Giant Perianal Condyloma Successfully Treated with Podophyllin at 25% in Solid Petrolatum

Abstract Purpose Giant perianal condyloma (GPC) is a rare condition. The effective treatment is a multidisciplinary challenge; topical treatments are usually ineffective, and surgical resection has significant morbidity. Podophyllin at 25% in solid petrolatum (25%PSP) can be an effective treatment option for GPC. The aim of the present study was to assess its response and tolerability. Methods This retrospective, single-center case series evaluated the clinical response of 14 patients with GPC treated with 25%PSP in a public hospital in Buenos Aires between December 2015 and December 2019. After obtaining a full history and performing a physical exam, the lesions were measured and photographed. Biopsies were performed to exclude malignancy, as well as exams to rule out pregnancy. Podophyllin at 25% in solid petrolatum was administered topically in cases of GPC and washed off by the patients at home after 4 hours. The patients underwent at least 4 weekly visits, which included interval history, photodocumentation of the lesions, and provider-applied 25%PSP. The response rate was assessed by comparingmeasurements and the overall decrease in volume of the GPC based on photos from the first and last sessions. Adverse outcomes were noted. Results In total, 10 men, 3 women, and 1 transgender woman with GPC unresponsive to prior treatments and a mean age of 34.5 years were included. A total of 12 patients were immunosuppressed. All the perianal lesions were circumferential and measured between 8 cm and 20 cm. Overall, 7 patients had genital condyloma outside of the anus and perianus; the histology showed low-grade squamous intraepithelial lesions in all cases. While on treatment, 7 patients reported dermatitis, and 71% of the patients had 75% reduction in lesion size. Conclusions Podophyllin at 25% in solid petrolatum is an effective, well-tolerated topical treatment option for GPC.

Abstract Objective: To evaluate prevalence, clinical manifestations, laboratory abnormalities and treatment in a multicenter cohort study including 847 childhood-onset systemic lupus erythematosus (cSLE) patients with and without diffuse alveolar hemorrhage (DAH), as well as concomitant parameters of severity. Methods: DAH was defined as the presence of at least three respiratory symptoms/signs associated with diffuse interstitial/alveolar infiltrates on chest x-ray or high-resolution computer tomography and sudden drop in hemoglobin levels. Statistical analysis was performed using Bonferroni correction (p < 0.0022). Results: DAH was observed in 19/847 (2.2%) cSLE patients. Cough/dyspnea/tachycardia/hypoxemia occurred in all cSLE patients with DAH. Concomitant parameters of severity observed were: mechanical ventilation in 14/19 (74%), hemoptysis 12/19 (63%), macrophage activation syndrome 2/19 (10%) and death 9/19 (47%). Further analysis of cSLE patients at DAH diagnosis compared to 76 cSLE control patients without DAH with same disease duration [3 (1–151) vs. 4 (1–151) months, p = 0.335], showed higher frequencies of constitutional involvement (74% vs. 10%, p < 0.0001), serositis (63% vs. 6%, p < 0.0001) and sepsis (53% vs. 9%, p < 0.0001) in the DAH group. The median of disease activity score(SLEDAI-2 K) was significantly higher in cSLE patients with DAH [18 (5–40) vs. 6 (0–44), p < 0.0001]. The frequencies of thrombocytopenia (53% vs. 12%, p < 0.0001), intravenous methylprednisolone (95% vs. 16%, p < 0.0001) and intravenous cyclophosphamide (47% vs. 8%, p < 0.0001) were also significantly higher in DAH patients. Conclusions: This was the first study to demonstrate that DAH, although not a disease activity score descriptor, occurred in the context of significant moderate/severe cSLE flare. Importantly, we identified that this condition was associated with serious disease flare complicated by sepsis with high mortality rate.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

Water quality and diversity of yeasts from tropical lakes and rivers from the Rio Doce basin in Southeastern Brazil

Yeast communities were assessed in 14 rivers and four lakes from the Doce River basin in Brazil, during the rainy and dry seasons of the years 2000 and 2001. Water samples were collected at the subsurface in all sites. The following physical and chemical parameters were measured: temperature, dissolved oxygen, pH, electrical conductivity, total phosphorus, ortho-phosphate, ammonium, nitrate, nitrite and total nitrogen and the counts of faecal coliforms and heterotrophic bacteria were carried out to characterize the aquatic environmental sampled. The yeast counts were higher in aquatic environments with the highest counts of coliform and heterotrophic bacteria. These environments receive a high influx of domestic and industrial waste. A total of 317 isolates identified in forty eight yeast species were recorded in the sites sampled and the specie Aureobasidium pullulans were found in eleven out of eighteen sites sampled and some opportunistic pathogens such as the yeast species Candida krusei were isolated only in the polluted rivers with a positive correlation with the biotic and abiotic parameters that indicate sewage contamination.

O potencial de Trametes villosa e Pycnoporus sanguineus de descolorir corantes têxteis reativos utilizados na manufatura de algodão no estado de Minas Gerais foi avaliado. Halos de crescimento e descoloração foram determinados em agar extrato malte (MEA) com 0,002g L-1 do corante. T. villosa descoloriu os 28 corantes testados e P. sanguineus apenas 9. A influência de condições de cultivo (agitação, concentração de corante e concentração de nitrogênio) na degradação do corante azul brilhante Drimaren foi avaliada durante crescimento dos fungos em meio líquido sintético. Agitação favoreceu a degradação e não foi observada repressão da descoloração pelo nitrogênio. Em cultura pura, T. villosa e P. sanguineus descoloriram efluente sintético constituído por uma mistura de dez corantes. Maior descoloração do efluente sintético foi observada no cultivo misto destes dois fungos. Este estudo evidenciou diferenças entre espécies de basidiomicetos tropicais na capacidade em degradar corantes reativos e reforçou o potencial deste grupo de fungos para a descoloração de efluentes têxteis.
The potential of Trametes villosa and Pycnoporus sanguineus to decolorize reactive textile dyes used for cotton manufacturing in the State of Minas Gerais, Brazil, was evaluated. Growth and decolorization halos were determined on malt extract agar containing 0.002g L-1 of the dye. T. villosa decolorized all 28 of the tested dyes while P. sanguineus decolorized only 9. The effect of culture conditions (shaking and dye and nitrogen concentration) on the degradation of Drimaren Brilliant Blue dye was evaluated during growth of the fungi in liquid synthetic medium. Shaking favored degradation and decolorization was not repressed by nitrogen. In pure culture, T. villosa and P. sanguineus decolorized synthetic effluent consisting of a mixture of 10 dyes. Higher decolorization of the synthetic effluent was observed when a mixed culture of the two fungi was used. This study demonstrated differences between tropical basidiomycete species in terms of their ability to degrade reactive dyes, and reinforces the potential of this group of fungi for the decolorization of textile effluents.

OBJETIVOS: Verificar (a) a tendência das citações de artigos de pediatria oriundos de instituições brasileiras, no MEDLINE, de 1990 até 2004; (b) o número de artigos publicados em periódicos com os maiores fatores de impacto; e (c) a distribuição regional das instituições. MÉTODOS: Busca no PubMed com os seguintes limites: idade de 0 a 18 anos, língua inglesa, subconjuntos MEDLINE e humanos, afiliação brasileira. Para cada ano, comparamos os artigos encontrados com o total de citações no MEDLINE, obedecendo aos mesmos limites de busca, com exceção da afiliação; o mesmo foi feito com o total da produção científica brasileira citada no MEDLINE, sem limites de idade. Realizamos uma análise descritiva, usando o teste qui-quadrado para a tendência. Agregamos os dados relativos à publicação em periódicos com os maiores fatores de impacto em períodos de 3 anos. RESULTADOS: Entre 1990 e 2004, o MEDLINE citou 7.222 artigos de pediatria brasileiros, correspondendo a 0,95% de todos os artigos tratando da faixa etária de 0 a 18. O número absoluto de artigos brasileiros aumentou cinco vezes durante esse período. A fração de artigos brasileiros em relação ao número total de artigos aumentou de 0,51 a 1,60% (p < 0,01). A produção científica permanece fortemente concentrada na Região Sudeste do Brasil. CONCLUSÃO: As atividades brasileiras de pesquisa em pediatria demonstram uma tendência ascendente constante, proporcional ao crescimento de toda a produção científica brasileira.
OBJECTIVES: To assess (a) the trend of MEDLINE citation of pediatrics articles associated with Brazilian institutions from 1990 through 2004; (b) the number of Brazilian pediatrics articles published in journals with the highest impact factor; and (c) the regional distribution of institutions. METHODS: PubMed search limited to ages 0 to 18 years, English language, MEDLINE and humans subsets, Brazilian affiliation. For each year, we compared the articles retrieved to the whole of MEDLINE citations with the same search limits, except for affiliation, as well as to the total Brazilian scientific production cited in MEDLINE, without age limits. We made a descriptive analysis, and used the chi-square test for trend. Data concerning publication in journals with the highest impact factor were aggregated into three-year periods. RESULTS: A total of 7,222 Brazilian pediatrics articles were listed in MEDLINE from 1990 through 2004, corresponding to 0.95% of all articles concerning the age group from 0 to 18 years. There was a fivefold increase in the absolute number of Brazilian articles along the study period. The ratio of Brazilian to total articles increased from 0.51 to 1.60% (p < 0.01). Scientific knowledge production remains strongly concentrated in the Southeast of Brazil. CONCLUSION: Brazilian research activities in pediatrics have had a steady upward trend, which relates to the proportional growth of the Brazilian scientific production as a whole.

Os avanços recentes na ultra-sonografia têm ampliado a possibilidade de detecção de tumores hepáticos. Isto tem auxiliado na perspectiva de melhora do prognóstico destes pacientes, à medida que novas técnicas terapêuticas têm surgido. Neste artigo os autores relatam achados ao Doppler que podem auxiliar na identificação e caracterização dos tumores hepáticos, avaliando dados do Doppler colorido, pulsado e do Doppler de amplitude ("power Doppler"). Fazem, também, referência a novas modalidades de imagem, como o uso da harmônica.
Recent advances in ultrasound have optimized the detection of liver tumors and helped to improve the prognosis of patients with this condition as newly developed and improved therapeutic modalities have been established. The authors review important Doppler findings which may help in the identification and characterization of some hepatic tumors through the evaluation of color Doppler, pulsed Doppler and power Doppler features. New imaging methods such as the use of harmonics imaging are also reviewed.

O trabalho de psicólogos na área da saúde vem se desenvolvendo de forma crescente. O objetivo deste relato é descrever o desenvolvimento e estágio atual do Serviço de Psicologia do Hospital de Base da FAMERP, iniciado em 1981 com a contratação de uma psicóloga para atuar na enfermaria de Pediatria. Atualmente, o hospital conta com 40 psicólogos (docentes, contratados e aprimorandos) desenvolvendo atividades de extensão de serviços à comunidade, ensino e pesquisa em Psicologia da Saúde. A atuação abrange os níveis primário, secundário e terciário de atendimento e é realizada no ambulatório, no hospital, em Centro de Saúde Escola e na comunidade, principalmente em equipes interdisciplinares. O ensino envolve aulas para os cursos de graduação em medicina e enfermagem, estágio para alunos de psicologia, um Programa de Aprimoramento em Psicologia da Saúde, Cursos de Extensão, de Especialização e docência e orientação no Programa de Pós-Graduação em Ciências da Saúde (mestrado e doutorado) da FAMERP. As pesquisas visam principalmente o estudo das relações entre comportamento e saúde, abordando temas como promoção da saúde, prevenção e auxílio no tratamento e manejo de doenças. Embora a área esteja em expansão, é preciso pensar que a manutenção da credibilidade e do espaço conquistado dependem de uma sólida formação profissional, de um desempenho ético e do desenvolvimento de pesquisas na área.
Psychologists have had an increasing role in the medical area. The objective of this study is to report the development and current status of the Psychology Service at Hospital de Base, São José do Rio Preto Medical School (FAMERP), which started in 1981, when a psychologist was hired to work at the Pediatrics Outpatient Ward. Currently, the hospital has a staff of 40 psychologists (professors, supervisors, and residents), developing community services, education and research activities in Health Psychology. Such activities are carried out at a primary, secondary and tertiary level at the outpatient ward of the hospital and at a Medical School Health Community Center, especially with interdisciplinary teams. Teaching involves classes in post-graduation courses in medicine and nursing; internships for psychology students; a Residency Program in Health Psychology; Extension, Specialization, and Teaching Courses; and assistance to graduate students from the Post-Graduation Program in Health Sciences (masters degree and PhD) at FAMERP. Research is aimed at assessing the relationship between behavior and health, approaching issues such as providing health, prevention and assistance in the treatment and management of diseases. Even though this is an expanding area, the maintenance of credibility and the progress achieved depend on a solid professional training, ethics and the development of research in the area.
Les psychologues ont un rôle croissant dans le secteur médical. Ce rapport a pour objectif de décrire le développement et le stade actuel du Service de Psychologie de l’Hôpital de Base de la FAMERP, commencé en 1981 par l’embauche d’une psychologue pour travailler à l’infirmerie de pédiatrie. Aujourd’hui, l’hôpital compte 40 psychologues (professeurs, superviseurs et résidents) qui développent des activités d’extension de services à la communauté, d’enseignement et de recherche en Psychologie Médicale. Ces activités comprennent les niveaux primaires, secondaire et tertiaire de soins en urgence, à l’hôpital, au centre Hospitalier Universitaire, principalement en équipes interdisciplinaires. L’enseignement comprend des cours de 3ème cycle en médecine et infirmerie, des stages pour des étudiants en psychologie, un programme de Perfectionnement en Psychologie de la Santé, Cours de Spécialisation, de Spécialisation professoral, de recherches du programme de post-graduation en Sciences de la santé (maîtrise et doctorat) à la FAMERP. Les recherches visent principalement l’étude des relations entre comportement et santé, en abordant des thèmes comme la promotion de la santé, de la prévention et l’aide au traitement et lê manège des maladies. Bien que ce secteur soit en expansion, il faut penser que l’entretien de la crédibilité et de l’espace conquis dépendent d’une solide formation professionnelle, d’éthique et de développement des recherches dans ce secteur.
