ABSTRACT Background Low back pain (LBP) is a common musculoskeletal condition among elders and is associated with falls. However, the underlying biological risk factors for falling among elders with LBP has been poorly investigated. The Physiological Profile Assessment (PPA) is a validated fall-risk assessment tool that involves the direct assessment of sensorimotor abilities and may contribute to the understanding of risk factors for falls among elders with LBP. Objective To assess fall risk using the PPA in elders with and without LBP. Method This is an observational, comparative, cross-sectional study with elders aged ≥65 years. The present study was conducted with a subsample of participants from the Back Complaints in the Elders (BACE) - Brazil study. Fall risk was assessed using the PPA, which contains five tests: visual contrast sensitivity, hand reaction time, quadriceps strength, lower limb proprioception, and postural sway. Results Study participants included 104 individuals with average age of 72.3 (SD=4.0) years, divided into two groups: GI) 52 participants with LBP; GII) 52 participants without LBP. The participants with LBP had a significantly higher fall risk (1.10 95% CI 0.72 to 1.48), greater postural sway (49.78 95% CI 13.54 to 86.01), longer reaction time (58.95 95% CI 33.24 to 84.65), and lower quadriceps strength (–4.42 95% CI –8.24 to –0.59) compared to asymptomatic participants. There was no significant difference for vision and proprioception tests between LBP and non-LBP participants. Conclusion Elders with LBP have greater risk for falls than those without LBP. Our results suggest fall-risk screening may be sensible in elders with LBP.

RESUMOObjetivou-se, neste trabalho, avaliar os atributos químicos de um solo cultivado com mamona (Ricinus communis L.) variedade BRS Energia, adubada com composto de lodo de esgoto e silicato de cálcio e magnésio. O experimento foi conduzido no ICA/UFMG em Cambissolo Háplico, em esquema fatorial 2 x 4, no delineamento em blocos casualizados com três repetições. Este arranjo consistiu em duas doses de silicato de cálcio e magnésio (0 e 1 t ha-1) e quatro doses de composto de lodo de esgoto (0; 23,81; 47,62 e 71,43 t ha-1, em base seca). Foram avaliados o teor de matéria orgânica, pH, Soma de Bases (SB), CTC efetiva (t), CTC potencial (T), saturação por base (V%) e a acidez potencial (H + Al) do solo. Não foram constatadas interações significativas entre as doses de composto de lodo de esgoto e as de silicato de cálcio e magnésio sobre os atributos edáficos do solo e tampouco houve efeito da adubação com silicato sobre esses atributos; no entanto, a adubação com composto de lodo de esgoto promoveu a redução do pH e o aumento da acidez potencial, do teor de matéria orgânica e da capacidade de troca catiônica, sendo a dose de composto de lodo de esgoto de 71,43 t ha-1 a que promoveu melhores condições químicas para o solo.

ABSTRACTThis study aimed to evaluate the chemical attributes of an Inceptisol cultivated with castor bean (Ricinus communis L.), variety ‘BRS Energia’, fertilized with sewage sludge compost and calcium (Ca) and magnesium (Mg) silicate. The experiment was conducted at the ICA/UFMG, in a randomized block design, using a 2 x 4 factorial scheme with three replicates, and the treatments consisted of two doses of Ca-Mg silicate (0 and 1 t ha-1) and four doses of sewage sludge compost (0, 23.81, 47.62 and 71.43 t ha-1, on dry basis). Soil organic matter (OM), pH, sum of bases (SB), effective cation exchange capacity (CEC(t)), total cation exchange capacity (CEC(T)), base saturation (V%) and potential acidity (H + Al) were evaluated. There were no significant interactions between doses of sewage sludge compost and doses of Ca-Mg silicate on soil attributes, and no effect of silicate fertilization on these attributes. However, fertilization with sewage sludge compost promoted reduction in pH and increase in H + Al, OM and CEC. The dose of 71.43 t ha-1 of sewage sludge compost promoted the best soil chemical conditions.

Introduction Guillain-Barre syndrome is an acute neuropathy that rarely compromises bladder function. Conservative management including clean intermittent catheterization and pharmacotherapy is the primary approach for hypocompliant contracted bladder. Surgical treatment may be used in refractory cases to improve bladder compliance and capacity in order to protect the upper urinary tract. We describe a case of pure laparoscopic augmentation ileocystoplasty in a patient affected by Guillain-Barre syndrome. Presentation A 15-year-old female, complaining of voiding dysfunction, recurrent urinary tract infection and worsening renal function for three months. A previous history of Guillain-Barre syndrome on childhood was related. A voiding cystourethrography showed a pine-cone bladder with moderate post-void residual urine. The urodynamic demonstrated a hypocompliant bladder and small bladder capacity (190mL) with high detrusor pressure (54 cmH2O). Nonsurgical treatments were attempted, however unsuccessfully. The patient was placed in the exaggerated Trendelenburg position. A four-port transperitoneal technique was used. A segment of ileum approximately 15-20cm was selected and divided with its pedicle. The ileal anastomosis and creation of ileal U-shaped plate were performed laparoscopically, without staplers. Bladder mobilization and longidutinal cystotomy were performed. Enterovesical anastomosis was done with continuous running suture. A suprapubic cystostomy was placed through a 5mm trocar. Results The total operative time was 335 min. The blood loss was minimal. The patient developed ileus in the early days, diet acceptance after the fourth day and was discharged on the seventh postoperative day. The urethral catheter was removed after 2 weeks. At 6-month follow-up, a cystogram showed a significant improvement in bladder capacity. The patient adhered well to clean intermittent self-catheterization and there was no report for febrile infections or worsening of renal function. We did not experience any complication related to the intestinal anastomosis fully prepared intracorporeally. Conclusions Albeit technically challenging, pure laparoscopic enterocystoplasty was feasible and safe. Preparing the enteral anastomosis and the pouch intracoporeally may prolong surgical time and contribute to postoperative ileus. Surgical staplers can assist in the procedure, however they are not essential.
