ABSTRACT Background: Critical illness is a major ongoing health care burden worldwide and is associated with high mortality rates. Sodium-glucose cotransporter-2 inhibitors have consistently shown benefits in cardiovascular and renal outcomes. The effects of sodium-glucose cotransporter-2 inhibitors in acute illness have not been properly investigated. Methods: DEFENDER is an investigator-initiated, multicenter, randomized, open-label trial designed to evaluate the efficacy and safety of dapagliflozin in 500 adult participants with acute organ dysfunction who are hospitalized in the intensive care unit. Eligible participants will be randomized 1:1 to receive dapagliflozin 10mg plus standard of care for up to 14 days or standard of care alone. The primary outcome is a hierarchical composite of hospital mortality, initiation of kidney replacement therapy, and intensive care unit length of stay, up to 28 days. Safety will be strictly monitored throughout the study. Conclusion: DEFENDER is the first study designed to investigate the use of a sodium-glucose cotransporter-2 inhibitor in general intensive care unit patients with acute organ dysfunction. It will provide relevant information on the use of drugs of this promising class in critically ill patients. ClinicalTrials.gov registry: NCT05558098

RESUMO Antecedentes: A doença crítica é um importante ônus permanente da assistência médica em todo o mundo e está associada a altas taxas de mortalidade. Os inibidores do cotransportador de sódio-glicose do tipo 2 têm demonstrado consistentemente benefícios nos desfechos cardiovasculares e renais. Os efeitos dos inibidores do cotransportador de sódio-glicose do tipo 2 em doenças agudas ainda não foram devidamente investigados. Métodos: O DEFENDER é um estudo de iniciativa do investigador, multicêntrico, randomizado, aberto, desenhado para avaliar a eficácia e a segurança da dapagliflozina em 500 participantes adultos com disfunção orgânica aguda hospitalizados na unidade de terapia intensiva. Os participantes aptos serão randomizados 1:1 para receber 10mg de dapagliflozina e o tratamento padrão por até 14 dias ou apenas o tratamento padrão. O desfecho primário é um composto hierárquico de mortalidade hospitalar, início de terapia renal substitutiva e tempo de internação na unidade de terapia intensiva, até 28 dias. O monitoramento da segurança será rigoroso durante todo o estudo. Conclusão: O DEFENDER é o primeiro estudo desenvolvido para investigar o uso de um inibidor do cotransportador de sódio-glicose do tipo 2 em pacientes de unidade de terapia intensiva geral com disfunção orgânica aguda. O estudo fornecerá informações relevantes sobre o uso de medicamentos dessa classe promissora em pacientes críticos. Registro ClincalTrials.gov: NCT05558098

Abstract This study aimed to evaluate the contribution of oral and maxillofacial pathology laboratories (OMPLs) in Brazilian public universities to the diagnosis of lip, oral cavity, and oropharyngeal squamous cell carcinoma (SCC). A cross-sectional study was performed using biopsy records from a consortium of sixteen public OMPLs from all regions of Brazil (North, Northeast, Central-West, Southeast, and South). Clinical and demographic data of patients diagnosed with lip, oral cavity, and oropharyngeal SCC between 2010 and 2019 were collected from the patients’ histopathological records. Of the 120,010 oral and maxillofacial biopsies (2010-2019), 6.9% (8,321 cases) were diagnosed as lip (0.8%, 951 cases), oral cavity (4.9%, 5,971 cases), and oropharyngeal (1.2%, 1,399 cases) SCCs. Most cases were from Brazil’s Southeast (64.5%), where six of the OMPLs analyzed are located. The predominant profile of patients with lip and oral cavity SCC was Caucasian men, with a mean age over 60 years, low schooling level, and a previous history of heavy tobacco consumption. In the oropharyngeal group, the majority were non-Caucasian men, with a mean age under 60 years, had a low education level, and were former/current tobacco and alcohol users. According to data from the Brazilian National Cancer Institute, approximately 9.9% of the total lip, oral cavity, and oropharyngeal SCCs reported over the last decade in Brazil may have been diagnosed at the OMPLs included in the current study. Therefore, this data confirms the contribution of public OMPLs with respect to the important diagnostic support they provide to the oral healthcare services extended by the Brazilian Public Health System.

Resumo: Atingidos pela sofisticação dos mecanismos de flexibilização e controle do trabalho, os sindicatos enfrentam talvez o maior desafio ao seu poder coletivo de sua história. Este texto pretende trazer argumentos para o debate sobre esta “crise do sindicalismo”, a partir de uma perspectiva analítica (de “recursos de poder”) que valoriza esforços de ressignificação de práticas de resistência, coletivismo e solidariedade, em nível nacional e internacional, e vê nesse contexto pandêmico (anos 2020 e 2021) uma oportunidade para a recomposição de seus repertórios de ação coletiva (Alonso, 2012; Schmalz et al., 2018). A referência empírica é a experiência sindical metalúrgica de um setor industrial brasileiro com predomínio de empresas transnacionais, considerando sua capacidade de responder, de forma proativa, às ameaças ao seu poder consolidado, ao longo das últimas seis décadas, no ABC paulista. Os dados utilizados têm como base sete entrevistas gravadas (online) com lideranças sindicais metalúrgicas, realizadas em 2020 e 2021, e um levantamento, pelo método da raspagem de dados, de temas extraídos do jornal Tribuna Metalúrgica, do Sindicato dos Metalúrgicos do ABC.

Abstract: Affected by the sophistication of labor flexibilization and control mechanisms, unions face perhaps the greatest challenge to their collective power in their history. This text intends to bring arguments to the debate on this “crisis of unionism”, from an analytical perspective (of “power resources”) that values efforts to re-signify practices of resistance, collectivism and solidarity, at the national and international level, and which sees in this pandemic context (2020 and 2021) an opportunity for the recomposition of its repertoires of collective action. (Alonso, 2012; Schmalz et al., 2018). The empirical reference is the metallurgical union experience of a Brazilian industrial sector with the predominance of transnational companies considering their ability to respond proactively to threats to their consolidated power over the last six decades in São Paulo’s ABC. The data used are based on seven recorded interviews (online), conducted in 2020 and 2021, with metallurgical union leaders and a survey, by the method of scraping data, of topics extracted from the newspaper Tribuna Metalúrgica of ABC Metalworkers Union.

Abstract Next-generation sequencing (NGS) has altered clinical genetic testing by widening the access to molecular diagnosis of genetically determined rare diseases. However, physicians may face difficulties selecting the best diagnostic approach. Our goal is to estimate the rate of possible molecular diagnoses missed by different targeted gene panels using data from a cohort of patients with rare genetic diseases diagnosed with exome sequencing (ES). For this purpose, we simulated a comparison between different targeted gene panels and ES: the list of genes harboring clinically relevant variants from 158 patients was used to estimate the theoretical rate of diagnoses missed by NGS panels from 53 different NGS panels from eight different laboratories. Panels presented a mean rate of missed diagnoses of 64% (range 14%-100%) compared to ES, representing an average predicted sensitivity of 36%. Metabolic abnormalities represented the group with highest mean of missed diagnoses (86%), while seizure represented the group with lowest mean (46%). Focused gene panels are restricted in covering select sets of genes implicated in specific diseases and they may miss molecular diagnoses of rare diseases compared to ES. However, their role in genetic diagnosis remains important especially for well-known genetic diseases with established genetic locus heterogeneity.

A proposta do texto é relacionar as alterações recentes na legislação brasileira do trabalho (principalmente a partir da reforma trabalhista aprovada pelo Congresso Nacional, em 2017) com os interesses das empresas multinacionais instaladas no país, tomando como exemplo a indústria automotiva. Busca identificar de que forma, nesses últimos anos, a redução da proteção laboral e o aprofundamento da instabilidade no emprego têm sido utilizados pelas empresas para aumentar o controle sobre os trabalhadores e para desqualificar a ação de suas instituições de representação. A precarização dos laços de emprego e o uso, agora legal, de terceirização e quarteirização foram incorporados às atividades produtivas e aprofundaram estratégias voltadas para o trabalho como formas de obter melhores resultados econômicos.

The text aims at relating recent changes in the Brazilian labour legislation (mainly after the Labour Reform, approved by the National Congress in 2017) with the interests of the multinational companies based in the country, taking as an example the automotive industry. The purpose is to identify how, in the last years, the reduction of labour protection and the increase in employment instability have been used by companies to raise the control over workers and to disqualify trade union action. Precarious jobs, and the use, now legal, of subcontracting, have been incorporated to production, and have strengthened strategies that stress on work to improve their economic results.

La proposition du texte est de faire le lien entre les récents changements apportés à la législation brésilienne du travail (principalement de la réforme du travail approuvée par le Congrès national, en 2017) avec les intérêts des multinationales installées dans le pays, en prenant comme un exemple de l’industrie automobile. Il cherche à identifier comment, ces dernières années, la réduction de la protection du travail et l’aggravation de l’instabilité de l’emploi ont été utilisées par les entreprises pour accroître le contrôle sur les travailleurs et disqualifier l’action de leurs institutions de représentation. La précarisation des liens d’emploi et l’utilisation, désormais légale, externalisée et quarteirisation ont été incorporées dans les activités productives et des stratégies approfondies visant à travailler comme moyens d’obtenir de meilleurs résultats économiques.

Resumo O sindicalismo metalúrgico do ABC Paulista criou, a partir do enfrentamento político e das greves durante o período da ditadura militar de 1964, condições para defender suas demandas econômicas na esfera pública, ao mesmo tempo em que se constituiu em uma força política no cenário nacional. A eleição de Luiz Inácio Lula da Silva para a Presidência da República do Brasil, em 2002, e sua permanência no cargo por oito anos (2003-2010), foi a confirmação da força política desse sindicalismo que estabeleceu novos parâmetros e influenciou os rumos da organização dos trabalhadores no país. O artigo busca - a partir de um levantamento de dados estatísticos referentes ao trabalho e ao emprego na região do ABC Paulista - estabelecer uma relação entre o contexto econômico e político do período Lula/Dilma Rousseff e as estratégias sindicais dos metalúrgicos, levando em conta as ligações políticas evidentes com o governo do Partido dos Trabalhadores, principalmente com o presidente operário. A intenção é avaliar, como resultado dessa proximidade, a existência de avanços e/ou recuos nas práticas de luta do sindicato no sentido de obter melhores salários, criar novas oportunidades de emprego, aumentar a atuação junto à sociedade civil e buscar interferir no debate sobre estratégias de desenvolvimento regional.

Abstract Since the political confrontation and strikes during the 1964 Brazilian military dictatorship, the metalworker trade unionism of the ABC has created viable conditions of defending its economic demands in the public sphere, and at the same time has become a political force in Brazil. The election of Lula in 2002 and his permanence in this position for eight years (2003-2010) were the confirmation of the political strength of this trade unionism, which established new parameters and influenced in the organization of the workers in the country. Based on statistical data collection on work and employment in the ABC region, this paper seeks to determine an association between the economic and political context of the Lula/Dilma period and the strategies of the metalworkers trade union, taking into account its clear political relationship with the Brazilian Workers´ Party, especially with a worker president. The objective is to evaluate, as a result of this relationship, the existence of advancements or regressions in the union’s struggles for better wages, new job opportunities, approximating themselves to civil society, and its attempt to interfere in the debate about regional development strategies.

RESUMO Os avanços significativos ocorridos desde o III Consenso Brasileiro sobre H. pylori realizado em 2012, em Bento Gonçalves, justificam este quarto consenso. O evento foi organizado pelo Núcleo Brasileiro para Estudo do Helicobacter e Microbiota, associação vinculada à Federação Brasileira de Gastroenterologia, tendo sido realizado novamente em Bento Gonçalves, RS, nos dias 25 a 27 de agosto de 2017. Participaram 26 delegados provenientes das cinco regiões brasileiras incluindo gastroenterologistas, endoscopistas e patologistas, além de um convidado internacional (EUA). Os participantes foram convidados pelo conhecimento e contribuição ao estudo da infecção por H. pylori. O encontro buscou rever diferentes aspectos relacionados ao tratamento da infecção, suas inter-relações com a dispepsia, microbiota e outras afecções, com ênfase especial ao câncer gástrico, além de promover uma reavaliação dos aspectos epidemiológicos e diagnósticos desta infecção. Os participantes foram alocados em quatro grupos, a saber: 1) Epidemiologia e diagnóstico, 2) Dispepsia, microbiota e outras afecções, 3) Neoplasias gástricas, e 4) Tratamento. Previamente à reunião do Consenso, os participantes receberam um tema a ser discutido e elaboraram texto com uma revisão recente da literatura, contendo uma assertiva de sua revisão. Todas as assertivas foram avaliadas em dois turnos de votação. Inicialmente, cada participante apresentava sua compilação e assertiva ao seu grupo, para eventuais modificações e votação. Posteriormente, em uma segunda votação, agora em sessão plenária, as assertivas eram novamente votadas e eventualmente modificadas. As votações obedeceram a cinco alternativas: 1) concorda fortemente; 2) concorda com reservas; 3) indeciso; 4) discorda e; 5) discorda fortemente. O índice de consenso adotado para cada afirmativa foi de 80% dos votantes respondendo que concorda fortemente ou concorda com reservas. As recomendações aqui apresentadas foram baseadas nas evidências científicas mais relevantes para o manuseio da infecção por H. pylori na população adulta no Brasil.

ABSTRACT Significant progress has been obtained since the III Brazilian Consensus Conference on H. pylori infection held in 2012, in Bento Gonçalves, Brazil, and justify a fourth meeting to establish updated guidelines on the current management of H. pylori infection. Therefore, the Núcleo Brasileiro para Estudo do Helicobacter pylori e Microbiota (NBEHPM), association linked to Brazilian Federation of Gastroenterology (FBG) held its fourth meeting again in Bento Gonçalves, RS, Brazil, on August 25-27, 2017. Twenty-six delegates, including gastroenterologists, endoscopists, and pathologists from the five regions of Brazil as well as one international guest from the United States, participated in the meeting. The participants were invited based on their knowledge and contribution to the study of H. pylori infection. The meeting sought to review different aspects of treatment for infection; establish a correlation between infection, dyspepsia, intestinal microbiota changes, and other disorders with a special emphasis on gastric cancer; and reassess the epidemiological and diagnostic aspects of H. pylori infection. Participants were allocated into four groups as follows: 1) Epidemiology and Diagnosis, 2) Dyspepsia, intestinal microbiota and other afections, 3) Gastric Cancer, and, 4) Treatment. Before the consensus meeting, participants received a topic to be discussed and prepared a document containing a recent literature review and statements that should be discussed and eventually modified during the face-to-face meeting. All statements were evaluated in two rounds of voting. Initially, each participant discussed the document and statements with his group for possible modifications and voting. Subsequently, during a second voting in a plenary session in the presence of all participants, the statements were voted upon and eventually modified. The participants could vote using five alternatives: 1) strongly agree; 2) partially agree; 3) undecided; 4) disagree; and 5) strongly disagree. The adopted consensus index was that 80% of the participants responded that they strongly or partially agreed with each statement. The recommendations reported are intended to provide the most current and relevant evidences to management of H. pylori infection in adult population in Brazil.

Resumo O artigo pretende fazer um balanço da incorporação do trabalho e da ação coletiva sindical à abordagem das redes globais de produção (RGPs) e de seus limites e potencial para o desenvolvimento de uma concepção sociológica multiagente e multiescalar do mundo do trabalho. Partindo da organização em rede das estratégias corporativas e seus impactos sobre as condições e relações de trabalho, a intenção é aprofundar o entendimento: (1) da conformação de redes laborais dispersas geograficamente, embora funcionalmente integradas; (2) do impacto destas sobre as formas de agência individual de trabalhadores, em suas variadas segmentações etária, étnica, de gênero e geográfica; e (3) das restrições e oportunidades à emergência de formas de ação e poder coletivo transnacionais condicionantes da agência econômica e política e influentes sobre a forma concreta dessas redes.

Abstract In this paper, we aim to review the incorporation of labor, union and collective action into the global production networks (GPNs) approach, highlighting their limits and potential for developing a multi-actor and multi-scale, sociological conception of labor. From the network-based corporate strategies and their impacts on industrial relations and working conditions, the discussion argues for a deeper understanding of: (1) the formation of geographically dispersed, however functionally integrated, labor networks; (2) their impact on workers’ individual agency according to their variegated age, ethnic, gender and geographic segmentations; and (3) the constraints and opportunities for the emergence of transnational forms of collective action and power, in order to affect the economic and political agency, and the concrete shape of these networks.

Symbiosis of common bean with nitrogen-fixing bacteria can lead to a reduction in production costs and in environmental impacts. Considering the diversity of soils and climates, it is important to validate bacterial strains under different conditions to consolidate their recommendation as inoculants. Studies concerning the economic viability of Rhizobium inoculation in common bean are few, but they could assist in choosing the strain to be adopted. The aim of this study was to evaluate the performance of Rhizobium strains isolated from acid soils from the Amazon inoculated in seeds of the BRSMG Madrepérola cultivar and to evaluate the economic viability of their use in the field. Four field experiments were conducted in the state of Minas Gerais, Brazil. A randomized block experimental design was used with four replications and seven treatments: inoculation with the strains CIAT 899 (Rhizobium tropici), UFLA 02-100 (R. etli), UFLA 02-68 (R. etli bv. mimosae), UFLA 02-127 (R. leguminosarum bv. phaseoli), and UFLA 04-173 (R. miluonense),plus two controls, one without inoculation + mineral nitrogen (N-urea) at a rate of 80 kg N ha-1 and another without inoculation and without mineral N. The strain CIAT 899 is already recommended for the manufacture of a commercial inoculant, and the others, collected in the Amazon Forest, have proven to be highly effective in biological nitrogen fixation. Symbiosis of common bean with all the strains evaluated leads to satisfactory yields, with reduction in the application rate of mineral nitrogen and in environmental impacts and, consequently, reduction in economic costs, with higher profitability.

Um novo ciclo de expansão econômica sobre o território da Amazônia brasileira, a partir dos anos 1960, redefiniu estratégias de desenvolvimento para a região e teve importantes repercussões sobre o meio ambiente e os grupos sociais locais. Este texto vai centrar sua análise em um desses projetos, o Programa Grande Carajás, que estimulou a implantação de indústrias produtoras de ferro-gusa ao longo da Estrada de Ferro Carajás, com a implantação de polos siderúrgicos nos municípios de Marabá (estado do Pará) e Açailândia (estado do Maranhão) e tentar mostrar como este tipo de intervenção se por um lado incrementou atividades econômicas regionais, fez crescer as áreas urbanas e criou novos empregos; por outro lado, teve desdobramentos que significaram “precarização" das relações de trabalho, degradação do meio ambiente, e desrespeito aos direitos humanos. O objetivo é avaliar as características desse mercado de trabalho a partir da atuação do sindicato dos metalúrgicos e dos movimentos sociais em diferentes tipos de mobilização social ocorridas no município de Açailândia, especialmente nas lutas desenvolvidas pelo sindicato contra o desemprego industrial e nas campanhas contra o “trabalho escravo".

From the 1960s onwards, the Brazilian Amazon witnessed a new cycle of economic expansion. This expansion redefined the development strategies for the region and had significant repercussion of the environment and local social groups. This texts analyses one of the projects implemented since then, the Grande Carajás Program, which stimulated the settlement of pig iron producing industries throughout the Estrada de Ferro Carajás, and of metallurgical plants in the municipalities of Marabá (State of Pará) and Açailandia (State of Maranhao). The article shows the dual impact of these policies. On the one hand it increased regional economic activity, the size of cities and towns, and created new jobs. On the other, it significantly affected the precarization of labour, environmental degradation and offenses to human rights. The objective is to analyze the characteristics of the labour markets through the actions of the metalworkers' union and other social movements in the municipalities of Açailandia, especially the struggles around industrial unemployment and campaigns against 'slave labour'.

Un nuevo ciclo de expansión económica sobre el territorio del Amazonas brasilero, a partir de los años 60s, redefinió estrategias de desarrollo para la región y tuvo importantes repercusiones sobre el medio ambiente y los grupos sociales locales. Este texto centra su análisis en uno de esos proyectos, el Programa Grande Carajás, que estimuló la implantación de industrias productoras de arrabio a lo largo de la Estrada de Ferro Carajás, con la implantación de polos siderúrgicos en los municipios de Marabá (estado de Pará) y Açailandia (estado de Maranhao) e intentar mostrar como este tipo de intervención por un lado incrementó las actividades económicas regionales, hizo crecer las áreas urbanas y creó nuevos empleos; por otro lado tuvo desdoblamientos que significaron la 'precarización' de las relaciones de trabajo, la degradación del medio ambiente y falta de respeto a los derechos humanos. El objetivo es analizar las características de este mercado de trabajo a partir de la actuación del sindicato de metalúrgicos y de los movimientos sociales en diferentes tipos de movilización social ocurridas en los municipios de Açailandia, especialmente en las luchas desarrolladas por el sindicato contra el desempleo industrial y en las campañas contra el 'trabajo esclavo'.

Resumo A entrevista com o sociólogo britânico Huw Beynon, autor de Trabalhando para a Ford, entrelaça as dinâmicas de trajetória intelectual, desenvolvimento institucional da sociologia (do trabalho, em particular), da política britânica e da tradição marxista. O autor reflete sobre uma vasta experiência na construção e difusão de uma sociologia crítica, centrada no engajamento com seu objeto e público extra- acadêmico. Destacam-se aqui as experiências em educação de adultos e de extensão com trabalhadores, as primeiras expressões da globalização e neoliberalismo como "movimento de fechamento" de fábricas - assim como as reações acadêmicas e sindicais ao processo -, além de respostas político-culturais recentes acerca da preservação de uma identidade operária enraizada e da reconstrução do Partido Trabalhista em torno da adesão da juventude. A entrevista com Huw Beynon sintetiza os elementos de uma obra que é parte inseparável das transformações no mundo do trabalho nos últimos cinquenta anos, assim como se apresenta como inestimável para sua compreensão sociológica.

Abstract In this interview with British sociologist Huw Beynon, author of Working for Ford, the dynamics of his intellectual trajectory, the institutional development of sociology (of labor, specially), British politics and Marxist tradition are entwined. The author reflects on an extensive involvement in setting up and advancing critical sociology, focused on engaging with its object and extra academic audience. We highlight his practice in adult education and "extra mural" activities with workers, the first expressions of globalization and neoliberalism as a "closure movement" of factories - and the academic and trade union reactions - as well as recent political and cultural responses towards the preservation of labor identity and the reconstruction of the Labour Party around youth membership. The interview with Huw Beynon summarizes a body of work that is as much inseparable of the changes in the world of labor over the past fifty years as is priceless to its sociological understanding.

O objetivo deste estudo foi avaliar o uso de uma fonte de colina vegetal em substituição ao cloreto de colina em dietas para frangos de corte de um a 21 e 22 a 42 dias de idade. No primeiro experimento foram usados 1500 frangos de corte machos Cobb com um dia de idade, distribuídos em um delineamento inteiramente ao acaso com dez tratamentos, cinco repetições e 30 aves por unidade experimental. No segundo experimento foram usados 1000 frangos de corte machos Cobb com 22 dias de idade distribuídos em um delineamento inteiramente ao acaso com dez tratamentos, cinco repetições e 20 aves por unidade experimental. Os tratamentos consistiram em cinco níveis de cloreto de colina 60% (400, 500, 600, 700 e 800 mg/kg) e cinco da fonte de colina vegetal (100, 150, 200, 250 e 300 mg/kg). Não houve diferença entre as médias dos parâmetros produtivos nas duas fases experimentais. O uso de até 100 mg/kg de uma fonte vegetal de colina pode substituir o uso de cloreto de colina, nos níveis estudados, em dietas a base de milho e farelo de soja para frangos de um a 21 e 22 a 42 dias de idade.

The objective of this study was to evaluate the use of a vegetal source of choline as a replacement of choline chloride in the diet for broilers from one to 21 and 22 to 42 days of age. In the first experiment, we used 1500 Cobb male broilers at one day of age, distributed in a completely randomized design with ten treatments, five repetitions and 30 birds per experimental unit. In the second experiment, we used 1000 Cobb male broiler at 22 days of age, distributed in a completely randomized design with ten treatments, five repetitions and 20 birds per experimental unit. The treatments consisted of five levels of choline chloride 60% (400, 500, 600, 700 and 800 mg kg-1) and five levels of vegetal source of choline (100, 150, 200, 250 and 300 mg kg-1). There was no difference between the performance parameters of broilers in the periods of one to 21 and 22 to 42 days of age. The use of up to 100 mg kg-1 of a vegetal source of choline can replace the use of choline chloride, in the studied levels, in corn-soybean meal diets for broilers from one to 21 and 22 to 42 days of age.
