Abstract Background Cardiovascular complications of COVID-19 are important aspects of the disease’s pathogenesis and prognosis. Evidence on the prognostic role of troponin and myocardial injury in Latin American hospitalized COVID-19 patients is still scarce. Objectives To evaluate myocardial injury as independent predictor of in-hospital mortality and invasive mechanical ventilation support in hospitalized patients, from the Brazilian COVID-19 Registry. Methods This cohort study is a substudy of the Brazilian COVID-19 Registry, conducted in 31 Brazilian hospitals of 17 cities, March-September 2020. Primary outcomes included in-hospital mortality and invasive mechanical ventilation support. Models for the primary outcomes were estimated by Poisson regression with robust variance, with statistical significance of p<0.05. Results Of 2,925 patients (median age of 60 years [48-71], 57.1% men), 27.3% presented myocardial injury. The proportion of patients with comorbidities was higher among patients with cardiac injury (median 2 [1-2] vs. 1 [0-2]). Patients with myocardial injury had higher median levels of brain natriuretic peptide, lactate dehydrogenase, creatine phosphokinase, N-terminal pro-brain natriuretic peptide, and C-reactive protein than patients without myocardial injury. As independent predictors, C-reactive protein and platelet counts were related to the risk of death, and neutrophils and platelet counts were related to the risk of invasive mechanical ventilation support. Patients with high troponin levels presented a higher risk of death (RR 2.03, 95% CI 1.60-2.58) and invasive mechanical ventilation support (RR 1.87, 95% CI 1.57-2.23), when compared to those with normal troponin levels. Conclusion Cardiac injury was an independent predictor of in-hospital mortality and the need for invasive mechanical ventilation support in hospitalized COVID-19 patients.

Resumo Fundamento As complicações cardiovasculares da COVID-19 são aspectos importantes da patogênese e do prognóstico da doença. Evidências do papel prognóstico da troponina e da lesão miocárdica em pacientes hospitalizados com COVID-19 na América Latina são ainda escassos. Objetivos Avaliar a lesão miocárdica como preditor independente de mortalidade hospitalar e suporte ventilatório mecânico em pacientes hospitalizados, do registro brasileiro de COVID-19. Métodos Este estudo coorte é um subestudo do registro brasileiro de COVID-19, conduzido em 31 hospitais brasileiros de 17 cidades, de março a setembro de 2020. Os desfechos primários incluíram mortalidade hospitalar e suporte ventilatório mecânico invasivo. Os modelos para os desfechos primários foram estimados por regressão de Poisson com variância robusta, com significância estatística de p<0,05. Resultados Dos 2925 pacientes [idade mediana de 60 anos (48-71), 57,1%], 27,3% apresentaram lesão miocárdica. A proporção de pacientes com comorbidades foi maior nos pacientes com lesão miocárdica [mediana 2 (1-2) vs. 1 (0-20)]. Os pacientes com lesão miocárdica apresentaram maiores valores medianos de peptídeo natriurético cerebral, lactato desidrogenase, creatina fosfoquinase, N-terminal do pró-peptídeo natriurético tipo B e proteína C reativa em comparação a pacientes sem lesão miocárdica. Como fatores independentes, proteína C reativa e contagem de plaquetas foram relacionados com o risco de morte, e neutrófilos e contagem de plaquetas foram relacionados ao risco de suporte ventilatório mecânico invasivo. Os pacientes com níveis elevados de troponina apresentaram um maior risco de morte (RR 2,03, IC95% 1,60-2,58) e suporte ventilatório mecânico (RR 1,87;IC95% 1,57-2,23), em comparação àqueles com níveis de troponina normais. Conclusão Lesão cardíaca foi um preditor independente de mortalidade hospitalar e necessidade de suporte ventilatório mecânico em pacientes hospitalizados com COVID-19.

RESUMO A teleconsulta pode ser definida como uma prática avançada de enfermagem que requer do enfermeiro raciocínio clínico fundamentado em um arcabouço teórico consistente para aplicação no processo de enfermagem. No presente estudo, realizou-se uma análise teórico-reflexiva, fundamentada no Modelo de Adaptação de Callista Roy e na Teoria de Transição de Chick-Meleis, sobre a contribuição da teleconsulta como prática avançada de enfermagem no atendimento a pacientes idosos e com doenças crônicas no contexto da pandemia de covid-19. A reflexão é apresentada em dois momentos: “teorias de enfermagem e o enfrentamento da covid-19” e “formas de adaptação a novos modelos de assistência e as práticas avançadas em enfermagem”, norteados pelas tecnologias de comunicação e informação. O agravamento da pandemia no Brasil trouxe mudanças relacionadas aos ciclos de vida, aos processos saúde/doença e organizacionais, demandando o desenvolvimento de um estado adaptativo-transacional por parte dos usuários e profissionais de saúde. Neste contexto, as tecnologias da informação e comunicação aliadas à prática avançada de enfermagem representam papel fundamental para atenuar o distanciamento social e suas repercussões na assistência à saúde.

ABSTRACT Teleconsultation can be classified as an advanced practice nursing that requires nurses’ clinical reasoning based on a consistent theoretical framework to use in the nursing process. Our study conducted a theoretical-reflective analysis, based on Callista Roy’s Adaptation Model of Nursing and Chick-Meleis’ Transition Theory, about the contribution of teleconsultation as an advanced practice nursing in the care of older adults with chronic diseases during the COVID-19 pandemic. We reflect on this in two moments: “nursing theories and dealing with COVID-19” and “ways of adapting to new care models and advanced practice nursing,” based on communication and information technologies. The worsening of the pandemic in Brazil changed life cycles, health/disease and organizational processes, demanding the development of an adaptive-transactional state by users and health care providers. Thus, information and communication technologies combined with advanced practice nursing can relieve social distancing and its repercussions on health care.

RESUMEN La teleconsulta es una herramienta de la enfermería de práctica avanzada que requiere un razonamiento clínico de los profesionales de enfermería basado en un marco teórico consistente para aplicarse en el proceso de enfermería. Este estudio realizó un análisis teórico-reflexivo, basado en el Modelo de Adaptación de Callista Roy y en la Teoría de las Transiciones de Chick-Meleis, sobre el aporte de la teleconsulta como herramienta de la enfermería de práctica avanzada a la atención de pacientes adultos mayores y de aquellos con enfermedades crónicas en el contexto de la pandemia del Covid-19. La reflexión se presenta en dos momentos: “teorías de enfermería y enfrentamiento al Covid-19” y “modos de adaptación a los nuevos modelos de asistencia y las prácticas avanzadas en enfermería”, fundamentados en las tecnologías de la información y la comunicación. El avance de la pandemia en Brasil trajo cambios relacionados con los ciclos de vida, los procesos de salud/enfermedad y procesos organizacionales, que requirieron el desarrollo de un estado adaptativo-transaccional por parte de los usuarios y los profesionales de la salud. En este contexto, las tecnologías de la información y la comunicación, sumadas a la enfermería de práctica avanzada, jugaron un papel clave para mitigar el distanciamiento social y sus repercusiones en la asistencia sanitaria.

ABSTRACT: Seasonal variations and production factors can influence the quality of grapes. Considering the Sub-middle region of the São Francisco Valley, Brazil, differentiated by production in rainy and warmer periods of the year, the climatic conditions in each season affect the duration of the phenological phases and quality of the grapes. The objective of this study was to characterize the changes in the quality components during maturation of ‘BRS Magna’ grapes on different rootstocks, in rainy seasons under tropical conditions, as a support to define the harvest time. The experiment was conducted in Petrolina, Pernambuco, Brazil, from Jan to Apr 2017 (cycle 1) and Dec 2017 to Apr 2018 (cycle 2) evaluating rootstocks and age of the fruits. Bunches were picked from the start of maturation to the harvest: 49, 56, 61, 64 and 68 days after fruit set (DAF) for cycle 1, and 35, 41, 48, 55, 61, and 66 DAF for cycle 2. In cycle 1 ‘IAC 572’ and ‘Paulsen 1103’ rootstocks provided higher sugar contents in grapes, which was only repeated in cycle 2 for ‘Paulsen 1103’. The accumulation of anthocyanins and yellow flavonoids was favored by ‘IAC 313’ and ‘IAC 572’ rootstocks in cycle 1, and by ‘IAC 313’ and ‘IAC 766’ in cycle 2. A higher content of polyphenols was conferred on ‘IAC 572’ in cycle 1 and on ‘IAC 766’ in cycle 2, which also had a greater antioxidant capacity. Although there were differences between rootstocks, harvesting at approximately 64 DAF maximized grape quality for all treatments.

Resumo As unidades neonatais devem ser organizadas como uma linha de cuidados progressivos com leitos de cuidado intensivo e intermediário (convencional e canguru). O objetivo deste estudo foi avaliar a situação e a adequação dos leitos neonatais em maternidades da Rede Cegonha. Estudo descritivo, realizado em 606 maternidades em todas as regiões do Brasil. Os bancos de dados utilizados foram os da Avaliação da Atenção ao Parto e Nascimento em Maternidades da Rede Cegonha e do Sistema Nacional de Nascidos Vivos. Para avaliar a distribuição de leitos neonatais por tipologia, foram utilizados os parâmetros propostos na Portaria GM/MS nº 930/2012. A minoria das unidades se organiza como uma linha de cuidados progressiva com as três tipologias de leito previstas (24,42%). Os leitos de cuidado intermediário Canguru são a minoria dos leitos implantados (11,27%). Há uma concentração de leitos intensivos e intermediários nas regiões Sudeste e Sul, que apresentam déficit de leitos de cuidado intermediário Canguru. Ao analisar a adequação dos leitos pelo número de nascidos vivos, verifica-se inadequação dos leitos de cuidado Canguru em todas as regiões do Brasil, déficit de leitos intensivos nas regiões Norte e Nordeste e adequação de leitos de cuidado intermediário convencional em todas as regiões.

Abstract Neonatal units should be organized as a progressive care line, with intermediate and intensive care beds (conventional and kangaroo). The aim of this study was to evaluate the status and adequacy of neonatal beds in maternity hospitals linked to the ‘Stork Network’ (“Rede Cegonha”). A descriptive study was conducted in 606 maternity hospitals in all regions of Brazil. The databases used belonged to the Stork Network Evaluation Survey and the National Live Birth System. To assess the distribution of neonatal beds by typology, the parameters proposed in Ordinance N. 930/2012 of the Ministry of Health were used. Most neonatal units are not organized as a progressive care line with the three types of bed planned. Kangaroo intermediate care beds comprise the minority of implanted beds. There is a concentration of intensive and intermediate beds in the Southeast and South regions, which show a kangaroo intermediate care bed deficit. Analyzing the adequacy of beds by the number of live births, one can observe an inadequacy of Kangaroo care beds in all regions of Brazil, as well as intensive bed deficit in the North and Northeast regions, and adequacy of conventional intermediate care beds in all regions.

Resumo O artigo tem como objetivo comparar práticas do cuidado ao recém-nascido saudável no momento do nascimento em hospitais públicos e mistos conveniados ao SUS, segundo o tipo de parto, verificadas na pesquisa “Nascer no Brasil/2011” (NB) e no último ciclo avaliativo da RC, aqui denominada “Avaliação da Rede Cegonha/2017” (ARC). O NB incluiu uma amostra com representatividade nacional de 266 hospitais e a ARC foi conduzida em 606 maternidades inseridas na estratégia Rede Cegonha, totalizando 15.994 e 8.047 pares de puérperas e recém-nascidos saudáveis, respectivamente. Entre os dois estudos, NB-2011 e ARC-2017, embora a proporção de cesariana tenha se mantido em torno de 44%, a prevalência do contato pele a pele com o RN, da amamentação na sala de parto e nas primeiras 24h de vida aumentaram, 140%, 82% e 6%, respectivamente. Já a proporção de aspiração de vias aéreas superiores do RN, caiu 65%. Os resultados indicam que o uso de diretrizes baseadas em evidências para o cuidado de recém-nascidos saudáveis aumentou na prática clínica, considerando o período de seis anos entre os estudos comparados. Apesar desse progresso, permanecem importantes desafios para garantir as melhores práticas para a totalidade de mulheres e recém-nascidos, principalmente em relação aos partos cesáreos.

Abstract This paper aims to compare best practices for healthy newborns in public and mixed hospitals affiliated with SUS, according to type of birth, between “Nascer no Brasil/2011” (NB – Birth in Brazil) and in the last assessment cycle of Rede Cegonha, here called “Avaliação da Rede Cegonha/2017” (ARC – Stork Network Assessment). NB included a sample with national representativeness of 266 hospitals, and ARC was conducted in 606 maternity hospitals included in the Rede Cegonha strategy, totaling 15,994 and 8,047 pairs of healthy mothers and newborns, respectively.Between the two studies, NB-2011 and ARC-2017, although the proportion of cesarean sections remained around 44%, the prevalence of skin-to-skin contact with newborns, breastfeeding in the delivery room and breastfeeding in the first 24h of life increased by 140%, 82% and 6%, respectively. The proportion of upper airway aspiration of newborns dropped 65%. The results indicate that the use of evidence-based guidelines for the care of healthy newborns has increased in clinical practice, considering the six-year period between the compared studies. Despite the progress, important challenges remain to ensure best practices for all women and newborns, especially in relation to cesarean births.

Abstract The increasing use of natural biodegradable polymers to preserve foods is a consequence of the concern of society with sustainability. Starch stands out amongst the natural polymers due to its great availability on a worldwide scale, high extraction yield and nutritional value, low cost, biodegradability and biocompatibility. Apart from the traditional sources, various others exist with extraction potential. The objective of this study was to identify films from different non-traditional starch sources with properties revealing their potential to be applied as coating for fruits and vegetables. The biodegradable films were prepared with three starch concentrations (2, 3 and 3.5%) for each of the following sources: cassava, yam, jackfruit seed and mango kernel. The starches from the sources studied are promising ingredients for the formulation of films, presenting good physical, optical (such as transparency) and mechanical characteristics, principally tensile strength whose maximum value was 36.63 MPa. Increases in starch concentration directly influenced the thickness, water vapor permeability, tensile strength and puncture force. The films with the greatest potentials for use according to their characteristics were 3% cassava, 3% yam, 3.5% jackfruit seed and 2% mango.

RESUMO O umbuzeiro e umbu-cajazeira demandam informações sobre as características de acessos potenciais para implantação comercial e preservação. Assim, objetivou-se avaliar o teor foliar e ciclagem dos nutrientes de 15 acessos de umbuzeiro e um de umbucajazeira. Os tratamentos constaram de 16 acessos: BGU -44, BGU-45, BGU-47, BGU-48, BGU-50, BGU-75, EPAMIG-01, EPAMIG-03, EPAMIG-04, EPAMIG-05, EPAMIG-06, EPAMIG-07, EPAMIG-09, EPAMIG-13, umbuzeiro de origem desconhecida e umbu-cajazeira Princesa. Adotou-se o delineamento experimental inteiramente casualizado, com três repetições, constituídas de uma planta cada. Foram mensurados os teores de nutrientes nas folhas, nas fases de emissão de fluxo foliar e senescência das folhas, nas safras de 2015/2016 e 2016/2017, e determinou-se a taxa de retranslocação de N, P, K e Mg. Verificou-se que os acessos denotaram teores médios de nutrientes na folha de 28,6; 27,6; 9,5; 4,20; 2,5 e 2,0 g kg-1 com ordem decrescente N>Ca>K>Mg>S>P, respectivamente, e 103,61; 86,22; 82,12; 60,2; 16,0 e 3,52 mg kg-1 de Fe>B>Na>Mn>Zn>Cu, respectivamente. O acesso BGU-48 revelou maior eficiência de retranslocação de N e P nas folhas e o BGU-50 expressou eficiência de reciclagem para K e Mg. A ordem das taxas de retranslocação é K>P>N>Mg sendo maior na safra 2015/2016.

ABSTRACT Information is needed on the characteristics of potential accessions of Spondias tuberosa Arruda Câmara (umbu) and Spondias sp.(umbu-caja) for commercial planting and preservation. Thus, the objective was to evaluate the leaf contents and cycling of nutrients of 15 accessions of umbu and one of umbu -caja. The treatments consisted of 16 accessions: BGU-44, BGU-45, BGU-47, BGU-48, BGU-50, BGU-75, EPAMIG-01, EPAMIG-03, EPAMIG-04, EPAMIG-05, EPAMIG-06, EPAMIG-07, EPAMIG-09, EPAMIG-13, umbu of unknown origin and the umbu-caja Princesa. A completely randomized experimental design was adopted, with three replicates, consisting of one plant each. Nutrient contents in the leaves were measured in the leaf flushing and leaf senescence phases in the 2015/2016 and 2016/2017 seasons, and the N, P, K and Mg retranslocation rates were determined. The accessions showed average leaf nutrient contents of 28.6, 27.6, 9.5, 4.20, 2.5 and 2.0 g kg-1 with the descending order N>Ca>K>Mg>S>P, respectively, and 103.61, 86.22, 82.12, 60.2, 16.0 and 3.52 mg kg-1 of Fe>B>Na>Mn>Zn>Cu, respectively. The accession BGU-48 had higher N and P retranslocation efficiency in the leaves and BGU-50 showed recycling efficiency for K and Mg. The order of retranslocation rates is K>P>N>Mg, being higher in the 2015/2016 season.

RESUMO Inhame, mandioca, semente de jaca e amêndoa da manga têm potencial para extração de amido e utilização na indústria alimentícia ou para formulação de revestimentos biodegradáveis. Como revestimento, o amido pode ser aplicado em frutas de maturação rápida, como manga, que requer tecnologias para incrementar sua vida útil. O objetivo desse trabalho foi caracterizar o amido de fontes não tradicionais e avaliar o potencial como revestimento em manga ‘Palmer’. Os amidos foram extraídos de mandioca, amêndoa de manga, semente de jaca e inhame, tendo sido caracterizados para propriedades físicas, ópticas e químicas, para posterior uso como revestimentos em manga ‘Palmer’. Os revestimentos utilizados foram amido de mandioca a 3%; de semente de jaca a 3,5%; de amêndoa de manga a 3,5% e de inhame a 3,5%, que foram comparados ao controle (sem revestimento). Os frutos foram armazenados a 24,4 ± 0,3 °C e 87 ± 2% de UR e avaliados por até 12 dias. O delineamento experimental foi inteiramente casualizado, em fatorial 5x7. O teor de amido total foi superior a 70% nas quatro fontes. Os amidos de semente de jaca e de inhame apresentaram maior teor de amilose. Os amidos das quatro fontes apresentaram baixa solubilidade em água e poder de inchamento, destacando-se o de semente de jaca. Os revestimentos mantiveram a qualidade dos frutos, destacando-se o amido de amêndoa de manga por reduzir a respiração e a perda de massa em 27,7% e 33,8%, respectivamente, bem como o da semente de jaca que também atrasou o amarelecimento da casca.

ABSTRACT Yam, cassava, jackfruit seed and mango seed kernel have potential for the extraction and use as starch in the food industry of starch or for the formulation of biodegradable coatings. As a biodegradable coating, starch can be applied in fruits characterized by a fast maturation, such as mango, which requires technologies to increase its shelf life. The aim of this study was to characterize starch from four non-traditional sources and to evaluate their potential as coating for ‘Palmer’ mango fruit. Starches used were extracted from cassava, mango seed kernel, jackfruit seed, and yam, and had their physical, optical, and chemical properties characterized for later use as coatings of ‘Palmer’ mango fruit. Fruits were coated with 3% cassava starch, 3.5% jackfruit seed starch, 3.5% mango seed kernel starch and 3.5% yam starch, and were compared to the control (uncoated). They were then stored at 24.4 ± 0.3 °C and 87 ± 2% RH and evaluated for 12 days. A 5x7 factorial arrangement in a completely randomized experimental design was adopted. Total starch content was higher than 70% in the four sources of starch. Starches from jackfruit and yam had higher amylose content. The four sources of starch had low water solubility and swelling power, with jackfruit seed starch having the highest values. The coating sources were effective in maintaining quality, particularly mango seed kernel starch because it reduced respiratory rate and weight loss in 27.7% and 33.8%, respectively, as well as jackfruit seed starch as it delayed fruit skin yellowing.

RESUMO A fibrose pulmonar idiopática (FPI) é uma forma de pneumopatia intersticial crônica fibrosante de causa desconhecida, que acomete preferencialmente homens idosos, com história atual ou pregressa de tabagismo. Mesmo sendo uma doença incomum, ela assume grande importância devido a sua gravidade e prognóstico reservado. Nas últimas décadas, diversas modalidades terapêuticas farmacológicas foram investigadas para o tratamento dessa doença, de tal modo que conceitos clássicos vêm sendo revisados. O objetivo destas diretrizes foi definir recomendações brasileiras baseadas em evidências em relação ao emprego de agentes farmacológicos no tratamento da FPI. Procurou-se fornecer orientações a questões de ordem prática, enfrentadas pelos clínicos no seu cotidiano. As perguntas PICO (acrônimo baseado em perguntas referentes aos Pacientes de interesse, Intervenção a ser estudada, Comparação da intervenção e Outcome [desfecho] de interesse) abordaram aspectos relativos ao uso de corticosteroides, N-acetilcisteína, tratamento medicamentoso do refluxo gastroesofágico, inibidores dos receptores da endotelina, inibidores da fosfodiesterase-5, pirfenidona e nintedanibe. Para a formulação das perguntas PICO, um grupo de especialistas brasileiros atuantes na área foi reunido, sendo realizada uma extensa revisão bibliográfica sobre o tema. As revisões sistemáticas com meta-análises previamente publicadas foram analisadas quanto à força das evidências compiladas e, a partir daí, foram concebidas recomendações seguindo a metodologia Grading of Recommendations Assessment, Development and Evaluation. Os autores acreditam que o presente documento represente um importante avanço a ser incorporado na abordagem de pacientes com FPI, objetivando principalmente favorecer seu manejo, e pode se tornar uma ferramenta auxiliar na definição de políticas públicas relacionadas à FPI.

ABSTRACT Idiopathic pulmonary fibrosis (IPF) is a form of chronic interstitial lung disease of unknown cause, which predominantly affects elderly men who are current or former smokers. Even though it is an uncommon disease, it is of great importance because of its severity and poor prognosis. In recent decades, several pharmacological treatment modalities have been investigated for the treatment of this disease, and the classic concepts have therefore been revised. The purpose of these guidelines was to define evidence-based recommendations regarding the use of pharmacological agents in the treatment of IPF in Brazil. We sought to provide guidance on the practical issues faced by clinicians in their daily lives. Patients of interest, Intervention to be studied, Comparison of intervention and Outcome of interest (PICO)-style questions were formulated to address aspects related to the use of corticosteroids, N-acetylcysteine, gastroesophageal reflux medications, endothelin-receptor antagonists, phosphodiesterase-5 inhibitors, pirfenidone, and nintedanib. To formulate the PICO questions, a group of Brazilian specialists working in the area was assembled and an extensive review of the literature on the subject was carried out. Previously published systematic reviews with meta-analyses were analyzed for the strength of the compiled evidence, and, on that basis, recommendations were developed by employing the Grading of Recommendations Assessment, Development and Evaluation approach. The authors believe that the present document represents an important advance to be incorporated in the approach to patients with IPF, aiming mainly to improve its management, and can become an auxiliary tool for defining public policies related to IPF.

ABSTRACT: The biosynthesis of phenolic compounds might be influenced by environmental factors, such as solar radiation, temperature and relative air humidity, and production system. Some components of the production system, particularly the trellis system and rootstock, might change the phenolic composition of grapes due to microclimate of the cultivation region. In this study, the phenolic profile of ‘Chenin Blanc’ grapes was characterized with different trellis systems and rootstocks in two consecutive production cycles by using high-performance liquid chromatography under tropical semi-arid conditions. In the second production cycle (Jan-May/2016), accumulation of (-) - epicatechin gallate and (-) - epigallocatechin gallate was higher when vigorous rootstocks were trellised to the lyre system, whereas the lyre system associated to rootstock ‘SO4’ resulted in higher rutin accumulation in the same cycle. There was little influence of the factors studied on trans-resveratrol and piceatannol accumulation. However, a higher accumulation of piceatannol was observed in the second productive cycle compared to cis-resveratrol and trans-resveratrol. This study highlights that accumulation of phenolic compounds is influenced not only by environmental factors typical of the production year season, but also by the trellis system and rootstock adopted. The identification of phenolic compounds in the accumulation stimulated by the combination of trellis system, rootstock, and production season allows to differentiate grape quality and add value to products derived from such combination in a tropical semi-arid region.

ABSTRACT The Brazilian Cerrado, a global biodiversity hotspot, has been widely converted by agricultural expansion and pasture establishment. As a result of these activities, several plant species have experienced population declines such as pequi (Caryocar brasiliense ). We analyzed the feasibility of economic use of C. brasiliense based on population structure and fruit productivity. The average fruit production was high and significantly different between 2013 (515 fruits ind-1) and 2014 (344 fruits ind-1) (t = 2.32, p < 0.01), with 33% less fruit production in 2014. The canopy area (r2 = 0.22, p < 0.05) was the only population parameter related to productivity (2013). One hectare may yield an average of one ton of complete fruits, which is equivalent to up to 16 liters of pulp oil. The family income obtained with oil pulp sale could up to R$ 1,080 (US$ 327). We support the hypothesis that high pequi fruit productivity makes the extraction of this resource viable, especially for giant earthworm extractors.

ABSTRACT Objective: to assess the results of the ethical-professional nursing inspection at Long-Term Care Institutions for the Elderly. Method: observational, retrospective study with documentary analysis. Between January 2010 and December 2013, 2,650 control inspections were undertaken in the Rio de Janeiro Regional Nursing Council. Of these, 159 (6%) were Long-Term Care Institutions for the Elderly. The final sample consisted of 51 processes that complied with the criterion of having at least two inspections for comparison. The data collection instrument rested on the four pillars of inspection: legal practice of the profession; staffing; systemization of nursing care and laws related to professional practice. For the descriptive and inferential data analysis, SPSS and Excel 2007 were used. Results: of the 51 institutions assessed, 80.4 were private. The main motive for the inspections was to comply legal orders by the Public Prosecution (56.9%) difference was found between the first and last inspection: nursing professionals registered in the council (0.006); classification of care by dependence level (0.008); presenting a scale of professionals (0.006); not having other professional in the nursing scale, mainly caregivers (0.001); single file (0.039); nursing standards and routines (0.000); presenting standard operating protocols (0.000); and Nursing Process (0.001). Conclusion: the strengthening of the inspections is recommended, in line with the Public Prosecution and the National Health Surveillance Agency. In addition, the functioning in compliance with ethical-professional regularities should be made feasible, including the Long-Term Care Institutions for the elderly in the social-health sphere, with nursing at the largest group of workers.

RESUMEN Objetivo: evaluar los resultados de la fiscalización ético-profesional de enfermería en Instituciones de Larga Permanencia para Ancianos. Método: estudio observacional y retrospectivo de análisis documentario. En el período de Enero del 2010 hasta Diciembre del 2013 se realizaron 2.650 inspecciones fiscales en el Consejo Regional de Enfermería del Estado de Rio de Janeiro. Así, 159 (6%) de las mismas se referían a las Instituciones de Larga Permanencia para Ancianos. La muestra final incluyó 51 procesos que atendieron al criterio de poseer, como mínimo, dos fiscalizaciones para comparaciones. El instrumento de recolección de datos se fundamentó en los cuatro pilares de la fiscalización: ejercicio legal de la profesión; dimensionamiento del personal; sistematización de la asistencia de enfermería y legislaciones afines con el ejercicio profesional. Para el análisis descriptivo e inferencial de los datos se usaron los programas SPSS y Excel 2007. Resultados: el 80,4% de las 51 instituciones evaluadas eran privadas. El principal motivo de las fiscalizaciones fue cumplir órdenes del Ministerio Público (56,9%). Hubo diferencia entre la primera y la última fiscalización: profesionales de enfermería con registro en el consejo (0,006); clasificación de cuidados por dependencia (0,008); presentar la escala de los profesionales (0,006); no tener otros profesionales en la escala de enfermería, principalmente cuidadores (0,001); prontuario único (0,039); normas y rutinas de enfermería (0,000); presentar protocolos operacionales estándares (0,000) y Proceso de Enfermería (0,001). Conclusión: se recomienda fortalecer las fiscalizaciones en consonancia con el Ministerio Público y la Agencia Nacional de Vigilancia Sanitaria. Además, se debe viabilizar el funcionamiento conforme a las regularidades ético-profesionales, incluyendo a las Instituciones de Larga Permanencia para Ancianos en la esfera socio-sanitaria que tiene a la enfermería como su mayor clase trabajadora.
