ABSTRACT Objective: To describe the IMPACTO-MR, a Brazilian nationwide intensive care unit platform study focused on the impact of health care-associated infections due to multidrug-resistant bacteria. Methods: We described the IMPACTO-MR platform, its development, criteria for intensive care unit selection, characterization of core data collection, objectives, and future research projects to be held within the platform. Results: The core data were collected using the Epimed Monitor System® and consisted of demographic data, comorbidity data, functional status, clinical scores, admission diagnosis and secondary diagnoses, laboratory, clinical, and microbiological data, and organ support during intensive care unit stay, among others. From October 2019 to December 2020, 33,983 patients from 51 intensive care units were included in the core database. Conclusion: The IMPACTO-MR platform is a nationwide Brazilian intensive care unit clinical database focused on researching the impact of health care-associated infections due to multidrug-resistant bacteria. This platform provides data for individual intensive care unit development and research and multicenter observational and prospective trials.
RESUMO Objetivo: Descrever o IMPACTO-MR, um estudo brasileiro de plataforma nacional em unidades de terapia intensiva focado no impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Métodos: Descrevemos a plataforma IMPACTO-MR, seu desenvolvimento, critérios para seleção das unidades de terapia intensiva, caracterização da coleta de dados, objetivos e projetos de pesquisa futuros a serem realizados na plataforma. Resultados: Os dados principais foram coletados por meio do Epimed Monitor System® e consistiram em dados demográficos, dados de comorbidades, estado funcional, escores clínicos, diagnóstico de internação e diagnósticos secundários, dados laboratoriais, clínicos e microbiológicos e suporte de órgãos durante a internação na unidade de terapia intensiva, entre outros. De outubro de 2019 a dezembro de 2020, 33.983 pacientes de 51 unidades de terapia intensiva foram incluídos no banco de dados principal. Conclusão: A plataforma IMPACTO-MR é um banco de dados clínico brasileiro de unidades de terapia intensiva focado na pesquisa do impacto das infecções por bactérias multirresistentes relacionadas à assistência à saúde. Essa plataforma fornece dados para o desenvolvimento e pesquisa de unidades de terapia intensiva individuais e ensaios clínicos observacionais e prospectivos multicêntricos.

Characterization of the virulence, agr typing and antimicrobial resistance profile of Staphylococcus aureus strains isolated from food handlers in Brazil

Abstract Staphylococcus aureus is one of the main pathogens associated with foodborne outbreaks in Brazil and food handlers can carry toxigenic and resistant S. aureus strains. The aims of this study were to verify the frequency of virulence genes, to identify the agr groups and to determine the antimicrobial resistance profile of S. aureus strains isolated from food handlers of pilot kitchens located in São Paulo, Brazil. A total of 74 strains of the Staphylococcus genus were detected and 50% were identified as of the species S. aureus. The enterotoxin genes detection, tst and luk-PV detection, agr typing, mecA detection, ccr complex detection and SCCmec typing were performed using PCR. The antimicrobial resistance testing was performed by the disk diffusion method. The enterotoxin genes were identified in 36 S. aureus, including sea (83.8%). The tst gene was detected in 18.92% of the strains and the luk-PV was detected in only one isolate. Agr typing classified 58.3% of the strains as type I. Seven (18.92%) strains were classified as MRSA and the ccr2 complex was detected in six of these isolates. The SCCmec typing characterized strains as type II, III, IV and V. Moreover, there were also a greater number of resistant strains to penicillin (83.78%) and clarithromycin (67.57%). In conclusion, the study revealed a significant prevalence of S. aureus, and the presence of different virulence genes and a worrying resistance profile in S. aureus strains isolated from food handlers in this country.

RESUMO Objetivo: Descrever as práticas de ressuscitação volêmica em unidades de terapia intensiva brasileiras e compará-las com as de outros países participantes do estudo Fluid-TRIPS. Métodos: Este foi um estudo observacional transversal, prospectivo e internacional, de uma amostra de conveniência de unidades de terapia intensiva de 27 países (inclusive o Brasil), com utilização da base de dados Fluid-TRIPS compilada em 2014. Descrevemos os padrões de ressuscitação volêmica utilizados no Brasil em comparação com os de outros países e identificamos os fatores associados com a escolha dos fluidos. Resultados: No dia do estudo, foram incluídos 3.214 pacientes do Brasil e 3.493 pacientes de outros países, dos quais, respectivamente, 16,1% e 26,8% (p < 0,001) receberam fluidos. A principal indicação para ressuscitação volêmica foi comprometimento da perfusão e/ou baixo débito cardíaco (Brasil 71,7% versus outros países 56,4%; p < 0,001). No Brasil, a percentagem de pacientes que receberam soluções cristaloides foi mais elevada (97,7% versus 76,8%; p < 0,001), e solução de cloreto de sódio a 0,9% foi o cristaloide mais comumente utilizado (62,5% versus 27,1%; p < 0,001). A análise multivariada sugeriu que os níveis de albumina se associaram com o uso tanto de cristaloides quanto de coloides, enquanto o tipo de prescritor dos fluidos se associou apenas com o uso de cristaloides. Conclusão: Nossos resultados sugerem que cristaloides são usados mais frequentemente do que coloides para ressuscitação no Brasil, e essa discrepância, em termos de frequências, é mais elevada do que em outros países. A solução de cloreto de sódio 0,9% foi o cristaloide mais frequentemente prescrito. Os níveis de albumina sérica e o tipo de prescritor de fluidos foram os fatores associados com a escolha de cristaloides ou coloides para a prescrição de fluidos.
Abstract Objective: To describe fluid resuscitation practices in Brazilian intensive care units and to compare them with those of other countries participating in the Fluid-TRIPS. Methods: This was a prospective, international, cross-sectional, observational study in a convenience sample of intensive care units in 27 countries (including Brazil) using the Fluid-TRIPS database compiled in 2014. We described the patterns of fluid resuscitation use in Brazil compared with those in other countries and identified the factors associated with fluid choice. Results: On the study day, 3,214 patients in Brazil and 3,493 patients in other countries were included, of whom 16.1% and 26.8% (p < 0.001) received fluids, respectively. The main indication for fluid resuscitation was impaired perfusion and/or low cardiac output (Brazil: 71.7% versus other countries: 56.4%, p < 0.001). In Brazil, the percentage of patients receiving crystalloid solutions was higher (97.7% versus 76.8%, p < 0.001), and 0.9% sodium chloride was the most commonly used crystalloid (62.5% versus 27.1%, p < 0.001). The multivariable analysis suggested that the albumin levels were associated with the use of both crystalloids and colloids, whereas the type of fluid prescriber was associated with crystalloid use only. Conclusion: Our results suggest that crystalloids are more frequently used than colloids for fluid resuscitation in Brazil, and this discrepancy in frequencies is higher than that in other countries. Sodium chloride (0.9%) was the crystalloid most commonly prescribed. Serum albumin levels and the type of fluid prescriber were the factors associated with the choice of crystalloids or colloids for fluid resuscitation.

Conyza bonariensis growth and development according to thermal time accumulation and photoperiod

Resumo: O objetivo deste trabalho foi caracterizar o crescimento e o desenvolvimento da buva (Conyza bonariensis) em razão do acúmulo térmico e do fotoperíodo, em diferentes épocas de semeadura, e propor uma escala para representar os principais estádios de desenvolvimento das plantas. O experimento foi realizado com dez repetições, no período de cultivo 2011/2012. As datas de semeaduras foram: 31/05/2011, 04/07/2011, 03/08/2011, 09/09/2011 e 07/11/2011. A altura de planta (cm) e a fenologia foram avaliadas semanalmente. A duração dos diferentes estádios (dias) e a soma térmica acumulada (°C dia) foram determinadas. A regressão linear mostrou que a altura de plantas relacionou-se à soma térmica acumulada. Independentemente da data de semeadura, a fase vegetativa teve maior duração (em dias e em ºC dia) do que a fase reprodutiva. A semeadura em 07/11/2011 promoveu o encurtamento da fase vegetativa, e o estádio roseta não ocorreu. Houve indução do florescimento em fotoperíodo entre 12,5 e 13,5 horas de luz, independentemente da época de semeadura. Observou-se crescimento mais lento na condição de menor temperatura, em que as plantas acumularam 30,9 e 16,3 °C dia por centímetro de altura, nas semeaduras em 31/05/2011 e 07/11/2011, respectivamente. A escala fenológica prevê adequadamente as fases de desenvolvimento da buva.
Abstract: The objective of this work was to characterize the growth and development of hairy fleabane (Conyza bonariensis) according to thermal accumulation and photoperiod, at different sowing times, and to propose a scale representing the main plant development stages. The experiment was carried out with ten replicates in the 2011/2012 growing season. The sowing dates were: 05/31/2011, 07/04/2011, 08/03/2011, 09/09/2011, and 11/07/2011. Plant height (cm) and phenology were evaluated weekly. The duration of the different stages (days) and thermal time accumulation (°C day) were determined. The linear regression analysis showed that plant height was related to thermal time accumulation. Regardless of the sowing date, the vegetative stage had a longer duration (in days and in ºC day) than the reproductive stage. Sowing on 11/07/2011 promoted the shortening of the vegetative stage, and the rosette stage did not occur. Flowering was induced in the photoperiod between 12.5 and 13.5 hours of light, regardless of the sowing date. Slow growth was observed at lower temperature conditions, when plants accumulated 30.9 and 16.3°C day per centimeter of height for the 05/31/2011 and 11/07/2011 sowing dates, respectively. The phenology scale adequately predicts the development stages of hairy fleabane.

Resumo: O objetivo deste trabalho foi modelar a competição de arroz-daninho (Oryza sativa) e capim-arroz (Echinochloa spp.) com o arroz irrigado por inundação e introduzi-la como um submodelo no modelo SimulArroz. A competição das duas ervas daninhas com o arroz irrigado foi modelada com o uso da equação da hipérbole retangular. Os coeficientes “i” e “a” da hipérbole retangular referentes a cada erva foram obtidos na literatura e em experimentos de campo conduzidos no Rio Grande do Sul. No SimulArroz, a perda de produtividade foi aplicada para penalizar a produtividade nos três níveis tecnológicos (alto, médio e baixo) para maturidade fisiológica. Para o arroz-daninho, os valores do coeficiente imédio, para os níveis tecnológicos alto, médio e baixo, foram 1,04, 1,50 e 3,57, respectivamente, e, para o capim-arroz, foram 4,70, 10,49 e 15,51, respectivamente. O coeficiente “a” foi 100 para o arroz-daninho, e o amédio, para o capim-arroz, foi 101,63, 104,92, 96,88 para os níveis tecnológicos alto, médio e baixo, respectivamente. A abordagem de perda de produtividade foi adequada para modelar a competição de arroz-daninho e capim-arroz com o arroz irrigado. O submodelo de perda de produtividade pela competição de arroz-daninho e capim-arroz com o arroz irrigado melhora a capacidade preditiva do modelo SimulArroz.
Abstract: The objective of this work was to model weedy rice (Oryza sativa) and barnyard grass (Echinochloa spp.) competition with flood-irrigated rice, introducing it as a submodule into the SimulArroz model. The competition of both weeds with irrigated rice was modeled using the rectangular hyperbola equation. The “i” and “a” coefficients of the rectangular hyperbola for each of these weeds were obtained from the literature and from field experiments carried out in the state of Rio Grande do Sul, Brazil. In SimulArroz, yield loss was applied to penalize yield in all three technological levels (high, medium, and low) of physiological maturity. For weedy rice, the coefficient values of imean for the high, medium, and low technological levels were 1.04, 1.50, and 3.57 respectively, and, for barnyard grass, 4.70, 10.49, and 15.51 respectively. Coefficient “a” was 100 for weedy rice, and amean values for barnyard grass were 101.63, 104.92, and 96.88 for the high, medium, and low levels, respectively. The yield loss approach was suitable to model the competition of weedy rice and barnyard grass with irrigated rice. The submodule yield loss caused by the competition of weedy rice and barnyard grass with irrigated rice improves the predictive capacity of the SimulArroz model.

O objetivo deste trabalho foi determinar a taxa de acúmulo de matéria seca e a duração do período de enchimento de grãos de cultivares de arroz irrigado com diferentes grupos de maturação e épocas de semeadura. Para a determinação da taxa e duração do período de enchimento de grãos, a partir do florescimento foram realizadas amostragens por cultivar. Foi ajustada a relação entre dias após florescimento e peso de grão. Os resultados indicaram que a antecipação da época de semeadura promoveu um atraso no período de ocorrência da maior taxa de enchimento de grãos durante o ciclo de desenvolvimento. As cultivares de ciclo tardio tiveram uma maior duração do período de enchimento de grãos (31 dias), em relação as cultivares de ciclo precoce e médio. As cultivares com maiores taxas de enchimento de grãos apresentaram maior peso em R9, menor número de grãos por panícula e menor duração do período de enchimento de grãos. Houve variação na taxa de enchimento de grãos e na duração do período de enchimento de grãos de acordo com o grupo de maturação, em que as cultivares de ciclo tardio apresentaram menor taxa e maior duração no período de enchimento de grão.
The objective of this work was to determine the rate of accumulation of dry matter and the duration of the grain filling period of irrigated rice cultivars with different maturation groups and sowing dates. To determinate the rate and duration of the grain filling period, samples per cultivar were taken. The relation was adjusted between days after flowering and grain weight. The results indicated that the anticipation of the sowing date promoted a delay in the period of occurrence of the highest rate of grain filling. The late cycle cultivars had a longer duration of the grain filling period (31 days), in relation to the cultivars of early and medium cycle. Then, the cultivars with higher grain filling rates showed higher grain weight in R9, lower number of grain per panicle and shorter grain filling period. Thus, it was possible to conclude that there was a variation in the grain filling rate and the grain filling period of irrigated rice cultivars with different maturation groups, which in the late cycle cultivars had a lower rate and a longer duration in the grain filling period. Then, it was identified that the grain weight is associated with the grain filling rate.

RESUMO: Este trabalho teve por objetivo avaliar a fitossociologia de plantas daninhas em arroz irrigado em diferentes sistemas de manejo do solo e intensidade de rotação de culturas. Para tanto, foram conduzidos dois estudos de campo. O primeiro estudo foi realizado em área cultivada desde 1994 com três sistemas de cultivo: plantio direto, pré-germinado e sistema convencional. O segundo estudo foi realizado em área experimental sob cinco Sistemas Integrados de Produção Agropecuária, com rotação de culturas. As avaliações fitossociológicas foram realizadas quando o arroz estava em pleno florescimento no primeiro estudo, e no final do enchimento de grãos no segundo. O sistema pré-germinado de cultivo de arroz apresenta predominância de plantas daninhas aquáticas. O sistema de cultivo convencional, quando comparado ao plantio direto, possui menores densidades de plantas daninhas. Nos Sistemas Integrados de Produção Agropecuária com maior intensidade de rotação de culturas houve maior infestação de plantas daninhas perenes.
ABSTRACT: This research aimed to evaluate the phytosociology of weeds in irrigated rice in different soil management systems and crop rotation intensity. Therefore, two field studies were carried out. Study 1 was conducted in an area that has been cultivated since 1994 with three cultivation systems: direct, pre-germinated and conventional ones. Study 2 was carried out in an experimental area in five Integrated Farming Systems, with crop rotation. Phytosociological evaluations were conducted when rice was fully blooming, in the first study, and when grain filling was ending, in the second study. Pre-germinated system of rice cultivation has predominance of aquatic weeds. Conventional system when compared with direct sowing has lower weed densities. Integrated Agricultural Production Systems with higher intensity of crop rotation have been higher infested of perennial species.

ABSTRACT In 2012, the first-generation protease inhibitors telaprevir (TVR) and boceprevir (BOC) were introduced in the Brazilian health system for treatment of chronic hepatitis C, after their approval by the National Committee for Health Technology Incorporation (CONITEC). However, these medicines were discontinued in 2015. The short period of use in therapy and their high cost require a discussion about the consequences for patients and for the health system of the early incorporation of new therapies. The article presents a qualitative analysis of the incorporation process of both medications in Brazil and the results of a multicenter study that included patients treated with BOC or TVR between January 2011 and December 2015 in five Brazilian cities. The study included 855 patients (BOC: n=247) and (TVR: n=608). The document analysis showed that CONITEC’s decision to incorporate BOC and TVR was based on results of phase III clinical trials that compared sustained virologic response (SVR) rates of patients treated with BOC and TVR with rates of those that received placebo. However, these studies included a low percentage of cirrhotic patients. The SVR rates observed in this multicenter study were worse than clinical trials pointed out (BOC: 45.6%; TVR: 51.8%), but similar to those achieved with previously adopted therapies. The discontinuation rate due to adverse events was (BOC: 15.4%; TVR: 12.7%). Based on these unsatisfactory results, the study brings a discussion that goes beyond the therapy outcomes, exploring the incorporation of these high-cost medicines and the related decision-making process, contributing to future decisions in medicine policies and in the treatment of chronic hepatitis C.

O objetivo do estudo foi avaliar a taxa de resposta viral sustentada (RVS) e as características clínicas e do tratamento dos pacientes portadores de hepatite C crônica. Realizou-se uma coorte retrospectiva não controlada com recorte temporal dos anos de 2005 a 2008, dos pacientes atendidos no Polo de Aplicação e Monitoramento de Medicamentos Injetáveis, em Florianópolis, SC. Os critérios de inclusão foram: pacientes maiores de 18 anos, com diagnóstico confirmado de hepatite C crônica de acordo com o protocolo brasileiro, tratados com PEG-IFN alfa-2a ou 2b associado a ribavirina. Total de 188 pacientes foi incluído no estudo, 70% homens, 59% genótipo 1, 27% co-infectados com o HIV e 31% apresentando cirrose. A taxa de RVS calculada através da teoria das probabilidades foi de 26% (max=57,4% and min=12,8%) e por intenção de tratamento de 12,8%. Verificou-se a associação da RVS com as variáveis: sexo (p=0,017), idade (p=0,003), genótipo (p=0,648) e presença de cirrose (p=0,275). Somente sexo e idade foram associados significativamente com a RVS. A taxa de RVS foi considerada baixa e, em parte, pode ser explicada pelas características desfavoráveis dos pacientes para a obtenção de RVS.
This study aimed to evaluate the rate of sustained virological response (SVR) and the clinical and treatment characteristics of patients with chronic hepatitis C (CHC). A retrospective uncontrolled cohort study was conducted among patients who received treatment for CHC between 2005 and 2008 attended at the Center for the Application and Monitoring of Injectable Medications, in Florianopolis, SC, Brazil. The inclusion criteria were: patients over 18 years of age, with a confirmed diagnosis of chronic hepatitis C according to Brazilian guidelines, treated with PEG-IFN alfa-2a or 2b associated with RBV. A total of 188 patients were included in the study: 70% men, 59% genotype 1, 27% coinfected with HIV, 31% with cirrhosis. The SVR rate, calculated by probability theory, was determined as 26% (max=57.4% and min=12.8%) and the intention to treat was 12.8%. Associations between Sustained Virological Response (SVR) and the variables sex (p=0.017), age (p=0.003), genotype (p=0.648) and cirrhosis (p=0.275), were determined in the bivariate analysis and only sex and age were significantly associated with SVR. The SVR rate was considered low, which can be partially explained by patients' unfavorable pretreatment characteristics.

O bom calibre e a coloração adequada dos frutos cítricos para consumo in natura são importantes fatores de qualidade que elevam o seu consumo e melhoram os preços do produto. Este trabalho teve como objetivo avaliar o efeito de diferentes intensidades de raleio manual dos frutos e da poda sobre a produção e qualidade dos frutos da tangerina Montenegrina. O experimento foi realizado em pomar comercial no município de Dois Vizinhos, no Sudoeste do Paraná. O experimento bifatorial (raleio x poda) constituiu-se dos seguintes tratamentos. 1) sem raleio e sem poda (testemunha); 2) sem raleio, com poda; 3) 33% de raleio, sem poda; 4) 33% de raleio, com poda; 5) 66% de raleio, sem poda; e 6) 66% de raleio, com poda. O delineamento experimental foi de blocos completamente casualizados, com quatro repetições, sendo cada unidade experimental constituída por uma planta. Nas condições em que o experimento foi realizado, concluiu-se que o raleio nas intensidades de 33 e 66% promove aumento da massa fresca, melhora a cor da epiderme, aumenta o diâmetro e melhora a classificação comercial dos frutos da tangerina Montenegrina. A poda leve, de abertura e levantamento da base da copa, não contribui para a melhoria da qualidade dos frutos.
The good size and adequate color of fresh citrus are important factors of quality that increase their consumption and improve the prices received by farmers. This work aimed to study the effect of different intensities of thinning and pruning of fruit on production and fruit quality of mandarin 'Montenegrina'. The experiment was conducted in a commercial orchard in the city of Dois Vizinhos, in Southwestern Paraná, Brazil. The factorial experiment consisted of the following treatments. 1) Without thinning and without pruning (control), 2) Without thinning, with pruning; 3) 33 % thinning, without pruning; 4) 33 % thinning, with pruning; 5) 66,% thinning, without pruning; 6) 66 % thinning, with pruning. The experimental design was completely randomized block with four replications and each experimental unit consists of one plant. Under conditions in which the experiment was conducted, it appears that the thinning intensities of 33 % and 66 % promote an increase in the fresh weight, improve the skin color, increase the diameter and improve the commercial classification of fruis of 'Montenegrina' mandarin. Light pruning does not contribute to improving the quality of the fruits.
