Abstract An adequate amount of soil organic matter is considered essential for long-term sustainable agriculture, ensuring good productivity. Therefore, the present study quantified carbon and nitrogen stocks and microbiological activity in soil of three different areas: Forest-pasture System, Pasture System and preserved Caatinga - used as a reference of the experiment - located in a semi-arid region, in Watershed of Pajeú River, state of Pernambuco, Brazil. Eight trenches were opened in each system to collect soil samples at depths of 0 - 10, 20 - 30 and 50 - 60 cm, for chemical and microbiological analysis. In general, attributes of soil showed better results of land use in the following order: preserved Caatinga - Forest-pasture System - Pasture System.

RESUMO A microirrigação associada à fertilização nitrogenada visa suprir a demanda hídrica e nutricional da palma forrageira. Objetivou-se, com este estudo, avaliar o crescimento e a produtividade da palma forrageira irrigada por microirrigação e fertilizada com nitrogênio. O experimento foi conduzido a campo, na fazenda Poço Redondo, Santa Luzia, PB, Brasil (6° 52’ 27’’ S e 36º 56’ 0’’ W e altitude de 299 m), entre abril e dezembro de 2017. O delineamento experimental foi em blocos casualizados no esquema fatorial 5 x 2, sendo cinco frequências de irrigação (7, 14, 21, 28 e 35 dias) e dois tipos de fertilização (solo fertilizado com nitrogênio e não fertilizado), na cultivar Orelha de Elefante Mexicana, com três repetições. Diferentes frequências de irrigação promovem decréscimo na produtividade de biomassa verde e acréscimo na produtividade econômica da água quando se utiliza o cladódio para semente na palma forrageira cv. Orelha de Elefante Mexicana. A fertilização nitrogenada aumentou o crescimento e as produtividades de biomassa verde, da água e econômica. O maior retorno econômico é obtido com a produção de cladódios comercializados para material propagativo. A redução da frequência de irrigação proporciona maior economia de água, aumentando a área irrigada e a produção, ao utilizar menor volume de água com alta frequência.

ABSTRACT Microirrigation associated with nitrogen (N) fertilization aims to meet the water and nutritional demands of cactus pear. The objective of this study was to evaluate the growth and yield of cactus pear (Opuntia ficus-indica) irrigated by microirrigation and fertilized with N. The experiment was conducted in the field at the Poço Redondo farm, Santa Luzia, PB, Brazil (6° 52’ 27’’ S and 36º 56’ 0’’ W and altitude of 299 m), between April and December 2017. The experimental design was in randomized blocks in a 5 x 2 factorial scheme, with five irrigation frequencies (7, 14, 21, 28 and 35 days) and two types of fertilization (soil fertilized with N and not fertilized) in the cultivar ‘Orelha de Elefante Mexicana’, with three replicates. Different irrigation frequencies promote reduction in green biomass yield and increase in the economic water productivity when using of the cladode for propagative material. Nitrogen fertilization increased growth, green biomass yield, water productivity and economic water productivity. The greatest economic return is obtained by producing cladodes marketed for propagative material. Reducing the frequency of irrigation promotes greater water saving, increasing the irrigated area and production by using less water at high frequency.

Abstract Introduction: vitamin A deficiency (VAD) is one of the biggest problems in public health worldwide, and night blindness (NB) is the first functional change caused by deficiency of this vitamin. In this context, electroretinography stands out as the gold-standard diagnostic method; however, it is a high-cost method and its applicability in clinical practice presents some difficulties. Objective: to compare NB diagnosis through the use of the standardized interview issued by the World Health Organization/Pan American Health Organization (WHO/PAHO) and with electroretinography, and to evaluate the association of these diagnoses with serum concentrations of retinol in individuals with class III obesity. Methods: adult patients of both genders, in the 20-60 years age group, with BMI ≥ 40 kg/m², were studied. NB was diagnosed through electroretinography and the standardized interview validated by the WHO/PAHO. Serum retinol was quantified by the HPLC-UV method, and VAD was diagnosed when levels were < 1.05 μmol/L; severity was also evaluated. The statistical analysis was carried out using the Statistical Package for the Social Sciences, version 21.0 (p< 0.05). Results: mean BMI was 44.9 ± 11.8 kg/m², and a negative correlation was found for serum levels of retinol (p= 0.01). The prevalence of VAD, according to the serum concentration of retinol, was 14 %; of this percentage 23.3 % had NB according to the standardized interview, and 22.0 % according to electroretinography. NB as diagnosed by both methods showed an association with VAD according to serum concentrations of retinol. Of these individuals with NB, according to the standardized interview, 6.9 % had severe VAD, 10.3 % moderate VAD, and 82.8 % marginal VAD. Conclusion: the standardized interview for the diagnosis of NB can be a good strategy to evaluate the nutritional status of vitamin A, and it is a simple, non-invasive, low-cost method.

Resumen Introducción: la deficiencia de vitamina A (DVA) es uno de los mayores problemas de salud pública a escala mundial, y la ceguera nocturna (CN) es el primer cambio funcional causado por la falta de esta vitamina. En este contexto, la electrorretinografía se destaca como el método de diagnóstico de referencia; sin embargo, es un método de coste elevado y su aplicabilidad en la práctica clínica presenta algunas dificultades. Objetivo: comparar el diagnóstico de CN mediante el uso de la entrevista estandarizada de la Organización Mundial de la Salud/Organización Panamericana de la Salud (OMS/OPS) con la electrorretinografía, y también evaluar la asociación de estos diagnósticos con las concentraciones séricas de retinol en las personas con obesidad de clase III. Métodos: se estudiaron pacientes adultos de ambos sexos, en el grupo de 20 a 60 años de edad y con IMC ≥ 40 kg/m². La NB se diagnosticó mediante electrorretinografía y la entrevista estandarizada validada por la OMS/OPS. El nivel sérico de retinol se cuantificó mediante el método HPLC-UV, y el DVA se diagnosticó cuando los niveles eran < 1.05 µmol/l; también se evaluó la gravedad. El análisis estadístico se realizó a través del Paquete Estadístico para las Ciencias Sociales, versión 21.0 (p < 0,05). Resultados: el IMC promedio fue de 44,9 ± 11,8 kg/m² y se encontró una correlación negativa en los niveles séricos de retinol (p = 0,01). La prevalencia de DVA, según las concentraciones séricas de retinol, fue del 14 %; de este porcentaje, el 23,3 % tenían NB de acuerdo con la entrevista estandarizada y el 22,0 % según la electrorretinografía. La NB diagnosticada por ambos métodos mostró asociación con el DVA según las concentraciones séricas de retinol. De estos individuos con NB, según la entrevista estandarizada, el 6,9 % tenían VAD grave, el 10,3 % VAD moderado y el 82,8 % VAD marginal. Conclusión: la entrevista estandarizada para el diagnóstico de NB puede ser una buena estrategia para evaluar el estado nutricional de la vitamina A y es un método simple, no invasivo y de bajo coste.

Introduction: The rise in prevalence of obesity has occurred concomitantly to that of vitamin D deficiency (VDD). The aim of this narrative review was to describe the relationship between obesity and such related diseases as VDD in adolescents, in an effort to warn of the risks of this deficiency during this period of growth and development. Methods: We searched the electronic databases PubMed, Medline, Scielo, Science Direct and Lilacs for articles from between 2000 and 2015 on the topics obesity and obesity-related diseases and VDD in adolescents. We included articles written in English, Spanish and Portuguese of the analytical variety (transverse and longitudinal), systematic reviews, meta-analysis and controlled clinical trials on humans, and excluded studies that were done on animals, inconclusive or with undefined methodology. Results: We produced an overview of VDD in obesity, in cardiovascular diseases, in type 2 diabetes mellitus, in systemic hypertension, and in dyslipidemia. The prevalence of VDD was considered high in obese adolescents and their relationship with the obesity and related diseases was found in adolescents. These findings forewarn of possible clinical repercussions in the health of the adolescents, foremost because of how essential vitamin D is to growth and development, and for its interaction with obesity and obesity-related diseases. Conclusion: The worldwide rise in the obesity rate alongside the progressively increasing of vitamin D deficiency in adolescents is alarming. This relationship of VDD with the obesity and related diseases was found in adolescents. Vitamin D supplementation is considered promising measure to take with obese adolescents.

Introducción: el aumento de la prevalencia de la obesidad se ha producido de forma concomitante a la deficiencia de vitamina D (DVD). El objetivo de esta revisión fue describir la relación entre la obesidad y las patologías asociadas a la obesidad y la DVD en adolescentes, en un esfuerzo por advertir los riesgos de esta deficiencia durante este periodo de crecimiento y desarrollo. Métodos: se realizaron búsquedas en las bases de datos electrónicas PubMed, Medline, Scielo, Science Direct y Lilacs para los artículos de entre 2000 y 2015 sobre temas como la obesidad y las patologías asociadas a la obesidad y DVD en adolescentes. Se incluyeron artículos escritos en inglés, español y portugués de la variedad analítica (transversal y longitudinal), revisiones sistemáticas, metaanálisis y ensayos clínicos controlados en seres humanos, y excluyendo los estudios que se realizan en animales, no concluyentes o con metodología indefinida. Resultados: se produjo una visión general de DVD en la obesidad, las enfermedades cardiovasculares, la diabetes mellitus, la hipertensión sistémica y en la dislipidemia. La prevalencia de DVD se considera alta en adolescentes obesos y su relación con las patologías asociadas a la obesidad fueran encontradas en adolescentes. Estos datos advierten sobre posibles repercusiones clínicas en la salud de los adolescentes, sobre todo debido a la forma esencial de vitamina D en el crecimiento y el desarrollo, y por su interacción con la obesidad y las patologías asociadas a la obesidad. Conclusión: el aumento mundial de la tasa de obesidad junto con el aumento progresivo de la deficiencia de vitamina D en los adolescentes es alarmante. Se encontró una relación en adolescentes entre VDD y obesidad y enfermedades relacionadas. La suplementación de vitamina D se considera una medida promisoria para tratar a los adolescentes obesos.

Abstract In this paper, we want to address the problem of teaching algebra from the point of view of the theoretical framework of Mathematical Working Spaces (MWS). In our study, we carried out two experiments, one in Quebec and one in Mexico. In this research, we consider the visualization has started, once the individual communicates his ideas to others (or himself) and builds relationships between the elements involved, including articulations among representations. The evolution of these cognitive structures within a perspective of the processes of objectification, requires shared experiences in which we have designated as a teaching and collaborative learning, self-reflection and scientific debate (ACODESA) under the point of view of activity theory. Specifically, it investigates polygonal numbers (especially triangular numbers) in an environment of paper, pencil, and technology. Our interpretation of students’ (14-15 year old) cognitive processes of a population at secondary school in Mexico, following the theoretical framework of the ETM. This interpretation focuses on a step towards a cognitive epistemological plane, where mathematical activity enables articulation among visualization processes, production processes, and arithmetico-algebraic validation providing a support precisely to an arithmetico-algebraic thinking.

Resumen En este documento nos interesa abordar el problema de la enseñanza del álgebra bajo el punto de vista del marco teórico de los Espacios de Trabajo Matemático (ETM). En nuestro estudio, realizamos dos experimentaciones, una en Québec y otra en México. En la investigación, hemos considerando la visualización que se pone en marcha, una vez que el individuo comunica sus ideas a otros (o a sí mismo), y construye relaciones entre los elementos en juego, incluyendo articulaciones entre representaciones. La evolución de estas construcciones cognitivas dentro de una perspectiva de los procesos de significación, necesita de experiencias compartidas, en lo que nosotros hemos designado como una enseñanza y aprendizaje en colaboración, debate científico y autorreflexión (ACODESA), bajo el punto de vista de la teoría de la actividad. Específicamente, se investiga en torno a los números poligonales (en especial los números triangulares), en un ambiente de papel, lápiz y tecnología. Nuestra interpretación sobre los procesos cognitivos de una población de alumnos (14-15 años) de una escuela secundaria en México siguen el marco teórico de los ETM. Esta interpretación se centra en el paso de un plano epistemológico hacia un plano cognitivo en donde la actividad matemática permite una articulación de procesos de visualización, producción y procesos de validación aritmético-algebraicos que proporcionan un suporte precisamente a un pensamiento aritmético-algebraico.

Um experimento foi conduzido com a cultivar de cana-de-açúcar (Saccharum spp.) RB92579, no período de 2010 a 2011 em Capim, PB, tendo como objetivo a análise dos componentes dos balanços de radiação e energia. Monitoraram-se as variáveis: radiação solar incidente e refletida; saldo de radiação; radiação atmosférica incidente à superfície, radiação efetiva emitida e o albedo da superfície vegetada. O balanço de energia, fundamentado na razão de Bowen, foi utilizado na determinação dos fluxos de calor latente e sensível para o ar e o solo. Para as fases de crescimento máximo e maturação observou-se que: a) não houve mudanças significativas no comportamento dos componentes do balanço de radiação; b) o albedo variou de 0,17 a 0,27 com média de 0,23; c) as regressões lineares entre o saldo de radiação e o saldo de radiação de ondas curtas apresentaram elevados níveis de correlação; d) o coeficiente térmico diminuiu e o coeficiente de transformação em ondas longas aumentou com o aumento da cobertura vegetal; e) o saldo de radiação diário e os fluxos de calor latente e sensível aumentaram do período chuvoso para o período seco e f) 82% do saldo de radiação foram utilizados como calor latente, 16% como calor sensível para aquecimento do ar e 2% como calor sensível para aquecimento do solo.

An experiment was conducted with the sugarcane (Saccharum spp.) variety RB92579, during the period of 2010 to 2011 in Capim,PB, aiming to analyse the components of the radiation and energy balances. The following variables were monitored: incident and reflected solar radiation, net radiation, atmospheric radiation incident to the surface, emitted effective radiation and the albedo of the vegetated surface. The energy balance based on the Bowen ratio was used to determine the latent and air and soil sensible heat fluxes. For the sugarcane maximum growth and maturation phases showed that: a) there were no significant changes in the behavior of the radiation balance components, b) the albedo ranged from 0.17 to 0.27 with an average of 0.23; c) the linear regressions between net radiation and net shortwave radiation showed high levels of correlation; d) the thermal coefficient decreased while the coefficient of transformation into long waves increased with increasing vegetation cover, e) the daily net radiation and the latent and sensible heat fluxes increased from the rainy to the dry seasons; f) 82% of net radiation was used as latent heat, 16% and 2% as sensible heat for heating air and the soil, respectively.

RACIONAL: A vitamina A participa de várias funções primordiais no organismo humano e as suas concentrações séricas podem estar diminuídas nas doenças crônicas não transmissíveis. OBJETIVO: Avaliar a relação entre o estado nutricional da vitamina A, e a regressão da esteatose hepática em indivíduos submetidos à gastroplastia em Y-de-Roux para tratamento da obesidade classe III. MÉTODOS: Foram estudados 30 pacientes obesos classe III, de ambos os sexos, com esteatose hepática, submetidos à gastroplastia em Y-de-Roux. Seis meses após a operação, os pacientes foram submetidos à ultrassonografia abdominal e distribuídos em dois grupos: grupo 1 - pacientes com esteatose detectada na ultrassonografia e grupo 2 - pacientes sem esteatose detectada na ultrassonografia. No pré-operatório e seis meses após a operação foram realizadas análises antropométricas e exames bioquímicos: insulina basal, glicemia, Homeostasis Model Assessment Index (HOMA IR), colesterol, HDL, LDL, triglicerídeos, AST, ALT, Gama-GT, albumina, bilirrubina total, retinol, e beta caroteno. RESULTADOS: A média de perda de peso foi de 35,05 + 10,47 (p<0,01) e a diminuição no Índice de Massa Corporal (IMC) de 11,6 + 4,99 Kg/m² (p<0,01). Após seis meses, todos os exames bioquímicos apresentaram diminuição significativa das concentrações (p<0,05). O grupo 2 (sem esteatose) apresentou no pós-operatório, concentrações significativamente inferiores (p<0,05), nas variáveis: peso, IMC, AST, ALT, gama GT, HOMA IR, insulina basal. As concentrações séricas de albumina apresentaram valores médios aproximados, sem diferença significativa nos dois tempos avaliados. Observaram-se concentrações séricas mais elevadas de retinol e de β-caroteno e redução menor em relação as concentrações do pré-operatório nos pacientes sem esteatose, porém sem diferença estatística. CONCLUSÃO: Adequado estado nutricional de vitamina A pode contribuir na regressão da esteatose hepática em pacientes submetidos à Gastroplastia em Y-de-Roux para tratamento da obesidade classe III.

BACKGROUND: Vitamin A participates in several essentials functions in the human body and their serum concentrations may be decreased in non-transmissible diseases. AIM: To assess the relationship of the nutritional status of Vitamin A through the serum concentrations of retinol and beta carotene, with regression of hepatic steatosis in individuals who undergone Roux-en-Y gastric bypass surgery for treatment of class III obesity. METHODS: Were included 30 individuals, male and female, submitted to Roux-en-Y gastric bypass for treatment of class III obesity, who were diagnosed through an abdominal ultrasonography as presenting hepatic steatosis. From the result of an ultrasonography screened six months after the surgical procedure those subjects were divided into two groups: group 1 - patients with steatosis detected in the ultrasonography (16 subjects) and group 2 - patients without steatosis detected in the ultrasonography (14 subjects). Before and six months after the surgery, were carried out anthropometrical analyses and biochemical exams (basal insulin, glicemy, Homeostasis Model Assessment Index (HOMA IR), cholesterol, HDL, LDL, triglycerides, AST, ALT, Gamma-GT, albumin, total bilirubin, retinol, and beta carotene. RESULTS: The individuals presented an average weight loss of 35.05 + 10.47 (p<0.01) and a decrease in the Body Mass Index (BMI) of 11.6 + 4.99 Kg/m² (p<0.01). After six months, all the biochemical exams presented a significant decrease in their basal concentrations (p<0.05). In the post-operative period the group 2 (without steatosis) presented concentrations significantly lower (p<0.05) in the following variables: weight, BMI, AST, ALT, Gamma-GT, HOMA IR, basal insulin. The albumin serum concentrations presented close average values, with no significant difference in the two periods evaluated. In the assessment of retinol and beta carotene, higher serum concentrations and a small decrease in relation to the concentrations of the pre-operative period in the group without steatosis were observed, however, no statistical difference was found. CONCLUSION: An adequate nutritional status of vitamin A might contribute in the improvement of the hepatic esteatosis after Roux-en-Y gastric bypass for class III obesity treatment.

OBJETIVO: Investigar a associação entre doença hepática gordurosa não alcoólica (DHGNA) e os marcadores de lesão e função hepática com os componentes da síndrome metabólica (SM) em indivíduos obesos classe III. MÉTODOS: A população estudada foi constituída por 144 pacientes com obesidade classe III (IMC > a 40 kg/m²). A SM foi identificada segundo o critério do NCEP ATP III, por meio da determinação do perfil lipídico, glicemia e insulina basal. Foram quantificados ainda os marcadores de função e lesão hepática. A resistência à insulina (RI) foi verificada pelo índice HOMA-IR e o diagnóstico da DHGNA por ressonância magnética. Os cálculos estatísticos foram realizados pelo programa estatístico SPSS na versão 13.0. A associação foi verificada pelo teste Mann-Whitney e qui-quadrado, com nível de significância de 5%. RESULTADOS: Foi encontrada associação significativa entre o diagnóstico de SM e DHGNA (χ² = 6,84; p = 0,01). Quanto aos componentes diagnósticos para SM, constatou-se associação positiva e significativa entre HDL-c (p = 0,05), circunferência da cintura (p < 0,05) e hipertensão arterial (χ² = 4,195; p = 0,041) com a DHGNA. O índice HOMA-IR (p < 0,001) também apresentou associação positiva com a doença hepática. CONCLUSÃO: Constatouse associação positiva e significativa entre DHGNA e componentes da SM em indivíduos obesos classe III, sugerindo a necessidade e importância do monitoramento desses componentes para rastreamento da DHGNA.

OBJECTIVE: To investigate the association between non-alcoholic fatty liver disease (NAFLD) and liver function/injury markers with components of metabolic syndrome (MS) in class III obese individuals. METHODS: The study population consisted of 144 patients with class III obesity (body mass index [BMI] > 40 kg/m²). MS was diagnosed according to the National Cholesterol Education Program - Adult Treatment Panel III (NCEP ATP III) criteria, by determining the lipid profile, blood glucose and basal insulin. Liver function/injury markers were also quantified. Insulin resistance (IR) was measured by HOMA-IR and NAFLD diagnosis was established by magnetic resonance imaging (MRI). Statistical calculations were performed by SPSS version 13.0. The association was assessed by the Mann-Whitney and Chi-square tests, with a level of significance set at 5%. RESULTS: There was a significant association between the diagnosis of MS and NAFLD (χ² = 6.84, p = 0.01). As for the diagnostic components of MS, there was a positive and significant association between HDL-C (p = 0.05), waist circumference (p < 0.05) and hypertension (χ² = 4.195, p = 0.041) with NAFLD. HOMA-IR (p < 0.001) also showed a positive association with liver disease. CONCLUSION: A positive and significant association between NAFLD and components of metabolic syndrome in class III obese individuals was observed, suggesting the need and importance of monitoring these components for NAFLD screening.

OBJETIVOS: Avaliar a concordância entre a ressonância magnética (RM) e a ultrassonografia abdominal (USG) no diagnóstico da doença hepática gordurosa não-alcoólica (DHGNA), bem como a concordância entre cada método com o exame padrão-ouro histopatológico. MÉTODOS: A população estudada foi constituída por 145 pacientes com obesidade grau III (IMC > 40 Kg/m²), de ambos os sexos. O diagnóstico da DHGNA foi realizado por ultrassonografia e ressonância magnética. Em uma subamostra foi realizado o diagnóstico por biópsia hepática (n=40). Para avaliar a concordância entre os diagnósticos por USG e RM, utilizou-se o coeficiente Kappa. RESULTADOS: Observou-se uma concordância fraca entre os dois métodos (Kappa ajustado= 0,27; IC 95%= 0,07-0,39). Foi encontrada uma concordância moderada entre o diagnóstico da doença por USG e biópsia hepática, com 83,3% de resultados concordantes e Kappa ajustado de 0,67. Já a concordância entre o diagnóstico por RM e histopatológico foi ausente, com 53,6% de resultados concordantes e Kappa ajustado de 0,07. CONCLUSÃO: A boa concordância encontrada entre o diagnóstico realizado pela USG e a biópsia hepática reforça a necessidade de condução de mais estudos como os que vêm recomendando uma padronização da avaliação diagnóstica por USG como forma de minimizar a necessidade da realização da biópsia hepática para diagnóstico de formas mais graves da doença.

OBJECTIVES: To evaluate the concordance between abdominal ultrasound and an MRI (Magnetic Resonance Imaging) in the diagnosis of non-alcoholic fatty liver disease (NAFLD), and concordance of these two methods with the histopathological exam. METHODS: The population studied was comprised of 145 patients with morbid obesity (BMI > 40 Kg/m²), of both genders. NAFLD diagnosis was performed by MRI and Ultrasound. Liver biopsy was performed in a sub-sample (n=40). To evaluate the concordance of these two methods, the kappa coefficient was used. RESULTS: Concordance between both methods (MRI and Ultrasound) was poor and not significant (Kappa adjusted= 0.27; CI 95%= 0.07-0.39.) Nevertheless a slight concordance was found between diagnosis of NAFLD by ultrasound and the hepatic biopsy, with 83.,3% of concordant results and Kappa adjusted= 0.67.Results of an MRI and the histopathological exam were compared and results showed 53.6% of concordant results and kappa adjusted= 0.07. CONCLUSION: The concordance found in the diagnosis performed using the ultrasound method and the hepatic biopsy, shows a need to implement and perform more research on the use of ultrasound to validate and reconsider these methods. This would minimize the need to perform biopsies to detect and diagnose such disease.

CONTEXTO E OBJETIVO: A daunorrubicina lipossomal tem sido usada no tratamento em várias doenças hematológicas malignas, incluindo mieloma múltiplo (MM). O objetivo deste estudo foi avaliar a eficácia, efeitos colaterais e toxicidade da daunorrubicina lipossomal and dexametasona no Protocolo DD. TIPO DE ESTUDO E LOCAL: Estudo prospectivo, realizado nos hospitais Sírio Libanês, São Camilo, Brasil e no Hospital Alemão Oswaldo Cruz. MÉTODOS: 20 pacientes com MM ativo receberam daunoxome (25-30 mg/m²/dia) por três dias consecutivos, mensal, por quatro meses (total de quatro ciclos), e dexametasona, 10 mg a cada seis horas por quatro dias consecutivos (dia 1 - 4, 9 - 12 e 17 - 20), também mensal. RESULTADOS: A mediana entre o diagnóstico e o início do protocolo DD foi de 13 meses. Quinze pacientes receberam alguma quimioterapia anterior ao protocolo DD. Uma redução maior que 50% do pico monoclonal sérico foi observada em seis paciente após o primeiro ciclo do DD (30%), em seis pacientes após o segundo ciclo (30%), em quatro pacientes após o terceiro ciclo (20%) e em quatro pacientes não houve redução (20%). No início do protocolo, 17 pacientes (85%) apresentavam anemia e em 12 destes pacientes (70%) a anemia foi corrigida. Doença progressiva foi observada em três pacientes (15%), um apresentava resposta mínima, quatro pacientes (20%) apresentaram resposta parcial e 12 (60%) apresentaram resposta completa. A toxicidade hematológica foi aceitável.Toxicidade em trato gastrointestinal foi leve, consistindo em náusea (10%) e anorexia (15%), sem episódios de vômito. CONCLUSÃO: Este tratamento apresentou uma baixa toxicidade, uma boa taxa de resposta e pode ser usado previamente ao transplante de medula óssea autogênico.

CONTEXT AND OBJECTIVE: Liposomal daunorubicin has been used to treat hematological malignancies, including multiple myeloma (MM). The goal was to evaluate efficacy, side-effects and toxicity of liposomal daunorubicin and dexamethasone ("DD Protocol"). DESIGN AND SETTING: Prospective study at Sírio-Libanês, São Camilo, Brasil and Alemão Oswaldo Cruz hospitals. METHODS: Twenty consecutive patients with active MM received four cycles of liposomal daunorubicin intravenously for two hours (25-30 mg/m²/day) on three consecutive days per month, with oral dexamethasone (10 mg every six hours) on four consecutive days three times a month. RESULTS: The male/female ratio was 1:1 and median age 60. Nine patients were stage IIA, ten IIIA and one IIIB. The median from diagnosis to starting DD was 13 months. All patients received four cycles, except one. Fifteen had already received chemotherapy before DD. Responses of > 50% reduction in serum monoclonal paraprotein were observed in six patients after first cycle (30%), six after second (30%) and four after third (20%), while four (20%) did not obtain this. Initially, 17 patients (85%) had anemia: 12 (70%) achieved correction. Progressive disease was observed in three patients (15%), while one had minimal response, four (20%) partial and 12 (60%) complete. Hematological toxicity was acceptable: three patients (15%) had neutrophils < 1,000/mm³; none had thrombocytopenia. Gastrointestinal toxicity was mild: nausea (10%), anorexia (15%) and no vomiting. CONCLUSIONS: This treatment has mild toxicity and good response rate. It may therefore be feasible before autologous bone marrow transplantation.

Os autores pesquisaram três marcas comerciais de silicones acéticos de fabricação nacional e os submeteram a testes de dureza "Shore A" e de resistência ao rasgamento, com o objetivo de verificar qual proporção desses materiais misturados ao silicone Silastic 4-4210 mais se aproximaria dos valores considerados ideais para as propriedades físicas dos silicones. Concluíram que, à medida em que a quantidade de Silastic 4-4210 era aumentada, as características físicas dos silicones acéticos estudados pioravam. The authors analyzed three commercial brands of acetic silicone made in Brazil, submitting them to tests for "Shore A" hardness and tear resistance, to ascertain which proportion of these materials should be mixed to the MDX 4 - 4210 Silastic rubber silicone in order to attain close to ideal physical properties. They conclude that physical properties of acetic silicones deteriorate with greater proportions of MDX 4 - 4210 Silastic. The latter, however, presents certain advantages when compared to the in natura materials due to a greater similarity with natural skin texture and to the greater flexibility of the material regarding the desired characteristics for use in the construction of facial prosthesis
