RESUMO As residências em saúde da família demonstram avanços para o trabalho, sobretudo na Atenção Básica, que no contexto da pandemia da Covid-19 permanece como porta de entrada na rede de saúde. A resiliência de um sistema de saúde se refere à sua capacidade de absorver, adaptar, transformar e se manter funcionante quando submetido a uma crise. Objetivou-se analisar o papel dos residentes em saúde para a resiliência da Atenção Básica no contexto da Covid-19. O estudo, de caráter qualitativo e quantitativo, por meio de questionário on-line autoaplicável, contemplou como sujeitos as(os) residentes em saúde que atuaram na Atenção Básica durante a pandemia. Os dados observaram o perfil das(os) participantes e aspectos relativos à resiliência destas(es). Utilizaram-se o programa Epi Info e a análise temática de conteúdo para a análise dos dados. Participaram 289 residentes, a maioria mulheres, cisgênero, brancas. Identificaram-se as categorias analíticas: Contribuição para a continuidade das ações na Atenção Básica; Capacidade de resposta às necessidades da pandemia; e Inovações assistenciais e educativas. As(os) residentes parecem ter contribuído para a resiliência dos serviços de saúde na Atenção Básica durante a pandemia apesar das suas atividades não terem sido desenvolvidas integralmente, haja vista as limitações estruturais, financeiras, políticas e teórico-práticas.

ABSTRACT Family health residencies show advances for work, especially in Primary Care, which in the context of the COVID-19 pandemic remains as a gateway to the health network. The resilience of a health system refers to its ability to absorb, adapt, transform, and remain functional when subjected to a crisis. The objective was to analyze the role of health residents for the resilience of Primary Care in the context of COVID-19. The study, of a qualitative and quantitative nature, through a self-administered online questionnaire, included health residents who worked in Primary Care during the pandemic as subjects. The data observed the profile of the participants and aspects related to their resilience. The Epi Info program and thematic content analysis were used for data analysis. 289 residents participated, mostly women, cisgender, white. The analytical categories were identified: Contribution to the continuity of actions in Primary Care; Responsiveness to the needs of the pandemic; and Assistance and educational innovations. The residents seem to have contributed to the resilience of health services in Primary Care during the pandemic, despite their activities not being fully developed, given the structural, financial, political, and theoretical-practical limitations.

ABSTRACT Objective To verify whether the presence of related nutritional risk indicators prior to COVID-19 diagnosis is associated with poor survival in patients with cancer. Methods We retrospectively analyzed the data of hospitalized cancer patients who tested positive for COVID-19 between March 2020 and February 2021. Nutritional risk was defined as the presence of one of the following characteristics: body mass index <20kg/m 2 , scored Patient-generated Subjective Global Assessment ≥9 points or classification B, albumin level <3.5g/dL, and C-reactive protein level ≥10mg/L, evaluated between 7 and 60 days prior to the date of patient inclusion. The endpoint measure was all-cause mortality within 30 days of COVID-19 diagnosis. Results A total of 253 patients were included, most of whom were elderly (62.4%) and female (63.6%). Overall, 45.4% of the patients were at nutritional risk. Survival was significantly lower in patients at nutritional risk (8 days; interquartile range [IQR]: 3-29) than in patients not at nutritional risk (16 days; IQR: 6-30) (p<0.001). The presence of prior nutritional risk was associated with increased 30-day mortality (HR: 1.42; 95%CI: 1.03-1.94), regardless of age, gender, tumor site or stage, and other risk factors, and the model had good discrimination accuracy (concordance statistic: 0.744). Conclusion The presence of prior nutritional risk indicators is related to poor prognosis in patients with cancer and COVID-19, emphasizing the importance of nutritional care, notably during this pandemic.

Abstract Oral candidiasis is a common fungal infection that affects the oral mucosa, and happens when Candida albicans interacts with bacteria in the oral microbiota, such as Streptococcus mutans, causing severe early childhood caries. C. albicans and S. mutans mixed biofilms are challenging to treat with conventional antimicrobial therapies, thus, new anti-infective drugs are required. Objective This study aimed to test a drug delivery system based on chitosan microparticles loaded with geranium and lemongrass essential oils to inhibit C. albicans and S. mutans mixed biofilms. Methodology Chitosan microparticles loaded with essential oils (CM-EOs) were obtained by spray-drying. Susceptibility of planktonic were performed according CLSI at 4 to 2,048 µg/mL. Mixed biofilms were incubated at 37ºC for 48 h and exposed to CM-EOs at 256 to 4,096 µg/mL. The antimicrobial effect was evaluated using the MTT assay, with biofilm architectural changes analyzed by scanning electron microscopy. RAW 264.7 cell was used to evaluate compound cytotoxicity. Results CM-EOs had better planktonic activity against C. albicans than S. mutans. All samples reduced the metabolic activity of mixed C. albicans and S. mutans biofilms, with encapsulated oils showing better activity than raw chitosan or oils. The microparticles reduced the biofilm on the slides. The essential oils showed cytotoxic effects against RAW 264.7 cells, but encapsulation into chitosan microparticles decreased their toxicity. Conclusion This study demonstrates that chitosan loaded with essential oils may provide an alternative method for treating diseases caused by C. albicans and S. mutans mixed biofilm, such as dental caries.

Abstract Objective: To provide guidelines on the coronavirus disease 2019 (COVID-19) vaccination in patients with immune-mediated rheumatic diseases (IMRD) to rheumatologists considering specific scenarios of the daily practice based on the shared-making decision (SMD) process. Methods: A task force was constituted by 24 rheumatologists (panel members), with clinical and research expertise in immunizations and infectious diseases in immunocompromised patients, endorsed by the Brazilian Society of Rheumatology (BSR), to develop guidelines for COVID-19 vaccination in patients with IMRD. A consensus was built through the Delphi method and involved four rounds of anonymous voting, where five options were used to determine the level of agreement (LOA), based on the Likert Scale: (1) strongly disagree; (2) disagree, (3) neither agree nor disagree (neutral); (4) agree; and (5) strongly agree. Nineteen questions were addressed and discussed via teleconference to formulate the answers. In order to identify the relevant data on COVID-19 vaccines, a search with standardized descriptors and synonyms was performed on September 10th, 2021, of the MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, ClinicalTrials.gov, and LILACS to identify studies of interest. We used the Newcastle–Ottawa Scale to assess the quality of nonrandomized studies. Results: All the nineteen questions-answers (Q&A) were approved by the BSR Task Force with more than 80% of panelists voting options 4—agree—and 5—strongly agree—, and a consensus was reached. These Guidelines were focused in SMD on the most appropriate timing for IMRD patients to get vaccinated to reach the adequate covid-19 vaccination response. Conclusion: These guidelines were developed by a BSR Task Force with a high LOA among panelists, based on the literature review of published studies and expert opinion for COVID-19 vaccination in IMRD patients. Noteworthy, in the pandemic period, up to the time of the review and the consensus process for this document, high-quality evidence was scarce. Thus, it is not a substitute for clinical judgment.

Objective to understand the reasons that support the practice of double working hours in nursing. Method qualitative research, conducted with 30 nursing workers with double working hours practiced in public and/or private sector institutions, in nursing care and/or management activities, recruited by the snowball technique, between January and March 2019, in Eunápolis, Bahia, Brazil. Data were collected by applying a questionnaire and semi-structured interview, processed by Iramuteq® and submitted to lexical analysis through Descending Hierarchical Classification. Results two classes emerged: The precariousness of nursing work; and Aspirations and needs of nursing workers. Final Thoughts the practice of double working hours in nursing is related to the strong influence of the neoliberal model on the nursing work process, which does not offer the worker adequate material remuneration according to the activities performed and is also related to the cultural issue resulting from the ease of reconciling multiple jobs.

Objetivo entender las razones que apoyan la práctica de doble jornada laboral en enfermería. Método investigación cualitativa, realizada con 30 trabajadores de enfermería con doble jornada de trabajo practicada en instituciones del sector público y/o privado, en cuidados de enfermería y/o actividades de gestión, captadas por la técnica de bola de nieve, entre enero y marzo de 2019, en Eunápolis, Bahía, Brasil. Los datos se recopilaron aplicando un cuestionario y una entrevista semiestructurada, procesada por Iramuteq® y sometidas a análisis léxicos a través de la Clasificación Jerárquica Descendente. Resultados surgieron dos clases: La precariedad del trabajo de enfermería; y aspiraciones y necesidades de los trabajadores de enfermería. Consideraciones finales la práctica de la doble jornada de trabajo en enfermería está relacionada con la fuerte influencia del modelo neoliberal en el proceso de trabajo de enfermería, que no ofrece al trabajador una remuneración material adecuada de acuerdo con las actividades realizadas y también está relacionada con la cuestión cultural resultante de la facilidad de conciliación de múltiples puestos de trabajo.

Método pesquisa qualitativa, realizada com 30 trabalhadores de enfermagem com dupla jornada de trabalho praticada em instituições do setor público e/ou privado, em atividades assistenciais e/ou gerenciais de enfermagem, captados pela técnica snowball, entre janeiro e março de 2019, em Eunápolis, Bahia, Brasil. Os dados foram coletados mediante aplicação de questionário e entrevista semiestruturada, processados pelo Iramuteq® e submetidos à análise lexical via Classificação Hierárquica Descendente. Resultados emergiram duas classes: A precarização do trabalho de enfermagem; e Aspirações e necessidades dos trabalhadores de enfermagem. Considerações Finais a prática da dupla jornada de trabalho na enfermagem está relacionada à forte influência do modelo neoliberal sobre o processo de trabalho da enfermagem, que não oferece ao trabalhador a retribuição material adequada em função das atividades desempenhadas e também se relaciona à questão cultural decorrente da facilidade de conciliar múltiplos empregos.

Resumo Objetivo compreender por meio da lexicografia básica os vocabulários mais frequentes expressos por trabalhadores de enfermagem sobre a experiência de ter uma dupla jornada de trabalho. Método pesquisa qualitativa, realizada com 30 trabalhadores de enfermagem com dupla jornada de trabalho captados pela técnica snowball, entre janeiro e março de 2019, na cidade de Eunápolis, Bahia, Brasil. Os dados foram obtidos por meio de uma entrevista semiestruturada, processados com o auxílio do software Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires (IRAMUTEQ®). A partir da nuvem de palavras deu-se a análise lexical. Resultado os léxicos mais frequentes identificados no corpus foram “não” e “gente”. Conclusão e implicações para a prática os vários “nãos” evocados pelos participantes apontam que a experiência de trabalho é marcada por negatividade, restrições e sofrimento, fato que coloca em risco a saúde dessa “gente”, ou seja, desse coletivo profissional, suas relações sociais e até mesmo a qualidade da assistência prestada ao cliente. A compreensão desse contexto é fundamental para produzir reflexões que contribuam para a construção de políticas públicas voltadas à valorização e proteção dos trabalhadores de enfermagem bem como para reforçar a indispensável necessidade da enfermagem se fortalecer como classe trabalhadora.

Abstract Objective To understand, through basic lexicography, the most frequent words expressed by nurses on the experience of working double shifts. Method A qualitative research study carried out with 30 nurses working double shifts, captured by the snowball technique, between January and March 2019, in the city of Eunápolis, state of Bahia, Brazil. Data was obtained through a semi-structured interview and processed with the aid of the Interface de R pour les Analyses Multidimensionnelles de Textes et de Questionnaires (IRAMUTEQ®) software. From the word cloud, lexical analysis took place. Result The most frequent words identified in the corpus were “no” and “we”. Conclusion and implications for practice The many “nos” evoked by the participants point out that the experience is marked by negativity, restrictions and suffering, a fact that puts the health of the “we” at risk, that is, of this professional group, their social relationships, and even the quality of the assistance provided to the customer. Understanding this context is essential to produce reflections that contribute to the creation of public policies aimed at valuing and protecting nurses, as well as to reinforce the indispensable need for Nursing to strengthen as a working class.

Resumen Objetivo Comprender, a través de la lexicografía básica, los vocabularios más frecuentes expresados por los trabajadores de enfermería sobre la experiencia de tener una doble jornada laboral. Método Investigación cualitativa, realizada con 30 trabajadores de enfermería con doble jornada capturada por la técnica de bola de nieve, entre enero y marzo de 2019, en la ciudad de Eunápolis, Bahía, Brasil. Los datos se obtuvieron a través de una entrevista semiestructurada, procesada con la ayuda del software Interface de pour pour Analyzes Multidimensionnelles de Textes et de Questionnaires (IRAMUTEQ®). A partir de la nube de palabras, se realizó el análisis léxico. Resultado Los léxicos más frecuentes identificados en el corpus fueron “no” y “personas”. Conclusión e implicaciones para la práctica los distintos “no” evocados por los participantes señalan que la experiencia laboral está marcada por la negatividad, las restricciones y el sufrimiento, hecho que pone en riesgo la salud de estas “personas”, es decir, de este grupo professional, su relaciones sociales e incluso la calidad de la asistencia prestada al cliente. Comprender este contexto es fundamental para producir reflexiones que contribuyan a la construcción de políticas públicas orientadas a la valoración y protección de los trabajadores de enfermeira, así como a reforzar la indispensable necesidad de que la enfermería se fortalezca como clase trabajadora.

Abstract The impact of herbal salt as a natural antioxidant on the lipids of roasted Dolphinfish (Coryphaena hippurus Linnaeus, 1758), as well as in the sensory characteristics of this food was evaluated after using refined or herbal salts, by analyzing the differences in its fatty acid contents, thiobarbituric acid reactive substances (TBARS), product acceptability and consumption intent. Centesimal composition, total phenols and characterization of phenolic compounds were also determined. The cooking caused a significant increase (p < 0.05) in the protein, lipid, ash, whose percentages were higher in fish salt roasted with herbs, compared to fish roasted with refined salt, except for ash. Rutin phenolic compound presented higher concentrations in fish treated with the three herbs. After cooking, the total monounsaturated and polyunsaturated fatty acids of herbal salt-roasted Dolphinfish increased by 128% and 109% compared to refined salt-roasted. TBARS values for salt-roasted gold were 558% higher than in natura and for herbal salt-roasted gold they corresponded to 174%. These results show that the phenolic compounds detected in herbs exerted an antioxidant effect on the preservation of monounsaturated and polyunsaturated fatty acids from fish roasted with herbal salt. Sensory analysis resulted in good acceptability and purchase intent for herbal salt-roasted Dolphinfish.

RESUMO Determinou-se a frequência e a concordância entre diferentes critérios de sarcopenia em idosas comunitárias. Trata-se de um estudo transversal com 64 idosas, no qual determinou-se a massa muscular pela análise de impedância bioelétrica (BIA) e cálculo do índice muscular esquelético, a força muscular foi determinada pelo dinamômetro de preensão palmar, a capacidade funcional pelos testes Timed Up and Go (TUG) e Sit-to-Stand test (STS). Os idosos foram classificados em não sarcopênicos, pré-sarcopênicos, sarcopênicos moderados e graves utilizando diferentes critérios de sarcopenia. Os dados foram analisados utilizando teste qui-quadrado e estatística Kappa. Obteve-se como resultado que 37,5% das idosas apresentaram baixa massa muscular, 34,4%, fraqueza muscular, 3,1% apresentaram incapacidade funcional para levantar e andar e 25,9% para sentar e levantar da cadeira. Considerando apenas a massa muscular, 37,5% da amostra foi classificada como sarcopênica (moderada ou grave) e, considerando os critérios do European Working Groups on Sarcopenia in Older People, 15,6% obteve essa classificação ao avaliar a capacidade funcional com TUG e 22,4% com STS. A concordância entre as definições de sarcopenia variou de moderada a excelente (p<0,001). Entre as idosas com integridade da massa muscular (n=40), 30% apresentaram fraqueza de preensão palmar, 2,5% incapacidade no TUG e 25,7% no STS. Foi concluído que as idosas apresentaram alta frequência de sarcopenia independentemente do critério utilizado. Observou-se moderada a excelente concordância entre os critérios de sarcopenia investigados. A identificação de idosos com integridade da massa muscular coexistindo com fraqueza muscular e incapacidade funcional reforça a importância da avaliação dos três parâmetros no cenário clínico-científico.

ABSTRACT The frequency and concordance between different sarcopenia criteria was assessed in community women of age. This is a cross-sectional study with 64 women of age, in which muscle mass was determined by bioelectrical impedance analysis (BIA) and skeletal muscle index calculation, muscle strength was determined with a handgrip strength dynamometer, functional capacity was determined with the tests Timed Up and Go (TUG) and Sit to Stand test (STS). This older population was classified in sarcopenic, pre-sarcopenic, moderate sarcopenic and severe sarcopenic by using different sarcopenia criteria. The data were analyzed with use of Chi-square test and Kappa statistics. The results obtained demonstrated that 37.5% of the women suffered of low muscle mass, 34.4% of muscle weakness, 3.1% of functional impairment to stand and walk and 25.9% to sit and raise from the chair. Considering only muscle mass, 37.5% of the sample was classified as sarcopenic (moderate or severe) and, considering the criteria of the European Working Groups on Sarcopenia in Older People, 15.6% obtained this classification when assessing functional capacity with TUG and 22.4% with STS. Concordance between sarcopenia definitions ranged from moderate to excellent (p<0.001). Among women with muscle mass integrity (n=40), 30% showed low handgrip strength, 2.5% showed impairment in TUG and 25.7% in STS. It was concluded that the women presented high frequency of sarcopenia, regardless of the criteria used. Moderate to excellent concordance was observed between the sarcopenia criteria investigated. Identification of older people with muscle mass integrity coexisting with muscle weakness and functional impairment reinforces the importance of the evaluation of the three parameters in the clinical scientific setting.

RESUMEN Se determinó la frecuencia y la concordancia entre distintos criterios de sarcopenia en ancianas comunitarias. Se trata de un estudio transversal con 64 ancianas, en el cual se determinó la masa muscular por el análisis de impedancia bioeléctrica (BIA) y cálculo del índice muscular esquelético, la fuerza muscular fue determinada por el dinamómetro de prensión palmar, la capacidad funcional por las pruebas Timed Up and Go (TUG) y Sit-to-Stand test (STS). Los ancianos fueron clasificados en no sarcopénicos, pre-sarcopénicos, sarcopénicos moderados y graves utilizando distintos criterios de sarcopenia. Los datos fueron analizados utilizando prueba chi cuadrada y estadística Kappa. Se obtuvo como resultado que el 37,5% de las ancianas presentaron baja masa muscular, el 34,4%, debilidad muscular, el 3,1% presentaron incapacidad funcional para levantarse y caminar y el 25,9% para sentarse y levantarse de la silla. Considerando solamente la masa muscular, el 37,5% de la muestra fue clasificada como sarcopénica (moderada o grave) y, considerando los criterios del European Working Groups on Sarcopenia in Older People, el 15,6% obtuvo esa clasificación al evaluar la capacidad funcional con TUG y el 22,4% con STS. La concordancia entre las definiciones de sarcopenia varió de moderada a excelente (p<0,001). Entre las ancianas con integridad de la masa muscular (n=40), el 30% presentaron debilidad de prensión palmar, el 2,5% incapacidad en el TUG y el 25,7% en el STS. Fue concluido que las ancianas presentaron alta frecuencia de sarcopenia independientemente del criterio utilizado. Se observó moderada la excelente concordancia entre los criterios de sarcopenia investigados. La identificación de ancianos con integridad de la masa muscular coexistiendo con debilidad muscular e incapacidad funcional refuerza la importancia de la evaluación de los tres parámetros en el escenario clínico-científico.

ABSTRACT The use of dark septate fungi (DSE) to promote plant growth can be beneficial to agriculture, and these organisms are important allies in the search for sustainable agriculture practices. This study investigates the contribution of dark septate fungi to the absorption of nutrients by rice plants and their ensuing growth. Four dark septate fungi isolates that were identified by Internal transcribed spacer phylogeny were inoculated in rice seeds (Cv. Piauí). The resulting root colonization was estimated and the kinetic parameters Vmax and Km were calculated from the nitrate contents of the nutrient solution. The macronutrient levels in the shoots, and the NO3--N, NH4+-N, free amino-N and soluble sugars in the roots, sheathes and leaves were measured. The rice roots were significantly colonized by all of the fungi, but in particular, isolate A103 increased the fresh and dry biomass of the shoots and the number of tillers per plant, amino-N, and soluble sugars as well as the N, P, K, Mg and S contents in comparison with the control treatment. When inoculated with isolates A103 and A101, the plants presented lower Km values, indicating affinity increases for NO3--N absorption. Therefore, the A103 Pleosporales fungus presented the highest potential for the promotion of rice plant growth, increasing the tillering and nutrients uptake, especially N (due to an enhanced affinity for N uptake) and P.

Resumo Introdução: A adoção do estilo de vida saudável pelo idoso a partir da prática de exercício físico regular está vinculada à independência física e ao bem-estar social. A crescente inserção da tecnologia, inclusive no âmbito da saúde do idoso, amplia de forma inovadora e interativa a abordagem perante os aspectos próprios do envelhecimento. Objetivo: Relatar a elaboração do aplicativo “Idoso Ativo” com proposta de exercícios para membros inferiores para a população idosa. Métodos: O estudo é do tipo artigo original que descreve como foi o desenvolvimento e realização do aplicativo para a saúde do idoso. O estudo teve a participação de uma modelo idosa para realização dos exercícios e gravação em vídeo. O estudo teve aprovação do Comitê de Ética em Pesquisa da UCB e apoio do CNPQ. Resultados: A proposta de desenvolvimento do software envolveu, por meio de um trabalho interdisciplinar, a seleção de exercícios para postura, equilíbrio e marcha, elaboração do aplicativo, além da aplicação de uma linguagem de programação específica com apresentação de exercícios em telas de fácil compreensão para o público idoso. Conclusão: O aplicativo desenvolvido pode ser adotado como proposta em estudos científicos na área da fisioterapia e ser aplicado como recurso inovador aliando promoção de saúde e prevenção de agravos.

Abstract Introduction: The introduction of a healthy lifestyle for the elderly from regular physical exercise is linked to physical independence and social wellness. The increasing integration of technology, including in the elderly’s health area, extends an innovative and interactive approach towards the specific aspects of aging. Objective: To report the development of the application "Idoso Ativo” (Active Aging) with proposed exercises for the elderly population’s lower limbs. Methods: The study belongs to the type ‘original paper’ which describes the development and accomplishment of the application for the health of the elderly. The study had the participation of an elderly model to perform exercises and video recording. The study was approved by the Research Ethics Committee of UCB and supported by CNPQ. Results: The Software development proposal involved interdisciplinary work for the selection of exercises for posture, balance and gait, development of the application and the use of a specific programming language with presentation of exercises on screens for easy understanding for the elderly public. Conclusion: The application developed can be adopted as a proposal in scientific studies in the physical therapy area and applied as an innovative resource combining health promotion and disease prevention.

RESUMO A ocupação urbana intensa e não planejada acarreta problemas diretos à população. Um desses problemas é a diminuição do índice de permeabilidade do solo gerada pela pavimentação crescente que sempre acompanha o desenvolvimento urbano de uma região. Em consequência, o sistema de drenagem pode sofrer sobrecargas uma vez que a capacidade de infiltração do solo é alterada e a pavimentação aumenta a velocidade de escoamento e as vazões de pico. Este trabalho apresenta uma análise integrada da dinâmica do uso do solo ao longo de 32 anos e do comportamento do sistema de microdrenagem urbana para o cenário atual de ocupação dos bairros Catolé e Sandra Cavalcante, em Campina Grande (PB). Verifica-se a deficiência do sistema de microdrenagem em não acompanhar o acelerado crescimento da região, acarretando em pontos de alagamento que geram prejuízos e desconfortos à população em períodos de chuva mais concentrada.

ABSTRACT The intense and unplanned urban occupation entails direct problems to the population. One of these problems is the decrease in the permeability index of soil generated by increasing paving that always accompanies the urban development of a region. As a result, the drainage system may experience overloads, once the infiltration capacity of the soil is changed and the paving increases the flow rate and the peak flows. This paper presents an integrated analysis of land use dynamics over the course of 32 years and the behavior of the urban micro-drainage system to the current scenario of occupation of Catolé and Sandra Cavalcante neighborhoods, in Campina Grande, PB. It verifies the disability of the micro-drainage system to accompany the accelerated growth of the region, leading in points of flooding that generate losses and discomforts to the population in more concentrated rain periods.

Resumo A febre chikungunya tem se tornado um importante problema de saúde pública nos países onde ocorrem as epidemias, visto que metade dos casos evolui com artrite crônica, persistente e incapacitante. Os dados na literatura sobre terapêuticas específicas nas diversas fases da artropatia ocasionada pela infecção pelo vírus chikungunya (CHIKV) são limitados, não existem estudos randomizados de qualidade que avaliem a eficácia das diferentes terapias. Há algumas poucas publicações sobre o tratamento das manifestações musculoesqueléticas da febre chikungunya, porém com importantes limitações metodológicas. Os dados atualmente disponíveis não permitem conclusões favoráveis ou contrárias a terapêuticas específicas, bem como uma adequada avaliação quanto à superioridade entre as diferentes medicações empregadas. O objetivo deste trabalho foi elaborar recomendações para o tratamento da febre chikungunya no Brasil. Foi feita uma revisão da literatura com seleção de artigos baseados em evidência, nas bases de dados Medline, SciELO, PubMed e Embase e de resumos de anais de congressos, além da opinião dos especialistas para dar apoio às decisões tomadas para definir as recomendações. Para a definição do grau de concordância foi feita uma metodologia Delphi, em duas reuniões presenciais e várias rodadas de votação on line. Este artigo refere-se à parte 2 das Recomendações da Sociedade Brasileira de Reumatologia para Diagnóstico e Tratamento da Febre Chikungunya, que trata especificamente do tratamento.

Abstract Chikungunya fever has become an important public health problem in countries where epidemics occur because half of the cases progress to chronic, persistent and debilitating arthritis. Literature data on specific therapies at the various phases of arthropathy caused by chikungunya virus (CHIKV) infection are limited, lacking quality randomized trials assessing the efficacies of different therapies. There are a few studies on the treatment of musculoskeletal manifestations of chikungunya fever, but these studies have important methodological limitations. The data currently available preclude conclusions favorable or contrary to specific therapies, or an adequate comparison between the different drugs used. The objective of this study was to develop recommendations for the treatment of chikungunya fever in Brazil. A literature review was performed via evidence-based selection of articles in the databases Medline, SciELO, PubMed and Embase and conference proceedings abstracts, in addition to expert opinions to support decision-making in defining recommendations. The Delphi method was used to define the degrees of agreement in 2 face-to-face meetings and several online voting rounds. This study is part 2 of the Recommendations of the Brazilian Society of Rheumatology (Sociedade Brasileira de Reumatologia - SBR) for the Diagnosis and Treatment of chikungunya fever and specifically addresses treatment.

Resumo A febre chikungunya tem se tornado um importante problema de saúde pública nos países onde ocorrem as epidemias. Até 2013, as Américas haviam registrado apenas casos importados quando, em outubro desse mesmo ano, foram notificados os primeiros casos na Ilha de Saint Martin, no Caribe. No Brasil, os primeiros relatos autóctones foram confirmados em setembro de 2014 e até a semana epidemiológica 37 de 2016 já haviam sido registrados 236.287 casos prováveis de infecção pelo chikungunya vírus (CHIKV), 116.523 confirmados sorologicamente. As mudanças ambientais causadas pelo homem, o crescimento urbano desordenado e o número cada vez maior de viagens internacionais têm sido apontados como os fatores responsáveis pela reemergência de epidemias em grande escala. Caracterizada clinicamente por febre e dor articular na fase aguda, em cerca de metade dos casos existe evolução para a fase crônica (além de três meses), com dor persistente e incapacitante. O objetivo deste trabalho foi elaborar recomendações para diagnóstico e tratamento da febre chikungunya no Brasil. Para isso, foi feita revisão da literatura nas bases de dados Medline, SciELO e PubMed, para dar apoio às decisões tomadas para definir as recomendações. Para a definição do grau de concordância foi feita uma metodologia Delphi, em duas reuniões presenciais e várias rodadas de votação on line. Foram geradas 25 recomendações, divididas em três grupos temáticos: (1) diagnóstico clínico, laboratorial e por imagem; (2) situações especiais e (3) tratamento. Na primeira parte estão os dois primeiros temas e o tratamento na segunda.

Abstract Chikungunya fever has become a relevant public health problem in countries where epidemics occur. Until 2013, only imported cases occurred in the Americas, but in October of that year, the first cases were reported in Saint Marin island in the Caribbean. The first autochthonous cases were confirmed in Brazil in September 2014; until epidemiological week 37 of 2016, 236,287 probable cases of infection with Chikungunya virus had been registered, 116,523 of which had serological confirmation. Environmental changes caused by humans, disorderly urban growth and an ever-increasing number of international travelers were described as the factors responsible for the emergence of large-scale epidemics. Clinically characterized by fever and joint pain in the acute stage, approximately half of patients progress to the chronic stage (beyond 3 months), which is accompanied by persistent and disabling pain. The aim of the present study was to formulate recommendations for the diagnosis and treatment of Chikungunya fever in Brazil. A literature review was performed in the MEDLINE, SciELO and PubMed databases to ground the decisions for recommendations. The degree of concordance among experts was established through the Delphi method, involving 2 in-person meetings and several online voting rounds. In total, 25 recommendations were formulated and divided into 3 thematic groups: (1) clinical, laboratory and imaging diagnosis; (2) special situations; and (3) treatment. The first 2 themes are presented in part 1, and treatment is presented in part 2.
