Abstract The aim of the present study was to assess morphologic and genetic data on ascariasis in swine (Sus scrofa domesticus) and humans in low-resource rural and periurban communities in the state of Piauí, Brazil. Our cross-sectional survey included 100 fecal samples obtained from swine and 682 samples from humans. Fifteen pigs were necropsied. Human and porcine fecal samples were examined to identify Ascaris eggs. Parasites obtained in the swine necropsies were studied using scanning electron microscopy (SEM), and the mitochondrial gene encoding the cytochrome oxidase 1 (cox1) enzyme was partially amplified and sequenced for molecular taxonomy and phylogenetic analyses. The overall prevalence of Ascaris eggs in the swine fecal samples was 16/100 (16%). No Ascaris eggs were identified in the human fecal samples. SEM of six worms recovered from pigs demonstrated morphological characteristics of A. suum. Cox1 sequences were compatible with A. suum reference sequences. Original and reference (GenBank) nucleotide sequences were organized into clusters that did not segregate the parasites by host species or and region. The largest haplogroups were dominated by haplotypes H01, H02 and H31. In the communities studied, there was no epidemiological evidence of the zoonotic transmission of ascariasis at the human-swine interface.

Resumo O presente estudo teve como objetivo acessar dados morfológicos e genéticos sobre a ascaridíase em suínos (Sus scrofa domesticus) e humanos, em comunidades rurais e periurbanas no estado do Piauí. O estudo transversal incluiu 100 amostras fecais de suínos e 682 amostras obtidas de humanos. Quinze suínos foram necropsiados. Amostras fecais suínas e humanas foram examinadas para detecção de ovos de Ascaris. Os parasitas adultos, obtidos nas necropsias, foram estudados através de microscopia eletrônica de varredura (MEV), e o gene mitocondrial codificante da enzima citocromo oxidase 1 (cox1) foi parcialmente amplificado e sequenciado para análises filogenéticas e de taxonomia molecular. A prevalência de Ascaris em amostras fecais de suínos foi 16/100 (16%), não sendo identificado nenhum caso de infecção por este parasita em humanos. A análise por MEV de parasitas recuperados de suínos demonstrou características morfológicas de Ascaris suum. As sequências nucleotídicas de cox1 foram compatíveis com A. suum. As sequências originais e de referência (obtidas no GeneBank) foram organizadas em clusters que não segregaram os parasitas por hospedeiro ou região geográfica. Os maiores haplogrupos foram dominados pelos haplótipos H01, H02 e H31. Nas comunidades estudadas, não foi evidenciada transmissão zoonótica de A. suum na interface suíno-humana.

Resumo Fundamento O manejo efetivo de pacientes com infarto agudo do miocárdio (IAM) é tempo-dependente. Objetivos Avaliar os impactos da implantação do atendimento pré-hospitalar nas taxas de internação e de mortalidade associadas ao IAM. Métodos Estudo retrospectivo e ecológico, que avaliou dados do Sistema Único de Saúde, de todos os 853 municípios de Minas Gerais, de 2008 a 2016. A assimetria excessiva da mortalidade geral e intra-hospitalar por IAM foi suavizada usando o método empírico de Bayes. Este estudo avaliou a relação entre o do Serviço de Atendimento Médico de Urgência (SAMU) em cada município e os seguintes 3 desfechos: taxa de mortalidade geral por IAM, taxa de mortalidade intra-hospitalar por IAM e taxa de internação por IAM, utilizando o modelo hierárquico de Poisson. As taxas foram corrigidas pela estrutura etária e destendenciadas pela sazonalidade e influências temporais. Foi adotado um intervalo de confiança de 95%. Resultados As taxas de mortalidade por IAM diminuíram ao longo do estudo, em média 2% por ano, com variação sazonal. A mortalidade intra-hospitalar também apresentou tendência de queda, de 13,81% em 2008 para 11,43% em 2016. A implantação do SAMU foi associada à diminuição da mortalidade por IAM ( odds ratio [OR] = 0,967, IC 95% 0,936 a 0,998) e mortalidade intra-hospitalar por IAM (OR = 0,914, IC 95% 0,845 a 0,986), sem associação significativa com internações (OR 1,003, IC 95% 0,927 a 1,083). Conclusão A implantação do SAMU esteve associada a uma redução modesta, mas significativa, na mortalidade intra-hospitalar. Esse achado reforça o papel fundamental do cuidado pré-hospitalar no cuidado do IAM e a necessidade de investimentos nesse serviço para melhorar os desfechos clínicos em países de baixa e média renda.

Abstract Background The effective management of patients with acute myocardial infarction (AMI) is time-dependent. Objectives To assess the impacts of the implementation of prehospital care on admission rates and mortality associated with AMI. Methods Retrospective, ecological study, which assessed data from the Brazilian Universal Health System, from all 853 municipalities of Minas Gerais, from 2008 to 2016. Excessive skewness of general and in-hospital mortality rates was smoothed using the empirical Bayes method. This study assessed the relationship between Mobile Emergency Care Service (SAMU) in each municipality and the following 3 outcomes: mortality rate due to AMI, AMI in-hospital mortality, and AMI hospitalization rate, using the Poisson hierarchical model. Rates were corrected by age structure and detrended by seasonality and temporal influences. A confidence interval of 95% was adopted. Results AMI mortality rates decreased throughout the study, on average 2% per year, with seasonal variation. AMI in-hospital mortality also showed a decreasing trend, from 13.81% in 2008 to 11.43% in 2016. SAMU implementation was associated with decreased AMI mortality (odds ratio [OR] = 0.967, 95% confidence interval [CI] 0.936 to 0.998) and AMI in-hospital mortality (OR = 0.914, 95% CI 0.845 to 0.986), with no relation with hospitalizations (OR = 1.003, 95% CI 0.927 to 1.083). Conclusion SAMU implementation was associated with a modest but significant decrease in AMI in-hospital mortality. This finding reinforces the key role of prehospital care in AMI care and the need for investments on this service to improve clinical outcomes in low- and middle-income countries.

ABSTRACT Poverty, malnutrition and neglected tropical diseases such as soil-transmitted helminthiases (STHs) interact in a multi-causal feedback network. This study aimed to assess the relationships between STHs, income and nutritional status of children in impoverished communities in the city of Caxias, Maranhao State, Northeastern Brazil. A cross-sectional survey (n=259 children) was carried out with the collection of fecal samples and assessment of sociodemographic, anthropometric, dietary and sanitation data. Hookworm infection and ascariasis presented prevalence rates of 14.3% and 9.3%, respectively. The logistic regression analysis showed that hookworm infection was more frequent in males (odds ratio [OR]=3.43; 95% confidence interval [CI]=1.45-8.08), children aged 11-15 years old (OR=3.72; 95% CI=1.19-11.62), children living in poor families (OR=2.44; 95% CI=1.04-5.68) and those living in rented houses (OR=5.74; 95%CI=1.91-17.25). Concerning ascariasis, living in the Caldeiroes community (OR=0.01; 95%CI=0-0.17) and belonging to the 11-15 years age group (OR=0.21; 95%CI=0.04-1.02) were protection factors. Poor children have a significantly lower frequency of consumption of meat, milk, vegetables, tubers and fruits than not poor children. The frequent consumption of meat, milk and tubers was associated with significant higher values in the parameter height-for-age, whereas the consumption of meat and milk positively influenced the weight-for-age. The frequencies of stunting, underweight and wasting were 8.1%, 4.9% and 2.9%, respectively. The multivariate model demonstrated that stunting was significantly associated with economic poverty (OR=2.82; 95%CI=1.03-7.70) and low weight was associated with male sex (OR=6.43; 95% CI=1.35-30.68). In conclusion, the study describes the interactions between the dimensions of development represented by income, STHs and nutritional status revealing the importance of raising income levels to improve the living conditions of families in impoverished communities in Northeastern Brazil.

The influence of copper on the ciliate Euplotes sp. and associated bacteria isolated from sediment samples of Guanabara Bay were investigated in bioassays. This region is highly affected by heavy metals such as copper, from solid waste constantly dumped in the bay and other sources such as industrial effluents, antifouling paints, atmospheric deposition and urban drainage, and even today there are few data on the metal toxicity to the ecosystem of the Bay of Guanabara. Bioassays were conducted to estimate the LC50-24 h of copper, in order to determine the concentration of metal bearing 50% of the population mortality. The results indicated that the concentrations of 0.05 and 0.009 mg L-1 presented no toxicity to Euplotes sp. The associated bacteria are tolerant to copper concentrations used in bioassays, and suggest that they could be used as a potential agent in the bioremediation of areas affected by copper.

La influencia del cobre sobre el ciliado Euplotes sp. y las bacterias asociadas aisladas de muestras de sedimentos de la bahía de Guanabara fueron investigados en bioensayos. Esta región está altamente afectada por metales pesados, como el cobre, a partir de residuos sólidos constantemente vertidos en la bahía y de otras fuentes, como los efluentes industriales, pinturas antiincrustantes, deposición atmosférica y drenaje urbano, y hasta hoy hay pocos datos sobre la toxicidad del metal para el ecosistema de la bahía de Guanabara. Se efectuaron bioensayos para estimar la CL50-24 h de cobre para determinar la concentración de metales que lleva al 50% de la mortalidad de la población. Los resultados indicaron que las concentraciones de 0,05 y 0,009 mg L-1, no representan toxicidad para Euplotes sp. Las bacterias asociadas son tolerantes a las concentraciones de cobre utilizadas en los bioensayos, lo que sugiere que podrían ser utilizadas como un agente potencial en la biorremediación de las zonas afectadas por el cobre.

O objetivo deste estudo foi avaliar o perfil dos bolsistas de produtividade científica da área de medicina no Conselho Nacional de Desenvolvimento Científico e Tecnológico. Foram analisados os currículos Lattes de todos os pesquisadores da área de medicina no triênio 2005-2007. As variáveis estudadas foram: gênero, categoria do bolsista, instituição de origem, tempo de conclusão do doutorado, artigos nacionais e estrangeiros com o respectivo Qualis, publicação de livros e capítulos, orientações de iniciação científica, mestrado e doutorado, e os periódicos utilizados para publicação. Entre os pesquisadores, houve prevalência do gênero masculino em relação ao feminino (1,94:1), com a maioria dos bolsistas na categoria 2 (51,4%), distribuídos por 13 estados da federação, com predomínio de São Paulo e Rio de Janeiro. Do total, 97,1% dos bolsistas estão vinculados a universidades e 49,4% deles concluíram o doutorado entre 6 e 15 anos. Na produção científica, verifica-se prevalência de artigos internacionais Qualis A e C e nacional B. Quanto à orientação, há prevalência na formação de mestres, seguida de iniciação científica e doutores. Os periódicos que concentraram a maior parte das publicações foram Brazilian Journal of Medical and Biological Research e Transplantation Proceedings. Estudos com metodologias similares poderão ser importantes para melhor aferição da produção científica brasileira em outras áreas do conhecimento, visto que existem poucos estudos nacionais sobre o perfil da produção científica gerada pelos bolsistas de produtividade científica do CNPq

Scientific output by Brazilian researchers has increased. The aim of this study was to evaluate the profile of medical research projects funded with scientific productivity grants from the Brazilian National Research Council (CNPq). The authors analyzed the Lattes curricula of all Brazilian medical researchers from 2005 to 2007. The independent variables were gender, type of grant, origin of the resident institution, year of the PhD, number of national and international published articles with respective Qualis classification, number of books and book chapters, number of Master's and PhD theses and scientific initiation monographs supervised, and the journals used for publication. The male/female ratio was 1.94:1. Most of the grantees were in the second class (51.4%), spread across 13 States of Brazil but with the largest contingent in Sao Paulo and Rio de Janeiro. 97.1% of the researchers were in universities, and 49.4% of them had completed their PhDs program in the previous 6 to 15 years. Articles were mainly published in Qualis A and C international journals and Qualis B Brazilian journals. The most common form of supervision was for Master's theses, followed by scientific initiation monographs and PhD dissertations. The two journals that published the most articles were the Brazilian Journal of Medical and Biological Research and Transplantation Proceedings. Studies in other areas with similar methodologies could provide better knowledge of Brazil's scientific output and help define strategies to meet research needs, since there are few Brazilian studies on the scientific output generated by CNPq grants

Trinta amostras de sedimento foram coletadas na Baía de Guanabara em agosto, novembro e dezembro de 2005. Foram analisados matéria orgânica, coliformes totais e fecais, bactéria heterotrófica, atividade respiratória bacteriana, atividade de esterase e do sistema transportador de elétrons. A matéria orgânica no sedimento superficial variou entre 4 e 6%. O maior nível de coliformes totais e fecais foi 1.7 x 10(5) MPN/g e 1.1 x 10³ MPN/g, respectivamente. Bactérias heterótrofas mostraram o maior valor na estação 7 (4.1 x 10(6) CFU/g) e o menor na estação 3 (7 x 10(4) CFU/g) (nordeste da Baía de Guanabara). A enzima esterase mostrou atividade em todos os sedimentos das 30 estações. A atividade do sistema transportador de elétrons variou entre 0.047 miL O2/h/g e 0.366 miL O2/h/g em seis estações. A cadeia microbiana bêntica é anaeróbia. Os processos como fermentação, desnitrificação e sulfato redução são responsáveis pelos ciclos biogeoquímicos no sedimento da Baía de Guanabara. O despejo de esgoto não tratado no Canal do Mangue da Baía de Guanabara tem favorecido o aumento da carga orgânica e a manutenção de coliformes totais e fecais no sedimento. Os coliformes fecais são bons indicadores de poluição e contaminação orgânica na coluna de água, mas os coliformes totais também poderão ser um grupo candidato para testes de qualidade de sedimento.

Thirty sediment samples were collected in Guanabara Bay in August, November and December 2005. The material was analyzed for organic matter, total and faecal coliforms, heterotrophic bacteria, bacterial respiratory activity, esterase activity and electron transport system activity. The organic matter in the superficial sediments (2 cm) ranged from 4 to 6%. The highest level of total coliforms and faecal coliforms were 1.7 x 10(5) MPN/g and 1.1 x 10³ MPN/g, respectively. Heterotrophic bacteria presented the highest values at station 7 (4.1 x 10(6) CFU/g) and the lowest values at station 3 (7 x 10(4) CFU/g) (northwestern part of Guanabara Bay). Esterase enzyme activity showed activity in the sediment of all 30 stations. Electron transport system activity ranged between 0.047 muL O2/h/g and 0.366 muL O2/h/g at six stations. The benthic microbial foodweb is anaerobic. Anaerobic processes such as fermentation, denitrification and sulphate-reduction are responsible for the biogeochemical cycles in the sediment of Guanabara Bay. The input of untreated sewage in the Mangue Channel outflow in Guanabara Bay has favored the increase of the organic load, and the maintenance of the total coliforms and faecal coliforms in the sediment. Faecal coliforms have been reported to be a good indicator of aquatic pollution and organic contamination in the water column, but total coliforms may be an ideal candidate group for sediment quality tests as well.
