Resumo Objetivo analisar o conhecimento e o uso das Práticas Integrativas e Complementares (PIC) na atuação dos enfermeiros da Atenção Primária à Saúde (APS). Método estudo transversal realizado em 85 Unidades Básicas de Saúde de Curitiba-PR em 2021. A amostra foi de 195 enfermeiros recrutados pela técnica Snowball. Os dados foram coletados por um instrumento eletrônico estruturado contendo 20 afirmativas com respostas tipo Likert e analisados por métodos estatísticos descritivos. Resultados 43(22,1%) dos participantes utilizam PIC na assistência, sendo que 31(15,9%) referiram ter formação em alguma modalidade. Identificou-se a utilização de 17 modalidades de PIC, sendo 33(47,8%) em auriculoterapia e sete (10,1%) em reiki. O ranking médio do item sobre o conhecimento teórico dos participantes sobre o tema foi >3,89, demonstrando concordância às afirmativas. Sobre as vivências dos participantes em relação às PIC, verificou-se o reconhecimento dessas práticas pelos enfermeiros. Conclusão o estudo contribui para a caracterização da atuação dos enfermeiros na APS, a partir das PIC. Os achados servem de subsídio para os profissionais e os gestores identificarem as lacunas no conhecimento, na atuação, bem como no gerenciamento dos serviços de saúde da APS, com o intuito de fortalecer a autonomia profissional daqueles que se utilizam dessas práticas na assistência.

Abstract Objective to analyze the knowledge and use of integrative and complementary practices in the work of nurses in Primary Health Care. Method a cross-sectional study carried out in 85 Basic Health Units in Curitiba-PR in 2021. The sample consisted of 195 recruits recruited using the Snowball technique. The were confirmed by means of an electronic data selection instrument and methods by electronic descriptive data methods. Results 43 (22.1%) of the participants use integrative and complementary practices (PIC) in care, and 31 (15.9%) reported having training in some modality. The use of 17 PIC modalities was identified, 33 (47.8%) in auriculotherapy and 7 (10.1%) in reiki. The Ranking item on the theoretical knowledge of the average participants on the topic was >3.89, demonstrating agreement with the statements. About how participants experience the PIC, due to the recognition of nursing practices. Conclusion the study contributes to the characterization of nurses' performance in primary care based on integrative and complementary practices. The findings serve as a subsidy for professionals and managers to identify gaps in knowledge, in action, as well as in the management of primary care health services in order to strengthen the professional autonomy of those who use these practices in care.

Resumen Objetivo analizar el conocimiento y uso de prácticas integradoras y complementarias en el trabajo de enfermeros en la Atención Primaria de Salud. Método estudio transversal realizado en 85 Unidades Básicas de Salud de Curitiba-PR en 2021. La muestra estuvo compuesta por 195 reclutas reclutados mediante la técnica Snowball. Se confirmaron por medio de un instrumento electrónico de selección de datos y métodos por métodos electrónicos de datos descriptivos. Resultados 43 (22,1%) de los participantes utilizan prácticas integradoras y complementarias (PIC) en el cuidado, y 31 (15,9%) relataron tener formación en alguna modalidad. Se identificó el uso de 17 modalidades de PIC, 33 (47,8%) en auriculoterapia y 7 (10,1%) en reiki. El ítem del Ranking sobre el conocimiento teórico de los participantes promedio sobre el tema fue >3,89, demostrando concordancia con los enunciados. Sobre cómo los participantes experimentan el PIC, debido al reconocimiento de las prácticas de enfermería. Conclusión el estudio contribuye para la caracterización de la actuación de los enfermeros en la atención primaria a partir de prácticas integradoras y complementarias. Los hallazgos sirven de subsidio para que los profesionales y gestores identifiquen lagunas en el conocimiento, en la acción, así como en la gestión de los servicios de salud de la atención básica para fortalecer la autonomía profesional de quien utiliza esas prácticas en el cuidado.

Abstract This study aimed to evaluate the contribution of oral and maxillofacial pathology laboratories (OMPLs) in Brazilian public universities to the diagnosis of lip, oral cavity, and oropharyngeal squamous cell carcinoma (SCC). A cross-sectional study was performed using biopsy records from a consortium of sixteen public OMPLs from all regions of Brazil (North, Northeast, Central-West, Southeast, and South). Clinical and demographic data of patients diagnosed with lip, oral cavity, and oropharyngeal SCC between 2010 and 2019 were collected from the patients’ histopathological records. Of the 120,010 oral and maxillofacial biopsies (2010-2019), 6.9% (8,321 cases) were diagnosed as lip (0.8%, 951 cases), oral cavity (4.9%, 5,971 cases), and oropharyngeal (1.2%, 1,399 cases) SCCs. Most cases were from Brazil’s Southeast (64.5%), where six of the OMPLs analyzed are located. The predominant profile of patients with lip and oral cavity SCC was Caucasian men, with a mean age over 60 years, low schooling level, and a previous history of heavy tobacco consumption. In the oropharyngeal group, the majority were non-Caucasian men, with a mean age under 60 years, had a low education level, and were former/current tobacco and alcohol users. According to data from the Brazilian National Cancer Institute, approximately 9.9% of the total lip, oral cavity, and oropharyngeal SCCs reported over the last decade in Brazil may have been diagnosed at the OMPLs included in the current study. Therefore, this data confirms the contribution of public OMPLs with respect to the important diagnostic support they provide to the oral healthcare services extended by the Brazilian Public Health System.

ABSTRACT Many infants are nurtured with milk supplied by human banks, whose bacteriological and physical-chemical profiles are a major issue. We investigated the bacteriological and physical-chemical characteristics, as well as genotypic and phenotypic and profiles of Staphylococcus species isolated from 240 samples of breast milk from a bank in a teaching hospital. Dornic acidity of milk revealed that 95.4% (229/240) had acceptable limits (< 8.0 oD). Caloric intake showed a wide variation in cream content (4%), fat (4%) and energy values (559.81 Kcal/L). Staphylococcus (105/186 or 56.5%) and Enterobacter (25/186 or 13.4%) were the most prevalent genera, although other microorganisms were identified, including Klebsiella pneumoniae and Pseudomonas aeruginosa. Amoxicillin/clavulanic acid (125/157 or 79.6%), vancomycin (115/157 or 73.2%), and cephalexin (112/157 or 71.3%) were the most effective antimicrobials. High resistance rates of isolates were found to penicillin G (141/157 or 89.8%), ampicillin (135/157 or 86%), and oxacillin (118/157 or 75.2%). Multidrug resistance to ≥ 3 antimicrobials occurred in 66.2% (123/186) of the isolates. Residues of microbial multiplication inhibitory substances were found in 85% (204/240) of samples. Among the coagulase-positive-CPS and negative-CoNS staphylococci, the mecA gene was detected in 53.3% (8/15) and 75% (30/40), respectively. Genes sea, seb and sec were detected in 20% (3/15) of CPS, while tsst-1 was detected in 13.34% (2/15). In addition, 13.3% (2/15) of S. aureus were toxin-producers. Genes sea, seb and sec were detected in 90% (36/40), 5% (2/40) and 15% (6/40) CoNS, respectively. Enterotoxin production was identified in 5% (2/40) of CoNS. The identification of multidrug-resistant bacteria, staphylococci species toxin-producers harboring methicillin-resistance genes, and residues of microbial multiplication inhibitory substances reinforce the need for a continuous vigilance of milk quality offered to infant consumption by human banks.

ABSTRACT CONTEXT: Pancreatic metastases from primary malignant tumors at other sites are rare, constituting about 2% of the neoplasms that affect the pancreas. Pancreatic metastasis from breast cancer is extremely rare and difficult to diagnose, because its clinical and radiological presentation is similar to that of a primary pancreatic tumor. CASE REPORT: A 64-year-old female developed a lesion in the pancreatic tail 24 months after neoadjuvant therapy, surgery and adjuvant radiation therapy for right-side breast cancer (ductal carcinoma). She underwent distal pancreatectomy with splenectomy and left adrenalectomy, and presented an uneventful outcome. The immunohistochemical analysis on the surgical specimen suggested that the lesion originated from the breast. CONCLUSION: In cases of pancreatic lesions detected in patients with a previous history of breast neoplasm, the possibility of pancreatic metastasis should be carefully considered.

RESUMO Introdução: A fístula pancreaticopleural é complicação rara da pancreatite crônica. Objetivo: Descrever a fístula pancreaticopleural consequente à pancreatite crônica e fazer revisão extensa da literatura sobre o tópico. Métodos: Revisão narrativa abrangente através de pesquisa online nas bases de dados Medline e Lilacs para artigos publicados nos últimos 20 anos. Resultados: Houve 22 relatos de casos e quatro séries de casos selecionadas. A principal indicação para o tratamento cirúrgico é a falha de tratamentos clínicos e/ou endoscópicos. A cirurgia é baseada na drenagem pancreática interna, especialmente por meio de pancreaticojejunostomias e/ou ressecções pancreáticas. Conclusão: A fístula pancreaticopleural é complicação rara da pancreatite crônica e o procedimento de Frey pode ser opção terapêutica apropriada em casos selecionados quando os tratamentos clínico e endoscópico não obtiverem êxito.

ABSTRACT Introduction: Pancreaticopleural fistula is a rare complication of chronic pancreatitis. Objective: To describe pancreaticopleural fistula due to chronic pancreatitis and perform an extensive review of literature on this topic. Methods: Comprehensive narrative review through online research on the databases Medline and Lilacs for articles published over the last 20 years. There were 22 case reports and four case series selected. Results: The main indication for surgical treatment is the failure of clinical and/or endoscopic treatments. Surgery is based on internal pancreatic drainage, especially by means of pancreaticojejunostomy, and/or pancreatic resections. Conclusion: Pancreaticopleural fistula is a rare complication of chronic pancreatitis and the Frey procedure may be an appropriate therapeutic option in selected cases when clinical and endoscopic treatments are unsuccessful.

A hepatectomia tem sido o tratamento padrão para metástase de origem não colorretal (NCR) metacrônica, principalmente quando o intervalo livre de doença é maior do que dois anos. A hepatectomia por laparoscopia tem se tornado padrão principalmente para as ressecções à esquerda, haja vista a menor morbidade, menor tempo de internação, reabilitação precoce e melhor resultado estético. Os autores relatam um caso de paciente com duas metástases metacrônicas (10 anos de sobrevida livre de doença), de etiologia não colorretal (adenocarcinoma de intestino delgado), tratada com segmentectomia lateral esquerda (lobectomia hepática esquerda) laparoscópica. Paciente apresentou boa evolução pós-operatória sem recidiva (seis meses de seguimento). Segmentectomia lateral esquerda laparoscópica pode ser satisfatoriamente realizada em casos selecionados de metástases hepáticas, acarretando menor morbidade e melhor resultado estético. A falta de tratamentos alternativos e o prognóstico reservado nos casos de metástases NCR não operadas justificam a ressecção com o objetivo de prolongar a sobrevida. No entanto, essa abordagem deve ser realizada por equipe especializada em cirurgia hepática com treinamento em procedimentos laparoscópicos avançados.

Hepatectomy has been the standard treatment for metachronic metastases of non-colorectal (NCR) origin, mainly when the disease-free interval is more than two years. Laparoscopic hepatectomy has become the golden standard mainly for left side resections, due to lower morbidity, shorter hospital stay, early recovery and good cosmetic outcome. The authors report the case of a female patient with two metachronic metastases (ten years of disease-free survival), of non-colorectal origin (adenocarcinoma of small intestine), treated by laparoscopic left lateral segmentectomy (left hepatic lobectomy) with success. The postoperative progress was satisfactory. To date, the patient has presented no tumoral recurrence (six months of follow-up period). Laparoscopic left lateral segmentectomy can be satisfactorily performed in selected cases of hepatic metastasis. This approach presents low morbidity and good cosmetic result. The lack of alternative treatments and the poor prognosis of untreated cases have justified surgical resection in order to increase overall survival. Nevertheless, this approach should be performed by hepatic surgery expertise teams trained on advanced laparoscopic procedures.

Quasiexperimental piece of research, 2004, on arterial oxygen saturation (SaO2) during the bath of the pneumopathic neonates staying in Intensive Therapy Unit. The objectives were as follows: to demonstrate the use of the SaO2 transcutaneous (SpO2) as oxymetric measure during the bath of the pneumopathic neonate; to discuss the relevance of water temperature in the bath, as per its effects on oxygen consumption. Data collection consisted of the measure and serial registration of biophysiological data: cardiac and SpO2. Results demonstrated the inexistence of significant oxyhemodynamic variations during the bath, except for two clients, who presented variations higher than expected (>10%). Physiologically acceptable variation of oxygen saturation occurred in more than three quarters of the population (n = 08). It was concluded that the bath did not turn out to be a risk factor. However, further studies in a greater sample are required for confirmation of these findings.

Estudio casi experimental sobre la saturación arterial de oxígeno (SaO2) durante el baño de 10 neonatos pneumopatas, internados en Unidad de Terapia Intensiva, realizado en 2004, en hospital do Rio de Janeiro Brasil. Los objetivos fueron: demostrar la utilización de SaO2 transcutáneo (SpO2) como medida oximétrica del baño del neonato pneumopata; discutir la importancia de la temperatura del agua utilizada en el baño y sus efectos en el consumo de oxígeno. La colecta de datos consistió en la mensuración y registro seriado de datos biofisiológicos: frecuencia cardíaca y SpO2. Los resultados demostraron la inexistencia de variaciones oxi-hemodinámicas significativas durante el baño, a pesar de que dos clientes presentaron variaciones mayores que la esperada (>10%). La variación de la saturación de oxígeno fisiológicamente aceptable sucedió en más de tres cuartos de la población (n = 8). Se concluye que el baño no constituyó factor de riesgo el neonato, siendo necesario otros estudios similares con muestra representativa de la población para retificar esos hallazgos.

Os resultados demonstraram a inexistência de variações oxi-hemodinâmicas significativas durante o banho, apesar de dois clientes terem apresentado variações maiores que a esperada (>10%). A variação da saturação de oxigênio fisiologicamente aceitável ocorreu em mais de três quartos da população (n = 8). Concluiu-se que o banho não constituiu fator de risco para o recém-nascido, necessitando outros estudos semelhantes com amostra representativa da população para ratificar esses achados.

Visando investigar a possível patogenicidade do vírus isolado (GOI 4191) de um evento adverso fatal pela vacinação antiamarílica, realizou-se um ensaio experimental em Syrian hamsters (Mesocricetus auratus), usando-se a cepa vacinal 17DD como parâmetro. As amostras virais foram inoculadas por via intracerebral, intra-hepática e subcutânea. Nos soros foram determinados níveis de viremia, resposta imune e aminotransferases, e nas vísceras a presença de vírus, antígeno e lesões teciduais. Não se detectou viremia para as duas amostras, a resposta imune foi maior para GOI 4191, e as aminotransferases não apresentaram alterações compatíveis com danos hepáticos. Nos animais inoculados por via intracerebral o vírus foi recuperado somente a partir do cérebro, sendo o antígeno viral detectado, por imuno-histoquímica, no cérebro e fígado. Infiltrado inflamatório e corpúsculos acidófilos foram observados no fígado e lesões tipo encefalite viral no sistema nervoso central. Alterações histológicas e antígeno viral foram observados, também, no fígado dos animais infectados por via intra-hepática, e ausentes naqueles inoculados por via subcutânea. Os resultados foram similares para as duas amostras testadas, entretanto distintos daqueles relatados na literatura para cepas silvestres do vírus amarílico.

In order to investigate the pathogenicity of the virus strain GOI 4191 that was isolated from a fatal adverse event after yellow fever virus (YFV) vaccination, an experimental assay using hamsters (Mesocricetus auratus) as animal model and YFV 17DD vaccine strain as virus reference was accomplished. The two virus strains were inoculated by intracerebral, intrahepatic and subcutaneous routes. The levels of viremia, antibody response, and aminotransferases were determined in sera; while virus, antigen and histopathological changes were determined in the viscera. No viremia was detected for either strain following infection; the immune response was demonstrated to be more effective to strain GOI 4191; and no significant aminotransferase levels alterations were detected. Strain GOI 4191 was recovered only from the brain of animals inoculated by the IC route. Viral antigens were detected in liver and brain by immunohistochemical assay. Histothological changes in the viscera were characterized by inflammatory infiltrate, hepatocellular necrosis, and viral encephalitis. Histological alterations and detection of viral antigen were observed in the liver of animals inoculated by the intrahepatic route. These findings were similar for both strains used in the experiment; however, significant differences were observed from those results previously reported for wild type YFV strains.
