Resumo Atualmente, uma fração elevada e diversificada de conhecimento científico é publicado. Contudo, a publicação dos resultados científicos no campo académico e entre pares não garante que o conhecimento chegue à sociedade. Os cientistas têm o dever de transformar o conhecimento em produtos de fácil consumo, acessíveis às pessoas, no momento em que necessitam de tomar decisões conscientes e informadas, contribuindo para o empoderamento sobre aspetos da sua saúde e da sua vida. Analisar o léxico utilizado por entidades internacionais nas suas estruturas online para comunicar a ciência ao cidadão. Estudo exploratório, de natureza descritiva, com recurso a análise documental dos conteúdos publicados nos websites de 16 instituições científicas internacionais. O corpus textual foi organizado em 21 textos e submetido à análise com auxílio do software Interface de R pour Analyses Multidimensionnelles de Textes et de Questionneires (IRAMUTEQ). O corpus foi organizado em dois campos contextuais: “Aproximação ao cidadão” e “Da compreensão pública de ciência à comunicação estratégica”. Posteriormente, foram categorizados os mundos lexicais “Interação”, “Envolvimento”, “Acessibilidade” e “Capacitação”. O léxico revelado nos discursos das instituições científicas permitiu identificar os campos conceptuais e os mundos lexicais que caracterizam os três modelos de comunicação existentes. Nas estratégias para comunicação de ciência reveladas é possível verificar elementos da transição e evolução dos próprios modelos ao longo dos anos. O modelo do défice, fortemente criticado na literatura, é representado pelo mundo lexical de menor representatividade, demonstrando que ainda possui raízes nas estratégias vigentes, porém constitui um modelo que já não responde suficientemente aos interesses e necessidade do público atual. Em contrapartida, os dois modelos subsequentes, apesar de apresentarem-se distantes nas análises de CHD, AFC e similitude, concorrem para objetivos semelhantes, e aproximam-se, de modo mais equitativo, a uma comunicação de ciência democrática e interativa.

Abstract Currently, a high and diverse fraction of scientific knowledge is published. However, the publication of scientific results in the academic field and among peers does not guarantee that knowledge reaches society. Scientists have a duty to transform knowledge into products that are easy to consume, accessible to people, at a time when they need to make conscious and informed decisions, contributing to empowerment in aspects of their health and their lives. To analyze the lexicon used by international entities in their online structures to communicate science to lay citizens. Exploratory study, of a descriptive nature, using document analysis of content published on the websites of 16 international scientific institutions. The textual corpus was organized into 21 texts and submitted to analysis using the software Interface de R pour Analyses Multidimensionnelles de Textes et de Questionneires (IRAMUTEQ). The corpus was organized into two contextual fields “Approaching the citizen” and “From public understanding of science to strategic communication”. Subsequently, the lexical worlds “Interaction”, “Involvement”, “Accessibility” and “Enabling” were categorized. The lexicon revealed in the speeches of the scientific institutions made it possible to identify the conceptual fields and the lexical worlds that characterize the three existing models of communication. In the scientific dissemination strategies revealed, there are elements of the transition and evolution of the models themselves over the years. The deficit model, strongly criticized in the literature, is represented by the less representative lexical world, demonstrating that it still has roots in current strategies, but constitutes a model that no longer responds sufficiently to the interests and needs of the current public. In contrast, the two subsequent models, although distant in CHD, AFC and similarity analyses, compete for similar goals, and more equitably address the democratic and interactive communication of science.

OBJECTIVES: This observational, cross-sectional study based aimed to test whether heart failure (HF)-disease management program (DMP) components are influencing care and clinical decision-making in Brazil. METHODS: The survey respondents were cardiologists recommended by experts in the field and invited to participate in the survey via printed form or email. The survey consisted of 29 questions addressing site demographics, public versus private infrastructure, HF baseline data of patients, clinical management of HF, performance indicators, and perceptions about HF treatment. RESULTS: Data were obtained from 98 centers (58% public and 42% private practice) distributed across Brazil. Public HF-DMPs compared to private HF-DMP were associated with a higher percentage of HF-DMP-dedicated services (79% vs 24%; OR: 12, 95% CI: 94-34), multidisciplinary HF (MHF)-DMP [84% vs 65%; OR: 3; 95% CI: 1-8), HF educational programs (49% vs 18%; OR: 4; 95% CI: 1-2), written instructions before hospital discharge (83% vs 76%; OR: 1; 95% CI: 0-5), rehabilitation (69% vs 39%; OR: 3; 95% CI: 1-9), monitoring (44% vs 29%; OR: 2; 95% CI: 1-5), guideline-directed medical therapy-HF use (94% vs 85%; OR: 3; 95% CI: 0-15), and less B-type natriuretic peptide (BNP) dosage (73% vs 88%; OR: 3; 95% CI: 1-9), and key performance indicators (37% vs 60%; OR: 3; 95% CI: 1-7). In comparison to non- MHF-DMP, MHF-DMP was associated with more educational initiatives (42% vs 6%; OR: 12; 95% CI: 1-97), written instructions (83% vs 68%; OR: 2: 95% CI: 1-7), rehabilitation (69% vs 17%; OR: 11; 95% CI: 3-44), monitoring (47% vs 6%; OR: 14; 95% CI: 2-115), GDMT-HF (92% vs 83%; OR: 3; 95% CI: 0-15). In addition, there were less use of BNP as a biomarker (70% vs 84%; OR: 2; 95% CI: 1-8) and key performance indicators (35% vs 51%; OR: 2; 95% CI: 91,6) in the non-MHF group. Physicians considered changing or introducing new medications mostly when patients were hospitalized or when observing worsening disease and/or symptoms. Adherence to drug treatment and non-drug treatment factors were the greatest medical problems associated with HF treatment. CONCLUSION: HF-DMPs are highly heterogeneous. New strategies for HF care should consider the present study highlights and clinical decision-making processes to improve HF patient care.

RESUMO A proposta deste estudo foi identificar as leveduras envolvidas na fermentação espontânea de cacau da Amazônia brasileira. A fermentação foi realizada em Medicilândia e Tucumã, Pará, Brasil, durante 6 dias. Em total foram obtidos 44 isolados de leveduras de Medicilândia e 29 de Tucumã. A identificação molecular foi realizada por sequenciamento do fragmento da região D1/D2 do gene rRNA 26S, amplificado com primers universais para eucariotos NL1GC e LS2. Em Medicilândia, foram identificadas Pichia manshurica e Saccharomyces cerevisiae. Em Tucumã foram identificadas cinco espécies (Pichia fermentans, P. kudriavzevii, P. manshurica, S. cerevisiae e Zygosaccharomyces bailii). Os resultados sugerem que P. manshurica e S. cerevisiae podem ter potencial para uso como culturas starter em estudos futuros, para melhorar a qualidade das sementes de cacau fermentadas na Amazônia brasileira.

ABSTRACT The purpose of this study was to identify the yeasts involved in spontaneous fermentation of cocoa from the Brazilian Amazon region. The fermentation process was carried out experimentally with cocoa seeds from two sites (Medicilândia and Tucumã), State of Pará, northern Brazil, during a six-day period. Totals of 44 yeasts were isolated from Medicilândia and 29 from Tucumã. Molecular identification was carried out by sequencing the D1/D2 region fragment of the rRNA 26S gene, expanded with universal primers for the NL1GC and LS2 eukaryotes. Pichia manshurica and Saccharomyces cerevisiae were identified in Medicilândia and five yeast species (Pichia fermentans, P. kudriavzevii, P. manshurica, S. cerevisiae and Zygosaccharomyces bailii) were identified in Tucumã. The results showed that P. manshurica and S. cerevisiae may have potential for use as starter cultures in future studies to improve the quality of cocoa seeds fermented in the Brazilian Amazon region.

A localized Chikungunya virus (CHIKV; East/Central/South African genotype) outbreak (50 cases, 70% laboratory-confirmed; attack rate: 5.3 confirmed cases/100 people) occurred in a Salvador, Brazil neighborhood, between Apr-Jun/2017. Highly clustered cases in space and time, mostly along a single street, highlight an increased risk of CHIKV transmission among pockets of susceptible populations. This finding underscores the need for ongoing local level surveillance for arboviral outbreaks.

RESUMO A doença pulmonar obstrutiva crônica (DPOC) ocasiona prejuízos na mecânica pulmonar, interferindo na atuação, mobilidade e conformidade do diafragma. A estimulação elétrica diafragmática tradicional é capaz de gerar benefícios ao pneumopata; poderia a corrente russa ser outra opção? Objetivou-se identificar as alterações após estimulação diafragmática elétrica transcutânea pela corrente russa em indivíduos portadores de DPOC. Trata-se de estudo prospectivo, quase experimental, com os seguintes critérios de inclusão: estabilidade medicamentosa, cessação tabágica, DPOC grau III e IV e manutenção do estilo de vida. Foram avaliadas medidas antropométricas, respiratórias e funcionais. A estimulação diafragmática se deu pelo Endophasys R ET 9701 por quatro meses, duas vezes por semana, com 30 sessões. O tempo de terapia e frequência para cada sessão foram: 18 min. (20 a 30 Hz) e 12 min. (70 a 100 Hz), respectivamente. Para análise dos dados foi aplicado teste “t” de Student (p<0,05). Participaram do tratamento 13 DPOC, sendo 11 (84,6%) do sexo masculino, todos brancos com idade de 68,46±11,11 anos e carga tabágica de 74,03±56,2 maços-ano. Ao final da intervenção houve mudanças no: volume minuto de 14,47±4,72 para 13,03±4,00 L/min.; índice BODE de 3,92±2,10 para 3,23±1,87; e distância no teste de caminhada de 6 minutos (TC6) de 336±76,36 para 402,76±51,29 m. Concluiu-se que a estimulação elétrica diafragmática por meio da corrente russa promove benefícios significativos ao portador de DPOC, proporcionando melhora respiratória e funcional.

ABSTRACT Chronic Obstructive Pulmonary Disease (COPD) causes losses in lung mechanics, interfering in performance, mobility and conformity of the diaphragm, and traditional diaphragmatic electrical stimulation is able to generate benefits to the lung patient. Could Russian current be another option? We aimed to identify changes after transcutaneous diaphragmatic electrical stimulation through the Russian current in individuals with COPD. Prospective and nearly experimental study with inclusion criteria: drug stability, smoking interruption, COPD degrees III and IV and lifestyle maintenance. Anthropometric and functional measurements have been evaluated. Diaphragmatic stimulation occurred by Endophasys R ET 9701 for four months, twice a week, 30 sessions. Therapy time and frequency for each session were: 18 min. (20 to 30 Hz) and 12 min. (70 to 100 Hz), respectively. For data analysis Student’s t-test was applied (p<0.05). Thirteen individuals participated in the COPD treatment being 11 (84.6%) male, all white, aged 68.46±11.11 years and with tobacco load of 74.03±56.2 per year. At the end of the intervention, there were changes on minute volume from 14.47 min ± 4.72 to 13.03±4.00 l/min; BODE index from 3.92±2.10 to 3.23±1.87 and distance on the 6-minute walk test from 336±76.36 to 402.76±51.29 m. As a conclusion, diaphragmatic electrical stimulation through Russian current promotes significant benefits to COPD patients, providing respiratory and functional improvement.

RESUMEN La enfermedad obstructiva crónica (EPOC) perjudica la mecánica pulmonar, interfiriendo en la acción, movilidad y conformidad del diafragma. La estimulación diafragmática eléctrica tradicional es benéfica a los portadores de enfermedades pulmonares, ¿es posible ser otra opción la corriente rusa? El propósito de este trabajo es identificar los resultados tras la estimulación diafragmática eléctrica transcutánea por corriente rusa en pacientes con EPOC. Se trata de un estudio prospectivo, casi experimental, con los siguientes criterios de inclusión: la estabilidad de fármaco, el abandono del tabaco, la EPOC grados III y IV y el mantenimiento del estilo de vida. Se evaluaron medidas antropométricas, respiratorias y funcionales. La estimulación diafragmática fue realizada por el Endophasys R ET 9701 durante cuatro meses, dos veces semanales, con 30 sesiones. El tiempo de terapia y la frecuencia de cada sesión fueron los siguientes: 18 min. (20 a 30 Hz) y 12 min. (70 a 100 Hz), respectivamente. En el análisis de datos se empleó la prueba t de Student (p<0,05). Del estudio participaron 13 portadores de EPOC, siendo 11 (84,6%) varones, blancos, cuya edad fue de 68,46±11,11 años y el tiempo del tabaco fue de 74,03±56,2 paquetes-año. Al final de la intervención ocurrieron cambios: en el volumen minuto de 14,47±4,72 a 13,03±4,00 L/min.; en el índice BODE de 3,92±2,10 a 3,23±1,87; y en la distancia de la prueba de caminata de 6 minutos (TC6) de 336±76,36 a 402,76±51,29 m. Se concluye que la estimulación diafragmática eléctrica por corriente rusa es benéfica a los portadores de EPOC y les proporciona mejoras respiratoria y funcional.

Considerando que diferentes estratégias são utilizadas pelas organizações para potencializar seu desempenho por meio da inovação, este estudo teve como objetivo identificar as ligações de aprendizagem entre um instituto de P&D e pequenas empresas da cadeia produtiva da caprinocultura inserida no sistema de inovação local, tendo como base a seguinte questão: os institutos de P&D podem funcionar como porteiro tecnológico (gatekeepers) provendo conhecimento novo às pequenas empresas em estudo? Para isso foi elaborado o marco teórico do estudo baseado nas discussões referentes às estratégias e aos sistemas de inovação, articulando ainda as temáticas sobre pesquisa e desenvolvimento - P&D e os processos de aprendizagem correspondentes. A metodologia está baseada na estratégia do estudo multicaso. Os dados empíricos foram examinados segundo a abordagem qualitativa, sendo coletados através de entrevistas semiestruturadas, durante participações em reuniões e análises documentais. Nas empresas pesquisadas, observou-se que, apesar de essas manterem ligações de aprendizagem ainda incipientes com o instituto de P&D, evidências mostraram que estas estão incidindo na ampliação da base de conhecimento das pequenas empresas. Relevou-se ainda que estas ligações de aprendizagem estejam sendo ampliadas e potencializadas através das fontes externas e abertas entre as empresas e as demais instituições do sistema e que tiveram como base estratégias de inovação do tipo catching-up.

Considering that different strategies are used by the organizations to increase their performance through innovation, this study had the objective of identifying the learning connections between an R&D institute and in goat small companies of the local innovation system, tends as base the following question: can R&D institutes work as technological gatekeepers providing new knowledge to the companies under study? For that, the theoretical mark of the study was elaborated based on the discussions about strategies and innovation systems, also articulating the themes on research and development - R&D and the processes of corresponding learning. The methodology is based on the strategy of multi case study. The empiric data were examined according to the qualitative approach, being collected through semi-structured interviews, during participation in meetings and documentary analysis. In the researched companies, it was observed that, those firms still maintain incipient connections with the R&D institute. In the other hands, evidence showed that they are focusing on expanding the knowledge base of small businesses. It is also shown that these links are being expanded and potentiated learning by external and open sources between companies and other institutions of the system, which had catching-up innovation strategies as a base.

The Principal Components Analysis was used to characterize the summery North Mendoza rawinsondes, evaluating their capacity to identify patterns associated to the convection occurrence and defining an instability index. It was selected 326 cases, building an entry matrix with the temperature and dew point temperature anomalies for the standard levels between 850 and 300 hPa. The analysis showed 6 significant components that explain more than 91% of variance. Therefore, 12 patterns were obtained, which represent different atmospheric conditions. The main correlations between the patterns and convection (C) and non convection (NC) cases were obtained for the components 6, 2, 4 and 3. The relatively wet environments with a strong gradient of T at mean and high levels would favour the convection, while it is inhibited with dry layers in mean and high levels, and weak gradients of T in low layers. The C/NC index can be estimated by a logistic multiple regression dependent to the component loadings, with a correlation of 0,5, determining a stability/instability index calculable from the T and Td profiles and it depends on the climatological features of the region. The index showed an efficiency of 72%.

Se utilizó el análisis de componentes principales para caracterizar radiosondeos estivales del norte de Mendoza, evaluando su capacidad para identificar patrones asociados a la ocurrencia de convección y definiendo un índice de inestabilidad. Se tomaron 326 casos, construyendo una matriz de anomalías de temperatura (T) y temperatura de punto de rocío (Td) de los niveles estándar entre 850 y 300 hPa. El análisis arrojó 6 componentes significativas que explican más del 91% de varianza. Así se obtuvieron 12 patrones que representan condiciones atmosféricas diferenciadas. Las mayores correlaciones entre patrones y casos de convección (C) y no convección (NC), se obtuvieron para las componentes 6, 2, 4 y 3. La convección se favorecería en entornos relativamente húmedos con un marcado gradiente de T en niveles medios y altos, mientras se inhibiría con capas secas en niveles medios y altos, y débiles gradientes de T en capas bajas. El indicador de C/NC puede estimarse por regresión múltiple logística dependiente de las componentes de carga, con una correlación conjunta de 0,5, determinando un índice de estabilidad/inestabilidad calculable a partir de los perfiles de T y Td y dependiente de las características climatológicas de la región. El índice mostró una efectividad del 72%.

Um novo derivado de antraquinona, denominado guepinone (1), juntamente com as conhecidas substâncias isossulocrina (2) e cloroissosulocrina (3) foram isolados de uma cultura em arroz de Pestalotiopsis guepinii, um fungo endofitico de Virola michelii. Os compostos foram identificados pela análise de seus dados espectrométricos de RMN 1D e 2D e EM. A atividade antimicrobiana dos compostos isolados foi avaliada e a cloroisossulcrina (3) foi a mais ativa.

A new anthraquinone derivative, named guepinone (1), along with the known substances isosulochrin (2) and chloroisosulochrin (3), were isolated from a rice culture of Pestalotiopsis guepinii, an endophytic fungus of Virola michelii. The compounds were identified by analysis of 1D and 2D NMR and MS spectral data. The antimicrobial activity of these compounds was evaluated and chloroisosulchrin (3) was the most active.

OBJETIVOS: Identificar os sorotipos de pneumococo mais frequentemente isolados de crianças internadas com pneumonia invasiva, comparar os sorotipos com os incluídos em vacinas conjugadas e analisar sua sensibilidade aos antimicrobianos mais utilizados na faixa etária pediátrica. MÉTODOS: Estudo descritivo, retrospectivo das pneumonias pneumocócicas identificadas em crianças internadas no hospital universitário da Universidade de São Paulo, no período de janeiro de 2003 a outubro de 2008. Os critérios de inclusão foram: faixa etária de 29 dias até 15 anos incompletos com diagnóstico clínico e radiológico de pneumonia e com cultura de sangue e/ou líquido pleural com crescimento de Streptococcus pneumoniae. RESULTADOS: Foram incluídas no estudo 107 crianças. Os sorotipos mais frequentes foram: 14 (36,5%), 1 (16,7%), 5 (14,6%), 6B (6,3%) e 3 (4,2%). A proporção de sorotipos contidos na vacina conjugada heptavalente seria de 53,1%, na vacina 10-valente de 86,5% e na 13-valente seria de 96,9%. De acordo com os padrões do Clinical and Laboratory Standards Institute 2008, 100 cepas (93,5%) de pneumococos foram sensíveis à penicilina (concentração inibitória mínima, CIM < 2 µg/mL), 7 cepas (6,5%) com resistência intermediária (CIM = 4 µg/mL) e nenhuma com resistência (CIM > 8 µg/mL). Verificamos alta taxa de sensibilidade para as cepas testadas para vancomicina, rifampicina, ceftriaxone, clindamicina, cloranfenicol e eritromicina. CONCLUSÕES: Nossos resultados confirmam um expressivo impacto potencial das vacinas conjugadas, principalmente pela 10-valente e 13-valente, sobre os casos de pneumonias invasivas. Os resultados de sensibilidade à penicilina evidenciam que a opção terapêutica de escolha para o tratamento das pneumonias invasivas continua sendo a penicilina.

OBJECTIVES: To identify the most common pneumococcal serotypes in children hospitalized with invasive pneumonia, correlate isolated serotypes with those included in conjugate vaccines, and ascertain the sensitivity of the isolated pneumococcal strains to penicillin and other antibiotics. METHODS: From January 2003 to October 2008, a retrospective study of hospitalized children with a diagnosis of Streptococcus pneumoniae pneumonia was conducted at the university hospital of Universidade de São Paulo. Criteria for inclusion were: age greater than 29 days and less than 15 years, radiological and clinical diagnosis of pneumonia, and isolation of Streptococcus pneumoniae in blood cultures and/or pleural effusion. RESULTS: The study included 107 children. The most common serotypes were 14 (36.5%), 1 (16%), 5 (14.6%), 6B (6.3%) and 3 (4.2%). The proportion of identified serotypes contained in the heptavalent, 10-valent and 13-valent conjugate vaccines was 53.1, 86.5, and 96.9%, respectively. Pneumococcal strains were sensitive to penicillin (minimum inhibitory concentration, MIC < 2 µg/mL) in 100 cases (93.5%) and displayed intermediate resistance (MIC = 4 µg/mL) in 7 cases (6.5%). No strains were penicillin-resistant (MIC > 8 µg/mL) according to the Clinical and Laboratory Standards Institute 2008 standards. Tested isolates were highly sensitive to vancomycin, rifampicin, ceftriaxone, clindamycin, erythromycin, and chloramphenicol. CONCLUSIONS: Our results confirm a significant potential impact of conjugate vaccines, mainly 10-valent and 13-valent, on invasive pneumonia. Furthermore, susceptibility testing results show that penicillin is still the treatment of choice for invasive pneumonia in our setting.

OBJETIVO: Analisar e comparar os cuidados primários prestados à população materno-infantil e contribuir para a avaliação da assistência integral a esse grupo. MÉTODOS: Inquérito populacional realizado por entrevistas, no principal posto de vacinação do Município de Teresópolis, RJ, no Dia Nacional de Vacinação, que abrangeu questões sobre utilização de serviços de saúde e prestação de cuidados primários preventivos. RESULTADOS: Foram colhidas informações de 329 crianças e suas respectivas mães. Mais de 90% das crianças haviam comparecido à consulta pediátrica nos três meses anteriores e quase todas possuíam o cartão da criança, embora em 30% desses cartões não havia qualquer peso registrado no período. Observou-se associação positiva entre consulta de puericultura e registro de peso no cartão da criança (RP = 1,34; IC: 1,13-1,58; p = 0,0002). Cerca de 59% das mães compareceram à consulta de revisão de parto, mas 25% referiram nunca ter feito exame colpocitológico-oncótico e 36% nunca haviam realizado exame de mama. Observou-se associação positiva entre a idade materna acima de 20 anos e a realização de algum exame colpocitológico-oncótico durante a vida reprodutiva (RP = 1,56; IC: 1,08-2,26; p = 0,03). Quase 70% das mães relataram uso de algum método anticoncepcional, principalmente pílula, condom e laqueadura tubária. CONCLUSÕES: Apesar de algumas limitações, os resultados sugerem a viabilidade da metodologia utilizada, permitindo a identificação de deficiências importantes na prestação de cuidados primários de saúde para crianças e principalmente para mães.

OBJECTIVE: To describe and analyze primary health care provided to mother-and-child population in Teresópolis, state of Rio de Janeiro, Brazil. METHODS: A population survey was performed at the main vaccination station on the local Vaccine National Day, to collect information about the use of health care services and preventive primary care. RESULTS: Information was collected from 329 mothers and their children. More than 90% of children had at least one pediatric visit in the past three months. Almost all had their growth chart, but in 30% of them the child's weight was not registered. There was a positive association between having a routine visit and the registration of the child's weight in the card (RP = 1.34; IC: 1.13-1.58; p = 0.0002). Around 59% of the mothers had a medical visit after giving after birth; 25% said they had never had a Pap smear test before, and 36% never had a breast examination. There was a positive association between the mother's age being over 20 years and ever having a Pap smear test (RP = 1.56; IC: 1.08-2.26; p = 0.03). Almost 70% of the mothers referred the use of any contraceptional method, and the most common used were contraceptive pills, condoms and tubal ligation. CONCLUSIONS: Despite some limitations, the results suggest a valid and useful methodology, which allows the identification of important needs in primary health care delivery to children and their mothers.
