ABSTRACT: Skin grafting is a simple and very useful surgical technique for wound repair, especially where there is difficulty in performing direct synthesis or other reconstructive techniques. low-intensity laser (LIL) has already been used successfully in cases where the graft was applied to wounds shortly after its creation. However, LIL still lacks standardization due to conflicting studies on its results. Thus, this study compared the effectiveness of continuous wave LIL with pulsed wave LIL in stimulating the healing of skin grafts, applied to newly created wounds in rabbits. For this purpose, 31 rabbits were distributed into: control group (CG, n = 10), continuous wave laser group (GLC; n = 10) and pulsed wave laser group (GLP; n = 11). Postoperative management was similar between all groups, with the exception of laser application, as indicated by the group. The GLC and GLP groups showed similar evolution, with a satisfactory healing process of the graft, unlike the GC group. These results were maintained in the microscopic evaluation, as the groups treated with laser showed less inflammation, better integration with the receptor area, greater re-epithelialization and collagenization. There was no difference in vascular density between the treatment groups on immunohistochemical analysis. The newly created wound was not able to provide nutrition for the skin graft in rabbits, but LIL is an efficient adjuvant therapy in stimulating healing between the recipient area and the graft, promoting full integration. However; although, statistically there was no difference in the form of light emission, pulsed or continuous, patients who received continuous wave LIL showed superior microscopic evolution.

RESUMO: A enxertia cutânea é uma técnica cirúrgica simples e bastante útil para o reparo de feridas, principalmente, quando há dificuldade em realizar a síntese direta ou outras técnicas reconstrutivas. O laser de baixa intensidade (LBI) já foi utilizado com sucesso em casos em que o enxerto foi aplicado em feridas logo após a sua criação. No entanto, a LBI ainda carece de padronizações, mediante estudos conflitantes sobre seus resultados. Desta forma, este trabalho objetivou comparar a eficácia da LBI de onda contínua com onda pulsada na estimulação da cicatrização de enxertos cutâneos, aplicados em feridas recém-criadas em coelhos. Para tanto, foram utilizados 31 coelhos distribuídos em: grupo controle (GC, n = 10), grupo laser onda contínua (GLC; n = 10) e grupo laser onda pulsada (GLP; n = 11). O manejo pós operatório foi semelhante entre todos os grupos, com exceção da aplicação do laser, conforme o grupo indicado. Os grupos GLC e GLP apresentaram evolução semelhante, com processo cicatricial satisfatório do enxerto, ao contrário do grupo GC. Tais resultados se mantiveram na avaliação microscópica, pois os grupos tratados com laser apresentaram menor inflamação, melhor integração à área receptora, maior reepitelização e colagenização. Não houve diferença na densidade vascular entre os grupos de tratamento na análise imuno-histoquímica. A ferida recém-criada não é capaz de fornecer nutrição para o enxerto cutâneo nos coelhos, mas a LBI é uma terapia adjuvante eficiente em estimular a cicatrização entre a área receptora e o enxerto, promovendo integração total. Entretanto, embora não houve diferença estatística na forma de emissão de luz, pulsada ou contínua, os pacientes que receberam LBI em onda contínua apresentam evolução microscópica superior.

Abstract The objective of the study was to measure the impacts of Pronaf access on family farming (FF) performance measures with analysis for the FF and its typologies Pronaf B and Pronaf V of the 2017 Agricultural Census. Variables related to family farmers, their establishments and climate were used. The methodology was composed of Entropy Balancing for treatment analysis and Minimal Weighted Squares to measure the impacts. The use of different levels of intensity confirmed the presence of regional concentrations in access in all analyzes. The results of the impacts on FF showed to be related to the Pronaf V typology, confirming the need to analyze the disaggregated FF. The impacts on the Pronaf B typology were negative, suggesting that these farmers are potentially using the obtained credit inappropriately and/or inefficiently. This reaffirms the need to integrate Pronaf with other policies, such as technical assistance and rural extension.

Resumo O objetivo do estudo foi mensurar os impactos do acesso ao Pronaf nas medidas de desempenho da agricultura familiar (AF) com análise para o AF e suas tipologias Pronaf B e Pronaf V do Censo Agropecuário 2017. Foram utilizadas variáveis relacionadas aos agricultores familiares, seus estabelecimentos e o clima. A metodologia foi composta por Balanceamento por Entropia para análise do tratamento e Mínimos Quadrados Ponderados para a mensuração dos impactos. O uso de diferentes níveis de intensidade confirmou a presença de concentrações regionais no acesso em todas as análises. Os resultados dos impactos sobre a AF mostraram-se relacionados à tipologia Pronaf V, confirmando a necessidade de analisar a AF desagregada. Os impactos na tipologia Pronaf B foram negativos, sugerindo que esses agricultores estão potencialmente utilizando o crédito obtido de forma inadequada e/ou ineficiente. Isso reafirma a necessidade de integrar o Pronaf a outras políticas, como assistência técnica e extensão rural.

ABSTRACT Objective: The benefit of atropine in pediatric tracheal intubation is not well established. The objective of this study was to evaluate the effect of atropine on the incidence of hypoxemia and bradycardia during tracheal intubations in the pediatric emergency department. Methods: This is a single-center observational study in a tertiary pediatric emergency department. Data were collected on all tracheal intubations in patients from 31 days to incomplete 20 years old, performed between January 2016 and September 2020. Procedures were divided into two groups according to the use or not of atropine as a premedication during intubation. Records with missing data, patients with cardiorespiratory arrest, cyanotic congenital heart diseases, and those with chronic lung diseases with baseline hypoxemia were excluded. The primary outcome was hypoxemia (peripheral oxygen saturation ≤88%), while the secondary outcomes were bradycardia (decrease in heart rate >20% between the maximum and minimum values) and critical bradycardia (heart rate <60 bpm) during intubation procedure. Results: A total of 151 tracheal intubations were identified during the study period, of which 126 were eligible. Of those, 77% had complex, chronic underlying diseases. Atropine was administered to 43 (34.1%) patients and was associated with greater odds of hypoxemia in univariable analysis (OR: 2.62; 95%CI 1.15–6.16; p=0.027) but not in multivariable analysis (OR: 2.07; 95%CI 0.42–10.32; p=0.37). Critical bradycardia occurred in only three patients, being two in the atropine group (p=0.26). Bradycardia was analyzed in only 42 procedures. Atropine use was associated with higher odds of bradycardia in multivariable analysis (OR: 11.00; 95%CI 1.3–92.8; p=0.028). Conclusions: Atropine as a premedication in tracheal intubation did not prevent the occurrence of hypoxemia or bradycardia during intubation procedures in pediatric emergency.

RESUMO Objetivo: Avaliar o efeito da atropina na incidência de hipoxemia e bradicardia durante a intubação orotraqueal no departamento de emergência pediátrica. Métodos: Estudo observacional, realizado em departamento de emergência pediátrica terciário em que foram analisados os registros de intubações orotraqueais de pacientes com 31 dias a 20 anos incompletos, entre janeiro de 2016 e setembro de 2020. Os procedimentos foram divididos em dois grupos de acordo com o uso ou não da atropina como pré-medicação durante a intubação. Foram excluídos os procedimentos com falhas no preenchimento dos dados, pacientes com parada cardiorrespiratória, cardiopatias congênitas cianóticas, e aqueles com pneumopatias crônicas com hipoxemia basal. O desfecho primário foi hipoxemia (saturação periférica de oxigênio ≤88%), enquanto os desfechos secundários foram bradicardia (queda >20% entre a frequência cardíaca máxima e mínima) e bradicardia crítica (frequência cardíaca <60 bpm) durante o procedimento de intubação Resultados: Foram identificados 151 procedimentos de intubação orotraqueal, sendo 126 elegíveis para o estudo. Desses, 77% tinham doenças subjacentes complexas e crônicas. A atropina foi administrada em 43 (34,1%) pacientes e foi associada a maiores chances de hipoxemia na análise univariada (OR: 2,62; IC95% 1,15–6,16; p=0,027), porém, não na análise multivariada (OR: 2,073; IC95% 0,416–10,32; p=0,373). A bradicardia crítica ocorreu em apenas três pacientes, sendo dois no grupo atropina (p=0,268). A bradicardia foi analisada em apenas 42 procedimentos. O uso de atropina foi associado a maior probabilidade de bradicardia (OR: 11,00; IC95% 1,3–92,8; p=0,028) na análise multivariável. Conclusões: Atropina como pré-medicação na intubação orotraqueal não evitou a ocorrência de hipoxemia ou bradicardia durante os procedimentos de intubação na emergência pediátrica.

Resumo Este estudo objetiva analisar as experiências de um homem trans durante o período gravídico-puerperal e a perspectiva de enfermeiras obstetras em formação, a partir das dinâmicas e da organização dos cuidados de saúde obstétricos em ambiente hospitalar. Trata-se de um estudo de abordagem qualitativa, baseado em estudo de caso, em que utilizou entrevistas e observações diretas para a coleta das informações. A análise foi feita a partir do marco teórico e normativo do Processo de Enfermagem, da Teoria dos Cuidados e da perspectiva teórico/crítica do transfeminismo. Os resultados estão organizados em seis categorias: Homem trans em contexto de gestação, parto e puerpério; parceria e dimensões parentais; dilemas enfrentados pelo casal grávido; impressões registradas pela profissional de enfermagem; compreensão do caso sob a lente teórica e epistemológica; implicações para os profissionais da saúde. Destaca-se a necessidade de promover espaços de educação permanente junto aos profissionais da saúde e reformular legislações de maneira a viabilizar a elaboração de políticas públicas baseada no respeito à diversidade e cuidado equânime, reconhecendo as especificidades da população trans nos contextos da gestação, parto e puerpério.

Abstract This study aims to analyze the experiences of a transgender man during the gestational-puerperal period and the perspective of obstetric nurses in training based on the dynamics and organization of obstetric healthcare in a hospital setting. This qualitative study is based on a case study approach, employing interviews and direct observations to collect data. The analysis was based on the theoretical and normative framework of the Nursing Process, the Theory of Caring, and the theoretical/critical perspective of transfeminism. The results are organized into six categories: Transgender man in the context of pregnancy, childbirth, and postpartum; partnership and parental dimensions; dilemmas faced by the pregnant couple; impressions recorded by the nursing professional; understanding of the case through a theoretical and epistemological lens; implications for healthcare professionals. We underscore the need to promote spaces for continuing education among healthcare professionals and to reformulate legislation in a way that enables the development of public policies based on respect for diversity and equitable care, recognizing the transgender population’s specificities in the contexts of pregnancy, childbirth, and postpartum.

Abstract Civil servants have different levels of work engagement throughout their working lives. These variations are the work engagement cycles, which occur based on available resources and work environment demands. This study describes the work engagement cycles of federal civil servants based on their professional life histories, highlighting the demands and resources of the work environment in their professional trajectory. Using the job demands-resources (JD-R) model and the cognitive maps methodology, it was possible to identify a positive cycle of work engagement, a reinforcing cycle (related to opportunities and appreciation), and two disequilibrium cycles, one related to dysfunctional productivity and the other to administrative discontinuity. The analysis of the engagement cycles allowed the identification of work environment resources that interfere in the engagement of public servants in different ways. Finally, the concept of “coping cycle” was used as a subsidy of policies for disengaged servants.

Resumen Los servidores públicos tienen diferentes niveles de compromiso en el trabajo a lo largo de su vida laboral. Estas variaciones, llamadas ciclos de compromiso laboral, ocurren debido a los recursos disponibles y las demandas del entorno laboral. Frente a eso, el presente trabajo buscó describir los ciclos de compromiso en el trabajo de los servidores públicos federales a partir de sus historias de vida profesional, destacando demandas y recursos del ambiente laboral relevantes en la trayectoria laboral de esas personas. Utilizando el modelo job demands-resources (JD-R) y la metodología de mapas cognitivos, fue posible identificar un ciclo positivo de compromiso en el trabajo, un ciclo de refuerzo ‒ relacionado con oportunidades y apreciación ‒ y dos ciclos de desequilibrio, uno relacionado con la productividad disfuncional y el otro con la discontinuidad administrativa. El análisis de los ciclos de compromiso permitió identificar los recursos del ambiente de trabajo que interfieren de diferentes formas en el compromiso de los servidores públicos. Finalmente, se utilizó el concepto de “ciclo de afrontamiento” como base de las políticas para los servidores desinteresados.

Resumo Servidores públicos têm diferentes níveis de engajamento no trabalho ao longo da vida laboral. Essas variações, denominadas “ciclos de engajamento no trabalho”, ocorrem graças aos recursos disponíveis e às demandas do ambiente de trabalho. Diante disso, o presente artigo busca descrever os ciclos de engajamento no trabalho de servidores públicos federais com base em suas histórias de vida profissional, evidenciando demandas e recursos relevantes do ambiente ocupacional na trajetória dessas pessoas. Usando o modelo job demands-resources (JD-R) e a metodologia de mapas cognitivos, foi possível identificar um ciclo positivo do engajamento no trabalho, um ciclo reforçador - relacionado com oportunidades e valorização - e dois ciclos de desequilíbrio, um ligado à produtividade disfuncional e, outro, à descontinuidade administrativa. A análise dos ciclos de engajamento permitiu identificar recursos do ambiente de trabalho que interferem de diferentes maneiras no engajamento de servidores públicos. Por fim, foi utilizado o conceito de “ciclo de enfrentamento” como subsídio de políticas para servidores desengajados.

ABSTRACT Membranous nephropathy is a glomerulopathy, which main affected target is the podocyte, and has consequences on the glomerular basement membrane. It is more common in adults, especially over 50 years of age. The clinical presentation is nephrotic syndrome, but many cases can evolve with asymptomatic non-nephrotic proteinuria. The mechanism consists of the deposition of immune complexes in the subepithelial space of the glomerular capillary loop with subsequent activation of the complement system. Great advances in the identification of potential target antigens have occurred in the last twenty years, and the main one is the protein “M-type phospholipase-A2 receptor” (PLA2R) with the circulating anti-PLA2R antibody, which makes it possible to evaluate the activity and prognosis of this nephropathy. This route of injury corresponds to approximately 70% to 80% of cases of membranous nephropathy characterized as primary. In the last 10 years, several other potential target antigens have been identified. This review proposes to present clinical, etiopathogenic and therapeutic aspects of membranous nephropathy in a didactic manner, including cases that occur during kidney transplantation.

RESUMO A nefropatia membranosa é uma glomerulopatia, cujo principal alvo acometido é o podócito, e acarreta consequências na membrana basal glomerular. Tem maior frequência em adultos, principalmente acima dos 50 anos. A apresentação clínica é a síndrome nefrótica, mas muitos casos podem evoluir com proteinúria não nefrótica assintomática. O mecanismo consiste na deposição de complexos imunes no espaço subepitelial da alça capilar glomerular com subsequente ativação do sistema do complemento. Grandes avanços na identificação de potenciais antígenos alvo têm ocorrido nos últimos vinte anos, e o principal é a proteína “M-type phospholipase-A2 receptor” (PLA2R) com o anticorpo anti-PLA2R circulante, o que possibilita avaliar a atividade e o prognóstico dessa nefropatia. Essa via de lesão corresponde aproximadamente a 70% a 80% dos casos da nefropatia membranosa caracterizada como primária. Nos últimos 10 anos vários outros antígenos alvo potenciais têm sido identificados. Esta revisão se propõe a apresentar de modo didático aspectos clínicos, etiopatogênicos e terapêuticos da nefropatia membranosa, incluídos os casos com ocorrência no transplante renal.

Abstract: The article examines the party system in Cape Verde, officially a multiparty state that in practice has a two-party system. The aim is to understand the stabilization of two parties, the Partido Africano da Independência de Cabo Verde and the Movimento para a Democracia, in the alternation of power even in the most recent legislative elections of 2016, the broadest party competition in the country’s political history. To this end, this discussion focuses on theoretical approaches to parties and party systems that allow us to understand this bipolarization. The analysis suggests that this practical effect makes a connection with the system of representation’s structure and also sheds light on political alignments at the parties’ origin.

Resumen: El artículo examina el sistema de partidos en Cabo Verde, oficialmente un estado multipartidista que en la práctica tiene un sistema bipartidista. El objetivo es entender la estabilización de dos partidos, el Partido Africano da Independência de Cabo Verde y el Movimento para a Democracia, en el relevo del poder incluso en las últimas elecciones legislativas de 2016, la más amplia competencia partidista de la historia política del país. Para lograrlo, se discuten los enfoques teóricos sobre los partidos y los sistemas de partidos que permiten entender su bipolarización. El análisis sugiere que ese efecto práctico vincula la estructura del sistema de representación y arroja luz sobre acontecimientos políticos de alineación en el origen de los partidos.

ABSTRACT Objectives: To validate the homeostasis model assessment (HOMA) of insulin resistance (IR) as a surrogate to the hyperglycemic clamp to measure IR in both pubertal and postpubertal adolescents, and determine the HOMA-IR cutoff values for detecting IR in both pubertal stages. Subjects and methods: The study sample comprised 80 adolescents of both sexes (aged 10-18 years; 37 pubertal), in which IR was assessed with the HOMA-IR and the hyperglycemic clamp. Results: In the multivariable linear regression analysis, adjusted for sex, age, and waist circumference, the HOMA-IR was independently and negatively associated with the clamp-derived insulin sensitivity index in both pubertal (unstandardized coefficient – B = −0.087, 95% confidence interval [CI] = −0.135 to −0.040) and postpubertal (B = −0.101, 95% CI, −0.145 to −0.058) adolescents. Bland-Altman plots showed agreement between the predicted insulin sensitivity index and measured clamp-derived insulin sensitivity index in both pubertal stages (mean = −0.00 for pubertal and postpubertal); all P > 0.05. The HOMA-IR showed a good discriminatory power for detecting IR with an area under the receiver operator characteristic curve of 0.870 (95% CI, 0.718-0.957) in pubertal and 0.861 (95% CI, 0.721-0.947) in postpubertal adolescents; all P < 0.001. The optimal cutoff values of the HOMA-IR for detecting IR were > 3.22 (sensitivity, 85.7; 95% CI, 57.2-98.2; specificity, 82.6; 95% CI, 61.2-95.0) for pubertal and > 2.91 (sensitivity, 63.6; 95% CI, 30.8-89.1, specificity, 93.7; 95%CI, 79.2-99.2) for postpubertal adolescents. Conclusion: The threshold value of the HOMA-IR for identifying insulin resistance was > 3.22 for pubertal and > 2.91 for postpubertal adolescents.

Este estudo analisou as ações judiciais de pacientes que solicitaram ao Sistema Único de Saúde produtos à base de canabidiol (CBD) durante o período de 2019 a 2022, descrevendo características sociodemográficas, clínicas e jurídicas. Trata-se de um estudo transversal composto pela avaliação das notas técnicas emitidas pelos Núcleos de Apoio Técnico do Judiciário (NatJus), que embasaram as decisões judiciais. Os dados foram obtidos do sistema e-NatJus, do Ministério da Justiça, utilizando técnicas de web scraping. Regressão logística foi empregada para estimar razões de chances com intervalos de 95% de confiança. Foram analisadas 1.115 notas técnicas das ações demandantes de CBD, das quais 54,7% dos pacientes eram do sexo masculino, com idade média de 18,4 anos, em sua maioria da Região Sul do país (38,8%), e 49,6% buscavam tratamento para epilepsia. Das ações com pareceres favoráveis, 28,8% não tinham evidências científicas, 26,5% pleitearam produtos sem registro na Agência Nacional de Vigilância Sanitária e 25,3% dos que tinham registro não estavam em conformidade com a indicação terapêutica. Os pacientes da Região Nordeste tiveram a chance de parecer favorável aumentada em 3 vezes; e os que tinham diagnóstico de epilepsia, em 2,3 vezes. Os pareceres técnicos que deram suporte aos magistrados para as decisões judiciais das demandas de pacientes por produtos à base de canabidiol no Brasil estavam, em sua maioria, em conformidade com evidências científicas, denotando a importância dos NatJus na qualificação do acesso a produtos medicinais no país.

El estudio analizó las acciones legales de pacientes que solicitaron al Sistema Único de Salud brasileño productos a base de cannabidiol (CBD) durante el período de 2019 a 2022, describiendo características sociodemográficas, clínicas y legales. Se trata de un estudio transversal compuesto por la evaluación de las notas técnicas emitidas por los Núcleos de Apoyo Técnico del Poder Judicial (NatJus) que basaron las decisiones judiciales. Los datos se obtuvieron del sistema e-NatJus, del Ministerio de Justicia brasileño, mediante técnicas de web scraping. La regresión logística se empleó para estimar los odds ratios con intervalos del 95% de confianza. Fueron analizadas 1.115 notas técnicas de las acciones demandantes de CBD que tenían 54,7 % de los pacientes del género masculino, con una edad media de 18,4 años, en su mayoría de la Región Sur del país (38,8%) y 49,6% buscaban tratamiento para la epilepsia. De las acciones con opiniones favorables, el 28,8% no tenían evidencias científicas, el 26,5% pleitearon productos sin registro en la Agencia Nacional de Vigilancia Sanitaria y el 25,3% de los que tenían registro, no estaban en conformidad con la indicación terapéutica. Los pacientes de la Región Nordeste tuvieron la posibilidad de opiniones favorables aumentada en 3,0 veces y los que tenían diagnóstico de epilepsia en 2,3. Los dictámenes técnicos que dieron apoyo a los magistrados para las decisiones judiciales de las demandas de los pacientes por productos a base de cannabidiol en Brasil estaban en su mayoría en conformidad con las evidencias científicas, denotando la importancia de NatJus en la calificación del acceso a productos medicinales en el país.

The study analyzed the lawsuits of patients who requested cannabidiol (CBD)-based products from the Brazilian Unified National Health System during the period from 2019 to 2022, describing sociodemographic, clinical, and legal characteristics. This is a cross-sectional study composed of the evaluation of the technical notes issued by the Center for Technical Support of the Judiciary (NatJus), which supports judicial decisions. The data were obtained from the e-NatJus system, of the Brazilian Ministry of Justice, using web scraping techniques. Logistic regression was used to estimate odds ratios with 95% confidence intervals. We analyzed 1,115 technical notes of the CBD plaintiffs, of which 54.7% of the male patients, with a mean age of 18.4 years, mostly from the South Region of the country (38.8%), and 49.6% sought treatment for epilepsy. Regarding the actions with favorable opinions, 28.8% had no scientific evidence, 26.5% pleaded for products without registration with the Brazilian Health Regulatory Agency, and 25.3% of those that had registration were not in compliance with the therapeutic indication. Patients from the Northeast Region had a chance of a favorable opinion increased by 3.0 times and those diagnosed with epilepsy by 2.3. The expert opinions that supported the magistrates for the judicial decisions regarding the demands of patients for cannabidiol-based products in Brazil were mostly in accordance with scientific evidence, denoting the importance of NatJus in the qualification of access to medicinal products in the country.

Abstract Introduction: Glyphosate is the most widely used herbicide worldwide and in Brazil. There is currently increasing concern about the effects of glyphosate on human health. The Brazilian Institute for Consumer Protection showed data on the presence of glyphosate in some of Brazil’s most consumed ultra-processed products. Currently, regulations on the upper limit for these residues in ultra-processed foods have yet to be established by the National Health Surveillance, and ultra-processed food consumption is independently associated with an increased risk of incident chronic kidney disease. Methods: Since an unbalanced diet can interfere with kidney function, this study aims to investigate the effect of daily intake of 5 mg/kg bw glyphosate in conjunction with a balanced diet and the possible impact on renal function in rats. Kidney function, kidney weight, markers of renal injury, and oxidative stress were evaluated. Results: There was a decrease in kidney weight. The main histopathological alterations in renal tissues were vacuolation in the initial stage and upregulation of the kidney injury marker KIM-1. Renal injury is associated with increased production of reactive oxygen species in mitochondria. Conclusion: This study showed changes in the kidney of rats exposed to a balanced diet with glyphosate, suggesting a potential risk to human kidney. Presumably, ultra-processed food that contain glyphosate can potentiate this risk. The relevance of these results lies in drawing attention to the need to regulate glyphosate concentration in ultra-processed foods in the future.

RESUMO Introdução: O glifosato é o herbicida mais utilizado no mundo e no Brasil. Atualmente, há uma preocupação crescente com os efeitos do glifosato na saúde humana. O Instituto Brasileiro de Defesa do Consumidor apresentou dados sobre a presença de glifosato em alguns dos produtos ultraprocessados mais consumidos no Brasil. Atualmente, as regulamentações sobre o limite máximo desses resíduos em alimentos ultraprocessados ainda não foram estabelecidas pela Vigilância Sanitária Nacional, e o consumo de alimentos ultraprocessados está independentemente associado a um risco maior de doença renal crônica incidente. Métodos: Como uma dieta desbalanceada pode interferir na função renal, este estudo tem como objetivo investigar o efeito da ingestão diária de 5 mg/kg pc de glifosato em conjunto com uma dieta equilibrada e o possível impacto na função renal em ratos. Foram avaliados função renal, peso dos rins, marcadores de lesão renal e estresse oxidativo. Resultados: Houve redução no peso dos rins. As principais alterações histopatológicas nos tecidos renais foram vacuolização no estágio inicial e regulação positiva do marcador de lesão renal KIM-1. A lesão renal está associada à produção aumentada de espécies reativas de oxigênio nas mitocôndrias. Conclusão: Esse estudo mostrou alterações nos rins de ratos expostos a uma dieta balanceada com glifosato, sugerindo um risco potencial ao rim humano. Presumivelmente, alimentos ultraprocessados que contenham glifosato podem potencializar esse risco. A relevância desses resultados está no fato de chamar a atenção para a necessidade de regulamentar a concentração de glifosato em alimentos ultraprocessados no futuro.

Abstract Studies have provided evidence that morphological awareness contributes to word reading in opaque languages such as English. However, this relationship is not yet established for Brazilian Portuguese, a much less opaque language. The present study performed a longitudinal investigation of the relationship between morphological awareness and word reading in 162 children from 2nd to 5th grades of elementary school. The children were evaluated in the final quarter of the respective school year (time 1) and 1 year later (time 2). Hierarchical regression analyses controlling for intelligence, phonological awareness, and phonological working memory were conducted. The hierarchical regression analyses were followed up by cross-lagged panel correlations, and both results converged to word reading measured in the 2nd and 3rd grades contributing to morphological awareness 1 year later. In addition, morphological awareness measured in the 4th grade contributed to word reading 1 year later. The greater transparency of Brazilian Portuguese may make morphological awareness less important for word reading in the early grades; however, in older children, morphological awareness is important for word reading performance.

ABSTRACT. In soils with adequate levels of fertility, it is possible to manage phosphate fertilization aiming at the grain production system, instead of each isolated crop. The objective was to evaluate the effects of the management of phosphate fertilization, place and time, on the soil and leaf P content, and crop yield in grain production systems. An experiment was conducted at the farm level in the municipality of Nazareno, state of Minas Gerais, Brazil, for four years on soil with adequate level of fertility. The treatments consisted of the management of phosphate fertilization by broadcast or furrow and dose for each crop or for the production system (spring/summer and autumn/winter crops), being: Control = without phosphate fertilization; Conventional = phosphate fertilization in the furrow in each crop; BTP = phosphate fertilization of the grain production system to broadcast; TPS = phosphate fertilization of the grain production system in the furrow of the spring/summer crop; TPW = phosphate fertilization of the grain production system in the furrow of the autumn/winter crop. Soil P content was evaluated in the third year of implantation, and the crops yield every season crop (maize, soybean, common bean and wheat). The P content in the leaves of maize and soybeans were evaluated in the spring/summer crops. The application of the total dose of phosphate fertilization for both crops in the sowing furrow of the spring/summer or autumn/winter crops (TPS and TPW) promoted operational advantages and increased soil P content in the 0-0.20 m layer, without reducing the yield of the spring/summer crops. Leaf P content was not affected. The Conventional management (phosphate fertilization in the furrow in each crop) provide greater yield of common beans and wheat during autumn/winter crops, which are more subject to water restriction due to less rainfall, even without increasing the soil P content.

ABSTRACT Objective The incidence of thrombotic events and acute kidney injury is high in critically ill patients with COVID-19. We aimed to evaluate and compare the coagulation profiles of patients with COVID-19 developing acute kidney injury versus those who did not, during their intensive care unit stay. Methods Conventional coagulation and platelet function tests, fibrinolysis, endogenous inhibitors of coagulation tests, and rotational thromboelastometry were conducted on days 0, 1, 3, 7, and 14 following intensive care unit admission. Results Out of 30 patients included, 13 (43.4%) met the criteria for acute kidney injury. Comparing both groups, patients with acute kidney injury were older: 73 (60-84) versus 54 (47-64) years, p=0.027, and had a lower baseline glomerular filtration rate: 70 (51-81) versus 93 (83-106) mL/min/1.73m2, p=0.004. On day 1, D-dimer and fibrinogen levels were elevated but similar between groups: 1780 (1319-5517) versus 1794 (726-2324) ng/mL, p=0.145 and 608 (550-700) versus 642 (469-722) g/dL, p=0.95, respectively. Rotational thromboelastometry data were also similar between groups. However, antithrombin activity and protein C levels were lower in patients who developed acute kidney injury: 82 (75-92) versus 98 (90-116), p=0.028 and 70 (52-82) versus 88 (78-101) µ/mL, p=0.038, respectively. Mean protein C levels were lower in the group with acute kidney injury across multiple time points during their stay in the intensive care unit. Conclusion Critically ill patients experiencing acute kidney injury exhibited lower endogenous anticoagulant levels. Further studies are needed to understand the role of natural anticoagulants in the pathophysiology of acute kidney injury within this population.

ABSTRACT Objective: To evaluate the etiology of and factors associated with pulmonary infection in kidney and kidney-pancreas transplant recipients. Methods: This was a single-center case-control study conducted between December of 2017 and March of 2020 at a referral center for kidney transplantation in the city of Belo Horizonte, Brazil. The case:control ratio was 1:1.8. Cases included kidney or kidney-pancreas transplant recipients hospitalized with pulmonary infection. Controls included kidney or kidney-pancreas transplant recipients without pulmonary infection and matched to cases for sex, age group, and donor type (living or deceased). Results: A total of 197 patients were included in the study. Of those, 70 were cases and 127 were controls. The mean age was 55 years (for cases) and 53 years (for controls), with a predominance of males. Corticosteroid use, bronchiectasis, and being overweight were associated with pulmonary infection risk in the multivariate logistic regression model. The most common etiologic agent of infection was cytomegalovirus (in 14.3% of the cases), followed by Mycobacterium tuberculosis (in 10%), Histoplasma capsulatum (in 7.1%), and Pseudomonas aeruginosa (in 7.1%). Conclusions: Corticosteroid use, bronchiectasis, and being overweight appear to be risk factors for pulmonary infection in kidney/kidney-pancreas transplant recipients, endemic mycoses being prevalent in this population. Appropriate planning and follow-up play an important role in identifying kidney and kidney-pancreas transplant recipients at risk of pulmonary infection.

RESUMO Objetivo: Avaliar a etiologia da infecção pulmonar e os fatores a ela associados em pacientes que receberam transplante de rim ou rim-pâncreas. Métodos: Estudo unicêntrico de caso-controle realizado entre dezembro de 2017 e março de 2020 em um centro de referência em transplantes de rim em Belo Horizonte (MG). A proporção caso:controle foi de 1:1,8. Os casos foram pacientes que haviam recebido transplante de rim ou rim-pâncreas e que foram hospitalizados em virtude de infecção pulmonar. Os controles foram pacientes que haviam recebido transplante de rim ou rim-pâncreas e que não apresentaram infecção pulmonar, emparelhados com os casos pelo sexo, faixa etária e tipo de doador (vivo ou falecido). Resultados: Foram incluídos no estudo 197 pacientes. Destes, 70 eram casos e 127 eram controles. A média de idade foi de 55 anos (casos) e 53 anos (controles), com predomínio de pacientes do sexo masculino. O uso de corticosteroides, bronquiectasias e sobrepeso relacionaram-se com risco de infecção pulmonar no modelo de regressão logística multivariada. O agente etiológico de infecção mais comum foi o citomegalovírus (em 14,3% dos casos), seguido de Mycobacterium tuberculosis (em 10%), Histoplasma capsulatum (em 7,1%) e Pseudomonas aeruginosa (em 7,1%). Conclusões: O uso de corticosteroides, bronquiectasias e sobrepeso parecem ser fatores de risco de infecção pulmonar em pacientes que receberam transplante de rim ou rim-pâncreas, e as micoses endêmicas são prevalentes nessa população. O planejamento e acompanhamento adequados desempenham um papel importante na identificação de pacientes transplantados de rim/rim-pâncreas nos quais haja risco de infecção pulmonar.
