Background General Hospital Psychiatric Units have a fundamental importance in the mental health care systems. However, there is a lack of studies regarding the level of improvement of patients in this type of facility. Objective To assess factors related to good and poor outcomes in psychiatric inpatients using an index composed by clinical parameters easily measured. Methods Length of stay (LOS), Global Assessment of Functioning (variation and at discharge) and Clinical Global Impression (severity and improvement) were used to build a ten-point improvement index (I-Index). Records of psychiatric inpatients of a general hospital during an 18-month period were analyzed. Three groups (poor, intermediate and good outcomes) were compared by univariate and multivariate models according to clinical and sociodemographic variables. Results Two hundred and fifty patients were included, with a percentage in the groups with poor, regular and good outcomes of 16.4%, 59,6% and 24.0% respectively. Poor outcome at the discharge was associated mainly with lower education, transient disability, antipsychotics use, chief complaint “behavioral change/aggressiveness” and psychotic features. Multivariate analysis found a higher OR for diagnoses of “psychotic disorders” and “personality disorders” and others variables in relation to protective categories in the poor outcome group compared to the good outcome group. Discussion Our I-Index proved to be an indicator of that allows an easy and more comprehensive evaluation to assess outcomes of inpatients than just LOS. Different interventions addressed to conditions such as psychotic disorders and disruptive chief complaints are necessary.

O objetivo deste trabalho foi analisar a frequência de lesões macroscópicas sugestivas de tuberculose em carcaças de bovinos reagentes ao teste intradérmico de tuberculose. Os animais do estudo foram provenientes de rebanhos de exploração leiteira, localizados nos municípios de Perdizes, Patos de Minas, Lagoa Formosa e Uberlândia, Minas Gerais. Foram avaliadas 140 carcaças de bovinos positivos no teste cervical comparativo. Durante o abate sanitário foi realizada inspeção post mortem das carcaças, e as lesões macroscópicas sugestivas de tuberculose foram registradas. Foram avaliados os linfonodos mediastínicos, fígado, pulmão e carcaça. Dos 140 bovinos examinados, 78 (55%) apresentaram algum tipo de lesão macroscópica sugestiva de tuberculose, 38 (49%) ocorreram exclusivamente nos linfonodos mediastínicos, 22 (28%) no fígado e 11 (14%) no pulmão; 5 (6%) carcaças apresentaram lesões no fígado, pulmão e linfonodo mediastínico, e 2 (4%) tiveram lesões no pulmão e linfonodo. Concluem-se que as lesões não visualizadas em bovinos reagentes ao teste tuberculínico podem ocorrer e estariam relacionadas principalmente ao estágio de evolução da doença, tempo insuficiente na inspeção post mortempara detecção, bem como reações inespecíficas para outras micobactérias.

The aim of this study was to analyze the frequency of macroscopic lesions suggestive of tuberculosis in cattle reacting to the tuberculin test. Cattle of this study were from dairy herds located in the cities of Perdizes, Patos de Minas, Lagoa Formosa and Uberlândia, Minas Gerais, Brazil. We evaluated 140 cattle carcasses positive to the cervical comparative intradermal tuberculin test. During the slaughter a post-mortem inspection of carcasses was done, and the lesions suggestive of tuberculosis were recorded. We evaluated the mediastinal lymph nodes, liver, lung and carcass. Of the 140 cattle examined, 78 (55%) carcasses had some kind of macroscopic lesions suggestive of tuberculosis, 38 (49%) occurred only in the mediastinal lymph nodes, 22 (28%) in the liver and 11 (14%) in the lungs; 5 (6%) carcasses showed lesions in liver, lungs and lymph node, and 2 (4%) showed lesions in lung and lymph nodes. We concluded that lesions that were not visualized in bovine tuberculin skin test reagents may occur and are related mainly to the stage of the disease, insufficient time to detect the lesions, and nonspecific reactions to other mycobacteria.

INTRODUCTION: Sliding mechanics is widely used during orthodontic treatment. One of the disadvantages of this mechanics is the friction generated at the bracket/archwire interface, which may reduce the amount of desired orthodontic movement obtained. Due to the application and great acceptance of this type of mechanics, the role of friction in Orthodontics has been of interest for both clinicians and scientists. OBJECTIVE: Therefore, this article discussed how friction affects orthodontic tooth movement, with an approach to its clinical implications as well as the evolution of dental materials and its properties regarding resistance to sliding.

INTRODUÇÃO: o atrito gerado na interface braquete/fio durante a mecânica de deslizamento pode reduzir a eficiência da movimentação ortodôntica. O método de ligação do fio ao braquete exerce importante papel na determinação desse atrito. MÉTODOS: o presente estudo comparou a força de atrito gerada por quatro tipos de braquetes autoligáveis (Time®; Damon 2®; In-Ovation R® e Smart Clip®) com um grupo de braquetes ortodônticos convencionais (Dynalock®) associados a ligaduras elásticas tradicionais (Dispens-A-Stix®), que serviu como grupo controle. A força de atrito estático foi mensurada através da máquina universal de ensaios EMIC® DL 500 com dois fios de aço inoxidável com secção transversal 0,018" e 0,017" x 0,025". RESULTADOS: a análise de variância ANOVA e o teste de Tukey mostraram baixos níveis de atrito nos quatro braquetes autoligáveis associados ao fio 0,018" (P<0,05). Entretanto, os resultados observados quando os braquetes autoligáveis foram testados com fios 0,017" x 0,025" mostraram alta resistência ao deslizamento nos grupos de braquetes autoligáveis ativos.

INTRODUCTION: Friction generated at the bracket/archwire interface during sliding mechanics can reduce the efficiency of orthodontic movement. The ligation method employed to tie the archwire to the bracket plays an important role in determining this friction. METHODS: This study compared the frictional force generated by four different types of self-ligating brackets (Time™, Damon 2™, In-Ovation R™ and Smart Clip™) with a group of conventional orthodontic brackets (Dynalock™) that require the use of traditional elastomeric ligatures (ExDispens-A-Stix™), which served as the control group. Static friction force was measured using an EMIC DL™ 500 universal testing machine using stainless steel round 0.018-in and rectangular 0.017x0.025-in archwires. RESULTS: ANOVA and Tukey's test showed low levels of friction in the four self-ligating brackets in tests with the 0.018-in wire (P <0.05). However, the results noted when the self-ligating brackets were tested using 0.017x 0.025-in archwires showed high resistance to sliding in the self-ligating groups.

FUNDAMENTO: As cardiopatias são doenças de alta prevalência, sendo a cardite reumática uma doença de grande relevância em países em desenvolvimento. As alterações em câmaras cardíacas esquerdas se associam à disfunção endotelial, com aumento dos níveis de endotelina-1 (ET-1) e consequências sobre a circulação pulmonar, muitas vezes determinando a hipertensão pulmonar (HP). No entanto, a presença de ET-1 e seus receptores na própria valva mitral, promovendo alterações vasculares pulmonares e aumentando a deformação valvar reumática, ainda é um assunto não abordado na literatura. OBJETIVO: Determinar, mediante técnicas moleculares, a expressão dos genes da endotelina e dos seus receptores em valvas mitrais reumáticas. MÉTODOS: 27 pacientes submetidos à troca valvar mitral tiveram seu tecido valvar analisado, a fim de determinar a presença de genes de ET-1 e seus receptores A e B. Foram feitas análises histológica e molecular das valvas (divididas em fragmentos M1, M2 e M3) e colhidos dados clínicos e epidemiológicos dos pacientes. Foram divididos em três grupos: valvopatia mitral, mitroaórtica e pacientes reoperados. RESULTADOS: O estudo mostrou a manifestação do gene da ET-1 em 40,7% dos espécimes e de seu receptor A em todas as amostras, com manifestação minoritária do gene do receptor B (22,2%). CONCLUSÃO: Todos os pacientes expressaram a presença do gene do receptor A. Não houve diferença estatística quanto à gravidade da doença, expressa em classe funcional, e aos subgrupos estudados (valvopatas mitrais, mitroaórticos e pacientes reoperados), ou quanto à expressão dos genes da ET-1 e seus receptores entre os subgrupos estudados (valvopatas mitrais, mitroaórticos e pacientes reoperados).

BACKGROUND: Cardiopathies are high prevalence conditions. Among them, rheumatic carditis is of high relevance in developing countries. Left cardiac chamber changes are associated to endothelial dysfunction and ET-1 levels increase. Pulmonary circulation is then affected, and not seldom leading to pulmonary hypertension (PH). However, the presence of ET-1 and its receptors in the mitral valve itself - promoting pulmonary vascular changes, with increased rheumatic valvular deformation - has not been discussed in the literature. OBJECTIVE: To determine the expression of endothelin gene and its receptors in rheumatic mitral valves through techniques of molecular genetics. METHODS: Twenty-seven patients submitted to mitral valve replacement had their valvular tissue examined to determine the presence of ET-1 genes and their A and B receptors. Histological and molecular analysis of the valves was performed (divided into M1, M2 and M3 fragments), with patients' clinical and epidemiological data collected. Patients were divided into 3 groups (mitral valvopathy, mitroaortic valvopathy, and reoperation patients). RESULTS: The study showed endothelin-1 gene expression in 40.7% specimens and A receptor in all samples; receptor gene B had lower expression (22.2%). CONCLUSION: All patients showed A receptor gene expression. No statistically significant difference was observed in regard to condition severity, expressed according to functional class, and subgroups (mitral valvopathy, mitroaortic valvopathy, and reoperation patients).

FUNDAMENTO: Las cardiopatías son enfermedades de alta prevalencia, siendo la carditis reumática una enfermedad de gran relevancia en países en desarrollo. Las alteraciones en cámaras cardíacas izquierdas se asocian a la disfunción endotelial, con aumento de los niveles de endotelina-1 (ET-1) y consecuencias sobre la circulación pulmonar, muchas veces determinando la hipertensión pulmonar (HP). Mientras que, la presencia de ET-1 y sus receptores en la propia válvula mitral, promoviendo alteraciones vasculares pulmonares y aumentando la deformación valvar reumática, aún es un asunto no abordado en la literatura. OBJETIVO: Determinar, mediante técnicas moleculares, la expresión de los genes de la endotelina y de sus receptores en válvulas mitrales reumáticas. MÉTODOS: 27 pacientes sometidos a reemplazo valvular mitral tuvieron su tejido valvar analizado, a fin de determinar la presencia de genes de ET-1 y sus receptores A y B. Fueron hechos análisis histológico y molecular de las valvas (divididas en fragmentos M1, M2 y M3) y recogidos datos clínicos y epidemiológicos de los pacientes. Fueron divididos en tres grupos: valvopatía mitral, mitroaórtica y pacientes reoperados. RESULTADOS: El estudio mostró la manifestación del gen da ET-1 en 40,7% de los sujetos y de su receptor A en todas las muestras, con manifestación minoritaria del gen del receptor B (22,2%). CONCLUSIÓN: Todos los pacientes expresaron la presencia de gen del receptor A. No hubo diferencia estadística en cuanto a la gravedad de la enfermedad, expresada en clase funcional, y a los subgrupos estudiados (valvopatías mitrales, mitroaórticos y pacientes reoperados), o en cuanto a la expresión de los genes de la ET-1 y sus receptores entre los subgrupos estudiados (valvopatías mitrales, mitroaórticos y pacientes reoperados).
