RESUMO As características hematológicas e bioquímicas de alevinos de pirarucu Arapaima gigas alimentados com diferentes concentrações de mananoligossacarídeos derivados de leveduras e algas na dieta foram avaliadas para verificar o efeito sobre sua fisiologia. Quatro tratamentos foram conduzidos com três repetições (12 peixes em cada). O produto avaliado (MycosorbA+(r)) foi incorporado à ração comercial, em quatro níveis: 0, 1, 2 e 4 g kg-1 de ração, denominados M0%, M0,1%, M0,2% e M0,4%, respectivamente. Após 45 dias de alimentação, amostras de sangue de seis peixes de cada repetição foram coletadas para realização das análises. O peso e comprimento foram obtidos para cálculo do fator de condição e ganho de peso, entretanto, não foram observadas diferenças (P>0,05) entre os tratamentos. Assim como não foram observadas alterações nos valores de hematócrito, hemoglobina, eritrócitos e índices hematimétricos dos pirarucus. A concentração de glicose e triglicérides dos pirarucus dos grupos M0,1% e M0,2% foram significativamente menores que o tratamento M0%. Os peixes do tratamento M0,2% apresentaram níveis de albumina maior (P<0,05) que do M0% e M0,4%. Os peixes alimentados com M0,4% mostraram nível de colesterol total significativamente maior que todos os demais tratamentos. MycosorbA+(r) contribuiu para o aumento das células de defesa de A. gigas, podendo ser utilizado nas concentrações de 0,1% a 0,2%, devido ao aumento do número de certas células de defesa e dos níveis plasmáticos de albumina e por não ter sido observadas outras alterações nos parâmetros hematológicos, nos níveis plasmáticos de colesterol e triglicérides e no fator de condição.

ABSTRACT The hematological and biochemical responses of pirarucu fingerlings (Arapaima gigas) fed with diets containing different concentrations of a glucomannan product derived from yeast and algae were evaluated in order to ascertain the effect of these diets on fish physiology. Four treatments were conducted, with three replications, with 12 fish in each tank. The product evaluated (MycosorbA+(r)) was incorporated into the commercial diet, at four concentrations: 0, 1, 2 and 4 g.kg-1, called M0%, M0.1%, M0.2% and M0.4%, respectively. After 45 days of feeding, blood samples from six fish in each replicate were collected to perform the analyses. Their weight and length were determined to calculate the condition factor and weight gain, but no differences (P > 0.05) were observed among the treatments. No changes to the hematocrit, hemoglobin or erythrocyte levels or to the hematimetric indices of the pirarucus were observed. The glucose and triglyceride levels of the pirarucus in the M0.1% and M0.2% groups were significantly lower than those of the M0% group. The M0.2% group showed higher albumin levels (P < 0.05) than M0% and M0.4%. The M0.4% group showed a total cholesterol level that was significantly higher than in all other treatments. MycosorbA+(r) contributed towards increasing the levels of defense cells in A. gigas. It would be possible to use this product at concentrations of between 0.1% and 0.2%, given that they increase the levels of some defense cells and plasma albumin concentrations, without changes to hematological parameters, cholesterol and triglyceride plasma levels or condition factor.

Uma das mais importantes aplicações clínicas dos potenciais evocados auditivos de tronco cerebral (BERA) é a sua utilização na avaliação da surdez infantil. Atualmente o BERA também é utilizado na triagem das síndromes cócleo-vestibulares a procura de lesões retro-cocleares, na monitoração dos estados de coma (morte cerebral), na monitoração do tronco cerebral em cirurgias da base do crânio, etc. Uma das qualidades do BERA é a sua capacidade de avaliar a integridade neurofisiológica das vias auditivas do tronco cerebral. Desta maneira, algumas vezes durante a pesquisa dos limiares auditivos infantis, nos deparamos com BERAs que sugerem a presença de lesões retro-cocleares das vias auditivas (assimetria de traçados, aumento dos intervalos interpicos), muitas vezes comprovadas através de exames de imagem. Trata-se de achado ocasional de moléstia neurológica por ocasião da pesquisa dos limiares auditivos infantis. Neste trabalho relataremos dois casos de doenças neurológicas, diagnosticadas ocasionalmente através do BERA realizado com o intuito de se pesquisar os limiares auditivos.

One of the most important applications of the Brainstem evoked response audiometry (ABR) is in the evaluation of hearing loss in children. Today the ABR is also indicated in the screening of cochleo-vestibular syndromes to detect retrocochlear lesions, to monitor patients in a coma (brain death), in monitoring the brainstem during skull base surgery, etc. Among the many BERA qualities, is its capacity to evaluate the neurophysiologic integrity of the auditory brainstem pathway. In doing so, sometimes while evaluating hearing function in children we are faced with ABR waves that suggest the presence of retrocochlear lesions (trace asymmetry, increased interpeak intervals), many times confirmed through image studies. These cases are seen as occasional findings of neurologic disorders during children hearing loss evaluation. In this study we report 2 cases of neurologic disorders diagnosed with the use of the ABR to evaluate hearing loss in children.

As neocavidades pós-timpanomastoidectomias de cavidade aberta (TMCA) são normalmente preenchidas por cadarço ou fita cardíaca untados com pomada antibiótica. A remoção deste tampão usualmente causa sangramento e desconforto para o paciente. Propomos para tanto a utilização de uma biomembrana de látex natural para forrar a neocavidade, servindo como uma interface entre o osso cruento e o material de tamponamento. FORMA DE ESTUDO: Clínico prospectivo. OBJETIVOS: Estudar o desempenho da biomembrana como uma interface entre o osso cruento e o material de tamponamento e analisar seu papel na epitelização da neocavidade. MATERIAL E MÉTODO: Foram analisadas 64 orelhas de pacientes submetidos à TMCA e meatoplastia. A biomembrana foi utilizada em 54 das orelhas operadas, sendo que nas outras 10 orelhas o tamponamento da cavidade foi realizado somente com fita cardíaca. RESULTADOS: Na maioria das 54 orelhas onde a biomembrana foi utilizada observou-se maior facilidade na remoção do curativo tampão (sem sangramento ou desconforto para o paciente), menor demanda de tempo para sua retirada, além da epitelização mais precoce da neocavidade. CONCLUSÃO: A utilização da biomembrana de látex revelou-se método eficaz no revestimento da neocavidade, facilitando a remoção do tampão e a epitelização da neocavidade.

The new cavity created after an open cavity tympanomastoidectomy (OCTM) is filled with an antibiotic impregnated cotton pack (cotton tape, umbilical tape, gauze). The removal of this pack usually causes some bleeding and discomfort for the patient. We propose the use of a latex biomembrane to cover the cavity, which will act as an interface between the raw bone surface and the packing. STUDY DESIGN: clinical prospective. AIM: To study the performance of the latex biomembrane as an interface between the raw bone surface and the pack, and to analyze its role in cavity epithelization. MATERIAL AND METHODS: 64 ears of patients submitted to OCTM were studied. The biomembrane was used in the packing of 54 ears and in the 10 remaining ears the regular cotton tape packing was used. RESULTS: In the majority of the cases where the biomembrane was used the packing was removed much easier with no bleeding or pain for the patient and also showed an earlier cavity epithelization. CONCLUSION: The use of the latex biomembrane has proven to be an effective method to cover the mastoid cavity facilitating epithelization and removal of mastoid cavity packing.

OBJETIVO: Monitoramento do tronco cerebral através da análise dos traçados do BERA em cirurgia cardíaca com hipotermia profunda e parada circulatória total (PCT), registrando a recuperação funcional (eletrofisiológica) do tronco cerebral. FORMA DE ESTUDO: Coorte Longitudinal. MATERIAL E MÉTODO: Foram estudados o comportamento do traçado do BERA de 8 pacientes submetidos a cirurgia cardíaca com hipotermia profunda e PCT para correção de aneurisma de aorta torácica. Etapas: resfriamento corporal até 18ºC C; PCT por até 60 minutos; reaquecimento. Registros do BERA: 35ºC, 32ºC, 26ºC, 18ºC, 27ºC, 35ºC. RESULTADOS: Os traçados iniciais (35ºC) se apresentavam normais. Aos 26ºC há o desaparecimento de todas as ondas. Aos 18ºC o traçado do BERA mostrava um padrão isoelétrico. Aos 27ºC, recuperação das ondas I, III e V. A partir de 35ºC as latências das ondas retornam ao normal. CONCLUSÃO: O BERA constitui-se em competente instrumento de monitoramento da integridade funcional do tronco cerebral em cirurgia cardíaca com hipotermia profunda e PCT. As ondas desaparecem no período de resfriamento corporal, atingem um padrão isoelétrico a 18ºC, e reaparecem durante o reaquecimento. No final do procedimento, os médicos anestesistas não dispõem de exame clínico fidedigno para acessar a integridade funcional do tronco cerebral, devido ao uso de drogas depressoras do SNC. O reaparecimento das ondas do BERA é o parâmetro proposto para este fim. A manutenção do padrão isoelétrico após o reaquecimento corporal sugere grave comprometimento funcional do tronco cerebral.

AIM: The authors propose the use of ABR as an instrument for the detection of brainstem dysfunction in the trans- and post-operatory period of surgeries with total circulatory arrest (TCA) and deep hypothermia for correction of aneurysms of the thoracic aorta. STUDY DESIGN: Longitudinal Cohort. MATERIAL AND METHOD: Eight adult patients that underwent surgery for correction of aneurisms of thoracic aorta had their brainstem monitored through ABR. The patients had their body temperature lowered to 18ºC. At this moment, TCA was performed for a period of up to 60 minutes. The recordings were done at the following: before lowering the body temperature, during the cooling process, at the time of TCA (18ºC), and during the re-warming process. RESULTS: The initial tracings (35ºC) were normal. At 26ºC, ABR waves disappeared. At 18ºC and TCA, ABR showed isoelectric tracings. Waves I, III and V reappeared at 27ºC, although with prolonged latencies. When temperature reached 35ºC, latencies were back to normal. CONCLUSIONS: The ABR seems to be a useful monitor for evaluating brainstem function during surgeries with TCA and deep hypothermia. Our experience showed the extreme usefulness of the evoked auditory potentials in such procedures as a noninvasive, reliable and objective method for the monitoring of variations in the neurophysiological pattern of the brainstem. This can be of great value in presuming the patient's neurological outcome in a moment when the neurological status cannot be clinically accessed due to the several drugs used during surgery.
