Abstract Purpose: The denervation of the intestine with benzalkonium chloride (BAC) reduces mortality and improves weight gain in rats with short bowel syndrome (SBS). Nevertheless, translating these promising findings from bench to bedside is not feasible because BAC promotes peritonitis and irreversible denervation which may be followed by an uncontrolled dilatation of the viscera. The use of botulinum toxin (BT) instead of BAC to achieve the denervation of the remaining small intestine in SBS could be an interesting option because it leads to a mild and transient denervation of the intestine. Methods: Here we evaluated the effects of the ileal denervation with BT in rats with SBS by verifying the body weight variation and intestinal morphological parameters. Four groups with 6 animals each were submitted to enterectomy with an ileal injection of saline (group E) or BT (group EBT). Control groups were submitted to simulated surgery with an ileal injection of BT (group BT) or saline (group C - control). Results: We observed that the treatment of the remaining ileum with BT completely reversed the weight loss associated to extensive small bowel resection. Conclusion: This may provide a new promising approach to the surgical treatment of SBS.

OBJETIVO: Avaliar o papel do pré-tratamento com antioxidantes dietéticos em um modelo experimental de lesão intestinal de isquemia-reperfusão (I/R) em ratos. MÉTODOS: Noventa ratos Wistar adultos machos foram utilizados. Um segmento intestinal foi isolado baseado em seu pedículo vascular. Uma biópsia controle foi realizada e o pedículo foi seccionado e anastomosado novamente, garantindo um tempo de isquemia de 60 minutos, seguido por reperfusão. Biópsias sequenciais foram realizadas ao término do período isquêmico e a cada 15 minutos, durante a reperfusão. O tratamento consistiu de solução salina ou vitamina C ou vitamina E ou a associação destas. Avaliações quantitativa e qualitativa das biópsias foram realizadas. RESULTADOS: Os grupos tratados com vitamina E isolada ou associada com vitamina C apresentaram uma atenuação estatisticamente significativa da lesão de isquemia-reperfusão, com diminuição da perda de altura dos vilos e menor infiltração neutrofílica ao final do estudo quando comparados ao grupo controle e vitamina C exclusiva. CONCLUSÃO: Neste modelo experimental de isquemia-reperfusão, o pré-tratamento com vitamina E atenuou a lesão de I/R no intestino delgado, demonstrado pela diminuição da perda de altura dos vilos e pela atenuação da infiltração neutrofílica.

OBJECTIVE: To evaluate the role of pre-treatment with dietary antioxidants in an experimental model of intestinal injury of ischemia-reperfusion (I/R) in rats. METHODS: Ninety adult male Wistar rats were used. An intestinal segment was isolated based on its vascular pedicle. A control biopsy was performed and the pedicle was sectioned and sutured again, ensuring a time of 60 minutes of ischemia followed by reperfusion. Sequential biopsies were performed at the end of the ischemic period and every 15 minutes during reperfusion. The treatment consisted of saline, vitamin C, vitamin E or a combination of the latter two. Quantitative and qualitative assessments of the biopsies were performed. RESULTS: The groups treated with vitamin E alone or vitamin E combined with vitamin C showed a statistically significant attenuation of ischemia-reperfusion, with reduced loss of height of the villi and lower neutrophilic infiltration at the end of the study when compared to the control and vitamin C-exclusive groups. CONCLUSION: In this experimental model of ischemia-reperfusion, pre-treatment with vitamin E attenuated the I/R injury in the small intestine of Rats, demonstrated by reduced loss of height of the villi and the attenuation of neutrophil infiltration.

OBJETIVO: verificar a associação entre sinais ultrassonográficos durante a gestação e evoluções pós-natais em casos de fetos com uropatias obstrutivas bilaterais, acompanhados de forma expectante. MÉTODOS: fetos com uropatias obstrutivas bilaterais apresentando oligoâmnio grave e tórax estreito foram comparados a fetos com uropatias obstrutivas bilaterais que não desenvolveram estas alterações com relação à presença ou ausência de cistos em ambos os rins e à presença ou ausência de hiperecogenicidade de parênquima em ambos os rins. Casos em que houve óbito do neonato foram comparados com aqueles em que o neonato teve alta do berçário em relação aos mesmos aspectos ecográficos renais acima citados, à presença de oligoâmnio grave e de tórax estreito. A sensibilidade, a especificidade, os valores preditivos positivo e negativo da presença de cistos renais bilaterais, hiperecogenicidade renal bilateral, oligoâmnio grave e tórax fetal estreito para óbito do neonato foram calculados. RESULTADOS: o oligoâmnio grave e o tórax estreito foram mais frequentes (p=0,03; p<0,001) nos fetos que tiveram cistos renais bilaterais quando comparados àqueles com parênquimas renais ecograficamente normais. O óbito neonatal foi mais frequente entre os casos que tiveram oligoâmnio grave (p<0,001), tórax estreito (p<0,001) e cistos renais bilaterais (p<0,002) quando respectivamente comparados aos casos sem essas alterações. Os melhores valores de sensibilidade, especificidade, valores preditivos positivo e negativo para óbito do neonato/lactente foram obtidos com o uso do aspecto ecográfico tórax estreito, tendo sido de 81,8, 100, 100 e 79,3%, respectivamente. CONCLUSÕES: Em casos de fetos com uropatias obstrutivas bilaterais acompanhados de forma expectante, os sinais ultrassonográficos mais associados ao mau prognóstico são o oligoâmnio grave, o tórax fetal estreito e a presença de cistos renais bilaterais.

PURPOSE: to verify the association between ultrasonographic signs during gestation and post-delivery evolution in fetuses with bilateral obstructive uropathies, followed up in an expectant way. METHODS: fetuses with bilateral obstructive uropathies presenting severe oligoamnios and narrow thorax have been compared with fetuses with bilateral obstructive uropathies without those alterations, concerning the presence or absence of cysts in both kidneys, and the presence or absence of parenchymal hyperechogenicity in both kidneys. Cases of neonatal death were compared with cases of neonatal discharge from the nursery, regarding the same renal echographic aspects mentioned above, the presence of severe oligoamnios and narrow thorax. The sensitivity, specificity, positive and negative predictive value of the presence of bilateral renal cysts, bilateral renal hyperechogenicity, severe oligoamnios and narrow fetal thorax for the neonatal death were calculated. RESULTS: severe oligoamnios and narrow thorax were more frequent (p=0.03; p<0.001) in fetuses with bilateral renal cysts, as compared to those with echographically normal renal parenchyma. Neonatal death was more frequent among cases with severe oligoamnios (p<0.001), narrow thorax (p<0.001) and bilateral renal cysts (p<0.002), when respectively compared with cases without those alterations. The best values of sensitivity, specificity, positive and negative predictive value for the death of neonatal/breastfeeding infants were obtained using the echographic aspect of narrow thorax, and were 81.8, 100, 100 and 79.3%, respectively. CONCLUSIONS: in cases of fetuses with bilateral obstructive uropathies followed up in an expectant way, the ultrasonographic signs more associated to bad prognosis are severe oligoamnios, narrow fetal thorax and presence of bilateral renal cysts.

RESUMO Objetivo: avaliar o papel do diagnóstico pré-natal da gastrosquise na qualidade do atendimento e na evolução de neonatos (RN) com gastrosquise. Métodos: estudo retrospectivo de 25 casos de RN com gastrosquise, abordados na Universidade Estadual de Campinas, entre janeiro de 1989 e junho de 1998. Resultados: comparando-se os grupos com diagnóstico pré-natal (PN) e neonatal (NN), não foram observadas diferenças quanto à incidência de prematuridade, peso ao nascer e relação peso com idade gestacional. A mediana do tempo entre o nascimento e a correção da gastrosquise foi de 5 horas. Tal intervalo foi menor no grupo PN que no grupo NN (2,25 h versus 13 h; p<0,05). Em 17 RN (68%) foi possível o fechamento primário do defeito, sem diferença no número de casos entre os grupos PN e NN. Embora esta diferença não tenha atingido significância estatística, a sobrevida foi maior no grupo PN (85,7 versus 45,5%). Conclusões: o diagnóstico pré-natal de gastrosquise contribui favoravelmente para o atendimento perinatal adequado, resultando em melhora sensível dos índices de mortalidade destas crianças.

SUMMARY Purpose: to evaluate the role of prenatal diagnosis of gastroschisis in the quality of assistance to and in the evolution of neonates with gastroschisis. Methods: a retrospective survey of 25 charts of neonates with gastroschisis treated at the State University of Campinas, between January 1989 and June 1998. Results: comparing the groups with prenatal (PN) and neonatal (NN) diagnosis, no differences regarding incidence of prematurity, birth weight and relations hip between weight and gestational age were observed. The median time from birth to surgery was 5 h. Such interval was shorter in the PN than the NN group (2.25 h versus 13 h; p<0.05). Primary closure of the defect was achieved in 17 neonates (68%) of both groups. Although this difference was not statistically significant, survival was higher among infants of the PN group (85.7% versus 45.5%). Conclusions: prenatal diagnosis of gastroschisis contributes favorably to improve perinatal assistance, resulting in reduced mortality of these children.
