This guideline aimed to update the concepts and formulate the standards of conduct and scientific evidence that support them, regarding the diagnosis and treatment of the Cardiomyopathy of Chagas disease, with special emphasis on the rationality base that supported it.nbsp;
Chagas disease in the 21st century maintains an epidemiological pattern of endemicity in 21 Latin American countries. Researchers and managers from endemic and non-endemic countries point to the need to adopt comprehensive public health policies to effectively control the interhuman transmission of T. cruzi infection, and to obtain an optimized level of care for already infected individuals, focusing on diagnostic and therapeutic opportunistic opportunities.

Pathogenic and pathophysiological mechanisms of the Cardiomyopathy of Chagas disease were revisited after in-depth updating and the notion that necrosis and fibrosis are stimulated by tissue parasitic persistence and adverse immune reaction, as fundamental mechanisms, assisted by autonomic and microvascular disorders, was well established. Some of them have recently formed potential targets of therapies.nbsp;
The natural history of the acute and chronic phases was reviewed, with enhancement for oral transmission, indeterminate form and chronic syndromes. Recent meta-analyses of observational studies have estimated the risk of evolution from acute and indeterminate forms and mortality after chronic cardiomyopathy. Therapeutic approaches applicable to individuals with Indeterminate form of Chagas disease were specifically addressed. All methods to detect structural and/or functional alterations with various cardiac imaging techniques were also reviewed, with recommendations for use in various clinical scenarios. Mortality risk stratification based on the Rassi score, with recent studies of its application, was complemented by methods that detect myocardial fibrosis.nbsp;
The current methodology for etiological diagnosis and the consequent implications of trypanonomic treatment deserved a comprehensive and in-depth approach. Also the treatment of patients at risk or with heart failure, arrhythmias and thromboembolic events, based on pharmacological and complementary resources, received special attention. Additional chapters supported the conducts applicable to several special contexts, including t. cruzi/HIV co-infection, risk during surgeries, in pregnant women, in the reactivation of infection after heart transplantation, and others.nbsp; nbsp;nbsp;
Finally, two chapters of great social significance, addressing the structuring of specialized services to care for individuals with the Cardiomyopathy of Chagas disease, and reviewing the concepts of severe heart disease and its medical-labor implications completed this guideline.
Esta diretriz teve como objetivo principal atualizar os conceitos e formular as normas de conduta e evidências científicas que as suportam, quanto ao diagnóstico e tratamento da CDC, com especial ênfase na base de racionalidade que a embasou.
A DC no século XXI mantém padrão epidemiológico de endemicidade em 21 países da América Latina. Investigadores e gestores de países endêmicos e não endêmicos indigitam a necessidade de se adotarem políticas abrangentes, de saúde pública, para controle eficaz da transmissão inter-humanos da infecção pelo T. cruzi, e obter-se nível otimizado de atendimento aos indivíduos já infectados, com foco em oportunização diagnóstica e terapêutica.
Mecanismos patogênicos e fisiopatológicos da CDC foram revisitados após atualização aprofundada e ficou bem consolidada a noção de que necrose e fibrose sejam estimuladas pela persistência parasitária tissular e reação imune adversa, como mecanismos fundamentais, coadjuvados por distúrbios autonômicos e microvasculares. Alguns deles recentemente constituíram alvos potenciais de terapêuticas.
A história natural das fases aguda e crônica foi revista, com realce para a transmissão oral, a forma indeterminada e as síndromes crônicas. Metanálises recentes de estudos observacionais estimaram o risco de evolução a partir das formas aguda e indeterminada e de mortalidade após instalação da cardiomiopatia crônica. Condutas terapêuticas aplicáveis aos indivíduos com a FIDC foram abordadas especificamente. Todos os métodos para detectar alterações estruturais e/ou funcionais com variadas técnicas de imageamento cardíaco também foram revisados, com recomendações de uso nos vários cenários clínicos. Estratificação de risco de mortalidade fundamentada no escore de Rassi, com estudos recentes de sua aplicação, foi complementada por métodos que detectam fibrose miocárdica.
A metodologia atual para diagnóstico etiológico e as consequentes implicações do tratamento tripanossomicida mereceram enfoque abrangente e aprofundado. Também o tratamento de pacientes em risco ou com insuficiência cardíaca, arritmias e eventos tromboembólicos, baseado em recursos farmacológicos e complementares, recebeu especial atenção. Capítulos suplementares subsidiaram as condutas aplicáveis a diversos contextos especiais, entre eles o da co-infecção por T. cruzi/HIV, risco durante cirurgias, em grávidas, na reativação da infecção após transplante cardíacos, e outros.nbsp;nbsp;nbsp;
Por fim, dois capítulos de grande significado social, abordando a estruturação de serviços especializados para atendimento aos indivíduos com a CDC, e revisando os conceitos de cardiopatia grave e suas implicações médico-trabalhistas completaram esta diretriz.nbsp;

Impact of COVID-19 on education, health and lifestyle behaviour of Brazilian urology residents

ABSTRACT Objectives: To evaluate the impact of COVID-19 on clinical and surgical practice, educational activities, health and lifestyle behavior of Brazilian urology residents. Materials and Methods: A web-based survey was sent to 468 Brazilian urology residents from postgraduate years (PGY) 3 to 5 to collect data on clinical practice and training after 4 months of COVID-19. We also assessed health-related and behavior changes, rate of infection by SARS-CoV-2, deployment to the front line of COVID-19, residents’ concerns, and access to personal protective equipment (PPE). Results: Massive reductions in elective and emergency patient consultations, diagnostic procedures and surgeries were reported across the country, affecting PGY 3 to 5 alike. Most in-person educational activities were abolished. The median damage to the urological training expected for 2020 was 6.0 [3.4 – 7.7], on a scale from 0 to 10, with senior residents estimating a greater damage (P< 0.001). Educational interventions developed included online case-based discussions, subspeciality conferences and lectures, and grand rounds. Most senior residents favored extending residency to compensate for training loss and most younger residents favored no additional training (p< 0.001). Modifications in health and lifestyle included weight gain (43.8%), reduced physical activity (68.6%), increased alcoholic intake (44.9%) and cigarette consumption (53.6%), worsening of sexual life (25.2%) and feelings of sadness or depression (48,2%). Almost half were summoned to work on the COVID-19 front-line and 24.4% had COVID-19. Most residents had inadequate training to deal with COVID-19 patients and most reported a shortage of PPE. Residents’ concerns included the risk of contaminating family members, being away from residency program, developing severe COVID-19 and overloading colleagues. Conclusions: COVID-19 had a massive impact in Brazilian urology residents´ training, health and lifestyle behavior, which may reflect what happened in other medical specialties. Studies should confirm these findings to help developing strategies to mitigate residents’ losses.

The use of bioprocess to convert low valued biomass or agroindustrial byproducts into high-value chemicals is an emerging area. However, laboratories usually use outdated analytical techniques to identify bioproducts, losing valuable information, such as the identification of unexpected compounds. In this work, we presented a successful systematic and modern analytical approach based on direct infusion mass spectrometry (DIMS) for an automated high-throughput screening of untargeted compounds from glycerin bioconversion process. We describe advantages of DIMS and its combined application with chemometrics towards an untargeted metabolomics approach to analyze several samples in a short time (11 samples per h). Three batches with 34 samples from the bioconversion of glycerin using several filamentous fungi strains were analyzed. Batch 347 was selected as promising, since high value chemicals, such as amide, phenolic and acid compounds, were identified. The platform presented was fast, robust, and versatile, then it could be applied to different bioprocesses.

ABSTRACT Presence of toxic compounds in marine coastal waters has increased exponentially since Industrial Revolution. In this way, we aimed to evaluate biochemical and physiological changes occurring within Hypnea musciformis after short-term exposure to gasoline. Hypnea musciformis was cultivated without gasoline and then exposed to various concentrations of it (0.001 % - 1.0 %, v/v) for periods of 30 min, 1 h, 12 h and 24 h. A Pricncipal Compound Analysis of UV-vis spectral window (200-700 nm) was able to discriminate gasoline-exposed samples according to both exposure time and gasoline concentration. Changes in carotenoid profile composition were observed. Decreased carotenoid content was associated to gasoline exposure time, being lutein and trans-β-carotene the major compounds found. Higher gasoline concentrations negatively interfered with phenolic compounds accumulation. In addition, increased gasoline concentrations corresponded to decreased intracellular starch grains content as well as increased its deposition on cell wall external surface. Data obtained allow us to conclude that gasoline can damage Hypnea musciformis physiology and cell morphology. This is important, considering Hypnea musciformis carotenoids and phenolics are potential biomarkers of environmental stress investigated, as well as its increased cell wall thickness to avoid gasoline diffusion.

Simple synthetic procedures have been applied to obtain luminescent carbon quantum dots, also referred as C-dots, from an abundant carbon source, that is, from the brewing industry waste. The synthetic procedures have been conducted aiming to investigate the effects of the oxidation stage on the properties of the nanomaterial. C-dots down- and up-conversion properties, as well as their potential for cellular imaging experiments in live (and adhered) cells, are disclosed herein.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Propôs-se com este estudo conhecer a variação do potencial da água no solo e na folha da videira "Superior Seedless", tal como o crescimento da baga da uva sob diferentes condições de irrigação na fase de maturação da uva, na Fazenda Agrobrás Tropical do Brasil S/A, em Casa Nova, BA, em outubro de 2007. A videira foi irrigada nas épocas 21, 13 e 5 dias antes da colheita, com lâminas de 100, 50 e 0% da evapotranspiração da cultura e um tratamento adicional ("manejo do produtor"). O potencial da água na folha e no solo foi aferido com a câmara de pressão e tensiômetros, respectivamente. O potencial da água da folha variou de -0,95 a -1,80 MPa, ao meio- dia e de -0,10 a -0,27 MPa, ao alvorecer. O potencial mátrico da água do solo atingiu valores de -5 a -79,5 kPa para os tratamentos menos e mais deficitários. O potencial da água na folha ao meio-dia sugere que, em alguns dias, o parreiral se encontrava sob estresse moderado e, em outros, sob estresse severo, independendo dos tratamentos de déficit hídrico. Ao alvorecer, o potencial indicou que as videiras estavam sob estresse suave ou em condições adequadas de manejo de água, mesmo a potencial matricial do solo elevado, em que não houve redução significativa no crescimento das bagas das uvas.
This study aimed to measure the variation of soil and leaf water potential in the Sugraone (Superior Seedless) grape, as well as the berry growth, under different irrigation conditions in the Fazenda Agrobrás Tropical do Brazil S/A, in Casa Nova, BA, in October, 2007 during maturation stage. The Superior Seedless cultivar was irrigated at 21, 13 e 5 days before harvest with water depths of 100, 50 e 0% of crop evapotranspiration (ETc) and an additional treatment consisting of the farmer management. The results of leaf water potential using a pressure chamber and the respective soil matric potential from tensiometric readings are shown. Measurements of leaf water potential reached values from -0,95 to -1,80 MPa, at noon and from -0,10 to -0,27 MPa, at pre-dawn. Soil water matric potential varied from -5 to -79,5 kPa, for the least water deficient treatments. Results from leaf water potential show that in some days the plants were under moderate stress and in others under severe stress, independently of water deficit treatments. Although pre-dawn leaf water potential indicated that the vines were under mild stress or under adequate water management practices, even under low soil matric potentials, there was no significant reduction in berry growth.

Objetivou-se, com o estudo, avaliar a adição do estéril resultante da extração da gipsita, como condicionante do solo, sobre a disponibilidade de água em um Neossolo Quartzarênico coletado no Município de Petrolina, PE. A capacidade de campo (CC) foi obtida pela saturação e subseqüente drenagem de um perfil simulado de solo em uma coluna de cilindros de PVC com 100 cm de altura, nas proporções de 0, 20, 40, 60 e 80 g kg-1 do estéril, incorporadas nos 40 cm superiores. O ponto de murcha permanente (PMP) foi determinado pelo método fisiológico; adicionalmente, a CC e o PMP o foram pela metodologia usual utilizando-se o extrator de Richards com uma pressão de 10 e 1500 kPa, respectivamente. A incorporação de doses crescentes do estéril aumentou a água disponível do solo, de 4,8 para 8,67%, quando determinado pelo método tradicional, nas doses 0 e 80 g kg-1, respectivamente, e de 14,76 para 15,08%, nas mesmas doses, quando a água disponível foi determinada a partir da CC e o PMP obtidas pelo método da coluna de drenagem e método fisiológico, respectivamente. Inserindo-se a umidade gravimétrica da capacidade de campo obtida pelo método da coluna de solo na curva de retenção de umidade, vê-se que este parâmetro foi melhor representado por um potencial matricial próximo -6 kPa.
This work aimed at evaluating the addition of the sterile material resulting from gypsite mining as a soil conditioner on the water availability of a Quartzsandy Neosol collected in the municipality of Petrolina, PE. Field capacity (FC) was obtained by the saturation and subsequent drainage of a simulated soil profile in a column made up of PVC cylinders 100 cm high where doses of 0, 20, 40, 60 and 80 g kg-1 of the sterile material were incorporated to the upper 40 cm. The permanent wilting point (PWP) was determined by the physiological method. In addition, the FC and the PWP were determined by the usual methodology using Richard's pressure plate with the pressures of 10 and 1500 kPa, respectively. The incorporation of increasing doses of sterile material increased the available soil water from 4.8 to 8.67%, determined by the traditional method, and from 14.76 to 15.08% in the same doses when available water was defined with the FC and the PWP determined by the column and physiological methods, respectively. By inserting the gravimetric water content of FC obtained by the column method in the soil water release curve, this parameter was best represented by a matric potencial close to -6 kPa.

OBJETIVO: Aplicação de energia por ultra-som pode facilitar a remoção da placa ateromatosa, mas o efeito desse procedimento em vasos próximos ainda é matéria de estudos experimentais. MÉTODOS: Para determinar se a energia ultra-sônica compromete a produção de óxido nítrico, segmentos de artérias coronárias caninas foram expostos a baixos (0-10 W) e altos (25 W) níveis de energia por 15 segundos, utilizando-se protótipo de aparelho para a realização de endarterectomia. Após exposição, segmentos das artérias coronarianas foram estudados em organ chambers. Para os ensaios farmacológicos foram utilizadas as seguintes drogas:difosfato de adenosina (ADP), acetilcolina (Ach) e fluoreto de sódio (NaF) para a avaliação do relaxamento dependente do endotélio. O nitroprussiato de sódio (NPS) e o isoproterenol foram utilizados para a avaliação do relaxamento independente do endotélio. RESULTADOS: A aplicação de alta energia ultra-sônica comprometeu o relaxamento dependente do endotélio induzido por ADP (10-9 - 10-4 M), Ach (10-9 - 10-4 M) e NaF (0,5 -9,5 mM) em artérias coronarianas epicárdicas. Entretanto, baixos valores de energia ultra-sônica não alteraram o relaxamento dependente do endotélio (nem o relaxamento máximo e nem a EC50) induzido pelos mesmos agonistas. O relaxamento da musculatura lisa vascular induzido por isoproterenol (10-9 - 10-5 M) ou NPS (10-9 - 10-6 M) não foi comprometido, tanto por baixos, quanto por altos níveis de energia ultra-sônica. CONCLUSÃO: Os experimentos demonstram que altas energias ultra-sônicas alteram a função endotelial. Entretanto, o ultra-som não altera a habilidade de relaxamento da musculatura lisa vascular de artérias caninas epicárdicas.
OBJECTIVE: Application of ultrasound energy by an endarterectomy probe can facilitate the removal of atheromatous plaque, but the effect of this procedure on surrounding vessel structure and function is still a matter of experimental investigations. METHODS: To determine whether ultrasound energy impairs the production of nitric oxide or damages vascular smooth muscle function, isolated canine epicardial coronary artery segments were exposed to either high (25 W) or low (0-10 W) ultrasonic energy outputs, for 15 seconds, using an endarterectomy device prototype. After exposure, segments of epicardial coronary artery were studied in organ chambers. The following drugs were used: adenosine diphosphate (ADP), acetylcholine (Ach) and sodium fluoride (NaF) to study endothelium-dependent relaxation and sodium nitroprusside (SNP) and isoproterenol to evaluate endothelium-independent relaxation. RESULTS: Application of high ultrasonic energy power impaired endothelium-dependent relaxation to ADP (10-9 - 10-4 M), Ach (10-9 - 10-4 M) and NaF (0.5 - 9.5 mM) in epicardial coronary arteries. However, low ultrasound energy output at the tip of the probe did not alter the endothelium-dependent relaxation (either maximal relaxation or EC50) to the same agonists. Vascular smooth muscle relaxation to isoproterenol (10-9 - 10-5 M) or SNP (10-9 - 10-6 M) was unaltered following exposure to either low or high ultrasonic energy outputs. CONCLUSION: These experiments currently prove that ultrasonic energy changes endothelial function of epicardial coronary arteries at high power. However, ultrasound does not alter the ability of vascular smooth muscle of canine epicardial coronary arteries to relax.

Medidas diretas de evapotranspiração de referência são importantes na avaliação e na calibração de métodos indiretos. Avaliou-se o desempenho de lisímetro de pesagem hidráulica de baixo custo, visando à determinação direta da evapotranspiração de referência (ETo), em área de assentamento rural no Semi-Árido nordestino. O lisímetro é de concepção simplificada, com travesseiros hidráulicos confeccionados com mangueira flexível de butil e revestida em náilon, interligados a um sistema manométrico com mercúrio e água. O equilíbrio do lisímetro foi estável, sendo o maior erro médio posicional de 3,93%. As calibrações geraram coeficientes de correlação superiores a 0,9993 e resoluções de leituras entre 0,095 e 0,134 mm. As leituras lisimétricas foram comparadas com estimativas da ETo obtidas, utilizando-se dos métodos de Penman-Monteith, Blaney-Criddle-FAO, Jensen-Haise e do Tanque Classe A-FAO. Adicionalmente, avaliou-se o desempenho de um tanque evaporimétrico simplificado, construído a partir de material reciclado, o qual vem sendo utilizado no manejo da irrigação em áreas de agricultura familiar. Todos os métodos analisados apresentaram bom desempenho, com medidas bem correlacionadas aàs do lisímetro, para o período estudado.
Direct evapotranspiration measurements are important to the calibration of indirect methods. The performance of a low cost hydraulic weighing lysimeter was evaluated, aiming to estimate the reference evapotranspiration (ETo) in a settlement area in the Semi Arid Brazilian northeast. The lysimeter presents a simplified design, with loading cells of butyl internally covered with nylon, connected to a manometric system with mercury and water. The lysimeter equilibrium was stable, with the largest positional average error of 3.93%. Calibrations produced correlation coefficients higher than 0.9993, and reading resolutions between 0.095 and 0.134 mm. Lysimeter readings were compared to ETo estimate Penman-Monteith, Blaney-Criddle FAO, Jensen-Haise, and Class A methods. Additionally, the performance of a simplified evaporimetric tank was evaluated, built with recycled material, which has been used for irrigation management in communal areas. All the considered methods produced good results, well correlated to the readings from the lysimeter, for the studied period.

Endothelium-dependent relaxation in response to poly-L-arginine in canine coronary arteries: implications about hyperpolarization as a mechanism of vasodilatation

OBJECTIVE: To study the mechanism by which poly-L-arginine mediates endothelium-dependent relaxation. METHODS: Vascular segments with and without endothelium were suspended in organ chambers filled with control solution maintained at 37ºC and bubbled with 95% O2 / 5% CO2. Used drugs: indomethacin, acetycholine, EGTA, glybenclamide, ouabain, poly-L-arginine, methylene blue, N G-nitro-L-arginine, and verapamil and N G-monomethyl-L-arginine. Prostaglandin F2á and potassium chloride were used to contract the vascular rings. RESULTS: Poly-L-arginine (10-11 to 10-7 M) induced concentration-dependent relaxation in coronary artery segments with endothelium. The relaxation to poly-L-arginine was attenuated by ouabain, but was unaffected by glybenclamide. L-NOARG and oxyhemoglobin caused attenuation, but did not abolish this relaxation. Also, the relaxations was unaffected by methylene blue, verapamil, or the presence of a calcium-free bathing medium. The endothelium-dependent to poly-L-arginine relaxation was abolished only in vessels contracted with potassium chloride (40 mM) in the presence of L-NOARG and indomethacin. CONCLUSION: These experiments indicate that poly-L-arginine induces relaxation independent of nitric oxide.

Com o objetivo de erradicar o BHV-1 de um rebanho bovino leiteiro de alto valor genético sem a utilização de vacina, foi realizado um exame sorológico prévio em 154 animais, onde constatou-se 15,6% de reagentes ao BHV-1. A técnica utilizada foi a soroneutralização em microplacas. Dentre os animais soropositivos, as vacas vazias foram descartadas imediatamente e as prenhes isoladas e descartadas após o parto. Os bezerros apresentaram anticorpos colostrais até os seis meses de idade, motivo pelo qual não foram descartados; nos bezerros de 6 a 12 meses de idade e nas novilhas não foram diagnosticados animais soropositivos. Os animais foram examinados trimestralmente, por 21 meses, seguido de mais duas coletas semestrais. As vacas secas, prenhes e em lactação, soropositivas, revelaram ser a fonte de infecção do BHV-1. A manutenção de rebanho livre é possível, desde que sejam adotadas medidas como a utilização de sêmen livre de BHV-1, realização de quarentena no ingresso de animais e exames sorológicos anuais visando impedir a reintrodução do vírus. Com o conjunto destas medidas adotadas, a fazenda encontra-se há 18 meses livre do BHV-1.
To eradicate the BHV-1 from a dairy cattle herd without vaccination, a preliminary serological survey was performed in 154 animals, that revealed 15.6% seropositives for BHV-1. The test used was serum neutralization with the microtitration technique. The seropositive nonpregnant cows were immediately eliminated from the herd and the pregnant cows were isolated and eliminated after parturiation. The calves with colostral antibodies were kept in the herd. There were no seropositive animals between the 6 to 12 months old calves and the heifers. The animals were monitored serologically every 3 months for 21 months and afterwords in 2 semestral intervals. The seropositive nonpregnant, pregnant and the nursing cows were the source of BHV-1 infecction. In order to maintain a BHV-1 free herd, measurements of control had been taken, as the utilization of virus-free semen, quarantine when animals were introduced into the herd, and semestral serological surveys. Through these measurements the farm has been BHV-1 free for 18 months.
