Resumo Objetivo: O objetivo do estudo foi avaliar o alinhamento radiográfico na artroplastia total do joelho (ATJ) entre o pós-operatório imediato versus o tardio. Método: O estudo foi retrospectivo e observacional. A população do estudo foi composta por 200 pacientes com osteoartrite primária do joelho submetidos à artroplastia total do joelho. Resultados: Apenas a incidência em perfil evidenciou significância estatística entre as diferenças das medições das radiografias pós-operatória imediata da ATJ e com 3 meses de pós-operatório. Conclusão: O nosso estudo demonstrou uma variação entre a radiografia pós-operatória imediata da artroplastia total do joelho e a radiografia de controlo com 3 meses de pós-operatório apenas na incidência em perfil. Assim sendo, podemos inferir que a radiografia pós-operatória imediata na artroplastia total do joelho poderá ser um exame adequado para a análise pós-operatória.

Abstract Objective: The objective of the study was to evaluate the radiographic alignment in total knee arthroplasty (TKA) between the immediate versus the late postoperative period. Method: The study was retrospective and observational. The study population consisted of 200 patients with primary knee osteoarthritis undergoing total knee arthroplasty. Results: Only the lateral view showed statistical significance between the differences in measurements on the immediate postoperative radiographs of TKA and 3 months after the operation. Conclusion: Our study showed variation between the immediate postoperative radiograph of total knee arthroplasty and the control radiography 3 months after the operation only in the lateral view. Therefore, we can infer that immediate postoperative radiograph of total knee arthroplasty might be a suitable exam for postoperative analysis.

Abstract Objective: To descriptively analyze the epidemiological data, clinical stage, and outcomes of oropharyngeal squamous cell carcinoma in the state of São Paulo, Brazil, and to estimate the influence of clinical stage and treatment type on overall and disease-free survival. Methods: We retrospectively analyzed epidemiological data from the São Paulo Cancer Center Foundation database relative to patients with oropharyngeal squamous cell carcinoma diagnosed between 2004 and 2014 in the state of São Paulo. Univariate and multivariate Cox regression analyses were performed to assess factors associated with the outcomes. A forward stepwise selection procedure was used. Survival curves were estimated by the Kaplan-Meier method and compared by the Gehan-Breslow-Wilcoxon test. Results: A total of 8075 individuals with oropharyngeal squamous cell carcinoma were identified. Of these, 86.3% were diagnosed at an advanced stage and 13.7% at an early stage. Only 27.2% of patients were treated surgically, whereas 57.5% were treated medically. Patients undergoing surgery had longer overall survival than those receiving medical treatment in both early- and advanced-stage oropharyngeal squamous cell carcinoma. However, there was no significant difference in disease-free survival between surgical and medical treatment. Conclusion: No significant difference in disease-free survival between medical and surgical treatment suggests similar complete remission rates with both approaches. Patients receiving medical treatment had shorter overall survival, which may be due to complications from chemotherapy and radiotherapy. However, we cannot confirm this relationship based on the data provided by the São Paulo Cancer Center Foundation. Prospective studies are warranted to assess whether the lower overall survival rate in patients receiving medical treatment is secondary to complications from chemotherapy and radiotherapy. Resumo O Brasil, assim como outros países do mundo, vem apresentando um crescimento populacional cada vez mais longevo. Na última década, muitas pesquisas se concentraram em compreender a complexidade e os impactos sobre o envelhecimento. Porém, ainda não está claro se existe alguma relação entre os modos específicos de vida dos idosos com o sentido de lugar que eles vivenciam. Questionamos: que lugares e tipos de atividades podem contribuir para uma melhor qualidade de vida do idoso na sua casa, vizinhança, bairro e cidade? Com o objetivo de analisar como o idoso convive com os lugares que habita na urbanidade contemporânea, a fim de proporcionar novas estratégias para um planejamento urbano inclusivo, a pesquisa utilizou o método dos diários fotográficos para compreender essas dinâmicas. O estudo foi aplicado nas cidades brasileiras de Pelotas/RS, Belo Horizonte/MG e Brasília/DF, cada uma com três recortes de bairros escolhidos por distintas faixas de renda (alta, média e baixa). Mesmo com suas grandes diferenças urbanas, econômicas e culturais, essas cidades têm em comum as transformações na pirâmide etária, tornando o estudo mais heterogêneo e diverso. Como resultado, foi possível fazer a aproximação com o conceito de “devir-criança”, agenciando a filosofia contemporânea francesa com o sentido do "bom lugar" encontrado nos idosos da pesquisa. Essas coexistências nos levaram a propor o exercício de composição de um “devir-criança-idoso”, na direção de uma vontade de potência e afirmação da vida arquitetônica e urbana para um idoso "cri-ativo".

Abstract Brazil, like other countries in the world, has been showing an increasingly long-lasting population growth. Over the last decade, much research has focused on understanding the complexity and impacts of aging. However, it is still not clear whether there is any relationship between the specific ways of life of the elderly and the sense of place they experience. We ask: what places and types of activities can contribute to a better quality of life for the elderly in their home, neighborhood, neighborhood and city? In order to analyze how the elderly live with the places they inhabit in contemporary urbanity, in order to provide new strategies for inclusive urban planning, the research used the method of photographic diaries to understand these dynamics. The study was applied in the Brazilian cities of Pelotas/RS, Belo Horizonte/MG and Brasília/DF, each one with three sections of neighborhoods chosen by different income groups (high, medium and low). Even with their great urban, economic and cultural differences, these cities have in common the transformations in the age pyramid, making the study more heterogeneous and diverse. As a result, it was possible to approach the concept of “becoming-child”, bringing together contemporary French philosophy with the sense of "good place" found in the elderly in the research. These coexistences led us to propose the exercise of composition of a “becoming-child-elderly”, towards a will to power and affirmation of architectural and urban life for a “creative-active” elderly person.

Resumen Brasil, al igual que otros países del mundo, ha venido mostrando un crecimiento demográfico cada vez más duradero. Durante la última década, muchas investigaciones se han centrado en comprender la complejidad y los impactos del envejecimiento. Sin embargo, todavía no está claro si existe alguna relación entre las formas específicas de vida de las personas mayores y el sentido de lugar que experimentan. Nos preguntamos: ¿qué lugares y tipos de actividades pueden contribuir a una mejor calidad de vida de las personas mayores en su hogar, barrio, barrio y ciudad? Con el fin de analizar cómo las personas mayores conviven con los lugares que habitan en la urbanidad contemporánea, con el fin de brindar nuevas estrategias de planificación urbana inclusiva, la investigación utilizó el método de los diarios fotográficos para comprender estas dinámicas. El estudio se aplicó en las ciudades brasileñas de Pelotas / RS, Belo Horizonte / MG y Brasília / DF, cada una con tres secciones de barrios elegidos por diferentes grupos de ingresos (alto, medio y bajo). Incluso con sus grandes diferencias urbanísticas, económicas y culturales, estas ciudades tienen en común las transformaciones en la pirámide de edades, lo que hace que el estudio sea más heterogéneo y diverso. Como resultado, fue posible abordar el concepto de “convertirse en niño”, uniendo la filosofía francesa contemporánea con el sentido de “buen lugar” que se encuentra en los ancianos en la investigación. Estas coexistencias nos llevaron a proponer el ejercicio de la composición de un “hacerse-niño-anciano”, hacia una voluntad de poder y afirmación de la vida arquitectónica y urbana para un anciano “creativo-activo”.

Resumo Este estudo teve como objetivo identificar os efeitos da pandemia de Covid-19 na saúde mental de populações adultas e verificar os recursos de avaliação empregados. A partir de uma revisão sistemática, segundo as recomendações do PRISMA, foram identificados 1.158 artigos nas bases de dados Pubmed, PsycINFO, PePSIC e Scielo, dos quais 54 atenderam aos critérios de elegibilidade para análise. As populações pesquisadas contemplaram adultos em geral, profissionais de saúde e adultos idosos; sintomas de ansiedade e depressão foram os mais relatados; houve expressiva diversidade nos recursos de avaliação aplicados on-line. Políticas de prevenção, promoção de saúde mental para populações mais vulneráveis, assim como o desenvolvimento de testes de avaliação com evidências de validade para aplicação on-line, são uma realidade premente.

Abstract This study aimed to identify the effects of the COVID-19 pandemic on the mental health of adult populations and verify the assessment resources employed. From a systematic review according to PRISMA recommendations, 1,158 articles were identified in the databases Pubmed, PsycINFO, PePSIC and Scielo, of which 54 met the eligibility criteria for analysis. The populations surveyed included adults in general, health professionals and elderly adults; symptoms of anxiety and depression were the most reported; there was expressive diversity in the assessment resources applied on-line. Prevention policies, promotion of mental health for the most vulnerable populations, as well as the development of evaluation tests with evidence of validity for on-line application, are a pressing reality.

RESUMO Objetivo: Objetivo: Determinar o método radiográfico pré-operatório para aferição do ângulo de Cobb que mais se aproxima dos resultados pós-operatórios em pacientes com deformidade escoliótica. Métodos: Estudo de coorte retrospectivo de avaliação radiográfica da coluna vertebral (posteroanterior pré-operatória [PA], inclinações, tração, tração com anestesia e posteroanterior pós-operatória imediata [PO]) de 26 pacientes com deformidades escolióticas no período de Janeiro de 2017 a Setembro de 2019 tratados com cirurgia . Avaliou-se a média final do ângulo de Cobb e a sua diminuição com relação ao PA nas três curvas em pacientes com escoliose idiopática (EI) e não idiopática. Resultados: Todas as médias das curvas têm significância estatística, exceto a inclinação na escoliose não idiopática (não EI). A tração com anestesia apresenta média de valores mais próximos ao PO. Com relação ao delta (diminuição) das manobras referentes ao PA, foi observado que não houve significância estatística nas não EI. A manobra de tração com anestesia tem delta maior em todas as curvas. Conclusões: A manobra de tração com anestesia em pacientes com escoliose idiopática configura-se como o método com maior flexibilidade e que melhor prediz o resultado pós-operatório. ABSTRACT Objective: To determine the preoperative radiographic method for measuring the Cobb angle that is closest to the postoperative result in patients with scoliotic deformity. Methods: Retrospective cohort study of radiographic spinal evaluation (preoperative posteroanterior (PA), bending, traction, traction under anesthesia and immediate postoperative posteroanterior (PO)) of 26 patients treated surgically for scoliotic deformities during the period from January 2017 to September 2019. The final mean Cobb angle and its decrease in relation to the PA value were evaluated in the three curves in patients with idiopathic (IS) and non-idiopathic scoliosis. Results: All the mean curve values were statistically significant, except for bending in non-idiopathic scoliosis (non-IS). The mean traction under anesthesia values were closer to the PO values. Regarding the delta (decrease) of the maneuvers in relation to the PA, no statistical significance was observed in the non-IS group. The traction under anesthesia maneuver had a greater delta in all curves. Conclusions: The traction under anesthesia maneuver in patients with idiopathic scoliosis is the method with the greatest flexibility and which best predicts the postoperative result. RESUMEN Objetivo: Determinar el método radiográfico preoperatorio para medición del ángulo de Cobb que más se aproxima a los resultados postoperatorios en pacientes con deformidad escoliótica. Métodos: Estudio de cohorte retrospectivo de evaluación radiográfica de la columna vertebral (posteroanterior preoperatoria (PA), inclinaciones, tracción, tracción con anestesia y posteroanterior postoperatoria inmediata (PO)) de 26 pacientes con deformidades escolióticas en el período de enero de 2017 a septiembre de 2019 tratados con cirugía. Se evaluó el promedio final del ángulo de Cobb y su disminución con relación al PA en las tres curvas en pacientes con escoliosis idiopática (EI) y no idiopática. Resultados: Todos los promedios de las curvas tienen significancia estadística, excepto la inclinación en la escoliosis no idiopática (no EI). La tracción con anestesia presenta promedio de valores más próximos al PO. Con relación al delta (disminución) de las maniobras referentes al PA, se observó que no hubo significancia estadística en las no EI. La maniobra de tracción con anestesia tiene un delta mayor en todas las curvas. Conclusiones: La maniobra de tracción con anestesia en pacientes con escoliosis idiopática se configura como el método con mayor flexibilidad y que mejor predice el resultado postoperatorio.Nivel de evidencia III; Estudio diagnóstico.

RESUMO Avaliar a qualidade dos solos sob diferentes culturas de cobertura ou sob diferentes sistemas de manejo torna-se essencial à sua conservação. Objetivou-se neste estudo avaliar um Oxisol cultivado com milho e algodão, após diferentes sucessões culturas e sob sistema plantio direto (SPD) e plantio convencional (PC), por meio de índice de qualidade do solo (IQS), utilizando como referência uma área de Cerrado nativo. O estudo foi realizado no município de Luís Eduardo Magalhães, Oeste da Bahia, Brasil. Amostras de solo com estrutura preservada e não preservada foram coletadas nas camadas de 0-0,05 m; 0,05-0,10 m e 0,10-0,20 m, para determinação da macroporosidade, densidade do solo, água disponível, teores de carbono orgânico total, argila dispersa em água e grau de floculação. As médias dos atributos mensurados nos tratamentos e o índice de qualidade do solo, que foi elaborado pelo método dos desvios dos valores dos atributos medidos nos tratamentos em relação à área de referência, seguido da normalização, foram comparados pelo teste de Duncan (p ≤ 0,05). O solo sob PC, em todos os tratamentos, teve a sua qualidade reduzida quando comparado com o SPD, além disso, o IQS utilizado foi sensível para detectar as alterações provocadas pelos sistemas de manejo e atribuir escores coerentes à qualidade do solo avaliado.

ABSTRACT Assessing soil quality under different cover crops or different management systems is essential to its conservation. This study aimed to evaluate an Oxisol cultivated with corn and cotton, after different crop successions and under no-tillage system (NTS) and conventional tillage system (CT), through the soil quality index (SQI), using an area of native Cerrado as reference. The study was carried out in the municipality of Luís Eduardo Magalhães, Western Bahia, Brazil. Soil samples with the preserved and non-preserved structure were collected in the layers of 0-0.05 m, 0.05-0.10 m, and 0.10-0.20 m to determine the macroporosity, the soil bulk density, the available water, the levels of total organic carbon, the clay dispersed in water, and the degree of flocculation. The averages of the attributes measured in the treatments and the soil quality index, which was elaborated by the method of deviations of the values of the attributes measured in the treatments concerning the reference area, followed by normalization, were compared by the Duncan test (p ≤ 0.05). The soil under CT, in all treatments, had its quality reduced when compared to the NTS. Also, the SQI used was sensitive to detect the changes caused by the management systems and assign consistent scores to the evaluated soil quality.

This letter discusses the epidemic situation of Covid-19 in Brazil, in the face of the emergence of a new strain, called P1, more transmissible and with possible associated reinfection.  Given the collapse of hospital care in Manaus in January 2021 and the results of three recent preprints, all of which found increased transmissibility of the P.1 variant, we propose some urgent actions: the establishment of genomic surveillance based on multi-step diagnostics, starting with RT-PCR type tests  to sequencing; an immediate effort to identify reinfections associated with the new variant, updating its definition protocols; and studies on the efficacy of currently available vaccines in Brazil in respect to the new variant.  We also propose the improvement of the Brazilian health surveillance system, which should be articulated with genomic surveillance, in order to respond more timely to future emergencies. We call on the public agents involved in health surveillance to share data and information regarding the epidemic in a clear, fast and transparent way. Finally, we propose a greater engagement in inter-institutional cooperation of all those involved in the response and production of knowledge about the pandemic in our country.

Esta carta discute a situação epidêmica da Covid-19 no Brasil frente aoaparecimento de uma nova linhagem, chamada P1, mais transmissível e compossível re-infecção associada. RESUMO O objetivo do presente estudo foi avaliar o grau de conversão, citotoxicidade, solubilidade e pH de cimentos à base de cálcio fotopolimerizáveis submetidos a pré-aquecimento. O grau de conversão foi analisado por espectroscopia no infravermelho com transformada de Fourier, a citotoxicidade pelo teste de MTT e a solubilidade através da perda de massa. Os dados foram submetidos a testes estatísticos (ANOVA/Tukey, p<0,05). Os materiais fotopolimerizáveis apresentaram baixo grau de conversão, independente do pré-aquecimento. Todos os materiais causaram redução da viabilidade celular nas análises de 24 horas e 7 dias, sendo que o Dycal (controle) apresentouse mais citotóxico e o calor apresentou efeito positivo sobre o Biocal na análise de 7 dias. O Dycal é o material mais solúvel e o calor não causou efeito na solubilidade e pH dos materiais polimerizáveis. Assim, conclui-se que os cimentos à base de cálcio fotopolimerizáveis apresentam baixo grau de conversão e são solúveis, que resulta em citotoxicidade suave e moderada.

ABSTRACT The aim of this study was to evaluate the degree of conversion, cytotoxicity, solubility and pH of photopolymerizable calciumbased cements submitted to preheating. The degree of conversion was analyzed by Fourier transform infrared, cytotoxicity by the MTT test and solubility through loss of mass. The data were subjected to statistical tests (ANOVA / Tukey’s, p<0.05). The photopolymerizable materials showed a low degree of conversion, regardless of preheating. All materials caused a reduction in cell viability at 24 hours and 7 days, with the Dycal (control) being more cytotoxic. Heat had a positive effect on Biocal at 7 days. Dycal is the most soluble material. Heat had no effect on the solubility or pH of the polymerizable materials. It is concluded that photopolymerizable calcium-based cements have a low degree of conversion and are soluble, which results in mild to moderate cytotoxicity.

RESUMO A partir do desastre socioambiental ocorrido em Mariana (MG), em 5 de novembro de 2015, o estudo buscou compreender a atividade dos técnicos da Defesa Civil que atuaram no desastre. A reflexão que aqui se desenvolve parte do esforço de dar visibilidade à atividade desses profissionais que atuam em condições permeadas de imprevisibilidades e pressões. O estudo tem como referenciais os aportes do campo da Saúde do Trabalhador e da ergologia. Trata-se de uma pesquisa empírica, de abordagem qualitativa, realizada por meio de observação direta e entrevistas individuais semiestruturadas com trabalhadores da Defesa Civil. A análise dos resultados foi organizada em duas grandes seções com os seus respectivos desdobramentos: a primeira traz uma contextualização do desastre narrada pelos diversificados profissionais que atuaram; a segunda evidencia desafios e perspectivas que os agentes de defesa civil puderam tirar da catástrofe. Diante da complexidade das situações e da dimensão imprevisível do risco, os agentes de defesa civil são constantemente convocados a ‘renormatizar’ e a operar escolhas a partir de sua história e de seus valores para lidar com as infidelidades do meio.

ABSTRACT From the socio-environmental disaster that took place on November 5th, 2015 in Mariana (MG), the study sought to understand the activity of Civil Defense technicians who acted in the disaster, aiming to collaborate for actions of worker’s health surveillance. The reflection that develops here is part of the effort to give visibility to the activity of those professionals who work in conditions permeated with unpredictability and pressure. The study has as references the contributions from the field of Occupational Health and ergology. This is an empirical research with a qualitative approach, conducted through direct observation and semi-structured individual interviews with Civil Defense professionals. RESUMO A partir do desastre socioambiental ocorrido em Mariana (MG), em 5 de novembro de 2015, o estudo buscou compreender a atividade dos técnicos da Defesa Civil que atuaram no desastre. A reflexão que aqui se desenvolve parte do esforço de dar visibilidade à atividade desses profissionais que atuam em condições permeadas de imprevisibilidades e pressões. O estudo tem como referenciais os aportes do campo da Saúde do Trabalhador e da ergologia. Trata-se de uma pesquisa empírica, de abordagem qualitativa, realizada por meio de observação direta e entrevistas individuais semiestruturadas com trabalhadores da Defesa Civil. A análise dos resultados foi organizada em duas grandes seções com os seus respectivos desdobramentos: a primeira traz uma contextualização do desastre narrada pelos diversificados profissionais que atuaram; a segunda evidencia desafios e perspectivas que os agentes de defesa civil puderam tirar da catástrofe. Diante da complexidade das situações e da dimensão imprevisível do risco, os agentes de defesa civil são constantemente convocados a ‘renormatizar’ e a operar escolhas a partir de sua história e de seus valores para lidar com as infidelidades do meio.

ABSTRACT From the socio-environmental disaster that took place on November 5th, 2015 in Mariana (MG), the study sought to understand the activity of Civil Defense technicians who acted in the disaster, aiming to collaborate for actions of worker’s health surveillance. The reflection that develops here is part of the effort to give visibility to the activity of those professionals who work in conditions permeated with unpredictability and pressure. The study has as references the contributions from the field of Occupational Health and ergology. This is an empirical research with a qualitative approach, conducted through direct observation and semi-structured individual interviews with Civil Defense professionals. RESUMO Objetivo: Estudar o custo-efetividade do ranibizumabe e bevacizumabe no tratamento da degeneração macular relacionada à idade neovascular. Métodos: Utilizamos um modelo de árvore de decisão para analisar a relação custo-efetividade do ranibizumabe e bevacizumabe no tratamento da degeneração macular relacionada à idade, sob a perspectiva do Sistema Único de Saúde. O ranibizumabe e bevacizumabe foram administrados a pacientes com o mesmo procedimento de tratamento, e a diferença nos custos do tratamernto foi calculada com base no custo dos medicamentos. Os custos diretos foram estimados utilizando as informações fornecidas pelo SUS. A efetividade foi determinada em anos de vida ajustados pela qualidade (QALY) baseados em valores de utilidade em deficiênciavisual. A razãoincremental custo-efetividadefoicalculada comparando os dois tratamentos. O horizonte analítico foi de um ano. Resultados: A análise da árvore de decisão mostrou que a diferença na efetividade do tratamento foi de 0,01 QALY. A razão incremental de custo-efetividade mostrou que o tratamento com ranibizumabe exigiu um custo anual incremental de R$ 2 milhões para gerar um QALY adicional, em comparação ao bevacizumabe. Conclusões: Do ponto de vista do SUS, o bevacizumabe é mais custo-efetivo que o ranibizumabe no tratamento da degeneração macular relacionada à idade neovascular. O seu uso poderia gerar uma grande economia anual para o orçamento em saúde.

ABSTRACT Purpose: To study the cost-effectiveness of ranibizumab and bevacizumab for the treatment of age-related macular degeneration. Methods: We used a decision tree model to analyze the cost-effectiveness of ranibizumab and bevacizumab for the treatment of age-related macular degeneration, from the Brazilian Public Health System (SUS) perspective. Ranibizumab and bevacizumab were administered to patients with the same treatment procedure, and the difference in treatment costs was calculated based on the cost of the drugs. Direct costs were estimated using the information provided by the Brazilian SUS. Effectiveness in terms of quality-adjusted life years (QALYs) was calculated based on the utility values for visual impairment. Incremental cost-effectiveness ratio was calculated by comparing both treatments. The analytical horizon was one year. Results: The decision tree analysis showed that the difference in treatment effectiveness was 0.01 QALY. Incremental cost-effectiveness ratio showed that ranibizumab treatment required an incremental annual cost of more than R$ 2 million to generate 1 additional QALY, as compared to bevacizumab. Conclusions: From the Brazilian SUS perspective, bevacizumab is more cost-effective than ranibizumab for the treatment of neovascular age-related macular degeneration. Its use could allow potential annual savings in health budget.
