disponíveis sobre síndromes que cursam com cardiopatia congênita associada a alterações oculares, salientando os tipos de alterações anatômicas e funcionais descritas. Métodos: Dois pesquisadores independentes fizeram uma busca sistemática utilizando as bases eletrônicas Medline (PubMed, Embase, Cochrane, Lilacs), de trabalhos publicados até o mês de janeiro de 2016. Os critérios de elegibilidade utilizados pelos autores incluíram somente artigos publicados sob a forma de relatos de caso ou revisão, que abordassem a associação de alterações oftalmológicas e cardiológicas em pacientes menores de 18 anos e que apresentassem alguma síndrome genética. Resultados: As síndromes genéticas mais frequentes foram: Síndrome de Down, Síndrome Velo-cardio-facial / DiGeorge, Síndrome de Charge e Síndrome de Noonan. Entre as malformações cardíacas, a comunicação interatrial (77,4%), a comunicação interventricular (51.6%), a persistência do canal arterial (35,4%), estenose da artéria pulmonar (25,8%) e a tetralogia de Fallot (22,5%) foram as mais associadas com achados oculares. Conclusão: Devido à sua variedade clínica, as malformações cardíacas congênitas revelam defeitos que evoluem de maneira assintomática até aqueles que provocam grande morbimortalidade. Dessa forma, encontrar características extra-cardíacas que, de alguma maneira, possam auxiliar no diagnóstico da doença ou revelar a gravidade dessa enfermidade tornam-se de grande relevância.
Abstract Background: Numerous genetic syndromes associated with heart disease and ocular manifestations have been described. However, a compilation and a summarization of these syndromes for better consultation and comparison have not been performed yet. Objective: The objective of this work is to systematize available evidence in the literature on different syndromes that may cause congenital heart diseases associated with ocular changes, focusing on the types of anatomical and functional changes. Method: A systematic search was performed on Medline electronic databases (PubMed, Embase, Cochrane, Lilacs) of articles published until January 2016. Eligibility criteria were case reports or review articles that evaluated the association of ophthalmic and cardiac abnormalities in genetic syndrome patients younger than 18 years. Results: The most frequent genetic syndromes were: Down Syndrome, Velo-cardio-facial / DiGeorge Syndrome, Charge Syndrome and Noonan Syndrome. The most associated cardiac malformations with ocular findings were interatrial communication (77.4%), interventricular communication (51.6%), patent ductus arteriosus (35.4%), pulmonary artery stenosis (25.8%) and tetralogy of Fallot (22.5%). Conclusion: Due to their clinical variability, congenital cardiac malformations may progress asymptomatically to heart defects associated with high morbidity and mortality. For this reason, the identification of extra-cardiac characteristics that may somehow contribute to the diagnosis of the disease or reveal its severity is of great relevance.

Specimens of Leonotis nepetiflolia were obtained from cultivated and wild environments to verify their influences in chemical composition. Phytochemical analyses were conducted for the ethyl acetate phase obtained by partitioning the crude ethanol extract from the cultivated leaves of L. nepetifolia. In doing so, flavonoid 3',4',5-trimethoxy-6,7-dihidroxyflavone (cirsiliol), a chemotaxonomic marker of the family Lamiaceae, was isolated. The results reveal that all phases and extracts tested exhibited weak to moderate antimicrobial activity against the strains of bacteria tested. The evaluation of in vitrocytotoxic and antitumor activity showed that the ethyl acetate phases obtained from both wild and cultivated leaves exhibit high potential cytotoxic and antitumor (> 78.0% of inhibition) activity. The major component of these phases was identified by high-performance liquid chromatography-diode array detector and nuclear magnetic resonance analyses using both 1D and 2D methods. The results further indicate that the flavonoid cirsiliol is the agent responsible for the activity observed in these phases.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Perfil sanguíneo e peso de órgãos internos de bezerros leiteiros criados em diferentes sistemas de aleitamento

Objetivou-se avaliar o perfil sanguíneo e o tamanho absoluto e relativo dos órgãos internos de bezerros em sistema de aleitamento convencional ou fracionado. Foram utilizadas 10 fêmeas, com peso corporal inicial médio de 37,53 kg (±2,36) e 12 machos com peso corporal médio de 81,6 kg (±8,6), ambos da raça Holandesa. O aleitamento convencional constituiu-se de quatro litros de leite fornecidos diariamente durante 54 dias de avaliação e o fracionado, de seis litros do 6º ao 25º dias de idade, quatro litros do 26º ao 45º dias de idade e dois litros do 46º ao 59º dias de idade, além de concentrado, feno de Cynodon sp., água e suplemento mineral fornecidos ad libitum. O delineamento experimental foi inteiramente ao acaso, com cinco repetições para os parâmetros sanguíneos e seis repetições para o desenvolvimento dos órgãos internos. As coletas de sangue foram realizadas nas fêmeas nos dias sete, 21, 28, 42, 49 e 56 de idade. Para a avaliação dos órgãos internos os machos foram abatidos aos dois meses de idade, após jejum de 16 horas. Não foram observadas diferenças para os valores de proteínas totais, albumina, globulina, creatinina, ureia, colesterol e β-hidroxibutirato nas idades avaliadas (p>0,05). Entretanto, para a concentração de glicose houve interação entre os sistemas de aleitamento e os períodos avaliados, podendo ser observado que com 21 dias de idade a concentração foi maior nas bezerras mantidas em sistema fracionado, quando comparadas aos animais do grupo convencional. O sistema de aleitamento não alterou o peso corporal final, peso corporal final vazio e o peso dos órgãos internos, exceto para o coração, demonstrando que o sistema de aleitamento fracionado não altera as avaliações dos órgãos internos.
The objective was to evaluate the blood profile together with the relative and absolute size of the internal organs of calves in conventional or fractionated feeding system. Ten females were used, with an average initial body weight of 37.53 kg (±2.36), and 12 males with a mean body weight of 81.6 kg (±8.6), both Holstein breed. The conventional feeding consisted of four liter milk supplied daily for 54 days of evaluation, and fractioned six liter milk was given from 6 to 25 days of age, four liter from 26 to 45 days of ager, and two liter from 46 to 59 days of age. Cynodon sp. hay, water and mineral supplement was provided ad libitum. The experimental design was completely randomized with five replicates for blood parameters and six replicates for the development of internal organs. Blood collections were performed in females on days seven, 21, 28, 42, 49 and 56 of age. For the evaluation of internal organs males were slaughtered at two months of age, after 16 hours of fasting. No differences occurred in the values of total protein, albumin, globulin, creatinine, urea, cholesterol and β-hydroxybutyrate in ages evaluated (p>0.05). However, for the concentration of glucose no interaction was found between lactation systems and periods evaluated. It can be noted that at 21 days of age the concentration was higher in calves kept in the fractionated system, when compared with animals of the conventional group. The feeding system did not affect the final body weight, the empty body weight or the weight of the internal organs, except for the heart, demonstrating that the fractional feeding system does not alter the assessment of the internal organs.

O objetivo deste trabalho foi avaliar o efeito do óleo essencial de Eucalyptus globulus (OEEG) sobre a eclosão e desenvolvimento de larvas de Haemonchus contortus. A determinação da composição química do OEEG foi feita por meio de cromatografia gasosa acoplada à espectrometria de massa. No teste de eclosão de ovos (TEO), utilizaram-se as concentrações 21,75; 17,4; 8,7; 5,43 e 2,71 mg.mL-1; e no de desenvolvimento larvar (TDL) as concentrações foram 43,5; 21,75; 10,87; 5,43 e 2,71 mg.mL-1. Cada ensaio foi acompanhado por um controle positivo, 0,02 mg.mL-1 tiabendazol, no TEO e 0,008 mg.mL-1 ivermectina no TDL, e por um controle negativo com o diluente Tween 80 (3%). A eficácia máxima obtida pelo OEEG sobre ovos foi de 99,3% na concentração de 21,75 mg.mL-1 e, sobre larvas, 98,7% na concentração de 43,5 mg.mL-1. A concentração do OEEG, que inviabilizou 50% dos ovos e das larvas do parasito, foi de 8,3 e 6,92 mg.mL-1, respectivamente. A análise química do óleo identificou como principal componente o monoterpeno 1,8-cineol. O OEEG apresentou atividade ovicida e larvicida in vitro sobre H. contortus, determinando um bom potencial para utilização no controle de nematóides gastrintestinais de ovinos e caprinos.
The objective of this work was to evaluate ovicidal and larvicidal effects of Eucalyptus globulus essential oil (EGEO) on Haemonchus contortus. The chemical composition determination of EGEO was through gas chromatography and mass spectrometry. Egg hatch test (EHT) was performed in concentrations 21.75; 17.4; 8.7; 5.43 e 2.71 mg.mL-1. In larval development test (LDT) were used the concentrations 43.5; 21.75; 10.87; 5.43 e 2.71 mg.mL-1. Each trial was conducted by negative control with Tween 80 (3%) and positive control, 0.02 mg.mL -1 of thiabendazole in EHT and 0.008 mg.mL-1 of ivermectin in LDT. The maximum effectiveness of EGEO on eggs was 99.3% in concentration of 21.75 mg.mL-1 and on larvae was 98.7% in concentration 43.5 mg.mL-1. The concentration of EGEO that inhibits 50% of the eggs and larvae was 8.3 and 6.92 mg.mL-1, respectively. The oil chemical analysis identified as main component the monoterpen 1,8-cineol. EGEO presented ovicidal and larvicidal activities in vitro, revealing a good potential for use in the control of sheep and goat gastrointestinal nematodes.

Albizia lebbeck (L.) Benth. é popularmente conhecida no Brasil por coração-de-negro. É uma espécie aclimatada nesse país. As sementes dessa espécie são utilizadas no tratamento de diarréia, disenteria e hemorróidas e as folhas, no tratamento de úlceras e nas conjuntivites alérgicas. Este trabalho foi realizado com a finalidade de determinar as características macroscópicas e microscópicas das folhas e sementes dessa planta, uma vez que estas são as partes usadas popularmente. Algumas características de valor na sua diagnose foram apontadas e ilustradas por fotomicrografias. Reações de fitoquímica e a determinação do conteúdo de água e de cinzas foram realizadas no pó das folhas e sementes de A. lebbeck. Saponinas, alcalóides e cumarinas foram detectados nas folhas e sementes; flavonóides foram também identificados nas folhas. O teor de umidade foi 6,78% e 7,91%, respectivamente, para o pó das folhas e sementes. Os valores de cinzas totais e cinzas insolúveis em ácido foram 9,60% e 0,62% para as folhas e 4,13% e 0,21% para as sementes. Os dados obtidos são importantes para o controle de qualidade dessa planta medicinal. O extrato bruto da planta não apresentou atividade antibacteriana.
Albizia lebbeck (L.) Benth., is popularly known in Brazil as "coração-de-negro". It is a species acclimatized in Brazil. The seeds of this species are used to treat diarrhea, dysentery and hemorrhoids and the leaves are used for the treatment of ulcers and allergic conjunctivitis. The purpose of this work was to determine the macroscopic and microscopic characteristics of the leaves and seeds of this plant, since these are the parts used popularly. Some characters of diagnostical value are pointed out and illustrated by photomicrographs. Phytochemistry reactions and determination of water and ash were done on the powder of A. lebbeck. Saponins, alkaloids, coumarins were detected in the leaves and seeds; flavonoids were also detected in the leaves. The content of water was 6.78% and 7.91%, respectively, for leaf and seed powder. The values of total ashes and acid insoluble ashes were 9.60% and 0.62% for leaf, and 4.13% and 0.21%, for seed powder. The data obtained are important for the quality control of this medicinal plant. The crude extract of this plant did not show antibacterial activity.

O objetivo deste trabalho foi estudar a prevalência de adultos infectados por L. L. chagasi entre doadores de sangue do Hemocentro Regional de Montes Claros/MG .Realizou-se estudo epidemiológico, transversal e quantitativo, no período de 16/09/08 a 13/11/08. Participaram da pesquisa 421 doadores aptos na triagem clínica, sendo realizada imunofluorescência indireta para L.L.chagasi. Aqueles que apresentaram resultados positivos foram submetidos ao teste rápido antígeno-específico para Leishmania donovani. A análise das variáveis gênero, faixa etária, procedência, número de doações, resultados sorológicos para leishmaniose e chagas, foi realizada pelos testes estatísticos qui-quadrado (x2), x2 com tendência linear e teste Fisher. Foi considerado o nível de significância de 5% (p<0,05). O perfil da amostra foi semelhante ao perfil geral dos doadores. Os participantes, em sua maioria procedentes da zona urbana (92,7%), residentes em Montes Claros (67,9%), homens (61,3%), com faixa etária de 18 a 29 anos. Em relação aos resultados sorológicos, 23 (5,5%) apresentaram positividade para imunofluorescência indireta e nenhum destes foi positivo no teste rápido. Ao comparar os resultados da imunofluorescência para leishmaniose e a sorologia Elisa chagas, dois foram positivos para ambos os testes, sendo demonstrada correlação estatística significante (p=0,003). Porém, 21 foram positivos para leishmaniose e negativos para chagas. Os resultados permitiram conhecer a prevalência da infecção por L. l. chagasi em indivíduos assintomáticos, adultos, doadores de sangue do Hemocentro Regional de Montes Claros/MG e apontam para a necessidade de maiores estudos quanto ao possível risco de transmissão transfusional da doença.
The objective of this work was to study the prevalence of adults infected by Leishmania (Leishmania) chagasi among blood donors of the Hemominas Foundation in Montes Claros, Brazil. A cross-sectional, quantitative epidemiological study was performed of 421 blood donors from September 16 2008 to November 13 2008. The L. l. chagasi indirect immunofluorescence test (RIFI) was utilized. Donors that presented with positive results in RIFI were retested using the fast immunochromatographic test (Trald). The gender, age, place of origin, number of donations, leishmania and chagas disease serum results were studied with statistical correlations being analyzed utilizing the chi-square test (x2), x2 with linear tendency and the Fisher test; a level of significance of 5% (p <0.05) was considered acceptable. The profile of the study sample was similar to the overall donor profile. The participants were mostly donors from urban areas (92.7%), living in Montes Claros (67.9%), men (61.3%) and with ages between 18 and 29 years old. In relation to the serum results, 23 (5.5%) were positive according to the RIFI however none of them were positive by the Trald. On comparing the results of RIFI and the chagas disease serum test (Elisa), two individuals were positive for both tests, thereby giving a statistically significant correlation (p=0.003). However the other 21 were positive only by RIFI and negative for chagas disease. The results show the prevalence of infection by L. l. chagasi in asymptomatic, adult blood donors in the Hemominas Foundation in Montes Claros, and highlight the need for further studies on the possible risk of disease transmission via blood transfusion.

OBJETIVO: Comparar os efeitos do uso de antiinflamatórios não-esteróides tradicionais (AINES) e AINES que são inibidores seletivos da ciclooxigenase-2 (COX-2), no processo de regeneração óssea em ratos. MATERIAL E MÉTODO: Quarenta e quatro ratos da linhagem Wistar submetidos a osteotomia do femur direito e divididos em três grupos, conforme o medicamento que receberam (diclofenaco, rofecoxib e placebo). Cada grupo foi dividido em dois subgrupos, conforme o tempo até o sacrifício, após a cirurgia. Os animais do subgrupo 1 foram sacrificados duas semanas após a cirurgia e os do subgrupo 2, quatro semanas após a cirurgia. Foram analisados exames radiográficos e a histomorfometria do calo ósseo. RESULTADOS: Não foram encontradas diferenças estatísticas na área do calo ósseo 2 e 4 semanas após a cirurgia. No que se refere à área de neoformação óssea dentro do calo, observou-se diferença estatisticamente significante apenas dentro do grupo do diclofenaco, que apresentou menos tecido. CONCLUSÕES: A consolidação da fratura em ratos Wistar ocorre dentro de 2 semanas e o uso de antiinflamatórios não-esteróides não influi de forma significante neste processo.
OBJECTIVE: The aim of this study is to compare the effects of traditional nonsteroidal anti-inflammatory drugs with nonsteroidal anti-inflammatory drugs that are selective cyclooxygenase-2 (COX-2) inhibitors in the process of bone regeneration in a rat model. MATERIALS AND METHODS: Forty-four Wistar strain rats were subjected to osteotomy of the right femur and randomly divided into 3 groups according to the drug to be given (diclofenac, rofecoxib, or placebo). Each group was divided into 2 subgroups according to the time to euthanasia after the surgery. The animals of Subgroup 1 were submitted to euthanasia 2 weeks after surgery, and those of Subgroup 2, underwent euthanasia 4 weeks after surgery. Radiographic examinations and bone callus histomorphometry were analyzed. RESULTS: No intergroup statistical difference was found in the bone callus area or in bone formation area 2 and 4 weeks after surgery. Intra-group analysis concerning the bone neoformation area inside the callus showed a significant difference within the diclofenac group, which presented less tissue. CONCLUSIONS: Fracture consolidation in Wistar rats occurs within less than 2 weeks, and the use of nonsteroidal anti-inflammatory drugs does not significantly influence this process.

Two new marine metabolites, 3Z, 6Z, 9Z-dodecatrien-1-ol (1) from the ascidian Botrylloides giganteum and 4H-pyran-2ol acetate from the sponge Ircinia felix (4) are herein reported. The known bromotyrosine compounds, 2-(3,5-dibromo-4-methoxyphenyl)-N,N,N-dimethylethanammonium (2) and 2,6-dibromo-4-(2-(trimethylammonium)ethyl)phenol (3), have been isolated from the sponge Verongula gigantea. Serotonin (5) is reported for the first time from the sponge Cliona delitrix, and tambjamines A (15) and D (16) isolated as their respective salts from the nudibranch Tambja eliora. Only tambjamine D presented cytotoxicity against CEM (IC50 12.2 µg/mL) and HL60 (IC50 13.2 µg/mL) human leukemya cells, MCF-7 breast cancer cells (IC50 13.2 µg/mL), colon HCT-8 cancer cells (IC50 10.1 µg/mL) and murine melanoma B16 cancer cells (IC50 6.7 µg/mL).

OBJECTIVE: To assess the results observed during the early postoperative period in patients who had the posterior coronary arteries revascularized without cardiopulmonary bypass (CPB), in regard to the following parameters: age, sex,bypass grafts types, morbidity and mortality. METHODS: From January 1995 to June 1998, 673 patients underwent myocardial revascularization (MR). Of this total, 607 (90.20%) MR procedures were performed without CPB. The posterior coronary arteries (PCA) were revascularized in 298 (44.27%) patients, 280 (93.95%) without CPB. The age of the patients ranged from 37 to 88 years (mean, 61 years). The male gender predominated, with 198 men (70.7%). The revascularization of the posterior coronary arteries had the following distribution: diagonalis artery (31 patients, 10%); marginal branches of the circumflex artery (243 patients, 78.7%); posterior ventricular artery (4 patients, 1.3%); and posterior descending artery (31 patients, 10%). RESULTS: Procedure-related complications without death occurred in 7 cases, giving a morbidity of 2.5%. There were 11 deaths in the early postoperative period (mortality of 3.9%). CONCLUSION: Similarly to the anterior coronary arteries, the posterior coronary arteries may benefit from myocardial revascularization without CPB.

Crude extracts of eggs (SEA) adult worms (SWAP) or cercariae (Cerc) have been used to stimulate Peripheral Blood Mononuclear cells (PBMC) and have provided rather distinct profiles of responses in different types of patients. In genenral it is clear that patients with early infections respond strongly to SEA while response to SWAP are developed more slowly. As infection progresses into the more chronic phases, a general pattern is seen whic leads to lower anti-SEA proliferative responses in the face of higher responses to SWAP and variable anti-cerc responsiveness. Cured not re-exposed patients express very high levels of anti-SEA proliferation. It has recently been seen that those individuals who live in endemic areas and have continued water contact, but are reapeatedly stool-negative (who are presumed to have self-cured or be putatively resistant; endemic normals) are strongly responsive to antigenic extracts, particularly to SEA. Furthermore, our results show that endemic normal individuals have significantly higher IFN gamma production upon PBMC stimulation with schistosome antigens than infected individuals. With the emergence of more studies it is becoming apparent that both the intensity and the prevalence of a given area may influence or shape the general responsiveness of the population under study.
