Se desconoce la prevalencia de hipertensión arterial (HTA) en múltiples regiones de Venezuela. El objetivo fue determinar la prevalencia de HTA en cinco poblaciones de tres regiones. Durante 2006 a 2010, 1392 sujetos de 20 o más años de edad fueron seleccionados mediante un muestreo aleatorio estratificado polietápico, de todas las casas en 5 municipios de: Estado Lara (región Oeste), Estado Mérida (región Andina) y Distrito Capital (región Capital). Se obtuvieron medidas antropométricas, presión arterial y análisis bioquímico. La media de edad fue 45,2 ± 0,4 años, y 68% fueron mujeres. La prevalencia de HTA fue 31,3% (IC 95%: 28,9 - 33,8), mayor en hombres que en mujeres (38,1% [33,5 - 42,8] vs 28,2% [25,4 - 31,2], respectivamente; p <0,001). Los sujetos hipertensos mostraron mayor edad, índice de masa corporal (IMC), glucemia, colesterol total, c-LDL y triglicéridos; y valores más bajos de c-HDL, que los sujetos pre-hipertensos y normotensos (p< 0,05). En las mujeres, la prevalencia de HTA aumentó linealmente en cada década de la vida y en cada categoría de IMC; en los hombres, se incrementó hasta la quinta década, siendo similar en sujetos con sobrepeso y obesidad. El riesgo de HTA se incrementó con la edad, presencia de sobrepeso/obesidad, diabetes, e historia familiar de HTA. Sólo 17,7% de los sujetos hipertensos se encontraban tratados y controlados. En conclusión, un tercio de los sujetos evaluados presentó HTA y alrededor de un quinto está bajo control. En Venezuela, la HTA es un grave problema de salud pública exacerbado por la edad y el sobrepeso.

The prevalence of hypertension in multiple regions of Venezuela is unknown. The objective of this study was to estimate the prevalence of hypertension in five populations from three regions. During 2006 to 2010, 1392 subjects aged 20 or older were selected by multistage stratified random sampling from all households in five municipalities from: Lara State (Western region), Merida State (Andean region), and Capital District (Capital region). Anthropometric measurements, blood pressure, and biochemical analysis were obtained from each participant. Mean age was 45.2 ± 0.4 years and 68% were females. The overall prevalence of hypertension was 31.3% (CI 95% 28.9 - 33.8), it was higher in men than women (38.1% [33.5 - 42.8] vs. 28.2% [25.4 - 31.2], respectively; p <0.001). The hypertensive participants were older, with higher body mass index (BMI), glucose, total cholesterol, LDL-c, and triglycerides; and lower values of HDL-c, than pre-hypertensive and normotensive participants (p < 0.05). In women, hypertension prevalence increased linearly for every decade of life and by category of BMI; whereas in men it increased until the fifth decade of life, and was similar in patients with overweight and obesity. The risk of hypertension increased with age, the presence of obesity, diabetes, overweight and family history of hypertension. Only 17.7% of the hypertensive subjects were both treated and controlled. In conclusion, about one third of the subjects evaluated had hypertension and about one fifth of them had their hypertension under control. In Venezuela, hypertension is a serious public health problem exacerbated by age and overweight.

Introducción: La Organización Mundial de la Salud (OMS) indica que en 2015, la cobertura vacunal debería alcanzar al menos el 90% a nivel nacional y al menos el 80% en cada distrito o unidad administrativa equivalente. En el Programa Ampliado de Inmunización (PAI) el monitoreo y la evaluación sirve para verificar periódicamente si las coberturas vacúnales se están desarrollando de acuerdo a lo planificado. El objetivo principal del siguiente estudio es evaluar las metas de cobertura vacunal del programa ampliado de inmunizaciones (PAI) administrado en el Estado Táchira y observar si estas se corresponden con los estándares establecidos por la OMS. Métodos: Estudio epidemiológico, descriptivo, observacional, transversal y retrospectivo donde se obtuvieron las coberturas correspondientes al período 2007 al 2014, de BCG, vacunas combinada de difteria-toxoide-pertusas de células completas- Haemophilus influenzae tipo B-hepatitis B (pentavalente), polio oral, antirotavirus y vacuna combinada de sarampión -rubéola-parotiditis (trivalente viral o SRP). Se analizaron las coberturas por inmunógenos y por distritos sanitario. Resultados: El año con menor cobertura global fue el 2009 con 69,38% y el de con mayor cobertura fue el año 2012 con 86,46%. El inmunógeno con mayor cobertura fue la BCG con 86,5%; y el de menor cobertura la antirotavirus con un 68,2%. Ningún inmunógeno alcanzó una cobertura mayor al 90% entre los diferentes distritos sanitarios. Discusión: Las metas de cobertura establecidas de acuerdo a los estándares de la OMS no se logran cumplir ni a nivel global ni en cada uno de los distritos que componen el sistema regional de salud del estado Táchira.

Introduction: The World Health Organization (who) indicates that in 2015, immunization coverage should reach at least 90 per cent and at least 80 per cent in every district or equivalent administrative unit. In the expanded programme on immunization the monitoring and evaluation is used to periodically check if coverage immunize them are developing according to plan. The main objective of the following study is to evaluate the goals of vaccination coverage of the expanded programme of immunization administered in Tachira State and see if these correspond to the standards set by the who. Methods: Epidemiological, descriptive, observational, cross-sectional and retrospective study where obtained coverage for the period 2007 to 2014, BCG, vaccines combined difteria-toxoide - pertussis cells complete-Haemophilus influenzae type B-hepatitis B (pentavalent), oral polio, anti-rotavirus and measles-rubeola-parotiditis combination vaccine (viral trivalent or SRP). Hedges were analyzed by immunogens and sanitary districts. Results: Was 2009 the year with lower global coverage with 69.38% and of more coverage later in 2012 with 86,46% . The vaccine with the largest coverage was the BCG with 86.5; and the less coverage the anti-rotavirus with a 68.2%. No vaccine reached one higher coverage to 90% between the different health districts. Discussion: The goals of coverage set according to who standards fail to meet or at the global level or in each of the districts that make up the regional health system of Táchira State.

Objetivo: el objeto del estudio es analizar los indicadores hospitalarios del hospital de niños JM de los ríos (HJMR), año 2012, las muertes hospitalarias de 2011 y las causas de hospitalización en el servicio de urgencias del hospital, año 2011. Métodos: estudio descriptivo de corte trasversal de las estadísticas de salud del HJMR, 2011 y 2012. Fuente de información: registros e indicadores del servicio de estadística e historias Médicas. Análisis estadístico descriptivo, cálculo de tasas, medidas de resumen, distribuciones de frecuencia y tabulaciones. Resultados: Para el año 2012: capacidad 203 camas con un porcentaje de ocupación de 67% siendo mayor en Medicina crítica (Mc, incluye urgencias y terapia Intensiva) hasta un 89% y el de cirugía (c) con 58%. los intervalos de sustitución y los índices de rendimiento son 3,8 días y 31,4 pacientes por cama/año. se reportan traslados interdepartamentales durante la hospitalización. la mortalidad hospitalaria (2011) es 3,8%, de las cuales 2/3 fueron en menores de 5 años; la mayoría de las muertes (70%) ocurren en Mc. causas de mortalidad: sepsis, infección respiratoria y diarrea, seguidas de neoplasias y malformaciones congénitas. la morbilidad en el servicio de urgencias (2011) incluye predominantemente enfermedades infecciosas y respiratorias, abarcando más del 50%, seguidas de traumatismos y enfermedades hemáticas y neoplasias. Conclusión: el HJMR es un hospital de referencia que recibe pacientes con patologías que requieren tratamientos a largo plazo, cirugía compleja y medicina crítica.

Objective: the aim of the study is to analyze hospital indicators and causes of mortality and hospitalization of the emergency department at the children’s hospital JM de los ríos (HJMR) in caracas, Venezuela, during years 2011 and 2012. Methods: A descriptive cross-sectional study of health statistics obtained from the department of medical records. descriptive statistical analysis, fee calculation, summary measures, frequency distributions and cross-tabulations. Results: current capacity 203 beds with an occupancy rate of 67%, which is higher in critical care Medicine with 89% and in the surgery department with 58%. replacement intervals and rates of capacity are 3.8 days and 31.4 patients per bed/year. Interdepartmental transfers are reported during hospitalization. hospital mortality was 3.8% and 2/3 occurs in children under 5 years; most deaths occur in critical Medicine (70%). the causes of death are sepsis, respiratory infections and diarrhea, followed by neoplasms and congenital malformations. Morbidity in the emergency department is predominantly secondary to infectious and respiratory diseases which account for over 50%, followed by trauma, hematological diseases and malignancies. Conclusion: hJMr is a referral hospital that receives patients with conditions that require long-term treatment, complex surgery and critical care medicine.
