OBJETIVO: Traduzir para o português brasileiro e adaptar transculturalmente o Motor Activity Log (MAL), instrumento específico que avalia a função do membro superior mais afetado (MSMA) em hemiplégicos. MÉTODOS: O MAL foi traduzido e adaptado seguindo instruções padronizadas e submetido a exame de confiabilidade teste-reteste (coeficiente de correlação intraclasse, CCI). As demais propriedades psicométricas foram investigadas pela análise de Rasch em 77 hemiplégicos crônicos (41 homens, média de idade = 57,5 ± 12,4 anos). RESULTADOS: Tanto a escala de quantidade como a escala de qualidade do MAL obtiveram excelente CCI (0,98) para as pontuações totais. Quanto ao grau de dificuldade, "usar a chave para destrancar a porta" foi o item mais difícil na escala quantitativa, sendo "lavar as mãos" o item mais fácil. Na escala qualitativa, o mais difícil foi "utilizar o controle remoto da TV", e o mais fácil, "secar as mãos". A análise mostrou que o conjunto dos itens enquadrou-se no modelo; entretanto, quatro itens não se encaixaram nas expectativas do modelo nas escalas quantitativa (itens 21, 16, 14 e 13) e qualitativa (itens 9, 21, 23 e 22). Foram localizadas pessoas com padrão errático de respostas, e cinco indivíduos tiveram pontuação mínima. Constatou-se discrepância entre dificuldade dos itens e habilidade da amostra, indicando que as habilidades dos indivíduos estavam abaixo da dificuldade dos itens. Houve correlação significativa entre a força de preensão do MSMA e as medidas de habilidade dos indivíduos na escala quantitativa (r = 0,51; P & lt; 0,0001) e qualitativa (r = 0,57; P < 0,0001), e ainda entre as duas escalas nas medidas de habilidades dos indivíduos (r = 0,97; P < 0,0001). CONCLUSÕES: O MAL-Brasil apresenta potencial para avaliação do uso do MSMA de hemiplégicos crônicos brasileiros. Entretanto, apresenta limitação para uso em indivíduos com comprometimento severo do MSMA. Ainda, a validade de construto foi comprometida pela presença de itens erráticos. O MAL-Brasil deve ser aplicado em outras amostras para que sua validade seja mais amplamente investigada.

OBJECTIVE: To describe the translation into Brazilian Portuguese and cross-cultural adaptation of the Motor Activity Log (MAL), an instrument specifically designed to assess function of the more severely affected upper limb in hemiplegics. METHODS: The MAL was translated and adapted according to standardized procedures and submitted to test-retest reliability assessment (intraclass correlation coefficient, ICC). Other psychometric properties were investigated using Rasch analysis in 77 chronic hemiplegics (41 men, mean age = 57.5 ± 12.4 years). RESULTS: An excellent ICC (0.98) was obtained for the total scores of both the quantity and quality MAL scales. When assessing degree of difficulty, "using a key to unlock the door" was the most difficult item on the quantity scale, whereas "washing hands" was the easiest one. On the quality scale, the most difficult item was "using the TV remote control," and the easiest one was "drying hands." The analyses showed that the set of items as a whole fit into the model; however, the individual analyses indicated that four items did not meet the expectations of the model in both the quantity (items 21, 16, 14, and 13) and quality (items 9, 21, 23, and 22) scales. Irregular response patterns were observed, and five subjects obtained the minimum score. There was disagreement between item difficulty and sample ability, suggesting that the abilities of individuals were below the degree of difficulty of the assessed items. A significant correlation was observed between grip strength of the more severely affected upper limb and motor skill measurement among individuals on the quantity (r = 0.51, P < 0.0001) and quality scales (r = 0.57, P < 0.0001), and also between the two scales when measuring the individuals' motor skills (r = 0.97, P < 0.0001). CONCLUSIONS: MAL-Brazil is potentially useful to evaluate the more severely affected upper limb in Brazilian patients with chronic hemiplegia. However, the instrument has limitations for use with individuals with severe limb impairments. Also, construct validity was affected by the presence of irregular score patterns. MAL-Brazil should be applied to additional samples to further investigate its validity.

O objetivo deste estudo foi comparar hemiparéticos com e sem histórico de quedas nos últimos seis meses (caidores e não-caidores) segundo os componentes da Classificação Internacional de Funcionalidade, Incapacidade e Saúde. Dezesseis hemiparéticos não-caidores (62,3±16,6 anos), com tempo de acidente vascular encefálico (AVE) de 38,0±48,3 meses, e 16 caidores (61,0±17,0 anos), tempo de AVE de 48,0±50,7 meses, de ambos os sexos, comunitários e deambuladores, foram avaliados quanto a funções e estruturas do corpo (torque do músculo quadríceps do lado parético e escala de depressão geriátrica), atividade - velocidade de marcha (VM) natural e máxima, teste de levantar e caminhar cronometrado (timed up & go) e escala de equilíbrio de Berg - e participação (pelo perfil de saúde de Nottingham e escala de qualidade de vida específica para AVE). Os dados foram tratados estatisticamente, com nível de significância α<0,05. Não houve diferença significativa entre os grupos quanto à idade, sexo e tempo de AVE (0,56<p<0,82), nem quanto às demais caracteristicas medidas (torque, depressão, VM, equilíbrio e qualidade de vida (0,12<p<0,60). Portanto, hemiparéticos caidores apresentaram-se semelhantes aos não-caidores nos domínios funções e estruturas do corpo, atividades e participação. Fatores contextuais ambientais, não analisados, podem estar mais relacionados a episódio(s) de queda nesses indivíduos.

The aim of this study was to compare post-stroke hemiparetic subjects with and without history of falls over the last six months by using the International Classification of Functioning, Disability and Health (ICF) components. Sixteen community-dwelling hemiparetic subjects without history of falls (62.3±16.6 years, time since stroke 38±48.3 months), and 16 with history of fall(s) (60.9±17 years, time since stroke 47.9±50.8 months) of both sexes and able to walk, with or without assistive devices, were assessed as to ICF components body functions and structures (torque of the paretic limb quadriceps muscle and depression, by the geriatric depression scale), activities (natural and maximal gait speed, timed up-and-go test and Berg balance scale), and participation (by the Nottingham health profile and the stroke-specific quality of life scale). Groups were statistically compared at significance level α<0.05. No statistically significant differences were found between the groups as to age, gender or time since stroke (0.56<p<0.82), neither as to all measured features (torque, depression, gait speed, balance, health-related quality of life (0.12<p<0.60). Therefore, stroke subjects with history of falls were similar to those without history of falls in all measures related to the ICF, suggesting that non-analysed environmental factors might be more related to falls of hemiparetic post-stroke subjects.

INTRODUÇÃO: Levantar/sentar em uma cadeira são atividades de grande demanda mecânica e comumente alteradas em indivíduos hemiparéticos. OBJETIVOS: Determinar as características que distinguem os desfechos relacionados à biomecânica musculoesquelética durante as atividades de levantar/sentar em uma cadeira, enfatizando a abordagem clínica de hemiparéticos com incapacidades, a partir de uma revisão sistemática da literatura científica. METODOLOGIA: Uma ampla busca na literatura foi realizada nas bases de dados MEDLINE, CINAHL, EMBASE, PEDro, LILACS e SciELO, seguida por busca manual para selecionar estudos que reportaram a biomecânica musculoesquelética durante ambas as atividades de levantar/sentar em uma cadeira, em indivíduos hemiparéticos, sem restrição quanto ao idioma e publicados até dezembro/2007. RESULTADOS: Dos 432 estudos encontrados, apenas 11 reportaram dados relacionados com a biomecânica musculoesquelética durante ambas as atividades, os quais não atingiram a pontuação total dos parâmetros de qualidade. Além disso, a maioria dos estudos experimentais que compararam grupos diferentes não atingiu pontuações aceitáveis de qualidade metodológica (PEDro). Apenas um estudo comparou variáveis biomecânicas entre ambas as atividades, mas avaliaram apenas o tempo para desempenhá-las. Poucas variáveis biomecânicas foram investigadas, sendo a descarga de peso nos membros inferiores e a duração das atividades as mais reportadas. CONCLUSÃO: Há pouca informação a respeito da biomecânica musculoesquelética durante ambas as atividades de levantar/sentar em uma cadeira em hemiparéticos, portanto, nenhuma conclusão a respeito das características que distinguem cada atividade desempenhada por hemiparéticos pode ser obtida com os dados já publicados.

INTRODUCTION: Sit-to-stand and stand-to-sit are two of the most mechanically demanding activities undertaken in daily life and which are usually impaired in stroke subjects. OBJECTIVES: To determine the distinguishing characteristics in musculoskeletal biomechanical outcomes of the sit-to-stand and stand-to-sit activities with stroke subjects, with an emphasis on the clinical management of stroke disabilities, in a systematic review. MATERIAL AND METHODS: An extensive literature search was performed with the MEDLINE, CINAHL, EMBASE, PEDro, LILACS, and SciELO databases, followed by a manual search, to select studies on musculoskeletal biomechanical outcomes in both activities with stroke subjects, without language restrictions, and published until December/2007. RESULTS: Out of the 432 studies, only 11 reported biomechanical outcomes of both activities and none reached the total score on the selected quality parameters. The majority of the experimental studies which compared groups did not achieve acceptable scores on their methodological quality (PEDRo). The investigated conditions and interventions were also restricted. Only one study compared biomechanical outcomes between the activities, but only evaluated the time spent to perform them. Few musculoskeletal biomechanical outcomes have been investigated, being weight bearing on the lower limbs and duration of the activities the most investigated. CONCLUSION: There is little information regarding musculoskeletal biomechanical outcomes during these activities with stroke subjects and no definite conclusions can be drawn regarding the particularities of these outcomes on their performance with stroke survivors.
