Hypertension is one of the primary modifiable risk factors for morbidity and mortality worldwide, being a major risk factor for coronary artery disease, stroke, and kidney failure. Furthermore, it is highly prevalent, affecting more than one-third of the global population.
Blood pressure measurement is a MANDATORY procedure in any medical care setting and is carried out by various healthcare professionals. However, it is still commonly performed without the necessary technical care. Since the diagnosis relies on blood pressure measurement, it is clear how important it is to handle the techniques, methods, and equipment used in its execution with care.
It should be emphasized that once the diagnosis is made, all short-term, medium-term, and long-term investigations and treatments are based on the results of blood pressure measurement. Therefore, improper techniques and/or equipment can lead to incorrect diagnoses, either underestimating or overestimating values, resulting in inappropriate actions and significant health and economic losses for individuals and nations.
Once the correct diagnosis is made, as knowledge of the importance of proper treatment advances, with the adoption of more detailed normal values and careful treatment objectives towards achieving stricter blood pressure goals, the importance of precision in blood pressure measurement is also reinforced.
Blood pressure measurement (described below) is usually performed using the traditional method, the so-called casual or office measurement. Over time, alternatives have been added to it, through the use of semi-automatic or automatic devices by the patients themselves, in waiting rooms or outside the office, in their own homes, or in public spaces. A step further was taken with the use of semi-automatic devices equipped with memory that allow sequential measurements outside the office (ABPM; or HBPM) and other automatic devices that allow programmed measurements over longer periods (HBPM).
Some aspects of blood pressure measurement can interfere with obtaining reliable results and, consequently, cause harm in decision-making. These include the importance of using average values, the variation in blood pressure during the day, and short-term variability. These aspects have encouraged the performance of a greater number of measurements in various situations, and different guidelines have advocated the use of equipment that promotes these actions. Devices that perform HBPM or ABPM, which, in addition to allowing greater precision, when used together, detect white coat hypertension (WCH), masked hypertension (MH), sleep blood pressure alterations, and resistant hypertension (RHT) (defined in Chapter 2 of this guideline), are gaining more and more importance.
Taking these details into account, we must emphasize that information related to diagnosis, classification, and goal setting is still based on office blood pressure measurement, and for this reason, all attention must be given to the proper execution of this procedure.
La hipertensión arterial (HTA) es uno de los principales factores de riesgo modificables para la morbilidad y mortalidad en todo el mundo, siendo uno de los mayores factores de riesgo para la enfermedad de las arterias coronarias, el accidente cerebrovascular (ACV) y la insuficiencia renal. Además, es altamente prevalente y afecta a más de un tercio de la población mundial.
La medición de la presión arterial (PA) es un procedimiento OBLIGATORIO en cualquier atención médica o realizado por diferentes profesionales de la salud. Sin embargo, todavía se realiza comúnmente sin los cuidados técnicos necesarios. Dado que el diagnóstico se basa en la medición de la PA, es claro el cuidado que debe haber con las técnicas, los métodos y los equipos utilizados en su realización.
Debemos enfatizar que una vez realizado el diagnóstico, todas las investigaciones y tratamientos a corto, mediano y largo plazo se basan en los resultados de la medición de la PA. Por lo tanto, las técnicas y/o equipos inadecuados pueden llevar a diagnósticos incorrectos, subestimando o sobreestimando valores y resultando en conductas inadecuadas y pérdidas significativas para la salud y la economía de las personas y las naciones.
Una vez realizado el diagnóstico correcto, a medida que avanza el conocimiento sobre la importancia del tratamiento adecuado, con la adopción de valores de normalidad más detallados y objetivos de tratamiento más cuidadosos hacia metas de PA más estrictas, también se refuerza la importancia de la precisión en la medición de la PA.
La medición de la PA (descrita a continuación) generalmente se realiza mediante el método tradicional, la llamada medición casual o de consultorio. Con el tiempo, se han agregado alternativas a través del uso de dispositivos semiautomáticos o automáticos por parte del propio paciente, en salas de espera o fuera del consultorio, en su propia residencia o en espacios públicos. Se dio un paso más con el uso de dispositivos semiautomáticos equipados con memoria que permiten mediciones secuenciales fuera del consultorio (AMPA; o MRPA) y otros automáticos que permiten mediciones programadas durante períodos más largos (MAPA).
Algunos aspectos en la medición de la PA pueden interferir en la obtención de resultados confiables y, en consecuencia, causar daños en las decisiones a tomar. Estos incluyen la importancia de usar valores promedio, la variación de la PA durante el día y la variabilidad a corto plazo. Estos aspectos han alentado la realización de un mayor número de mediciones en diversas situaciones, y diferentes pautas han abogado por el uso de equipos que promuevan estas acciones. Los dispositivos que realizan MRPA o MAPA, que además de permitir una mayor precisión, cuando se usan juntos, detectan la hipertensión de bata blanca (HBB), la hipertensión enmascarada (HM), las alteraciones de la PA durante el sueño y la hipertensión resistente (HR) (definida en el Capítulo 2 de esta guía), están ganando cada vez más importancia.
Teniendo en cuenta estos detalles, debemos enfatizar que la información relacionada con el diagnóstico, la clasificación y el establecimiento de objetivos todavía se basa en la medición de la presión arterial en el consultorio, y por esta razón, se debe prestar toda la atención a la ejecución adecuada de este procedimiento.
A hipertensão arterial (HA) é um dos principais fatores de risco modificáveis para morbidade e mortalidade em todo o mundo, sendo um dos maiores fatores de risco para doença arterial coronária, acidente vascular cerebral (AVC) e insuficiência renal. Além disso, é altamente prevalente e atinge mais de um terço da população mundial.
A medida da PA é procedimento OBRIGATÓRIO em qualquer atendimento médico ou realizado por diferentes profissionais de saúde. Contudo, ainda é comumente realizada sem os cuidados técnicos necessários. Como o diagnóstico se baseia na medida da PA, fica claro o cuidado que deve haver com as técnicas, os métodos e os equipamentos utilizados na sua realização.
Deve-se reforçar que, feito o diagnóstico, toda a investigação e os tratamentos de curto, médio e longo prazos são feitos com base nos resultados da medida da PA. Assim, técnicas e/ou equipamentos inadequados podem levar a diagnósticos incorretos, tanto subestimando quanto superestimando valores e levando a condutas inadequadas e grandes prejuízos à saúde e à economia das pessoas e das nações.
Uma vez feito o diagnóstico correto, na medida em que avança o conhecimento da importância do tratamento adequado, com a adoção de valores de normalidade mais detalhados e com objetivos de tratamento mais cuidadosos no sentido do alcance de metas de PA mais rigorosas, fica também reforçada a importância da precisão na medida da PA.
A medida da PA (descrita a seguir) é habitualmente feita pelo método tradicional, a assim chamada medida casual ou de consultório. Ao longo do tempo, foram agregadas alternativas a ela, mediante o uso de equipamentos semiautomáticos ou automáticos pelo próprio paciente, nas salas de espera ou fora do consultório, em sua própria residência ou em espaços públicos. Um passo adiante foi dado com o uso de equipamentos semiautomáticos providos de memória que permitem medidas sequenciais fora do consultório (AMPA; ou MRPA) e outros automáticos que permitem medidas programadas por períodos mais prolongados (MAPA).
Alguns aspectos na medida da PA podem interferir na obtenção de resultados fidedignos e, consequentemente, causar prejuízo nas condutas a serem tomadas. Entre eles, estão: a importância de serem utilizados valores médios, a variação da PA durante o dia e a variabilidade a curto prazo. Esses aspectos têm estimulado a realização de maior número de medidas em diversas situações, e as diferentes diretrizes têm preconizado o uso de equipamentos que favoreçam essas ações. Ganham cada vez mais espaço os equipamentos que realizam MRPA ou MAPA, que, além de permitirem maior precisão, se empregados em conjunto, detectam a HA do avental branco (HAB), HA mascarada (HM), alterações da PA no sono e HA resistente (HAR) (definidos no Capítulo 2 desta diretriz).
Resguardados esses detalhes, devemos ressaltar que as informações relacionadas a diagnóstico, classificação e estabelecimento de metas ainda são baseadas na medida da PA de consultório e, por esse motivo, toda a atenção deve ser dada à realização desse procedimento.

Abstract Hereditary transthyretin amyloidosis with peripheral neuropathy (ATTRv-PN) is an autosomal dominant inherited sensorimotor and autonomic polyneuropathy with over 130 pathogenic variants identified in the TTR gene. Hereditary transthyretin amyloidosis with peripheral neuropathy is a disabling, progressive and life-threatening genetic condition that leads to death in ~ 10 years if untreated. The prospects for ATTRv-PN have changed in the last decades, as it has become a treatable neuropathy. In addition to liver transplantation, initiated in 1990, there are now at least 3 drugs approved in many countries, including Brazil, and many more are being developed. The first Brazilian consensus on ATTRv-PN was held in the city of Fortaleza, Brazil, in June 2017. Given the new advances in the area over the last 5 years, the Peripheral Neuropathy Scientific Department of the Brazilian Academy of Neurology organized a second edition of the consensus. Each panelist was responsible for reviewing the literature and updating a section of the previous paper. Thereafter, the 18 panelists got together virtually after careful review of the draft, discussed each section of the text, and reached a consensus for the final version of the manuscript.
Resumo Polineuropatia amiloidótica familiar associada a transtirretina (ATTRv-PN) é uma polineuropatia sensitivo-motora e autonômica hereditária autossômica dominante com mais de 130 variantes patogênicas já identificadas no gene TTR. A ATTRv-PN é uma condição genética debilitante, progressiva e que ameaça a vida, levando à morte em ~ 10 anos se não for tratada. Nas últimas décadas, a ATTRv-PN se tornou uma neuropatia tratável. Além do transplante de fígado, iniciado em 1990, temos agora 3 medicamentos modificadores de doença aprovados em muitos países, incluindo o Brasil, e muitas outras medicações estão em desenvolvimento. O primeiro consenso brasileiro em ATTRv-PN foi realizado em Fortaleza em junho de 2017. Devido aos novos avanços nesta área nos últimos 5 anos, o Departamento Científico de Neuropatias Periféricas da Academia Brasileira de Neurologia organizou uma segunda edição do consenso. Cada panelista ficou responsável por rever a literatura e atualizar uma parte do manuscrito. Finalmente, os 18 panelistas se reuniram virtualmente após revisão da primeira versão, discutiram cada parte do artigo e chegaram a um consenso sobre a versão final do manuscrito.

Soil chemical attributes under combinations of organic fertilizing and water salinity

ABSTRACT The use of brackish water causes chemical changes in cultivated soils. It is therefore necessary to apply strategies that can minimize its negative impacts, such as the use of organic fertilizers. This study aimed to evaluate in the field the chemical attributes of a Red Yellow Argisol under irrigation with saline water and organic fertilizing in an area cultivated with corn. The design was randomized blocks, in a split-plot scheme, with the plots comprising salinity levels for the irrigation water (0.8 and 3.0 dS m-1) and the subplots combinations of organic fertilizers (cattle manure + poultry biofertilizer + goat biofertilizer; cattle manure + goat biofertilizer; cattle manure + poultry biofertilizer; control), with four replications. The 3.0 dS m-1 irrigation has a negative effect on the soil chemical attributes, particularly reducing the pH and increasing the sodium, exchangeable sodium percentage and electrical conductivity of the soil saturation extract. The combination of cattle manure + poultry biofertilizer + goat biofertilizer mitigates the salt stress by favouring the accumulation of nitrogen and organic matter, while the cattle manure + poultry biofertilizer favours the potassium accumulation in the soil. The combination of cattle manure + poultry biofertilizer + goat biofertilizer increases the contents of phosphorus and potassium when using the 0.8 dS m-1 irrigation.
RESUMO O uso de água salobra provoca alterações químicas em solos cultivados. Nesse sentido, é necessário aplicar estratégias que possam minimizar os seus impactos negativos, como, por exemplo, o uso de fertilizantes orgânicos. Objetivou-se avaliar em campo os atributos químicos de um Argissolo Vermelho Amarelo sob irrigação com água salina e adubação orgânica, em área cultivada com milho. O delineamento foi o de blocos ao acaso, em esquema de parcelas subdivididas, sendo as parcelas níveis de salinidade da água de irrigação (0,8 e 3,0 dS m-1) e as subparcelas combinações de aplicação de fontes de adubos orgânicos (esterco bovino + biofertilizante de aves + biofertilizante caprino; esterco bovino + biofertilizante caprino; esterco bovino + biofertilizante de aves; controle), com quatro repetições. A irrigação de 3,0 dS m-1 altera negativamente os atributos químicos do solo, especialmente com redução do pH e elevação de sódio, percentagem de sódio trocável e condutividade elétrica do extrato de saturação do solo. A combinação de esterco bovino + biofertilizante de aves + biofertilizante caprino mitiga o estresse salino, favorecendo o acúmulo de nitrogênio e matéria orgânica, enquanto o esterco bovino + biofertilizante de aves favorece o acúmulo de potássio no solo. A combinação de esterco bovino + biofertilizante de aves + biofertilizante caprino aumenta o conteúdo de fósforo e de potássio quando utilizada a irrigação de 0,8 dS m-1.

RESUMO Considerando o debate contemporâneo sobre as questões florestais, este artigo faz uma análise dos principais pontos comuns que foram destacados durante o Web-Seminário “Construindo Diálogos sobre Governança Florestal” e através dos artigos reunidos no Dossiê Governança florestal da revista Estudos Avançados do Instituto de Estudos Avançados da Universidade de São Paulo. Destacamos que a governança florestal se apresenta como um processo em constante evolução e deverá seguir promovendo a aproximação dos multiatores nos diversos níveis e escalas de decisão sobre as florestas e paisagens multifuncionais, e assim, fortalecer uma agenda de governança interdisciplinar e promotora de soluções com base nas melhores relações da sociedade com os sistemas socioecológicos.
ABSTRACT Considering the contemporary debate on forest issues, this article analyzes the main common points highlighted during the Webinar “Building Dialogues on Forest Governance” and in the articles of the Forest Governance Dossier of the Estudos Avançados journal of Institute of Advanced Studies of the University of São Paulo. We emphasize that forest governance is a process in constant evolution and should continue to help to bring together multiple stakeholders at different levels and scales of decision on forests and multifunctional landscapes, and thus, strengthen an interdisciplinary governance agenda that promotes solutions based on the best relations between society with socio-ecological systems.

ABSTRACT: The term rickets refers to insufficient or retarded mineralization of the osteoide matrix. X-linked hypophosphatemic (XLH) rickets is a rare genetic disorder characterized by biochemical changes in bone mineralization due to inactivation of the phosphate regulating gene and primary defect of the osteoblasts. The aim of this article was to report a clinical case of XLH, its oral manifestations, periapical changes and dental management. A 31-year old woman female patient was referred to the school of dentistry with pain and sensitivity in the teeth. She had a childhood history of rickets, hypophosphatemia and alteration in Vitamin D. In the oral exam, enamel hypoplasia, microdontia, fistula, caries and periapical lesions and periodontal disease were diagnosed. The radiographic and tomographic exams exhibited the presence of periapical lesions involving various teeth with radiolucent images, suggestive of granuloma or periapical cysts. The treatme nt prioritized the urgency of eliminating pain and removing the foci of infection. Endodontic treatment began in the teeth that had fistula or periapical lesions and in parallel, oral hygiene guidance was provided and periodontal treatment was performed. There was an improvement in the clinical condition with reduction in inflammation and mobility of the teeth. Dentists and health professionals must evaluate the patient as a whole, considering the relations between systemic and oral health. Knowledge of systemic diseases associated with rickets and their characteristics is essential for making a correct oral diagnosis and planning the dental treatment.
RESUMEN: El término raquitismo se refiere a la mineralización insuficiente o retardada de la matriz osteoide. El raquitismo hipofosfatémico ligado al cromosoma X (XLH) es un trastorno genético caracterizado por cambios bioquímicos en la mineralización ósea debido a la inactivación del gen regulador del fosfato y al defecto primario de los osteoblastos. El objetivo de este artículo fue reportar un caso clínico de XLH, sus manifestaciones orales, cambios periapicales y manejo dental. La paciente, una mujer de 31 años, acudió a la Clínica de Semiología de la UFPR con dolor y sensibilidad en varios dientes. Tenía antecedentes infantiles de raquitismo, hipofosfatemia y alteración de la vitamina D. En el examen oral se diagnosticó hipoplasia del esmalte, microdoontia, fístula, caries y lesiones periapicales y enfermedad periodontal. Los exámenes radiográficos y tomográficos mostraron la presencia de lesiones periapicales en varios dientes con imágenes radiolúcidas, sugestivas de granuloma o quistes periapicales. El tratamiento priorizó la urgencia de eliminar el dolor y remover los focos de infección. Se inició tratamiento de endodoncia en los dientes que presentaban fístula o lesiones periapicales y paralelamente se brindó orientación de higiene oral y se realizó tratamiento periodontal. Hubo una mejoría en la condición clínica con reducción de la inflamación y movilidad de los dientes. Los odontólogos y profesionales de la salud deben evaluar al paciente como un todo, teniendo en cuenta las relaciones entre salud sistémica y oral. El conocimiento de las enfermedades sistémicas asociadas al raquitismo y sus características es fundamental para realizar un correcto diagnóstico oral y planificar el tratamiento odontológico.

Loss of an eye to mucormycosis following corticosteroid therapy for COVID-19

ABSTRACT Mucormycosis is a rare, sometimes severe fungal infection that has emerged as a possible complication of COVID-19. We report a case of a non-diabetic, apparently immunocompetent patient diagnosed with rhino-orbital-cerebral mucormycosis shortly after COVID-19 treatment with dexamethasone. The patient received optimized systemic antifungal therapy and extensive surgical treatment. So far, four months after the last hospital discharge, the patient has been in good general condition. This case is a dramatic reminder that beneficial corticosteroid therapy in general inevitably carries a risk of opportunistic infection, and corticosteroid therapy for COVID-19 risks orbital-rhinocerebral mucormycosis that clinicians should watch for with vigilance.

Abstract: The osteolytic activity of odontogenic cysts and tumors is directly associated with their growth and aggressiveness. The influence of proteins expressed by epithelial and mesenchymal cells on this biological event differs between indolent cystic lesions, aggressive cystic lesions, and odontogenic tumors. The objective of this study was to compare the immunohistochemical expression of factors that stimulate (receptor activator of nuclear factor kappa-Β ligand – RANKL, cathepsin K – CatK and matrix metallopeptidase 8 – MMP-8) and inhibit (osteoprotegerin – OPG) osteoclastogenesis between dentigerous cyst (DC), glandular odontogenic cyst (GOC), odontogenic keratocyst (OKC), and ameloblastoma (AB). Paraffin-embedded sections of nine DCs, nine GOCs, 20 OKCs, 21 ABs, and four dental follicles (DFs) were subjected to immunohistochemistry. Immunoreactivity was analyzed semiquantitatively and quantitatively in epithelium and connective tissue, respectively. The proteins were immunoexpressed in epithelial and mesenchymal cells of all lesions studied. The expression of RANKL and CatK was higher in OKC, AB, and GOC (p<0.005). Higher expression of OPG was found in DF and DC compared to the other markers (p<0.005). MMP-8 expression was high in GOC and OKC. This study demonstrated the differential expression of factors that inhibit and stimulate bone resorption during the development of DC, GOC, OKC, and AB. Higher expression of RANKL and CatK was observed in more aggressive lesions. OPG appears to be one of the molecules responsible for the slower growth of DC.

RESUMO O DC de Neuroimunologia da ABN e o BCTRIMS trazem, nesse documento, as recomendações sobre vacinação da população com doenças desmielinizantes do sistema nervoso central (SNC) contra infecções em geral e contra o coronavírus da síndrome respiratória aguda grave 2 (SARS-CoV-2), causador da COVID-19. Destaca-se a gravidade do atual momento frente ao avanço da COVID-19 em nosso País, o que torna mais evidente e importante a criação de guia de referência para orientação aos médicos, pacientes e autoridades de saúde pública quanto à vacinação, meio efetivo e seguro no controle de determinadas doenças infecciosa. O DCNI/ABN e o BCTRIMS recomendam que os pacientes com doenças desmielinizantes do SNC (ex., EM e NMOSD) sejam constantemente monitorados, quanto a atualização do seu calendário vacinal, especialmente, no início ou antes da mudança do tratamento com uma droga modificadora de doença (DMD). É importante também salientar que as vacinas são seguras e os médicos devem estimular o seu uso em todos os pacientes. Evidentemente, deve ser dada especial atenção às vacinas com vírus vivos atenuados. Por fim, é importante que os médicos verifiquem qual DMD o paciente está em uso e quando foi feita a sua última dose, pois cada fármaco pode interagir de forma diferente com a indução da resposta imune.
ABSTRACT The Scientific Department of Neuroimmunology of the Brazilian Academy of Neurology (DCNI/ABN) and Brazilian Committee for Treatment and Research in Multiple Sclerosis and Neuroimmunological Diseases (BCTRIMS) provide recommendations in this document for vaccination of the population with demyelinating diseases of the central nervous system (CNS) against infections in general and against the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), which causes COVID-19. We emphasize the seriousness of the current situation in view of the spread of COVID-19 in our country. Therefore, reference guides on vaccination for clinicians, patients, and public health authorities are particularly important to prevent some infectious diseases. The DCNI/ABN and BCTRIMS recommend that patients with CNS demyelinating diseases (e.g., MS and NMOSD) be continually monitored for updates to their vaccination schedule, especially at the beginning or before a change in treatment with a disease modifying drug (DMD). It is also important to note that vaccines are safe, and physicians should encourage their use in all patients. Clearly, special care should be taken when live attenuated viruses are involved. Finally, it is important for physicians to verify which DMD the patient is receiving and when the last dose was taken, as each drug may affect the induction of immune response differently.

RESUMO Enquanto o Inventário Florestal Nacional Brasileiro (IFN) está em andamento, há uma demanda crescente para entender o efeito da área do conglomerado sobre a exatidão e precisão da estimativa de atributos florestais. O objetivo deste estudo foi determinar a área mínima de um conglomerado para estimar o volume comercial (VC) com a mesma acurácia e precisão que as estimativas derivadas do conglomerado original de 8.000 m². A base de dados é proveniente de um inventário realizado em uma unidade florestal (Floresta Nacional do Bom Futuro) no sudoeste da Amazônia brasileira, onde 22 conglomerados foram distribuídos em um desenho amostral em dois estágios. Foram avaliados três produtos: (i) VC de árvores com diâmetro à altura do peito (DAP) ≥ 20 cm (P1); (ii) VC de árvores com DAP ≥ 50 cm (P2); e (iii) VC de espécies comerciais com DAP ≥ 50 cm e qualidade de fuste ‘nível 1’ ou ‘nível 2’ (P3). O estudo avaliou dez cenários em que a área do conglomerado foi reduzida de 8.000 a 800 m². A acurácia de P1, P2 e P3 foi significativamente menor para reduções < 2.400 m². A precisão foi mais sensível à variação no tamanho do conglomerado, sobretudo para P2 e P3. Os tamanhos mínimos de conglomerado foram ≥ 2.400 m² para estimar P1, ≥ 4.800 m² para estimar P2 e ≥ 7.200 m² para estimar P3. Concluímos que é possível reduzir a área do conglomerado sem perder acurácia e precisão do conglomerado original do IFN. Um conglomerado de 2.400 m² fornece estimativas com a mesma acurácia que o conglomerado original, independentemente do produto avaliado.
ABSTRACT While the Brazilian National Forest Inventory (NFI) is in progress, there is a growing demand to understand the effect of cluster size on the accuracy and precision of forest-attribute estimation. We aimed to find the minimum cluster size (in area) to estimate merchantable volume (MV) with the same accuracy and precision as the estimates derived from the original cluster of 8,000 m2. We used data from an inventory carried out in a forest unit (Bom Futuro National Forest) in the southwestern Brazilian Amazon, where 22 clusters were distributed as a two-stage sampling design. Three products were evaluated: (i) MV of trees with a diameter at breast height (DBH) ≥ 20 cm (P1); (ii) MV of trees with DBH ≥ 50 cm (P2); and (iii) MV of commercial species with DBH ≥ 50 cm and stem quality ‘level 1’ or ‘level 2’ (P3). We assessed ten scenarios in which the cluster size was reduced from 8,000 m2 to 800 m2. The accuracy of P1, P2 and P3 was highly significantly lower for reductions < 2,400 m². The precision was more sensitive to variations in cluster size, especially for P2 and P3. Minimum cluster sizes were ≥ 2,400 m² to estimate P1, ≥ 4,800 m² to estimate P2, and ≥ 7,200 m² to estimate P3. We concluded that it is possible to reduce the cluster size without losing the accuracy and precision given by the original NFI cluster. A cluster of 2,400 m² provides estimates as accurate as the original cluster, regardless of the evaluated product.

OBJECTIVES: This observational, cross-sectional study based aimed to test whether heart failure (HF)-disease management program (DMP) components are influencing care and clinical decision-making in Brazil. METHODS: The survey respondents were cardiologists recommended by experts in the field and invited to participate in the survey via printed form or email. The survey consisted of 29 questions addressing site demographics, public versus private infrastructure, HF baseline data of patients, clinical management of HF, performance indicators, and perceptions about HF treatment. RESULTS: Data were obtained from 98 centers (58% public and 42% private practice) distributed across Brazil. Public HF-DMPs compared to private HF-DMP were associated with a higher percentage of HF-DMP-dedicated services (79% vs 24%; OR: 12, 95% CI: 94-34), multidisciplinary HF (MHF)-DMP [84% vs 65%; OR: 3; 95% CI: 1-8), HF educational programs (49% vs 18%; OR: 4; 95% CI: 1-2), written instructions before hospital discharge (83% vs 76%; OR: 1; 95% CI: 0-5), rehabilitation (69% vs 39%; OR: 3; 95% CI: 1-9), monitoring (44% vs 29%; OR: 2; 95% CI: 1-5), guideline-directed medical therapy-HF use (94% vs 85%; OR: 3; 95% CI: 0-15), and less B-type natriuretic peptide (BNP) dosage (73% vs 88%; OR: 3; 95% CI: 1-9), and key performance indicators (37% vs 60%; OR: 3; 95% CI: 1-7). In comparison to non- MHF-DMP, MHF-DMP was associated with more educational initiatives (42% vs 6%; OR: 12; 95% CI: 1-97), written instructions (83% vs 68%; OR: 2: 95% CI: 1-7), rehabilitation (69% vs 17%; OR: 11; 95% CI: 3-44), monitoring (47% vs 6%; OR: 14; 95% CI: 2-115), GDMT-HF (92% vs 83%; OR: 3; 95% CI: 0-15). In addition, there were less use of BNP as a biomarker (70% vs 84%; OR: 2; 95% CI: 1-8) and key performance indicators (35% vs 51%; OR: 2; 95% CI: 91,6) in the non-MHF group. Physicians considered changing or introducing new medications mostly when patients were hospitalized or when observing worsening disease and/or symptoms. Adherence to drug treatment and non-drug treatment factors were the greatest medical problems associated with HF treatment. CONCLUSION: HF-DMPs are highly heterogeneous. New strategies for HF care should consider the present study highlights and clinical decision-making processes to improve HF patient care.

Abstract The Program for Biodiversity Research (PPBio) is an innovative program designed to integrate all biodiversity research stakeholders. Operating since 2004, it has installed long-term ecological research sites throughout Brazil and its logic has been applied in some other southern-hemisphere countries. The program supports all aspects of research necessary to understand biodiversity and the processes that affect it. There are presently 161 sampling sites (see some of them at Supplementary Appendix), most of which use a standardized methodology that allows comparisons across biomes and through time. To date, there are about 1200 publications associated with PPBio that cover topics ranging from natural history to genetics and species distributions. Most of the field data and metadata are available through PPBio web sites or DataONE. Metadata is available for researchers that intend to explore the different faces of Brazilian biodiversity spatio-temporal variation, as well as for managers intending to improve conservation strategies. The Program also fostered, directly and indirectly, local technical capacity building, and supported the training of hundreds of undergraduate and graduate students. The main challenge is maintaining the long-term funding necessary to understand biodiversity patterns and processes under pressure from global environmental changes.

RESUMO O cultivo de mudas de espécies com raízes tuberosas como Moringa oleifera Lam. (moringa), requer recipientes bem dimensionados e o uso de substratos adequados, considerando que a muda será retirada do recipiente antes do plantio. Bagaço de cana, composto de lixo urbano (composto) e vermicomposto são resíduos promissores na composição de substratos. O trabalho teve como objetivo avaliar a qualidade das mudas de moringa cultivadas em substratos produzidos a partir de bagaço de cana associado com composto ou vermicomposto em tubetes de diferentes volumes. O delineamento experimental foi em blocos casualizados, em esquema fatorial 2x7, sendo: volumes dos tubetes (50 e 240 mL) x sete substratos (substrato comercial; bagaço de cana misturado a composto de lixo urbano em tres diferentes proporções (1:3; 1:1 e 3:1) e bagaço de cana misturado a vermicomposto nas proporções 1:3; 1:1 e 3:1). Concluiu-se que o recipiente de 240 mL foi o mais adequado para a produção de mudas de moringa. Substratos com maiores proporções de composto orgânico resultaram em maior crescimento da parte aérea e das raízes tuberosas, conteúdo e eficiência do uso de nitrogênio, sendo equivalente ao substrato comercial. Maiores proporções de bagaço de cana na composição do substrato proporcionaram maior facilidade de retirada das mudas dos tubetes e melhor qualidade física do torrão após a retirada da muda do tubete.
ABSTRACT The cultivation of tuberous-root species such as Moringa oleifera Lam. (moringa) requires well-dimensioned containers and the use of appropriate substrates, since seedlings will be removed from the container before their planting. Sugarcane bagasse, urban waste compost (compost), and vermicompost are promising wastes for substrate composition. The present study aims to assess the quality of moringa grown in substrate produced from sugarcane bagasse associated with compost or vermicompost in different-volume tubes. The study followed a randomized blocks design, at 2x7 factorial arrangement, namely: tubes’ volumes (50 and 240 mL) x seven substrates (commercial substrate; sugarcane bagasse associated with urban waste compost at three different ratios (1:3; 1:1 and 3:1) and sugarcane bagasse associated with vermicompost at ratios 1:3; 1:1 and 3:1). In conclusion, the 240 mL container was the most appropriate one for moringa seedlings’ production. Substrates presenting higher organic compost ratios led to greater shoot and tuberous root growth and to greater nitrogen-use accumulation and efficiency, which was equivalent to that of the commercial substrate. Higher sugarcane bagasse rates in substrate composition made it easier to remove the seedlings from the tubes and led to better physical quality of the clod after seedling removal from the tubes.

ABSTRACT Currently, the Milwaukee protocol presents healing results in human beings affected by the rabies virus. However, there are many points to clarify on the action of drugs and the immune mechanism involved in the evolution of the disease. One of the drugs used is biopterin, which is an important cofactor for nitric oxide, important for preventing vasospasm. Thus, we describe the effect of biopterin on some inflammatory factors in a rabies virus infection developed in an animal model. The immunological mediators studied in animals infected with rabies virus submitted to doses of sapropterin were Anti-RABV, IL-6, IL-2, IL-17a, INF-gamma and Anti-iNOS. It is suggested that the medication in the context of a RABV infection already installed, had the effect of modulating the inflammatory mechanisms mainly linked to the permeability of the blood-brain barrier and the migration of cytotoxic cells.
