Acute suppurative thyroiditis with Graves disease – A very rare association

SUMMARY Acute suppurative thyroiditis is an uncommon disorder caused by a bacterial infection, usually presenting with normal thyroid function. It is a serious condition that requires a prompt diagnosis and treatment with antibiotics and supportive measures. A 62 years-old female presented with a painful cervical induration and odynophagia a week after a fish bone had been removed from her pharynx. She was febrile, and tachycardic and, on physical examination, a painful thyroid mass was detected. High inflammatory parameters and thyrotoxicosis were confirmed: thyroid stimulating hormone (TSH) < 0.01 mIU/L (normal range [NR] 0.27-4.2); free thyroxine (FT4) 3.86 ng/dL (NR 0.9-1.7) and anti-TSH receptor antibodies (TRABs) 5.3 U/L (NR < 1.5). Thyroid scintigraphy showed a diffuse uptake of the thyroid parenchyma suggesting Graves disease. Cervical ultrasonography revealed an abscess of the left thyroid lobe of 36 × 36 mm and fine needle aspiration biopsy (FNAB) with partial drainage was performed. Staphylococcus aureus and Streptococcus viridans were isolated, and directed antibiotic therapy was started. Clinical improvement was observed as well as a decrease of inflammatory parameters and the patient was discharged after 9 days of hospitalization. Eighteen days after discharge, thiamazole was initiated due to persistent thyrotoxicosis. Complete resolution of the abscess was documented within 6 months and the patient became euthyroid under thiamazole one year after initial presentation. To our knowledge, this is the third case reporting an association between acute thyroiditis and Graves disease. Furthermore, this is the first case detailing the simultaneous diagnosis of acute suppurative thyroiditis caused by a foreign body and Graves disease.

Resumo A Amazônia possui o maior sistema fluvial do planeta, abrigando uma biota diversa. Mesmo assim, muitas espécies permanecem desconhecidas, devida imensa escala e complexidade deste bioma, e porque ele vem sofrendo com uma constante pressão antropogênica. Macrófitas são importantes para os processos físicos e biológicos dos ecossistemas aquáticos, porém ainda são pouco estudadas no Norte do Brasil. O objetivo deste estudo é fornecer uma checklist de espécies de macrófitas que ocorrem em municípios que fazem parte do Arco do desmatamento, trazendo informações sobre seus hábitos e formas de vida. Nós amostramos 36 pontos distribuídos em três tipos de ecossistemas aquáticos (riachos, lagos e brejos). No total, registramos 50 espécies, 38 gêneros e 24 famílias. A maioria das espécies era emergente ou anfíbia. Riachos degradados apresentam características similares a ambientes lênticos, o que pode ter oferecido maior disponibilidade de habitat para macrófitas que provavelmente não ocorreriam em condições de ambientes lóticos, o que explicaria a diversidade neste tipo de ecossistema. A diversidade de macrófitas desta região segue a maioria dos padrões de outras regiões do Brasil. Este estudo contribui para a avaliação da diversidade de macrófitas aquáticas na Amazônia, especialmente em locais que sofrem impacto antrópico, como o Arco do Desmatamento.
Abstract The Amazon possesses the largest fluvial system on the planet, harboring a diverse biota. Still, many species remain undescribed, because of the Amazon’s immense scale and complexity, and because many habitats are now increasingly under pressure from anthropogenic activities. Macrophytes are important to physical and biological processes in aquatic ecosystems but remain poorly studied in Northern Brazil. The objective of this study was to provide a checklist of macrophyte species that occur in municipalities that form part of the Arc of Deforestation, Pará state, Brazil, bringing information on their habits and life-forms. We sampled 36 sites at three types of aquatic ecosystems (streams, ponds and lakes). In total, we recorded 50 species, 38 genera and 24 families. Most species were amphibious or emergent. Degraded streams have environmental characteristics similar to lentic habitats, which could provide more suitable habitats for macrophytes that otherwise would not occur in lotic habitats, thus explaining the higher diversity in these ecosystems. Macrophyte diversity in this region follows similar patterns to other Brazilian regions. This study contributes to the assessment of aquatic macrophytes in the Amazon, especially in more degraded regions, such as the Amazon deforestation arc.

Conhecimentos sobre contagem de hidratos de carbono e controlo metabólico de utentes com sistema de perfusão subcutânea contínua de insulina

A prevalência da Diabetes Mellitus tipo 1 tem vindo a aumentar nos últimos anos. O tratamento intensivo através de Sistema de Perfusão Subcutânea Contínua de Insulina é o método mais fisiológico de administração de insulina atualmente disponível. Para que este seja eficaz é fundamental ter conhecimentos sobre a composição dos alimentos, particularmente os Hidratos de Carbono. O presente estudo teve como objetivos avaliar e caracterizar o controlo metabólico, compará-lo antes e depois do Sistema de Perfusão Subcutânea Contínua de Insulina, avaliar os conhecimentos de contagem de Hidratos de Carbono e relacioná-los com o controlo metabólico. Trata-se de um estudo observacional, analítico e retrospetivo, com uma amostra de conveniência constituída pelos utentes com consulta entre novembro 2017 e março de 2018 no Hospital Garcia de Orta. A avaliação do controlo metabólico foi realizada através da HbA1c. Na avaliação dos conhecimentos sobre contagem de Hidratos de Carbono utilizou-se uma adaptação do questionário PedCarbQuiz. Avaliaram-se 60 indivíduos com uma média de idades 21,3 ± 14,27 anos [2 - 57]. Aquando a colocação do Sistema de Perfusão Subcutânea Contínua de Insulina, apenas 23,6% da amostra se encontrava com a HbA1c dentro dos valores recomendados. Três meses após a colocação do Sistema de Perfusão Subcutânea Contínua de Insulina verificou-se uma diminuição significativa da HbA1c, que estabilizou após os 6 primeiros meses. Relativamente aos conhecimentos sobre contagem de HC, 78,3% possuíam nível muito bom e os restantes nível bom. Não foi detetada correlação significativa entre a HbA1c e os conhecimentos de contagem de Hidratos de Carbono, ou seja, ter mais conhecimentos não demonstrou ter influência no controlo metabólico da amostra.
The prevalence of type 1 diabetes mellitus has been increasing in recent years. Intensive treatment through Continuous Subcutaneous Insulin Perfusion System is the most physiological method of insulin administration currently available. For this to be effective it is essential to have knowledge about the composition of the food, particularly about its content in carbohydrates. The aim of the present study was to evaluate and characterize metabolic control, to compare it before and after Continuous Subcutaneous Insulin Perfusion System, to evaluate the carbohydrates count knowledge and to relate it to metabolic control. This is an observational, analytical and retrospective study, with a convenience sample of patients consulted between November 2017 and March 2018 at the Garcia de Orta Hospital. The evaluation of metabolic control was performed through HbA1c. In the evaluation of the knowledge about carbohydrates count, an adaptation of the PedCarbQuiz questionnaire was used. Sixty individuals with a mean age of 21.3 ± 14.27 years were evaluated [2-57]. At the Subcutaneous Insulin Perfusion System, placement, only 23.6% of the sample was HbA1c within the recommended values. 3 months after Continuous Subcutaneous Insulin Perfusion System placement there was a substantial decrease in HbA1c, which stabilized after the first 6 months. Regarding the knowledge about carbohydrates count, 78.3% had “very good” level and the remaining “good” level. No significant correlation was detected between HbA1c and carbohydrates counting knowledge. Having more knowledge has not been shown to influence the metabolic control of the sample.

Urban influence on the water quality of the Uberaba River basin: an ecotoxicological assessment

Resumo Testes ecotoxicológicos com Tradescantia pallida, Allium cepa e Lactuca sativa foram utilizados para avaliar a qualidade da bacia do rio Uberaba, sob influência da área urbana. Amostras de água coletadas em oito pontos, no período seco, foram avaliadas pelos indicadores de toxicidade: porcentagem de micronúcleos em grãos de pólen de T. pallida (TRAD-MN), germinação das sementes, crescimento radicular, índice mitótico (MI) e micronúcleos em células da raiz de A. cepa, e germinação e crescimento radicular de L. sativa. Os parâmetros físico-químicos da água: temperatura, oxigênio dissolvido (OD), pH e condutividade elétrica foram analisados no momento da coleta. As três espécies vegetais foram eficientes como bioindicadores, tendo bom custo-benefício e resultados rápidos e facilmente interpretáveis, complementando os parâmetros físico-químicos. Dentre os parâmetros ecotoxicológicos analisados em L. sativa e A. cepa, foi verificada forte correlação entre a germinação das sementes e o crescimento radicular. A porcentagem de micronúcleos em T. pallida e MI em A. cepa apresentaram forte correlação com condutividade elétrica da água e correlação moderada e negativa com OD. A condutividade elétrica da água variou de 75 a 438 µS.cm-1 e as concentrações de OD, de 0,5 a 6,9 mg.L-1. Evidencia-se a importância de medidas de controle da poluição na bacia do rio Uberaba, que a partir do ponto de captação de água para abastecimento, encontra-se altamente impactado, principalmente os afluentes que cortam o município, apresentando estreita faixa de mata ciliar ou nenhuma, depósito de resíduos nas margens, lançamento de esgoto in natura, entre outras ações antrópicas.
Abstract Ecotoxicological tests applied to Tradescantia pallida, Allium cepa and Lactuca sativa were used to assess the quality of the Uberaba River basin under urban area influence. Water samples were collected at eight different points during the dry season. The samples were assessed using the following toxicity indicators: micronuclei percentage in T. pallida pollen grains (TRAD-MN), seed germination, root growth, mitotic index (MI) and micronuclei in A. cepa root-cells, and seed germination and root growth in L. sativa. Water physicochemical parameters such as temperature, dissolved oxygen (DO), pH and electric conductivity were assessed in situ. The three plant species were efficient bio-indicators, since they presented good cost-benefit and fast and easily interpreted results, thus completing the physicochemical parameters. There was strong correlation between seed germination and root growth among the ecotoxicological parameters assessed in L. sativa and A. cepa. The micronuclei percentage in T. pallida and the MI in A. cepa presented strong correlation with water electric conductivity and moderate and negative correlation with DO. Water electric conductivity ranged from 75 to 438 µS.cm-1; and the DO concentrations ranged from 0.5 to 6.9 mg.L-1. The importance of pollution control measures in the Uberaba River basin stands out. From the supply-water capture point, the basin is strongly affected by pollution, mainly in the tributaries that cross the city. It presents a short, or almost absent, riparian forest line, residues on the river banks, and it is impacted by discharges of untreated sewage, among other anthropic actions.

X-linked adrenal hypoplasia congenita: clinical and follow-up findings of two kindreds, one with a novel NR0B1 mutation

X-linked adrenal hypoplasia congenita typically manifests as primary adrenal insufficiency in the newborn age and hypogonadotropic hypogonadism in males, being caused by mutations in NR0B1 gene. We present the clinical and follow-up findings of two kindreds with NR0B1 mutations. The proband of kindred A had a diagnosis of primary adrenal insufficiency when he was a newborn. Family history was relevant for a maternal uncle death at the newborn age. Beyond 2 year-old steroid measurements rendered undetectable and delayed bone age was noticed. Molecular analysis of NR0B1 gene revealed a previously unreported mutation (c.1084A>T), leading to a premature stop codon, p.Lys362*, in exon 1. His mother and sister were asymptomatic carriers. At 14 year-old he had 3 mL of testicular volume and biochemical surveys (LH < 0.1 UI/L, total testosterone < 10 ng/dL) concordant with hypogonadotrophic hypogonadism. Kindred B had two males diagnosed with adrenal insufficiency at the newborn age. By 3 year-old both siblings had undetectable androgen levels and delayed bone age. NR0B1 molecular analysis identified a nonsense mutation in both cases, c.243C>G; p.Tyr81*, in exon 1. Their mother and sister were asymptomatic carriers. At 14 year-old (Tanner stage 1) hypothalamic-pituitary-gonadal axis evaluation in both males (LH < 0.1UI/L, total testosterone < 10 ng/dL) confirmed hypogonadotropic hypogonadism. In conclusion, biochemical profiles, bone age and an X-linked inheritance led to suspicion of NR0B1 mutations. Two nonsense mutations were detected in both kindreds, one previously unreported (c.1084A>T; p.Lys362*). Mutation identification allowed the timely institution of testosterone in patients at puberty and an appropriate genetic counselling for relatives.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Paciente de 82 anos apresentando-se com nódulo tireoidiano de crescimento progressivo e ptose palpebral esquerda. O exame oftalmológico revelou ainda miose ipsilateral e achados diagnósticos de síndrome de Horner. A tomografia computadorizada mostrou massa tireoidiana de 7,5 cm infiltrando os grandes vasos do pescoço. Apesar dos dados clínicos e imagiológicos sugestivos de um carcinoma pouco diferenciado da tireoide, a citologia aspirativa foi diagnóstica de carcinoma papilar. Em função do estádio avançado da neoplasia e das comorbilidades significativas, foi proposta para terapêutica paliativa. A síndrome de Horner é uma manifestação clínica infrequente em tumores tireoidianos, estando as condições benignas maioritariamente implicadas. As neoplasias malignas da tireoide representam uma causa rara de síndrome de Horner. Contudo, um diagnóstico adequado e expedito é fundamental para o tratamento atempado nos raros casos de malignidade da tireoide.
An 82-year-old patient presented a progressively growing hard thyroid nodule, and left ptosis. Additionally, ophthalmologic evaluation revealed ipsilateral miosis, diagnostic findings of Horner syndrome. Computerized tomography revealed a 7.5-cm thyroid mass infiltrating the main neck vessels. Although clinical and imaging data were suggestive of poorly differentiated thyroid carcinoma, fine-needle aspiration led to the diagnosis of papillary carcinoma. Paliative care was proposed to the patient due to the advanced stage of the neoplasm and to significant comorbidities. Horner syndrome is an infrequent manifestation of thyroid disorders and benign etiologies are more often implied. Malignant thyroid neoplasms represent a rare cause of Horner syndrome. However, an appropriate and prompt diagnosis is paramount for timely treatment of rare thyroid malignancies.

Resumen El análisis de los verbos de la clase gustar es complejo, por lo menos formalmente, puesto que la caracterización gramatical del objeto no es evidente de forma inmediata. De acuerdo con la tradición de la lingüística española, algunos verbos como gustar, molestar, encantar, interesar, convenir y otros, tienen la siguiente estructura: objeto en forma de dativo experimentante + verbo + sujeto; como en “A Luisa le gustan los discursos”. El propósito de esta investigación es intentar demostrar que el llamado dativo experimentante es realmente el sujeto lógico dativo y el constituyente que tradicionalmente se reconoce como sujeto, debido a la concordancia con el verbo, es un objeto nominativo. Para este propósito, la Hipótesis de la Asimetría del Sujeto y del Predicado con respecto a la Negación de Peter F. Strawson (1971) sirve como fundamento; así como La Teoría de la Revisión de Ura (2000).
Abstract The analysis of the gustar-class verbs is complex, at least formally, since the grammatical characterization of the object is not evident in an immediate way. In accordance with the tradition of Spanish linguistics, some verbs like gustar, molestar, encantar, interesar, convenir and others have the following structure: object, with the form of dative experiencer + verb + subject, as in “A Luisa le gustan las conferencias”. The purpose of this investigation is to try to demonstrate that the so called dative experiencer is actually the logical dative subject, and that the constituent traditionally recognized as the subject, due to the concordance with the verb in personal features, is a nominative object. For this purpose both P. Strawson’s (1971) Hypothesis of the Subject and Predicate Asymmetry with respect to Negation and Ura’s (2000) Checking Theory are used.
Résumé L’analyse des verbes de la classe plaire est, au moins formellement, complexe, étant donné que la caractérisation grammaticale de l’objet n’est pas évidente au premier abord. D’après la tradition de la linguistique espagnole, quelques verbes comme gustar (plaire), molestar (déranger), encantar (charmer), interesar (intéresser), convenir (convenir) et d’autres, ont la structure suivante: objet sur la forme de datif expérimental+verbe+sujet ; comme par exemple à la phrase « A Luisa le gustan los discursos ». Le but de cette recherche est essayer de montrer que ce que l’on appelle datif expérimental est vraiment le sujet logique datif et le constituant qu’on reconnaît traditionnellement comme sujet, du fait de la concordance avec le verbe, est un objet nominatif. Pour cet objectif, l’hypothèse de l’asymétrie du sujet et du prédicat par rapport à la négation de Peter F. Strawson (1971) sert comme fondement. Il en va de même avec la théorie de la Révision d’Ura (2000).

Esta pesquisa estudou o funcionamento e os arranjos familiares de crianças de uma escola pública da cidade do Recife. A amostra se constituiu de 100 pessoas, sendo 50 adultos (pais ou responsáveis pelas crianças) e 50 crianças de 06 a 11 anos. Foram utilizados como instrumento a Entrevista do Genograma e o Desenho da Família com Estória. Os resultados mostram vários tipos de arranjos familiares entre estas crianças, predominando a família nuclear. Quanto ao funcionamento, a maioria dessas famílias se organiza em torno de mulheres, sendo as figuras masculinas fragilizadas pelo desemprego, uso de álcool ou outras drogas.
This research was regarding family functioning and ways of dealing with low income family children from a public school in the city of Recife. The sample consisted of 100 people of both sexes (parents and children caretakers) from 25 to 55 years of age. The instrument used was the Genogram Interview and Family Drawing with Story Telling. The results show that there are many types of family arrangements for taking care of these children, being the most common, the nuclear one. As for the family functioning, the great majority follows the female leadership due to the fragile role played by the male figure, who is frequently associated to unemployment and addiction to drugs.

Deltamethrin-impregnated PVC dog collars were tested to assess if they were effective in protecting dogs from sand fly bites of Lutzomyia longipalpis and Lu. migonei. A protective effect against Old World species Phlebotomus perniciosus was demonstrated before. Four dogs wearing deltamethrin collars and three dogs wearing untreated collars (not impregnated with deltamethrin) were kept in separate kennels for over eight months in a village on the outskirts of Fortaleza in Ceará, Brazil. Periodically, a dog from each group was sedated, placed in a net cage for 2 h in which 150 female sand flies had been released 10-15 min before. Lu. longipalpis were used 4, 8, 12, 16, 22, 27, and 35 weeks after the attachment of the collars. Lu. migonei were used 3, 7, 11, 15, 22, 26, and 36 weeks after attachment. During 35 weeks, only 4.1% (81 of 2,022) Lu. longipalpis recovered from the nets with the deltamethrin collared dogs were engorged, an anti-feeding effect of 96%. Mortality initially was over 90% and at 35 weeks was 35% with half of the sand flies dying in the first 2 h. In contrast, 83% of the 2,094 Lu. longipalpis recovered from the nets containing the untreated collared dogs were engorged and the mortality ranged from zero to 18.8% on one occasion with 1.1% dying in the first 2 h. Similar findings were found with Lu. migonei: of 2,034 sand flies recovered over this period, only 70 were engorged, an anti-feeding effect of 96.5%, and mortality ranged from 91% initially to 46% at 36 weeks. In contrast, engorgement of controls ranged from 91 to71% and a mortality ranged from 3.5 to 29.8%. These studies show that deltamethrin impregnated collars can protect dogs against Brazilian sand flies for up to eight months. Thus, they should be useful in a program to control human and canine visceral leishmaniasis.

The synthesis of new isochromanylacetylarylhydrazones designed as probable non-addictive analgesic agents

A síntese e o perfil anti-inflamatório e analgésico dos novos agentes isocromanilacetilarilidrazônicos são descritos neste artigo. A rota sintética utilizada neste trabalho explora como passo chave para a construção de um anel heterocíclico de seis membros, a ciclização catalisada por um ácido de Lewis, que representa uma modificação da reação de Friedel-Crafts. Estes novos derivados (3) são obtidos em ca. de 85% de rendimento, a partir do safrol (4) utilizado como produto natural abundante, isolado do óleo de Sassafrás. A análise dos espectros de RMN destes novos derivados indica a natureza diastereoisomérica a nível da ligação C=N, na razão de 70:30, onde a maior contribuição é relativa ao isômero (E). Os resultados obtidos da avaliação farmacológica de (3) no teste do edema de pata de rato induzido por carragenina e no teste de contorções abdominais induzidas pelo ácido acético, indicam a natureza farmacofórica da unidade acilidrazona para a atividade analgésica observada nesta série. O papel dos substituintes na sub-unidade arila na atividade farmacológica parece indicar que a presença de grupos hidrofóbicos podem elevar o perfil analgésico. Estes novos derivados isocromanilacetilarilidrazônicos (3) representam nova classe de agentes analgésicos não-convencionais.
The synthesis and pharmacological profile as analgesic and anti-inflammatory agents of new isochromanylacetylarylhydrazone derivatives (3) are described in this paper. The synthetic route used in this work to construct the heterocyclic six member ring explored a Lewis acid-catalyzed cyclization process as the key step, which represents a modified Friedel-Crafts reaction. These new derivatives (3) were obtained in ca. 85 % overall yields from the starting material safrole (4), an abundant natural product isolated from Sassafras oil. The NMR spectral analysis of these new derivatives indicated, at the C=N double bond level, the diastereomeric nature in a 70:30 ratio, where the major compound is the (E)-isomer. The results obtained from the pharmacological evaluation of (3) using the carrageenan-induced rat paw edema test and the acetic acid solution-induced constrictions in mouse test, indicated the pharmacophoric nature of the acylarylhydrazone moiety to the analgesic activity observed in this series.The role of the aryl substituents in the bioactivity seems to indicate that the presence of hydrophobic groups may improve the analgesic profile. These new isochromanylacetylarylhydrazone derivatives (3) represent a new class of non-addictive analgesic agents.
