ABSTRACT Objective To compare the sociodemographic and clinico-functional characteristics of patients admitted to a stroke unit immediately before and during two different COVID-19 pandemic phases. Methods This exploratory study was conducted in the stroke unit of a public hospital in Brazil. Patients consecutively admitted to a stroke unit for 18 months with primary stroke aged ≥20 years were included and divided into three groups: G1: Pre-pandemic; G2: Early pandemic; and G3: Late pandemic. The sociodemographic and clinico-functional characteristics of the groups were compared (α=0.05). Results The study included 383 individuals (G1=124; G2=151; G3=108). The number of risk factors (higher in G2; p≤0.001), smoking (more common in G2; p≤0.01), type of stroke (ischemic more common in G3; p=0.002), stroke severity (more severe in G2; p=0.02), and level of disability (more severe in G2: p≤0.01) were significantly different among the groups. Conclusion A greater number of serious events and risk factors including smoking and higher level of disability was observed in patients in the beginning of the pandemic than in the late phases. Only the occurrence of ischemic stroke increased in the late phase. Therefore, these individuals may have an increased need for rehabilitation services monitoring and care during their lifespan. Additionally, these results indicate that health promotion and prevention services should be strengthened for future health emergencies.

Resumo A verdadeira medicina combina conhecimento biotecnocientífico ao cuidado. No entanto, as escolas médicas têm dado maior ênfase ao conhecimento científico em detrimento da relação médico-paciente. Objetivos: Este relato tem como objetivo descrever a experiência de imersão vivida por quatro estudantes de medicina do sexto período nos meses de fevereiro e março de 2020. Método: Tal imersão deu-se em um projeto de ensino denominado Anjos da Guarda, o qual é usado como ferramenta de educação médica no Hospital Universitário Gaffrée e Guinle da Universidade Federal do Estado do Rio de Janeiro (HUGG-UNIRIO). Resultados: O Projeto Anjos da Guarda desenvolve o profissionalismo médico, a empatia e os fundamentos da medicina centrados na pessoa do paciente como parte do processo de cuidado, sem menosprezar a importância do conhecimento biotecnocientífico e do raciocínio clínico. Conclusão: A experiência possibilitou ganhos em aprendizado para os estudantes, além de contribuir com o sistema de saúde e de beneficiar os pacientes envolvidos.

Abstract Conventional medicine combines biotechnoscientific knowledge with care. However, medical schools have placed greater emphasis on scientific knowledge to the detriment of the doctor-patient relationship. Objectives: This report aims to describe the immersion experience lived by four sixth-term medical students during the months of February and March 2020. Method: Such immersion took place in a teaching project named Anjos da Guarda, which is used as a medical education tool at the Gaffrée e Guinle University Hospital of the Federal University of the State of Rio de Janeiro (HUGG-UNIRIO). Results: The Anjos da Guarda Project develops medical professionalism, empathy and the fundamentals of medicine centered on the patient as part of the care process, without underestimating the importance of biotechnoscientific knowledge and clinical reasoning. Conclusion: The experience enabled gains in learning for students, in addition to contributing to the health system and benefiting the patients involved.

Resumo: A família é o primeiro alicerce de uma criança, imprescindível para seu crescimento e desenvolvimento adequados. Em diversos contextos a criança passa por momentos de estresse, como na doença e na hospitalização. Essa última pode levar à instabilidade emocional e trauma. Logo, neste processo, é normal a criança sentir medo e outros sentimentos desagradáveis, devido à falta de maturidade para assimilar seu estado de saúde e por estar em um ambiente desconhecido e afastado de seus familiares. É imprescindível que a equipe multidisciplinar desenvolva um cuidado baseado no compartilhamento efetivo de informações, no acesso irrestrito ao filho, na participação efetiva dos pais na tomada de decisões e no respeito mútuo. Objetivo: Compreender a percepção do enfermeiro acerca da atuação dos pais ou responsáveis da criança internada em Unidade de Terapia Intensiva Pediátrica. Métodos: Estudo descritivo de abordagem qualitativa, por meio de entrevistas online guiadas por roteiro semiestruturado. Foi utilizada a estratégia bola de neve, a amostragem por saturação teórica e os depoimentos foram analisados pelo referencial de Bardin. Resultados: Participaram 13 profissionais enfermeiros. As categorias temáticas elaboradas foram: Atuação dos pais ou responsáveis no apoio emocional e melhora clínica; Execução e restrição do cuidado realizado pelos pais ou responsáveis. Conclusão: O enfermeiro entende como positiva a permanência e a realização de cuidados pelos pais ou responsáveis dentro da Unidade de Terapia Intensiva Pediátrica, pois auxilia na melhora clínica e no apoio psicológico, além de reaproximar a família do cuidado do filho.

Abstract: The family is a child's first foundation, essential for proper growth and development. In different contexts, the child goes through moments of stress, such as illness and hospitalization, which can lead to emotional instability and trauma. Therefore, in this process, it’s normal for children to feel fear and other unpleasant feelings, due to the lack of maturity to assimilate their health status and for being in an unknown environment and away from their family members. It’s essential that the multidisciplinary team develop care based on the effective sharing of information, unrestricted access to the child, the effective participation of parents in decision-making and mutual respect. Objective: To understand the perception of nurses about the performance of the parents or guardians of the child hospitalized in Pediatric Intensive Care Unit. Methods: Descriptive study with a qualitative approach, through online interviews guided by a semi-structured script. The snowball strategy was used, sampling by theoretical saturation and the testimonies were analyzed using the Bardin framework. Results: 13 nurses participated. The thematic categories elaborated were: Performance of parents or guardians in emotional support and clinical improvement; Execution and restriction of care provided by parents or guardians. Conclusion: The nurses understand as positive the permanence and the performance of care by parents or guardians within the Pediatric Intensive Care Unit, as it helps in clinical improvement and psychological support, in addition to bringing the family closer to the care of the child.

RESUMO O diagnóstico precoce e adequado da tuberculose é um dos pilares mais importantes no controle da doença. A proposta deste consenso brasileiro é apresentar aos profissionais da área de saúde um documento com as evidências mais atuais e úteis para o diagnóstico da tuberculose. Para tanto, a Comissão de Tuberculose da Sociedade Brasileira de Pneumologia e Tisiologia reuniu 14 membros da Sociedade com reconhecida experiência em tuberculose no Brasil. Foi realizada uma revisão não sistemática dos seguintes tópicos: diagnóstico clínico, diagnóstico bacteriológico, diagnóstico radiológico, diagnóstico histopatológico, diagnóstico da tuberculose na criança e diagnóstico da tuberculose latente.

ABSTRACT Early, accurate diagnosis of tuberculosis is one of the major pillars of the control of the disease. The purpose of this consensus statement is to provide health professionals with the most current, useful evidence for the diagnosis of tuberculosis in Brazil. To that end, the Tuberculosis Committee of the Brazilian Thoracic Association brought together 14 members of the Association with recognized expertise in tuberculosis in Brazil to compose the statement. A nonsystematic review of the following topics was carried out: clinical diagnosis, bacteriological diagnosis, radiological diagnosis, histopathological diagnosis, diagnosis of tuberculosis in children, and diagnosis of latent tuberculosis infection.

RESUMO OBJETIVOS Indivíduos vivendo com HIV parecem mais propensos às alterações na redistribuição da gordura corporal, caracterizada como lipodistrofia, podendo acontecer em conjunto com as metabólicas. No presente estudo avaliaram-se tais impactos em adultos com e sem HIV e se associou ao tempo de diagnóstico do vírus e tratamento com antirretroviral. MÉTODOS Estudo tipo transversal, com 123 adultos, no qual 87 tinham HIV e 36 sem HIV, de ambos os sexos, em seguimento ambulatorial no Serviço de Atendimento Especializado (SAE) em Macaé - RJ. Foram feitos: 1) Alteração na distribuição da gordura corporal, mensurados por parâmetros antropométricos e lipodistrofia autorreferida; 2) Perfil bioquímico; 3) Associação entre tempo diagnóstico do HIV e tratamento com antirretroviral. RESULTADOS Incluíram-se 54,47% (n=67) do sexo masculino, 45,52% (n=56) do feminino, com média de idade de 37 anos. Destes, 87 eram pessoas vivendo com HIV, 29% (n=25) possuíam lipodistrofia autorreferida; tempo médio de infecção pelo vírus e tratamento antirretroviral (5,80±4,56 e 5,14±3,82 anos), respectivamente. Os pacientes com lipodistrofia autorreferida tiveram maior alteração na distribuição da gordura corporal entre 3-6 anos de diagnóstico do HIV e um perfil colesterolêmico negativo. O tempo de tratamento com antirretroviral influenciou o colesterol total e os triglicerídeos, mesmo para os pacientes sem lipodistrofia autorreferida, com mais de nove anos sob tratamento. CONCLUSÃO Neste estudo, o perfil colesterolêmico negativo se relacionou principalmente ao tempo de tratamento com antirretroviral, mesmo para os pacientes sem lipodistrofia autorreferida e as alterações na distribuição da gordura corporal, mensuradas por antropometria, se associaram especialmente ao tempo de infecção pelo HIV naqueles com lipodistrofia autorreferida.

SUMMARY OBJECTIVES Individuals living with HIV seem to be more prone to changes in the redistribution of body fat, characterized as lipodystrophy, which may occur in conjunction with metabolic diseases. In the present study, such impacts were assessed in adults with and without HIV and associated with the time of virus diagnosis and treatment with antiretroviral. METHODS A cross-sectional study with 123 adults, in which 87 had HIV and 36 without HIV, of both sexes, in outpatient follow-up at the Specialized Care Service (SAE) in Macaé-RJ. The following were made: 1) Alteration in body fat distribution, measured by anthropometric parameters and self-reported lipodystrophy; 2) Biochemical profile; 3) Association between HIV diagnosis time and antiretroviral treatment. RESULTS 54.47% (n = 67) males, 45.52% (n = 56) females, mean age 37 years. Of these 87 were people living with HIV, 29% (n = 25) had self-reported lipodystrophy, mean time of virus infection, and antiretroviral treatment (5.80 ± 4.56 and 5.14 ± 3.82 years), respectively. Patients with self-reported lipodystrophy had a greater change in body fat distribution between 3-6 years of HIV diagnosis and a negative cholesterol profile. The antiretroviral treatment time influenced total cholesterol and triglycerides, even for patients without self-reported lipodystrophy, with a further nine years under treatment. CONCLUSION In this study, the negative cholesterol profile was mainly related to antiretroviral treatment time, even for patients without self-reported lipodystrophy, and changes in body fat distribution, measured by anthropometry, was especially associated with time for HIV infection in those with lipodystrophy self-reported.

ABSTRACT The aim of this study was to evaluate the concordance between two versions of the scoring system (2011 and 2019), recommended by the Brazilian Ministry of Health, for the diagnosis of pulmonary tuberculosis (PTB) in children and adolescents. A retrospective descriptive study was performed to assess the medical records of children and adolescents with PTB, in TB units from Brazilian cities located in Rio de Janeiro, Minas Gerais, and Parana States, from January 1 st , 2004, to December 1 st , 2018. Patients aged 0 to 18 years old with a diagnosis of PTB were included. The comparison between the two scoring systems showed a moderate concordance according to the κ coefficient value = 0.625. Fourteen patients showed a reduction in the TB score, going from 30 points in the 2011, to 25 points or less in the 2019 one. Seventy one percent of these 14 patients had radiological changes suggestive of PTB and 86% had tuberculin skin tests greater than 10 mm. The study concluded that a moderate agreement was observed between the 2011 and 2019 scoring systems, with an increase in the number of patients scoring 25 points or less in 2019, which can eventually hinder the diagnosis of PTB.

RESUMO. Ferramentas clínicas adicionais devem ser investigadas para facilitar e auxiliar o diagnóstico prévio do declínio cognitivo. O controle postural piora com o envelhecimento e este fato pode estar relacionado com o comprometimento cognitivo patológico. Objetivo: comparar o equilíbrio de adultos idosos sem demência no grupo controle (GC) e com doença de Alzheimer (DA), observar as possíveis associações com as variáveis independentes (diagnóstico, idade, sexo e estado cognitivo global) e verificar as melhores análises posturográficas para determinar a diferença entre os grupos. Métodos: 86 idosos (DA = 48; GC=38) foram avaliados utilizando a escala de equilíbrio Berg (EEB) e o controle postural pela estabilometria no Wii Balance Board ® (WBB). Testes T independente, Mann Whitney U, o tamanho de efeito (TE) e uma regressão linear foram realizados. Resultados: houve diferença significativa para AE, VT, ML com OA e OF, AP com OF e EEB entre os grupos. Estas variáveis mostraram um TE grande para EEB (-1.02), AE (0,83) com OF, ML (0,80; 0,96) e VT (0,92; 1,10) com OA e OF, respectivamente. A regressão indicou que a cognição global acompanhada da idade, gênero e diagnóstico contribuem para as alterações do controle postural. Conclusão: pacientes com DA apresentam comprometimento do controle postural quando comparados a idosos saudáveis. A VT com OF foi o parâmetro mais sensível para diferenciar os grupos e deve ser melhor investigada como possível biomarcador motor de demência na análise posturográfica com o WBB.

ABSTRACT. Additional clinical tools should be investigated to facilitate and aid the early diagnosis of cognitive decline. Postural control worsens with aging and this may be related to pathological cognitive impairment. Objective: to compare the balance of older adults without dementia in a control group (CG) and with Alzheimer’s disease (AD), to observe the possible association with the independent variables (diagnosis, age, gender, and global cognition) and to verify the best posturographic analyses to determine the difference between the groups. Methods: 86 older adults (AD = 48; CG = 38) were evaluated using the Berg Balance Scale (BBS) and postural control was assessed by stabilometry on the Wii Balance Board ® (WBB). Independent T, Mann-Whitney U-tests, Effect Size (ES) and a linear regression were performed. Results: there was a significant difference for Elliptical Area, Total Velocity, Medio-Lateral displacements with closed eyes and open eyes, antero-posterior, with closed eyes and BBS between groups. These variables showed a large effect size for BBS (-1.02), Elliptical Area (0.83) with closed eyes, Medio-Lateral (0.80, 0.96) and Total Velocity (0.92; 1.10) with eyes open and eyes closed, respectively. Regression indicated global cognition accompanied by age, gender, and diagnosis influenced postural control. Conclusion: patients with AD showed impaired postural control compared to Control Group subjects. Total Velocity with closed eyes was the most sensitive parameter for differentiating groups and should be better investigated as a possible motor biomarker of dementia in posturographic analysis with WBB.

RESUMO. A capacidade aeróbica diminui significativamente ao longo da vida, começando com a idade de 30 anos e acelerando a partir dos 60 anos, onde se prevê um declínio de 17% por década. Objetivo: Investigar a associação entre a capacidade aeróbica medida e o risco de desenvolvimento de comprometimento cognitivo leve (CCL), DA leve e moderado em idosos, considerando as classificações de risco de perda funcional para o teste do STEP. Métodos: Nesse estudo de corte-transversal, foram avaliados 93 idosos (>60 anos) (saudáveis=36; CCL=18, DA=39). Para acessar a capacidade aeróbica foi utilizado o teste de STEP e o estado cognitivo global foi medido através do MEEM. Os grupos foram divididos em relação às classificações de risco de perda funcional em abaixo ou acima do ponto de corte padronizado para idosos. Resultados: Idosos incluídos no grupo de risco de perda funcional tinham aproximadamente dez a catorze vezes mais chances de serem diagnosticados com DA leve (O.R:10.7; p=0.001) e moderado (O.R.=14.7; p=0.002), do que os seus pares mais bem condicionados. Menores níveis de capacidade aeróbica também foram associados com o diagnostico de CCL (O.R=4.5; p=0.05), entretanto apenas após controle por idade, escolaridade e sexo. Na amostra total (N=93) houve uma associação entre a capacidade aeróbica e o MEEM (R2=0.35; p<0.001) depois de controlado pelas variáveis confundidoras. Conclusão: Idosos que se encontram dentro da classificação de risco de perda funcional medida através do teste de STEP possuem maiores chances de desenvolverem CCL, Alzheimer leve e moderado mesmo após controle por idade, sexo e escolaridade.

ABSTRACT. Aerobic capacity declines significantly throughout life, beginning at the age of 30 years and accelerating from 60 years, where a decline of 17% per decade is expected thereafter. Objective: To investigate the association between aerobic capacity and a diagnosis of mild cognitive impairment (MCI), mild AD or moderate AD in older adults, considering the risk classification of functional loss of the Step test. Methods: In this cross-sectional study, 93 patients (age >60 years) were evaluated (Healthy=36; MCI=18, AD=39). The step test was used to assess aerobic capacity, while overall cognitive status was measured using the MMSE. The groups were divided according to the risk classifications of functional loss into below or above the standard cut-off point for aerobic capacity. Results: Subjects in the functional loss risk group were approximately ten to fourteen times more likely to be diagnosed with mild (OR:10.7; p=0.001) or moderate (OR.=14.7; p=0.002) AD than their fitter counterparts. Low aerobic fitness was also associated with the MCI condition (OR=4.5; p=0.05), but only after controlling for educational level, age and sex. In the overall sample (N=93), there was an association between aerobic capacity and MMSE performance (R2=0.35; p<0.001) after controlling for confounding variables. Conclusion: low aerobic capacity was associated with cognitive decline, and older adults at risk of functional loss on the STEP test had greater chance of being diagnosed with MCI or AD after controlling for age, sex and education.

Resumo Introdução: Estudos apontam para a efetividade de ações inclusivas relacionadas aos processos escolares, assim como a necessidade de formação especializada para terapeutas ocupacionais, capacitando-os para abordagens e ações colaborativas. Objetivos: Identificar ações desenvolvidas por terapeutas ocupacionais do Estado do Paraná no processo de inclusão escolar de crianças e adolescentes com necessidades educacionais especiais, e refletir sobre o contexto da educação inclusiva, a partir de perspectivas apresentadas. Método: Realizou-se levantamento de dados (survey), com a participação de 18 terapeutas ocupacionais. Aplicou-se questionário relacionado a inclusão escolar e atuação do profissional, com divulgação através de plataforma online e rede social. Submeteram-se os dados coletados à análise qualitativa, com análises exploratórias descritivas nas questões objetivas e de conteúdo, nas descritivas. Resultados: O principal envolvimento dos participantes com a inclusão escolar ocorre a partir de atendimento em ambiente clínico, porém 50% atendem, também, na escola regular. As principais formas de contribuição ocorrem através de avaliação, planejamento e desenvolvimento de atividades relacionadas a processos de inclusão, e de orientações às escolas. Destacam-se a participação da família e de outros profissionais envolvidos, em uma perspectiva colaborativa, e a falta de preparo da escola e dos professores para a efetivação dos processos de inclusão. Conclusão: Os resultados do estudo reafirmam a relevância da contribuição do terapeuta ocupacional para o enfrentamento dos desafios e a concretização da inclusão escolar. Enfatizam-se a necessidade de maior inserção destes profissionais na escola comum do ensino regular e a importância de ampliar as reflexões e problematizações em relação à construção de um sistema educacional mais democrático.

Abstract Introduction: Studies show the effectiveness of inclusive actions in educational processes, as well as the need for specialized training of occupational therapists, enabling them for collaborative approaches and actions. Objective: To identify actions developed by occupational therapists in the process of school inclusion of children and teenagers with special educational needs, and think over the context of inclusive education from the presented perspectives. Method: A survey with 18 occupational therapists for data collection. A questionnaire related to school inclusion and professional practice was applied through an on-line platform and a social network. Data gathered were forwarded to qualitative analysis, with descriptive, exploratory analysis for objective questions, and content analysis for the descriptives. Results: The main participants’ involvement in school inclusion occurred from their office practice, but 50% of them also assist in regular school. The main method of contribution occurred through assessment, planning and developing inclusion and guidance activities in schools. The participation of family and other professionals in a collaborative approach, the school and teachers lack of preparation to complete the inclusion processes were noteworthy. Conclusion: Study results reassure the importance of occupational therapist’s contribution to cope with the challenges and effectiveness of school inclusion. Emphasis is given to the need for greater integration of those professionals in regular school and the importance of expanding the considerations and questions regarding the construction of a more democratic educational system.

Uma vez modificado o complexo equilíbrio dos atributos do solo, seja através do manejo inadequado ou de alterações no conteúdo de água e no seu estado de energia, a retenção e o armazenamento de água no solo, as propriedades mecânicas, por exemplo, serão diretamente afetados, reduzindo a produtividade e aumentando os custos de produção. Diante disso, o presente trabalho objetivou avaliar as propriedades físico-hídricas de um Latossolo Vermelho distroférrico (LVdf) sob três sistemas de manejo: pastagem, cafeicultura e mata nativa. A densidade de partícula (Dp), densidade do solo (Ds) e a microporosidade (MiP) não apresentaram diferenças significativas entre os manejos. A maior retenção de água no solo foi obtida na área sob cafeicultura. Com base nos valores do parâmetro S os manejos não alteraram a qualidade física do solo. A condutividade hidráulica do solo sob mata foi maior do que dos demais sistemas de manejo estudados.

Therefore, this study aimed to evaluate the physical and hydraulic properties of an Oxisol (RH) under three management systems: pasture, coffee plantations and native forest. The particle density (SD), bulk density (Ds) and microporosity (IPM) showed no significant differences between the managements. The greater water retention in soil was obtained in the area under coffee. Based on the values of the parameter S managements did not alter the soil physical quality. FUNDAMENTOS: O impacto econômico do diagnóstico e do tratamento do melanoma cutâneo não tem sido analisado no Brasil. Uma vez que crescem tanto a incidência do câncer de pele quanto a preocupação com ele, é importante que isto seja avaliado. OBJETIVO: O objetivo deste trabalho foi estimar o custo direto de diagnosticar e tratar em seus diversos estádios o melanoma cutâneo em número de casos ocorridos no Estado de São Paulo, entre 2000 e 2007. MÉTODOS: Como modelo de procedimento, foi utilizado o projeto diretriz (Clinical Practice Guidelines in Oncology), editado pelo National Comprehensive Cancer Network (NCCN) e adequado aos procedimentos da Fundação SobecCan - Hospital do Câncer de Ribeirão Preto - SP. Os custos estimados se baseiam nos valores do tratamento médico pagos pelo setor público (Sistema Único de Saúde - SUS) e pelo setor privado (convênios) em 2007. Houve 2.740 casos diagnosticados e estadiados no Estado de São Paulo no período estudado. RESULTADOS: O custo total de tratamento dos melanomas malignos diagnosticados no estádio inicial, em valores de 2007, foi estimado em R$ 33.012.725,10 para o SUS e R$ 76.133.662,80 para os convênios.Os estádios iniciais 0, I e II da doença compreendem aproximadamente 4,2% (SUS) e 1, 3% (convênios) do custo total; os estádios III e IV consomem 95,8% e 98,7% do custo total, respectivamente. CONCLUSÃO: Os resultados apresentados reforçam o argumento de que o diagnóstico do melanoma cutâneo em seus estádios iniciais reduz os custos de tratamento, gerando considerável economia tanto para o sistema público de saúde (SUS) quanto para o sistema privado (convênios).

OBJECTIVE: The purpose of this study was to estimate the direct cost of diagnosing and treating melanoma disease in Sao Paulo (Brazil) between the years 2000 and 2007. METHODS: The project Clinical Practice Guidelines in Oncology was used, adapted to the proceedings of SOBECCan Foundation. The estimated costs were based on the values of the medical treatment paid by the Brazilian National Health System (SUS) and private healthcare insurance companies (PHIC) in 2007. RESULTS: The total cost was estimated in R$33,012,725.1 (SUS) and R$76,133,662.8 (PHIC). Stages 0, I and II comprised about 4.2% (SUS) and 1.3% (PHIC) of the total cost; stages III and IV amounted to 95.8% and 98.7% of the total cost, respectively. CONCLUSION: The diagnosis of malignant melanoma in its initial stages reduces treatment costs generating considerable savings of resources for both National Health System and private healthcare insurance companies.
