Abstract Background Most of the evidence about the impact of the post-acute COVID-19 Syndrome (PACS) reports individual symptoms without correlations with related imaging. Objectives To evaluate cardiopulmonary symptoms, their predictors and related images in COVID-19 patients discharged from hospital. Methods Consecutive patients who survived COVID-19 were contacted 90 days after discharge. The Clinic Outcome Team structured a questionnaire evaluating symptoms and clinical status (blinded for hospitalization data). A multivariate analysis was performed to address the course of COVID-19, comorbidities, anxiety, depression, and post-traumatic stress during hospitalization, and cardiac rehabilitation after discharge. The significance level was set at 5%. Results A total of 480 discharged patients with COVID-19 (age: 59±14 years, 67.5% males) were included; 22.3% required mechanical ventilation. The prevalence of patients with PACS-related cardiopulmonary symptoms (dyspnea, tiredness/fatigue, cough, and chest discomfort) was 16.3%. Several parameters of chest computed tomography and echocardiogram were similar in patients with and without cardiopulmonary symptoms. The multivariate analysis showed that PACS-related cardiopulmonary-symptoms were independently related to female sex (OR 3.023; 95% CI 1.319-6.929), in-hospital deep venous thrombosis (OR 13.689; 95% CI 1.069-175.304), elevated troponin I (OR 1.355; 95% CI 1.048-1.751) and C-reactive protein during hospitalization (OR 1.060; 95% CI 1.023-1.097) and depression (OR 6.110; 95% CI 2.254-16.558). Conclusion PACS-related cardiopulmonary symptoms 90 days post-discharge are common and multifactorial. Beyond thrombotic and markers of inflammation/myocardial injury during hospitalization, female sex and depression were independently associated with cardiopulmonary-related PACS. These results highlighted the need for a multifaceted approach targeting susceptible patients.

Resumo Fundamento A maioria da evidência sobre o impacto da síndrome COVID pós-aguda (PACS, do inglês, post-acute COVID-19 syndrome) descreve sintomas individuais sem correlacioná-los com exames de imagens. Objetivos Avaliar sintomas cardiopulmonares, seus preditores e imagens relacionadas em pacientes com COVID-19 após alta hospitalar. Métodos Pacientes consecutivos, que sobreviveram à COVID-19, foram contatados 90 dias após a alta hospitalar. A equipe de desfechos clínicos (cega quanto aos dados durante a internação) elaborou um questionário estruturado avaliando sintomas e estado clínico. Uma análise multivariada foi realizada abordando a evolução da COVID-19, comorbidades, ansiedade, depressão, e estresse pós-traumático durante a internação, e reabilitação cardíaca após a alta. O nível de significância usado nas análises foi de 5%. Resultados Foram incluídos 480 pacientes (idade 59±14 anos, 67,5% do sexo masculino) que receberam alta hospitalar por COVID-19; 22,3% necessitaram de ventilação mecânica. A prevalência de pacientes com sintomas cardiopulmonares relacionados à PACS (dispneia, cansaço/fadiga, tosse e desconforto no peito) foi de 16,3%. Vários parâmetros de tomografia computadorizada do tórax e de ecocardiograma foram similares entre os pacientes com e sem sintomas cardiopulmonares. A análise multivariada mostrou que sintomas cardiopulmonares foram relacionados de maneira independente com sexo feminino (OR 3,023; IC95% 1,319-6,929), trombose venosa profunda durante a internação (OR 13,689; IC95% 1,069-175,304), nível elevado de troponina (OR 1,355; IC95% 1,048-1,751) e de proteína C reativa durante a internação (OR 1,060; IC95% 1,023-1,097) e depressão (OR 6,110; IC95% 2,254-16,558). Conclusão Os sintomas cardiopulmonares relacionados à PACS 90 dias após a alta hospitalar são comuns e multifatoriais. Além dos marcadores trombóticos, inflamatórios e de lesão miocárdica durante a internação, sexo feminino e depressão foram associados independentemente com sintomas cardiopulmonares relacionados à PACS. Esses resultados destacaram a necessidade de uma abordagem multifacetada direcionada a pacientes susceptíveis.

ABSTRACT CONTEXT: Heart failure in Brazil is a major public health problem and, even with advances in treatment, it still presents high morbidity and mortality. As a treatment option, mechanical circulatory assist devices (MCADs) have greatly increased in importance over the last decade. CASE REPORT: This report concerns a case of refractory cardiogenic shock due to acute myocarditis in a 35-year-old puerperal female patient who presented with retrosternal pain, fatigue and dyspnea. At the hospital, she was diagnosed with myocarditis. There was no improvement in perfusion even after receiving dobutamine, intra-aortic balloon passage (IAB) and venoarterial extracorporeal membrane oxygenation (VA-ECMO). Therefore, it was decided to implant a MCAD (CentriMag). During hospitalization, recovery from the bi-ventricular dysfunction was achieved. The CentriMag device was removed 10 days after it had been implanted, and the patient was discharged after another 8 days. The myocarditis was proven to be due to the Coxsackie virus. CONCLUSIONS: The decision to implant a MCAD should be individualized, as patient profiles do not always match the indications in the guidelines and protocols. In this study, clinical discussion of the case among the medical and multi-professional teams was essential in order to be able to successfully reverse the patient’s severe clinical condition without sequelae, through using a CentriMag implant.

Neste trabalho, relata-se os resultados obtidos no primeiro momento de uma pesquisa que objetiva conhecer o perfil e a formação de professores da área da saúde que usam a simulação como estratégia didática no Laboratório de Habilidades (LabHab) da Faculdade de Medicina da Universidade de São Paulo. Com previsão de dois anos, a pesquisa busca contribuir para uma proposta de formação docente que observe as prioridades e a relevância da simulação nas suas atividades didáticas. A ideia é analisar como esse profissional recria seus conhecimentos em grupo para trabalhar a simulação na atuação clínica e observar como ele percebe o alcance dos processos de simulação aprimorados pela tecnologia e pelas propostas de inovação educativa.

This article reports the results of the first stage of a study aimed at evaluating the profile and training of professors from the health fields that use simulation as a learning strategy in Laboratory Skills (LabHab) at the University of São Paulo School of Medicine. Scheduled to last two years, the study seeks to contribute to a proposal for teacher training that meets the priorities and relevance of simulation in teaching activities based on an analysis of how medical professors recreate their knowledge to work with simulation in clinical practice and how they perceive the scope of technology-enhanced simulation processes and proposals for educational innovations.

OBJETIVOS: Avaliar os efeitos da utilização do biofeedback respiratório (BR) associado ao padrão quiet breathing sobre a perimetria torácica, função pulmonar, força dos músculos respiratórios e os seguintes hábitos de respiradores bucais funcionais (RBF): vigília de boca aberta, boca aberta durante o sono, baba no travesseiro, despertar difícil, ronco e sono inquieto. MÉTODOS: Foram avaliadas 20 crianças RBF, as quais foram submetidas a 15 sessões de BR por meio do biofeedback pletsmovent (MICROHARD® V1.0), o qual proporciona o biofeedback dos movimentos tóraco-abdominais. Perimetria torácica, espirometria e medidas das pressões respiratórias máximas estáticas foram realizadas antes e após a terapia. Questões respondidas pelos responsáveis foram utilizadas para avaliar os hábitos dos RBF. Os dados foram analisados por meio de teste t de Student para dados pareados e testes não paramétricos. RESULTADOS: O uso do BR associado ao padrão quiet breathing não produziu alterações significativas na perimetria torácica e nos valores de volume expiratório forçado no primeiro segundo (VEF1), capacidade vital forçada (CVF), pico de fluxo expiratório (PFE), índice de Tiffeneau (IT) e na pressão expiratória máxima (PEmáx). Entretanto, a pressão inspiratória máxima (PImáx) apresentou diferença estatisticamente significativa (-53,6 ± 2,9 cmH2O vs. -65,0 ± 6,0 cmH2O; p< 0,05) e ocorreram mudanças significativas nos hábitos avaliados. CONCLUSÃO: Os resultados permitem concluir que o BR associado ao padrão quiet breathing melhora a força da musculatura inspiratória e hábitos em RBF, podendo ser, portanto, utilizado como uma das formas de terapia nesses indivíduos.

OBJECTIVE: To evaluate the effects of using respiratory biofeedback associated with a quiet breathing pattern, on chest circumference, pulmonary function, respiratory muscle strength and the following functional mouth-breathing habits: watching things with mouth open, sleeping with mouth open, dribbling on the pillow, difficulty in waking up, snoring and restlessness during sleep. METHOD: Twenty functional mouth-breathing children were evaluated. They underwent 15 sessions of respiratory biofeedback by means of the biofeedback pletsmovent (MICROHARD® V1.0), which provided biofeedback on thoracoabdominal movements. Chest circumference, spirometry and maximum static respiratory pressures were determined before and after the therapy. The adults responsible for these children were asked questions about the children's mouth-breathing habits. Student's t test for paired data and non-parametric tests were used to analyze the data. RESULTS: The use of respiratory biofeedback in association with a quiet breathing pattern did not produce significant alterations in chest circumference or in forced expiratory volume in the first second (FEV1), forced vital capacity (FVC), peak expiratory flow (PEF), Tiffeneau index (TI) or maximal expiratory pressure (MEP). However, there was a significant difference in maximal inspiratory pressure (MIP) (-53.6 ± 2.9 cmH2O vs. -65.0 ± 6.0 cmH2O; p< 0.05) and there were significant changes in the evaluated habits. CONCLUSION: The results allow the conclusion that respiratory biofeedback associated with a quiet breathing pattern improves the inspiratory muscle strength and habits of functional mouth-breathers. It can therefore be used as a therapeutic method for such individuals.
