RESUMEN Introducción: La espondiloartritis axial es una enfermedad reumatológica que afecta a individuos jóvenes y tiene una gran repercusión sociolaboral. El retraso en el diagnóstico y el tratamiento se asocia con un mayor deterioro funcional y un impacto negativo en la calidad de vida, por lo que requiere un abordaje multidisciplinario. Objetivo: Desarrollar y formular un conjunto de recomendaciones específicas basadas en la mejor evidencia disponible para la detección temprana, el diagnóstico, el tratamiento y el seguimiento de los pacientes adultos con espondiloartritis axial. Métodos: Se configuró un grupo desarrollador, se formularon preguntas clínicas contestables, se graduaron los desenlaces y se realizó la búsqueda sistemática de la evidencia. El panel de la guía fue multidisciplinario (incluyendo representantes de los pacientes) y balanceado, minimizando el sesgo por conflictos de intereses. Se utilizó la aproximación Grading of Recommendations Assessment, Development and Evaluation (GRADE) para evaluar la calidad de la evidencia, al igual que la dirección y la fortaleza de las recomendaciones. Se presentan 11 recomendaciones relacionadas con diagnóstico (n = 2), tratamiento farmacológico (n = 6), tratamiento no farmacológico (n = 2) y seguimiento (n = 1). Resultados: Se recomienda la radiografía de articulaciones sacroilíacas como primer método diagnóstico, y el uso de escalas de actividad para el seguimiento de los pacientes (ASDAS o BASDAI). Los antiinflamatorios no esteroideos son la primera opción de tratamiento; en caso de intolerancia o dolor residual se recomienda acetaminofén u opioides. En pacientes con compromiso axial se recomienda abstenerse de utilizar medicamentos antirreumáticos modificadores de la enfermedad convencionales ni glucocorticoides sistémicos o locales. En pacientes con falla a los antiinflamatorios no esteroideos, se recomienda un anti-TNFα o un anti-IL17A. En pacientes con falla a anti-TNFα, se recomienda iniciar un anti-IL17A. El ejercicio y la terapia física y ocupacional se recomiendan como parte del tratamiento. Se recomienda no utilizar las terapias no convencionales como única opción de tratamiento. Conclusiones: Este conjunto de recomendaciones proporciona una guía actualizada sobre el diagnóstico y el tratamiento de la espondiloartritis axial.

Abstract Introduction: Axial spondyloarthritis is a rheumatic condition affecting young patients with social and occupational consequences. Diagnosis delay is associated with functional impairment and impact on quality of life, requiring a multidisciplinary approach. Objective: To develop a set of recommendations based on the best available evidence for the early detection, diagnosis, treatment, and monitoring of adult patients with axial spondy-loarthritis. Methods: A working group was established, questions were developed, outcomes were graded, and a systematic search for evidence was conducted. A multidisciplinary panel of members was established (including patient representatives), minimizing bias in relation to conflicts of interest. The GRADE approach "Grading of Recommendations Assessment, Development and Evaluation" was used to assess the quality of the evidence as well as the direction and strength of recommendations. In total, 11 recommendations on diagnosis (n=2), pharmacological treatment (n=6), non-pharmacological treatment (n=2) and monitoring (n=1) are presented. Results: Sacroiliac joint radiography as the first diagnostic method, and the use of disease activity scales for patient monitoring (ASDAS or BASDAI), are recommended. Nonsteroidal anti-inflammatory drugs are the first treatment option; in case of intolerance or residual pain, acetaminophen or opioids are recommended. In patients with axial involvement, it is recommended not to use conventional disease-modifying antirheumatic drugs or systemic or local glucocorticoids. In patients with failure to non-steroidal anti-inflammatory drugs, anti-TNF or anti-IL17A are recommended. In those patients presenting with anti-TNF failure, starting an anti-IL17A is recommended. Exercise, physical and occupational therapy are recommended as part of treatment. It is recommended not to use unconventional therapies as the only treatment option. Conclusions: This set of recommendations provides an updated guideline for the diagnosis, treatment, and monitoring of patients with axial spondyloarthritis.

Resumo A hiperuricemia é comum na doença renal crônica (DRC) e pode estar presente em até 50% dos pacientes que se apresentam para diálise. A hiperuricemia pode ser secundária ao comprometimento da taxa de filtração glomerular (TFG) que ocorre na DRC. No entanto, ela também pode preceder o desenvolvimento da doença renal e mesmo prever uma DRC incidente. Estudos experimentais de modelos hiperuricêmicos descobriram que tanto o ácido úrico solúvel quanto o cristalino podem causar danos renais significativos, caracterizados por isquemia, fibrose tubulointersticial e inflamação. Entretanto, a maioria dos estudos de randomização Mendeliana falhou em demonstrar uma relação causal entre o ácido úrico e a DRC, e os ensaios clínicos têm apresentado resultados variáveis. Aqui sugerimos explicações potenciais para os achados clínicos e genéticos negativos, incluindo o papel do ácido úrico cristalino, do ácido úrico intracelular e da atividade da xantina oxidase na lesão renal mediada por ácido úrico. Propomos ensaios clínicos futuros, bem como um algoritmo para o tratamento de hiperuricemia em pacientes com DRC.

Abstract Hyperuricemia is common in chronic kidney disease (CKD) and may be present in 50% of patients presenting for dialysis. Hyperuricemia can be secondary to impaired glomerular filtration rate (GFR) that occurs in CKD. However, hyperuricemia can also precede the development of kidney disease and predict incident CKD. Experimental studies of hyperuricemic models have found that both soluble and crystalline uric acid can cause significant kidney damage, characterized by ischemia, tubulointerstitial fibrosis, and inflammation. However, most Mendelian randomization studies failed to demonstrate a causal relationship between uric acid and CKD, and clinical trials have had variable results. Here we suggest potential explanations for the negative clinical and genetic findings, including the role of crystalline uric acid, intracellular uric acid, and xanthine oxidase activity in uric acid-mediated kidney injury. We propose future clinical trials as well as an algorithm for treatment of hyperuricemia in patients with CKD.

Abstract A review is carried out to examine the risk of patients suffering from diabetes mellitus in the context of general morbidity and mortality and related to infection by SARS-CoV-2. Likewise, the general recommendations for food and the prevention of comorbidities that most these patients suffer most frequently are also studied. Finally, a review of the pharmacological recommendations on both oral and parenteral treatment in the outpatient, in hospitalization and in critical states infected with SARS-CoV-2 is made.

Resumen Se realiza una revisión sobre el riesgo de los pacientes que padecen diabetes mellitus en el contexto de morbimortalidad general y relacionada a infección por el coronavirus 2 del síndrome respiratorio agudo grave (SARS-CoV-2). Así mismo se repasan las recomendaciones generales, de alimentación y de la prevención de las comorbilidades que más frecuentemente padecen dichos enfermos. Finalmente se hace una revisión de las recomendaciones farmacológicas sobre el tratamiento tanto oral como parenteral en el paciente ambulatorio, en la hospitalización y en estados críticos infectados por el SARS-CoV-2.

Guadua tuxtlensis, a new species from the Los Tuxtlas region, Catemaco and San Andrés Tuxtla, Veracruz, Mexico, is here described and illustrated. Guadua tuxtlensis is characterized by hollow culms, wall thickness with thick walls, thorny branches from the first node, culm leaves basally persistent and with conspicuous falcate-fimbriate auricles, foliage leaves sheath fimbriate at the summit, with their blades ovate-lanceolate and adaxially strongly veined. A comparison of the vegetative morphology of G. tuxtlensis, G. aculeata and G. inermis is presented, as is a key to the Guadua species of Mexico.

Se describe e ilustra Guadua tuxtlensis, una especie nueva de la región de Los Tuxtlas, de los Municipios de Catemaco y San Andrés Tuxtla, Estado de Veracruz, México. Guadua tuxtlensis se caracteriza por tener culmos huecos con espesores de las paredes del culmo gruesas, ramas con espinas desde el primer nudo, hoja caulinar basalmente persistente, con conspicuas aurículas falcadas-fimbriadas, vaina foliar apicalmente fimbriada, lamina foliar ovoide-lanceolada y adaxialmente muy nervada. En este estudio se presenta una comparación de la morfología vegetativa entre G. tuxtlensis, G. aculeata y G. inermis, y se incluye una clave de las especies del género Guadua para México.

The grid integration of photovoltaic (PV) solar generation systems has become the most important application of PV systems, gaining interest over traditional stand-alone systems. This trend is being increased mainly because of the strong support provided by governments of different countries, as investment subsidies and incentives that establish an economically attractive feed-in tariff (FIT) on power generated from this renewable energy source (RES). In Germany, this situation has caused the increase of the installed PV generation capacity in recent years. High penetration levels of this intermittent energy source can affect many aspects of the electric grid, and might cause undesirable effects, particularly in those moments when generation exceeds the load with reverse power flow through lines and transformers. This situation can be aggravated by the demographic behavior of some regions in Germany, with a population decreasing tendency affecting the load. This paper aims at analyzing the impact of the increasing penetration of PV generation on the grid, for the different regions in Germany, and its future behavior.

La generación solar fotovoltaica (FV) se ha implementado en dos formas representativas, como plantas de gran potencia y como generación distribuida, esta última ha crecido notablemente en la última década. La principal razón es el resultado de los sistemas de tarifas adoptados por países desarrollados. Alemania es uno de los países que mayor crecimiento ha experimentado, estableciendo un sistema de tarifas (feed-in) que hace económicamente atractiva la generación eléctrica a partir de fuentes de energía renovables (FERs). La capacidad instalada de generación FV en Alemania creció vertiginosamente, lo que podría traer problemas en la red por el exceso de generación. Un agravante a estos problemas es el comportamiento demográfico presente en Alemania, el cual tiende a decrecer en algunas zonas. Cuando la generación supera la carga se presentan efectos indeseados en la red, como por ejemplo los flujos inversos en las líneas y transformadores. En este trabajo se desarrolla un análisis a futuro de la situación en Alemania en relación con la generación FV y la carga, planteado diferentes escenarios posibles. El objetivo es determinar si existirá exceso de generación y en qué zonas.

INTRODUÇÃO: O tratamento percutâneo com cateter-balão e stents não-farmacológicos (SNF) de lesões na artéria descendente anterior (DA), quando comparado ao tratamento das outras artérias epicárdicas, freqüentemente cursa com taxas maiores de reestenose, comprometendo o resultado tardio da abordagem percutânea. OBJETIVO: Comparar a segurança e a eficácia tardia dos stents farmacológicos (SF) versus os SNF no tratamento de lesões únicas na DA. MÉTODO: Entre junho de 2002 e junho de 2003, 166 pacientes com lesão única em DA foram tratados com SF e comparados a uma coorte histórica de 141 pacientes tratados com SNF sob os mesmos critérios de inclusão/exclusão. Seguimento clínico tardio (aproximadamente dois anos) foi obtido em todos os pacientes. Objetivou-se comparar a taxa de eventos cardíacos adversos maiores (ECAM), definidos como morte cardíaca, infarto do miocárdio e revascularização da lesão-alvo (RLA), entre os dois grupos. RESULTADOS: Os grupos não diferiram quanto às características basais. O sucesso do procedimento foi obtido em todos os casos, não havendo eventos maiores na fase intra-hospitalar. Ao final de dois anos, 95,7% dos pacientes tratados com SF e 73,7% dos que receberam SNF estavam livres de ECAM (p < 0,0001). RLA ocorreu em 15,6% e 1,2% dos pacientes tratados com SNF e SF, respectivamente (p < 0,0001). Ainda que não significativa, a mortalidade cardíaca foi maior no grupo que recebeu SNF, comparativamente ao que recebeu SF (6,38% vs. 1,81%, respectivamente; p = 0,07). CONCLUSÃO: O implante de SF para o tratamento de lesões únicas na DA mostrou-se superior ao de SNF, reduzindo de forma significativa a taxa de ECAM, sobretudo a necessidade de nova RLA.

INTRODUCTION: Percutaneous treatment (PCI) of single lesions in the LAD with balloon angioplasty or bare-metal stent (BMS) often have poorer outcomes when compared to PCI of lesions in the other major coronary arteries. OBJECTIVE: To compare the long term safety and efficacy of DES vs. BMS for the treatment of single lesions in the LAD. METHODS: Between June 2002 and June 2003, 166 patients with single lesions in the LAD were treated with DES and compared to a historic cohort of 141 patients treated with BMS under the same inclusion/exclusion criteria. Long term clinical follow-up (approximately 2 years) was obtained in all patients. The primary endpoint was the comparison of major adverse cardiac events (MACE) defined as cardiac death, non fatal MI and target-lesion revascularization (TLR) between the two groups. RESULTS: Baseline clinical characteristics did not differ between groups. Procedure success was achieved in all cases with no in hospital MACE. After two years of follow-up, 95.7% of patients treated with DES and 73.7% treated with BMS were free from any MACE (p < 0.0001). TLR occurred in 15.6% and 1.2% of the patients receiving BMS and DES respectively (p < 0.0001). Although not significantly reduced, cardiac death was less frequent among patients treated with these DES (6.38% vs. 1.81%, p = 0.07). CONCLUSION: The use of DES for the treatment of single LAD lesions was superior to BMS due to a marked reduction in MACE rates, mainly at the expense of a very low need for RLA.

INTRODUÇÃO: No tratamento de obstruções coronárias longas e complexas recomenda-se cobertura total da lesão. Quando mais de um stent é implantado, torna-se mandatário um "overlapping" ou sobreposição dos stents para evitar espaços não cobertos entre eles. Há pouca evidência na literatura sobre a eficácia e segurança de se realizar sobreposição de stents liberadores de sirolimus (Cypher, SES) e paclitaxel (Taxus, SEP). OBJETIVO: Avaliar, por meio de estudo seriado com ultra-sonografia intravascular (USIC), a eficácia na redução da proliferação neointimal e as mudanças na parede vascular no segmento com sobreposição de stents farmacológicos SES e SEP. MÉTODO: Um total de 52 pacientes (72 lesões) foram prospectivamente incluídos nesta análise e randomizados para tratamento percutâneo com implante de SES ou SEP. Catorze pacientes do grupo SES e 12 do grupo SEP apresentaram segmentos de sobreposição. Angiografia coronária quantitativa e USIC foram realizados imediatamente após o implante dos stents e repetidos após oito meses. RESULTADOS: Ambos os grupos apresentavam características clínicas e angiográficas basais equivalentes. A média da razão stent/lesão foi similar nos dois grupos (1,74 ± 0,89 para SES vs 2,01 ± 0,9 para SEP, p= 0,47). No reestudo de oito meses, SES e SEP mostraram-se equivalentes em reduzir hiperplasia neointimal no segmento com sobreposição (volume neointimal 2,24 ± 0,9 mm³ entre os pacientes tratados com SES vs 2,53 ± 1,5 mm³ no grupo tratado com SEP, p=0,1 e % de obstrução neointimal de 18,15 ± 8,5% após SES vs. 26,7 ± 16,8% após SEP, p=0,1). Não houve remodelamento positivo significativo no segmento com sobreposição em ambos os grupos (taxa de expansão 0,74 ± 0,18 no grupo SES vs 0,76 ± 0,14 no grupo SEP, p=0,74). As demais medidas do USIC também foram equivalentes entre os dois grupos. CONCLUSÃO: Nesta experiência inicial, a sobreposição de stents farmacológicos com eluição de sirolimus e paclitaxel demonstrou ser eficaz em reduzir formação neointimal e, ao mesmo tempo, provou ser seguro, uma vez que não foram identificados sinais de toxicidade local.

INTRODUCTION: To treat long complex coronary obstructions, total lesion coverage is recommended. When more than one stent is deployed an overlap segment is mandatory to avoid uncovered gaps between stents. There is no data comparing Sirolimus- (SES) versus Paclitaxel-eluting stents (PES) at overlapping segments in regards to neointimal inhibition or toxic effects on the vessel wall. OBJECTIVE: To evaluate, by means of serial intravascular ultrasound (IVUS), the efficacy in reducing neointimal proliferation and the vessel changes in the overlapping segment comparing these two drug-eluting stents. METHOD: Fifty-two patients with 72 de novo coronary lesions were randomized for SES or PES. Fourteen patients in the SES Group and twelve in the PES Group had overlapping segments. Quantitative coronary angiography and IVUS were performed at the time of the procedure and at 8 months follow-up. RESULTS: No significant baseline differences were identified between the 2 groups. The mean stent/lesion ratios were similar (1.74 ± 0.89 for SES vs. 2.01 ± 0.92 for PES; p= 0.47). SES and PES were comparable in reducing neointima hyperplasia in the overlapping zone (neointima volume 2.24 ± 0.9 mm³ after SES vs. 2.53 ± 1.5 mm³ after PES; p=0.1 and % neointima obstruction of 18.15 ± 8.5% after SES vs. 26.7 ± 16.8% after PES; p=0.1). There was no positive remodeling in the overlapping segment for both groups (expansion ratio 0.74 ± 0.18 vs. 0.76 ± 0.14, respectively; p=0.74). Other IVUS volumetric measurements were also equivalent between the two cohorts. CONCLUSION: In our preliminary experience, overlapping of DESs proved to be efficient in reducing neointimal formation and also safe, since no sign of local toxicity was observed.
