SUMMARY: The COVID-19 pandemic caused several changes in the teaching of human anatomy, among them the use of different teaching methodologies based on E-learning. However, studies evaluating electronic tools have not provided clear evidence of effective student learning. Thus, this was the first study to evaluate medical students after completing human anatomy courses in the E- learning modality. We recruited first term (Group A, n=25) and second term (Group B, n=13) medical students. We applied a practical assessment based on 50 human anatomy markingson real anatomical specimens and cadavers, to be completed in Questionnaire 01 (Phase I). After detecting low scores on Questionnaire 01 by both groups, we applied a practical and face-to-face tutoring system for 14 days (Phase II). Afterwards, we re-evaluated both groups by repeating the same scores (Questionnaire 02, Phase III). According to the paired analysis of questionnaires 01 and 02 by Student's T-test, both groups showed significant increase in the final scores obtained in questionnaire 02 (Phase III). The evidence showed that the remote format lecture system weakened the learning and retention process of basic anatomical knowledge by medical students. On the other hand, in-person practical teaching proved to be efficient in the formative process of the students, a fact proven by the significant increase in the scores of the students in the 02 questionnaire, for both groups.

RESUMEN: La pandemia por COVID-19 provocó varios cambios en la enseñanza de la anatomía humana, entre ellos el uso de diferentes metodologías de enseñanza basadas en E-learning. Sin embargo, los estudios que evalúan las herramientas electrónicas no han proporcionado evidencia clara de un aprendizaje efectivo de los estudiantes. Este fue el primer estudio que evaluó a estudiantes de medicina luego de cursar cursos de anatomía humana en la modalidad E-learning. Reclutamos estudiantes de medicina de primer término (Grupo A, n=25) y segundo término (Grupo B, n=13). Se aplicó una evaluación práctica basada en 50 marcas de anatomía humana sobre especímenes anatómicos reales y cadáveres, para ser completado en el Cuestionario 01 (Fase I). Tras detectar puntuaciones bajas en el Cuestionario 01 por parte de ambos grupos, se aplicó un sistema de tutorías prácticas y presenciales durante 14 días (Fase II). Posteriormente, reevaluamos ambos grupos repitiendo las mismas puntuaciones (Cuestionario 02, Fase III). Según el análisis apareado de los cuestionarios 01 y 02 por la prueba T de Student, ambos grupos mostraron aumento significativo en los puntajes finales obtenidos en el cuestionario 02 (Fase III). La evidencia mostró que el sistema de conferencias en formato remoto debilitó el proceso de aprendizaje y retención de conocimientos anatómicos básicos por parte de los estudiantes de medicina. Por otro lado, la docencia práctica presencial demostró ser eficiente en el proceso formativo de los alumnos, hecho que demuestra el aumento significativo de las puntuaciones de los alumnos en el cuestionario 02, para ambos grupos.

Abstract Objective To investigate whether the risk of sarcopenia in older adults diagnosed with covid-19 is associated with the need for intensive care, mechanical ventilation, and mortality. Method A multicenter cohort study was conducted, including older adults (≥65 years) with laboratory-confirmed covid-19 diagnosis. Data related to sociodemographic, clinical, and nutritional characteristics were collected. The risk of sarcopenia was assessed using the Sarcopenia Risk Screening questionnaire. Outcome variables included the need for intensive care, mechanical ventilation, and mortality. Logistic regressions were performed to assess the association between clinical outcomes and the risk of sarcopenia, adjusting for the following variables: age, gender, family income, physical activity, hypertension, diabetes, cardiovascular disease, chronic obstructive pulmonary disease, and body mass index. Results The study included 264 older adults with covid-19, with an average age of 71.7 (±8.2) years. One hundred and forty-eight older adults (56.1%) were at risk of sarcopenia. Hypertension, diabetes, and cardiovascular disease were the most common comorbidities identified in older adults, at 75.4%, 45.5%, and 28.4%, respectively. The presence of sarcopenia risk in hospitalized older adults with covid-19 increased the odds of ICU admission by more than 2-fold (OR: 2.71 [1.57; 4.68], p<0.001), nearly 5-fold for mechanical ventilation (OR: 5.19 [2.75; 9.78], p<0.001), and over 3-fold for mortality (OR: 4.05 [2.05; 7.98], p<0.001). Conclusion In hospitalized older adults with covid-19, pre-existing risk of sarcopenia was a predictor of unfavorable clinical outcomes.

Resumo Objetivo Verificar se o risco de sarcopenia em pessoas idosas diagnosticadas com covid-19 associa-se com demanda por cuidados intensivos, ventilação mecânica e óbito. Método Estudo multicêntrico do tipo coorte, que incluiu pessoas idosas (≥65 anos) com diagnóstico laboratorial confirmado de covid-19. Foram coletados dados relacionados às características sociodemográficas, clínicas e nutricionais. O risco de sarcopenia foi avaliado pelo questionário Sarcopenia Risk Screening. As variáveis de desfecho foram: necessidade de terapia intensiva, ventilação mecânica e óbito. Foram realizadas regressões logísticas para verificar a associação dos desfechos clínicos e o risco de sarcopenia, com as seguintes variáveis de ajuste: idade, sexo, renda familiar, atividade física, hipertensão, diabetes, doença cardiovascular doença pulmonar obstrutiva crônica e índice de massa corporal. Resultados Foram incluídas no estudo 264 pessoas idosas com covid-19, com média de idade de 71,7 (±8,2) anos. Cento e quarenta e oito pessoas idosas (56,1%) tinham risco de sarcopenia. Hipertensão, diabetes e doença cardiovascular foram as principais comorbidades identificadas nas pessoas idosas, respectivamente, 75,4%, 45,5% e 28,4%. O risco de sarcopenia nas pessoas idosas hospitalizadas com covid-19 aumentou em mais de 2 vezes as chances de internamento na UTI (OR: 2,71 [1,57;4,68], p<0,001), em cerca de 5 vezes as chances de ventilação mecânica (OR: 5,19 [2,75;9,78], p<0,001) e em mais de 3 vezes as chances de óbito (OR: 4,05 [2,05;7,98], p<0,001). Conclusão Em pessoas idosas hospitalizadas com covid-19, a pré-existência do risco de sarcopenia foi preditor de desfechos clínicos desfavoráveis.

Abstract Background The European Heart Surgery Risk Assessment System (EuroSCORE) and InsCor have been used to predict complications of cardiac surgery. However, their application to predict lung function and functionality is still uncertain. Objective To correlate surgical risk scales with functional independence and pulmonary function in patients undergoing coronary artery bypass grafting. Methods This was a prospective cohort study. In the preoperative period, the two surgical scales were applied, the maximal inspiratory pressure (MIP), maximal expiratory pressure (MEP), and peak expiratory flow (PEF) were measured, and functionality was assessed using the functional independence measure (FIM). On the seventh postoperative day, the pulmonary function and functionality variables were reevaluated, compared with the preoperative values (delta) and correlated with the risk scales. Correlations of pulmonary function, functional independence and muscle strength variables with the surgical scales were made by Pearson correlation test. The significance level adopted was 5%. Results Thirty-one patients were studied; most were male (77%), with a mean age of 56±8 years. Mean EuroSCORE was 2.3±0.5 and mean InsCOR was 1.2±0.5. MIP, MEP, and PEF reduced 30% (p<0.001), 33% (p<0.001) and 10% (p=0.23), respectively. The EuroSCORE correlated with MIP (r-0.78; p = 0.02) and FIM (r-0.79; p <0.01), and the InsCor correlated with MIP (r-0.77), MEP (r-0.73) and MIF (r-0.89; p=0.02). Conclusion The EuroSCORE showed a strong negative correlation with MIP and FIM, while InsCor had a strong negative correlation with MIP, MEP and FIM. Resumo Fundamentos: A ventilação mecânica (VM) e a circulação extracorpórea (CEC) estão associadas a um declínio da mecânica pulmonar que pode impactar as trocas gasosas. Objetivo: Avaliar o impacto da mecânica pulmonar sobre a duração da VM e trocas gasosas no pós-operatório de cirurgia cardíaca. Métodos: Estudo de coorte realizado com pacientes submetidos a cirurgia cardíaca. Todos os pacientes foram submetidos a avaliação da mecânica pulmonar (complacência estática e resistência das vias aéreas) e gasometria arterial assim que admitidos à unidade de terapia intensiva (UTI) e foram acompanhados até o momento da extubação e, em seguida, até a alta hospitalar. Para correlacionar as variáveis preditoras com o desfecho, foi utilizado o teste de Pearson. Valores de p < 0,05 foram considerados significativos. Resultados: O estudo incluiu 50 pacientes (46 mulheres, 52%), com idade média de 57,5 ± 13,5 anos. A duração da VM foi de 7,7 ± 3,0 horas, a complacência estática foi de 35,5 ± 9,1 cm H2O, a resistência foi de 6,0 ± 2,3 cm H2O, a duração média de estadia na UTI foi de 2,9 ± 1,1 dias e o índice de oxigenação foi de 228,0 ± 33,4 mmHg. Não houve uma correlação significativa entre a duração da VM e a complacência estática (p = 0,73), porém houve uma forte correlação entre a complacência estática e as trocas gasosas (r = 0,8, p < 0,001). Conclusão: A mecânica pulmonar apresenta forte correlação com as trocas gasosas e fraca correlação com a duração da VM no pós-operatório de cirurgia cardíaca.

Abstract Background: Mechanical ventilation (MV) and extracorporeal circulation (ECC) are associated with a decline in pulmonary mechanics that may affect gas exchange. Objective: To evaluate the impact of pulmonary mechanics on MV duration and gas exchange in the postoperative period of cardiac surgery. Methods: This was a cohort study in patients undergoing cardiac surgery. All patients underwent evaluation of pulmonary mechanics (static compliance and airway resistance) and arterial blood gas analysis upon admission to the intensive care unit (ICU) and were followed up until extubation and hospital discharge. Results: The study included 50 patients (46 women, 52%) with a mean age of 57.5 ± 13.5 years. The MV duration was 7.7 ± 3.0 hours, static compliance was 35.5 ± 9.1 cm H2O, resistance was 6.0 ± 2.3 cm H2O, mean length of ICU stay was 2.9 ± 1.1 days, and oxygenation index was 228.0 ± 33.4 mmHg. No significant correlation was found between MV duration and static compliance (p = 0.73), but a strong correlation was found between static compliance and gas exchange (r = 0.8 and p < 0.001). Conclusion: Pulmonary mechanics have a strong correlation with gas exchange and a weak correlation with MV duration after cardiac surgery.

Contexto - Os mecanismos pelos quais o H. pylori produz diferentes quadros patológicos no estômago e no duodeno não são totalmente conhecidos. Considerando a diversidade geográfica relacionada à prevalência dos fatores de virulência desse microrganismo e sua associação com o desenvolvimento de diferentes doenças, vem se intensificando a pesquisa de marcadores de patogenicidade, como o CagA e os alelos do VacA por técnicas moleculares. Objetivos - O objetivo desse estudo foi investigar a presença da infecção por H. pylori, e a frequência dos diferentes genótipos dessa bactéria em pacientes com doenças gastrointestinais da nossa região, procurando estabelecer sua associação com os achados histopatológicos. Métodos - Em estudo prospectivo, foram coletadas amostras de 554 pacientes com diferentes doenças gastrointestinais (gastrite, úlcera duodenal, úlcera gástrica e câncer gástrico), atendidos em hospital de referência para todo o Estado do Pará, localizado na região metropolitana de Belém. Foram analisados dados obtidos através de questionário epidemiológico, relacionados ao sexo e faixa etária desses pacientes. A presença do H. pylori e do genótipo bacteriano foi detectada utilizando a PCR. As biopsias gástricas foram avaliadas histologicamente. Resultados - Observou-se uma prevalência de 91% da infecção pelo H. pylori, sendo mais frequente nos portadores de úlcera gástrica e câncer gástrico, nos quais houve predomínio do sexo masculino e a idade foi maior que a dos outros dois grupos estudados. O genótipo bacteriano predominante foi o s1m1cagA positivo, sendo mais frequentes entre os pacientes com úlcera gástrica, úlcera duodenal e câncer gástrico. Houve associação significante das cepas com o genótipo s1m1cagA positivo com maior grau de inflamação, atividade neutrofílica e desenvolvimento de metaplasia intestinal. Conclusão - Nosso estudo demonstra a alta incidência da infecção pelo H. pylori nos pacientes analisados em nosso meio, especialmente em portadores de úlcera e câncer gástricos. As cepas virulentas s1m1cagA+ foram predominantes e estavam associadas a lesões mais graves.

Background - The mechanisms whereby Helicobacter pylori produces different pathological manifestations in the stomach and duodenum are not fully understood. Considering the geographic diversity in the prevalence of virulence factors of this microorganism and their association with the development of different diseases, the search for pathogenicity markers such as CagA and VacA alleles by molecular techniques has intensified. Objectives - To investigate the presence of H. pylori infection and the frequency of different genotypes of this bacterium in patients with gastrointestinal diseases from Northern Brazil, and to establish their association with the histopathological findings. Methods - In a prospective study, samples were collected from 554 patients with different gastrointestinal diseases (gastritis, duodenal ulcer, gastric ulcer, and gastric cancer) seen at a referral hospital attending the entire State of Pará, located in the metropolitan region of Belém. Data such as gender and age obtained with an epidemiological questionnaire were analyzed. The presence of H. pylori and the bacterial genotype were investigated by PCR. Gastric biopsies were assessed histologically. Results - The prevalence of H. pylori infection was 91%. Infection was more frequent among patients with gastric ulcer and gastric cancer. In these groups, there was a predominance of men and older patients when compared to the other two groups studied. The predominant bacterial genotype was s1m1cagA+, which was more frequent among patients with gastric ulcer, duodenal ulcer and gastric cancer. A significant association was observed between s1m1cagA+ strains and a higher degree of inflammation, neutrophil activity and development of intestinal metaplasia. Conclusion - The present study demonstrates a high incidence of H. pylori infection in the patients analyzed, especially among those with gastric ulcer and gastric cancer. Virulent s1m1cagA+ strains predominated and were associated with more severe lesions.

Resumo Objetivo: avaliar a qualidade de vida relacionada à saúde (QVRS) em portadores de artrite reumatoide (AR), antes e após o tratamento com terapia biológica. Métodos: estudo longitudinal, realizado no período de novembro de 2010 a setembro de 2011, com aplicação dos instrumentos HAQ II (Health Assessment Questionnaire) e SF-36 (Medical Outcomes Short-From Health Survey). Foram realizados testes de Barlett, Anova, Friedman e teste- t pareado para múltiplos extratos. Resultados: foram avaliados 30 pacientes, com idade média de 47,6 (DP: 12,25) anos e prevalência do gênero feminino (90%). A média do escore do HAQ II antes do tratamento foi de 1,97, com diminuição significativa de até 1,23 após seis meses de uso de terapia biológica (p<0,01). A maioria dos domínios do SF-36 apresentou significativa melhora após seis meses de tratamento (p< 0,01), destacando os aspectos sociais, dor, capacidade funcional, aspectos emocionais, vitalidade e aspectos físicos. Conclusão: o uso de terapia biológica em pacientes com AR, refratários aos tratamentos tradicionais, demonstrou ser uma importante estratégia farmacológica para a melhoria da QVRS.

Summary Objective: assessing health-related quality of life (HRQL) in patients with rheumatoid arthritis (RA), before and after treatment with biological therapy. Methods: a longitudinal study, conducted from November 2010 to September 2011, with implementation of the instruments HAQ II (health assessment questionnaire) and SF-36 (medical outcomes short-from health survey). Barlett test, Anova, Friedman and paired t-test were performed for multiple extracts. Results: 30 patients were evaluated, mean age of 47.6 (SD: 12.25) years and prevalence of females (90%). The mean score of HAQ II before treatment was 1.97, with significant reduction of up to 1.23 after six months of biological therapy (p<0.01). Most of the SF-36 domains showed significant improvement after six months of treatment (p<0.01), highlighting the social aspects, pain, physical functioning, emotional issues, vitality and physical aspects. Conclusion: the use of biologic therapy in patients with RA refractory to standard therapies proved to be an important pharmacological strategy for improving HRQL.

O objetivo deste estudo foi comparar o Volume Total de Trabalho (VTT), o Número de Repetições (NR), e a Percepção Subjetiva de Esforço (PSE) de duas sequências de exercícios executadas no método circuito. Onze homens treinados recreacionalmente (24.0 ± 4.8 years, 76.1 ± 8.5 kg, 1.75 ± 0.06 m) realizaram duas sequências diferentes de exercícios contra a resistência no método circuito. As sessões foram compostas por três passagens em oito estações (exercícios), executadas até a fadiga, alternados membros superiores e inferiores, com 1 minuto de intervalo entre os exercícios. A sequência A foi iniciada por exercícios multiarticulares e progrediu para exercícios monoarticulares, enquanto a sequência B foi executada na ordem inversa. O NR e a PSE foram comparadas pela MANOVA com medidas repetidas. O VTT foi comparado por meio do teste t pareado. Não houve diferenças significativas entre as ordens de exercícios para o VTT (p=0.47) e PSE (p>0.05). Para ambas as sequências, o NR foi maior quando os exercícios foram realizados no início das sequências (p&#x0003C;0,05). Esses resultados indicaram que a ordem dos exercícios afetou o número de repetições, mas não afetou o VTT e a PSE. Esses achados sugerem que, para aqueles que objetivam ganhos musculares gerais, resultados similares podem ser obtidos com exercícios mono e multiarticulares, para membros superiores e inferiores em diferentes ordens no método circuito. Da mesma forma, a intensidade do exercício será percebida de forma similar independente da sequência. Para aqueles que objetivam ganhos musculares específicos, exercícios prioritários devem ser executados no início da sessão.

The aim of this study was to compare total work volume, number of repetitions and rate of perceived effort performed in two different sequences of circuit training sessions. Eleven recreationally trained men (24.0 ± 4.8 years, 76.1 ± 8.5 kg, 1.75 ± 0.06 m) performed two different sequences of circuit training sessions. All sessions consisted of eight stations (exercises) of three sets in circuit, performed to volitional fatigue, alternating upper and lower body exercises, with 1-minute rest interval between exercises. Sequence A began with multiple joint exercises and progressed to single joint exercises, whereas sequence B was performed with the opposite exercise order. Number of repetitions and rating of perceived exertion (RPE) were compared by repeated measures multivariate analysis of variance (repeated measures MANOVA). Total work volume (TWV) was compared by paired t-test. There were no differences between exercise order for TWV (p=0.47) and RPE of all exercises (p>0.05). For both sequences, number of repetitions was greater when exercise was performed first (p&#x0003C;0.05). These results indicate that during a circuit training session, exercises order influenced the number of repetition, but did not influence TWV and RPE. These findings may suggest that for those who aim overall muscular gains, similar results will be obtained with single and multiple joint, upper and lower body circuit training exercise orders. Similarly, exercise intensity will be similarly perceived regardless of exercise sequences. For those who aim specific muscle group gains, priority exercises must be performed first.
