RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Anti-Ehrlichia properties of the dichloromethane fraction of Ageratum conyzoides associated with doxycycline: In vitro study

RESUMO: O crescente número de casos de erliquiose canina por Ehrlichia canis em hospitais e clínicas veterinárias tem demonstrado a necessidade de um novo protocolo de medicamentos para essa doença. A doxiciclina é usada para tratar a erliquiose, mas a resistência do microrganismo a esse protocolo de tratamento, bem como os diversos efeitos colaterais para os animais, tornou-se uma preocupação. Vários estudos têm demonstrado interação positiva com extratos de plantas e fármacos, que permitem a redução da concentração necessária para produzir o efeito desejado, minimizando os efeitos adversos. Este estudo determinou a eficiência da combinação da fração diclorometânica (DCM) de Ageratum conyzoides L. com atividade anti-Ehrlichia canis associada com doxiciclina por meio do ensaio de Checkerboard. O material vegetal foi coletado em São Luís, Maranhão, nordeste do Brasil, seguido pela extração em MeOH:H2O (8:2) e partição da fração diclorometânica. Após a determinação da Concentração Inibitória Mínima (CIM) da fração em estudo frente às células DH82 infectadas com Ehrlichia canis, a mesma foi combinada com a doxiciclina para derivação do Índice de Concentração Inibitória da Fração (Índice CIF). Observou-se uma redução de 5,83 vezes a concentração inibitória mínima da doxiciclina mostrando que esta fração de A. conyzoides, composta predominantemente por lignanas identificadas por espectrometria de massas, notavelmente intensificou a atividade desse fármaco contra E. canis, resultando em um efeito sinérgico.
ABSTRACT: The increasing number of cases of canine ehrlichiosis caused by Ehrlichia canis in hospitals and veterinary clinics has demonstrated the need for a new drug protocol for this disease. Doxycycline is used to treat ehrlichiosis, but the resistance of the microorganism to this treatment protocol, as well as the various side effects to the animals, has become a concern. Several studies have shown a positive interaction with extracts of plants and drugs, which allow for the reduction of the concentration necessary to produce the desired effect, minimizing adverse effects. This study determined the efficiency of the combination of the dichloromethane (DCM) fraction of Ageratum conyzoides L. with anti-Ehrlichia activity and doxycycline by using the checkerboard assay. Plant material was collected in São Luís, northeastern Brazil, followed by extraction in MeOH: H2O (8:2) and partitioning of the DCM fraction. After determining the Minimum Inhibitory Concentration (MIC) of the fraction under study against DH82 cells infected with Ehrlichia canis, it was combined with doxycycline to derive the Fractional Inhibitory Concentration Index (CIF Index). A reduction of 5.83 times the doxycycline minimum inhibitory concentration was observed, showing that this fraction of A. conyzoides composed predominantly by the class of lignans, identified by mass spectrometry notably intensified the activity of doxycycline against E. canis, resulting in a synergistic effect.

RESUMO Enxaqueca piora o sofrimento do paciente que tem esclerose múltipla (EM). ID-migraine é uma ferramenta útil para seleção de pacientes com enxaqueca e Migraine Disability Assessment (MIDAS) é um questionário que avalia o impacto da doença. O objetivo do presente estudo foi avaliar a presença e impacto de enxaqueca em pacientes com EM. Métodos: Pacientes diagnosticados com EM e tratados em clínicas especializadas foram convidados a responder um questionário online se também apresentassem cefaleia. Resultados: O estudo incluiu 746 participantes com cefaleia e EM que preencheram completamente as respostas. Foram 625 mulheres e 121 homens, sendo 69% dos pacientes com idade entre 20 e 40 anos. Enxaqueca foi identificada em 404 pacientes (54,1%) e moderado a grave impacto da doença foi observado em 68,3% dos casos. Conclusão: Enxaqueca é uma cefaleia primária frequente e incapacitante relatada por pacientes com EM.
ABSTRACT Migraine adds to the burden of patients suffering from multiple sclerosis (MS). The ID-migraine is a useful tool for screening migraine, and the Migraine Disability Assessment questionnaire can evaluate disease burden. The aim of the present study was to assess the presence and burden of migraine in patients with MS. Methods: Patients diagnosed with MS attending specialized MS units were invited to answer an online survey if they also experienced headache. Results: The study included 746 complete responses from patients with MS and headache. There were 625 women and 121 men, and 69% of all the patients were aged between 20 and 40 years. Migraine was identified in 404 patients (54.1%) and a moderate-to-high burden of disease was observed in 68.3% of the patients. Conclusion: Migraine is a frequent and disabling type of primary headache reported by patients with MS.

Self-nanoemulsion drug delivery systems (SNEDDS) represent a potential carrier to improve solubility and bioavailability of health care products. In this sense, copaiba oil (CO) loaded on cosurfactant-free SNEDDS formulations (CO-SNEDDS) were obtained by using phase diagram performed on different concentrations of constituents (oil, surfactant and water). The CO-SNEDDS are composed of 1% (m/m) of oil phase, 20% (m/m) of Tween 80, and 79% (m/m) of double-distilled water. Specifically, varying the oil phase to 1% of CO affords the so-called SNECO system, and 1% of CO blended on a 1:1 ratio with coconut oil or sunflower oil affords, respectively, the SNECO-COO and SNECO-SO derivative systems. CO sample was authenticated by gas chromatography equipped with a flame ionization detector and the CO-SNEDDS samples were characterized by droplet particle size, zeta potential, polarized light microscopy, pH, conductivity, refractive index, rheological and surface tension analyses. According to the antioxidant tests reducing power and hydroxyl inhibiting formation, the findings displayed 47.89 and 72.92% for SNECO, 46.48 and 84.11% for SNECO-COO, and 52.46 and 74.48% for SNECO-SO, respectively. From now on CO-SNEDDS based systems become available as promising targets for novel medicinal investigations and also could be undertaken for the oral administration of poorly water soluble drugs.

Lychee (Litchi chinensis Sonn.) peel flour: effects on hepatoprotection and dyslipidemia induced by a hypercholesterolemic diet

ABSTRACT Dyslipidemias are associated with the incidence of cardiovascular diseases, obesity, diabetes, hypertension and hepatic steatosis, being the cause of morbidity and mortality. This study investigated the effects of lychee peel flour (PF) on serum levels of total cholesterol (TC), low-density lipoprotein (LDL-c), triacylglycerols (TAG) and various parameters related to obesity, in rats fed a hypercholesterolemic diet. Therefore, 20 male rats were used. In the first 21 days, the animals were fed a hypercholesterolemic diet, except for control group. In the following 21 days, their diets were modified, and they received a standard diet (Control); hypercholesterolemic (Hyper); hypercholesterolemic + 5% PF (PF5) and hypercholesterolemic + 10% PF (PF10). The results revealed that PF intake attenuated weight gain, reduced body mass index, glucose and the levels of TAG, TC, LDL-c, hepatic enzymes and leptin, besides the percentage of hepatic lipids, liver lipid peroxidation and frequency of severe steatosis. Histological studies of the aorta did not show the formation of the atheromatous plaque. These results reinforce its potential to reduce the risk of diseases associated with obesity.

ABSTRACT Celtis iguanaea (Jacq.) Sarg., Cannabaceae, is popularly used in the treatment of diabetes mellitus. However, chemical and pharmacological investigations are lacking. In this study, we investigated the effects of the hydroalcoholic extract from C. iguanaea on markers of cardiovascular diseases and the glucose metabolism in cholesterol-fed rats. Therefore, hypercholesterolemic rats (1% cholesterol) were orally treated with C. iguanaea extract (C-150, CI-300, or CI-600 mg/kg) or simvastatin (4 mg/kg) (n = 6) once a day (30 days) with a hypercholesterolemic diet. A control group (C) was given saline. C. iguanaea extract showed significant decreases in serum levels of total cholesterol, LDL-cholesterol, HMG-CoA-reductase, interleukin-1 and 6, TNF-α and IFN-γ when compared to group C (p < 0.001). Hypoglycemic effects were observed along with a decrease of the activity of sucrase (CI-600), maltase (CI-150, CI-300), and an increase in muscle glycogen levels (CI-300). Antioxidant effects were observed in plasma by the decrease of TBARS and increase of nonprotein thiols levels (CI-600). The histopathological analysis showed a significant decrease in the liver fat area for C. iguanaea extract compared to group C (p < 0.001). Our results suggest that the biological effects of C. iguanaea extract could be related to the flavonoids that possibly exert antioxidant, enzymatic inhibitory, and insulin-mimetic effects.

ABSTRACT The aim of this research was to monitor the presence of females of Aedes aegypti (Linnaeus, 1762) on the Seropédica municipality, Rio de Janeiro State, from 2010 to 2013. For this purpose, the Intelligent Dengue Monitoring (IM-Dengue) and Intelligent Virus Monitoring (IM-Virus) developed by Universidade Federal de Minas Gerais (Ecovec – Minas Gerais, Brazil), were used. IM-Dengue is a tool that allows achieving a weekly overview of A. aegypti infestation, while IM-Virus is another tool that allows detecting dengue virus directly from the mosquito, by Real Time-PCR. Both tools were developed for diagnosis in a prepathogenesis period of the disease, before infection occurrence. Traps were distributed in 19 locations inside the municipality and the bugs were collected weekly during the years of the research. As a result, the presence of 163 females of A. aegypti was recorded over the period; there was no circulation of the virus in the municipality. In one of the 19 study sites, a high degree of disease transmission risk was verified. The study concluded that the municipality, as a whole, showed no risk of disease transmission throughout the field research period.

The obtention of silica and cyclodextrin hybrid materials was accomplished by refluxing them in xylol using citric acid as a binding agent. The materials were characterized by infrared spectroscopy, X-ray diffraction, thermogravimetric analysis, scanning electron microscopy, and elemental analysis. Evidence for the docking of cyclodextrins α and γ was substantiated based on the variation in band intensity for groups such as ≡Si-OH. Additional docking evidence includes the displacement of some of the bands that are related to cyclodextrin such as the deformation of the C-H axial bond. The α and γ-CDSi materials were characterized as amorphous compounds. The products obtained in the synthesis showed changes in the decomposition temperatures of their isolated constituents, in which the mass of α and γ-CD docked to the silica surface gave the estimated values of 41% and 47%, respectively. The elemental constituents were shown to be consistent and close to their relative theoretical values. Thermogravimetric analysis showed that a reduction in the percentage of the hybrids was proportional to the amount of lost mass. This new material is an improvement over synthesized organosilane materials because the operator and the environment benefit from a less toxic methodology. In addition, the material has several potential applications in complexation systems with cyclodextrin.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Identificação de espécies de Colletotrichum associados à antracnose em plantas de atemóia e colonização do fungo nos frutos

A atemóia é um híbrido Annona cherimola com A. squamosa. A antracnose, causada por Colletotrichum sp., é uma importante doença da atemóia, causando danos em diferentes órgãos da planta, destacando àqueles causados nos frutos, tanto na pré como na pós-colheita. Diante deste problema, o presente trabalho teve como objetivo realizar a identificação de espécies de Colletotrichum associados à antracnose em plantas de atemóia através do seqüenciamento de diferentes regiões do DNA deste fungo e acompanhar as etapas de colonização de frutos de atemóia por este fungo através de microscopia eletrônica de varredura. Após extração de DNA, foi realizado o seqüenciamento dos genes da β-tubulina e α-elongase e da região do ITS-5.8S rDNA do DNA dos fungos. Das 15 amostras sequenciadas seis foram identificadas como Colletotrichum acutatum e as outras foram identificadas como C. boninense. A espécie C. acutatum foi encontrada somente em amostras obtidas de folhas de atemóia, enquanto que a espécies C. boninense foi identificada de amostras obtidas de frutos, ramos e folhas doentes. Todas as etapas da doença ocorreram nas 48 horas, sendo que foi observada a germinação dos esporos entre duas e quatro horas após a inoculação
Atemoya is a hybrid fruit of Annona cherimola and A. squamosa. Anthracnose, caused by Colletotrichum sp., is an important disease affecting atemoya, causing damage to different organs, especially fruits, both in the pre and in the post harvest. In view of this problem, the present paper aims to identify Colletotrichum species associated with anthracnose in atemoya plants by sequencing DNA from different regions of this fungus and monitoring the steps of colonization by this fungus in atemoya fruits through Scanning Electron Microscopy. After DNA extraction, the genes from the fungus DNA β-tubulina and α-elongase and the regions ITS-5.8S rDNA were sequenced. Of the 15 sequenced samples, six were identified as Colletotrichum acutatum and others were identified as C. boninense. The species C. acutatum was found only in samples obtained from leaves of atemoya. The species C. boninense had widespread occurrence and was identified in samples obtained from diseased fruits, branches and leaves. All stages of the disease occurred within 48 hours, and spore germination was observed between two and four hours after inoculation.

Anaplasma marginale infection in cattle from south-western Amazonia

O presente estudo fornece os primeiros dados epidemiológicos relativos a infecção por Anaplasma marginale em bovinos criados na Amazônia Sul Ocidental brasileira. Foi adaptado um procedimento simples para a extração de DNA a partir de coágulos sanguíneos coletados em sete microrregiões do estado de Rondônia e duas mesoregiões do estado do Acre. A técnica da reação em cadeia da polimerase (PCR) foi aplicada para avaliar a freqüência da infecção por A. marginale em bovinos com idade entre 4 e 12 meses. Após a extração do DNA de cada amostra, a infecção nos bovinos foi investigada pela amplificação do gene "msp5" de A. marginale. As técnicas de amplificação do DNA revelaram que a freqüência de infecção por A. marginale foi de 98,6% (1.627/1.650) nas amostras provenientes de Rondônia e de 92,87% (208/225) nas amostras do Acre. A alta freqüência da infecção por A. marginale nos animais com idade entre 4 e 12 meses indica uma situação de estabilidade enzoótica nas regiões estudadas, as quais são comparáveis às detectadas por técnicas de imunodiagnóstico em outras regiões endêmicas no Brasil. A extração do DNA através do método aqui descrito pode ser utilizado em estudos epidemiológicos sobre a anaplasmose bovina e outros hemoparasitas.
The present study provides the first epidemiological data regarding infection by Anaplasma marginale in cattle reared in south-western Brazilian Amazonia. One simple procedure was adapted for the extraction of DNA from blood clots collected in seven microregions of Rondônia State and two mesoregions of Acre State. PCR method was used to asses the frequency of A. marginale infections in 4 to12-month-old cattle. The cattle infection was investigated by polymerase chain reaction (PCR) using the specific primer "msp5" for A. marginale. The DNA amplifications revealed that the mean frequency of A. marginale infection was 98.6% (1,627/1,650) in samples from Rondonia, and 92.87% (208/225) in samples from Acre. The high frequency of A. marginale infections in 4 to 12-month-old cattle indicate a situation of enzootic stability in the studied areas and are comparable to those detected by immunodiagnosis in different endemic regions in Brazil. The DNA extraction of clotted blood method described here can be used for epidemiological studies on anaplasmosis and other bovine hemoparasites.

O diagnóstico diferencial do parkinsonismo baseado em parâmetros clínicos pode ser difícil. Alguns exames complementares podem ser úteis, especialmente a ressonância magnética, um método não invasivo, de menor custo quando comparado a tomografia por emissão de pósitrons, proporcionando uma análise anatômica satisfatória. A ressonância por espectroscopia analisa o metabolismo cerebral, com resultados variáveis na literatura no estudo das síndromes parkinsonianas. Selecionamos 40 indivíduos para realização de ressonância magnética e espectroscopia, sendo 12 com doença de Parkinson, 11 com paralisia supranuclear progressiva, 7 com atrofia de múltiplos sistemas tipo parkinsoniana e 10 indivíduos sem manifestações neurológicas ou psiquiátricas (grupo controle). As escalas clínicas analisadas foram a de Hoenh e Yahr, unified Parkinson's disease rating scale e o mini-exame do estado mental. Os resultados encontrados revelaram que pacientes com doença de Parkinson e controle apresentavam em geral o mesmo aspecto por imagem enquanto os grupos paralisia supranuclear progressiva e atrofia de múltiplos sistemas com anormalidades, havendo significância estatística em algumas variáveis. A ressonância magnética e a espectroscopia podem ser úteis no diagnóstico diferencial do parkinsonismo.
The differential diagnosis of Parkinsonism based on clinical features, sometimes may be difficult. Diagnostic tests in these cases might be useful, especially magnetic resonance imaging, a noninvasive exam, not as expensive as positron emission tomography, and provides a good basis for anatomical analysis. The magnetic resonance spectroscopy analyzes cerebral metabolism, yielding inconsistent results in parkinsonian disorders. We selected 40 individuals for magnetic resonance imaging and spectroscopy analysis, 12 with Parkinson's disease, 11 with progressive supranuclear palsy, 7 with multiple system atrophy (parkinsonian type), and 10 individuals without any psychiatric or neurological disorders (controls). Clinical scales included Hoenh and Yahr, unified Parkinson's disease rating scale and mini mental status examination. The results showed that patients with Parkinson's disease and controls presented the same aspects on neuroimaging, with few or absence of abnormalities, and supranuclear progressive palsy and multiple system atrophy showed abnormalities, some of which statistically significant. Thus, magnetic resonance imaging and spectroscopy could be useful as a tool in differential diagnosis of Parkinsonism.

Annona glabra Linneau, Annonaceae, é uma árvore de pequeno porte encontrada em todo território brasileiro, principalmente nas áreas costeiras e conhecida popularmente como araticum-do-brejo e araticum-bravo. Este trabalho teve como objetivos investigar os efeitos do extrato de A. glabra e do ácido caurenóico dele purificado sobre a migração de granulócitos humanos e seu potencial imunomodulatório. Os resultados demonstraram que o extrato de A. glabra inibe a migração natural de granulócitos, de acordo com a dose, sugerindo potencial antiinflamatório, enquanto o ácido caurenóico demonstrou estimulá-la de forma significativa. Em contraste, nenhum efeito foi observado com relação a imunomodulação. Os efeitos apresentados ainda não foram descritos e, dessa forma, contribuem para ampliar a lista de atividades biológicas descritas não só do extrato de A. glabra, como também para o ácido caurenóico.
Annona glabra Linneau, Annonaceae, is a small tree that grows over the Brazilian territory particularly in its coast, and is known as "araticum-do-brejo" and "araticum-bravo". The aim of this study was to evaluate the effects of the extract of A. glabra and its purified kaurenoic acid on the locomotion of human granulocytes and their immunomodulatory potential. The results herein presented showed a dose-dependent inhibition of the granulocyte migration for the extract, suggesting an anti-inflammatory activity, in contrast with a striking stimulation observed for the kaurenoic acid. When focusing immunomodulation properties, no activity could be drawn. The effects presented in this work are reported for the first time and extend the list of biological activities already described for the A. glabra extract as well as for the kaurenoic acid.
