RESUMO Introdução: a expansão rápida da maxila assistida cirurgicamente (ERMAC) é um procedimento que reduz a resistência das suturas, corrigindo a mordida cruzada posterior em adultos. Objetivo: o objetivo deste estudo foi avaliar o status periodontal de 17 adultos submetidos a esse procedimento. Métodos: o nível clínico de inserção (NCI), a recessão gengival, gengiva inserida e sangramento foram avaliados nos primeiros pré-molares, molares, incisivos centrais e laterais superiores dos lados direito e esquerdo antes da cirurgia, e depois de 5 dias e 6 meses. Médias, desvios-padrão, medianas, valores mínimos e máximos foram comparados entre as avaliações, usando os testes de Friedman e McNemar. Resultados: houve aumento estatisticamente significativo no NCI no incisivo central direito, pré-molares direito e esquerdo e molares direito e esquerdo. Houve aumento estatisticamente significativo na recessão gengival nos pré-molares e nos molares direito e esquerdo. A quantidade de gengiva inserida diminuiu significativamente nos pré-molares direitos e molares direitos e esquerdos. Houve aumento no sangramento na maioria dos dentes. Conclusão: os resultados indicaram que a ERMAC pode causar alterações no tecido periodontal.

ABSTRACT Introduction: The surgically assisted rapid maxillary expansion is a procedure that reduces the resistance of the sutures correcting the posterior crossbite in adults. Objective: The aim of this study was to evaluate the periodontal status of 17 adults submitted to this procedure. Methods: The clinical attachment level (CAL), gingival recession, attached gingiva and bleeding were evaluated in maxillary first premolars and molars, central and lateral incisors of right and left sides before surgery, 5 days and 6 months after. Means, standard deviation, medians, minimum and maximum values were compared among the evaluations using the Friedman and McNemar tests. Results: There was a statistically significant increase in CAL in the right central incisor, right and left premolars and right and left molars. There was a statistically significant increase in gingival recession in the right and left premolars and molars. The amount of attached gingiva significantly decreased in right premolars and right and left molars. There was increase in bleeding in most of the teeth. Conclusion: Results indicated that the surgically assisted rapid maxillary expansion might cause alterations in periodontal tissue.

RESUMO Objetivo Traçar um perfil das condições periodontais de pacientes com doença renal crônica em hemodiálise e seu risco periodontal. Métodos Foram incluídos no estudo 115 pacientes em hemodiálise. Os parâmetros clínicos periodontais avaliados foram: índice de placa, índice gengival, profundidade de sondagem e perda de nível de inserção clínico. Os pacientes foram classificados de acordo com a presença e a gravidade da doença periodontal, bem como de acordo com o risco periodontal. Resultados Quanto aos parâmetros clínicos, 107 pacientes dentados (93%) apresentaram média de índice de placa de 1,53±0,78 e gengival de 0,95±0,85, profundidade de sondagem de 2,2±0,6mm e nível de inserção clínica de 3,18±1,75mm. Em relação à doença periodontal, 1 (0,94%) era saudável, 55 (51,40%) apresentavam periodontite leve, 28 (26,17%) moderada e 23 (21,49%) avançada. Com relação ao risco, dos 107 pacientes, 37 apresentavam risco baixo, 35 moderado e 35 alto. Os pacientes com doença periodontal avançada apresentaram 104,5 vezes mais chance de ter alto risco comparado ao baixo (odds ratio: 104,5; IC95%: 10,7-1017,2; p<0,0001). Conclusão A maioria dos pacientes com doença renal crônica apresentou doença periodontal, indicando um processo infeccioso e inflamatório crônico, que pode influenciar na condição sistêmica. Evidencia-se a necessidade de uma abordagem preventiva e intervencionista nesta população, enfatizando a importância da saúde bucal. A avaliação do risco periodontal seria uma ferramenta na elaboração de terapias periodontais individualizadas para uma melhor condição de saúde geral.

ABSTRACT Objective To establish a profile of periodontal conditions in chronic kidney disease patients on hemodialysis and their periodontal risk. Methods We included 115 patients on hemodialysis. Clinical periodontal parameters assessed were: plaque index, gingival index, probing depth and clinical attachment level. Patients were classified according to presence/absence and severity of periodontal disease and periodontal risk. Results In 107 dentate patients (93%) the plaque index was 1.53±0.78, the gingival index was 0.95±0.85, the probing depth was 2.2±0.6mm and the clinical attachment level was 3.18±1.75mm. We observed that 1 patient (0.94%) did not have periodontal disease, 55 patients (51.40%) had slight, 28 (26.17%) moderate and 23 (21.49%) severe periodontal disease. Among 107 patients, 37 (34.58%) had low risk, 35 (32.71%) moderate risk and 35 (32.71%) high risk. Patients with severe periodontal disease showed 104.5 more chance of high risk compared with low risk individuals (odds ratio: 104.5; 95%CI: 10.7-1017.2; p<0.0001). Conclusion Most of patients with chronic renal disease presented periodontal disease, indicating the presence of chronic inflammatory and infection process that may influence in systemic conditions. A prevention and interventionist approach in this population is needed, especially to emphasize the importance of oral health. The periodontal risk assessment is a useful tool to create individualized periodontal therapies and to improve general health condition.

This study aimed to investigate age-related changes in the biosynthetic capacity of prostaglandin E2 (PGE2) in the gingival crevicular fluid (GCF) during one month of orthodontic treatment. Twenty-five juvenile subjects (mean age 13 ± 2.1 years) and 23 adults (mean age 24 ± 2.1 years) were included. GCF was collected immediately before the force application at the baseline, 2, 21 and 28 days, with periopaper inserted into the gingival crevice of the maxillary lateral incisors. The mediator levels were determined with an EIA kit. The results showed that the PGE2 concentrations were significantly elevated from the baseline to 21 days (129.35 and 198.84 pg/µL, p = 0.0169) in juvenile subjects and reduced from 21 to 28 days (198.84 to 112.60 pg/µL, p = 0.0032). Adults, however, had no significant changes in the PGE2 levels. The total amounts of PGE2 from both groups changed between the baseline to 21 and 21 to 28 days (p = 0.0119 and p = 0.0076, respectively). The PGE2 initial and final levels showed significant differences between the juveniles and adults, being higher in adults (baseline: juvenile = 129.35 pg/µL vs. adult = 163.20 pg/µL, p = 0.0379; t3: juvenile = 112.60 pg/µL and adult = 175.30 pg/µL, p = 0.0005). In conclusion, the results demonstrate the presence of variation in the PGE2 levels according to age and the orthodontic activation period, which can explain why the speed of orthodontics treatment may be different in adults vs. juveniles.

OBJETIVO: O objetivo deste estudo foi avaliar a influência da doença periodontal sobre os parâmetros bioquímicos da saliva, incluindo taxa de fluxo salivar, pH, proteína total, atividade de fosfatase alcalina e a concentração de uréia, em indivíduos com periodontite crônica. METODOLOGIA: Quarenta indivíduos foram alocados em dois grupos: vinte indivíduos sem doença periodontal (grupo controle) e vinte indivíduos com periodontite crônica (grupo teste). Índice de placa (IP), índice gengival (IG), profundidade de sondagem (PD) e nível de inserção clínica (CAL) foram obtidos anteriormente por um examinador treinado e calibrado clinicamente. Saliva de ambos os grupos foram coletadas para análise de seus parâmetros bioquímicos. O teste T de Student e coeficiente de correlação (Pearson) foram utilizados em um nível 5% de significância. RESULTADOS: Os resultados mostraram alterações na atividade da fosfatase alcalina, uréia e proteína total nos indivíduos do grupo teste em comparação ao grupo controle (P<0,05). No entanto, não houve correlação entre os parâmetros clínicos periodontais e os parâmetros salivares (P>0,05). CONCLUSÃO: Os resultados sugerem que a periodontite crônica pode afetar a composição da saliva e que a análise dos parâmetros salivares pode ser útil como um exame complementar para o diagnóstico da doença periodontal.

PURPOSE: The aim of this study was to evaluate the influence of periodontal disease on the biochemical parameters of the saliva, including salivary flow rate, pH, total protein, alkaline phosphatase activity and urea concentration, in individuals with chronic periodontitis. METHODS: Forty subjects were allocated into two groups: twenty subjects without periodontal disease (control group) and twenty subjects with chronic periodontitis (test group). Plaque index (PI), gingival index (GI), probing depth (PD) and clinical attachment level (CAL) were obtained by a previously trained and calibrated clinical examiner. Saliva from both groups was collected for analysis of its biochemical parameters. The Student's t-test and correlation coefficient (Pearson) were used at a 5% level of significance. RESULTS: The results showed alterations in the alkaline phosphatase activity, urea concentration and total protein in the subjects of the test group compared to the control group (P<0.05). However, there was no correlation between the clinical periodontal parameters and the salivary parameters (P>0.05). CONCLUSION: The results suggest that chronic periodontitis can affect the composition of the saliva and that analysis of the salivary parameters can be useful as an additional exam for the diagnosis of periodontal disease.

OBJETIVO: verificar se existe relação entre os parâmetros periodontais e os níveis de estrógeno e densitometria óssea mineral (DOM). MÉTODOS: foram avaliadas 46 mulheres na pós-menopausa entre 44 e 68 anos de idade (52,2±4,8) e 15 mulheres como grupo controle entre 35 e 54 anos de idade (44,7±7,5). Parâmetros periodontais como profundidade à sondagem (PS), perda de inserção clínica (PIC) e dentes ausentes (DA) foram comparados com os níveis de estrógeno (suficiente e deficiente) e DOM em normais, osteopênicas e osteoporóticas. Os dados foram comparados pela diferença das médias entre os grupos e analisados pelo teste de Aspin-Welch. RESULTADOS: as médias dos parâmetros de PS, PIC e DA, quando associados ao grau da DOM em normais (2,1±0,5; 2,9±1,4 e 10,6±5,0), osteopênicas (2,3±0,7; 3,0±1,1 e 12,8±5,1) e osteoporóticas (2,4±0,6; 2,7±0,9 e 14,3±5,7), não mostraram diferenças significativas (p>0,05). Foi encontrada diferença significante entre o grupo controle e nas mulheres menopausadas para PIC e DA. Quando comparados com os níveis de estrógeno os resultados demonstraram igualdade para os parâmetros periodontais. CONCLUSÕES: apesar de alguns estudos demonstrarem correlação positiva da doença periodontal com osteoporose e com os níveis de estrogênio, na população de mulheres menopausadas estes dados não foram confirmados neste estudo.

PURPOSE: to verify if there is a relationship between the clinical periodontal parameters and estrogen levels and bone mineral density (BMD). METHODS: forty-six post-menopausal women aged 44 to 68 years (52.2±4.8) and 15 women aged 35 to 54 years (44.7±7.5) were evaluated. Periodontal parameters like probing depth (PD), clinical attachment loss (CAL), and missing teeth (MT) were compared with estrogen levels (sufficient and insufficient) and BMD (normal, osteopenic and osteoporotic). Data of the mean difference between the groups were compared by the Aspin-Welch test. RESULTS: the means of PD, CAL and MT, when associated with the normal (2.1±0.5; 2.9±1.4 and 10.6±5.0), osteopenic (2.3±0.7; 3.0±1.1 and 12.8±5.1) and osteoporotic BMD (2.4±0.6; 2.7±0.9 and 14.3±5.7), did not show statistical difference (p>0.05). A significant difference was found between the control group and postmenopausal women for CAL and MT. When compared with the estrogen levels the results did not show a difference regarding the periodontal parameters. CONCLUSION: although some studies showed a positive correlation with osteoporosis and estrogen level, in this population of menopausal women these findings were not confirmed.

O objetivo do presente estudo foi avaliar a prevalência de periodontite agressiva localizada, periodontite agressiva generalizada e periodontite incipiente em uma população de 15 a 25 anos de idade (19,4 ± 3,44) da região do Vale do Paraíba - SP que procuraram tratamento odontológico clínico geral no Departamento de Odontologia da Universidade de Taubaté, SP. Seiscentos pacientes, 244 do sexo masculino e 356 do sexo feminino, foram incluídos neste estudo. A condição periodontal da população estudada foi determinada em 6 sítios por dente por meio da avaliação das medidas de profundidade à sondagem e nível clínico de inserção, e confirmada por meio de exame radiográfico. Dez indivíduos (1,66%) apresentaram periodontite agressiva localizada, 2 do sexo masculino (18,5 ± 2,12) e 8 do sexo feminino (19,2 ± 3,91), 22 (3,66%) receberam diagnóstico de periodontite agressiva generalizada, sendo 6 do sexo masculino (19,1 ± 3,06) e 16 do sexo feminino (20,1 ± 2,71) e 86 (14,3%) foram diagnosticados com periodontite incipiente, 29 do sexo masculino (20,2 ± 2,87) e 57 do sexo feminino (21,1 ± 2,79). Houve correlação positiva entre sexo feminino e doença periodontal.

The aim of the present study was to determine the prevalence of localized and generalized aggressive periodontitis, as well as of incidental attachment loss, in a population of adolescents and young adults aging between 15 and 25 years (19.4 ± 3.44) from Vale do Paraíba - SP, who searched for general dental care at the Department of Dentistry, University of Taubaté, São Paulo. Six hundred patients, 244 male and 356 female subjects, were included in the studied sample. The periodontal status of this population was evaluated by measuring the depth of periodontal pockets, as well as attachment loss. The data were confirmed by means of radiographic examination. Measurements were performed in six sites per tooth. Ten subjects (1.66%) were diagnosed as having localized aggressive periodontitis, 2 males (aging 18.5 ± 2.12 years) and 8 females (aging 19.2 ± 3.91 years); 22 (3.66%) presented with generalized aggressive periodontitis, 6 males (aging 19.1 ± 3.06 years) and 16 females (aging 20.1 ± 2.71 years); and 86 individuals (14.3%) presented with incipient periodontitis, 29 males (aging 20.2 ± 2.87 years) and 57 females (aging 21.1 ± 2.79 years). There was a positive correlation between the female gender and the occurrence of periodontal disease.
