Foi realizado um estudo controlado para avaliar a eficácia terapêutica e a tolerância do nifurtimox e do benznidazole em pacientes com a doença de Chagas crônica. Todos os pacientes tinham as reações de imunofluorescência e fixação do complemento positivas para anticorpos anti-T. cruzi e pelo menos dois xenodiagnósticos positivos em três realizados, antes do tratamento, e foram submetidos a exames clínicos, eletrocardiográficos e radiográficos do coração e do esôfago. De 77 pacientes estudados, 27 foram tratados com nifurtimox, 26 com benznidazole, ambos na dose de 5mg/kg/dia, durante 30 dias consecutivos, e 24 receberam um placebo em comprimidos semelhantes aos do benznidazole. Dos 77 pacientes, 64 (83,1%) completaram o tratamento: 23 (88,4%) com benznidazole, 19 (70,3%) com nifurtimox e 22 (91,6%) com placebo. Os pacientes foram avaliados clinicamente, sorologicamente e parasitologicamente (seis xenodiagnósticos no período de um ano após o tratamento). O grupo do benznidazole mostrou apenas 1,8% de xenodiagnósticos positivos pós-tratamento, o grupo do nifurtimox 9,6% e o do placebo 34,3%. Todas as reações sorológicas continuaram positivas e não houve alterações clínicas, eletrocardiográficas ou radiológicas um ano após o tratamento.

A controled clinical trial was carried out to evaluate the therapeutic efficacy and tolerance of nifurtimox and benznidazole in patients with chronic Chagas' disease. All patients had immunofluorescence and complement fixation reactions positives for T. cruzi antibodies and at least two xenodiagnoses positives in three performed before treatment, and they were submitted to clinical examinations, ECG and X-ray of the heart and esophagus. Of 77 patients studied, 27 were treated with nifurtimox and 26 with benznidazole in the dosage of 5m/kg/day for 30 consecutives days, and 24 received a placebo in tablets similar to benznidazole. From the 77 patients, 64 (83,1%) accomplished the treatment: 23 (88,4%) with benznidazole, 19 (70,3%) with nifurtimox and 22 (91,6%) with placebo. The patients were evaluated, clinically, serologically and parasitologically (six xenodiagnoses within one year after treatment). The benznidazole group showed only 1,8% of positive xenodiagnoses post-treatment, the nifurtimox 9,6% and the placebo 34,3%. All serologic reations continued positives and there were no clinical, ECG or X-ray changes one year after treatment.

To evaluate the results of xenodiagnosis in chronic Chagas patients infected for ten years or over in an area where transmission has been stemmed as well as the performance of these tests applied one or more times to determine the presence of the paraiste in serum-positive patients for Trypanosoma cruzi infection, 570 xenodiagnosis were performed in 246 patients by exoposing each pacient to 40 Triatoma infestans nymphs of 3 rd/4th stage once, twice or three times, at 30 days intervals. The 570 xenodiagnosis showed overall positive results in 50.7% with a peak 78% in patients under 20 years of age, and 60.5% in those over 60. Of the l58 patients who underwent three xenodiagnosis, 51 (32.3%) had three positive tests, 48 (30.3%) had all negative results, and the remainder had alternating positve and negative findings. There was no difference in number of positive results between the 1st, 2nd and 2rd tests; however, the 1st and 2nd trials added up to 53.2% and the sum total of all three trials was 57.7%.

Foi realizado um estudo seccional de campo do tipo "caso controle" sobre a morbilidade da doença de Chagas em quatro áreas endêmicas no Brasil, duas no Estado de Minas Gerais, uma no Estado do Piauí e outra no Estado da Paraíba, incluindo 716 pares de indivíduos da mesma idade e sexo, cada par constando de um indivíduo com sorologia positiva e outro com sorologia negativa para a infecção chagásica. Com esse tipo de estudo procurou-se determinar o componente exclusivamente chagásico na morbidade da doença em diferentes áreas do país. O gradiente de manifestações clínicas e alterações eletrocardiográficas entre o grupo com sorologia positiva e outro com sorologia negativa, estudado em 264 pares na área de Iguatama-Paris e 274 em Virgem da Lapa, ambas em Minas Gerais, e em 109 pares estudados nas localidades de Colônia e Oitis, em Oeiras, Piauí e em 69 nos municípios de Aguiar e Boqueirão dos Cochos na Paraíba, mostra nitidas diferenças regionais na morbilidade da doença. Nas áreas de Minas Gerais, embora a transmissão natural da infecção estivesse interrompida há 15 e 5 anos, respectivamente, o grau de morbilidade cardiológica pelo componente chagásico, considerando apenas as alterações eletrocardiográficas mais expressivas e específicas, no momento do estudo, foi de aproximadamente 30%, enquanto em Oeiras, no Piauí e em Aguiar e Boqueirão dos Cochos, na Paraíba, mesmo com transmissão ativa da infecção, a morbidade cardiológica pelo componente chagásico foi inferior a 15 e 10%, respectivamente

A cross-section study, type "case-control" was performed in four endemic areas for Chagas' disease in Brazil, two of them in Minas Gerais State, one in the State of Piauíand the other in the State of Paraíba. A total of 716 pairs of persons with same age and sex, one with a positive serology and the other with a negative serology for Chagas' disease were included in the study. This kind of study was performed to determine the chagasic component responsible for the morbidity of the disease in different areas of the country. The clinical symptoms and signs and the eletrocardiographic changes between the serologic positive and serologic negative groups studied in 264 pairs of persons in the district of Iguatama-Pains and 274 in Virgem da Lapa, both in Minas Gerais State, when compared with the same parametres applied in 109 pairs studied in the localities of Colônia and Oitis in the district of Oeiras, State of Piauí and 69 in the districts of Aguiar and Boqueirão dos Cochos in the State of Paraíba, both in northeast of Brazil, showed a marked difference in the morbidity of the disease in different areas. In the areas of Minas Gerais State, although the natural transmission of Chagas' disease had been interrupted since 15 years ago in Iguatama-Pains and 5 years ago in Virgem da Lapa, the cardiac morbidity by the chagasic component was approximately of 30% at he moment of the study. In the areas of Piauí and Paraíba States, although the natural transmission of Chagas' disease is still active, the cardiac morbidity by the chagasic component was lower than 15 and 10%, respectively.

Descreve-se uma modificação da reação de imunofluorescencia indireta para o diagnóstico sorológico da paracoccidioidomicose na qual células formolizadas da fase leveduriforme do Paraccocidioides brasiliensis foram empregadas como antígeno. Outra modificação foi a de simplificar a técnica da reação sem prejuízo da sua sensibilidade. O teste pode ser considerado como suficientemente sensível e de fácil execução em qualquer laboratório equipado para reações de imunofluorescencia.

A modification of the indirect fluorescence test (IFT) for serological diagnosis of Paracoccidioidomycosis is described in which formolized budding forms (yeast cells) of the fungus Paracoccidioides brasiliensis are used as antigen. A further modification introduced is a less eiaborate technical procedure of the test without lowering the sensitivity of the reaction. The test may be considered as adequateiy accurate and easy to perform in any laboratory with immunofluorescence facilities.

Um inquérito sorológico pela reação de imunofluorescência indireta para doença de Chagas, foi realizado, incluindo 13.254 crianças de 168 escolas primárias sorteadas em 20 municípios do Estado do Rio de Janeiro. Os casos positivos pela imunofluorescência eram confirmados pela reação de fixação do complemento e submetidos a exame clínico, investigando-se em seguida suas residências quanto à existência ou não de triatomíneos. Somente nos municípios de Magé e Araruama houve um número significativo de crianças com sorologia positiva. Do total de amostras estudadas somente 143 (1%) foram positivas pela imunofluorescência e confirmadas pela reação de fixação do complemento para doença de Chagas. Destas, 57 eram de Magé, 58 de Araruama, 4 de Cabo Frio, 3 de Maricá, 1 de Saquarema e 1 de Casimiro de Abreu. Nenhuma das crianças examinadas clinicamente apresentava sinais ou sintomas importantes para a doença de Chagas e não foram encontrados triatomíneos em suas residências, admitindo-se que tenham sido infectadas acidentalmente ou pela existência no passado de triatomíneos domiciliares. Embora a doença de Chagas não pareça um problema importante na região estudada, recomendam-se investigações em escala mais ampla, principalmente nos municípios de Araruama e Magé para esclarecer os percentuais de positividade soro lógica alí encontrados e o mecanismo de transmissão da doença.

A serological survey for Chagas' disease was carried out in school children in the Rio de Janeiro State, a zone considered as non-endemic for the infection. A total of 168 schools in 20 municipalities have been visited and 13,254 blood samples were obtained. The blood eluates were screened by the indirect fluorescence test (IFT), and all positive samples were checked and confirmed in sera by the complement fixation test (CFT). AH serologically positive children were subject to a clinical scrutiny, and the houses where the children lived have been searched for triatomine bugs. Only in two municipalities, Magé and Araruama, there was a significant number of children found positive. The total number of reactive samples by IFT and CFT from 13,004 blood samples screened was 143 (1.00 per cent). No serious clinicai symptoms suggestive of Chagas' disease have been found in any of the positive children, and no triatomine bugs were discovered in the dwellings where the children lived. The overall small percentage of children with positive serology postulates that the infection is not a serious health problem in the area investigated. It is recommended, however, to carry out a more detailed study in Magé and Araruama to find the reason for the relatively high percentage of serologically positive children encountered in these two municipalities.

O presente trabalho baseia-se no estudo de um foco autóctone da doença de Chagas no município de Caxias, no Estado do Rio de Janeiro. O estudo incluiu a pesquisa de triatomíneos nos domicílios das localidades de Piranema, Amapá e Capivarí, inquérito sorológico pelas reações de imuno-fluorescência e fixação do complemento para doença de Chagas na população dessas localidades e xenodiagnóstico dos casos com sorologia positiva e em animais domésticos das residências onde foram encontrados triatomíneos infectados. De 115 casas pesquisadas na localidade de Piranema, 15 (13,0%) estavam infestadas com Triatoma infestans. Nas demais localidades não foram encontrados triatomíneos. De um total de 1936 T. infestans capturados, 146 estavam infectados com Trypanosoma cruzi. Do ponto de vista epidemiológico é importante assinalar a adaptação do Triatoma infestans ao domicílio humano numa área com grande densidade de população na baixada fluminense, em município próximo ao litoral do Estado do Rio de Janeiro.

An area believed to be an autochthonous focus for Chagas' disease was investigated in the municipality of Caxias, Rio de Janeiro State. The study included search for domestic triatomine bugs, serological test (IFT and CFT) in persons in whose house infested bugs were discovered, detailed clinicai examination and xenodiagnosis test of ali serologicall/ yy positive persons, and xenodiagnosis test on dogs from households in which infected triatomine bugs have been found. Only in one of the locatities (Piranema) domestic Triatoma infestans have been discovered. some of which were infected with T. cruzi. A small number of persons (mostly children) had a positive serologicál test for Chagas’ disease, but in all of them the infection was clinically asymptomatic. From two dogs, belonging to a household in which serologically positive children and infected T. infestans were discovered, T. cruzi was isolated by xenodiagnosis. The importunt epidemiological information obtained from this investigation was the discovery of domestic adaptation of T. infestans in an area with dense population .and with very low social and sanitary conditions, in a locality considered as non-endemic for the infection.
