Background: Temporomandibular-ankylosis brings extensive limitations on the patient quality of life. Surgical treatment is frequently necessary associated with a continuous rehabilitation. The anatomy of this region and potential complications of this surgery must be thoroughly known to avoid iatrogenic injuries. Case presentation: A young patient underwent surgical treatment of congenital bilateral ankylosis with disastrous consequences such as facial palsy, deafness and cerebrospinal leaks on the right side related to intraoperative trauma involving structures of the external, middle and inner ear. Conclusion: Since there is no corrective treatment for this type of injuries, the craniomaxillofacial surgeon and other professionals who carry out interventions of this nature need perfect knowledge of the anatomy of the temporal bone and lateral skull base, taking into account the risk of surgical disasters like the one here reported.

Fundamento: La anquilosis temporomandibular se asocia a importantes limitaciones de la calidad de vida del paciente. Con frecuencia, es necesario un tratamiento quirúrgico asociado a una rehabilitación continua. Para evitar las lesiones yatrogénicas, se requieren conocimientos exhaustivos de la anatomía de esta región y de las complicaciones potenciales de la cirugía. Presentación del caso: Un paciente joven se sometió a tratamiento quirúrgico de una anquilosis bilateral congénita con consecuencias catastróficas, como parálisis facial, sordera y extravasación de líquido cefalorraquídeo en el lado derecho relacionada con el traumatismo intraoperatorio, que afectó a las estructuras del oído externo, medio e interno. Conclusión: Puesto que no se dispone de un tratamiento corrector para este tipo de lesiones, tanto el cirujano experto en cabeza y cuello como otros profesionales que efectúan intervenciones de esta naturaleza necesitan unos conocimientos óptimos sobre la anatomía del hueso temporal y la base del cráneo, y deben tener en cuenta el riesgo de una catástrofe quirúrgica como la descrita en este paciente.

OBJETIVO: caracterizar mastigação, fase oral da deglutição e possíveis adaptações funcionais observadas nos portadores de Paralisia Facial Periférica. MÉTODO: participaram desta pesquisa 30 indivíduos com Paralisia Facial Periférica grau IV, com história de até 30 dias, sem distinção de etiologia e divididos em três grupos, os que apresentavam a paralisia em até 10 dias, de 11 a 20 e de 20 a 30 dias. As funções mastigação e fase oral da deglutição foram avaliadas tanto com alimento sólido e como com água natural. Os indivíduos responderam questões relacionadas às dificuldades imediatamente após a instalação da paralisia. Os dados foram analisados estatisticamente pelo Teste da Razão de Verossimilhança e pelo Teste Exato de Fisher. RESULTADOS: foram constatadas alterações nas funções de mastigação e fase oral da deglutição pela diminuição do tônus no músculo orbicular dos lábios e do músculo bucinador, que diminuindo a pressão intra-oral, favorece o escape de alimento e líquido. À observação da Fonoaudióloga a variável "derrama líquido enquanto bebe" apresentou dados estatisticamente significante (p=0,003) nos três grupos estudados. A variável "acúmulo de alimento entre os dentes e a gengiva no lado paralisado" foi estatisticamente significante nos grupos de 11 a 20 dias (p= 0,002). CONCLUSÃO: os indivíduos da amostra mastigam no lado paralisado com dificuldade, mediante ciclos mastigatórios lentos e inconsistentes. Ocorre um incremento nos movimentos de língua para limpeza de resíduos retidos no vestíbulo oral no lado paralisado. Este é o sintoma que mais incomoda o paciente. Apresentam dificuldade no beber de forma contínua. Desenvolvem adaptações para compensar suas dificuldades funcionais.

PURPOSE: to describe mastication, swallowing oral phase and possible functional adaptations observed in Facial Peripheral Paralysis subjects. METHOD: there were 30 subjects with grade IV Facial Peripheral Paralysis, with at the most 30 days paralysis history and no etiology differentiation. They were separated in three groups, 1 to 10 days paralysis, 11 to 20 days and 21 to 30 days. Mastication and swallowing oral phase functions were assessed with both solid food and water. Participants answered questions related to the difficulties right after the paralysis. Data were statistically analyzed using the Likelihood Ratio Test and Fisher Exact Test. RESULTS: changes were observed in mastication and swallowing oral phase due to the lowering of lips tonus, orbicular muscle and buccinator muscle that allows the escaping of food and liquid by decreasing intra-oral pressure. To the speech therapist's observation "spill liquid while drinking" presented statistically significant data (p=0,003) in the three observed groups. Variable "accumulate food between teeth and gums" was statistically significant in groups of 11 to 20 days (p= 0,002). CONCLUSION: sample subjects chew with difficulty in the paralyzed side, showing slow and inconsistent mastication cycles. There is an increased tongue movements for cleaning the residues kept in oral vestibule in the paralyzed side. This is the most annoying symptom according to the patients. They develop adaptation strategies to compensate their functional difficulties.

OBJETIVO: avaliar o protocolo proposto de reabilitação neuromuscular orofacial para paralisia facial periférica. MÉTODOS: observação clínica de 20 pacientes com paralisia grau IV, encaminhados para reabilitação orofacial no Ambulatório de Paralisia Facial do Hospital de Clínicas da Unicamp: estudo longitudinal prospectivo. A constatação da evolução funcional ou não, na reabilitação, baseou-se na melhora do tônus muscular, cuja variação foi aferida mediante modificação no ângulo da comissura labial. O estudo se fez sob imagens da documentação fotográfica pré (após quinze dias de instalação da paralisia facial) e pós-tratamento de um ano. Para comprovação da eficácia da reabilitação, mediu-se o ângulo da comissura labial pré e pós-reabilitação. O grupo estudado foi comparado a um grupo controle composto de nove sujeitos com paralisia facial grau IV, não submetidos à reabilitação orofacial. Os dados foram analisados estatisticamente pelo teste emparelhado das amostras (T-Student). RESULTADOS: a média de redução do ângulo da comissura labial, com o tratamento, foi de 7,9º, estatisticamente significantes (p<0.001). Para o grupo controle a média das medidas angulares foi de 100.9º + 1.9. Esse valor não diferencia dos medidos inicialmente no grupo estudado (p=0,723). Foram significantemente mais altos quando comparados à média dos valores angulares finais do grupo estudado com pacientes tratados (p=0.001). CONCLUSÃO: o protocolo de reabilitação empregado nesta amostra promoveu incremento marcante do tônus muscular, com melhora no repouso facial.

PURPOSE: to evaluate a proposed protocol of orofacial neuromuscular rehabilitation for peripheral facial paralysis. METHODS: clinical observation of 20 patients with fourth-degree paralysis, sent to orofacial rehabilitation in the Facial Paralysis Ambulatory from Hospital das Clínicas at UNICAMP: prospective longitudinal study. The verification of functional evolution or not, in the rehabilitation, was based on the improvement of muscular tonus, whose variation was measured by the modification in the labial commissure angle. The study was done with images from photographical documentation prior to (fifteen days after the installation of facial paralysis) and post one-year treatment. As proof of the effect concerning rehabilitation, the pre and post rehabilitation labial commissure angle was measured. The studied group was compared to a control group composed of nine subjects with fourth-degree orofacial paralysis, who did not undergo orofacial rehabilitation. The data were statistically analyzed by the two tail paired sample test (Student's T test). RESULTS: the average of labial commissure angle was 7.9º after treatment, with statistically significant difference (p<0.001). For the control group the average of the angular measurement was 100.9º + 1.9. This value did no differ from the values initially measured in the studied group (p=0.723). They were significantly higher when compared to the average of the values of the final angles of the studied group with treated patients (p=0.001). CONCLUSION: the rehabilitation protocol used in the sample promoted significantly increment of muscular tonus, with functional improvement and in the facial rest.
