ABSTRACT Objectives: Currently, not much is known about the interactions between voice and growth hormone (GH). We have described large kindred with isolated GH deficiency (IGHD) due to a GHRH receptor mutation, resulting in severe short stature and high-pitched voice. These IGHD individuals have little interest in GH treatment, as they consider themselves “short long-lived people”, rather than patients. Interestingly, they report normal general quality of life, but they rate their Voice-Related Quality of Life (V-RQOL) as low. Here, we assessed the social and auditory-perceptual impacts of artistic-intervention voice therapy with semioccluded vocal tract exercises (SOVTE) and choral singing, on their voices. Material and methods: Seventeen GH-naïve adult IGHD individuals were enrolled in a single-arm interventional pre-post study with 13 weekly sessions of choir singing over 90 days. Outcome measures were V-RQOL scores, self-assessment of voice, and auditory-perceptual analysis (GRBAS scale, G: grade of the severity of dysphonia; R: roughness; B: breathiness; A: asthenia; and S: strain). Results: Marked improvements in total (p = 0.0001), physical (p = 0.0002), and socioemotional (p = 0.0001) V-RQOL scores and in self-assessment of voice (p = 0.004) were found. The general grades of vocal deviation (p = 0.0001), roughness (p = 0.0001), breathiness (p = 0.0001) and strain (p = 0.0001) exhibited accentuated reductions. Conclusions: Voice therapy with semioccluded vocal tract exercises and choral training improved social impact and perceptual voice assessments in IGHD subjects and markedly improved their voice-related quality of life. This is particularly important in a setting where GH replacement therapy is not widely accepted.

Morita-Baylis-Hillman adducts (MBHA) is a class of polyfunctional molecules that has been standing out due to their versatility and expressive biological activities. Therefore, this paper describes the synthesis and antiproliferative activity of some new MBHA/7-choroquinoline hybrids. The Michael acceptors were obtained starting from 4,7-dichloroquinoline which were submitted to the Morita-Baylis-Hillman reaction with ortho, meta and para-nitrobenzaldehyde. The in vitro screening of the synthetized MBHA against NCI-H292, HCT-116 and MCF-7 cancer cells suggests the influence of the spacer chain in its inhibition potential. The 50% inhibitory concentration (IC50) obtained in the antiproliferative assay using MCF-7, HCT-116, HL-60 and NCI-H292 cancer cells indicate expressive cytotoxic potential of the adducts containing nitro group in the ortho position, with IC50 of 4.60 µmol L-1. MBHA/7-choroquinoline hybrids were more active than MBHA described in literature, indicating the improvement of the cytotoxic effect due to 7-chloroquinoline moiety in the molecular structure, with maximum selectivity index values of 11.89.

Early liver function improvement following successful treatment of chronic hepatitis C in patients with decompensated cirrhosis: a real-life study

OBJECTIVES: Despite higher rates of sustained virologic response (SVR), important concerns remain when patients with decompensated cirrhosis due to hepatitis C virus (HCV) are treated with direct-acting antiviral agents (DAA). Questions include efficacy, safety, and the magnitude of liver function improvement. Here, we aimed to evaluate HCV treatment data in this specific population in Brazil. METHODS: We included 85 patients with decompensated cirrhosis submitted to HCV therapy with DAA followed at two academic tertiary centers in the southeastern region of Brazil. RESULTS: Seventy-nine patients (92.9%) were Child-Pugh (CP) score B, and six (7.1%) were CP score C. The mean MELD score was 12.86. The most common treatment was sofosbuvir plus daclatasvir±ribavirin for 24 weeks. The overall intention-to-treat (ITT) SVR rate was 87.4% (74/85) and modified-ITT 96.1% (74/77). ITT SVR was associated with lower baseline INR values (p=0.029). Adverse events (AE) occurred in 57.9% (44/76) of patients. Serious AE were reported in 12.8% (10/78), and were related to the presence of hepatic encephalopathy (p=0.027). SVR was associated with improvement in CP (p<0.0001) and MELD scores (p=0.021). Among baseline CP score B patients with SVR, 46% (29/63) regressed to CP score A. Ascites was independently associated with no improvement in liver function in patients who achieved SVR (p=0.001; OR:39.285; 95% CI:4.301-258.832). CONCLUSIONS: Patients with decompensated HCV cirrhosis showed a high SVR rate with interferon-free therapy. Early liver function improvement occurred after successful HCV eradication. However, long-term follow-up of these patients after SVR remains strongly advised.

Abstract: Aristolochia triangularis Cham., is one of the most frequently used medicinal plant in Southern Brazil. Preparations containing the leaves and/or stems are traditionally used as anti-inflammatory, diuretic, as well as antidote against snakebites. This study screened A. triangularis extracts, fractions and isolated compounds for different bioactivities. A weak antiproliferative activity against human lung cancer cell line (A549) was observed only for chloroform fraction obtained from stems (CFstems - CC50: 2.93 µg/mL). Also, a moderate antimicrobial activity against Staphylococcus aureus was detected just for chloroform fraction obtained from leaves (CFleaves -13-16 mm inhibition zone). Additionally, two semi-purified fractions (CFstems-4 and CFleaves-4) selectively inhibited HSV-1 replication (IC50 values of 0.40 and 2.61 µg/mL, respectively), while only CFleaves showed promising results against Leishmania amazonensis. Fractionation of extracts resulted in the isolation of one neolignan (-) cubebin and one lignan (+) galbacin. However, these compounds are not responsible for the in vitro bioactivities herein detected. The presence of aristolochic acid I and aristolochic acid II in the crude ethanol extract of stems (CEEstems) and leaves (CEEleaves) was also investigated. The HPLC analysis of these extracts did not display any peak with retention time or UV spectra comparable to aristolochic acids I and II.

Este estudo teve como objectivo avaliar o desenvolvimento e produtividade da cultura do feijão (Phaseolus vulgaris L.), quando submetida à inoculação e aplicação de fertilizante nitrogenado (N) à sementeira e/ou cobertura. O delineamento experimental foi em blocos casualizados (DBC), em esquema fatorial 4 (N sementeira, N cobertura, N sementeira e cobertura e sem aplicação de N) x 2 (sem e com inoculação da estirpe CIAT 899T), com três repetições. Aos 28 dias após a sementeira, todos os tratamentos receberam aplicação via foliar de molibdénio na dose de 80,0 g ha-1. Nesse mesmo período, os tratamentos contendo N cobertura receberam aplicação de 40 kg ha-1. Na época de floração foram avaliados os parâmetros de matéria seca da parte aérea (MSPA), teor de nitrogénio da parte aérea (TNPA), número de nódulos (NN) e matéria seca dos nódulos (MSN). Aos 120 dias (época da colheita), foram avaliados o número de vagens (NVP) e de grãos por planta (NGP) e produtividade de grãos (PG - kg ha-1). A inoculação mostrou-se uma prática viável quando não foi utilizado nitrogénio mineral e o N foi utilizado somente na cobertura, sendo capaz de promover bom desenvolvimento vegetal e produtividade, no entanto, não equiparável à dose completa de N mineral.
The objective of this research was to evaluate the development and productivity of bean crop (Phaseolus vulgaris L.), when submitted to the inoculation and application of nitrogen fertilizer (N) at sowing and/or top dressing (cover). The experimental design was in randomized blocks (RBD) in 4 (N sowing, N top dressing, N sowing and top dressing and without N application) x 2 (with and without inoculation of strain CIAT 899T) factorial combination with three replications. At 28 days after sowing, all treatments received the foliar application of 80.0 g ha-1of molybdenum. In the same period, the treatments containing N top dressing received the application of 40 kg ha-1. At flowering time, the parameters, dry matter of the aerial part (DMAP), shoot nitrogen content (TNPA), number (NN) and dry matter of nodules (MSN) were evaluated. After 120 days (harvest time) the following parameters were evaluated: number of pods (NVP) and grains per plant (NGP) and grain yield (PG - kg ha-1). Inoculation proved to be a viable practice when it was not used mineral nitrogen. The use of inoculated seeds and also top dressing of nitrogen fertilization in cover promoted good development and productivity, however, not comparable to the full dose of mineral N.

RESUMO Tumor miofibroblástico inflamatório (IMT) é uma neoplasia mesenquimatosa rara, geralmente com origem nos tecidos moles abdominais. Uma paciente do sexo feminino com 50 anos apresentou-se com pólipo da vesícula biliar com 1,2 cm. No estudo microscópico, observou-se proliferação de células fusiformes em fundo edematoso, rico em plasmócitos e linfócitos. Imuno-histoquicamente, notou-se positividade para vimentina, actina músculo liso e cinase do linfoma anaplásico (ALK), com negatividade de outros marcadores. Observou-se rearranjo do gene ALK por hibridização fluorescente in situ (FISH). O diagnóstico foi IMT da vesícula biliar, um caso único, considerando que foi identificado no estadio inicial de desenvolvimento dessas neoplasias.
ABSTRACT Inflammatory myofibroblastic tumour (IMT) is a rare mesenchymal neoplasm that usually originates from abdominal soft tissues. A female patient aged 50 years presented with a 1.2 cm gallbladder polyp. The microscopic study showed spindle cell proliferation in an edematous background rich in lymphocytes and plasma cells. Immunohistochemistry showed positivity for vimentin, smooth muscle actin, and anaplastic lymphoma kinase (ALK), and negativity for other markers. Fluorescent in situ hybridization (FISH) revealed ALK gene rearrangement. The diagnosis was IMT of the gallbladder, a unique case considering that it was identified at the early stage of development of these neoplasms.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Objetivos: A fenilcetonúria (PKU) foi a primeira causa metabólica hereditária de retardamento mental para a qual foi desenvolvido um programa de triagem em recém-nascidos (NBS). O objetivo deste estudo foi avaliar a eficácia do NBS para a PKU e o manejo dos casos em Sergipe (SE), Brasil.Materiais e métodos: Revisamos as concentrações de fenilalanina no filtro de papel coletado do calcanhar (PKUneo) de 43.449 recém-nascidos, suas concentrações de sangue obtidas por punção venosa em indivíduos com PKUneo anormal, a idade das crianças em diversas fases do programa, a incidência da doença no período de janeiro de 2007 a junho de 2008 e o controle metabólico dos pacientes.Resultados: A cobertura da NBS/SE foi de 78,93%. A idade das crianças era de 10 ± 7 dias na coleta de PKUneo. Doze crianças foram reconvocadas com base no ponto de corte de PKUneo aos 28 ± 13 dias de idade. Destas, as concentrações de fenilalanina por venipunctura foram normais em cinco. A incidência da hiperfenilalaninemia foi 1/43.449 e de PKU foi 1/8.690 (5 casos), e um indivíduo suspeito foi a óbito. Outro óbito ocorreu na coorte em um caso de PKU confirmado. O tratamento para a PKU começou com 51 ± 12 dias. Nos quatro pacientes sob restrição de fenilalanina alimentar, o controle metabólico foi frequentemente difícil.Conclusões: PKU NBS/SE apresenta uma cobertura satisfatória e ponto de corte adequado para reconvocação e diagnóstico, mas o início do tratamento é atrasado e o controle no seguimento é frequentemente inadequado.
Objectives: Phenylketonuria (PKU) was the first inherited metabolic disease known to cause mental retardation for which a newborn screening program (NBS) was developed. The objective of this study was to evaluate the effectiveness of PKU NBS and the management of cases in the northeastern Brazilian state of Sergipe (SE).Materials and methods: We reviewed the phenylalanine concentrations in filter-paper collected from the heel (PKUneo) of 43,449 newborns; blood concentrations obtained by venipuncture in the subjects with abnormal PKUneo; the children’s age at several phases of the program, the incidence of the disease from January 2007 to June 2008; and metabolic control of the patients.Results: The coverage of NBS/SE was 78.93%. The children’s age was 10 ± 7 days at PKUneo collection. Twelve children were recalled based on the PKUneo cutoff value at 28 ± 13 days. From these, the concentrations of phenylalanine collected by venipuncture were normal in five children. The incidence of hyperphenylalaninemia was 1/43,449, and of PKU was 1/8,690 (5 cases). One suspected subject died. Another death occurred in the cohort, in a confirmed PKU case. PKU treatment began within 51 ± 12 days of life. In the four patients under dietary phenylalanine restriction, metabolic control was often difficult.Conclusions: PKU NBS/SE has satisfactory coverage and adequate cutoff for recalling patients and diagnosis, but the onset of treatment is delayed, and follow-up metabolic control is frequently inadequate.

Objetivos: O tratamento com GH é ainda controverso, salvo na deficiência grave de GH (SGHD). O objetivo deste estudo foi comparar a resposta ao tratamento com GH em indivíduos com insensibilidade parcial ao GH (PGHIS) e na deficiência moderada do GH (MGHD) com SGHD.Sujeitos e métodos: Quinze pacientes com PGHIS, 11 com MGHD e 19 com SGHD, seguidos por mais de um ano no Sistema Único de Saúde, foram avaliados antropométrica e laboratorialmente, no início, com um ano de tratamento e na última avaliação (tempo máximo de dez anos na SGHD, quatro anos na MGHD e oito anos na PGHIS).Resultados: O escore de desvio-padrão (EDP) da estatura inicial foi menor nos indivíduos com SGHD do que naqueles com MGHD e PGHIS. Embora o aumento no EDP da estatura no primeiro ano em comparação com o inicial não fosse diferente entre os grupos, o EDP da altura no primeiro ano de tratamento permaneceu menor na SGHD que na MGHD. Não houve diferença no EDP da estatura na última avaliação entre os três grupos. O tratamento com GH, no período completo da observação, provocou uma tendência a menor aumento no EDP da estatura nos pacientes com PGHIS que naqueles com SGHD, entretanto aumentos semelhantes foram encontrados nos grupos MGHD e SGHD.Conclusão: O tratamento com GH aumentou a estatura nos indivíduos com PGHIS e produziu efeitos similares na estatura em MGHD e SGHD.
Objectives: GH therapy is still controversial, except in severe GH deficiency (SGHD). The objective of this study was to compare the response to growth hormone (GH) therapy in children with partial GH insensitivity (PGHIS) and mild GH deficiency (MGHD) with those with SGHD.Subjects and methods: Fifteen PGHIS, 11 MGHD, and 19 SGHD subjects, followed up for more than one year in the Brazilian public care service, were evaluated regarding anthropometric and laboratory data at the beginning of treatment, after one year (1 st year) on treatment, and at the last assessment (up to ten years in SGHD, up to four years in MGHD, and up to eight years in PGHIS).Results: Initial height standard deviation score (SDS) in SGHD was lower than in MGHD and PGHIS. Although the increase in 1 st year height SDS in comparison to initial height SDS was not different among the groups, height-SDS after the first year of treatment remained lower in SGHD than in MGHD. There was no difference in height-SDS at the last assessment of the children among the three groups. GH therapy, in the entire period of observation, caused a trend towards lower increase in height SDS in PGHIS than SGHD but similar increases were observed in MGHD and SGHD.Conclusion: GH therapy increases height in PGHIS and produces similar height effects in MGHD and SGHD.

O presente trabalho teve como objetivo determinar coeficientes de cultura e avaliar a produtividade da cebola submetida a diferentes lâminas de irrigação. O experimento foi conduzido no campo experimental do DTCS/UNEB, em Juazeiro, no período de abril a setembro de 2011. Para determinação da ETc foram efetuadas medidas diárias em evapotranspirômetros. A ETo foi obtida através dos métodos do tanque classe A (TCA) e Penman-Monteith (PM) e o Kc pela relação: Kc = ETc/ETo. O delineamento utilizado foi inteiramente casualizado, com três tratamentos e sete repetições. As variáveis analisadas foram: diâmetro transversal, peso médio de bulbos, produção total, produção comercial, teor de sólidos solúveis totais e acidez total titulável. Os Kc's encontrados tomando-se como base a ETo determinada pelo método TCA, com o Kp calculado com dados climáticos obtidos in situ, foram bem próximos daqueles determinados pelo método PM; a produtividade total superou a média nacional em torno de 122% para os tratamentos T1 (irrigação feita na base de ETc) e T3 (irrigação em base na ETo calculado pelo método de PM) e em 86% para o tratamento T2 (irrigação na base de ETo calculado pelo método de TCA); o manejo de irrigação, tomando-se como base o método TCA, pode proporcionar produtividade elevada levando-se em consideração as condições climáticas locais.
The present study aimed to determine crop coefficients and evaluate the productivity of onion under different irrigation levels. The experiment was conducted in the experimental field at DTCS/UNEB Juazeiro during the period from April to September 2011. For determining the ETc, daily measurements were made in evapotranspirometers. The ETo was obtained by the methods of the Class A pan (TCA) and Penman-Monteith (PM) and the Kc by: Kc = ETc/ETo. The experimental design was completely randomized with three treatments and seven replications. The examined variables were: transverse diameter, mean bulb weight, total production, commercial production, total soluble solids and titratable acidity. The Kc values found by taking as a basis the ETo determined by the TCA method with the Kp calculated using weather data obtained in situ, were very close to those determined by the PM method; the total productivity surpassed the national average by 122% for T1 (irrigation on the basis of ETc) and T3 (irrigation on the basis of ETo calculated according to PM) treatments and 86% for the T2 (irrigation on the basis of ETo calculated according to TCA) treatment; the irrigation management, based on the TCA method can provide high productivity, taking into account local climatic conditions.

O objetivo deste trabalho é avaliar e comparar o efeito protetor da acetamida nas intoxicações por monofluoroacetato de sódio (MF) e por oito plantas brasileiras que causam "morte súbita" (PBCMS) (Palicourea marcgravii, P. juruana, Pseudocalymma elegans, Arrabidaea bilabiata, Amorimia (Mascagnia) rigida, M. pubiflora, Amorimia (Mascagnia) exotropica e M. aff. rigida) em ratos, bem como descrever o quadro clínico-patológico nos animais intoxicados. Foram utilizados 33 ratos da linhagem Wistar (Rattus norvegicus albinus), nove dos quais ingeriram espontaneamente folhas frescas de P. marcgravii nas doses de 2,0 e 4,0g/kg. Dois desses ratos receberam doses únicas de acetamida de 2,0 e 4,0g/kg, por via oral um minuto antes do fornecimento da planta. Outro rato recebeu 4,0g/kg de acetamida após ingerir 4,0g/kg da planta e manifestar sintomas graves. Nos experimentos com MF, foram administradas a quatro ratos doses de 4,0 e 8,0mg/kg; o intervalo de tempo entre a administração da acetamida e dos extratos concentrados ou do MF variou entre 2 a 4 horas. A dose de acetamida utilizada variou de 2,0 a 8,0g/kg. Nos experimentos com extratos concentrados das oito PBCMS, 20 ratos foram intoxicados por via oral com doses únicas ou repetidas. A acetamida, quando previamente administrada em doses suficientemente altas, evitou o aparecimento dos sinais clínicos ou morte dos animais intoxicados por MF, bem como pelas folhas frescas de P. marcgravii e com os extratos concentrados de cada uma das sete outras PBCMS utilizadas. Todos esses ratos foram novamente submetidos ao mesmo protocolo experimental, porém sem administração de acetamida. Nesses experimentos posteriores todos os ratos manifestaram sinais clínicos e morte (exceto M. aff. rigida, cuja amostra não se revelou tóxica). Todos os ratos apresentaram aurículas de leve a acentuadamente ingurgitadas e, por vezes, também as veias cava cranial e caudal. Havia moderada dilatação cardíaca direita e esquerda em três animais. O fígado de todos os animais apresentava-se de leve a acentuadamente congesto, e em alguns ratos, verificou-se evidenciação do padrão lobular. Observou-se ainda discreta a leve presença de líquido espumoso na superfície de corte dos pulmões em três ratos. O exame histopatológico evidenciou nos rins de seis dos 30 ratos leve a moderada tumefação citoplasmática dos túbulos uriníferos contornados distais e, por vezes, também nos túbulos coletores, mas somente em quatro (dois intoxicados por P. marcgravii, um por P. elegans e outro por A. exotropica) havia a clássica lesão de degeneração hidrópico-vacuolar com picnose nuclear evidente. No fígado de 26 animais havia congestão e, destes, três apresentaram estreitamento compressivo dos cordões hepáticos, oito corpúsculos de choque; em outros três, necrose focal de discreta a moderada. Observou-se ainda, de discreta a moderada vacuolização de hepatócitos em 16 animais. Este trabalho demonstra que a acetamida possui acentuado efeito protetor, capaz de prevenir os sinais clínicos e evitar a morte dos ratos intoxicados por MF, folhas frescas de P. marcgravii e extratos concentrados de outras sete PBCMS. O quadro clínico-patológico observado nos ratos intoxicados pelo MF e pelas PBCMS deste estudo, associado ao efeito protetor da acetamida, confirma que o MF é o princípio tóxico responsável pela morte dos ratos, e, por extensão, também dos bovinos que ingeriram PBCMS.
The protective effect of acetamide in poisoning by sodium monofluoroacetate (MF) and by eight Brazilian sudden death causing plants (BSDCP) (Palicourea marcgravii, P. juruana, Pseudocalymma elegans, Arrabidaea bilabiata, Amorimia (Mascagnia) rigida, M. pubiflora, Amorimia (Mascagnia) exotropica and M. aff. rigida) was studied using rats. Additionally the clinical and pathological picture of the poisoning was described. In these experiments 33 Wistar rats (Rattus norvegicus albinus) were used. Nine rats ate spontaneously the fresh leaves of P. marcgravii at amounts of 2.0 and 4.0g/kg. Two of the rats received orally single doses of acetamide (2.0 and 4.0g/kg) one minute before the plant was supplied. A third rat received 4.0g/kg of acetamide after consumption of 4.0g/kg of the plant and showed severe symptoms of poisoning. In the experiments with MF, doses of 4.0 and 8.0mg/kg were administered to four rats. The interval between the administration of either acetamide and concentrated plant extracts or MF ranged from 2 to 4 hours; the dose of acetamide ranged from 2.0 to 8.0g/ kg. In the experiments with concentrated extracts of eight BSDCP, 20 rats were orally poisoned with single or repeated doses. Acetamide, when previously administered, prevented the appearance of clinical signs and death in all of the rats poisoned by MF, as well as of the ones poisoned by fresh P. marcgravii leaves and by the concentrated extracts of each of the other BSDCP. These rats were re-subjected to the same experimental protocol, yet without the administration of acetamide. In these experiments, all of the rats evidenced clinical signs and death (except M. aff. rigida). All of the rats showed mild to markedly engorged atria, and sometimes also of the cranial and caudal vena cava. In three of the rats, there was a moderate right and left cardiac dilatation. The liver of all rats was slightly or markedly congested, with lobular pattern in some. There was frothy liquid on the cut surface of the lungs in three of the rats. The histopathology of the kidneys in six rats showed slight cytoplasmic swelling of the distal convoluted tubules and sometimes also of the collecting tubules. But only in four rats a vacuolar-hydropic degeneration with nuclear pyknosis was seen. Twenty-six rats showed liver congestion, three of them with compressive narrowing of the hepatic cords, and in eight rats shock corpuscles were observed; another three rats showed slight to moderate focal liver necrosis. A slight to moderate vacuolation of hepatocytes was observed in 16 rats. It can be concluded that acetamide had a protective effect, capable to prevent clinical signs and death of the rats poisoned by MF, and by fresh P. marcgravii leaves and concentrated extracts of seven other BSDCP. The clinical and pathological picture observed in the rats poisoned by MF and BSDCP, associated with the protective effect of acetamide, indicate that MF is the toxic principle responsible for death of the rats, and by extension, for the death of cattle that ate BSDCP.

Avaliamos as concentrações do TSH em papel-filtro colhido no calcanhar (TSHneo) de 48.039 crianças triadas do programa de triagem neonatal (PTN) para o hipotireoidismo congênito (HC) de Sergipe, as concentrações de TSH, T4 total e T4 livre colhidas em sangue periférico nas crianças convocadas suspeitas de HC, a idade nas diversas fases do programa, a cobertura e a freqüência do PTN de janeiro de 2005 a agosto de 2006, comparando-as com dados da literatura. Utilizamos para análise os seguintes parâmetros: média, mediana, coeficiente de variação e distribuição de freqüência. A idade da criança por ocasião da coleta em papel filtro no calcanhar foi 10 ± 9 dias (média ± desvio-padrão) e a idade na reali-zação do ensaio do TSHneo foi de 31 ± 13 dias. Em 2005, a cobertura do PTN, para o interior e para a capital de Sergipe, foi de 77% e 73%, respectivamente. Verificamos que em 99,484% das crianças triadas as concentrações do TSH coletado em papel-filtro encontravam-se entre 0,01 e 5,20 µU/mL. As concentrações do TSH decrescem com o aumento da idade até estabilizar entre 11 e 15 dias de vida. Foram convocadas 248 crianças a partir do TSH coletado em papel-filtro (1/194). Na convocação, as concentrações do TSH, T4 e T4 livre coletado por punção venosa estavam normais em 119 crianças (1/404). A freqüência de HC suspeito foi de 1/485 (99 casos), de HC foi de 1/6.005 (8 casos) e de hipotiroxinemia foi de 1/16.013 (3 casos). A terapia para o HC foi iniciada com 51 ± 12 dias.
It was evaluated the concentration of TSH in blood spot (TSHneo) of 48.039 children included in the Neonatal Screening Program (NSP) for Congenital Hypothyroidism (CH) of Sergipe (SE), a state in the northeast of Brazil. It was also evaluated the concentration of serum TSH, total T4 and free T4 in the recalled children suspicious of having CH, their age in several phases of the program, the covering and frequency of the NSP in the cases from January 2005 to August 2006, comparing them with literature data. The following parameters were used or the analysis: mean, standard deviation, median, coefficient of variation and frequency distribution. The children's age at the collection in filter-paper specimen was 10 ± 9 days (Mean±SD) and the TSHneo execution assay was done in the period of 31 ± 13 days. In 2005 the covering by the NSP was about 77% in the countryside and 73% in Aracaju, the capital of Sergipe. It was verified that in 99,484% of the children included in the screening, the TSHneo varied from 0,01 to 5,20µU/ml, decreasing according to the age and stabilizing when they were between 11 and 15 days. 248 children were recalled from the TSHneo (1/194).The concentrations of TSH, T4 and free T4 collected by venous puncture were normal in 119 children (1/404). The frequency of suspected CH was 1/485 (99 cases), of CH was 1/6005 (8 cases) and of hypothyroxinemia was 1/16013 (3 cases). Therapy for CH began within 51 ± 12 days.

A podridão peduncular, uma das principais doenças associadas à pós-colheita do mamão (Carica papaya L.), tem sido pouco investigada no que se refere a aspectos ecológicos e epidemiológicos nas condições brasileiras. Para que estudos sejam conduzidos nas referidas áreas é necessário que se disponham, a priori, de metodologias adequadas que permitam a reprodução dos sintomas dessa doença em condições de laboratório. No presente trabalho, o objetivo foi estabelecer um método de inoculação de frutos de mamão com o intuito de se reproduzir os sintomas da podridão peduncular, por meio de três procedimentos: 1) corte do pedúnculo seguido por deposição de um disco de micélio do fungo; 2) deposição de suspensão de conídios na região do pedúnculo, seguido por ferimento, e 3) aplicação de suspensão de conídios na região peduncular sem ferimento. Foram utilizados os fungos: Phoma caricae-papayae, Colletotrichum gloeosporioides, Botryodiplodia theobromae e Fusarium solani, sendo os frutos inoculados, submetidos a temperaturas de 20 e 25ºC. Os resultados indicaram que não houve efeito das temperaturas testadas no desenvolvimento dos fungos, nos diferentes métodos de inoculação. A maior severidade da podridão peduncular (notas de 1 a 5) foi observada em frutos inoculados pelo método de injeção no pedúnculo, independente do patógeno utilizado. Os fungos C. gloeosporioides e P. caricae-papayae foram os que apresentaram as maiores médias de severidade da doença seguido por B. theobromae e F. solani.
The stem-end-rot, one of the major post-harvest diseases in papaya (Carica papaya L.), has not been very well investigated concerning ecological and epidemiological aspects of the Brazilian conditions. So that studies can be developed in the referred to areas, it is necessary to use the right methodologies that allow the reproduction of the symptons of this disease in laboratory conditions. This study aimed to stablish a method for inoculating papaya fruits to reproduce the symptons of the stem-end-rot through tree procedures: 1) cut of the stem followed by deposition of fungal mycelium in agar discs; 2) deposition of the spore suspension into the stem's region followed by injuries, and 3) application of spore suspension in the stem's region without injuries. The inoculation was performed with the fungi P. caricae-papayae, C. gloeosporioides, B. theobromae e F. solani, and inoculated fruits were placed under temperatures of 20º and 25ºC. The results indicate that there were no effects of the tested temperatures in the development of all fungi, in the different methods of inoculation. The highest severity of stem-end-rot (score 1-5) was observed in fruits inoculated by the methods of injection in the stem, independing of the fungus used. The fungi C. gloeosporioides and P. carica-papaya presented the highest levels of severity in the disease followed by B. theobromae and F. solani.

Objetivo: Analisar os resultados obtidos com o implante de bioprótese porcina em posição aórtica, comparando dois grupos de pacientes operados consecutivamente em um mesmo período: Grupo 1 (G1): pacientes com válvula aórtica sem suporte ("stentless"); Grupo 2 (G2): pacientes com válvula convencional com suporte ("standart"). Casuística e Métodos: No período de janeiro de 1990 a dezembro de 1996, foram realizadas, no Biocor Instituto, 752 operações de troca valvar aórtica, cujos prontuários foram analisados retrospectivamente. Foram levantados dados relacionados às características clínicas dos dois grupos, assim como resultados relacionados à operação: número das próteses implantadas, tempo de circulação extracorpórea (CEC) e pinçamento aórtico (PINC), permanência em Centro de Tratamento Intensivo (CTI) e hospitalar, morbidade e mortalidade pós-operatória. Os ecocardiogramas dos pacientes em acompanhamento ambulatorial foram analisados e os seguintes dados obtidos: área válvular efetiva, índice de área válvular efetiva, gradiente transvalvular médio e grau de insuficiência válvular. Resultados: Foram realizadas 169 operações no G1 e 583 operações no G2. As características clínicas dos dois grupos foram semelhantes, com exceção da idade (38,0 ± 17,5 vs 46,0 ± 19,0 anos) (p < 0,05) e de um número proporcionalmente maior de endocardite, primária e prostética, e de retrocas valvares no G1 (p < 0,02). Quanto aos dados per e pós-operatórios, mais de 70% dos pacientes nos dois grupos receberam próteses de números 19 a 25, sendo o critério para escolha da prótese a medida do anel valvar. Os tempos de CEC (99,9 ± 28,8 vs 86,3 ± 31,0 minutos) e de PINC (80,9 ± 25,2 vs 65,0 ± 24,4 minutos) foram significativamente diferentes (p < 0,0001). No entretanto, os tempos de permanência hospitalar e em CTI, assim como a morbimortalidade hospitalar foram semelhantes. Com relação ao acompanhamento ecocardiográfico, a área valvular efetiva (1,94 ± 0,31 vs 1,48 ± 0,33 cm2) (p < 0,000001), assim como o índice de área valvular efetiva (1,15 ± 0,23 vs 0,89 ± 0,06 cm2/m2) (p < 0,000001), foram significativamente maiores no G1, o que resultou em menores gradientes transvalvulares médios (8,3 ± 4,9 vs 13,2 ± 4,6 mmHg) (p < 0,000001). Os índices de insuficiência valvular foram discretos e sem diferenças significativas entre os dois grupos. Conclusão: As válvulas aórticas compostas porcinas "stentless" quando comparadas às biopróteses convencionais com suporte apresentaram melhor desempenho hemodinâmico a longo prazo, sem aumento de morbimortalidade ou de custos hospitalares. Os benefícios hemodinâmicos observados sugerem um aumento da durabilidade da válvula e regressão mais eficaz das alterações estruturais do ventrículo esquerdo, pressupondo uma melhor qualidade de vida.
Objective: The purpose of this retrospective study is to compare clinical performance of 2 heart valve substitutes used in the aortic position. Both groups were operated consecutively during the same period of time: Group 1 (G1): patients with composite porcine aortic stentless valves; Group 2 (G2): patients with composite porcine stented aortic bioprostheses. Material and Methods: The clinical data of 725 patients were reviewed and we found 752 operations for aortic valve replacement performed between January 1990 and December 1996. Only patients with at least one year of follow-up were included. Data collected included clinical characteristics, valve size implanted, cardiopulmonary bypass and aortic crossclamp time and in house and outpatient follow-up. Echocardiographic data were also compared as far as the following parameters: effective orifice area, effective orifice area index, mean transvalvar gradient and severity of aortic insufficiency. Results: We performed 169 operations in G1 and 583 operations in G2. Clinical characteristics of the 2 groups were similar, except the data related to age (38.0 ± 17.5 vs 46.0 ± 19.0 years) (p < 0.05) and etiology of aortic valve disease, as there were more native and prosthetic endocardites in G1 (p < 0.02). Perioperative data showed that more than 70% of patients of the 2 groups received bioprostheses nº 25 or less, chosen in all cases based on the size of the aortic annulus. Cardiopulmonary by-pass (99.9 ± 28.8 vs 86.3 ± 31.0 minutes) and aortic crossclamp time (80.9 ± 25.2 vs 65.0 ± 24.4 minutes) were longer in G1 (p < 0.0001). Nevertheless, intensive care unit and hospital stay, morbidity and mortality of the 2 groups were similar. The echocardiographic studies disclosed effective orifice area (1.94 ± 0.31 vs 1.48 ± 0.33 cm2) and effective orifice area index (1.15 ± 0.23 vs 0.89 ± 0.06 cm2/m2) significantly larger in the stentless group (p < 0.000001), thus leading to lower mean transvalvar gradient (8.3 ± 4.9 vs 13.2 ± 4.6 mmHg) (p < 0.0001). Postoperative aortic insufficiency was negligible in both subsets. Conclusion: Biocor porcine stentless aortic bioprostheses compared to Biocor stented bioprostheses have shown better hemodynamic performance, without increasing morbididy or mortality rates, which may enhance valve durability and ventricular remodeling with important prognostic implications for patient survival. Longer follow-up will provide the test of time.
