Toads of the Rhinella genus have a pair of paratoid glands that store biological secretions of high toxicity and varied chemical composition, rich in biologically active compounds. The present work aimed to carry out the investigation of the metabolomic profile and evaluation of the biological potential of the secretion paratoid glands (PGS) from Rhinella granulosa. The paratoid secretion was collected in the Piauí state (Brazil), extracted with methanol and the extract was analyzed by ultra-performance liquid chromatography with quadrupole time-of-flight mass spectrometry. Fifty chemical constituents were identified. The extract showed cytotoxicity against tumor cell lines of the central nervous system (half maximal inhibitory concentration (IC50) = 1.9 µg mL-1) and prostate (IC50 = 1.6 µg mL-1), unsatisfactory antimicrobial potential (minimal inhibitory concentration (MIC) > 312 µg mL-1) and inhibited the enzyme acetylcholinesterase (IC50 = 5.119 mg mL-1). The results presented relevant information about the PGS and contributed to the understanding of the metabolomic and biological potential of R. granulosa.

Resumo Objetiva-se identificar concepções, experiências, métodos e técnicas em Vigilância Popular da Saúde, Ambiente e Trabalho (VPSAT). Trata-se de uma revisão integrativa com os descritores: participação da comunidade, vigilância em saúde, vigilância da saúde, saúde ambiental e saúde do trabalhador, envolvendo cinco bancos de dados: Biblioteca Virtual da Saúde, EBSCOhost, Embase, Scopus e Web Of Science. A revisão selecionou 15 estudos, a partir dos critérios de inclusão: experiências de vigilância com protagonismo comunitário; e exclusão: pesquisas sem dados primários e desenvolvidas apenas pelo serviço de saúde. Identificaram-se como bases teóricas e metodológicas dos estudos a ciência cidadã, educação popular e justiça ambiental; e experiências como mapeamentos e monitoramentos participativos; métodos como pesquisa-ação, “faça você mesmo” e investigação baseada na comunidade; e técnicas como “Photovoice” e Jornal Comunitário. Destacam-se as comunidades urbanas de baixa renda, indígenas, jovens e trabalhadores como protagonistas. O reconhecimento da VPSAT como importante fonte de dados e de intervenção pelos sistemas de saúde públicos e pela academia contribui para que a vigilância em saúde seja mais dialógica e efetiva.

Abstract The objective is to identify concepts, experiences, methods, and techniques in Popular Health, Environmental and Occupational Surveillance (VPSAT). This is an integrative review that used the descriptors: Community Participation, Public Health Surveillance, Environmental Health, and Occupational Health, using five databases: Virtual Health Library, EBSCOhost, Embase, Scopus and Web of Science. The review selected 15 studies, based on the inclusion criteria: surveillance experiences with community protagonism; and exclusion criteria: research without primary data and developed only by the health service. The theoretical and methodological bases of the studies were identified as citizen science, popular education, and environmental justice; experiences such as participatory mapping and monitoring; methods such as action research, ‘do-it-yourself’, and community-based research; and techniques such as “Photovoice” and Community Journal. Low-income urban communities, indigenous peoples, young individuals, and workers stand out as the protagonists. The recognition of the VPSAT as an important source of data and intervention by public health systems and academia contributes to making health surveillance more dialogic and effective.

RESUMO Este ensaio objetivou analisar como o Sistema Único de Saúde (SUS) tem lidado com o aumento da obesidade com indicação para cirurgia bariátrica no Brasil e apontar caminhos para a atenção à saúde integral dessa população. Fez-se um resgate do momento político-histórico de transformação epistemológica da obesidade e suas repercussões para indivíduos, sociedade, sistema de saúde e outros setores; expuseram-se alguns ataques sofridos pelo SUS, em especial os mais recentes, que afetam o já dificultoso acesso à cirurgia bariátrica; e refletiu-se sobre estratégias que buscam garantia da atenção à essa população e a sustentabilidade do sistema de saúde. Destacam-se os documentos produzidos pelo próprio Ministério da Saúde para a orientação do cuidado da obesidade, haja vista sua consonância com as evidências científicas mais atuais e sua utilização por outros países na construção de suas políticas. Ademais, reforça-se a importância do compartilhamento de responsabilidades entre todos os atores envolvidos; a regulamentação da publicidade voltada ao público que possui obesidade; o mandatório aumento de financiamento do SUS; e a utilização da avaliação em saúde de políticas, serviços e ações, para que se façam os ajustes necessários em tempo oportuno, garantindo uma melhor gestão do cuidado em saúde.

ABSTRACT This essay aims to analyze how the Unified Health System (SUS) has dealt with the increase in obesity with indication for bariatric surgery in Brazil, and to point out pathways to provide comprehensive health care for such population. There was a rescue of the political-historic moment of epistemological transformation of obesity and its repercussions for individuals, society, the health system, and other sectors; some attacks suffered by the SUS were exposed, especially the most recent ones, which affect the already difficult access to bariatric surgery; and it reflect on strategies that seek to guarantee care for that population and the sustainability of the health system. The documents produced by the Ministry of Health to guide obesity care stand out, given their consonance with the most current scientific evidence, and their use by other countries in the construction of their policies. Furthermore, it is reinforced the importance of sharing responsibilities among all the actors involved; the regulation of advertising aimed at the public with obesity; the mandatory increase in SUS funding; and the use of health evaluation of policies, services, and actions, so that the necessary adjustments are made in a timely manner and guarantee a better management of health care.

Abstract This paper presents a synthesis of critical analyses about the lack of transparency and the problem of biases in the use of artificial intelligence (AI) systems as support or for effective judicial decision making from the Brazilian context, based on researches coordinated by the authors. From the methodological point of view, bibliographic, documentary and empirical researches were developed in the investigations. Analytical-interpretative studies of the materials gathered were made in order to ground the considerations. In the approach, a brief description of the state of the art regarding the use of AI by the national judiciary was elaborated, followed by analyzes on the opacity of algorithms and cognitive biases, according to topics related to the structural incompatibility between data processing by AI and the application of Law; the inability of the AI to make value judgments; the accentuation of cognitive biases by AI; and lack of transparency in relation to the algorithms and data used by the AI. At the end, proposals are presented to face the problems related to the use of AI for judicial decision making.

Resumo Neste artigo, é apresentada a síntese de análises críticas realizadas em pesquisas coordenadas pela autora e pelo autor sobre a falta de transparência e o problema dos vieses no uso de sistemas de inteligência artificial (IA) como apoio ou para a efetiva tomada de decisão judicial, a partir do contexto brasileiro. Do ponto de vista metodológico, foram desenvolvidas pesquisas bibliográficas, documentais e empíricas nas investigações. Foram feitos exames analítico-interpretativos acerca dos materiais levantados para alicerçar as considerações. Na abordagem, foi elaborada uma breve descrição acerca do estado da arte quanto ao uso de IA pelo Judiciário nacional, seguida pelas análises sobre a opacidade dos algoritmos e os vieses cognitivos, conforme os tópicos relacionados com a incompatibilidade estrutural entre o processamento de dados pela IA e a aplicação do Direito; a incapacidade da IA realizar juízos de valor; a acentuação dos vieses cognitivos pela IA; e a falta de transparência em relação aos algoritmos e aos dados utilizados pela IA. Ao final, são apresentadas propostas para enfrentamento dos problemas com o uso de IA para a tomada de decisão judicial.

Abstract In the last few decades, there have been considerable improvements in the diagnosis and care of Duchenne muscular dystrophy (DMD), the most common childhood muscular dystrophy. International guidelines have been published and recently reviewed. A group of Brazilian experts has developed a standard of care based on a literature review with evidence-based graded recommendations in a two-part publication. Implementing best practice management has helped change the natural history of this chronic progressive disorder, in which the life expectancy for children of the male sex in the past used to be very limited. Since the previous publication, diagnosis, steroid treatment, rehabilitation, and systemic care have gained more significant insights with new original work in certain fields. Furthermore, the development of new drugs is ongoing, and some interventions have been approved for use in certain countries. Therefore, we have identified the need to review the previous care recommendations for Brazilian patients with DMD. Our objective was to create an evidence-based document that is an update on our previous consensus on those topics.

Resumo Nas últimas décadas, houve progressos significativos no diagnóstico e no tratamento da distrofia muscular de Duchenne (DMD), considerada a distrofia muscular mais comum na infância. Diretrizes internacionais foram publicadas e revisadas recentemente. Um grupo de especialistas brasileiros desenvolveu um padrão de atendimento baseado em revisão de literatura, com recomendações graduadas pautadas em evidências compiladas em uma publicação dividida em duas partes. A implementação de melhores práticas de manejo ajudou a modificar a história natural desta doença crônica, progressiva, que, no passado, oferecia uma expectativa de vida muito limitada para crianças do sexo masculino. Desde a publicação desse consenso anterior, o diagnóstico, o tratamento com esteroides, a reabilitação e os cuidados sistêmicos ganharam novas possibilidades a partir da divulgação dos resultados de trabalhos originais em algumas dessas áreas. Além disso, as pesquisas e o desenvolvimento de novos fármacos estão em andamento, e algumas intervenções já foram aprovadas para uso em determinados países. Nesse contexto, identificamos a necessidade de rever as recomendações anteriores sobre o manejo dos pacientes brasileiros com DMD. Nosso objetivo principal foi elaborar uma atualização baseada em evidências sobre esses tópicos do consenso.

Abstract The puma population is constantly decreasing, and cloning by somatic cell nuclear transfer can be used to conserve the species. One of the factors determining the success of the development of cloned embryos is the cell cycle stage of the donor cells. We evaluated the effects of full confluency (~100%), serum starvation (0.5% serum), and roscovitine (15 µM) treatments on the cell cycle synchronization in G0/G1 of puma skin-derived fibroblasts by flow cytometric analysis. Also, we assessed the effects of these synchronization methods on morphology, viability, and apoptosis levels using microscopy tools. The results showed that culturing the cells to confluence for 24 h (84.0%), 48 h (84.6%), and 72 h (84.2%) and serum starvation for 96 h (85.4%) yielded a significantly higher percentage of cells arrested in the G0/G1 (P < 0.05) phase than cells not subjected to any cell cycle synchronization method (73.9%). Nevertheless, while serum starvation reduced the percentage of viable cells, no difference was observed for the full confluence and roscovitine treatments (P > 0.05). Moreover, roscovitine for 12 h (78.6%) and 24 h (82.1%) was unable to synchronize cells in G0/G1 (P > 0.05). In summary, full confluency induces puma fibroblast cell cycle synchronization at the G0/G1 stage without affecting cell viability. These outcomes may be valuable for planning donor cells for somatic cell nuclear transfer in pumas.

Abstract This publication is an update of the “Consensus on the therapeutic management of atopic dermatitis - Brazilian Society of Dermatology” published in 2019, considering the novel, targeted-oriented systemic therapies for atopic dermatitis. The initial recommendations of the current consensus for systemic treatment of patients with atopic dermatitis were based on a recent review of scientific published data and a consensus was reached after voting. The Brazilian Society of Dermatology invited 31 experts from all regions of Brazil and 2 international experts on atopic dermatitis who fully contributed to the process. The methods included an e-Delphi study to avoid bias, a literature search and a final consensus meeting. The authors added novel approved drugs in Brazil and the indication for phototherapy and systemic therapy for AD. The therapeutical response to systemic treatment is hereby reported in a suitable form for clinical practice and is also part of this updated manuscript.

RESUMO: O presente estudo tem como objetivo geral analisar o acompanhamento pedagógico hospitalar realizado no setor de oncologia de um hospital público do Recife no que se refere às crianças em processo de alfabetização. Os objetivos específicos são verificar o conhecimento dos pais e professoras da oncologia sobre as leis que asseguram a continuidade do processo ensino-aprendizagem no ambiente hospitalar; analisar se e como ocorre a relação entre a escola e as professoras da oncologia no processo de alfabetização das crianças; averiguar as ações do pedagogo no atendimento hospitalar no setor de oncologia. Realizamos pesquisa com abordagem qualitativa utilizando como ferramentas de coleta de dados, observações e entrevistas semiestruturadas. Através do estudo, foi possível constatar que o acesso à pedagogia hospitalar proporciona a criança e ao adolescente em situação de hospitalização, não só a continuidade do processo de escolarização, mas oferece perspectiva de futuro, contribui para a tão almejada cura, para o prosseguimento de projetos de vida. Ressaltamos a necessidade do desenvolvimento de novas pesquisas que visem contribuir com a continuidade do processo ensino-aprendizagem das crianças em tratamento, em especial às que estão em processo de alfabetização. RESUMEN: El presente estudio tiene como objetivo general analizar el seguimiento pedagógico hospitalario realizado en el sector de oncología de un hospital público de Recife en relación con los niños en proceso de alfabetización. Los objetivos específicos son verificar el conocimiento de los países y de los profesores de oncología sobre las leyes que aseguran la continuidad del proceso de enseñanza-aprendizaje en el ambiente hospitalario; analizar si y cómo ocurre la relación entre la escuela y los profesores de oncología en el proceso de alfabetización de los niños; averiguar las acciones del pedagogo en la atención hospitalaria en el ámbito de la oncología. Realizamos una investigación con un enfoque cualitativo, utilizando como instrumentos la recopilación de datos, las observaciones y las entrevistas semiestructuradas. A través del estudio, fue posible verificar que el acceso a la pedagogía hospitalaria proporciona a los niños y adolescentes en situación de hospitalización, no sólo la continuidad del proceso de escolarización, sino que también ofrece perspectiva de futuro, contribuye a la tan deseada cura, a la continuación de los proyectos de vida. Enfatizamos la necesidad de desarrollar nuevas investigaciones que tengan como objetivo contribuir a la continuidad del proceso de enseñanza-aprendizaje de los niños en tratamiento, especialmente los que están en proceso de alfabetización. ABSTRACT: The present study aims at the general analysis of the pedagogical hospital monitoring carried out in the oncology sector of a public hospital in Recife with regard to children in the literacy process. The specific objectives are to verify the knowledge of parents and teachers of oncology about the laws that ensure the continuity of the teaching-learning process in the hospital environment; analyze whether and how the relationship between the school and how oncology teachers occur in the children's literacy process; investigate the actions of the educator in hospital care in the oncology sector. We conducted research with a qualitative approach, using data collection, decoding and semi-structured changes as tools. Through the study, it was possible to verify that the access to hospital pedagogy offers a child and adolescent in a situation of hospitalization, not only the continuity of the schooling process, but offers a perspective for the future, I contributed to a cure so desired, for the continuation of life projects. We emphasize the need to develop new research aimed at contributing to the continuity of the teaching-learning process of children undergoing treatment, especially those who are in the process of literacy. ABSTRACT Objective: To create and validate a model for predicting septic or hypovolemic shock from easily obtainable variables collected from patients at admission to an intensive care unit. Methods: A predictive modeling study with concurrent cohort data was conducted in a hospital in the interior of northeastern Brazil. Patients aged 18 years or older who were not using vasoactive drugs on the day of admission and were hospitalized from November 2020 to July 2021 were included. The Decision Tree, Random Forest, AdaBoost, Gradient Boosting and XGBoost classification algorithms were tested for use in building the model. The validation method used was k-fold cross validation. The evaluation metrics used were recall, precision and area under the Receiver Operating Characteristic curve. Results: A total of 720 patients were used to create and validate the model. The models showed high predictive capacity with areas under the Receiver Operating Characteristic curve of 0.979; 0.999; 0.980; 0.998 and 1.00 for the Decision Tree, Random Forest, AdaBoost, Gradient Boosting and XGBoost algorithms, respectively. Conclusion: The predictive model created and validated showed a high ability to predict septic and hypovolemic shock from the time of admission of patients to the intensive care unit.

RESUMO Objetivo: Criar e validar um modelo de predição de choque séptico ou hipovolêmico a partir de variáveis de fácil obtenção coletadas na admissão de pacientes internados em uma unidade de terapia intensiva. Métodos: Estudo de modelagem preditiva com dados de coorte concorrente realizada em um hospital do interior do nordeste brasileiro. Foram incluídos pacientes com 18 anos ou mais sem uso de droga vasoativa no dia da admissão e que foram internados entre novembro de 2020 e julho de 2021. Foram testados os algoritmos de classificação do tipo Decision Tree, Random Forest, AdaBoost, Gradient Boosting e XGBoost para a construção do modelo. O método de validação utilizado foi o k-fold cross validation. As métricas de avaliação utilizadas foram recall, precisão e área sob a curva Receiver Operating Characteristic. Resultados: Foram utilizados 720 pacientes para criação e validação do modelo. Os modelos apresentaram alta capacidade preditiva com área sob a curva Receiver Operating Characteristic de 0,979; 0,999; 0,980; 0,998 e 1,00 para os algoritmos de Decision Tree, Random Forest, AdaBoost, Gradient Boosting e XGBoost, respectivamente. Conclusão: O modelo preditivo criado e validado apresentou elevada capacidade de predição do choque séptico e hipovolêmico desde o momento da admissão de pacientes na unidade de terapia intensiva.

The main objective of this work was to develop a nanoemulsion based on linseed oil and betulinic acid, stabilized with Pluronic F127 and polyglycerol polyricinoleate, for anticancer applications. The nanoemulsions were synthesized by ultrasound and evaluated for in vitro cytotoxicity, particle size, polydispersity index, zeta potential, morphology, encapsulation efficiency, storage stability, rheology and in vitro release kinetics. In vitro cytotoxicity assays were performed by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay (72 h) against HCT-116 (colorectal carcinoma), SNB-19 (glioblastoma), NCI-H460 (lung carcinoma) and L-929 (normal fibroblasts) cells. The determination of 50% inhibitory concentration (IC50) showed an increased selectivity for the emulsified betulinic acid when compared to its free form for the HCT-116 cells. The IC50 values for the synthesized nanoemulsions showed a range from 3.2 to 3.7 µM (HCT-116), 5.6 and 11.5 µM (NCI-H460), 5.8 and 7.3 µM (SNB-19) and > 16.5 µM for normal fibroblasts. In the 48 h in vitro release assays, it presented controlled release explained by the Korsmeyer-Peppas model, releasing 572.25 and 619.95 µg of betulinic acid in a controlled way, generating promising perspectives for the prolonged release of betulinic acid in anticancer applications.

The genus Chamaecrista comprises more than 330 species, with only a few studies on their chemical composition and biologic activities. In this study, the phytochemical profile of leaf, stems, and fruits extracts of the C. duckeana were examined by UPLC ESI HRMS analysis to determine possibly bioactive constituents. The antioxidant activity was carried out through in vitro assay, by the sequestration of the free radical DPPH. To evaluate the cytotoxic activity of the extracts, an MTT assay was used and the IC50 was determined against HL60 and RAJI cell lines. The metabolic profiles of the botanical parts are dominated by flavonoid class, highlighting isoflavonoids such as daidzin and ononin. All these compounds are reported for the first time in C. duckeana. The extracts presented antioxidant potential, and the activity of the stems extract was higher than the standard butylated hydroxytoluene. In the cytotoxic assay, only HL60 line (leukemia) had growth inhibition over 80%. The stems presented more expressive cytotoxicity with IC50 of 137.3 (104.6-180.1) and 106.8 (96.52-118.3) μmol. L-1 for HL60 and RAJI, respectively. In conclusion, the present work provides an in-depth knowledge about the chemical profile of C. duckeana, a species rich in bioactive secondary metabolites with cytotoxic activity.

Resumo Conhecer o ambiente alimentar a que as famílias beneficiárias do Programa Bolsa Família (PBF) estão expostas é necessário para identificar a exposição a ambientes que favoreçam piores escolhas alimentares. O objetivo deste trabalho foi caracterizar o ambiente alimentar comunitário da zona urbana do município de Ouro Preto e do entorno das residências das famílias beneficiárias do PBF. Trata-se de um estudo ecológico, conduzido com dados do Sistema de Cadastro Único e da Secretaria Estadual da Fazenda, ambos de 2014, e do Censo 2010. Foram realizadas análises espaciais para avaliar as aglomerações de famílias beneficiárias e de estabelecimentos. Os setores censitários (SC) periféricos e com menor renda per capita apresentaram pouco ou nenhum local de venda de alimentos, bem como maior aglomeração de famílias beneficiárias do PBF, que estão mais próximas a estabelecimentos considerados não saudáveis (mediana: 65,73 metros). Verificou-se maior concentração de locais de venda de alimentos, sobretudo não saudáveis, nos SC de localização central e de maior renda per capita (59,2%). Nesse caso, torna-se relevante o diálogo com outras políticas públicas interrelacionadas com a nutrição, visando reduzir as iniquidades e contribuir para melhorar o acesso físico a estabelecimentos de venda de alimentos saudáveis.

Abstract Knowing the food environment to which the households benefiting from the Bolsa Familia Program (PBF) are exposed is necessary to identify exposure to environments that favor worse food choices. This work aimed to characterize the community food environment of the urban area of the municipality of Ouro Preto (MG), Brazil, and the surroundings of the homes of households that are beneficiaries of the PBF. This ecological study was conducted with data from the Single Registry and the State Treasury Department Systems, both from 2014, and from the 2010 Census. Spatial analyses were conducted to evaluate the clusters of beneficiary households and establishments. The peripheral and with lower per capita income census tracts (CTs) had little or no establishment for food sales, and greater agglomeration of PBF beneficiary households closer to unhealthy establishments (median: 65.73 meters). There was a higher concentration of - mainly unhealthy - food sales establishments in the central CTs and higher per capita income (59.2%). Given the above, it becomes important to dialogue with other nutrition-related public policies to reduce inequalities and improve physical access to healthy food sales establishments.
