ABSTRACT Purpose To correlate the dysphagia quality of life and symptoms of anxiety and depression before and after thyroidectomy. Methods Observational, longitudinal, prospective, and experimental study. Twenty patients participated, with a mean age of 54 years, prevalence of females (n=17; 85%) and partial thyroidectomy (n=14; 70%). All subjects underwent laryngeal visual examination and answered the MD Anderson Dysphagia Questionnaire (MDADI) and the Hospital Anxiety and Depression Scale (HADS) in three different moments: preoperatively, immediately postoperatively (maximum one week) and three months after surgery. Results There was a significant difference in dysphagia quality of life for the physical and total domains in the three different moments. Regarding anxiety and depression, a statistically significant difference was observed between the scores in all domains, with a greater difference observed between the preoperative period and after 1 week. Higher values were observed in the preoperative period for mild anxiety traits, being more frequent in relation to depression, with a reduction after 1 week and an increase after three months of surgery. There was no significant correlation between the MDADI and HADS protocols. Conclusion Patients undergoing thyroidectomy self-report better quality of life in dysphagia and reduced anxiety/depression scores after three months of surgery. There was no correlation between anxiety, depression and quality of life in dysphagia at the moments evaluated.

RESUMO Objetivo Correlacionar a qualidade de vida em disfagia e sintomas de ansiedade e depressão pré e pós-tireoidectomia. Método Estudo observacional, longitudinal, prospectivo e quantitativo. Participaram 20 pacientes, com média de idade de 54 anos, maior prevalência do sexo feminino (n=17; 85%) e de tireoidectomia parcial (n=14; 70%). Todos realizaram exame visual laríngeo e responderam ao Questionário de Disfagia M. D. Anderson (MDADI) e à Escala Hospitalar de Ansiedade e Depressão (HADS), no pré-operatório, pós-operatório recente (máximo uma semana) e três meses pós-operatório. Resultados Houve diferença significante na qualidade de vida em disfagia, para os domínios físico e total, nos três momentos. Quanto à ansiedade e depressão foi observada diferença estatisticamente significante entre as pontuações em todos os domínios, com maior diferença observada entre o pré-operatório e pós-operatório recente. Foram observados valores mais altos no pré-operatório para traços leves de ansiedade, sendo mais frequentes em relação à depressão, com redução após 1 semana e aumento após três meses de cirurgia. Não houve correlação significante entre os protocolos MDADI e HADS. Conclusão Os pacientes submetidos à tireoidectomia autorreferem melhor qualidade de vida em disfagia e redução dos escores de ansiedade/depressão após três meses de cirurgia. Não houve correlação entre ansiedade, depressão e qualidade de vida em disfagia nos momentos avaliados.

RESUMO Objetivo Correlacionar a qualidade de vida em voz e sintomas de ansiedade e depressão pré e pós-tireoidectomia. Método Estudo observacional, longitudinal, prospectivo e quantitativo. Participaram 20 pacientes, com média de idade de 54 anos, submetidos à tireoidectomia, ao exame visual laríngeo e aos questionários Qualidade de Vida em Voz e Escala Hospitalar de Ansiedade e Depressão nos momentos: pré-operatório, 1 semana e 3 meses pós-tireoidectomia, com maior prevalência do sexo feminino (85%; n=17) e de tireoidectomia parcial (70%; n=14). Resultados Não houve diferença estatística quanto à qualidade de vida em voz entre os momentos, porém observaram-se menores escores no pré-operatório, com destaque para o domínio físico. Houve traço leve de ansiedade no pré-operatório com redução após 1 semana e aumento pós três meses. Houve correlação negativa moderada entre o domínio físico do QVV e a subescala de ansiedade e o escore total da HADS pós 1 semana e entre o domínio total do QVV com o escore total da HADS pós 1 semana, correlação negativa fraca entre o domínio total do QVV e a subescala de ansiedade da HADS pós 1 semana e o escore total da HADS pós 3 meses. Conclusão Os pacientes avaliados neste estudo autopercebem sua qualidade de vida relacionada à voz como positiva. Traços leves de ansiedade foram identificados, com redução no pós-operatório uma semana e aumento pós três meses. A autopercepção de melhor qualidade de vida em voz no pós-operatório relaciona-se, de forma fraca, com a redução dos níveis de ansiedade.

Twenty patients participated in the study, with a mean age of 54 years, who underwent thyroidectomy, laryngeal visual examination, and the Voice-Related Quality of Life and Hospital Anxiety and Depression Scale questionnaires at different times: preoperative, 1 week and 3 months post-thyroidectomy, with a higher prevalence of females (85%; n=17) and partial thyroidectomy (70%; n=14). Results There was no statistical difference in voice quality of life between the moments, but lower preoperative scores were observed, especially in the physical domain. We observed a slight trace of anxiety in the preoperative period, with a reduction after 1 week and an increase after 3 months. There was a moderate negative correlation between the physical domain of QVV and the anxiety subscale and the total HADS score after 1 week and between the total domain of QVV with the total HADS score after 1 week, weak negative correlation between the total domain of QVV and the HADS anxiety subscale after 1 week and the total HADS score after 3 months. Conclusion The patients evaluated in this study self-perceived their voice-related quality of life as positive. Mild anxiety traits were identified, with a reduction after one week postoperatively and an increase after three months. The self-perception of better voice-related quality of life in the postoperative period is weakly related to the reduction of anxiety levels.

RESUMO Objetivo: conhecer o perfil epidemiológico dos pacientes submetidos à cirurgia do carcinoma bem diferenciado da tireoide no Hospital Universitário Cassiano Antônio Moraes da Universidade Federal do Espírito Santo, assim como os resultados oncológicos e as principais complicações pós-operatórias. Métodos: estudo transversal e retrospectivo de pacientes portadores de carcinoma bem diferenciado da tireoide operados no período de janeiro de 2008 a dezembro de 2015. Resultados: no período do estudo, dos 353 pacientes submetidos a tratamento cirúrgico da glândula tireoide, 95 eram portadores de CBDT. O carcinoma papilífero da tireoide foi o mais frequente (91,57%). A tireoidectomia total não associada a esvaziamento cervical foi o procedimento cirúrgico mais frequente (65,26%). As complicações pós-operatórias ocorreram em 6,31% dos pacientes, sendo o hematoma a mais frequente. O tempo médio de seguimento foi de 36,9 meses. A recidiva ocorreu em quatro pacientes (4,21%), sendo locorregional em todos os casos. Os fatores prognósticos analisados, como sexo, idade, tamanho do tumor, acometimento linfonodal, estadiamento, tipo de cirurgia, histologia e iodoterapia complementar não demonstraram significância estatística. Conclusão: o carcinoma papilífero da tireoide foi a neoplasia maligna mais frequente, acometendo o sexo feminino na faixa etária dos 49 anos mais frequentemente. A recidiva locorregional ocorreu em quatro pacientes. O hematoma foi a complicação mais frequente.

ABSTRACT Objective: to know the epidemiological profile of patients undergoing surgery for well-differentiated thyroid carcinoma at the Cassiano Antônio Moraes University Hospital of the Federal University of Espírito Santo, as well as the oncological results and the main postoperative complications. Methods: we conducted a cross-sectional, retrospective study of patients with well-differentiated thyroid carcinoma (WDTC) operated from January 2008 to December 2015. Results: During the study period, 95 of the 353 patients undergoing surgical treatment of the thyroid gland had WDTC. Papillary carcinoma was the most frequent (91.57%). Total thyroidectomy not associated with cervical emptying was the most frequent surgical procedure (65.26%). Postoperative complications occurred in 6.31% of patients, hematoma being the most frequent. The mean follow-up time was 36.9 months. Relapse occurred in four patients (4.21%), being locoregional in all cases. The prognostic factors analyzed, such as gender, age, tumor size, lymph node involvement, staging, type of surgery, histology and complementary iodine therapy did not show statistical significance. Conclusion: papillary carcinoma was the most common thyroid malignant neoplasm, affecting women in the 49-year-old age group more frequently. Loco-regional recurrence occurred in four patients. Hematoma was the most frequent complication.

CONTEXTO: A doença de Kikuchi-Fujimoto (DKF) se manifesta na maioria das vezes como linfadenomegalia cervical unilateral, que pode ser acompanhada de febre. Afeta principalmente mulheres jovens, com evolução autolimitada. A prevalência desta doença é mais comum no oriente, havendo poucos relatos de sua ocorrência em nosso meio. DKF deve ser incluída no diagnóstico diferencial de casos suspeitos de infecções por vírus, tuberculose, linfadenite reacional, lúpus eritematoso sistêmico e doença metastática. Histologicamente pode ser confundida com linfoma. A doença é de caráter benigno, auto-limitada, a biópsia excisional do linfonodo acometido é necessária para o diagnóstico e não há tratamento específico. RELATO DE CASOS: Este trabalho relata três casos de pacientes não asiáticas do sexo feminino com DKF, atendidas em nosso serviço entre 2003 e 2006, e faz revisão da literatura com uma busca sistematizada sobre o assunto, com o objetivo de informar os médicos sobre essa entidade que se manifesta com massa cervical e febre.

CONTEXT: Kikuchi-Fujimoto disease (KFD) manifests in most cases as unilateral cervical lymphadenomegaly, with or without accompanying fever. The disease mainly affects young women and has a self-limited course. It is more common in oriental countries, with few reports of its occurrence in Brazil. KFD should be included in the differential diagnosis of suspected cases of viral infections, tuberculosis, reactive lymphadenitis, systemic lupus erythematosus and metastatic diseases. It can be histologically confused with lymphoma. The disease is benign and self-limiting and an excisional biopsy of an affected lymph node is necessary for diagnosis. There is no specific therapy. CASE REPORTS: This study reports on three cases of non-Asian female patients with KFD who were attended at our service between 2003 and 2006. A review of the literature was carried out, with a systematic search on this topic, with the aim of informing physicians about this entity that is manifested by cervical masses and fever.
