Resumo OBJETIVO Analisar as relações contextuais da atenção à saúde na alta em hanseníase. MÉTODO Estudo analítico pautado no referencial teórico de análise do contexto, elaborado mediante revisão integrativa de literatura nas bases de dados SCOPUS, PUBMED, LILACS, SCIELO e BDENF, com descritores Hanseníase e Alta do Paciente, obtendo-se 14 publicações. RESULTADOS O contexto imediato aborda a atenção em saúde na alta em hanseníase; o contexto específico trata da hanseníase como problema de saúde pública; as concepções simbólicas que envolvem a hanseníase são abarcadas pelo contexto geral; e no metacontexto estão descritos programas e políticas de saúde que subsidiam o atendimento à pessoa com hanseníase. CONCLUSÃO Os elementos contextuais ressaltam a necessidade de garantir a atenção em saúde para os casos de hanseníase, do diagnóstico até o pós-alta, reconhecendo a hanseníase como problema de saúde pública. Apesar das limitações dos estudos bibliográficos, estes possuem relevância para a área da saúde.

Abstract OBJECTIVE To analyze contextual relations of health care in the discharge of leprosy. METHOD An analytical, reflexive study based on the theoretical framework of context analysis, elaborated through an integrative review of literature in the databases SCOPUS, PUBMED, LILACS, SCIELO and BDENF, with uncontrolled descriptors Leprosy and Patient Discharge, obtaining 14 publications. RESULTS The immediate context addresses health care at discharge in leprosy; the specific context treats leprosy as a public health problem; the symbolic conceptions and marks involving leprosy are encompassed by the general context; and in the metacontext are described the health programs and policies that subsidize the care of leprosy patients. CONCLUSION The contextual elements emphasize the need to guarantee universal coverage of cases of leprosy, from diagnosis to the post-discharge, reinforcing leprosy as a public health problem. Despite the limitations of the bibliographic studies, these have relevance for the health area.

Resumen OBJETIVO Analizar las relaciones contextuales de la atención a la salud en el alta en lepra. MÉTODO Estudio analítico, pautado en el referencial teórico de análisis del contexto, elaborado mediante revisión integrativa de literatura en las bases de datos SCOPUS, PUBMED, LILACS, SCIELO y BDENF, con descriptores Lepra y Alta del Paciente, obteniendo 14 publicaciones. RESULTADOS El contexto inmediato aborda la atención en salud en el alto en lepra; el contexto específico trata de la lepra como problema de salud pública; las concepciones simbólicas que envuelven la lepra son abarcadas por el contexto general; y en el metacontexto se describen los programas y políticas de salud. CONCLUSIÓN Los elementos contextuales resaltan la necesidad de garantizar cobertura universal casos de lepra, del diagnóstico hasta el post-alta, reforzando la hanseniasis como problema de salud. A pesar de las limitaciones de los estudios bibliográficos, éstos tienen relevancia para el área de la salud.

RESUMO: O trabalho avaliou a atividade antiviral in vitro de própolis e espécies de Baccharis sobre três herpes vírus animais (bovino, equino e suíno). As própolis foram produzidas por duas espécies de abelhas. Pela Apis melifera (abelha africanizada) foram obtidas duas própolis, vermelha e verde, e uma terceira foi obtida pela abelha nativa Tetragonisca angustula (abelha jataí). Os extratos de Baccharis foram obtidos de 4 espécies diferentes: B. oblongifolia, B. burchellii, B. dracunculifolia e B. uncinella. A concentração máxima não tóxica dos extratos foi determinada pela ausência de alterações morfológicas nas células, e essas concentrações então utilizadas nos testes antivirais. A atividade antiviral foi avaliada pela redução do efeito citopático e calculada a partir da diferença entre o título viral do tratado pelo controle e feita a análise estatística. A própolis vermelha foi ativa contra os três herpes vírus, e a própolis verde apresentou inibição contra os herpes vírus equino e suíno, enquanto a própolis da abelha jataí não apresentou atividade antiviral. A maioria dos extratos dos indivíduos masculinos e femininos de todas as espécies de Baccharis apresentou atividade antiviral contra os herpes vírus bovino e suíno. Apenas o extrato do indivíduo feminino de B. oblongifolia foi inibidor contra o herpes vírus equino.

ABSTRACT: This study evaluated the in vitro antiviral activity of propolis and Baccharis sp. extracts on three animal herpesviruses (bovine, equine and swine). The propolis samples were produced by two species of bees. There was red and green propolis, which came from africanized Apis melifera, and a third type obtained from a native bee species, Tetragonisca angustula (jatai). The Baccharis extracts were obtained from four different species: B. oblongifolia, B. burchellii, B. dracunculifolia and B. uncinella. The maximum non-toxic concentration of the extracts was determined when no visible morphological changes were observed on the cells. These non-toxic concentrations were used in the antiviral tests. Antiviral activity was evaluated using a reduction assay of the cytopathic effect, which calculated the difference between treated and control virus titer by statistical analysis. Red propolis was active against the three herpesviruses and green propolis showed inhibition against the equine and swine herpesviruses. Conversely, jataí propolis showed no antiviral activity. Most extracts coming from male and female individuals of all of the Baccharis species showed antiviral activity against bovine and swine herpesviruses. Only the extract of the female specimen of B. oblongifolia was an inhibitor against equine herpesvirus.

Vochysia divergens (Vochysiaceae) is considered an invasive species in the wetlands of the Brazilian Pantanal, which hinders the cultivation of agricultural species. In this study, we evaluated the chemical profile by HPLC-DAD (high-performance liquid chromatography-diode array detector) of leaves extracts from V. divergens seedlings inoculated with endophytic fungi isolated from V. divergens roots. These fungi were collected on dry (D) and wet (W) seasons in the Pantanal. The presence of tannin hexahydroxydiphenoyl (HHDP)-galloyl-glucose and flavone 3',5'-dimethoxy-luteolin were predominant in the seedlings inoculated with endophytic fungi W experiments at 100 and 80%, respectively. Likewise, flavone 3',5-dimethoxy-luteolin-7-O-β-glucoside showed a similar representation in the two evaluated periods, compared with 5-methoxy-luteolin, which was detected only in seedlings inoculated with W endophytic fungi. This approach is new to V. divergens, which has no scientific data on its in vitro elicitation, in the search for a better understanding of the ecological relationships of this species.

ABSTRACT Background The effects of non-invasive ventilation (NIV) on the breathing pattern and thoracoabdominal motion of patients with amyotrophic lateral sclerosis (ALS) are unknown. Objectives 1) To analyze the influence of NIV on chest wall volumes and motion assessed by optoelectronic plethysmography in ALS patients and 2) to compare these parameters in the supine and sitting positions to those of healthy individuals (without NIV). Method Nine ALS patients were evaluated in the supine position using NIV. In addition, the ALS patients and nine healthy individuals were evaluated in both sitting and supine positions. Statistical analysis was performed using the paired Student t-test or Wilcoxon test and the Student t-test for independent samples or Mann-Whitney U test. Results Chest wall volume increased significantly with NIV, mean volume=0.43 (SD=0.16)L versus 0.57 (SD=0.19)L (p=0.04). No significant changes were observed for the pulmonary rib cage, abdominal rib cage, or abdominal contribution. The index of the shortening velocity of the diaphragmatic muscle, mean=0.15 (SD=0.05)L/s versus 0.21 (SD=0.05)L/s (p<0.01), and abdominal muscles, mean=0.09 (SD=0.02)L/s versus 0.14 (SD=0.06)L/s (p<0.01), increased during NIV. Comparisons between the supine and sitting positions showed similar changes in chest wall motion in both groups. However, the ALS patients presented a significantly lower contribution of the abdomen in the supine position compared with the controls, mean=56 (SD=13) versus 69 (SD=10) (p=0.02). Conclusions NIV improved chest wall volumes without changing the contribution of the chest wall compartment in ALS patients. In the supine position, ALS patients had a lower contribution of the abdomen, which may indicate early diaphragmatic dysfunction.

Resumo Objetivo Gerar equações de predição da espirometria em crianças de quatro a seis anos. Métodos Capacidade vital forçada, volume expiratório forçado em 0,5 segundo, volume expiratório forçado em um segundo, pico de fluxo expiratório e fluxo expiratório forçado com 25-75% da capacidade vital forçada foram avaliados em 195 crianças saudáveis que residem em Sete Lagoas, Estado de Minas Gerais, Sudeste do Brasil. O método dos mínimos quadrados foi usado para derivar as equações de predição. O nível de significância foi estabelecido como p < 0,05. Resultados No geral, 85% das crianças foram bem-sucedidas ao fazer as manobras espirométricas. Na equação de predição, a estatura foi a única variável preditora das variáveis espirométricas, da seguinte forma: capacidade vital forçada = exponencial [(-2,255) + (0,022 x estatura)], volume expiratório forçado em 0,5 segundo = exponencial [(-2,288) + (0,019 x estatura)], volume expiratório forçado em um segundo = exponencial [(-2,767) + (0,026 x estatura)], pico do fluxo expiratório = exponencial [(-2,908) + (0,019 x estatura)] e fluxo expiratório forçado com 25-75% da capacidade vital forçada = exponencial [(-1,404) + (0,016 x estatura)]. Nem a idade nem o peso influenciaram as equações de regressão. Não foi observada diferença significativa nos valores previstos em meninos e meninas. Conclusão Os valores previstos obtidos neste estudo são comparáveis àqueles relatados em crianças em idade pré-escolar tanto do Brasil quanto de outros países.

Abstract Objective To generate prediction equations for spirometry in 4- to 6-year-old children. Methods Forced vital capacity, forced expiratory volume in 0.5 s, forced expiratory volume in one second, peak expiratory flow, and forced expiratory flow at 25–75% of the forced vital capacity were assessed in 195 healthy children residing in the town of Sete Lagoas, state of Minas Gerais, Southeastern Brazil. The least mean squares method was used to derive the prediction equations. The level of significance was established as p < 0.05. Results Overall, 85% of the children succeeded in performing the spirometric maneuvers. In the prediction equation, height was the single predictor of the spirometric variables as follows: forced vital capacity = exponential [(−2.255) + (0.022 × height)], forced expiratory volume in 0.5 s = exponential [(−2.288) + (0.019 × height)], forced expiratory volume in one second = exponential [(−2.767) + (0.026 × height)], peak expiratory flow = exponential [(−2.908) + (0.019 × height)], and forced expiratory flow at 25–75% of the forced vital capacity = exponential [(−1.404) + (0.016 × height)]. Neither age nor weight influenced the regression equations. No significant differences in the predicted values for boys and girls were observed. Conclusion The predicted values obtained in the present study are comparable to those reported for preschoolers from both Brazil and other countries.

RESUMO É importante avaliar a função pulmonar em pré-escolares. Poucos estudos relacionados aos testes de função pulmonar nessa população estão disponíveis. O objetivo deste estudo foi avaliar a taxa de sucesso e reprodutibilidade teste-reteste do pico de fluxo da tosse (PFT) em uma amostra de crianças com idade entre 4 e 6 anos. O PFT foi estudado em 44 crianças saudáveis (26 meninos e 18 meninas), selecionadas de acordo com o questionário ATS-DLD-78-C, utilizado para detectar a presença de doenças respiratórias de base e exposição ambiental. O medidor de pico de fluxo expiratório (Piko-I Electronic Peak Flow Meter, Pulmonary Data Services, USA) foi usado para mensurar o PFT. A taxa de sucesso foi definida como a porcentagem de crianças capazes de realizar o teste de acordo com os critérios de aceitabilidade e reprodutibilidade. Para avaliar a reprodutibilidade teste-reteste, 10 crianças (de acordo com o cálculo amostral) foram reavaliadas após três semanas. A reprodutibilidade teste-reteste foi avaliada pelo teste t pareado, considerando significativo p<0,05 e coeficiente de correlação intraclasse (CCI). Os resultados mostraram uma taxa de sucesso de 91% para PFT, sendo de 80, 88 e 100% para crianças com 4, 5 e 6 anos, respectivamente. Quanto à reprodutibilidade teste-reteste, não houve diferença significativa entre os dados da primeira avaliação e da reavaliação (p=0,39) e foi observado CCI de 0,84. Esses resultados sugerem elevada taxa de sucesso na realização do PFT e reprodutibilidade teste-reteste de magnitude excelente para essa variável em pré-escolares saudáveis.

ABSTRACT It is important to evaluate lung function in preschoolers. There are few studies related to pulmonary function tests with this part of the population. The purpose of this study was to evaluate the success rate and test-retest reproducibility of the cough peak flow (CPF) in a sample with children between four and six years old. The CPF was tested in 44 healthy children (26 boys, 18 girls), selected according to the ATS-DLD-78-C questionnaire, used to detect the presence of common respiratory diseases or induced by environmental exposure. An expiratory peak flow meter (Piko-I Electronic Peak Flow Meter, Pulmonary Data Services, USA) was used to measure CPF. The success rate was defined as the percentage of children able to perform the test according to the acceptability and reproducibility criteria. To evaluate the test-retest reproducibility, 10 children (according with the sample calculation) were reevaluated after three weeks. The study was approved by the Ethics Research Committee of the Institution. The test-retest reproducibility was evaluated by the paired t-test, considering a significance of p<0.05 and intraclass correlation coefficient (ICC). The results showed a success rate of 91% for CPF, with 80%, 88% and 100% for children with four, five and six years, respectively. Regarding the test-retest reproducibility, there was no significant difference between data of the first assessment and reassessment (p=0.39) and an ICC of 0.84 was observed. These results suggest an elevated success rate in the performance of CPF and an excellent test-retest reproducibility for this variable in healthy preschoolers.

RESUMEN La evaluación de la función pulmonar en niños en edad preescolar es importante pues hay pocos estudios que investigan los test de función pulmonar en este grupo. Este estudio tuvo el propósito de evaluar la tasa de éxito y la reproducibilidad test-retest del pico de flujo de tos (PFT) en una muestra con niños entre 4 y 6 años de edad. Se estudió el PFT en 44 participantes saludables (26 chicos y 18 chicas) elegidos a través de un cuestionario ATS-DLD-78-C para detectar la presencia de enfermedades respiratorias de base y exposición ambiental. Se utilizó el medidor del pico de flujo espiratorio (Piko-I Electronic Peak Flow Meter, Pulmonary Data Services, USA) para medir el PFT. Se definió la tasa de éxito como el porcentaje de niños capaces de realizar el test según los criterios de aceptabilidad y de reproducibilidad. Para evaluar la reproducibilidad test-retest tras tres semanas se reevaluaron 10 niños según el cálculo de la muestra. Se analizó la reproducibilidad a través del test t pareado, considerando significativo <0,05 y el coeficiente de correlación intraclase (CCI). Los resultados mostraron una tasa de éxito del 91% para el PFT, siendo un 80, 88 y 100% para niños con 4, 5 y 6 años, respectivamente. En cuanto a la reproducibilidad test-retest no hubo diferencias significativas entre los datos de la primera evaluación y de la reevaluación (p=0,39) en que se observó el CCC de 0,84. Estos resultados proponen alta tasa de éxito en la ejecución del PFT y excelente reproducibilidad test-retest en esa variable en niños saludables en edad preescolar.

RESUMOObjetivo:analisar a implantação de registro de trauma em hospital universitário com atendimento ao SUS, bem como, sua capacidade em identificar pontos para melhorada qualidade no atendimento.Métodos:o grupo de coleta de dados foi composto por alunos dos cursos de medicina e enfermagem, orientados pelos coordenadores do projeto. Utilizamos o software itreg (ECO Sistemas-RJ/SBAIT) como ferramenta de banco de dados. Vários "filtros" de qualidade foram propostos no intuito de selecionar os casos a serem revistos no processo de controle de qualidade.Resultados:entre março e novembro de 2014, foram inseridos no itreg dados de 1344 vítimas de trauma. Cerca de 87% foram vítimas de trauma fechado, 59,6% apresentaram RTS>7,0 e, 67%, ISS<9. Os registros foram completos em 292 casos, que foram selecionados para a revisãono programa de qualidade. Os filtros de auditoria mais frequentemente anotadosforam a laparotomia após quatro horas da admissão e a drenagem dos hematomas subdurais agudos após quatro horas da admissão. Identificamos vários pontos a serem desenvolvidos, como o controle da "supertriagem" de doentes, a necessidade de diminuição do número de exames de imagem negativos, o desenvolvimento de protocolos para passagem de acessos venosos centrais e tratamento de trauma craniencefálico grave.Conclusão:o Registro de Trauma traz uma visão clara dos pontos a serem melhorados no atendimento ao traumatizado, contudo, há características específicas na implantação desta ferramenta.

ABSTRACTObjective:to analyze the implementation of a trauma registry in a university teaching hospital delivering care under the unified health system (SUS), and its ability to identify points for improvement in the quality of care provided.Methods:the data collection group comprised students from medicine and nursing courses who were holders of FAPESP scholarships (technical training 1) or otherwise, overseen by the coordinators of the project. The itreg (ECO Sistemas-RJ/SBAIT) software was used as the database tool. Several quality "filters" were proposed to select those cases for review in the quality control process.Results:data for 1344 trauma patients were input to the itreg database between March and November 2014. Around 87.0% of cases were blunt trauma patients, 59.6% had RTS>7.0 and 67% ISS<9. Full records were available for 292 cases, which were selected for review in the quality program. The auditing filters most frequently registered were laparotomy four hours after admission and drainage of acute subdural hematomas four hours after admission. Several points for improvement were flagged, such as control of overtriage of patients, the need to reduce the number of negative imaging exams, the development of protocols for achieving central venous access, and management of major TBI.Conclusion: the trauma registry provides a clear picture of the points to be improved in trauma patient care, however, there are specific peculiarities for implementing this tool in the Brazilian milieu.

INTRODUÇÃO: A assincronia toracoabdominal (TA) é um importante sinal da Doença Pulmonar Obstrutiva Crônica (DPOC). Estudos investigando a influência do treinamento de endurance sobre assincronia TA não foram encontrados. OBJETIVO: Analisar as repercussões do treinamento de endurance dos membros inferiores-MMI sobre a assincronia TA em indivíduos com DPOC. MATERIAIS E MÉTODOS: Dois pacientes com DPOC grave foram avaliados em um estudo experimental de caso único AB (A-baseline por seis semanas, B-treinamento em cicloergômetro com intensidade de 70% da carga pico atingida no baseline, por 12 semanas) com medidas repetidas das variáveis: relação da fase inspiratória (PhRIB), relação da fase expiratória (PhREB) e ângulo de fase (PhAng). Essas variáveis foram analisadas pela pletismografia respiratória por indutância durante testes de exercício incremental em cicloergômetro (isocarga e carga pico em cada teste). Os métodos estatísticos incluíram análise visual, two-standard deviation band e split middle line test, considerando significativo p < 0,05. Foram considerados os resultados com concordância de pelo menos duas análises. RESULTADOS: Durante a fase B, o paciente 1 apresentou redução significativa da PhRIB (22,7 ± 3,4 x 17,0 ± 4,9) e PhAng (16,5 ± 5,1 x 13,2 ± 2,1) para isocarga e PhREB (16,8 ± 3.1 x 13,3 ± 3,1) e PhAng (23,4 ± 1,7 x 20,1 ± 2,3) na carga pico. O paciente 2 mostrou redução significativa do PhRIB para isocarga (14,4 ± 3,8 x 13,9 ± 3,9) e na carga pico (23,4 ± 1,7 x 20,1 ± 2,3). CONCLUSÕES: Os resultados sugerem que o treinamento de resistência dos MMII reduziu a assincronia TA nos pacientes avaliados. Os resultados podem estar relacionados, de acordo com a literatura, com a menor demanda ventilatória e maior capacidade de exercício dos pacientes submetidos a um treinamento de endurance.

INTRODUCTION: Thoracoabdominal-TA asynchrony is an important sign of Chronic Obstructive Pulmonary Disease (COPD). Studies investigating the influence of endurance training on TA asynchrony have not been found. OBJECTIVE: To analyze lower-limb endurance training effects on TA asynchrony in patients with COPD. MATERIALS AND METHODS: Two patients with severe COPD were evaluated in a single-subject design AB (A-baseline for six weeks, B-training on cycle ergometer with intensity of 70% of baseline peak load, for 12 weeks) with repeated measures of variables: phase inspiratory relation (PhRIB), phase expiratory relation (PhREB) and phase angle (PhAng). These variables were assessed by respiratory inductive plethysmography during incremental exercise tests on a cycle ergometer (same load and peak load of each test). Statistical methods included visual analysis, two-standard deviation band test and split middle line test, considering significant p < 0.05. It was considered the results for variables with agreement of at least two analyses. Data are presented as mean ± SD for phases A and B. RESULTS: During phase B, Patient 1 presented significant decrease of PhRIB (22.7 ± 3.4 x 17.0 ± 4.9) and PhAng (16.5 ± 5.1 x 13.2 ± 2.1) for same load and PhREB (16.8 ± 3.1 x 13.3 ± 3.1) and PhAng (23.4 ± 1.7 x 20.1 ± 2.3) at peak load. Patient 2 showed significant decrease of PhRIB for same load and (14.4 ± 3.8 x 13.9 ± 3.9) at peak load (19.1 ± 2.5 x 15.7 ± 2.7). CONCLUSIONS: These results suggest that lower-limb endurance training reduced TA asynchrony in patients with severe COPD. The findings may be related, according to the literature, to the lower ventilatory demand and greater exercise capacity of patients with COPD undergoing endurance training.
