Resumo Para avaliar o volume de fissuras dentinárias levando em consideração a idade da dentina e o tipo de sistema de limas utilizadas para procedimentos endodônticos, 40 dentes unirradiculares, incisivos inferiores, apresentando similar volume radicular, recentemente extraídos foram divididos em dois grupos de acordo com a idade do paciente: Grupo Jovem (18 a 30 anos) e Grupo Idoso (60 anos ou mais). Cada espécime foi scaneada por microtomografia computadorizada (micro-CT) em três estágios: (i) antes de qualquer tratamento, (ii) após tratamento endodôntico com limas Reciproc (REC) e (iii) após subsequente retratamento endodôntico. Cada grupo foi subdividido em dois subgrupos, de acordo com a técnica de retratamento utilizada: retratamento com REC ou com lima ProTaper Universal Retreatment (PUR). As imagens foram analisadas quanto às diferenças no volume das fissuras dentinárias nos terços médio e apical dos dentes de acordo com a idade da dentina. Em ambos os estágios (antes e depois da instrumentação), as imagens de micro-CT da dentina radicular idosa apresentaram maior volume de trincas do que as jovens, sem significância estatística (p>0.01). O uso das limas REC mostram não ter diferença estatística significante na geração de fissuras, em qualquer tipo de dentina, quando comparadas às limas PUR (p>0.01). Quando retratada com PUR, a dentina radicular idosa apresentou um volume de trincas significativamente maior (p<0.01) quando comparado com a dentina radicular idosa inicialmente.

Abstract In order to evaluate the volume of dentinal cracks taking into account the age of the dentin and the type of file system used for endodontic procedures, forty freshly extracted single-rooted lower incisive teeth presenting similar root volume, were divided into two groups according to the age of the patient: Group Young (18 - 30 years old) and Group Old (60 years old or more). Each specimen was scanned by microcomputed tomography (micro-CT) in three stages: (i) before any treatment, (ii) after endodontic treatment with Reciproc files (REC), and (iii) after subsequent endodontic retreatment. Each group was subdivided into two subgroups, according to the retreatment technique used: retreatment with REC or with ProTaper Universal Retreatment (PUR) files. For each subgroup, the images were analyzed for differences in the volume of dentinal cracks in the middle and apical thirds of the teeth , according to the dentin age. In both stages (before and after instrumentation), the micro-CT images of the old root dentin presented with higher volume of cracks than those of the young root dentin, statistical significance notwithstanding (p>0.01). The use of REC files appeared to have no statistically significant differences in the generation of cracks in any type of root dentin when compared with PUR files (p>0.01). When retreated with PUR, the old root dentin presented with a significantly higher volume of cracks (p<0.01) when compared with the old root dentin initially.

Resumo Este estudo teve como objetivo comparar a citotoxicidade dos materiais restauradores indiretos Vita AC12, Lava Ultimate, Vita Enamic e InSync. Extratos de cada material foram preparados por incubação por 1, 7 e 40 dias, com lavagem diária. Fibroblastos gengivais humanos foram expostos aos extratos e a viabilidade celular foi medida por avaliação sequencial da atividade mitocondrial (XTT), integridade da membrana (NRU) e densidade celular (CVDE). Extratos de esferas de poliestireno e fragmentos de látex foram utilizados como controles negativos e positivos, respectivamente. As diferenças entre os grupos e os tempos experimentais foram avaliadas por análise de variância. Na extração de 24 h, observaram-se diferenças significativas entre o controle e Vita AC-12 e InSync no teste do XTT (p<0,05) e entre o controle e os materiais Enamic e Lava Ultimate, no teste CVDE (p<0,05 ). AC12, Lava Ultimate e InSync apresentaram viabilidade celular significativamente menor do que o Enamic e o grupo controle, no ensaio NRU (p<0,05). Os materiais cerâmicos híbridos Vita Enamic e Lava Ultimate apresentaram melhor biocompatibilidade no ponto de tempo de extração de 24 h do que os materiais cerâmicos AC12 e InSync. No entanto, uma simulação de remoção de componentes tóxicos por fluidos biológicos, realizada com o uso de tempos de extração mais prolongados e lavagem diária, levou à ausência de citotoxicidade em todos os materiais restauradores testados. Esses achados podem ser vistos como positivos para a indicação clínica desses materiais restauradores, considerando seu contato com tecidos macios adjacentes por longos períodos de tempo.

Abstract This study aimed to compare the cytotoxicity of the Vita AC12, Lava Ultimate, Vita Enamic and InSync indirect restorative materials. Extracts of each material were prepared by incubation for 1, 7 and 40 days, with daily washing. Human gingival fibroblasts were exposed to the extracts, and cell viability was evaluated by sequential assessment of mitochondrial activity (XTT), membrane integrity (NRU) and cell density (CVDE). Extracts of polystyrene beads and latex fragments were used as negative and positive controls, respectively. Differences between groups and experimental times were evaluated by analysis of variance. At the 24 h extraction, significant differences between the control and both Vita AC-12 and InSync were observed in the XTT assay (p<0.05), and between the control and both Enamic and Lava Ultimate, in the CVDE assay (p<0.05). AC12, Lava Ultimate, and InSync presented significantly lower cell viability than Enamic and the control group, in the NRU assay (p<0.05). The Vita Enamic and Lava Ultimate hybrid ceramic-like materials presented better biocompatibility at the 24 h extraction time point than the AC12 and InSync ceramic materials. However, a simulation of the removal of toxic components by biological fluids, conducted by using longer extraction times and daily washing, led to the absence of cytotoxicity in all the tested restorative materials. These findings can be viewed as positive for the clinical indication of these restorative materials, considering their contact with adjacent soft tissues for extended periods of time.

Abstract Denture adhesives (DA) improve the retention and stability of ill-fitting dentures, especially for older adults. These materials should be biocompatible, i.e., they cannot cause undesired biological responses and be non-cytotoxic to oral tissues. However, in vitro testing of DA biocompatibility employing primary cell culture may possibly be affected by other factors, such as the donor age. Objective To compare the cytotoxicity of three different denture adhesives when assessed in primary gingival fibroblasts from a young donor or from an older donor, as well as the release of the basic fibroblast growth factor (bFGF), and the inflammatory response marker interleukin-6 (IL-6). Material and Methods Gingival fibroblasts isolated from a 30- and a 62-year-old donor were assayed for proliferation (1-7 days) and sensitivity to latex (positive control). Fibroblasts were indirectly exposed to Corega Ultra (cream), Corega powder and Fixodent Original for a 24 h period and assayed by XTT and Crystal Violet tests. The release of IL-6 and bFGF by exposed cells was determined by ELISA. Results While cells from the young donor presented higher cell growth after 7 days, the sensitivity to increasing concentrations of latex extracts was very similar between young and older cells. Both XTT and CVDE detected no difference between the DA and the control group. All materials induced higher levels of IL-6 and bFGF compared to control. Cells from the older donor exposed to Corega Ultra released lower levels of cytokine and growth factor. Conclusions All materials were considered non-cytotoxic, but affected cytokine and growth factor release. The biological differences found between fibroblasts from both donors could be due to individual or age-related factors. The authors suggest the use of cells from older donors on studies of dental products aimed at older patients, to better simulate their physiological response.

Abstract Calcium silicate-based materials have been widely studied due to their resemblance to, and similar applicability of, mineral trioxide aggregate (MTA). Among these, Biodentine™ (BD) was specifically designed as a “dentin replacement” material for applications such as root perforations, apexification, treatment of resorptive lesions, and as a retrograde filling material. The present study aimed to assess the in vitro response of human primary osteoblasts to BD using MTA AngelusTM as a reference material, by simultaneously analyzing three different cell viability parameters, namely mitochondrial activity, membrane integrity, and cell density. BD and MTA extracts were prepared by incubation on culture media for 24 h or 42 days after mixing. Primary human osteoblasts were exposed to extracts for 24 h, at 37oC with 5% CO2, and cell viability was evaluated by the XTT, NRU, and CVDE assays. Both materials induced cell viability levels higher than 70% when extracted for 24 h. However, when cells were exposed to extracts with increased conditioning times, MTA presented significant cytotoxic effects (p < 0.05) in comparison to the control and MTA at 24 h. After 42 days, the XTT assay identified a significant reduction in cell viability by BD when compared to the control (p<0.05), despite the fact that levels above the 70% viability cutoff were attained for biocompatible materials. It can be concluded that BD is cytocompatible with human primary osteoblasts, indicating its adequacy in direct contact with bone tissues.

Abstract The aim of this study was to undertake a qualitative and quantitative assessment of nanoscale alterations and wear on the surfaces of nickel-titanium (NiTi) endodontic instruments, before and after use, through a high-resolution, noncontact, three-dimensional optical profiler, and to verify the accuracy of the evaluation method. Cutting blade surfaces of two different brands of NiTi endodontic instruments, Reciproc R25 (n = 5) and WaveOne Primary (n = 5), were examined and compared before and after two uses in simulated root canals made in clear resin blocks. The analyses were performed on three-dimensional images which were obtained from surface areas measuring 211 × 211 µm, located 3 mm from their tips. The quantitative evaluation of the samples was conducted before and after the first and second usage, by the recordings of three amplitude parameters. The data were subjected to statistical analysis at a 5% level of significance. The results revealed statistically significant increases in the surface wear of both instruments groups after the second use. The presence of irregularities was found on the surface topography of all the instruments, before and after use. Regardless of the evaluation stage, most of the defects were observed in the WaveOne instruments. The three-dimensional technique was suitable and effective for the accurate investigation of the same surfaces of the instruments in different periods of time.

Resumo Resinas bisacrílicas são usadas em restaurações dentárias provisórias e têm mostrado vantagens em relação a outros materiais. O objetivo deste trabalho foi avaliar a citotoxicidade in vitro de duas resinas compostas bisacrílicas (Protemp 4 e Luxatemp Star), obtidas após 1, 7 e 40 dias da mistura com os componentes da resina, usando um ensaio padronizado empregando células primárias humanas fortemente relacionadas aos tecidos orais. Culturas de células de fibroblastos gengivais humanos foram expostas por 24 h aos extratos das resinas bisacrílicas, esferas de poliestireno (controle negativo) e látex (controle positivo), obtidos após diferentes períodos de incubação, a 37 °C e com 5% CO2. A viabilidade celular foi avaliada usando procedimentos multiparamétricos que envolvem a avaliação sequencial (usando as mesmas células) da atividade mitocondrial (ensaio XTT), a integridade de membrana (teste do vermelho neutro) e a densidade celular total (teste de exclusão do corante cristal violeta). As células expostas aos extratos mostraram viabilidade celular acima de 75% depois de 24 h. Mesmo quando as células foram expostas aos extratos com aumento do tempo de condicionamento, as resinas bisacrílicas não apresentaram efeitos citotóxicos significativos (p<0,05), comparadas ao grupo controle ou em relação às primeiras 24 h de contato com os produtos. Não houve diferença entre os resultados obtidos para as resinas bisacrílicas avaliadas entre as 24 h, 7 e 40 dias depois da mistura. Concluímos que as resinas bisacrílicas Protemp 4 e Luxatemp Star foram citocompatíveis com os fibroblastos gengivais humanos, sugerindo que ambos materiais são adequados para uso em contato com tecidos humanos.

Abstract Bis-acryl resins are used for temporary dental restorations and have shown advantages over other materials. The aim of this work was to evaluate the in vitro cytotoxicity of two bis-acryl composite resins (Protemp 4 and Luxatemp Star), obtained at 1, 7 and 40 days after mixing the resin components, using a standardized assay employing human primary cells closely related to oral tissues. Human gingival fibroblast cell cultures were exposed for 24 h to either bis-acryl composite resins, polystyrene beads (negative control) and latex (positive control) extracts obtained after incubation by the different periods, at 37 °C under 5% CO2. Cell viability was evaluated using a multiparametric procedure involving sequential assessment (using the same cells) of mitochondrial activity (XTT assay), membrane integrity (neutral red test) and total cell density (crystal violet dye exclusion test). The cells exposed to the resin extracts showed cell viability indexes exceeding 75% after 24 h. Even when cells were exposed to extracts prepared with longer conditioning times, the bis-acryl composite resins showed no significant cytotoxic effects (p>0.05), compared to the control group or in relation to the first 24 h of contact with the products. There were no differences among the results obtained for the bis-acryl composite resins evaluated 24 h, 7 days and 40 days after mixing. It may be concluded that the bis-acryl resins Protemp 4 and Luxatemp Star were cytocompatible with human gingival fibroblasts, suggesting that both materials are suitable for use in contact with human tissues.

The aim of this study was to comparatively evaluate the bending resistance at 45º, the static and dynamic cyclic fatigue life, and the fracture type of the WaveOne (Dentsply Maillefer, Ballaigues, Switzerland) 25-08 and Reciproc (VDW, Munich, Germany) 25-08 instruments. A total of 60 nickel-titanium (NiTi) instruments (30 Reciproc and 30 WaveOne) from three different lots, each of which was 25 mm in length, were tested. The bending resistance was evaluated through the results of a cantilever-bending test conducted using a universal testing machine. Static and dynamic cyclic fatigue testing was conducted using a custom-made device. For the static and dynamic tests, a cast Ni-Cr-Mo-Ti alloy metal block with an artificial canal measuring 1.77 mm in diameter and 20.00 mm in total length was used. A scanning electron microscope was used to determine the type of fracture. Statistical analyses were performed on the results. The WaveOne instrument was less flexible than the Reciproc (p < 0.05). The Reciproc instrument showed better resistance in the static and dynamic cyclic fatigue tests (p < 0.05). The transverse cross-section and geometry of the instruments were important factors in their resistance to bending and cyclic fracture. Both of the instruments showed ductile-type fracture characteristics. It can be concluded that the Reciproc 25-08 instrument was more resistant to static and dynamic cyclic fatigue than the WaveOne 25-08 instrument, while the WaveOne 25-08 instrument was less flexible. Bending and resistance to cyclic fracture were influenced by the instruments’ geometries and transverse cross-sections. Both of the instruments showed ductile-type fracture characteristics.

The aim of this work was to evaluate the effects of different times of extraction on the cytotoxicity of six representatives of different root canal sealer groups-Real Seal SE, AH Plus, GuttaFlow, Sealapex, Roth 801, and ThermaSeal Plus-with human gingival fibroblasts. The materials were prepared according to manufacturers' specifications, and were incubated in culture medium (DMEM) at 37ºC for 1, 7, 14, 21, and 28 days, with daily washing, to simulate periodontal ligament clearance. Human fibroblasts were exposed to the final extracts at 24 hours, and cell viability was determined by MTT assay, with exposure to unconditioned DMEM as a negative control. Statistical analysis comparing cytotoxicities at each exposure time was performed by ANOVA with Scheffé adjustment for multiple comparisons at a 95% confidence level. Results indicated that GuttaFlow was significantly less cytotoxic than all other sealers (p < 0.05) at 1 day of extraction. After 7 days of extraction, cell viability for GuttaFlow was significantly increased as compared with that of all groups except sealer AH Plus. At day 14, cytotoxicity of Sealapex was significantly higher than that of all other sealers (p < 0.05). At days 21 and 28, there were no significant differences in cytotoxicity among sealer groups. All materials presented some level of cytotoxicity to fibroblasts, while GuttaFlow was the least cytotoxic sealer tested. However, the cytotoxicity of all materials seemed to decrease similarly in a time-dependent manner.

Oral health is a prerequisite for a good chewing function, which may have an impact on food choices and nutritional well-being. OBJECTIVE: This study was designed to evaluate the relationship between oral health status and nutritional status in the elderly. MATERIAL AND METHODS: In this cross-sectional study, 33 elderly people from the Group for the Elderly Interdisciplinary Geriatrics and Gerontology Program, at Fluminense Federal University, Niteroi, RJ, Brazil, completed a questionnaire to collect information on socioeconomic status, eating habits, physical activity and health habits, undertook a clinical oral examination, blood test, and anthropometric measurements, and were allocated into groups according to age. The oral health status was assessed using the index for decayed, missing and filled teeth (DMFT). The nutritional status was assessed using hemoglobin, hematocrit and albumin concentrations in blood, anthropometric values and the body mass index. RESULTS: Tooth loss was the biggest nuisance to the elderly subjects (57.6%), followed by the use of dentures (30.3%) and ill-fitting dentures (33.3%). 66.6% of patients had difficulty in chewing, and 54.5% reported this to be due to prostheses and 13.6% to the absence of teeth. A significant correlation was found between DMFT and the value of suprailiac skinfold thickness (rho=0.380, p=0.029). CONCLUSION: The results support the temporal association between tooth loss and detrimental changes in anthropometry, which could contribute to increased risk of developing chronic diseases.

OBJECTIVE: The aim of this investigation was to evaluate the cytotoxicity of two brands of root canal sealers, epoxy-resin based and zinc oxide-eugenol based, and one commercial calcium hydroxide paste on a monocyte cell line THP-1. MATERIAL AND METHODS: Undiluted (crude extract) and diluted extracts to 10%, 1%, 0.1%, 0.01%, 0.001% and 0.0001% of the sealers were tested for cytotoxicity to THP-1 cells using the trypan blue assay. Extracts were obtained according to ISO standard. Data were analyzed statistically by the Kruskal-Wallis and Mann-Whitney tests at 5% significance level. RESULTS: Crude extract of AH Plus and Fill Canal killed approximately 90% of THP-1 cells versus 36% of THP-1 cells killed by L&C crude extract (p<0.05). Ten-fold dilutions of L&C, Fill Canal and AH Plus killed 24, 35 and 61% of THP-1 cells (p<0.05), respectively. Dilutions lesser than 1% caused minimal cell death as compared to the control groups (p>0.05), except for L&C 1% extract. CONCLUSIONS: The results revealed that the L&C paste crude extract was less cytotoxic to THP-1 cells than AH Plus or Fill Canal crude extracts.

A eficácia das pastas de hidróxido de cálcio: Calen® e Calen-PMCC® associadas ao preparo químico-mecânico, foi avaliada sobre Enterococcus faecalis cultivados no interior dos canais radiculares. Setenta incisivos foram inseridos em caldo TSB, esterilizados e contaminados com E. faecalis. O meio de cultivo foi substituído a cada 24 h, sendo incubados a 37oC por 72 h. Após o preparo químico-mecânico, os canais radiculares foram preenchidos com Calen® ou Calen-PMCC® (7 ou 14 dias). As pastas foram removidas e os dentes inseridos em tubos contendo caldo Enterococcosel. A pasta Calen® (mantida por 7 ou 14 dias) induziu a eliminação dos enterococos em 70% dos dentes, enquanto a pasta Calen-PMCC® induziu a eliminação em 100% dos dentes, somente após a manutenção por 14 dias. Tais medicações foram significativamente mais efetivas (p<0,001) do que o protocolo do preparo químico-mecânico e o Calen-PMCC® mantido por 7 dias, ambos incapazes de eliminar os enterococos. As pastas de hidróxido de cálcio demonstraram efeitos adjuvantes importantes na eliminação dos enterococos durante o preparo químico-mecânico dos sistemas de canais radiculares. Quando associada ao PMCC, as pastas de hidróxido de cálcio devem ser mantidas por pelo menos 14 dias.

The effectiveness of calcium hydroxide pastes: Calen™ and PMCC-Calen™ associated to chemo-mechanical preparation was assessed on Enterococcus faecalis grown within root canals. Seventy incisors were inserted into TSB medium, sterilized and contaminated with E. faecalis. Culture medium was replaced each 24 h and incubated at 37oC for 72 h. After chemo-mechanical preparation, root canals were filled with Calen™ or PMCC-Calen™ (7 or 14 days). Pastes were removed and teeth were inserted into test tubes containing Enterococcosel broth. Calen™ paste (maintained for 7 and 14 days) induced 70% elimination of enterococci and PMCC-Calen™ 100% elimination only after maintenance for 14 days. These medications were significantly more effective (p<0.001) than chemo-mechanical protocol alone and PMCC-Calen™ maintained for 7 days, both incapable to eliminate the viability of enterococci. Calcium hydroxide pastes demonstrated important adjuvant effects in the elimination of enterococci during chemo-mechanical preparation of root canal systems. When associated with PMCC, calcium hydroxide pastes should be maintained for at least 14 days.

This study compared, by scanning electron microscopy (SEM), the efficacy of three solvents on the removal of filling materials from dentinal tubules during endodontic retreatment. Forty human maxillary canines with straight canals were prepared according to a crown-down technique and enlarged to a#30 apical file size, before obturation with gutta-percha and a zinc-oxide-eugenol based sealer. The samples were stored for 3 months before being randomly assigned to four groups: chloroform (n=10), orange oil (n=10), eucalyptol (n=10) and control (n=10). Solvents were applied to a reservoir created on the coronal root third using Gates Glidden drills. The total time for retreatment using the solvents was 5 minutes per tooth. Following retreatment the roots were split longitudinally for SEM evaluation. SEM images were digitized, analyzed using Image ProPlus 4.5 software, and the number of dentinal tubules free of filling material from the middle and apical thirds was recorded. No significant difference was found among the solvent groups regarding the number of dentinal tubules free of root filling remnants in the middle and apical root thirds (p>0.05). However, the control group had fewer dentinal tubules free of filling material (p<0.05). Under the tested conditions, it may be concluded that there was no significant difference among the solvents used to obtain dentinal tubules free of filling material remnants.

O objetivo do presente trabalho foi avaliar e comparar o efeito antimicrobiano do MTA Dentsply, MTA Angelus, hidróxido de cálcio e cimento Portland sobre quatro cepas bacterianas: Pseudomonas aeruginosa, Escherichia coli, Bacteroides fragilis, e Enterococcus faecalis. Placas contendo agar Muller-Hinton suplementadas com 5% de sangue de carneiro, hemina e menadiona foram inoculadas com as suspensões bacterianas. Poços foram confeccionados com auxílio de perfuradores e imediatamente preenchidos com os materiais, e incubados a 37ºC por 48 horas em atmosfera de anaerobiose, exceto P. aeruginosa. O diâmetro dos halos de inibição foi medido e os dados analisados usando o teste estatístico ANOVA e o de Tukey com nível de significância de 1%. O MTA Dentsply, MTA Angelus e Cimento Portland inibiram o crescimento da P.aeruginosa. O hidróxido de cálcio foi efetivo contra P. aeruginosa e B. fragillis. Sob atmosfera de anaerobiose, condição que pode impedir a formação de espécies reativas do oxigênio, nenhum dos materiais foi capaz de exercer efeitos sobre E. faecalis e E. coli.

The present study aimed to evaluate and compare the antimicrobial effect of MTA Dentsply, MTA Angelus, Calcium Hydroxide and Portland cement. Four reference bacterial strains were used: Pseudomonas aeruginosa, Escherichia coli, Bacteroides fragilis, and Enterococcus faecalis. Plates containing Mueller-Hinton agar supplemented with 5% sheep blood, hemin, and menadione were inoculated with the bacterial suspensions. Subsequently, wells were prepared and immediately filled with materials and incubated at 37ºC for 48 hours under anaerobic conditions, except P. aeruginosa. The diameters of inhibition zones were measured, and data analyzed using ANOVA and the Tukey test with 1% level of significance. MTA Dentsply, MTA Angelus and Portland cement inhibited the growth of P. aeruginosa. Calcium Hydroxide was effective against P. aeruginosa and B. fragillis. Under anaerobic conditions, which may hamper the formation of reactive oxygen species, the materials failed to inhibit E. faecalis, and E. coli.
