Secondary tracheoesophageal puncture with a flexible transillumination device: a new technique and its outcomes

Abstract Objectives: This study aims to describe a new secondary tracheoesophageal puncture technique with voice prosthesis insertion under local anesthesia with a low-cost device and evaluate its outcome. Methods: With a transoral flexible transillumination device of aluminum of 2.3 mm with fiberoptic light, the esophagus’s anterior wall is visualized through the tracheostomy. A tracheoesophageal fistula is made with a blade and passed through the fistula. Then a bent trocar is introduced into the lumen using the device as a guidewire. Once in the lumen, a thin guide wire is passed through up to the mouth. The voice prosthesis is positioned with retrograde insertion. Then, the patient is discharged without hospitalization. Results: 15 patients submitted to this technique had a successful surgical outcome. There were no complications as pneumothorax, esophageal perforation, bleeding, or hospitalization. Conclusion: The new device is feasible under local anesthesia.

Association between atypical swallowing and malocclusions: a systematic review

ABSTRACT Objective: This systematic review aims to answer the following focus question: “Is there an association between atypical swallowing and malocclusions?”. Methods: Appropriate word combinations were chosen and tailored specifically for each of the following electronic databases: EMBASE, Latin American and Caribbean Literature in Health Sciences (LILACS), LIVIVO, PubMed/Medline, Scopus, Web of Science, and gray literature, without any restrictions, up to February 2021. According to the selection criteria, only cross-sectional studies were included. The following inclusion criteria were considered: a sample composed of children, adolescents, and adults; patients clinically diagnosed with atypical swallowing; patients with normal swallowing; and outcome of interest of atypical swallowing in patients with malocclusion. The data consisted of study characteristics, sample characteristics, results, and conclusion of each study. The risk of bias was assessed using the JBI Critical Appraisal Checklist for Analytical Cross-Sectional Studies, and the certainty of evidence was assessed using the GRADE tool. Results: 4,750 articles were identified. After a two-step selection, four studies were included. A higher frequency of distal occlusion, extreme maxillary overhang, and open bite was related to swallowing disorders; most studies pointed to posterior crossbite as a malocclusion more associated with atypical swallowing. All studies had a moderate to high risk of bias, and the certainty of evidence was very low. Conclusion: The results indicate that atypical swallowing is associated with malocclusions and that posterior crossbite is the main malocclusion found, but only in the young population (3-11 years). RESUMO Objetivo: A presente revisão sistemática tem como objetivo responder à seguinte questão focal: “Existe associação entre deglutição atípica e más oclusões?”. Métodos: Combinações de palavras e truncamentos apropriados foram adaptados para as bases de dados eletrônicas: EMBASE, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), LIVIVO, PubMed/Medline, Scopus e Web of Science e a literatura cinzenta, sem qualquer restrição, até fevereiro de 2021. Os critérios de inclusão foram: estudos transversais; amostra composta por crianças, adolescentes e adultos; pacientes com diagnóstico clínico de deglutição atípica; pacientes com deglutição normal, e o desfecho de interesse foi deglutição atípica em pacientes com má oclusão. Os dados extraídos de cada estudo foram características do estudo, da amostra e a conclusão. O risco de viés foi avaliado usando a Lista de verificação de avaliação crítica da JBI para estudos transversais analíticos, e a certeza das evidências foi avaliada usando a ferramenta GRADE. Resultados: Foram identificados 4.750 artigos. Após uma seleção em duas fases, quatro estudos foram incluídos. Maior frequência de disto-oclusão, protuberância maxilar extrema e mordida aberta foram relacionadas a distúrbios da deglutição, com a maioria dos estudos apontando para mordida cruzada posterior como a má oclusão mais associada à deglutição atípica. Todos os estudos tiveram um risco de viés moderado a alto, e a certeza das evidências foi considerada muito baixa. Conclusão: Os resultados indicam que a deglutição atípica está associada à mordida cruzada posterior apenas na população jovem de 3 a 11 anos.

Patterns of species diversity in different spatial scales and spatial heterogeneity on beta diversity

ABSTRACT Patterns of species diversity are essential to understand community structure. We aimed to determine species diversity and patterns of beta diversity in different spatial scales. We sampled three thousand individuals between the coordinates 22°10'S to 22°16'S and 47°47'W to 48°00'W to assess species diversity in three spatial scales (maximum distances of 80 m, 1,400 m, and 12,000 m), using the point-centered-quarter method. We partitioned gamma diversity into alpha and beta components. Beta diversity was partitioned into dissimilarities produced by spatial species turnover and nestedness. The contribution of beta diversity to gamma diversity was greater than that of alpha diversity in all scales, although the patterns of species diversity were similar for the evaluated scales, and was similar to that described for larger spatial scales. The sampled fragments presented means of 15 exclusive species and 47.5 species per fragment, and dissimilarities [β(SØR)=0.7] almost completely explained (94 %) by spatial species turnover. The results indicate that the remnant fragments are residual patches of an originally heterogeneous vegetation. The fragmentation processes could have progressed differently in each portion of the original vegetation, producing the current heterogeneous vegetation. Thus, there is a potential of high local species extinctions if the remnant fragments are deforested.

Resumo Um levantamento atualizado das plantas com sementes e análises relevantes acerca desta biodiversidade são apresentados. Este trabalho se iniciou em 2010 com a publicação do Catálogo de Plantas e Fungos e, desde então vem sendo atualizado por mais de 430 especialistas trabalhando online. O Brasil abriga atualmente 32.086 espécies nativas de Angiospermas e 23 espécies nativas de Gimnospermas e estes novos dados mostram um aumento de 3% da riqueza em relação a 2010. A Amazônia é o Domínio Fitogeográfico com o maior número de espécies de Gimnospermas, enquanto que a Floresta Atlântica possui a maior riqueza de Angiospermas. Houve um crescimento considerável no número de espécies e nas taxas de endemismo para a maioria dos Domínios (Caatinga, Cerrado, Floresta Atlântica, Pampa e Pantanal), com exceção da Amazônia que apresentou uma diminuição de 2,5% de endemicidade. Entretanto, a maior parte das plantas com sementes que ocorrem no Brasil (57,4%) é endêmica deste território. A proporção de formas de vida varia de acordo com os diferentes Domínios: árvores são mais expressivas na Amazônia e Floresta Atlântica do que nos outros biomas, ervas são dominantes no Pampa e as lianas apresentam riqueza expressiva na Amazônia, Floresta Atlântica e Pantanal. Este trabalho não só quantifica a biodiversidade brasileira, mas também indica as lacunas de conhecimento e o desafio a ser enfrentado para a conservação desta flora.
Abstract An updated inventory of Brazilian seed plants is presented and offers important insights into the country's biodiversity. This work started in 2010, with the publication of the Plants and Fungi Catalogue, and has been updated since by more than 430 specialists working online. Brazil is home to 32,086 native Angiosperms and 23 native Gymnosperms, showing an increase of 3% in its species richness in relation to 2010. The Amazon Rainforest is the richest Brazilian biome for Gymnosperms, while the Atlantic Rainforest is the richest one for Angiosperms. There was a considerable increment in the number of species and endemism rates for biomes, except for the Amazon that showed a decrease of 2.5% of recorded endemics. However, well over half of Brazillian seed plant species (57.4%) is endemic to this territory. The proportion of life-forms varies among different biomes: trees are more expressive in the Amazon and Atlantic Rainforest biomes while herbs predominate in the Pampa, and lianas are more expressive in the Amazon, Atlantic Rainforest, and Pantanal. This compilation serves not only to quantify Brazilian biodiversity, but also to highlight areas where there information is lacking and to provide a framework for the challenge faced in conserving Brazil's unique and diverse flora.

O Grupo Barreiras é uma cobertura sedimentar terrígena, de idade Miocênica a pleistocênica inferior, que tem grande ocorrência ao longo do litoral brasileiro. Na região de Conde, localizada na porção nordeste do Estado da Bahia, as características sedimentológicas desses depósitos são indicativas de uma deposição por sistemas fluviais de padrão entrelaçado, com carga de leito areno-cascalhosa, associados a leques aluviais, em condições de clima árido a semi-árido. A porção basal do grupo é dominada por litofácies areno-cascalhosas depositadas por fluxos de detritos e fluxos de detritos pseudoplásticos, com uma ocorrência menor de depósitos de fluxo subaquoso, caracterizando uma deposição em um sistema fluvial proximal. A porção superior é composta por sedimentos areno-cascalhosos que incluem depositos subaquosos, de fluxos de detritos e de fluxos de detritos verdadeiros. As características desses depósitos sugerem uma deposição em um contexto deposicional mais distal, em relação à porção basal do grupo, inferido por uma maior ocorrência de depósitos de fluxo aquoso e pela presença, ainda que rara, de macroformas de acrescimento para jusante. Além do controle climático, a deposição do Grupo Barreiras foi fortemente influenciada pelo tectonismo intraplaca, que vem afetando a plataforma sul-americana a partir do Mioceno Médio e que marca o início da ação neotectônica no Brasil.
The Barreiras Group is a Miocene to Lower Pleistocene continental terrigenous sedimentary deposit exhibiting a large occurrence along the Brazilian coast. In the Conde region, located in the northeastern part of the State of Bahia, the sedimentological characteristics of these sediments are indicative of a deposition as gravelly and sandy bed load in braided fluvial systems, related to alluvial fans, under an arid to semi-arid climate. The basal portion of the group is dominated by a gravelly-sandy lithofacies deposited by debris flows and pseudoplastic debris flows, with lesser occurrences of subaqueous deposits, characterizing a proximal fluvial system deposition. The upper portion is made up of gravelly-sandy sediments that include subaqueous, debris flows and pseudoplastic debris flows deposits. They suggest deposition in a more distal zone as indicated by the larger occurrence of subaqueous deposits and the presence, though rare, of downstream-accretion macroforms. Besides the climate control, deposition of the Barreiras Group was strongly influenced by the intraplate tectonism, which has been affecting the South America Platform since the Middle Miocene, when neotectonism began in Brazil.
