Abstract Objective: This study aims to identify the possible factors that delay the time-to-diagnosis of Huntington's disease (HD). Methods: A cross-sectional study in HD patients was carried out. Variables registered were CAG repeats, age of onset, primary symptom at onset, age of molecular diagnosis, and time-to-diagnosis, among others. Results: 107 patients (50.5% female) with a mean age of 49 ± 12.8 years (y) were included in the study. Median CAG size was 45 (38-73). Mean age of onset, mean age of molecular diagnosis, and mean time-to-diagnosis were 39 ± 12.9, 45.1 ± 12.1, and 6.4 ± 6.4 years, respectively. In the comparative analysis, the neuropsychiatric- and the young-onset groups had a longer time-to-diagnosis than the motor- and typical-onset groups (p = 0.02 and p < 0.01, respectively). In the linear regression analysis, neuropsychiatric- and young-onset were independent risk factors. Conclusions: Delayed diagnosis showed relation to neuropsychiatric- and early-onset in HD.

Abstract Acute myeloid leukemia (AML) comprises a heterogeneous group of hematopoietic cell neoplasms of myeloid lineage that arise from the clonal expansion of their precursors in the bone marrow, interfering with cell differentiation, leading to a syndrome of bone marrow failure. AML is a consequence of genetic and epigenetic changes (point mutations, gene rearrangements, deletions, amplifications, and arrangements in epigenetic changes that influence gene expression) in hematopoietic precursor cells, which create a clone of abnormal cells that are capable of proliferating but cannot differentiate into mature hematopoietic cells or undergo programmed cell death. The diagnosis requires more than 20% myeloid blasts in the bone marrow and certain cytogenic abnormalities. Treatment will depend on age, comorbidities, and cytogenetic risk among the most frequent.

Abstract Hodgkin’s lymphoma is due to the clonal transformation of cells originating from B lymphocytes, generating the pathognomonic binucleate Reed-Sternberg cells. Hodgkin’s lymphoma is a B cell disease with a bimodal distribution, with higher incidence in adolescence and the third decade of life, showing a second peak in people over 55 years of age. Classic Hodgkin lymphoma cells routinely undergo gene expression reprogramming, as they lose the expression of most of the typical B-cell genes and acquire the expression of multiple genes that are typical of other types of cells in the immune system. The treatment algorithm will depend on whether it is classic or predominantly lymphocytic HL, if it is early stage with unfavorable prognostic markers or not, the initial management regimen, and whether there is bulky disease, among the most relevant variables.

Abstract To identify this increasingly common pathology, known as Multiple Myeloma, it is necessary to refer to the specific factors that characterize it; to this end, the classic criteria known as CRAB (hypercalcemia, renal failure, anemia and lytic lesions) are available, in which renal failure is one of the most frequent complications. Recently, 3 indisputable biomarkers have been described for the diagnostic support for Multiple Myeloma, which are: more than 10% of clonal plasma cells in BM or, a biopsy that corroborates the presence of a plasmacytoma, light chain ratio ≥100 mg/dL and more than one focal lesion on magnetic resonance imaging (MRI). A differential diagnosis for plasma cell leukemia, solitary bone plasmacytoma, and extramedullary plasmacytoma should always be considered. Being this an incurable disease, a lot of research has been done regarding its therapeutic management, whose main objective is the disappearance of plasma cells and the patient clinical improvement. Melphalan was the first drug that showed a benefit in 1958 and afterwards, with the addition of a steroid as a second drug, it was possible to improve response rates. Subsequently, different molecules were studied, forming multiple combinations, and achieving better rates of Overall Survival and Progression-Free Survival. Years later, with the arrival of proteasome inhibitors such as bortezomib, and immunomodulators such as thalidomide and lenalidomide, an important turnaround in the disease has been seen, as deeper responses, more prolonged remissions, and improvement in the quality of life of patients have been achieved. This consensus has the purpose of integrating a group of Mexican specialists and promoting the updating of this pathology.

Abstract Solar cells sensitized with an azo dye (dispersed red 1) mixed with graphene and graphene oxide by the electrophoresis method in acid medium applying a voltage of 3V at a time of 15 seconds were developed, these conditions allowed to deposit the layer satisfactorily semiconductor and subsequently properly confine the sensitizer. All cells were thermodynamically stable. The cells sensitized with disperse red 1 and disperse red 1 mixed with graphene and graphene oxide reported overall efficiencies of 0.12, 0.34 and 0.21 respectively. The maximum power calculated from the measured current density and voltage data for cells sensitized with dispersed red 1 and disperse red 1 mixed with graphene and graphene oxide were 9.68 W/cm2 x 10-3 W/cm2, 11.74 W/cm2 x 10-3 W/cm2, and 19.19 W/cm2 x 10-3 W/cm2 respectively. All photoelectrodes had stable confinement between the sensitizer and the semiconductor phase.

Objective Compare if localization of sounds and words discrimination in reverberant environment is different between children with dyslexia and controls. Method We studied 30 children with dyslexia and 30 controls. Sound and word localization and discrimination was studied in five angles from left to right auditory fields (-90o, -45o, 0o, +45o, +90o), under reverberant and no-reverberant conditions; correct answers were compared. Results Spatial location of words in no-reverberant test was deficient in children with dyslexia at 0º and +90o. Spatial location for reverberant test was altered in children with dyslexia at all angles, except –-90o. Word discrimination in no-reverberant test in children with dyslexia had a poor performance at left angles. In reverberant test, children with dyslexia exhibited deficiencies at -45o, -90o, and +45o angles. Conclusion Children with dyslexia could had problems when have to locate sound, and discriminate words in extreme locations of the horizontal plane in classrooms with reverberation.

AbstractGeospatial databases are a central part in applications and projects within the context of research in agriculture and natural resources. Performing operations with geographic information systems and with software for the enhancement, display or analysis of geographic data and information contained in digital maps, satellite images and aerial photographs generate results which are inserted within geospatial databases. These repositories are the core of geo-based applications, in particular, those related with a variety of research topics. Each system manages geospatial databases according to its own data model, and performs specific database management functions according to the procedures requested by users. A data model is an essential tool either for software developers or for users. A data model assists in the comprehension, at different levels of depth and detail, of the contents, organization and capabilities of databases, as well as to the comprehension of software restrictions and functionalities required to access the data. Current commercial informatics technology is designed for relational which are the most common types in use; in general this technology is not oriented to the needs of geospatial databases.Relational databases are the most common data repositories in managerial or business applications. In these databases the DBMS or data base management system is based on the relational model, a formal prescription for the organization and structure of data introduced in the 1970s by E. F. Codd. The relational model and the associated Structured Query Language (SQL) are recognized standards for relational databases. Thus, relational DBMS all are based on a common data model which means that at the logical and at the user levels the data are seen through a same lens, which facilitates data exchange among systems and users. Object databases are oriented towards applications where data are inherently more complex than relational tables, such as the data in digital maps. There is no standard object data model, but basic object-oriented concepts for structuring and relating objects such as aggregation, semantic links and the inheritance of properties are commonplace.Following recommendations by the Open Geospatial Consortium, some proprietary database management systems have included in recent years basic constructs to manage geospatial information. Anyway most database management systems, relational or otherwise, do not make public how they manage the internal storage of data items nor how the manipulation functionalities are implemented, since these are trade secrets. The lack of standards for geospatial databases makes the exchange, sharing and reutilization of results among diverse applications and among different users a very tedious process. Normally manual user intervention is required and the task is made difficult and prone to errors because many geographic information producers do not provide sufficient concise descriptive data (metadata) about the results of their work. Most systems offer export/import functions to move geospatial data to/from other systems, but the verification of compatibility for appropriate use of the data is left to the user. She is made responsible for correction or modification of aspects which are significant regarding the applicability of processes or functions. These aspects include file format compatibilities or cartographic projections which when misunderstood lead to repetitive and accumulative errors, with a negative impact on the quality of end products.Multidisciplinary and interdisciplinary research in Geomatics is present in many organizations and institutions worldwide. The objective of the research and development project reported here was to analyze ways to gather in a unique repository geospatial information products to be shared among the group of people working within the research line "Geomatics Applied to the Study and Management of Natural Resources and Agrosystems" of the Colegio de Postgraduados. In the medium term the repository should be able to support decision making using products derived from the research of this group. This article refers to the methodology to build the geospatial database "GeoBase-L9", which facilitates the construction of the repository and the exchange of geographical products among users. We present the part of the methodology that is based on our own metadata schema which adheres to Mexican national standards which in turn follow international standards. The methodology has allowed the implementation of a pilot version to visualize via web the items that have been input to the GeoBase-L9 and also check the research products offered to the user.

Background: in most cases, inflammatory bowel disease (IBD) debuts at reproductive age. The data available in the literature show infliximab (IFX) to be a safe drug during pregnancy but there is very little evidence about the activity of the disease following drug withdrawal during pregnancy. Aims: determine the drug´s safety in pregnant women in our setting and assess its effect on the foetus, drawing on the experience of several hospitals. Secondly, observe the effect of treatment withdrawal on disease activity during pregnancy. Material and methods: a retrospective study was conducted of women with IBD who had received IFX treatment during pregnancy in five hospitals in Spain. Disease activity was assessed using Crohn´s Disease Activity Index, while UC was assessed using the Truelove-Witts Index in each trimester of pregnancy. Gestational age, weight and diseases in the foetus were determined at birth. Results: the study included 12 women with a mean age of 29 years; 4 had ulcerative colitis and 8 Crohn´s disease, with mean disease duration of 7 years. All but one, who was diagnosed during pregnancy, was receiving IFX treatment at conception. Six patients received uninterrupted treatment throughout the pregnancy, 2 requested voluntary interruption and in 3 cases treatment was interrupted in the third trimester as a precaution. They received a mean IFX dose of 400 mg every 8 weeks. Of the 6 patients who received continuous treatment, in 50% disease was held in remission. The 6 remaining patients suspended treatment for different reasons, presenting disease recurrence in all but one case (83.3%). Eight deliveries were vaginal and 4 by caesarean section. Newborns presented no congenital anomalies, intrauterine growth retardation or low birth weight and there was only one premature delivery. Conclusions: although cases included in the stduy are not significant, in our experience, IFX during pregnancy is a safe treatment for the mother and the foetus. In fact, in our study and in some cases, its withdrawal may lead to a worsening of the disease. However, further control studies are required with larger samples to obtain more representative findings.

The first method is very sensitive to the time and space dependent gradients produced by the pulsed plasma, and it is applied to measure the resulting beam deflection over a He-Ne laser beam that is placed perpendicular to the propagation axis. The system with a quadrature detector has a rise time response of 10 ns and a resolution of 2 µrad. The Langmuir double probe is used along the axis and allows us to measure the time of flight under diverse plasmas conditions. We present comparative measurements for Nitrogen and Helium background gases from 50 to 500 mTorr, in a 50 cm long and 10 cm diameter chamber.

A theoretical and experimental study of the potential well that takes place behind the shock wave excited in an ion-beam plasma system was performed. The space distribution of the potential well was calculated with the fluid theory where, in addition to the two ionic species, two electronic species with different temperatures and densities were included. The width and depth of the well, which were measured experimentally, were compared with the theoretical results. Amplification of locally excited test waves within the plasma was measured, allowing us to calculate the ion beam acceleration in the potential well. From these measurements we calculated the space variation of ion beam velocity within the well.

Introduction. The availability of easily pliable skin has allowed the functional reconstruction of oral cavity defects. Although the radial forearm free flap is the most frequently used flap for the reconstruction of surface defects of the oral cavity, the lateral arm free flap may be preferable in some situations. Objectives. The aim of the present paper is to show the advantages and disadvantages and our indications and results for the lateral arm flap in intraoral reconstruction. Material and methods. This is a prospective work on the use of the lateral arm free flap for the reconstruction of oral cavity defects after ablative surgery. The parameters that have been evaluated are: flap viability, morbidity in the donor site, length of the pedicle, selection of recipient vessels, complications and functional results in the reconstructed area. Results. The lateral arm flap has been used in primary reconstruction after ablative surgery for squamous cell carcinoma of the oral cavity in ten patients. One flap was lost because of venous thrombosis. The donor site was repaired by direct closure in 8 cases and a split thickness skin graft had to be used in 2 cases. Mean pedicle length was 8.75 cm. In 9 cases a favorable functional result was achieved. Conclusions. Fasciocutaneous lateral arm flap allows the reconstruction of oral cavity defects achieving good functional results. Morbidity in the donor site is minimal and, in most cases, direct closure permits the repair of the donor site.

Inflammatory pseudotumor is a term that refers to a reactive pseudoneoplastic disorder that can appear in different locations of the human body. The lung is the most frequently affected organ. The etiology is still unknown. It affects individuals of both sexes and of a wide range of ages. The diagnosis is still difficult and it is based on the histological examination of the lesions composed of four cell-types: histiocytes, myofibroblasts, plasma cells and lymphocytes. With regard to the treatment regimes there is no agreement. Treatment ranges from surgical excision to radiotherapy, chemotherapy or steroids. The purpose of this article is to report one case of inflammatory pseudotumor located in the parotid gland and to make a special point of the difficulty in arriving at a correct diagnosis in order to achieve the most adequate treatment.

Experiments in a low pressure glow discharge on gases of environmental relevance: CO2,CO y O2 are reported. We studied the various atomic processes: ionization, excitation and molecular dissociation with typical discharge parameters of p~0.6 mbar, I< 0.1 A and V~1100 Volts. We used visible and near UV spectroscopy to observe the emitted radiation on both electrodes on pure gases and hydrogen seeded mixtures. We also set up Langmuir probes in both electrodes.

The aim of this paper is to evaluate the functional and aesthetic results of complex orofacial reconstructions by means of the combination of two simultaneous free flaps (fibula osteocutaneous and radial forearm flaps). Three patients who have undergone this kind of reconstruction are presented with good functional and aesthetics results. Reconstruction with two free flaps is indicated in complex defects in which reconstruction with a single flap would not obtain adequate aesthetic and functional results. The choice of each particular flap depends on its specific characteristics that make it ideal for the reconstruction of each particular type of defect, osseous defects and soft tissue defects, with the aim of obtaining a better result than that obtained with the use of a single flap.
