ABSTRACT Both systemic lupus erythematosus and antiphospholipid syndrome are autoimmune diseases with thromboembolic potential, especially due to the presence of thrombogenic antibodies. The lung is a common place where a thrombus usually settles and generates a thromboembolism, sometimes with subsequent infarction and cavitation. There are few studies reporting cavitary pulmonary infarction in a patient with lupus associated with antiphospholipid syndrome. We present the case of a 24-year-old woman with general symptoms and cavitated right lung lesion. She was initially treated as tuberculous or fungal infection. Laboratory tests and images guided and diagnosed lupus associated with antiphospholipid syndrome, complicated by pulmonary thromboembolism that later became cavitated. The patient improved considerably with anticoagulants, corticosteroids, and cyclophosphamide.

RESUMEN Tanto lupus eritematoso sistémico como el síndrome antifosfolípido son enfermedades autoinmunes con potencial tromboembólico, sobre todo por la presencia de anticuerpos trombogénicos. El pulmón es un lugar común donde suele asentarse un trombo y generar una tromboembolia, a veces con posterior infarto y cavitación. Existen pocos estudios que informen un infarto pulmonar cavitado en un paciente con lupus asociado a síndrome antifosfolípido. Presentamos el caso de una mujer de 24 años con síntomas generales y lesión pulmonar derecha cavitada. Fue tratada inicialmente como infección tuberculosa o fúngica. La analítica y las imágenes orientaron y diagnosticaron lupus asociado a síndrome antifosfolípido, complicado con tromboembolismo pulmonar que luego pasó a cavitarse. La paciente mejoró considerablemente con anticoagulantes, corticoides y ciclofosfamida.

Abstract: The identification of multiple risk factors that predispose to bleeding during the obstetric event, such as acquired hemophilia, which is a disorder that develops due to the generation of autoantibodies that inhibit coagulation factors, the objective interpretation of routine laboratory tests , the development of systematized thinking in diagnostic-therapeutic integration by health personnel, and the provision of hospital pharmacological resources, is what frequently determines the prognosis in obstetric patients with extreme morbidity who require multidisciplinary care in the different hospital units of the health sector of our country. The objective is to present a clinical case of extreme morbidity due to acquired hemophilia, its clinical presentation, evolution and fatal outcome. A case referred from another unit of the ISEM (Instituto de Salud del Estado de México) Health Sector, treated at the Obstetric Intensive Care Unit of the «Mónica Pretelini Sáenz» Hospital, is presented, highlighting the importance of diagnostic-therapeutic integration, and the multifactorial interaction of variables related to its fatal outcome. Conclusions: Ignorance of the pathology, delay in diagnosis, multiple conditioning procedures of iatrogenic hemorrhage and the limitation in therapeutic resources are factors that contribute to a fatal outcome.

Resumen: La identificación de múltiples factores de riesgo que predisponen a la hemorragia durante el evento obstétrico, como la hemofilia adquirida que es un trastorno que se desarrolla por la generación de autoanticuerpos inhibidores de factores de la coagulación, la interpretación objetiva de las pruebas de laboratorio rutinarias, el desarrollo de un pensamiento sistematizado en la integración diagnóstico-terapéutica por parte del personal de salud, y la disposición de los recursos farmacológicos hospitalarios, es lo que determina frecuentemente el pronóstico en pacientes obstétricas con morbilidad extrema que requieren atención multidisciplinaria en las diferentes unidades hospitalarias del sector salud de nuestro país. El objetivo es presentar un caso clínico de morbilidad extrema por hemofilia adquirida, su presentación clínica, evolución y desenlace fatal. Se presenta un caso referido de otra unidad del Sector Salud ISEM (Instituto de Salud del Estado de México), atendido en la Unidad de Cuidados Intensivos Obstétricos del Hospital «Mónica Pretelini Sáenz», resaltando la importancia en la integración diagnóstico-terapéutica y la interacción multifactorial de variables relacionadas con su desenlace fatal. Conclusiones: Desconocimiento de la patología, retraso en el diagnóstico, múltiples procedimientos condicionantes de hemorragia iatrógena y la limitación en recursos terapéuticos son factores que contribuyen a un desenlace fatal.

Resumen Introducción. Las infecciones asociadas con la atención en salud constituyen un problema de salud pública porque aumentan la morbimortalidad de los pacientes, sobre todo de aquellos con factores de riesgo, como la inmunosupresión debida a enfermedades oncológicas. Es importante conocer la diversidad genética de los principales microorganimos causantes de infecciones hospitalarias mediante la vigilancia epidemiológica tradicional y la epidemiología molecular, para hacer un mejor seguimiento y detectar brotes tempranamente. Objetivo. Determinar el grupo filogenético y la resistencia a antibióticos de las cepas de Escherichia coli aisladas de pacientes con cáncer hospitalizados. Materiales y métodos. Se hizo un estudio de tipo transversal que incluyó 67 cepas de Escherichia coli productoras de betalactamasas de espectro extendido (BLEE). Se determinó el grupo filogenético, el perfil de resistencia a los antibióticos, los genes de resistencia a betalactámicos, el tipo de las muestras y los servicios de hospitalización de donde fueron recuperadas. Resultados. El grupo filogenético más frecuente fue el B2 (36 %). El 57 % de las cepas B2 fueron aisladas de muestras de orina y el 33 % provenía del servicio de urología. La resistencia a ciprofloxacino y gentamicina fue de 91 y 53 %, respectivamente, y el 79 % de las cepas tenía el gen bla CTX-M. Se encontró una relación significativa (p<0,05) entre los grupos filogenéticos y la resistencia a ciprofloxacina, así como a la edad del paciente. Conclusión. El filogrupo de E. coli predominante fue el B2. Se evidenció una gran resistencia a ciprofloxacina y gentamicina, una proporción elevada de cepas BLEE con el bla CTX-M, y una relación entre el grupo filogenético y la resistencia a ciprofloxacino.

Abstract Introduction: Healthcare-associated infections are a public health problem due to the increased morbimortality of patients, especially those with risk factors such as immunosuppression due to oncological diseases. It is essential to determine the genetic diversity of the main microorganisms causing healthcare infections by combining traditional epidemiological surveillance and molecular epidemiology for better outbreak follow-up and early detection. Objective: To determine the phylogenetic group and antibiotic resistance of Escherichia coli isolated from hospitalized oncologic patients. Materials and methods: We conducted a cross-sectional study of 67 strains of ESBL- producing Escherichia coli to determine their phylogenetic group and described their antibiotic resistance profile, beta-lactam resistance genes, as well as the type of sample and the hospitalization areas from which they were recovered. Results: The most frequent phylogenetic group was B2 (36%); 57% of B2 strains were isolated from urine and 33% came from the urology department. Resistance to ciprofloxacin and gentamicin was 92% and 53%, respectively, and 79% of the strains had the bla CTX-M gene. A significant association (p<0.05) was found between the phylogenetic groups, ciprofloxacin resistance, and the age of the patients. Conclusion: The predominant E. coli phylogroup was B2. We evidenced high resistance to ciprofloxacin and gentamicin, a high proportion of ESBL strains with the bla CTX-M gene, and a significant association between the phylogenetic group and the resistance to ciprofloxacin.

RESUMEN Entre 2016-2020 se caracterizaron las especies y tamaños de las tortugas marinas presentes en el Mosaico de Conservación Bahía Málaga, Valle del Cauca por medio de monitoreo acuático y la entrega voluntaria por parte de los pescadores del área. El registro de individuos fue determinado mediante monitoreo acuático, que se dividió en tres fases: muestreo exploratorio, muestreo continuado, muestreo estandarizado, además de entregas voluntarias por los pescadores en sus faenas de pesca. Se emplearon 107 horas de esfuerzo para un total de 51 individuos de tortuga carey (Eretmochelys imbricata) y negra (Chelonia mydas) registradas. Estos resultados contribuyen como línea base para el conocimiento de las tortugas marinas como estrategia efectiva para la investigación y conservación de bahía Málaga.

ABSTRACT The species and sizes of sea turtles present in the Bahía Málaga Conservation Mosaic, Valle del Cauca, were characterized between 2016-2020 through aquatic monitoring and voluntary delivery by native fishermen. The registry of individuals was determined by aquatic monitoring, which was divided into three phases: exploratory sampling, continuous sampling, standardized sampling, plus voluntary deliveries by fishermen in their fishing tasks. A total of 107 hours of effort were used to detect 51 individuals of hawksbill (Eretmochelys imbricata) and black (Chelonia mydas) turtles. These results contribute as a baseline for the knowledge of sea turtles as an effective strategy for research and conservation in Bahía Málaga.

SUMMARY OBJECTIVE: The coronavirus disease 2019 pandemic is an economic, social, and health challenge. During the coronavirus disease 2019 lockdown, a telematics platform for respiratory physiotherapy and mindfulness was created, aiming to reduce dyspnea and anxiety and to increase quality of life in post-coronavirus disease 2019 patient. METHODS: A quasi-experimental study was performed on post-coronavirus disease 2019 patients, with breathing exercises and mindfulness with remote supervision by a respiratory physiotherapist. Dyspnea on exertion (Mahler Scale), quality of life (EuroQol-5D score), and anxiety (State-Trait Anxiety Inventory questionnaire) were measured before and after the rehabilitation program. RESULTS: A total of 20 subjects completed the program, with a significant decrease in the measures of dyspnea on exertion (p<0.001), state anxiety (p=0.004), and trait anxiety (p=0.001) and a significant increase in quality of life (p=0.016). CONCLUSIONS: Coronavirus disease 2019 should be treated using a multidisciplinary approach that includes respiratory rehabilitation. At present, there are few studies on respiratory rehabilitation and mindfulness in post-coronavirus disease 2019 patients. The results of this study showed that the implementation of breathing exercises and mindfulness with remote supervision was effective in decreasing dyspnea and anxiety and in increasing quality of life in post-coronavirus disease 2019 patients during confinement.

ABSTRACT BACKGROUND: Lifestyle is strongly involved in the pathogenesis and progression of non-communicable diseases, and has a great impact on quality of life. The goal of the present study was to analyze the lifestyle and body composition (BC) of young university students during the pandemic, and their relationship with health-related quality of life (HrQoL). DESIGN AND SETTING: Observational cross-sectional study conducted in the Universidad Europea de Madrid, Spain. METHODS: A total sample of 56 healthy university students was recruited. Activity, sitting time, adherence to Mediterranean diet and BC were measured. RESULTS: Regarding BC, only 5% and 10.7% of the subjects had health risk values for waist circumference and waist-to-height ratio, respectively. The mean daily sitting-time was 8.26 hours, while 19.64% of the subjects spent . 10 hours per day sitting. 92.86% of the subjects complied with the World Health Organization 2020 physical activity recommendations. The mean PREDIMED score was 7.41, while 51.8% of the subjects had low adherence to the Mediterranean diet. Regarding HrQoL, 22 subjects (39.2%) and 26 subjects (46.4%) were in the lowest quintile of physical component summary and mental component summary, respectively, according to the reference values for their age range. There was a negative correlation between physical function and sitting time (r = -0.38). CONCLUSIONS: There were high levels of sedentary behavior and low HrQoL values, with a negative moderate correlation between these variables. The findings from the present study especially highlight the importance of implementing public health programs targeting reduction of sitting time among university students.

ABSTRACT We analyzed the presence of the mcr-1 gene in 165 extended-spectrum beta-lactamase-producing enterobacterales (ESBL-PE) obtained during 2017, from blood (40), urine (57), lower respiratory secretions (12) and rectal swabs (56) of patients hospitalized in the Instituto Nacional de Enfermedades Neoplásicas (Peru). Antimicrobial identification and susceptibility were determined by the Phoenix M50 automated system; colistin resistance by Colistin Agar-Spot (CAS); mrc-1 detection by colistin pre-diffusion and inhibition with EDTA test (CPD-E) and by polymerase chain reaction (PCR). We found that from the 165 ESBL-PE, 25 were positive for mcr-1 by the CPD-E method and confirmed by PCR. Colistin resistance was found in 20/165 by using the CAS method. Additionally, they showed resistance to fluoroquinolones and gentamicin, while remaining sensitive to amikacin; two isolates presented metallo-carbapenemases. Obtaining data on resistance to last-line antimicrobials (colistin) is crucial to establish measures for its control.

RESUMEN La tortuga carey del Pacífico oriental es una de las unidades regionales de manejo más amenazadas del mundo. En el Pacífico colombiano ha sido avistada en áreas de alimentación y en estadios juveniles y/o subadultos. Sus zonas y temporada de anidación son desconocidas para el país. El 24 de febrero de 2020, se registra la primera hembra de tortuga carey para el Pacífico colombiano, en el Mosaico de Conservación Bahía Málaga. El largo curvo mínimo del caparazón fue de 90 cm, el largo curvo del caparazón nucal-supracuadal fue de 90,5 cm, el ancho curvo del caparazón fue de 73,6 cm y su peso fue de 69 kg. La presencia de una hembra y ser la principal zona de alimentación y residencia de juveniles y subadultos de la especie en el Pacífico de Colombia hace indispensable continuar con el monitoreo sistemático en agua e identificar y monitorear potenciales sitios de anidación para aportar a la conservación de la tortuga carey del Pacífico oriental.

ABSTRACT The Eastern Pacific hawksbill turtle is one of the most threatened management units in the world. In the Colombian Pacific it has been detected in feeding areas and in juvenile and / or sub adult’s stages. Its nesting areas and season are unknown to the country. On February 24th, 2020, the first female hawksbill turtle for the Colombian Pacific was reported, in the Conservation Mosaic of Bahía Málaga. The minimum curved carapace length was 90 cm, the curved carapace length nuchal-tip was 90.5 cm, the curved carapace width was 73.6 cm and its weight was 69 kg. The presence of a female and being the main feeding and residence area for juvenile and sub adult of the species in the Pacific of Colombia makes it essential to continue systematic monitoring in-water and identify and monitor potential nesting sites to contribute to the conservation of the hawksbill turtle at Eastern Pacific.

ABSTRACT Descriptive study conducted in order to determine the presence of the fimH and afa genes in urinary isolates of extended-spectrum beta-lactamases (ESBL) producing Escherichia coli. Isolates from project TO-06/09 of the Instituto Nacional de Salud del Niño in Lima, Peru were used. A total of 75 urinary isolates of Escherichia coli were included. Gene identification was performed by polymerase chain reaction. From the 75 isolates, 74 (98.7%) were positive for the fimH gene and 6 (8.0%) were positive for the afa gene. Virulence factors produced by the fimH and afa genes were evident in urinary isolates of ESBL producing Escherichia coli.

RESUMEN Con el objetivo de determinar la presencia de los genes fimH y afa en aislamientos urinarios de Escherichia coli productoras de betalactamasas de espectro extendido (BLEE), se realizó un estudio descriptivo, con aislamientos del cepario del proyecto TO-06/09 del Instituto Nacional de Salud del Niño en Lima, Perú. Se incluyeron 75 aislamientos urinarios de Escherichia coli. La identificación de genes se realizó por reacción en cadena de la polimerasa. De los 75 aislamientos, 74 (98,7%) fueron positivos para el gen fimH y 6 (8,0%) fueron positivos para el gen afa. Se evidenció la presencia de los factores de virulencia producidos por los genes fimH y afa en aislamientos urinarios de Escherichia coli productoras de BLEE.

ABSTRACT Introduction: Fascioliasis is a disease caused by the parasite Fasciola hepatica, according to its clinical picture, it can be classified into two phases: acute and chronic, currently the diagnosis is made during the chronic phase, however, early detection would allow to provide an effective treatment and timely. Objetive: This study is carried out with the objective of evaluating the diagnostic validity of serological tests for the timely diagnosis of hepatic fascioliasis. Materials and methods: a systematic search of the literature was carried out in Embase, MedLine, Cochrane Library and LILACS. Study selection was performed by pairs of reviewers, diagnostic test studies were selected using: ELISA, indirect ELISA, rapid ELISA, Fas2 ELISA, Arco2 compared to sedimentation techniques for the diagnosis of hepatic fascioliasis in adults. The studies were evaluated using the QUADAS-2 tool. Results: Seven studies were identified that answer the PICO question, with a total of 1,317 human serum samples, the tests reported high sensitivity and specificity, FhTA, ELISA, number of studies and participants identified, summary estimators with the corresponding confidence intervals / credibility, and the ranking of treatments. Discussion: In general, the specificity of the serological tests was slightly higher than the sensitivity, there is evidence of moderate to low quality that reports their performance, however, the quality of the evidence and the heterogeneity between the studies do not allow determining its usefulness during the acute phase of the disease. More studies are needed in this regard. The studies were clinically heterogeneous between them.

RESUMEN Introducción: La fascioliasis es una enfermedad causada por el parásito Fasciola hepática, según su cuadro clínico puede clasificarse en dos fases: aguda y crónica, actualmente el diagnóstico se realiza durante la fase crónica, sin embargo, la detección temprana permitiría brindar un tratamiento eficaz y oportuno. Objetivo: Este estudio se realiza con el objetivo de evaluar la validez diagnóstica de las pruebas serológicas para el diagnóstico oportuno de fascioliasis hepática. Materiales y métodos: se realizó una búsqueda sistemática de la literatura en Embase, MedLine, Cochrane Library y LILACS. La selección de estudios se realizó mediante pares de revisores, se seleccionaron estudios de pruebas diagnósticas utilizando: ELISA, ELISA indirecto, ELISA rápido, Fas2 ELISA, Arco2 comparada con técnicas de sedimentación para el diagnóstico de fascioliasis hepática en adultos. Los estudios fueron evaluados mediante la herramienta QUADAS-2. Resultados: Se identificaron siete estudios que responden a la pregunta PICO, con un total de 1317 muestras de suero humano, las pruebas reportaron alta sensibilidad y especificidad, FhTA, ELISA, número de estudios y participantes identificados, estimadores resumen con los correspondientes intervalos de confianza/credibilidad, también se puede discutir el ranking de tratamientos. Discusión: en general la especificidad de las pruebas serológicas fue ligeramente superior que la sensibilidad, existe evidencia de moderada a baja calidad que reporta su rendimiento, sin embargo, la calidad de la evidencia y la heterogeneidad entre los estudios no permite determinar su utilidad durante la fase aguda de la enfermedad. Se necesitan más estudios al respecto. Los estudios fueron clínicamente heterogéneos entre ellos.
