Abstract Background and Aims: The donor risk index (DRI) quantifies donor-related characteristics potentially associated with increased risk of early graft failure. We aimed to assess the impact of the DRI, recipient and perioperative factors on post liver transplant (LT) outcomes. Methods: This was a single-center retrospective cohort study including all adult (≥18 years) patients who underwent LT from 01/2019 to 12/2019 at Curry Cabral Hospital, Lisbon, Portugal. Primary endpoint was 1-year graft failure post LT. Associations were studied with logistic regression. Results: A total of 131 cadaveric donor LT procedures were performed in 116 recipients. Recipients’ median (IQR) age was 57 (47-64) years and 101/131 (77.1%) were males. Cirrhosis was the underlying etiology in 95/131 (81.2%) transplants. Based on 8 predefined donors’ characteristics, median (IQR) DRI was 1.96 (1.67-2.16). Following adjustment for MELDNa score pre LT and SOFA score (adjusted odds ratio [aOR], 95% confidence interval [CI] =0.91 [0.56-1.47]) or lactate (aOR [95% CI] = 2.76 [0.71-10.7]) upon intensive care unit (ICU) admission post LT, DRI was not associated with 1-year graft failure. However, higher SOFA score (aOR [95% CI] = 1.20 [1.05-1.37]) or lactate (aOR [95% CI] = 1.27 [1.10-1.46]) upon ICU admission post LT were independently associated with higher odds of 1-year graft failure. Conclusions: In a recent cohort of patients who under-went LT, DRI, despite being high, was not associated with 1-year graft failure, but SOFA score or lactate upon ICU ad-mission post LT were.

Abstract INTRODUCTION: The Portuguese man-of-war (Physalia physalis) is a cosmopolitan species, with a widespread distribution and responsible for a great number of injuries caused by cnidarians worldwide, including Brazil. Geoprocessing technology, however, has never been used to assess the spatial distribution of these animals on beaches. The aim of this study was to carry out a health risk assessment of Portuguese man-of-war (P. physalis) envenomations on the São Marcos and Calhau beaches in São Luís city, in the state of Maranhão, Brazil. METHODS: This is a descriptive and quantitative study concerning primary data on the occurrence of the Portuguese man-of-war (P. physalis) and human envenomations in the studied places, conducted over a two-year period in São Luís, Maranhão, northeastern Brazil. RESULTS: Envenomations mainly occurred on beaches presenting high density of P. physalis during the dry period. Vinegar has been incorporated as a first aid, according to recommendations set by the Brazilian Ministry of Health. CONCLUSIONS: In order to improve prevention and control actions of human envenomation, risk areas for this type of envenomation should be clearly indicated as alert areas. Inclusion of the geographical location of the envenomation in the Notification/Investigation SINAN Form was suggested for allowing the continuity of studies involving this public health issue.

Abstract Background: Maximal oxygen uptake (VO2max) and both first (VT1) and second (VT2) thresholds have been used as reference points for exercise prescription in different populations. Objective: We aimed to test the hypothesis that exercise prescription, based on VTs determined by treadmill cardiopulmonary exercise testing (CPET), is influenced by the rate of increase in treadmill workload. Methods: Nine healthy individuals underwent two CPETs, followed by two sessions of submaximal exercise, both in randomized order. For the “speed” protocol, there was an increment of 0.1 to 0.3 km.h-1 every 15s. The “grade” incremental protocol increased 1% every 30s and 0.1 km.h-1 every 45s. This was followed by submaximal exercise sessions lasting 40min at an intensity corresponding to heart rate (HR) between the VT1 and VT2. Results: The “speed” protocol resulted in higher VT1 (p = 0.01) and VT2 (p = 0.02) when compared to the “grade” incremental protocol, but there was no effect on VO2max. The target HR for the submaximal exercise sessions was higher in the “speed” protocol compared to the “grade” incremental protocol (p < 0.01) and remained stable during the two steady-state exercise sessions. Blood lactate remained stable during the submaximal exercise sessions, with higher values observed during the “speed” protocol than those “grade” incremental protocol (p < 0.01). Conclusions: Compared to a grade-based protocol, a speed-based protocol resulted in higher VT1 and VT2, which significantly affected cardiorespiratory and metabolic responses to prescribed exercise intensity in healthy young adults.

ABSTRACT Pseudomonas aeruginosa is associated with ocular infections such as keratitis. Multipurpose contact lens solutions can be used for washing, disinfection and storage of contact lenses, however, P. aeruginosa biofilm disinfection by this method is unsatisfactory. The present study aimed to investigate the effectiveness of ozonated water in reducing P. aeruginosa colony count. Lenses kept in storage cases were contaminated with P. aeruginosa and disinfected using ozonized water, chlorhexidine, ultrasound and multipurpose solutions. The multipurpose solutions and ultrasound methods reduced colony count from 1.17 to 1.63 log10 CFU/cm2 (92.93% to 97.31%), respectively, of P. aeruginosa biofilm cell viability when compared to the positive control. Both, ozonated water and chlorhexidine showed 7.42 log reduction in the number of viable cells of P. aeruginosa biofilm. As compared to chlorhexidine, ozonized water did not depose any known toxic residues, so that we recommend it as an alternative disinfectant solution for contact lenses storage cases.

ABSTRACT: Bovine respiratory disease (BRD) is considered the major cause of economic losses in dairy and beef cattle production. The study aimed to detect the most important bacteria related to respiratory disease in tracheobronchial fluid samples of healthy and dairy calves with clinical signs of BRD in Brazilian rural settlements. Hundred and forty-one mongrel dairy calves were randomly selected from 42 family farm dairy herds from Brazilian settlements. Physical examination was performed and calves were classified as healthy (n=100) and BRD (n=41). Tracheobronchial fluid samples were collected. Isolation and molecular detection of Mycoplasma dispar, M. bovis and M. mycoides subsp. mycoides SC besides isolation of other aerobic bacteria were performed. Abnormal lung sounds (crackle/snoring/whistle), mucopurulent/purulent nasal discharge, body temperature >39.5°C and respiratory rate >40 breaths/min were higher in BRD calves compared to healthy calves (P<0.05). Bacillus sp., Staphylococcus intermedius and non-fermentative Gram-negative were the most prevalent bacteria isolated. Non-identified species from Enterobacteriaceae family was higher in BRD calves compared to healthy calves (P<0.05). Mollicutes were isolated in 7.4% of samples and only M. dispar was detected. Mollicutes was associated with purulent/mucopurulent nasal discharge (P=0.017). Pantoea agglomerans was associated to tachypnea (P=0.020), and Streptococcus spp. was associated with hyperthermia. Statistical tendencies were observed to M. dispar and tachypnea (P=0.066), and P. agglomerans and tachycardia (P=0.066). The obtained results describe the microorganisms found in tracheobronchial fluid of calves with BRD in some herds of Brazilian family farming and their relation to clinical signs of BRD.

Abstract Background: Previous data has shown that patients in the indeterminate form of Chagas disease may present myocardial fibrosis as shown on through magnetic resonance imaging (MRI). However, there is little information available regarding the degree of severity of myocardial fibrosis in these individuals. This variable has the potential to predict the evolution of Chagas’ disease into its cardiac form. Objectives: To describe the frequency and extent of myocardial fibrosis evaluated using an MRI in patients in the indeterminate form, and to compare it with other forms of the disease. Methods: Patients were admitted one after another. Their clinical history was collected and they were submitted to laboratory exams and an MRI. Results: Sixty-one patients with Chagas’ disease, with an average age of 58 ± 9 years old, 17 patients in the indeterminate form, 16 in the cardiac form without left ventricular (LV) dysfunction and 28 in the cardiac form with LV dysfunction were studied. P <0.05 was considered to be statistically significant. Late enhancement was detected in 37 patients (64%). Myocardial fibrosis was identified in 6 individuals in indeterminate form (41%; 95% CI 23-66) in a proportion similar to that observed in cardiac form without LV dysfunction (44%); p = 1.0. Among the individuals with fibrosis, the total area of the affected myocardium was 4.1% (IIQ: 2.1 - 10.7) in the indeterminate form versus 2.3% (IIQ: 1-5) in the cardiac form without LV (p = 0.18). The left ventricular fraction ejection in subjects in the indeterminate form was similar to that of the individuals in the cardiac form without ventricular dysfunction (p = 0.09). Conclusion: The presence of fibrosis in the indeterminate form of Chagas’ disease has a frequency and extension similar to that of in the cardiac form without dysfunction, suggesting that the former is part of a subclinical disease spectrum, rather than lacking cardiac involvement.

This work describes the tissue culture of Plectranthus ornatus Codd. (Lamiaceae) on Murashige and Skoog (MS) medium with naphthalene acetic acid (NAA) (1.0, 2.0 and 4.0 mg L-1) and 6-benzylaminopurine (BAP) (1.0, 2.0 and 4.0 mg L-1) as growth regulators to obtain the overproduction of rosmarinic acid. The content of rosmarinic acid and its biosynthetic precursors were determined by high performance liquid chromatography (HPLC). The antioxidant activities (AA) of the extracts were also evaluated by 2,2-diphenyl-1-picrylhydrazyl (DPPH) assay. The best growing condition was observed with the addition of 1.0 mg L-1 of both NAA and BAP in the culture medium yielded an increase of 94 times of rosmarinic acid comparing with the wild plant. All extracts diplayed antioxidant activity, as evidenced by the DPPH free radical scavenging assay. However, those with 2.0 mg L-1 NAA and, 1.0 mg L-1 NAA and BAP in culture medium showed the lowest EC50 (26.0 and 29.8 µg mL-1, respectively). At concentration higher than 10 µg mL-1 of rosmarinic acid it was not observed the correlation with AA, suggesting some other anti-oxidant mechanism acting.

Abstract The objective of this study was to evaluate the stability and adaptability, using the method of harmonic means of the relative performance of genetic values and to estimate the genetic components of variance and average via mixed models of 14 genotypes of passion fruit in three environments. Data were obtained in a random block design with three replicates and nine plants per plot. For the hybrids in the final validation phase, the estimates of heritability and genetic gains in the evaluated environments showed good prospects for selection of superior genotypes. There was a pronounced effect of genotype-environment interaction (GxE) for all traits investigated except fruit length, percentage of pulp, soluble solids, titratable acidity and SS/TA ratio. The most stable and adaptable hybrids in the evaluated environments were BRS Gigante Amarelo, HFOP-09, H09-09, GP09-02, GP09-03 and BRS Sol do Cerrado.

Abstract Background: The importance of coronary anatomy in predicting cardiovascular events is well known. The use of traditional anatomical scores in routine angiography, however, has not been incorporated to clinical practice. SYNTAX score (SXscore) is a scoring system that estimates the anatomical extent of coronary artery disease (CAD). Its ability to predict outcomes based on a baseline diagnostic angiography has not been tested to date. Objective: To evaluate the performance of the SXscore in predicting major adverse cardiac events (MACE) in patients referred for diagnostic angiography. Methods: Prospective cohort of 895 patients with suspected CAD referred for elective diagnostic coronary angiography from 2008 to 2011, at a university-affiliated hospital in Brazil. They had their SXscores calculated and were stratified in three categories: no significant CAD (n = 495), SXscoreLOW-INTERMEDIATE: < 23 (n = 346), and SXscoreHIGH: ≥ 23 (n = 54). Primary outcome was a composite of cardiac death, myocardial infarction, and late revascularization. Secondary endpoints were the components of MACE and death from any cause. Results: On average, patients were followed up for 1.8 ± 1.4 years. The primary outcome occurred in 2.2%, 15.3%, and 20.4% in groups with no significant CAD, SXscoreLOW-INTERMEDIATE, and SXscoreHIGH, respectively (p < 0.001). All-cause death was significantly higher in the SXscoreHIGH compared with the 'no significant CAD' group, 16.7% and 3.8% (p < 0.001), respectively. After adjustment for confounding factors, all outcomes remained associated with the SXscore. Conclusions: SXscore independently predicts MACE in patients submitted to diagnostic coronary angiography. Its routine use in this setting could identify patients with worse prognosis.

Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

The coastal plain of the state of Rio Grande do Sul, in southern Brazil, includes four barrier-lagoon depositional systems formed by successive Quaternary sea-level highstands that were correlated to marine isotope stages (MIS) 11, 9, 5 and 1, despite the scarcity of absolute ages. This study describes a sea-level highstand older than MIS 5, based on the stratigraphy, ages and fossils of the shallow marine facies found in coastal barrier (Barrier II). This facies outcrops along the banks of Chuí Creek, it is composed of fine, well-sorted quartz sand and contains ichnofossils Ophiomorpha nodosa and Rosseliasp., and molluscan shells. The sedimentary record indicates coastal aggradation followed by sea-level fall and progradation of the coastline. Thermoluminescence (TL) and electron spin resonance (ESR) ages from sediments and fossil shells point to an age of ∼220 ka for the end of this marine transgression, thus correlating it to MIS 7 (substage 7e). Altimetric data point to a maximum amplitude of about 10 meters above present-day mean sea-level, but tectonic processes may be involved. Paleoceanographic conditions at the time of the highstand and correlations with other deposits in the Brazilian coasts are also discussed.

It is well known that pH is an important parameter for controlling the eucalyptus pulp bleaching when using the final chlorine dioxide stage, since it affects the effectiveness of the process. Recommendations found in the literature for operating are in the 3.5 to 4.0 range. However, in this paper it was shown that final chlorine dioxide has better performance, with significant brightness gain while also preserving pulp quality, when it is operated at near neutral pH. This result can be explained by the generation of sodium bicarbonate in situ upon adding carbon dioxide at this stage.

In drip irrigation with emitters positioned subsurfacely there is the potential risk of soil particles being suctioned through the water outlet port, due to the occurrence of vacuum which may result in clogging of drippers. The objective of this study was to evaluate the effect of vacuum on the performance of different models of subsurface positioned drippers under two conditions of soil texture. The treatments consisted of applying suction levels of -10, -15, -20 and -40 kPa in sandy and clayey soils in 23 models of used drippers with more than 1 year of operation. The performance of the subsurface drip emitters was influenced by soil texture. Overall, in 80% of drippers , -40 kPa vacuum caused the obstruction of emitter and reduced flow in both soils. The largest reduction of uniformity of water application was observed in model Irrimon Irridrip Plus 2.5 L h-1 in sandy soil and application of 40 kPa suction.

BACKGROUND:Interferential electrical stimulation (IES), which may be linked to greater penetration of deep tissue, may restore blood flow by sympathetic nervous modulation; however, studies have found no association between the frequency and duration of the application and blood flow. We hypothesized that 30 min of IES applied to the ganglion stellate region might improve blood flow redistribution.OBJECTIVES: The purpose of this study was to determine the effect of IES on metaboreflex activation in healthy individuals.METHOD: Interferential electrical stimulation or a placebo stimulus (same protocol without electrical output) was applied to the stellate ganglion region in eleven healthy subjects (age 25±1.3 years) prior to exercise. Mean blood pressure (MBP), heart rate (HR), calf blood flow (CBF) and calf vascular resistance (CVR) were measured throughout exercise protocols (submaximal static handgrip exercise) and with recovery periods with or without postexercise circulatory occlusion (PECO+ and PECO -, respectively). Muscle metaboreflex control of calf vascular resistance was estimated by subtracting the area under the curve when circulation was occluded from the area under the curve from the AUC without circulatory occlusion.RESULTS:At peak exercise, increases in mean blood pressure were attenuated by IES (p<0.05), and the effect persisted under both the PECO+ and PECO- treatments. IES promoted higher CBF and lower CVR during exercise and recovery. Likewise, IES induced a reduction in the estimated muscle metaboreflex control (placebo, 21±5 units vs.IES, 6±3, p<0.01).CONCLUSION: Acute application of IES prior to exercise attenuates the increase in blood pressure and vasoconstriction during exercise and metaboreflex activation in healthy subjects.

Obstructive sleep apnea (OSA), a risk factor for coronary artery disease, remains under diagnosed. We investigated if OSA identified by the Berlin Questionnaire (BQ) is associated with the risk of coronary artery disease. Cases were patients referred for elective coronariography. The cases were classified with significant coronary lesions (stenosis > 50% in an epicardial coronary) or without significant coronary lesions. Controls were selected from a population-based sample. Positive BQ results were identified in 135 (41.2%) of 328 cases, in contrast with 151 (34.4%) of 439 control subjects (p = 0.03). In a multinomial logistic analysis, the risk for OSA identified by the BQ was independently associated with coronary artery disease in cases with lesions of at least 50% (OR = 1.53; 95%CI: 1.02-2.30; p = 0.04). The risk from OSA identified by the BQ was higher in younger subjects (40-59 years) (OR = 1.76; 95%CI: 1.05-2.97; p = 0.03) and in women (OR = 3.56; 95%CI: 1.64-7.72; p = 0.001). In conclusion, OSA identified by the BQ greatly increases the risks of coronary artery disease in patients having significant coronary artery lesions indicated by anangiogram, particularly in younger individuals and in women.
