A rinite alérgica acomete 10 a 30% da população, interferindo na qualidade de vida e na capacidade produtiva. Está associada à sinusite, otite, roncopatias e asma. A Rupatadina é um anti-histamínico de segunda geração, com elevada afinidade ao receptor histamínico H1 e potente inibição do fator ativador plaquetário (PAF). Tem rápido início de ação, longa duração e reduz os efeitos crônicos da rinite. OBJETIVO: Avaliar a eficácia e segurança da rupatadina no tratamento da rinite alérgica persistente. MATERIAL E MÉTODO: Estudo multicêntrico, aberto, prospectivo. Foram selecionados 241 pacientes em 13 centros no Brasil durante o período de outubro de 2004 a agosto de 2005. Foram analisados os sinais e sintomas da rinite e a tolerabilidade após 1 e 2 semanas. RESULTADOS: Redução do escore geral de 8,65 para 3,21 na semana 2 (p<0,001). Todos os sinais e sintomas melhoraram significativamente, e no primeiro dia de tratamento (p<0,001), com exceção da obstrução e secreção nasal, a partir do segundo dia (P<0,001). A frequência de eventos adversos foi 19,9%, sendo 27,7% na 1ª semana. CONCLUSÕES: A rupatadina é eficaz no controle da rinite alérgica persistente, é segura e apresenta baixos índices de efeitos colaterais.

Allergic rhinitis affects 10-30% of the population, negatively impacting one's quality of life and productivity. It has been associated with sinusitis, otitis media, sleep disorders, and asthma. Rupatadine is a second generation antihistamine with increased affinity to histamine receptor H1; it is also a potent PAF (platelet-activating factor) antagonist. It starts acting quite quickly, offers long lasting effect, and reduces the chronic effects of rhinitis. AIM: this study aims to assess the efficacy and safety of rupatadine in the treatment of persistent allergic rhinitis. MATERIALS AND METHOD: this is a multi-centric open prospective study. This study included 241 patients from 13 centers in Brazil and was held between October of 2004 and August of 2005. Signs and symptoms of rhinitis and tolerance to medication were analyzed after one and two weeks of treatment. RESULTS: reduction on general scores from 8.65 to 3.21 on week 2 (p<0.001). All signs and symptoms improved significantly in the first day of treatment (p<0.001), except for nasal congestion and secretion, which improved from the second day of treatment (p<0.001). Adverse events occurred in 19.9% of the cases, 27.7% on week 1. CONCLUSION: rupatadine effectively controls persistent allergic rhinitis; it is safe and presents low incidence of side effects.

OBJETIVO: Avaliar a sobrevida em pacientes com câncer de cabeça e pescoço recidivado e sem possibilidade de tratamento curativo. MÉTODO: Foram revisados os prontuários dos pacientes com carcinoma epidermóide de boca, orofaringe, hipofaringe e laringe tratados cirurgicamente (tumor primário e esvaziamento cervical) entre 1978 e 2001, e selecionados 140 com recidiva da doença acompanhados até o óbito pelo câncer. Após 1999, 30 pacientes receberam cuidados paliativos de uma equipe multidisciplinar. Foi avaliado o intervalo de tempo entre a recidiva não resgatável e o óbito, considerando o sítio primário, estadiamento inicial, sobrevida livre de doença e cuidados paliativos. Os resultados foram expressos em medianas e médias, com os respectivos quartis e percentis. RESULTADOS: A sobrevida livre de doença apresentou média de 30 semanas. A sobrevida média após a recidiva foi de 17 semanas (Q25-75% = 8 a 34 semanas). Dezessete pacientes (12%) sobreviveram por mais de 12 meses após a recidiva. O sítio primário, estadiamento inicial, local da recidiva, sobrevida livre de doença e os cuidados paliativos não influenciaram a sobrevida após a recidiva. CONCLUSÃO: A sobrevida após uma recidiva não resgatável é similar ao relatado para os pacientes não tratados. Os cuidados paliativos não aumentaram a sobrevida destes pacientes.

BACKGROUND: To assess survival after squamous cell carcinoma (SCC) recurrence in surgical patients with head and neck cancer METHODS: Files of patients submitted to loco-regional treatment (primary tumor and neck dissection) of SCC of the mouth, oropharynx, hypopharynx and larynx between 1978 and 2001 were reviewed. One hundred-forty patients who developed disease recurrence were followed until death. After 1999, 30 of them received a multidisciplinary palliative care. Time interval between an unsalvageable recurrence and death was analyzed in front of primary site, initial stage of disease, disease-free survival and palliative care. Results were expressed as median and mean values, with their respective quartiles and percentiles. RESULTS: Median disease-free survival in this group was 30 weeks. The median survival after recurrence was 17 weeks (Q25-75% = 8 to 34 weeks). Seventeen patients (12%) survived more than 52 weeks after recurrence. The primary site, initial stage, recurrence site, disease-free survival and palliative care had no significant impact on survival after recurrence. CONCLUSION: Survival after unsalvageable recurrence of head and neck SCC is similar to reported in untreated patients. Palliative care do not increase survival of these patients.

A escolha de um cuidado alternativo para o bebê ou criança pequena geralmente não é tarefa fácil para os pais e envolve a análise de vários fatores. Isto fica mais difícil ainda quando esta escolha precisa ser feita nos primeiros meses de vida do bebê. O presente artigo tem por objetivo discutir os fatores que influenciam na escolha de um cuidado alternativo (i.e., parente, babá, creche) para o bebê e criança pequena e as vantagens e desvantagens apontadas pelos pais para as diferentes formas de cuidado. Dentre os fatores envolvidos no processo de decisão dos pais estão: a situação financeira e social da família; o apoio social recebido nos cuidados da criança; as práticas e crenças parentais sobre o cuidado das crianças; a escolaridade dos pais; a idade da criança; e o grupo étnico. Os estudos apontam que um cuidado alternativo adequado não só tranqüiliza os pais, como contribui para o desenvolvimento do bebê e para que seja mantida a qualidade da interação pais-bebê.

The choice of an alternative care for the infant is not an easy task for parents and it involves the analysis of several factors. It is even harder when this choice has to be made in the first months of the baby's life. This article aims to discuss the factors which have influence in choosing an alternative care (i.e., relatives, baby sitter, and day care) for the infant, and the advantages and disadvantages mentioned by parents for the different ways of care. The factors involved in the decision process for the parents are: the financial and social status of the family, the social support received in taking care of the child, and the ethnic group. Research shows that a proper alternative care not only relaxes and reassures parents as well as contributes to the development of the infant and keeps the quality of the parent-child bond.

O carcinoma epidermóide de lábio geralmente é diagnosticado em fase inicial e as metástases linfonodais são pouco freqüentes. OBJETIVO: Avaliar a incidência e a localização das metástases linfonodais no carcinoma epidermóide de lábio. FORMA DE ESTUDO: Estudo retrospectivo, série de casos. CASUÍSTICA E MÉTODO: Revisão de prontuários de 78 pacientes com carcinoma epidermóide de lábio, sem tratamento prévio, atendidos no período de 1990 a 2001. Foi avaliada a relação do tamanho do tumor primário, grau de diferenciação e comprometimento da comissura labial com a presença de metástases linfonodais, bem como a localização das metástases. RESULTADOS: As metástases linfonodais foram observadas em 7% dos tumores até 3 cm e em 41% nos tumores maiores do que 3 cm (p=0,002). Dez pacientes apresentavam metástases, sendo que todos estes tinham metástases no nível I e apenas 2 tinham metástases em outros níveis. Os pacientes submetidos ao esvaziamento eletivo apresentavam metástases apenas no nível I. CONCLUSÃO: As metástases são infreqüentes nos tumores menores do que 3 cm. Quando presentes, as metástases habitualmente acometem o nível I, portanto o esvaziamento suprahioideo pode ser indicado no tratamento eletivo do pescoço.

Squamous cell carcinoma of the lip is usually early diagnosed and shows low incidence of neck metastases. AIM: The study aimed at assessing the incidence and location of lymph node metastases in squamous cell carcinoma of the lip. STUDY DESIGN: Retrospective case-series study. MATERIAL AND METHOD: A review of 78 case records of patients treated between 1990 and 2001. The relation between primary tumor size, histological grading and commissure involvement with presence and location of lymph node metastases was evaluated. RESULTS: Lymph node metastases were detected in 7% of lesions < 3 cm and in 41% of tumors > 3 cm (p=0.002). Ten patients had metastases, out of which 8 were level I, and only 2 were of other levels. After elective treatment of the neck, only level I metastases were found. CONCLUSION: Metastases are not commonly found in lesions < 3 cm. If present, they are usually level I and, in this case, suprahyoid neck dissection can be indicated for elective treatment.

OBJETIVO: Analisar o padrão de disseminação local através da delimitação clínica da extensão da lesão primária assim como os subsítios invadidos. FORMA DE ESTUDO: Clínico retrospectivo. MATERIAL E MÉTODO: Foram analisados os prontuários de 290 pacientes com carcinoma epidermóide de base de língua no Departamento de Cirurgia de Cabeça e Pescoço e Otorrinolaringologia do Hospital Heliópolis, Hosphel, São Paulo - Brasil, de 1977 a 2000, sendo estadiados pelo TNM da UICC, e os resultados analisados pelo teste do Quiquadrado para tabelas Z x N (Cochran) para estudo da associação dos sítios e dimensão da neoplasia em relação à invasão da linha média. RESULTADOS: Com predomínio dos homens (8:1) e da 6ª década de vida (41,0%), 83,8% eram etilistas e tabagistas e em 4,7% os hábitos estavam ausentes. Quanto aos sintomas, odinofagia (37,6%), linfonodo (21,7%) e a média de tempo entre o 1º sintoma e o diagnóstico de 6 meses (62,0%). Quanto ao estadiamento, tivemos T1-T2 (18,3%), T3 (32,4%), T4(50,7%). Quanto à disseminação local, em direção à valécula (25,3%), epiglote (18,7%), glote (2,7%), anteriormente para o v lingual em (22,4%) e póstero lateralmente para a prega faringloepiglótica (6,6%) e seio piriforme (2,2%). Quanto a ultrapassagem da linha média, isso ocorreu em 66,2% dos casos, sendo 42,2% (T2), 54,2% (T3) e 82,9% (T4). CONCLUSÃO: o carcinoma epidermóide no estádio T4 ultrapassa a linha média da base da língua em 82,9%.

AIM: To analyse the local spreading pattern through clinical delimitation of primary lesion extension as well as subsites involvement. STUDY DESIGN: Chart review. MATERIAL AND METHOD: Files of 290 patients with squamous cell carcinoma (SCC) of the base of the tongue from Department of Head Neck and Surgery and Otorhinolaryngology of Hospital Heliópolis, Hosphel, São Paulo, Brazil from 1977 to 2000, were analysed. They were staged through TNM from UICC, and then through thoygh K square text with Z x N (Cochran) tables for assessment of sites and size association of neoplasia in relation of medium line invasion. RESULTS: Male predominance (8:1) in 6th decade (41,0%), 83,8% were alcohol and tobacco dependent and 4,7% without this habbit. About symptoms, odinophagia (37,6%), lymphonode (21,7%) and medium time between tho first sympton and diagnosis of 6 months 62,0%). About staging, T1-T2 (18,3%), T3 (32,4%), T4(50,7%). Concerning local spreading: valecula (25,3%), epiglotis (18,7%), glottis (2,7%), anteriorly to lingual V (22,4%) and postero-lateraly to pharyngoepiglotic fold (6,6%), piriform sinus (2,2%). About medium line involvement (66,2%), 42,2% were (T2), 54,2% (T3) and 82,9% (T4). CONCLUSION: the SCC in T4 stage reach the medium line of base of the tongue in 82,9%.

Polipose nasossinusal (PNS) é uma entidade de etiologia controversa, caracterizada por uma condição inflamatória da superfície mucosa das fossas nasais e seios paranasais, bilateralmente. A queixa principal do paciente consiste na obstrução nasal e, ao exame físico, observam-se freqüentemente massas polipóides ocupando as cavidades nasais em extensões variáveis. Além da rinoscopia anterior e da endoscopia nasal, o uso da tomografia computadorizada (TC) torna-se necessário para avaliação das fossas nasais e da presença ou não do acometimento dos seios paranasais por essas massas, bem como a sua extensão. Este trabalho tem como objetivo avaliar a concordância interobservadores, por meio da análise da tomografia computadorizada, de 32 casos de PNS. FORMA DE ESTUDO: Clínico prospectivo. CASUÍSTICA E MÉTODOS: Foram avaliadas 32 TC de pacientes portadores PNS por dois observadores experientes, separadamente, em relação à presença ou não de 3 sinais tomográficos sugestivos dessa doença: (1) alargamento infundibular do complexo ostiomeatal, (2) abaulamento lateral da lâmina papirácea e (3) apagamento do trabeculado ósseo etmoidal. RESULTADOS: Observou-se Qui-quadrado não significante para o primeiro e segundo sinais (p=0,7055 e p=0,2057) e significante para o terceiro (p=0,0040). Contudo, o coeficiente de correlação de Kendall entre os dois observadores foi significante para os três sinais tomográficos acima citados (p<0,001; p=0,01; p=0,03 respectivamente). CONCLUSÃO: A maior concordância entre os observadores esteve presente no alargamento infundibular com maior freqüência de positividade desse sinal.

Sinonasal polyposis (SNP) is a condition with a controversial aethiology, known by bilaterally inflammatory mucous membranes of nasal and paranasal sinuses. The major patient's complaint is nasal obstruction, and polypoid masses in different sizes can be found during nasal cavity examination. Beyond anterior rhinoscophy and nasal endoscopy, screening sinus computed tomography (SSCT) is necessary to measure the size and the extent of the polyps into nasal cavities and paranasal sinuses. The purpose of this study is to evaluate the concordance between two observers through SSCT of 32 cases with SNP. STUDY DESIGN: Clinical prospective. MATERIAL AND METHOD: CT scans were evaluated separately by two experts, taking into consideration three suggestive tomography signs of SNP: (1) Infundibular enlargement of ostiomeatal complex; (2) bulging of lamina papyracea and (3) bony attenuation of ethmoid trabeculae. RESULTS: Qui-square was not significant for either the first or the second signs (p=0,7055 and p=0,2057), but for the third one (p=0,0040). However, Kendall coefficient between the two observers was significant for all the three tomography signs mentioned before (p<0,001; p=0,01; p=0,03). CONCLUSION: The major concordance between the observers concerned the infundibular enlargement, which was also the most frequent sign.

INTRODUÇÃO: O Lúpus Eritematoso Sistêmico (LES) é uma doença sistêmica do tecido conectivo, de etiologia desconhecida, provavelmente multifatorial. Acomete principalmente o sexo feminino podendo afetar múltiplos órgãos, dentre eles o sistema auditivo. A orelha interna pode ser lesada por diversos mecanismos auto-imunes, sendo a manifestação mais freqüente a disacusia sensorioneural flutuante, geralmente bilateral, rapidamente progressiva e com boa responsividade a imunossupressores. OBJETIVO: O objetivo deste trabalho é relatar três casos de disacusia de etiologia auto-imune, enfocando formas de acometimento e manifestações clínicas, bem como correlacionando o efeito ototóxico da cloroquina - droga empregada no controle do LES - com a perda auditiva. CONCLUSÃO: As perdas auditivas sensorioneurais súbitas, rapidamente progressivas ou flutuantes, podem ocorrer em pacientes com doença auto-imune e devem ser sempre lembradas nos casos de disacusia sem causa aparente.

INTRODUCTION: The Systemic Lupus Erythematosus (SLE) is a systemic disease of the connective tissue, with unknown etiology, probably associated to multiple events. It is a multiple organs disease that affects mainly women. The inner ear can be damaged by several immunopathogenic mechanisms, and the most common symptom is a progressive sensorineural hearing loss, generally bilateral, with good response to immunossupression. AIM: The purpose of this article is to report three cases of women suffering from SLE and hearing loss and to establish a link between the autoimmune and the vascular mechanisms of the disease, also focusing attention on the ototoxicity due to chloroquine applied during the treatment of SLE. CONCLUSION: Sudden or fluctuant sensorineural hearing loss may affect patients with autoimmune disease, so it must always be taken into account when dealing with patients suffering from hearing loss without any apparent cause.

Com o passar da idade, ocorrem progressivamente ossificação e formação de medula óssea nas cartilagens laríngeas. O conhecimento dessas modificações é importante para a diferenciação com invasão por câncer. OBJETIVO: Avaliar comparativamente as porções cranial e caudal da cartilagem tireóidea quanto a alterações histológicas em indivíduos adultos do sexo masculino, observando a ossificação e a formação de medula óssea. FORMA DE ESTUDO: Experimental. MATERIAL E MÉTODO: Foram estudadas 7 laringes obtidas de cadáveres do sexo masculino submetidos a exame necrológico no Posto Médico-Legal de Santos, da Polícia Civil do Estado de São Paulo. A idade variou de 39 a 60 anos, com mediana de 53. Não se teve acesso à informação de hábito de tabagismo e etilismo. Os espécimes foram descalcificados e corados pela hematoxilina-eosina. A ossificação foi estudada pelo método histométrico baseado na estereologia e a presença de medula óssea foi realizada de forma qualitativa (presença ou ausência). RESULTADOS: Nos segmentos craniais, notamos uma média de 42,8% de ossificação e a presença de medula óssea em 57,1%. Nos segmentos caudais, a média de ossificação foi de 72,8%, com medula óssea em 87,5%. CONCLUSÃO: A ossificação da cartilagem tireóidea e a formação de medula óssea são mais intensas e precoces na sua topografia cranial.

The process of ossification and formation of bone marrow occurs in the larynx cartilages with age. The knowledge of this change is important for differentiation with cartilage invasion by cancer. AIM: Cranial and caudal segments of the thyroid cartilage were obtained from adult male larynges in order to evaluate comparatively the histological changes regarding to the ossification and the bone marrow formation. STUDY DESIGN: Experimental. MATERIAL AND METHOD: Seven male adult larynges were obtained from cadavers underwent necrological examination in Posto Médico-Legal de Santos, da Polícia Civil do Estado de São Paulo. The ages ranged from 39 to 60, with median of 53. There were no available data about tobacco and alcohol consumption. The specimens underwent decalcification and stained with H-E. Histometry was performed for quantitative analysis of the ossification while the bone marrow was analyzed according to qualitative criteria (presence or absence). RESULTS: The average ossification was 42.8% and the bone marrow presence was observed in 57.1% in the cranial segments. In the caudal segments, the average ossification was 72.8%, and the bone marrow was detected in 87.5% of the specimens. CONCLUSION: The ossification in the thyroid cartilage and the bone marrow formation increase and occur earlier moving downward from the cranial to the caudal aspects.

Teleangiectasia Hemorrágica Hereditária (Síndrome de Rendu-Osler-Weber) consiste de uma rara displasia fibrovascular sistêmica, de transmissão autossômica dominante, com incidência de 1-2/100000, afetando vasos sangüíneos da pele, mucosas, pulmões e trato gastrointestinal. É reconhecida pela tríade clássica de teleangiectasias em face, mãos e cavidade oral, epistaxes recorrentes e histórico familiar. Em 90% dos casos, a epistaxe recorrente é o principal sintoma, porém a doença pode afetar qualquer parte do organismo. O tratamento é paliativo, sendo ainda controverso. Neste estudo relatamos três pacientes portadores da doença, enfocando aspectos clínicos e tratamento.

Hereditary Haemorrhagic Telangiectasia (Rendu-Osler-Weber disease) is a rare disease with and incidence of 1-2/100000, affecting blood vessels of the skin, mucous membrane, lungs and gastrointestinal tract. It is recognized as a classic triad of teleangiectasia in face, hands and cavity; recurrent epistaxe and family occurrence. We reported three patients with Rendu-Osler-Weber disease and discussed about clinical manifestations and treatment.

Objetivo: avaliar um questionário para mensuração da qualidade de vida e sua relação com o prognóstico em pacientes com câncer de cabeça e pescoço. Forma de estudo: prospectivo clínico. Material e Método: estudo de 31 pacientes com carcinoma epidermóide de vias aerodigestivas superiores atendidos no Departamento de Cirurgia de Cabeça e Pescoço e Otorrinolaringologia do Hospital Heliópolis, Hosphel, São Paulo, no período de agosto de 1999 a novembro de 2000. Na avaliação da qualidade de vida empregou-se o questionário QLQ-C30 e o módulo QLQ-H&N35, ambos fornecidos pela EORTC, sendo realizada previamente ao tratamento. Foi avaliada a diferença de pontuação entre os pacientes que apresentavam doença controlada e aqueles com recidiva nos primeiros 12 meses após o tratamento. Resultados: foi observada uma diferença significativa na pontuação entre os pacientes assintomáticos e aqueles com recidiva da doença nas escalas relacionadas à fadiga, dificuldade de comer em público e qualidade de vida global (p <= 0,05). A pontuação obtida neste grupo de pacientes foi semelhante à observada em outras populações. Conclusão: a qualidade de vida prévia ao tratamento pode constituir um indicador prognóstico para os pacientes com câncer de cabeça e pescoço.

Aim: a quality life questionnaire (QLQ) assessment and its relationship with prognostic of patients with head and neck cancer. Study design: prospective clinical. Material and method: a study of 31 patients with squamous cell carcinoma of upper aerodigestive tract submitted to therapy in Head and Neck Surgery and Otorhinolaryngology Department of Heliópolis Hospital, Hosphel between August,1999 and November, 2000. For evaluation of quality of life it was used the questionnaire QLQ-C30 and the module QLQ-H&N35, both supplied by EORTC, accomplished previously to the treatment. The differences of scores among patients with controlled disease and those with recurrence in the first 12 months after treatment was evaluated. Results: a significant difference was observed in scores between assimptomatic patients and those with recurrence, when related to the fadiga, problems of eating in public and global quality of life (p <= 0,05). The scores in this group were similar to those observed in other populations. Conclusion: the pre-treatment quality of life can be a prognostic indicator for patients with head and neck cancer.

Introdução: Fazer a análise da presença da necrose tumoral microscópica no tumor primário inicial da laringe e correlacioná-la com algumas características clínicas e histopatológicas, objetivando identificar seu impacto na evolução. Forma de estudo: Retrospectivo clínico. Material e métodos: Estudo retrospectivo das fichas médicas e revisão dos cortes histológicos obtidos de 49 casos de carcinomas epidermóides da laringe, estadiados como T1 e T2, tratados no Serviço de Cirurgia de Cabeça e Pescoço do Complexo Hospitalar Heliópolis, de São Paulo/ SP, entre janeiro/1978 e dezembro/1997. Resultados: Houve forte associação entre a presença de necrose microscópica e a característica infiltrativa da lesão primária (p=0,004), lesões na supraglote (p=0,021), estádio clínico T2 (p=0,04), ocorrência de metástase cervical (p=0,04) e lesões menos diferenciadas (p=0,025). Aqueles casos que apresentaram necrose microscópica tenderam à melhor evolução. Conclusão: As informações obtidas do nosso estudo sugerem que a necrose por si, como classificada por técnicas histopatológicas, pode não ter influência exclusiva ou reflexo no crescimento volumétrico, refletindo a taxa de crescimento tumoral, mas pode estar relacionada a outros fatores tumorais e/ou do hospedeiro como a morte celular programada.

Introduction: To analyze the presence of microscopic tumoral necrosis in primary initial laryngeal tumor and to correlate it with clinical characteristics and histopathology to identify the impact in the evolution. Study design: Clinical retrospective. Material and methods: Retrospective study of the medical files and revision of histology sections of 49 squamous cell carcinoma of the larynx staged T1 and T2, treated in the Service of Head and Neck Surgery at Heliópolis Hospital, São Paulo /SP, between January/1978 and December/1997. Results: There was strong association between the presence of microscopic necrosis and the infiltrative characteristic of the primary lesion (p=0.004), lesions in the supraglottic (p=0.021), clinical staging T2 (p=0.04), occurrence of cervical metastasis (p=0.04) and less differentiated lesions (p=0.025). Those cases that presented microscopic necrosis tended to have the best evolution. Conclusion: The information gathered by our study suggested that necrosis itself, as classified by histopathology techniques, may not have exclusive influence or reflex on volume of growth, reflecting the rate of tumoral growth, but it may be related to other tumor factors and/or to a host, such as programmed cellular death.

OBJETIVO: Este trabalho tem como objetivo identificar as principais diferenças clínico-epidemiológicas do carcinoma epidermóide de cavidade oral no sexo feminino quando comparado ao sexo masculino. MÉTODOS: Foram estudados retrospectivamente os prontuários de 228 pacientes do sexo feminino portadoras de carcinoma epidermóide de cavidade oral, atendidos no Serviço de Cirurgia de Cabeça e Pescoço do Hospital Heliópolis, no período entre 1977 e 1996. As características clínico-epidemiológicas destas pacientes foram comparadas com aquelas de 849 pacientes do sexo masculino. RESULTADOS: A idade das pacientes das mulheres variou de 2 a 100 anos, com uma média de 60,7 anos. Nos homens encontramos idade entre 17 e 88 anos com uma média de 55,6 (p<0,00001). Quando o etilismo e o tabagismo estavam ausentes, a incidência do carcinoma epidermóide de boca ocorreu em uma faixa etária mais tardia. A presença do tabaco isoladamente não afetou a distribuição por faixa etária, enquanto que o álcool isoladamente ou em associação com o tabaco leva a uma incidência mais precoce do câncer de boca. Os dois principais sítios de incidência são a língua e o soalho. As mulheres têm uma menor incidência de lesões de língua e soalho (43%) quando comparado aos homens (61%). Aproximadamente 50% das mulheres eram estadio III e IV no momento do diagnóstico. Entre os homens este índice atinge 53%. CONCLUSÕES: O carcinoma epidermóide de cavidade oral nas mulheres tem características clínico-epidemiológicas peculiares que justificam o desenvolvimento de protocolos de diagnóstico e tratamento específico para ele.

BACKGROUND: Squamous cell carcinoma (SCC) of the oral cavity occurs mainly in the male population. The objective of the study is to identify the main clinical and epidemiological differences of the oral SCC between men and women. METHODS: Retrospective analysis of the charts of 228 cases of SCC of the oral cavity in women treated at the Head & Neck Surgery Service, Heliopolis Hospital, São Paulo, Brazil from 1977 to 1996. The clinical and epidemiological characteristics of these cases were compared with those of 849 male patients. RESULTS: The mean age of women was 60.7 years (ranging 2 to 100 years) and in the male group was 55.6 years (p<0.00001). In cases where the alcohol intake and tobacco use were absent, the tumors ocurred in older patients. The isolated tobacco use didn't affect the distribuction by age but for the alcohol intake (alone or combined with tobacco) the patients were younger. On the alcohol intake, 71% of the women and 8% of the man used no alcohol. The most common sites of tumors were tongue and floor of the mouth but in the women group the incidence of tumors in these sites were 43% compared with 61% in the men. CONCLUSIONS: The highest incidence of SCC the of oral cavity occurs later in women, than in men. This could be explained by a lesser exposure to alcohol and tobacco in women. Other factors seem to be involved in this issue.

Introdução: Avaliar quantitativamente o índice de fluxo salivar total não estimulado de pacientes portadores de carcinoma espinocelular de boca e orofaringe, após tratamento radioterápico, exclusivo, tratados com a associação de lauril-dietileno-glicol-éter-sulfato de sódio e hidróxido de cálcio. Forma de estudo: Prospectivo clínico não randomizado. Método: o fluxo salivar foi avaliado em 11 pacientes (n=11) em três fases: 1ª.) durante sete semanas, quando foram submetidos à telecobaltoterapia na dose total de 7.000 cGy, sem utilização de medicamento para estimular a salivação; 2ª.) em seguida, durante oito semanas após o tratamento radioterápico, quando os pacientes utilizaram a associação medicamentosa como estimulante da salivação; 3ª.) finalmente, durante oito semanas após ter sido interrompido o tratamento com a associação de drogas. Resultados: os valores obtidos foram analisados com vistas a avaliação da eficácia da medicação utilizada para evitar a hipofunção salivar no período pós-radioterapia. Dos 11 pacientes, 10 apresentaram melhora significante dos índices de fluxo salivar com o uso da associação de drogas, em relação aos valores do período final do tratamento radioterápico, e mantiveram esses índices mesmo com a suspensão do medicamento. Conclusões: a associação de drogas foi eficaz no tratamento da hipofunção salivar, e proporcionou aumento do índice salivar total não estimulado em 10 dos 11 pacientes, sendo esse aumento mantido por dois meses após a interrupção do uso do medicamento.

Introduction: Evaluation of quantitative non-stimulated salivary flow rate in 11 patients with oral and oropharyngeal squamous cell carcinoma after exclusive radiation therapy, treated with the association of lauryl-diethylene-glycol-ether-sodium sulfate and calcium hydroxide. Study design: Prospective clinical no randomized. Method: salivary flow was evaluated in three phases: 1) during seven weeks when they were submitted to radiotherapy with total dose of 7,000 cGy, without salivary stimulation with the association of drugs; 2) during eight weeks after radiation treatment, when the patients had salivary stimulation with the association of drugs; 3) for eight weeks after the end of treatment with the association of drugs. Results: These data were analysed in order to evaluate the efficacy of the medicament in avoiding salivary hypofunction after radiotherapy. Ten out of eleven patients showed significant better outcome in the salivary flow rates with the use of drugs related to the values obtained at the final period of radiotherapy and mantained these rates even without continuing using the drugs. Conclusions: the association of drugs was effective in the treatment of salivary hypofunction and promoted increase, in the total non-stimulated salivary flow rates in ten out of eleven patients, and this enhancement, was maintained for two months after interruption of the use of the medicament.

Forma de estudo: Retrospectivo clínico. Material e método: Para elaboração da presente pesquisa, foi realizada a revisão de 187 prontuários médicos de pacientes provenientes do Serviço de Infectologia do Ambulatório de Micoses Profundas do Hospital de Clínicas da Universidade Federal do Paraná em Curitiba-PR, no período de maio de 1985 a março de 1998. Os objetivos foram traçar o perfil epidemiológico, descrevendo as manifestações estomatológicas encontradas na amostra, discutir sobre os achados encontrados estabelecendo comparações com a literatura pertinente, observar possíveis mudanças e se estas são significativas, bem como observar a sensibilidade dos corantes que foram usados na histopatologia (HE, PAS e GROCOTT) em 20 lâminas. Foi estabelecido um protocolo prévio, para obtenção de dados, referentes à epidemiologia da doença, como as manifestações orais, descrevendo o tipo e localização destas lesões. Nos resultados encontrados, o sexo, a idade, a naturalidade, a procedência e a profissão dos pacientes foram fatores determinantes na descrição do perfil epidemiológico e demográfico da amostra estudada (187 casos), comparada com a literatura pertinente. Resultado: A avaliação dos resultados evidenciou mudanças pouco significativas em relação à epidemiologia da doença. E ficou bem evidenciada, também, a exuberância nas manifestações orais da doença, que são muito comuns e com freqüência levam à piora do estado geral do paciente ou mesmo à manutenção da doença, pela dificuldade de alimentação que aumenta a imunossupressão. Da amostragem dos 187 pacientes revista por este estudo, 167 (89,3%) eram do sexo masculino e 20 (10,7%) do feminino. Em relação à idade, 75 (40,1%) estão entre 41 e 50 anos, destacando-se alguns aspectos: 81 pacientes (43,3%) eram agricultores, sendo que 93 (49,7%) eram oriundos do interior do Paraná. A perda de peso foi relatada em 72 (17,22%) das 418 queixas, seguidas de dispnéia. Em relação à forma da lesão, 85 (41,46%) dos pacientes não apresentavam lesões visíveis. Naqueles em que as lesões eram visíveis, as mais comuns foram a ulcerada e a granulomatosa, seguida esta da moriforme. O local com maior incidência de lesões foi a região de lábios, seguida de orofaringe e de palato duro; foi também observado o aparecimento de mais de um tipo de lesão em um mesmo paciente. Dos 187 pacientes, cinco (2,65%) apresentavam tuberculose concomitantemente, e 140 (72,54%) não apresentavam linfonodos palpáveis. Dos pacientes 127 (67,92%) negaram fazer uso de bebida alcoólica, e 117 (62,57%) eram tabagistas. O envolvimento pulmonar foi predominante em 160 (51,78%) dos casos analisados. Na histopatologia, a técnica da coloração pela prata (GROCOTT) mostrou-se mais sensível, comparada às colorações HE e o PAS, para a visualização na demonstração do agente etiológico P. brasiliensis.

Study design: Clinical retrospective. Material and method: develop this research it had been realized a retrospective in reference book of 187 patients proceeding from the Infectology Service at the Deep Mycosis Ambulatory at Federal University of Paraná, in Curitiba, between may 1985 and march 1998. The purpose of this paper was to delineate the epidemic outline describing the stomatologicals manifestations found in the sample, to discuss about the data found and to establish comparison with the pertinent literature, to observe possible changes and if they were significant as well as to notice the dyes sensibility that were used in the histopathology (HE, PAS and GROCOTT), in 20 microscopic slides. It had been settled a previous protocol to get the data concerning to the desease epidemiology such as oral manifestations, describing the kind and localization of the lesions. In the results that were found, references like sex, age, naturalness, origien and profession of the patients were determinant factors in the epidemiologic and demographic outline of the sample studied (187 cases) compared with the pertinent literature. Results: The evaluation of the results showed changes that were not to much significant in relation to the desease epidemiology. On the other hand, it was evidenced the exuberance in oral manifestations of the desease that is very commom and frequently leads the patient to a worse condition or even to a maintenance of the desease by the difficulty of nutrition that increases the immunesuppression. In the sample of these 187 patients studied, 167 (89,3%) were male and 20 (10,7%) female. In relation to the age, 75 (40,1 %) were between 40 and 50 years, with some relevant aspects:81 patients (43,3 %) were agricultural workers, 93 (49,7 %) were from inland areas of Paraná. The loss of weight was related in 72 (17,22%) of the 187 complaints, followed dyspnea. In relation to the shape of the lesions, 85 (41,46%) of the patients did not show visible, the most commom were the ulcerated ones, the granulomatuos ones, followed by moriform one. The place with major incidence of lesions were lips, followed by oropharynx and hard palate, it was also noticed more than one kind of lesion in the same patient, between these 187 patients, five (2,65%) showed also tuberculosis, 140 (72,54%) did not palpable lymphonodes, 127 (67,91%) of the patients denied to use alcoholic drinks, 117 (62,57%) were smokers. The pulmonary envelopment was predominant in 166 (51,78%) of the analized cases. In the histopathology, the technique of dyeing by silver (GROCOTT) showed it self more sensitive than HE and PAS for visualization in demonstration of the etiologic agent P. brasiliensis.
